Abstract:SETTING Botswana, 1999. OBJECTIVE To measure knowledge, attitudes and beliefs about tuberculosis (TB) preventive therapy (PT) for persons infected with the human immunodeficiency virus (HIV). DESIGN A systematic sample of adult clinic attendees, using a standardised questionnaire. RESULTS A total of 275 patients at 38 clinics in five districts were interviewed. The majority were female (65%) and unmarried (84%). Knowing someone with TB or AIDS was common (78% and 53%, respectively). Respondents perceived a relationship between TB and HIV (80%), and the majority were willing to undergo tuberculin skin testing (92%). Of those, most were willing to undergo evaluation for active TB (98%), and to take PT, although willingness to take PT declined with proposed duration (97% 6 months, 90% 1 year, 81% lifetime, P < 0.01). Previous HIV testing was reported by 13%; those who had not undergone testing reported that they would if doctors could improve the quality (95%) or duration (93%) of life of persons with AIDS. The majority favoured receiving HIV test results on the day they were tested (60%). CONCLUSIONS Most clinic attendees in Botswana were willing to undergo HIV testing if it were beneficial to do so, such as by receiving PT. Pilot PT projects should be initiated. Voluntary HIV counselling and testing services should consider rapid HIV testing methods.
The effect of surface treatment and thermal aging on the bonding of clear aligner attachments to provisional resin-based material: shear bond strength analysis

Abstract:Objectives: The aim of this study is to evaluate the effect of different surface treatments on the shear bond strength (SBS) of clear aligner attachments bonded to Bis-acryl provisional crowns. Methods: 120 cylindrical bisacrylic composite material (ProTemp type) specimens were prepared and divided into six groups (n = 20) based on surface treatment, control: (no treatment); super coarse grit diamond bur, carbide bur, alumina-blasting, non-thermal plasma treatment, and Er:YAG laser treatment. The features of treated surfaces were examined using scanning electron microscopy (SEM). A flowable composite resin (Transbond XT; 3M Unitek) was bonded to the specimens forming the attachment. Half of specimens were subjected to thermal cycling (5,000 cycles). SBS was measured before and after thermal cycling. Each specimen was loaded at the attachment/resin interface at a speed of 0.5 mm/min until failure. The nature of the failure was analyzed using the composite remnants index (CRI). Two-way ANOVA and Tukey HSD were used for data analysis α = 0.5. For CRI scores analysis, Kruskal-Wallis test and Dunn's multiple comparison were used as post-hoc test. Results: SEM analysis showed that all surface treatments altered surface properties and increase surface bonding area. The specimens treated with plasma, Er:YAG laser, and alumina-blasting had higher SBS values before and after thermal cycling. In comparison to control plasma, Er:YAG laser, and alumina-blasting showed a significant increase in SBS (P < 0.001) while carbide and diamond bur groups showed no significant differences (P > 0.05). Thermal cycling significantly decreased the SBS of control, carbide bur, diamond bur, and Er:YAG laser while no significant effect of alumina-blasting and plasma group. Er:YAG laser and plasma groups significantly exhibited more dominance for scores 2 and score 3 and the absence of score 0. Conclusion: Alumina-blasting, Er:YAG laser, or non-thermal plasma surface treatments increased the shear bond strength between clear aligner attachments and resin-based restorations.
Effect of Different Surface Treatment Methods on the Shear Bond Strength of Resin Composite/Zirconia for Intra-oral Repair of Zirconia Restorations

Abstract:Objective: A durable resin/zirconia bond is essential for successful intra-oral repair of zirconia restorations. The purpose of this study was to evaluate the influence of two mechanical treatments followed by seven chemical treatments on the shear bond strength (SBS) of composite resin to zirconia. Materials and methods: In this in vitro study, 280 zirconia blocks (Y-TZP) were either air-abraded or bur roughened and divided into seven experimental groups (n = 20) in terms of primer/resin application: 1) ZPP, Z-Prime Plus; 2) ZPP + GP, Z-Prime Plus followed by G-premio bond; 3) ZPP + ALB, Z-Prime Plus followed by All Bond Universal; 4) ZPP + CLRF, Z-Prime Plus followed by Clearfil SE Bond; 5)GP, G-Premio Bond 6) ALB, All Bond Universal; and 7) CLRF, Clearfil SE Bond. After composite bonding and storage in distilled water (24 hours), half of each group specimen (n = 10) were thermocycled. All specimens were subjected to shear force. Statistical analysis was performed using Kruskal-Wallis and Mann-Whitney test (α = 0.05). Results: Significant reduction in SBS was observed in all groups after thermocycling(p < 0.05), except for the air-abraded ZPP + CLRF (p = 0.143). After aging, air-abraded CLRF exhibited the highest SBS (13.55 ± 7.8 MPa) and bur roughened ZPP showed the lowest SBS (1.16 ± 1.23 MPa). In the aged specimens, there was a significant difference between air abrasion and bur roughening in all groups (p < 0.05). Conclusion: Air-abrasion followed by application of adhesive (with/without prior primer application) is the most efficient technique for repair of veneered zirconia restorations with resin composite.

Abstract:The high cost of CAD/CAM systems and materials is a severe economic burden. Therefore, repair of CAD/CAM PMMA, selecting appropriate repair materials, and surface modifications are clinically important. This study aims to evaluate the shear bond strength of PMMA repair materials after various surface treatments on CAD/CAM PMMA denture base material. For this purpose, a total of 480 CAD/CAM PMMA denture base test specimens were manufactured. Then all test specimens were divided into 6 groups, and different surface treatments were applied. Group A: sandblasting, Group B: 4% hydro fluoric acid, Group C: tungsten carbide bur, Group D: dichloromethane + methyl methacrylate mixture, Group E: dichloromethane and methyl methacrylate, Group F: no surface treatment. Each group is then divided into 4 different subcategories; repair processes were performed using; heat-cured acrylic resin ( n: 20), auto-polymerized acrylic resin ( n: 20), gingiva composite ( n: 20), and CAD/CAM PMMA tooth material ( n: 20). After repairs, thermal aging was applied to half of the test specimens in each subcategory. The shear bond strength value was measured with a universal test device. Sandblasting group showed the highest surface roughness value in all test specimens ( p < 0.001). Heat-cured acrylic resin with sandblasting exhibited the highest bond strength, while the untreated gingiva composite resin exhibited the lowest value. Thermal aging decreased bond strength in all repair materials ( p < 0.001). Among the surface treatment groups, sandblasting with Al 2 O 3 particles exhibited the highest surface roughness value and repair bond strength. The application of organic solvents to the surface increased the surface roughness and repair bond strength. Applying dichloromethane and methyl methacrylate monomer separately is more effective than applying it as a mixture. The ideal bonding among repair materials was obtained with heat-cured acrylic resin.
Effect of Different Surface Treatments on Repair Bond Strength of CAD/CAM Resin-Matrix Ceramics

Abstract:The purpose of this study was to investigate the influence of different surface treatment methods on the micro-tensile bond strength (μTBS) of resin-matrix ceramic (RMC) blocks repaired with resin composite. Three different prefabricated RMC blocks including Lava Ultimate (LU), Grandio Blocs (GB), and Shofu Block HC (HC) were thermo-cycled and divided into five surface treatment groups: Control (C), bur grinding (G), airborne particle abrasion (APA), Er,Cr:YSGG laser irritation (LI), and APA combined with LI (APA+LI). After surface treatments, topographic alterations were examined by scanning electron microscopy. Then, Universal Adhesive (Single Bond Universal) was applied and repair was simulated with nanohybrid composite (Grandio SO). Bonded specimens were cut into 1 mm2 sized beams (n = 16) and a μTBS test was conducted by using a universal test machine. Fracture types were evaluated by using a stereomicroscope. The bond- strength data was evaluated by two-way ANOVA and Tukey post-hoc test (α = 0.05). The μTBS values were significantly affected by the surface treatment variable and the interaction terms of the variables (p ≤ 0.001). However, no significant effect of RMC type was detected (p > 0.05). Among all materials, GBAPA+LI indicated the highest μTBS value. Except for the GBC, all surface treatments showed clinically acceptable bond-strength values. However, the surface treatments applied to GB and LU before the repair processes increased the repair bond-strength values while causing a negative effect for HC. In addition, LI and APA+LI can be applied as an alternative route compared to other procedures recommended by the manufacturer for surface preparation in intraoral RMC repair.

Abstract:Objective This study aimed to evaluate the effects of different surface treatments on the repair bond strength between a fiber‐reinforced dentin composite and a posterior composite. Methods Forty fiber‐reinforced dentin composite resin blocks (4 mm × 4 mm × 4 mm) were separated into eight groups (n = 5) according to the surface preparation methods: (G1) negative control group, (G2) adhesive application, (G3) 50% dimethylsulfoxide (DMSO) application, (G4) 50% DMSO + adhesive application, (G5) 37% phosphoric acid etch + adhesive application, (G6) air abrasion + adhesive application, (G7) 37% phosphoric acid etch + 50% DMSO application + adhesive application, and (G8) air abrasion +50% DMSO application + adhesive application group. The composite surfaces were repaired in two layers with a posterior composite. Composite sticks were subjected to a micro tensile bond strength (μTBS) test. Fractured surfaces were evaluated using a stereomicroscope (×25). Short fiber‐reinforced composite samples' surfaces were investigated by scanning electron microscope (SEM). Shapiro Wilk, one‐way ANOVA, and Tukey HSD tests were used for statistical evaluation. Results The highest average (μTBS) values were observed in G8, whereas the lowest mean μTBS values were evident in the G1 group. Statistically significant μTBS values were found in all adhesive‐applied groups when compared with the negative control group. Notably, the application of 50% DMSO without adhesive did not lead to a statistically significant increase in μTBS values. SEM images demonstrated that acid etching partially eliminated residues on the composite surface, while air abrasion had a detrimental effect on the integrity of fiber structures. Conclusion In the repair of fiber‐reinforced dentin composite with a posterior composite, adhesive application is an effective approach. The treatment of 50% DMSO without adhesive did not confer a statistically significant advantage, and the supplemental use of acid etch or air abrasion did not show an additional benefit compared to adhesive‐only repairs. Clinical Significance Adhesive application emerges as a potent and effective strategy for the repair of bur‐roughened fiber‐reinforced dentin composites. With its limitations, the study highlights the efficacy of adhesive‐only repairs without the necessity for additional surface treatments.

Abstract:This study aims to evaluate the repair micro-shear bond strength of the CAD/CAM resin nanoceramic block treated using four different surface treatments and composite resins of different viscosities. For the current study, 96 samples with dimensions of 14 × 12 × 2 mm were obtained from a CAD/CAM resin nanoceramic block (Cerasmart) with a low-speed precision cutting saw under water cooling. The relevant samples were randomly divided into four groups according to the surface treatment processes: grinding with diamond bur, aluminum oxide airborne-particle abrasion, long-pulse laser, and short-pulse laser. Following silane application, universal adhesive was applied to all surface-treated samples and cured with an LED for 10 s. The samples prepared for the repair procedure were divided into two subgroups (microhybrid composite and injectable composite) according to the viscosity of the repair material to be used (n = 12). After the repair procedure, care was taken to keep the samples in distilled water in an incubator at 37 °C for 24 h. The repair micro-shear bond strength values (μSBSs) of CAD/CAM resin nanoceramic-composite resin complexes were tested. In addition, randomly selected samples from each group were examined with a scanning electron microscope to evaluate the surface topography after both surface treatments and the micro-shear bond strength test. Data were analyzed by two-way ANOVA and Bonferroni test. It was determined that the surface treatment preferred in the repair protocol significantly affected the μSBS value (p < 0.001). While the highest μSBS value was obtained with the short-pulse laser airradiation group, the lowest μSBS values were found in samples with long pulse laser irradiation. However, samples grinded with a bur and airborne-particle abrasion showed similar μSBS values (p > 0.05). The preferred composite viscosity in the repair procedure has a significant effect on the μSBS value (p < 0.001). However, the interaction between the surface treatment and the viscosity of the repair composite does not affect the μSBS values in a statistically significant way (p = 0.193). It may be recommended to clinicians to repair CAD/CAM resin nanoceramic restoration surfaces with injectable composites or after treatment with short-pulse lasers.

Abstract:Purpose The aim of this study was to analyze the impact of various surface treatments and production methods on the shear bond strength (SBS) between reline material and denture base resins. Materials and Methods One‐hundred‐twenty specimens were produced using conventional heat‐polymerization, subtractive, and additive techniques. Each group consisted of 40 specimens. The specimens were divided into four subgroups, each with 10 samples, for surface treatments. These subgroups were: (1) Control‐only monomer application, (2) 50 μm airborne‐particle abrasion, (3) 110 μm airborne‐particle abrasion, and (4) Roughening with tungsten carbide bur. Representative specimens from each subgroup were examined under SEM. Then, auto‐polymerized resin was condensed in the center of the specimens. Specimens were subjected to thermal aging (5000 cycles at 5–55°C). The SBS test was conducted and failure loads were recorded. The data were evaluated by two‐way ANOVA and Tukey pairwise multiple comparisons method (p < 0.05). Results The additively produced group showed significantly lower SBS than conventional and subtractive groups (p < 0.001), with no significant differences between the subtractive and conventional groups. Specimens that underwent monomer application only showed the lowest SBS among surface treatments, while 50 μm airborne‐particle abrasion showed the highest SBS. Based on the partial eta‐squared analysis results, the surface treatment mainly impacted SBS. Conclusions Among the surface treatment methods, treating denture bases with 50 μm airborne‐particle abrasion is more effective for maintaining adhesion, especially in the additive technique.

[Tone recognition ability for post-lingually deafened cochlear implant users using pitch information].

Abstract:OBJECTIVE The present study was aimed to assess the ability of tone recognition performance for postlingually deafed cochlear implanted adults using no-duration tone test materials. METHOD Sixteen cochlear implanted users have used their devices at least six months. Ten monosyllabic syllables were selected, and combined with four lexical tones which were made up of 40 tone tokens. Then, these original words were recorded using one adult male and one adult female native Beijing Mandarin speaker. The speakers were asked to record these 40 monosyllabic words multiple times, and the 80 tokens in which the durations of four tones in each monosyllabic word were within 5 ms precision were chosen as the original tone tokens. The subjects were asked to perform a four alternative forced-choice study and select the tone they had heard. RESULT The mean score of lexical tone perception tasks for 16 cochlear implanted users is (70.7 ± 22.0)%, and Tone 4 was the easiest to perceive, while Tone 2 was the hardest for cochlear implanted subjects to perceive. Tone 1 was more often misperceived as Tone 2 relative to other tones. Tone 2 was more often misperceived as Tone 1 and 3. Tone 3 was more often misperceived as Tone 2. The tone perception performance was positively correlated with duration of hearing aid use. CONCLUSION Duration cues may impact on the ability of tone recognition performance for cochlear implant subjects, especially for tone 3. Due to the limited ability for cochlear implant subjects on pitch perception, tone l and tone 2, and tone 2 and tone 3 are most often confused with each other without duration cues. The experience of hearing aid use may benefit for postlingual deafness cochlear implant recipients on tone perception performance.

Abstract:Background. Zirconia, with its excellent mechanical properties, has become a popular choice for esthetic and durable restorations due to the increasing demand of patients. It has overcome most of the limitations of all ceramic restorations. However, bonding to zirconia remains a challenge. Objectives. This study is aimed at assessing the effect of surface treatment with alkaline agents at two different temperatures on microshear bond strength (μSBS) of zirconia to composite resin. Materials and Methods. This in vitro, experimental study was conducted on zirconia blocks measuring 2×4×8 mm. The blocks were sandblasted with alumina powder and randomly assigned to 5 groups ( n=16 each). The blocks in groups 1 and 2 underwent surface treatment with sodium hydroxide (NaOH) and groups 3 and 4 with zirconium hydroxide (Zr(OH)4) at room temperature and 70°C. Group 5 served as the control group and did not receive any surface treatment. After the application of bonding agent and its light-curing, composite cylinders in plastic tubes were bonded to the surface of each block and cured. After incubation, they underwent μSBS test. Data were analyzed by one-way ANOVA and Tukey's test ( alpha=0.05 ). Results. The μSBS was significantly higher in all intervention groups than that in the control group ( P<0.05 ). The μSBS in Zr(OH)4 groups was significantly higher than that in NaOH groups ( P<0.05 ). The mean μSBS of heated groups was slightly, but not significantly, higher than the corresponding room temperature groups ( P>0.05 ). Conclusion. Surface treatment of zirconia with NaOH and Zr(OH)4 alkaline agents can increase its μSBS to composite resin; Zr(OH)4 was significantly more effective than NAOH for this purpose, but heating did not have a significant effect on μSBS.

The effect of surface treatments of tooth on the shear bond strength of direct composite veneers: An in vitro scanning electron microscope study

Abstract:Background: The bond strength between tooth structure and restorative materials is critical for the long-term success of dental restorations. This study sought to determine how the shear bond strength (SBS) and type of bond breakdown were affected by the inclusion of sandblasting (SB) as a conditioning step before acid etching (AE). Materials and methods: A total of 40 extracted human molars were split into two groups at random: intervention (SB followed by AE) and control (AE only). Composite resin cylinders were constructed on the tooth surface following the application of the bonding agent. A universal testing device was used to determine the SBS, and scanning electron microscopy was used to assess the kind of bond failure. Results: In comparison to the control group, the intervention group displayed a significantly higher SBS (P < 0.05). In contrast to the control group, which showed more adhesive failure at the tooth-resin interface, the intervention group also demonstrated a larger percentage of cohesive failure within the composite resin. Conclusion: As an extra conditioning step, AE after SB considerably increased the SBS between tooth structure and composite resin. The fact that the intervention group experienced a higher percentage of cohesive failure shows that this technique could boost the bond's longevity.
Improving the bonding stability between resin cements and zirconia-based ceramic using different surface treatments

Abstract:Purpose: To evaluate the effects of airborne-particle abrasion with alumina particles or silicamodified alumina particles on the bond strength between zirconia and conventional MDP-based (Ph; Panavia F 2.0, Kuraray) or self-adhesive (SA; RelyX U200, 3M ESPE) resin cements. Materials and methods: Five surface treatments were evaluated: C = no surface treatment; AB = airborne-particle abrasion with alumina particles (BIO-ART Dental Supplies and Equipment); ABP = AB combined with MDP-based primer (Alloy Primer, Kuraray); SS = airborne-particle abrasion with silica-modified alumina particles (CoJet, 3M ESPE) combined with silane (RelyX Ceramic Primer, 3M ESPE); and SSP = SS combined with MDP-based primer. The surface roughness (Ra) of the airborne particle-abraded samples (n = 5) was measured by a contact profilometer (Mitutoyo Surftest SJ-401, Mitutoyo). Cylinders of the resin cements tested were bonded to the surface-treated zirconia. The microshear test was performed by the application of a load with a wire loop parallel to the adhesive interface until debonding of the resin cement cylinders. The microshear bond strength (μSBS) of the samples was measured before and after being subjected to thermocycling (TC; 5°C to 55°C, 60 seconds dwell time for 3,000 cycles; MSCT, Marcelo Nucci). One-way (Ra) and two-way (μSBS) analysis of variance followed by Tukey post hoc test (α = .05) were used. Results: The SS (Ph: 12.6 MPa; Ph-TC: 6.37 MPa; SA: 11.8 MPa; SA-TC: 9.37) and SSP (Ph: 10.4 MPa; Ph-TC: 5.82 MPa; SA: 10.4 MPa; SA-TC: 10.0) surface treatments produced the highest surface roughness values (P < .001). The SS samples achieved the highest immediate bond strength for both resin cements. However, after TC, SA resin cement associated with any surface treatment produced the highest bond strength values. Conclusion: The self-adhesive resin cement promoted higher and more stable bond strength values when associated with a surface roughening method.
Repair of Bulk-Fill and Nanohybrid Resin Composites: Effect of Surface Conditioning, Adhesive Promoters, and Long-Term Aging

Abstract:The aim of the study was to investigate the effect of different repair procedures on the repair bond strength of bulk-fill and nanohybrid resin composites after different aging periods. The resin composite blocks (8 × 8 × 4 mm3) were prepared from a bulk-fill (reliaFIL Bulk) and a nanohybrid (reliaFIL LC) resin composite and grouped according to aging duration (6 months, 1 year, and 2 years). Following aging, the blocks were assigned to different surface treatments; air-abrasion with aluminum oxide powder, roughening with a diamond bur, and no treatment. After cleansing with phosphoric acid, a silane layer (Porcelain Primer) was applied on the surface of half of the specimens in each group. The specimens were subdivided into two groups (n = 5): Scotchbond Universal (3M Oral Care) and All-Bond Universal (Bisco). The blocks were repaired with the nanohybrid composite (8 × 8 × 8 mm 3). The repaired specimens were stored in distilled water (37 °C/24 h) and segmented into beams. Half of the beams were immediately subjected to microtensile μTBS testing (1 mm/min), while the other half was stored in distilled water (37 °C) for 6 months before testing. Failure modes were analyzed using stereomicroscope and SEM. Statistical analyses were performed with ANOVA and least significant difference tests (LSD) tests (p = 0.05). The extension of aging periods (6 months, 1 year, and 2 years) reduced the repair bond strength in some groups for both resin composites (p < 0.05). The air-abrasion and bur roughening improved the repair bond strength (p < 0.05). The silane application did not influence the repair bond strength and durability (p > 0.05). There was no difference among the universal adhesives in the same surface treatment groups (p > 0.05). The mechanical roughening treatments are necessary for the repair of resin composite. The universal adhesives might be used for the repair of resin composites regardless of silane content without prior silane application.

Abstract:Objective This study aimed to examine the shear bond strength (SBS) of repair material to conventionally, subtractive‐, and additive‐manufactured denture bases after different surface treatments. Materials and Methods Disk‐shaped test specimens (N = 300) were prepared from denture base materials produced by one conventional (Procryla), one subtractive (Yamahachi), and one additive (Curo Denture) method. The test specimens were randomly divided into five groups (n = 10) and exposed to a variety of surface treatments—Group A: no surface treatment; Group B: grinding with silicon carbide paper; Group C: sandblasting; Group D: erbium: yttrium‐aluminum‐garnet laser; and Group E: plasma. Repair was performed with autopolymerizing acrylic resin (Meliodent). Surface roughness analyses were performed with a profilometer. Abstract:OBJECTIVE: This clinical study aimed to evaluate the clinical performance of an alkasite-based bioactive material by comparing it with a resin composite (RC) in the restoration of Class II cavities over a year.METHODOLOGY: A hundred Class II cavities were restored at 31 participants. Groups were as follows: Cention N (CN) (Ivoclar Vivadent, Schaan, Liechtenstein) and G-ænial Posterior (GP) (GC, Tokyo, Japan) in combination with G-Premio Bond (etch&rinse). Restorative systems were applied following manufacturers' instructions. They were finished and polished immediately after placement and scored based on retention, marginal discoloration, marginal adaptation, sensitivity, surface texture, and color match using modified USPHS criteria after 1 week (baseline), 6 months, and 12 months. Statistical analyses were performed using chi-square, McNemar's, and Kaplan Meier tests.RESULTS: After 12 months, the recall rate was 87%. Survival rates of CN and GP restorations were 92.5% and 97.7%, respectively. Three CN and one GP restorations lost retention. Seven CN (17.9%) and five (11.6%) GP restorations were scored as bravo for marginal adaptation and no significant difference was seen between groups (p=0.363). One (2.7%) CN and two GP (4.7%) restorations were scored as bravo for marginal discoloration, but no significant difference was observed between groups(p=1.00). For surface texture, three (8.1%) CN and three (7%) GP restorations were scored as bravo (p=1.00). None of the restorations demonstrated post-operative sensitivity or secondary caries at any examinations.CONCLUSION: The tested restorative materials performed similar successful clinical performances after 12 months. Abstract:Objective: to evaluate microshear bond strength of CAD/CAM composite block and hybrid ceramic block repaired by direct resin composite restoration with and without surface treatment.Materials and Methods: Two different CAD/CAM restorative materials were tested in the present study: hybrid ceramic blocks (Vita Enamic) and resin composite blocks (Grandio). Six sections were prepared from each CAD/CAM blocks. The sections from each block type were randomly divided into three groups according to the surface treatment performed. Control group: the specimens were ground with a grinding machine. Etching gel Group: 37% phosphoric acid etching gel was applied occlusally on the surface of the section and CoJet Group. the surface of all groups were then silanized using Bis-Silane then bonding agent Futura bond DC was applied. A polyethylene micro tubes were prepared that were held in place with a tweezer on the uncured then light cured for 10 seconds Resin composite (Grandioso X-tra) was packed inside the microtubes and light cured. All specimens were prepared by the same operator at 22.0–22.5 ˚C (room temperature) and relative humidity of 50%. The specimens were further divided into two groups according to storage conditions. Half of the specimens were thermocycled in distilled water for 5000 cycles in a 5–55˚C water bath with a dwell time of 30 seconds and a transfer time of 5 seconds. The other specimens were stored in distilled water for 2 days. Microshear bond strength was performed and fractographic analysis to determine the failure mode. Statistical analysis was then performed using a commercially available software program (SPSS 19; SPSS, Chicago, IL, USA). As data were not normally distributed, significance of the difference between different groups within the same surface pretreatment was evaluated using Kruskall Wallis test. Mann Whitney U test was used for pairwise comparisons and to study the effect of surface pre-treatment. The interaction of restoration and type of pretreatment variables was evaluated using 3 ways ANOVA. The level of significance was set at P < 0.05.Results: Three ways ANOVA test revealed that each of the material type (composite/hybrid ceramic), the subgroup type group (control/etching/Cojet) and the thermocycling (Yes/No) variables resulted in a statistically significant difference (p=0.00, p=0.00, p=0.003 respectively). Interaction of the subgroup (control/etching/Cojet) variable with the thermocycling (Yes/No) variable and the material (composite/hybrid ceramic) variable showed a statistically significant difference (P=0.023, p=0.001 respectively). Moreover, interaction of thermocycling and material variable resulted in a significant difference (p=0.034). However, interaction of the 3 study variables (material, subgroup, thermocycling) was not statistically significant (p=0.165). The failure mode of the tested specimens analyzed using Scanning Electron Microscope at magnification 80X was commonly mixed type of failure (adhesive failure between resin composite restoration and CAD/CAM blocks with cohesive failure within resin composite restoration).Conclusions: Grandio composite blocks is very promising materials and their optimum long lasting repair potentiality that offers to the clinicians great chance to easily repair the defective restoration with safety. Surface treatment using Phosphoric acid or Cojet system greatly improve the bond strength between composite repair system and hybrid ceramic or composite blocks. The bond performance of repaired composite blocks was much more greater than than that of repaired hybrid ceramic. Thermocycling affect the bond durability of repaired hybrid ceramic blocks and composite blocks.Further investigations focusing on the effect of different surface treatments to yield results that lead to concrete clinical recommendations are needed to evaluate the long term durability of new CAD/CAM materials.

Abstract:Background Hybrid ceramics bonding with the dentinal substrate is crucial for clinical success and longevity. To enhance adhesion, the surface of ceramic restorations is modified through various conditioning techniques Aim Effect of Different Surface Conditioners Erbium-doped yttrium aluminum garnet (ER: YAG) laser and Low-level laser therapy (LLLT) activated Methylene blue (MB) on the surface roughness (Ra) and shear bond strength (SBS) of Polymer infiltrating ceramic network (PICN) discs bonded using different viscosity resin cement before and after thermal aging. Material and Methods One hundred and fifty human central incisors and one hundred and fifty-three PICN discs were prepared. PICN discs were randomly allocated into three groups based on the surface conditioning(n=51) Group 1:10% HF acid-S, Group 2: LLLT (MB), and Group 3: Er: YAG laser. Following conditioning Ra scores of ten samples were performed. Surface topography of samples was performed using a scanning electron microscope (SEM). Bonding of forty PICN discs from each conditioning group was performed with high (A) or low viscosity (B) dual-curing resin cement (n= 20 each). Each subgroup was divided into two cohorts and subjected to varying storage conditions. The SBS test and failure mode analysis were performed using a universal testing machine and stereomicroscope. Descriptive statistics of SBS and Ra for each group were analyzed using ANOVA and Tukey post hoc tests. HF acid-S attained the highest Ra scores. Results Group 1A (HF-S+HIGH) samples achieved the highest SBS values at baseline. Group 3A, on the other hand, displayed the lowest bond score (Er: YAG laser+HIGH) after thermal aging Intergroup comparison analysis at baseline unveiled that Group 1A and Group 1B (HF(S) + LOW) displayed no significant difference in their bond strength scores (p>0.05). Following artificial aging, it was observed that Group 2A (LLLT (MB) + HIGH) and Group 3A (Er:YAG laser + HIGH) ) presented comparable SBS(p>0.05). Thermal aging decreased SBS significantly in all groups. PICN conditioned with HF-S results in high surface roughness. Conclusion PICN conditioned with HF acid with silane and bonded with low-viscosity resin cement to the dentinal substrate is preferable. Aging influences SBS.
