Abstract:Dynamic arterial spin labeling (ASL) MRI measures the perfusion bolus at multiple observation times and yields accurate estimates of cerebral blood flow in the presence of variations in arterial transit time. ASL has intrinsically low signal-to-noise ratio (SNR) and is sensitive to motion, so that extensive signal averaging is typically required, leading to long scan times for dynamic ASL. The goal of this study was to develop an accelerated dynamic ASL method with improved SNR and robustness to motion using a model-based image reconstruction that exploits the inherent sparsity of dynamic ASL data. The first component of this method is a single-shot 3D turbo spin echo spiral pulse sequence accelerated using a combination of parallel imaging and compressed sensing. This pulse sequence was then incorporated into a dynamic pseudo continuous ASL acquisition acquired at multiple observation times, and the resulting images were jointly reconstructed enforcing a model of potential perfusion time courses. Performance of the technique was verified using a numerical phantom and it was validated on normal volunteers on a 3-Tesla scanner. In simulation, a spatial sparsity constraint improved SNR and reduced estimation errors. Combined with a model-based sparsity constraint, the proposed method further improved SNR, reduced estimation error and suppressed motion artifacts. Experimentally, the proposed method resulted in significant improvements, with scan times as short as 20s per time point. These results suggest that the model-based image reconstruction enables rapid dynamic ASL with improved accuracy and robustness.
Simultaneous Intracranial Artery Tracing and Segmentation from Magnetic Resonance Angiography by Joint Optimization from Multiplanar Reformation

Abstract:Time-of-flight (TOF) Magnetic Resonance Angiography (MRA) is a useful imaging technique which reflects blood flow and vasculature information. However, due to the low signal and contrast of arteries in TOF MRA, it is challenging to extract vascular features such as length, volume and tortuosity, through segmentation and tracing. Hence, in this paper, a simultaneous artery tracing and segmentation method is proposed to a generate quantitative intracranial vasculature map from TOF MRA. Instead of using original images, segmentation from a neural network model is used to initiate tracing, avoiding the low signal or contrast for small arteries. A tracing method is proposed based on cross-sectional best matching, followed by an optimization scheme from the multiplanar reformatted view. Centerline positions, lumen radii and centerline deviations are jointly optimized for robust tracing within artery regions. Finally, the refined artery traces are used for better artery segmentation. The method is validated on eight TOF MRAs of both healthy subjects and patients with cerebrovascular disease, showing good agreements with human supervised tracing and segmentation results for representative features such as artery length (<4% mean absolute difference), volume (>0.80 Dice), and tortuosity (<3% mean absolute difference). Our method out-performs three other popular tracing and segmentation methods by a large margin.

Abstract:PurposeTo evaluate the feasibility and performance of compressed sensing (CS) with magnitude subtraction regularization in accelerating non-contrast-enhanced dynamic intracranial MR angiography (NCE-dMRA). MethodsA CS algorithm was introduced in NCE-dMRA by exploiting the sparsity of the magnitude difference of the control and label images. The NCE-dMRA data were acquired using golden-angle stack-of-stars trajectory on six healthy volunteers and one patient with arteriovenous fistula. Images were reconstructed using (i) the proposed magnitude-subtraction CS (MS-CS); (ii) complex-subtraction CS; (iii) independent CS; and (iv) view-sharing with k-space weighted image contrast (KWIC). The dMRA image quality was compared across the four reconstruction strategies. The proposed MS-CS method was further compared with KWIC for temporal fidelity of depicting dynamic flow. ResultsThe proposed MS-CS method was able to reconstruct NCE-dMRA images with detailed vascular structures and clean background. It provided better subjective image quality than the other two CS strategies (P<0.05). Compared with KWIC, MS-CS showed similar image quality, but reduced temporal blurring in delineating the fine distal arteries. ConclusionsThe MS-CS method is a promising CS technique for accelerating NCE-dMRA acquisition without compromising image quality and temporal fidelity. Magn Reson Med 79:867-878, 2018. Deep Learning-Based High-Resolution Magnetic Resonance Angiography (MRA) Generation Model for 4D Time-Resolved Angiography with Interleaved Stochastic Trajectories (TWIST) MRA in Fast Stroke Imaging

Abstract:Purpose: The purpose of this study is to improve the qualitative and quantitative image quality of the time-resolved angiography with interleaved stochastic trajectories technique (4D-TWIST-MRA) using deep neural network (DNN)-based MR image reconstruction software. Materials and Methods: A total of 520 consecutive patients underwent 4D-TWIST-MRA for ischemic stroke or intracranial vessel stenosis evaluation. Four-dimensional DNN-reconstructed MRA (4D-DNR) was generated using commercially available software (SwiftMR v.3.0.0.0, AIRS Medical, Seoul, Republic of Korea). Among those evaluated, 397 (76.3%) patients received concurrent time-of-flight MRA (TOF-MRA) to compare the signal-to-noise ratio (SNR), image quality, noise, sharpness, vascular conspicuity, and degree of venous contamination with a 5-point Likert scale. Two radiologists independently evaluated the detection rate of intracranial aneurysm in TOF-MRA, 4D-TWIST-MRA, and 4D-DNR in separate sessions. The other 123 (23.7%) patients received 4D-TWIST-MRA due to a suspicion of acute ischemic stroke. The confidence level and decision time for large vessel occlusion were evaluated in these patients. Results: In qualitative analysis, 4D-DNR demonstrated better overall image quality, sharpness, vascular conspicuity, and noise reduction compared to 4D-TWIST-MRA. Moreover, 4D-DNR exhibited a higher SNR than 4D-TWIST-MRA. The venous contamination and aneurysm detection rates were not significantly different between the two MRA images. When compared to TOF-MRA, 4D-CE-MRA underestimated the aneurysm size (2.66 ± 0.51 vs. 1.75 ± 0.62, p = 0.029); however, 4D-DNR showed no significant difference in size compared to TOF-MRA (2.66 ± 0.51 vs. 2.10 ± 0.41, p = 0.327). In the diagnosis of large vessel occlusion, 4D-DNR showed a better confidence level and shorter decision time than 4D-TWIST-MRA. Conclusion: DNN reconstruction may improve the qualitative and quantitative image quality of 4D-TWIST-MRA, and also enhance diagnostic performance for intracranial aneurysm and large vessel occlusion.

Abstract:Purpose To develop a novel framework to improve the visualization of distal arteries in arterial spin labeling (ASL) dynamic MRA. Methods The attenuation of ASL blood signal due to the repetitive application of excitation RF pulses was minimized by splitting the acquisition volume into multiple thin 2D (M2D) slices, thereby reducing the exposure of the arterial blood magnetization to RF pulses while it flows within the brain. To improve the degraded vessel visualization in the slice direction due to the limited minimum achievable 2D slice thickness, a super‐resolution (SR) convolutional neural network (CNN) was trained by using 3D time‐of‐flight (TOF)‐MRA images from a large public dataset. And then, we applied domain transfer from 3D TOF‐MRA to M2D ASL‐MRA, while avoiding acquiring a large number of ASL‐MRA data required for CNN training. Results Compared to the conventional 3D ASL‐MRA, far more distal arteries were visualized with higher signal intensity by using M2D ASL‐MRA. In general, however, the vessel visualization with a conventional interpolation was prone to be blurry and unclear due to the limited spatial resolution in the slice direction, particularly in small vessels. Application of CNN‐based SR transferred from 3D TOF‐MRA to M2D ASL‐MRA successfully addressed such a limitation and achieved clearer visualization of small vessels than conventional interpolation. Conclusion This study demonstrated that the proposed framework provides improved visualization of distal arteries in later dynamic phases, which will particularly benefit the application of this approach in patients with cerebrovascular disease who have slow blood flow.

Automated Cerebrovascular Segmentation and Visualization of Intracranial Time-of-Flight Magnetic Resonance Angiography Based on Deep Learning

Abstract:Time-of-flight magnetic resonance angiography (TOF-MRA) is a non-contrast technique used to visualize neurovascular. However, manual reconstruction of the volume render (VR) by radiologists is time-consuming and labor-intensive. Deep learning-based (DL-based) vessel segmentation technology may provide intelligent automation workflow. To evaluate the image quality of DL vessel segmentation for automatically acquiring intracranial arteries in TOF-MRA. A total of 394 TOF-MRA scans were selected, which included cerebral vascular health, aneurysms, or stenoses. Both our proposed method and two state-of-the-art DL methods are evaluated on external datasets for generalization ability. For qualitative assessment, two experienced clinical radiologists evaluated the image quality of cerebrovascular diagnostic and visualization (scoring 0-5 as unacceptable to excellent) obtained by manual VR reconstruction or automatic convolutional neural network (CNN) segmentation. The proposed CNN outperforms the other two DL-based methods in clinical scoring on external datasets, and its visualization was evaluated by readers as having the appearance of the radiologists' manual reconstructions. Scoring of proposed CNN and VR of intracranial arteries demonstrated good to excellent agreement with no significant differences (median, 5.0 and 5.0, P ≥ 12) at healthy-type scans. All proposed CNN image quality were considered to have adequate diagnostic quality (median scores > 2). Quantitative analysis demonstrated a superior dice similarity coefficient of cerebrovascular overlap (training sets and validation sets; 0.947 and 0.927). Automatic cerebrovascular segmentation using DL is feasible and the image quality in terms of vessel integrity, collateral circulation and lesion morphology is comparable to expert manual VR without significant differences.

Abstract:Time-of-flight magnetic resonance angiography (TOF-MRA) is one of the most widely used non-contrast MR imaging methods to visualize blood vessels, but due to the 3-D volume acquisition highly accelerated acquisition is necessary. Accordingly, high quality reconstruction from undersampled TOF-MRA is an important research topic for deep learning. However, most existing deep learning works require matched reference data for supervised training, which are often difficult to obtain. By extending the recent theoretical understanding of cycleGAN from the optimal transport theory, here we propose a novel two-stage unsupervised deep learning approach, which is composed of the multi-coil reconstruction network along the coronal plane followed by a multi-planar refinement network along the axial plane. Specifically, the first network is trained in the square-root of sum of squares (SSoS) domain to achieve high quality parallel image reconstruction, whereas the second refinement network is designed to efficiently learn the characteristics of highly-activated blood flow using double-headed max-pool discriminator. Extensive experiments demonstrate that the proposed learning process without matched reference exceeds performance of state-of-the-art compressed sensing (CS)-based method and provides comparable or even better results than supervised learning approaches.

FlowMRI-Net: A generalizable self-supervised physics-driven 4D Flow MRI reconstruction network for aortic and cerebrovascular applications

Abstract:In this work, we propose FlowMRI-Net, a novel deep learning-based framework for fast reconstruction of accelerated 4D flow magnetic resonance imaging (MRI) using physics-driven unrolled optimization and a complexvalued convolutional recurrent neural network trained in a self-supervised manner. The generalizability of the framework is evaluated using aortic and cerebrovascular 4D flow MRI acquisitions acquired on systems from two different vendors for various undersampling factors (R=8,16,24) and compared to state-of-the-art compressed sensing (CS-LLR) and deep learning-based (FlowVN) reconstructions. Evaluation includes quantitative analysis of image magnitudes, velocity magnitudes, and peak velocity curves. FlowMRINet outperforms CS-LLR and FlowVN for aortic 4D flow MRI reconstruction, resulting in vectorial normalized root mean square errors of $0.239\pm0.055$, $0.308\pm0.066$, and $0.302\pm0.085$ and mean directional errors of $0.023\pm0.015$, $0.036\pm0.018$, and $0.039\pm0.025$ for velocities in the thoracic aorta for R=16, respectively. Furthermore, FlowMRI-Net outperforms CS-LLR for cerebrovascular 4D flow MRI reconstruction, where no FlowVN can be trained due to the lack of a highquality reference, resulting in a consistent increase in SNR of around 6 dB and more accurate peak velocity curves for R=8,16,24. Reconstruction times ranged from 1 to 7 minutes on commodity CPU/GPU hardware. FlowMRI-Net enables fast and accurate quantification of aortic and cerebrovascular flow dynamics, with possible applications to other vascular territories. This will improve clinical adaptation of 4D flow MRI and hence may aid in the diagnosis and therapeutic management of cardiovascular diseases.

Abstract:Purpose: To generate perfusion parameter maps from Time-of-flight magnetic resonance angiography (TOF-MRA) images using artificial intelligence to provide an alternative to traditional perfusion imaging techniques. Materials and methods: This retrospective study included a total of 272 patients with cerebrovascular diseases; 200 with acute stroke (from 2010 to 2018), and 72 with steno-occlusive disease (from 2011 to 2014). For each patient the TOF MRA image and the corresponding Dynamic susceptibility contrast magnetic resonance imaging (DSC-MRI) were retrieved from the datasets. The authors propose an adapted generative adversarial network (GAN) architecture, 3D pix2pix GAN, that generates common perfusion maps (CBF, CBV, MTT, TTP, Tmax) from TOF-MRA images. The performance was evaluated by the structural similarity index measure (SSIM). For a subset of 20 patients from the acute stroke dataset, the Dice coefficient was calculated to measure the overlap between the generated and real hypoperfused lesions with a time-to-maximum (Tmax) > 6 s. Results: The GAN model exhibited high visual overlap and performance for all perfusion maps in both datasets: acute stroke (mean SSIM 0.88-0.92, mean PSNR 28.48-30.89, mean MAE 0.02-0.04 and mean NRMSE 0.14-0.37) and steno-occlusive disease patients (mean SSIM 0.83-0.98, mean PSNR 23.62-38.21, mean MAE 0.01-0.05 and mean NRMSE 0.03-0.15). For the overlap analysis for lesions with Tmax>6 s, the median Dice coefficient was 0.49. Conclusion: Our AI model can successfully generate perfusion parameter maps from TOF-MRA images, paving the way for a non-invasive alternative for assessing cerebral hemodynamics in cerebrovascular disease patients. This method could impact the stratification of patients with cerebrovascular diseases. Our results warrant more extensive refinement and validation of the method.
4DFlowNet: Super-Resolution 4D Flow MRI Using Deep Learning and Computational Fluid Dynamics

Abstract:4D flow magnetic resonance imaging (MRI) is an emerging imaging technique where spatiotemporal 3D blood velocity can be captured with full volumetric coverage in a single non-invasive examination. This enables qualitative and quantitative analysis of hemodynamic flow parameters of the heart and great vessels. An increase in the image resolution would provide more accuracy and allow better assessment of the blood flow, especially for patients with abnormal flows. However, this must be balanced with increasing imaging time. The recent success of deep learning in generating super resolution images shows promise for implementation in medical images. We utilized computational fluid dynamics simulations to generate fluid flow simulations and represent them as synthetic 4D flow MRI data. We built our training dataset to mimic actual 4D flow MRI data with its corresponding noise distribution. Our novel 4DFlowNet network was trained on this synthetic 4D flow data and was capable in producing noise-free super resolution 4D flow phase images with upsample factor of 2. We also tested the 4DFlowNet in actual 4D flow MR images of a phantom and normal volunteer data, and demonstrated comparable results with the actual flow rate measurements giving an absolute relative error of 0.6–5.8% and 1.1–3.8% in the phantom data and normal volunteer data, respectively.

Perfusion Maps Acquired From Dynamic Angiography MRI Using Deep Learning Approaches

Abstract:Background: A typical stroke MRI protocol includes perfusion-weighted imaging (PWI) and MR angiography (MRA), requiring a second dose of contrast agent. A deep learning method to acquire both PWI and MRA with single dose can resolve this issue. Purpose: To acquire both PWI and MRA simultaneously using deep learning approaches. Study type: Retrospective. Subjects: A total of 60 patients (30-73 years old, 31 females) with ischemic symptoms due to occlusion or ≥50% stenosis (measured relative to proximal artery diameter) of the internal carotid artery, middle cerebral artery, or anterior cerebral artery. The 51/1/8 patient data were used as training/validation/test. Field strength/sequence: A 3 T, time-resolved angiography with stochastic trajectory (contrast-enhanced MRA) and echo planar imaging (dynamic susceptibility contrast MRI, DSC-MRI). Assessment: We investigated eight different U-Net architectures with different encoder/decoder sizes and with/without an adversarial network to generate perfusion maps from contrast-enhanced MRA. Relative cerebral blood volume (rCBV), relative cerebral blood flow (rCBF), mean transit time (MTT), and time-to-max (Tmax ) were mapped from DSC-MRI and used as ground truth to train the networks and to generate the perfusion maps from the contrast-enhanced MRA input. Statistical tests: Normalized root mean square error, structural similarity (SSIM), peak signal-to-noise ratio (pSNR), DICE, and FID scores were calculated between the perfusion maps from DSC-MRI and contrast-enhanced MRA. One-tailed t-test was performed to check the significance of the improvements between networks. P values < 0.05 were considered significant. Results: The four perfusion maps were successfully extracted using the deep learning networks. U-net with multiple decoders and enhanced encoders showed the best performance (pSNR 24.7 ± 3.2 and SSIM 0.89 ± 0.08 for rCBV). DICE score in hypo-perfused area showed strong agreement between the generated perfusion maps and the ground truth (highest DICE: 0.95 ± 0.04). Data conclusion: With the proposed approach, dynamic angiography MRI may provide vessel architecture and perfusion-relevant parameters simultaneously from a single scan. Abstract:Abstract Three-dimensional vessel model reconstruction from patient-specific magnetic resonance angiography (MRA) images often requires some manual maneuvers. This study aimed to establish the deep learning (DL)-based method for vessel model reconstruction. Time of flight MRA of 40 patients with internal carotid artery aneurysms was prepared, and three-dimensional vessel models were constructed using the threshold and region-growing method. Using those datasets, supervised deep learning using 2D U-net was performed to reconstruct 3D vessel models. The accuracy of the DL-based vessel segmentations was assessed using 20 MRA images outside the training dataset. The dice coefficient was used as the indicator of the model accuracy, and the blood flow simulation was performed using the DL-based vessel model. The created DL model could successfully reconstruct a three-dimensional model in all 60 cases. The dice coefficient in the test dataset was 0.859. Of note, the DL-generated model proved its efficacy even for large aneurysms (> 10 mm in their diameter). The reconstructed model was feasible in performing blood flow simulation to assist clinical decision-making. Our DL-based method could successfully reconstruct a three-dimensional vessel model with moderate accuracy. Future studies are warranted to exhibit that DL-based technology can promote medical image processing. Graphical Abstract

Optimizing High-Resolution MR Angiography: The Synergistic Effects of 3D Wheel Sampling and Deep Learning–Based Reconstruction

Abstract:Objective The aim of this study was to assess the utility of the combined use of 3D wheel sampling and deep learning–based reconstruction (DLR) for intracranial high-resolution (HR)–time-of-flight (TOF)–magnetic resonance angiography (MRA) at 3 T. Methods This prospective study enrolled 20 patients who underwent head MRI at 3 T, including TOF-MRA. We used 3D wheel sampling called “fast 3D” and DLR for HR-TOF-MRA (spatial resolution, 0.39 × 0.59 × 0.5 mm 3 ) in addition to conventional MRA (spatial resolution, 0.39 × 0.89 × 1 mm 3 ). We compared contrast and contrast-to-noise ratio between the blood vessels (basilar artery and anterior cerebral artery) and brain parenchyma, full width at half maximum in the P3 segment of the posterior cerebral artery among 3 protocols. Two board-certified radiologists evaluated noise, contrast, sharpness, artifact, and overall image quality of 3 protocols. Results The contrast and contrast-to-noise ratio of fast 3D-HR-MRA with DLR are comparable or higher than those of conventional MRA and fast 3D-HR-MRA without DLR. The full width at half maximum was significantly lower in fast 3D-MRA with and without DLR than in conventional MRA ( P = 0.006, P < 0.001). In qualitative evaluation, fast 3D-MRA with DLR had significantly higher sharpness and overall image quality than conventional MRA and fast 3D-MRA without DLR (sharpness: P = 0.021, P = 0.001; overall image quality: P = 0.029, P < 0.001). Abstract:Background: Moyamoya disease (MMD) is a rare cerebrovascular occlusive disease with progressive stenosis of the terminal portion of internal cerebral artery (ICA) and its main branches, which can cause complications, such as high risks of disability and increased mortality. Accurate and timely diagnosis may be difficult for physicians who are unfamiliar to MMD. Therefore, this study aims to achieve a preoperative deep-learning-based evaluation of MMD by detecting steno-occlusive changes in the middle cerebral artery or distal ICA areas.Methods: A fine-tuned deep learning model was developed using a three-dimensional (3D) coordinate attention residual network (3D CA-ResNet). This study enrolled 50 preoperative patients with MMD and 50 controls, and the corresponding time of flight magnetic resonance angiography (TOF-MRA) imaging data were acquired. The 3D CA-ResNet was trained based on sub-volumes and tested using patch-based and subject-based methods. The performance of the 3D CA-ResNet, as evaluated by the area under the curve (AUC) of receiving-operator characteristic, was compared with that of three other conventional 3D networks.Results: With the resulting network, the patch-based test achieved an AUC value of 0.94 for the 3D CAResNet in 480 patches from 10 test patients and 10 test controls, which is significantly higher than the results of the others. The 3D CA-ResNet correctly classified the MMD patients and normal healthy controls, and the vascular lesion distribution in subjects with the disease was investigated by generating a stenosis probability map and 3D vascular structure segmentation.Conclusions: The results demonstrated the reliability of the proposed 3D CA-ResNet in detecting stenotic areas on TOF-MRA imaging, and it outperformed three other models in identifying vascular steno
Generating 3D TOF-MRA volumes and segmentation labels using generative adversarial networks

Abstract:Deep learning requires large labeled datasets that are difficult to gather in medical imaging due to data privacy issues and time-consuming manual labeling. Generative Adversarial Networks (GANs) can alleviate these challenges enabling synthesis of shareable data. While 2D GANs have been used to generate 2D images with their corresponding labels, they cannot capture the volumetric information of 3D medical imaging. 3D GANs are more suitable for this and have been used to generate 3D volumes but not their corresponding labels. One reason might be that synthesizing 3D volumes is challenging owing to computational limitations. In this work, we present 3D GANs for the generation of 3D medical image volumes with corresponding labels applying mixed precision to alleviate computational constraints. We generated 3D Time-of-Flight Magnetic Resonance Angiography (TOF-MRA) patches with their corresponding brain blood vessel segmentation labels. We used four variants of 3D Wasserstein GAN (WGAN) with: 1) gradient penalty (GP), 2) GP with spectral normalization (SN), 3) SN with mixed precision (SN-MP), and 4) SN-MP with double filters per layer (c-SN-MP). The generated patches were quantitatively evaluated using the Fréchet Inception Distance (FID) and Precision and Recall of Distributions (PRD). Further, 3D U-Nets were trained with patch-label pairs from different WGAN models and their performance was compared to the performance of a benchmark U-Net trained on real data. The segmentation performance of all U-Net models was assessed using Dice Similarity Coefficient (DSC) and balanced Average Hausdorff Distance (bAVD) for a) all vessels, and b) intracranial vessels only. Our results show that patches generated with WGAN models using mixed precision (SN-MP and c-SN-MP) yielded the lowest FID scores and the best PRD curves. Among the 3D U-Nets trained with synthetic patch-label pairs, c-SN-MP pairs achieved the highest DSC (0.841) and lowest bAVD (0.508) compared to the benchmark U-Net trained on real data (DSC 0.901; bAVD 0.294) for intracranial vessels. In conclusion, our solution generates realistic 3D TOF-MRA patches and labels for brain vessel segmentation. We demonstrate the benefit of using mixed precision for computational efficiency resulting in the best-performing GAN-architecture. Our work paves the way towards sharing of labeled 3D medical data which would increase generalizability of deep learning models for clinical use.
Motion-resolved real-time 4D flow MRI with low-rank and subspace modeling

Abstract:Purpose: To develop a new motion-resolved real-time four-dimensional (4D) flow MRI method, which enables the quantification and visualization of blood flow velocities with three-directional flow encodings and volumetric coverage without electrocardiogram (ECG) synchronization and respiration control. Methods: An integrated imaging method is presented for real-time 4D flow MRI, which encompasses data acquisition, image reconstruction, and postprocessing. The proposed method features a specialized continuous ( k , t ) $$ \left(\mathbf{k},t\right) $$ -space acquisition scheme, which collects two sets of data (i.e., training data and imaging data) in an interleaved manner. By exploiting strong spatiotemporal correlation of 4D flow data, it reconstructs time-series images from highly-undersampled ( k , t ) $$ \left(\mathbf{k},t\right) $$ -space measurements with a low-rank and subspace model. Through data-binning-based postprocessing, it constructs a five-dimensional dataset (i.e., x-y-z-cardiac-respiratory), from which respiration-dependent flow information is further analyzed. The proposed method was evaluated in aortic flow imaging experiments with ten healthy subjects and two patients with atrial fibrillation. Results: The proposed method achieves 2.4 mm isotropic spatial resolution and 34.4 ms temporal resolution for measuring the blood flow of the aorta. For the healthy subjects, it provides flow measurements in good agreement with those from the conventional 4D flow MRI technique. For the patients with atrial fibrillation, it is able to resolve beat-by-beat pathological flow variations, which cannot be obtained from the conventional technique. The postprocessing further provides respiration-dependent flow information. Conclusion: The proposed method enables high-resolution motion-resolved real-time 4D flow imaging without ECG gating and respiration control. It is able to resolve beat-by-beat blood flow variations as well as respiration-dependent flow information.
Accurate and robust segmentation of cerebral vasculature on four-dimensional arterial spin labeling magnetic resonance angiography using machine-learning approach

Abstract:Segmentation of cerebral vasculature on MR vascular images is of great significance for clinical application and research. However, the existing cerebrovascular segmentation approaches are limited due to insufficient image contrast and complicated algorithms. This study aims to explore the potential of the emerging four-dimensional arterial spin labeling magnetic resonance angiography (4D ASL-MRA) technique for fast and accurate cerebrovascular segmentation with a simple machine-learning approach. Nine temporal features were extracted from the intensity-time signal of each voxel, and eight spatial features from the neighboring voxels. Then, the unsupervised outlier detection algorithm, i.e. Isolation Forest, is used for segmentation of the vascular voxels based on the extracted features. The total length of the centerlines of the intracranial arterial vasculature, the dice similarity coefficient (DSC), and the average Hausdorff Distance (AVGHD) on the cross-sections of small- to large-sized vessels were calculated to evaluate the performance of the segmentation approach on 4D ASL-MRA of 18 subjects. Experiments show that the temporal information on 4D ASL-MRA can largely improve the segmentation performance. In addition, the proposed segmentation approach outperforms the traditional methods that were performed on the 3D image (i.e. the temporal average intensity projection of 4D ASL-MRA) and the previously proposed frame-wise approach. In conclusion, this study demonstrates that accurate and robust segmentation of cerebral vasculature is achievable on 4D ASL-MRA by using a simple machine-learning approach with appropriate features.
A novel application of four-dimensional magnetic resonance angiography using an arterial spin labeling technique for noninvasive diagnosis of Moyamoya disease

Abstract:Background: Noncontrast-enhanced time-resolved four-dimensional magnetic resonance angiography using an arterial spin labeling technique (ASL-4D MRA) is emerging as a next generation angiography for the management of neurovascular diseases. This study evaluated the feasibility of ASL-4D MRA for the diagnosis of Moyamoya disease (MMD) and MMD staging by using digital subtraction angiography (DSA) and time-of-flight MRA (TOF MRA) as current standards. Methods: Eleven consecutive non-operated patients who underwent DSA for the diagnosis of MMD were recruited. Two independent observers evaluated the three tests. The data were analyzed for inter-observer and inter-modality agreements on MMD stage. Nine of 22 hemispheres underwent surgical revascularization and ASL-4D MRA was repeated postoperatively. Results: Time-resolved inflow of blood through the cerebral vessels, including moyamoya vessels, was visualized in all the 22 non-operated hemispheres. MMD stages assessed by DSA and ASL-4D MRA were completely matched in 18 hemispheres, with a significant positive correlation between these modalities (r=0.93, P<0.001). Inter-observer agreement with ASL-4D MRA (κ=0.91±0.04, P<0.001) and inter-modality agreement between ASL-4D MRA and DSA (κ=0.93±0.04, P<0.001) were both excellent. MMD stages assessed by ASL-4D MRA have also a significant positive correlation with those assessed by TOF MRA (r=0.68, P=0.004). Repeated ASL-4D MRA clearly demonstrated the bypassed arteries and changes in the dynamic flow patterns of cerebral arteries in all the nine hemispheres after surgical revascularization. Of these, postoperative focal hyperperfusion was detected by single photon emission tomography in 7 hemispheres. In five of the seven hemispheres (71%) with postoperative hyperperfusion, ASL-4D MRA demonstrated focal hyperintense signals in the bypassed arteries, although TOF MRA did not. Conclusions: Noninvasive ASL-4D MRA is feasible for the diagnosis of MMD staging. This next generation angiography may be useful for monitoring disease evolution and treatment response in cerebral arteries after revascularization surgery in MMD.

Abstract:Background: Four-dimensional (4D) flow magnetic resonance imaging (MRI) often relies on the injection of gadolinium- or iron-oxide-based contrast agents to improve vessel delineation. In this work, a novel technique is developed to acquire and reconstruct 4D flow data with excellent dynamic visualization of blood vessels but without the need for contrast injection. Synchronization of Neighboring Acquisitions by Physiological Signals (SyNAPS) uses pilot tone (PT) navigation to retrospectively synchronize the reconstruction of two free-running three-dimensional radial acquisitions, to create co-registered anatomy and flow images. Methods: Thirteen volunteers and two Marfan syndrome patients were scanned without contrast agent using one free-running fast interrupted steady-state (FISS) sequence and one free-running phase-contrast MRI (PC-MRI) sequence. PT signals spanning the two sequences were recorded for retrospective respiratory motion correction and cardiac binning. The magnitude and phase images reconstructed, respectively, from FISS and PC-MRI, were synchronized to create SyNAPS 4D flow datasets. Conventional two-dimensional (2D) flow data were acquired for reference in ascending (AAo) and descending aorta (DAo). The blood-to-myocardium contrast ratio, dynamic vessel area, net volume, and peak flow were used to compare SyNAPS 4D flow with Native 4D flow (without FISS information) and 2D flow. A score of 0-4 was given to each dataset by two blinded experts regarding the feasibility of performing vessel delineation. Results: Blood-to-myocardium contrast ratio for SyNAPS 4D flow magnitude images (1.5 ± 0.3) was significantly higher than for Native 4D flow (0.7 ± 0.1, p < 0.01) and was comparable to 2D flow (2.3 ± 0.9, p = 0.02). Image quality scores of SyNAPS 4D flow from the experts (M.P.: 1.9 ± 0.3, E.T.: 2.5 ± 0.5) were overall significantly higher than the scores from Native 4D flow (M.P.: 1.6 ± 0.6, p = 0.03, E.T.: 0.8 ± 0.4, p < 0.01) but still significantly lower than the scores from the reference 2D flow datasets (M.P.: 2.8 ± 0.4, p < 0.01, E.T.: 3.5 ± 0.7, p < 0.01). The Pearson correlation coefficient between the dynamic vessel area measured on SyNAPS 4D flow and that from 2D flow was 0.69 ± 0.24 for the AAo and 0.83 ± 0.10 for the DAo, whereas the Pearson correlation between Native 4D flow and 2D flow measurements was 0.12 ± 0.48 for the AAo and 0.08 ± 0.39 for the DAo. Linear correlations between SyNAPS 4D flow and 2D flow measurements of net volume (r2 = 0.83) and peak flow (r2 = 0.87) were larger than the correlations between Native 4D flow and 2D flow measurements of net volume (r2 = 0.79) and peak flow (r2 = 0.76). Conclusion: The feasibility and utility of SyNAPS were demonstrated for joint whole-heart anatomical and flow MRI without requiring electrocardiography gating, respiratory navigators, or contrast agents. Using SyNAPS, a high-contrast anatomical imaging sequence can be used to improve 4D flow measurements that often suffer from poor delineation of vessel boundaries in the absence of contrast agents.
