Abstract:The arteriovenous fistula (AVF), which is a vasculature created for end-stage renal disease patients who require haemodialysis, is susceptible to many vascular diseases. It is well known that disturbed hemodynamics is a factor in the initiation of vascular disease; however, only a limited number of experimental studies have been conducted to assess the flow behaviour within the AVF. The current study is an investigation of the complex three-dimensional flow within a physiological AVF geometry, using tomographic particle image velocimetry. To this end, a benchtop model of a patient-specific geometry was created by casting silicone around a soluble 3D print of the vessels. The patient-specific boundary conditions were reproduced by driving a refractive-index matched working fluid from a pulsatile pump, which was connected to the model via a tubing network inclusive of valves and compliance chambers. The seeded working fluid was illuminated with a 1 kHz double-pulsed laser, and four high-speed cameras placed at optimised locations were used to capture the particle images, with the volume reconstructions refined by a 3D internal mask created using morphological operations. The velocity magnitude contour plots of the resulting vector field revealed two zones of low velocity in the anastomosis, while locations of high turbulent kinetic energy are observed in the anastomosis and venous regions. These results suggested that the collision of the two opposing inlet flows and the curvature at the anastomosis cause disturbance in the flow that is carried downstream.

Abstract:Background and ObjectiveArteriovenous fistulae (AVF) are the preferred mode of hemodialysis vascular access and their successful maturation is critical to reduce patient morbidity, mortality, cost, and improve quality of life. Peri-anastomotic venous segment stenosis is the primary cause of AVF maturation failure. The objective is to develop a software protocol for the functional analysis of arteriovenous fistula.MethodWe have developed a standard protocol for the anatomical analysis of the AVF to better understand the mechanisms involved in AVF stenosis and to identify future imaging biomarkers for AVF success or failure using non-contrast magnetic resonance imaging (MRI). The 3D model of the AVF is created using a polar dynamic programming technique. Analysis has been performed on six Yorkshire cross domestic swine, but techniques can be applied into clinical settings.ResultsDifferences in AVF angles and vein curvature are associated with significant variability of venous cross-sectional area. This suggests that the pattern of stenosis is likely to be dependent upon hemodynamic profiles which are largely determined by AVF anatomical features and could play an important role in AVF maturation.ConclusionsThis protocol enables us to visualize and study the hemodynamic profiles indirectly allowing early stratification of patients into high and low risk groups for AVF maturation failure. High risk patients could then be targeted with an enhanced process of care or future maturation enhancing therapies resulting in a much-needed precision-medicine approach to dialysis vascular access.

Abstract:Background: Oscillatory wall shear stress and related metrics have been identified as potential predictors of dialysis access outcomes; however, the absence of a simple non-invasive method for measuring these haemodynamic forces has been prohibitive to their adoption into routine clinical practice. We present a computationally enhanced, single patient case study, offering a unique insight into the haemodynamic environment surrounding the development of flow limiting neointimal hyperplasia within the efferent vein of a previously functional arteriovenous fistula (AVF). Methods: Computational fluid dynamics (CFD) simulations were used to create a quantitative map of oscillatory shear stress as well as enabling visualisation of streamline patterns within the AVF. CFD data was compared to ultrasound-based turbulence quantification and examined alongside structural and functional changes in the access site over time. Results: This work further supports the notion that flow limiting neointimal hyperplasia development in vascular access fistulae, occurs in response to oscillatory wall shear stress, and provides proof of concept for the idea that non-invasive ultrasound turbulence quantification tools could play a role in predicting vascular access outcomes. Conclusions: In addition to providing insight into the haemodynamic environment surrounding the development of flow limiting neointimal hyperplasia, we hope that this paper will promote discussion and further thinking about how our learnings from in-silico studies can be incorporated into clinical practice through novel uses of existing diagnostic tools.

Abstract:Arteriovenous fistula (AVF) is the first choice for providing vascular access for hemodialysis patients, but maintaining its patency is challenging. AVF failure is primarily due to development of neointimal hyperplasia (NH) and subsequent stenosis. Using idealized models of AVF we previously suggested that reciprocating hemodynamic wall shear is implicated in vessel stenosis. The aim of the present study was to investigate local hemodynamics in patient-specific side-to-end AVF. We reconstructed realistic geometrical models of four AVFs from magnetic resonance images acquired in a previous clinical study. High-resolution computational fluid dynamics simulations using patient-specific blood rheology and flow boundary conditions were performed. We then characterized the flow field and categorized disturbed flow areas by means of established hemodynamic wall parameters. In all AVF, either in upper or lower arm location, we consistently observed transitional laminar to turbulent-like flow developing in the juxta-anastomotic vein and damping towards the venous outflow, but not in the proximal artery. High-frequency fluctuations of the velocity vectors in these areas result in eddies that induce similar oscillations of wall shear stress vector. This condition may importantly impair the physiological response of endothelial cells to blood flow and be responsible for NH formation in newly created AVF.

Abstract:The transitional flow which initiates within the junction (anastomosis) of an arteriovenous fistula (AVF) is known to be a contributing factor in the onset of vascular disease. A novel treatment method involving the implantation of a flexible stent across the anastomosis has enabled the retention of a large proportion of functioning AVFs, despite the propensity for stent malapposition to occur at the sharp inner curve of the anastomosis. Large eddy simulations of a single patient-specific AVF with and without the presence of a stent captured oscillatory flow behavior emanating from the interface of the two inlet flows in the stent-absent case, however, these oscillatory features were subdued in the stented case. The stent-absent case generally had higher turbulent kinetic energy (TKE) in the anastomosis which led to larger cycle-to-cycle variations in wall shear stress (WSS). The significantly lower TKE generated at the heel of the stented AVF was contained within the malapposed stent, thereby resulting in lower WSS fluctuations. However, a slight increase in turbulence downstream of the malapposed stent edge was noted. This detailed study reveals a significant decrease in turbulence within the AVF in the presence of the stent, thereby providing a level of understanding underpinning the success of the treatment strategy (in this patient case) from a fluid dynamic perspective. https://doi.org/10.1103/PhysRevFluids.7.123101 ©2022 American Physical Society

Abstract:Background: Guidelines recommend placing native arteriovenous fistulas (AVFs) as far distally in the upper extremity as possible. If there are adequate veins and adequate arteries, a wrist fistula, which offers notably lower risks than grafts and catheters, would be the first choice for long-term hemodialysis. With increasing failure and difficulty to create wrist fistulas, we reviewed outcomes of the proximal radial AVF (PRAAVF) and demonstrate that it is an effective technique. Methods: A systemic literature research was conducted in PubMed and related bibliographies. The focus of data extraction was primary failure, primary patency rates, and secondary patency rates after 1 and 2 years. Estimates were pooled with the random effects model, and meta-regression and sensitivity analysis were performed to explore heterogeneity. Results: According to selection criteria formulated a priori, 10 articles (n [ 1310) were included and finally analyzed after screening 1687 articles. The pooled primary failure was 12.3% (95% confidence interval [ CI], 7.6%-17.0%; chi(2) = 70.8, I-2 = 87.3%), the primary patency, including primary failure, was 73.6% (95% CI, 52.4%-94.9%; chi(2) = 71.3, I-2 = 97.2%) at 1 year and 70.5% (95% CI, 50.6%-90.5%; chi(2) = 58.8, I-2 = 96.6%) at 2 years. Secondary patency was 80.0% (95% CI, 72.8%-87.2%; chi(2) = 24.42, I-2 = 75.4%) at 1 year and 73.7% (95% CI, 65.2%-82.2%; chi(2) = 28.51, I-2 = 79.0%) at 2 years. Individual variate meta-regression analysis found the definition of primary failure was a significant source of heterogeneity (P = .009). Steal syndromes developed in four of 832 (0.5%) of the PRAAVFs, and venous hypertension developed in four of 284 (1.4%). Conclusions: The PRAAVF presented low to moderate primary failure and high primary and secondary patency rates with acceptable complications. Consideration of the specific fistula is required when creating a vascular access, especially when a wrist fistula has failed or is predicted to be unsuccessful.

Abstract:Arteriovenous fistula (AVF) is the preferred vascular access for hemodialysed patients. AVF is created surgically using the patient's artery and vein. Once the connection (anastomosis) is made, the maturation process begins. Studies have shown that most AVFs do not survive beyond one year. This study presents fluid–structure interaction (FSI) modelling of non-Newtonian blood flow through an end-to-side radio-cephalic AVF, investigated weekly during a 15-week follow-up period and 1.5 years postoperatively using ultrasound methods. The aim was to collect qualitative and quantitative data regarding changes in hemodynamics and alterations in the walls of AVF vasculature. Different material properties were assigned to the artery, suture zone (anastomosis), and vein, while the stiffening of the venous arm over time was also modelled. The proposed FSI methodology can be implemented in future follow-up studies involving groups of patients. The main findings revealed: a) counter-rotating vortices in the anastomosis cross-section affecting local pressure conditions; b) different temporal progression of vorticity, shear strain rate, and turbulent kinetic energy and similarity of the temporal progression of WSS obtained under the assumptions of the rigid-walled and FSI; c) a negligible low-WSS zone in the presented thrombosis-free AVF; d) migration of the zone of maximal temporal wall deformation over time.

Computational analysis for competition flows in arteriovenous fistulas based on non-contrast magnetic resonance imaging

Abstract:Introduction: Characteristics of hemodynamics strongly affect the patency of arteriovenous fistula (AVF) in hemodialysis patients. Because of pressure balance changes among arteries after AVF construction, regurgitating flow occurs in some patients. Methods: Based on phase-contrast MRI measurements, flow types around the anastomotic site are classified to the three different types of splitting, merging, and one-way, where merging type incorporates regurgitating flow. We have performed computational simulations to analyze characteristic differences among these types. Results: In the merging type, a characteristic spiral flow is observed in AVF causing strong wall shear stress and large pressure drop, whereas the splitting type shows a smooth flow and gives a smaller pressure drop. The one-way case is intermediate between splitting and merging types. Conclusion: Regurgitation brings about high wall shear stress near the anastomotic site because of instabilities induced by merging phenomena, for which type careful follow-up examinations are regarded as necessary.

Background:The development of a high flow rate arteriovenous fistula (AVF) can expose the patient to development of heart failure due to increased cardiac preload and pulmonary hypertension.Objectives:AVF flow measurement (Qa) is considered a screening tool for AVF surveillance, aiming to evaluate the access dysfunction and prevent complications, like a non-maturation, suspected stenosis, high-flow AVF, and distal ischemia. In the upper arm AVF, a high Qa may develops, which can expose the patient to the risk of high-output heart failure and ischemia. Although, the exact threshold to define high-flow access is not universally accepted, AVF with a Qa of 1–1.5 L/min or cardio-pulmonary recirculation (Qa/CO) >20% are considered at risk.Subjects:In our work we describe the treatment performed in three patients with high flow AVF treated with DRIL technique with interposition of a Prosthetic Patch, revascularization procedures such as distal inflow revision or RUDI and with innovative technique a "tench snout," removal the previous anastomosis and reconstruction of the integrity of the radial artery at the terminal in pre and post anastomosis. A PTFE prosthetic segment measuring 5 cm in length and 5 mm in diameter was interposed, terminally anastomosed with the efferent cephalic vein and terminally lateral with the radial artery, reducing the anastomosis to approximately 4 mm.Results:All treated patients showed a clear improvement in the clinical picture in particularly heart failure. The calculation of the post-intervention flow rate approximately 1500 mL/min.Conclusion:The patient on hemodialysis with arteriovenous fistula must be constantly monitored with clinical examination, monitoring during the hemodialysis session and color Doppler ultrasound of the AVF with calculation of the flow rate. The surgical technique used for flow reduction is chosen on the surgical experience of each operator with the main objective of preserving the autologous AVF.

Abstract:Purpose. Most computational fluid dynamics (CFD) studies on arteriovenous grafts (AVGs) adopt idealised geometries and simplified boundary conditions (BCs), potentially resulting in misleading conclusions when attempting to predict neointimal hyperplasia (NIH) development. Moreover, they often analyse a limited range of hemodynamic indices, lack validation, and fail to link the graft-altered hemodynamics with follow-up data. This study develops a novel patient-specific CFD workflow for AVGs using pathophysiological BCs. It validates the CFD results with patient medical data and assesses the co-localisation between CFD results and NIH regions at follow-up. Methods. Contrast-enhanced computed tomography angiography images were used to segment the patient-specific AVG geometry. A uniform Doppler ultrasound (DUS)-derived velocity profile was imposed at the inlet, and three-element Windkessel models were applied at the arterial outlets of the domain. Transient, rigid-wall simulations were performed using the k-omega SST turbulence model. The CFD-derived flow waveform was compared with the DUS image of the patient to ensure validation. Turbulent kinetic energy (TKE), helicity and near-wall hemodynamic descriptors were calculated and linked with regions presenting NIH from a 4-month follow-up fistulogram. Results. In the analysed patient, areas presenting high TKE and balanced helical flow structures at baseline exhibit NIH growth at follow-up. Transverse wall shear stress index is a stronger predictor of NIH than other commonly analysed near-wall hemodynamic indices, since luminal areas subjected to high values greatly co-localise with observed areas of remodelling. Conclusion. This patient-specific computational workflow for AVGs could be applied to a larger cohort to unravel the link between altered hemodynamics and NIH progression in vascular access.
Detection of Abnormal Wall Shear Stress and Oscillatory Shear Index via Ultrasound Vector Flow Imaging as Possible Indicators for Arteriovenous Fistula Stenosis in Hemodialysis

Abstract:Objective The arteriovenous fistula (AVF) is an essential vascular access for hemodialysis patients. AVF stenosis may occur at sites with abnormal wall shear stress (WSS) and oscillatory shear index (OSI), which are caused by the complex flow in the AVF. At present, an effective method for rapid determination of the WSS and OSI of the AVF is lacking. The objective of this study was to apply an ultrasound-based method for determination of the WSS and OSI to explore the risk sites of the AVF. Methods In this study, the ultrasound vector flow imaging technique V Flow was applied to measure the WSS and OSI at four different regions of the AVF to detect and analyze the risk sites: (i) anastomosis region, (ii) curved region, (iii) proximal vein and (iv) distal vein. Twenty-one patients were included in this study. The relative residence time was calculated based on the measured WSS and OSI. Results The curved region had the lowest WSS; the anastomosis region had a significantly higher OSI ( p < 0.05) compared with the venous regions, and the curved region had a significantly higher RRT ( p < 0.05) compared with the proximal vein region. Conclusion V Flow is a feasible tool for studying WSS variations in AVF. The possible risk site in the AVF may be located in the anastomosis and curved regions, where the latter could present a higher risk for AVF stenosis.

Abstract:The Journal of Vascular Access, Ahead of Print. Background:Ultrasonography is valuable for assessing arteriovenous fistula (AVF) maturation. Brachial artery flow volume (FV) measured at 6 weeks post-AVF creation can predict AVF failure. However, the association between early postoperative FV and AVF failure remains unclear. The resistance index (RI) may also serve as a prognostic factor for AVF maturation; however, it has not been extensively studied. Therefore, we aimed to investigate the relationship between AVF failure and early postoperative FV and RI.Methods:We retrospectively analyzed data from 102 patients (mean age, 67.8 ± 14.2 years; male, 68.6%; diabetes mellitus, 52.0%) who underwent new radiocephalic-AVF creation between January 2019 and December 2023 in Japan. An ultrasound device was used to measure brachial artery FV and RI on postoperative days 0 and 1. AVF failure was defined as occlusion or stenosis requiring vascular access intervention or surgical revision before the first cannulation, or cannulation difficulty or FV dysfunction requiring vascular access intervention or surgery at the first cannulation.Results:On postoperative day 0, FV was 383.1 ± 146.8 mL/min, and RI was 0.65 ± 0.11. On postoperative day 1, FV was 466.9 ± 179.2 mL/min, and RI was 0.62 ± 0.11. FV significantly increased, and RI was significantly reduced on postoperative day 1 compared to those on day 0 (p < 0.01). Multivariate analyses revealed that higher RI (per 0.1; odds ratio (OR), 2.16; 95% confidence interval (CI), 1.22–3.82), but not FV, was significantly associated with AVF failure on postoperative day 0. On postoperative day 1, lower FV (per 100 mL/min; OR, 0.63; 95% CI, 0.42–0.95) and higher RI (per 0.1; OR, 2.17; 95% CI, 1.16–4.05) were significantly associated with AVF failure.Conclusions:This study highlights RI as a predictor of AVF failure in the early postoperative period when vasospasm is likely to occur.

Abstract:A significant number of arteriovenous fistulas (AVFs) fail to maturate for dialysis. Although interventions promote maturation, functional primary patency loss is higher for AVFs with interventions (assisted maturation) than AVFs without interventions (un-assisted maturation). Although blood flow-associated hemodynamics have long been proposed to affect AVF remodeling, the optimal hemodynamic parameters for un-assisted maturation are unclear. Additionally, AVF maturation progress is generally not investigated until 6 weeks after AVF creation, and the examination is focused on the AVF’s venous limb. In this exploratory study, patients ( n = 6) underwent magnetic resonance imaging (MRI) at 1 day, 6 weeks, and 6 months after AVF creation surgery. Before successful use for hemodialysis, three AVFs required intervention and three did not. MRI of the AVFs were used to calculate lumen cross-sectional area (CSA) and perform computational fluid dynamics (CFD) to analyze hemodynamics, including velocity, wall shear stress (WSS), and vorticity. For the venous limb, the no-intervention group and intervention group had similar pre-surgery vein diameter and 1-day post-surgery venous CSA. However, the no-intervention group had statistically larger 1-day venous velocity (0.97 ± 0.67 m/s; mean ± SD), WSS (333 ± 336 dyne/cm 2 ) and vorticity (1709 ± 1290 1/s) than the intervention group (velocity = 0.23 ± 0.10 m/s; WSS = 49 ± 40 dyne/cm 2 ; vorticity = 493.1 ± 227 1/s) ( P < 0.05). At 6 months, the no-intervention group had statistically larger venous CSA (43.5 ± 27.4 mm 2 ) than the intervention group (15.1 ± 6.2 mm 2 ) ( P < 0.05). Regarding the arterial limb, no-intervention AVF arteries also had statistically larger 1-day velocity (1.17 ± 1.0 m/s), WSS (340 ± 423 dyne/cm 2 ), vorticity (1787 ± 1694 1/s), and 6-month CSA (22.6 ± 22.7 mm 2 ) than the intervention group (velocity = 0.64 ± 0.36 m/s; WSS = 104 ± 116 dyne/cm 2 , P < 0.05; vorticity = 867 ± 4551/s; CSA = 10.7 ± 6.0 mm 2 , P < 0.05). Larger venous velocity, WSS, and vorticity immediately after AVF creation surgery may be important for later lumen enlargement and AVF maturation, with the potential to be used as a tool to help diagnose poor AVF maturation earlier. However, future studies using a larger cohort are needed to validate this finding and determine cut off values, if any.

The aim of this study is to evaluate the outcomes of patients referred for AVF construction at a single center in Cork, Ireland.</p><h3 class="Heading">Methods</h3><p class="Para">The current study is a single-center retrospective review of all patients who underwent AVF creation between 2015 and 2017. Additionally, the kidney disease clinical patient management system was used to provide statistics on AVF use in Ireland.</p><h3 class="Heading">Results</h3><p class="Para">39.3% of hemodialysis patients in Ireland use an AVF for vascular access. Regional use ranged from 50 to 20% across Irish hemodialysis centers. At Cork University Hospital, 192 AVFs were created. The population was 69.3% male (<em class="EmphasisTypeItalic">n</em> = 133), 30.7% female (<em class="EmphasisTypeItalic">n</em> = 59) with a mean (±SEM) age of 58.8 ± 1.03 years. 69.5% of females received a brachiocephalic AVF (BCAVF) while 13.6% had a radiocephalic AVF (RCAVF) constructed. Significance was seen when comparing gender and AVF type (<em class="EmphasisTypeItalic">p</em> < 0.001). Fifty-four percent of the fistulae were brachiocephalic (<em class="EmphasisTypeItalic">n</em> = 103), 33% were radiocephalic (<em class="EmphasisTypeItalic">n</em> = 63), and 4% were brachiobasilic (<em class="EmphasisTypeItalic">n</em> = 8). BCAVF patients (62.7 ± 1.2 years) were significantly older than patients receiving a RCAVF (54.5 ± 1.9 years, <em class="EmphasisTypeItalic">p</em> < 0.001). A post-operative thrill or continuous flow on Doppler was present in 99% of patients (<em class="EmphasisTypeItalic">n</em> = 190) with maturation and complication rates of 82.7% (<em class="EmphasisTypeItalic">n</em> = 153) and 5.7% (<em class="EmphasisTypeItalic">n</em> = 11) respectively. 69.9% of AVFs were needled for hemodialysis (<em class="EmphasisTypeItalic">n</em> = 114).</p><h3 class="Heading">Conclusions</h3><p class="Para">AVF outcomes at this center are consistent with reported statistics in the literature. Patient age, sex, and diabetic status may influence the use of proximal AVF. AVF creation rates in Ireland are below international reported recommendations.</p>

Abstract:An arteriovenous fistula (AVF) is the preferred vascular access for chronic hemodialysis, yet the rates of AVF maturation failure and reintervention remain high. We investigated the AVF geometric parameters and their associations with AVF physiologic maturation and reintervention in a prospective multi-center study.From 2011 to 2016, patients undergoing a vein end-to-artery side upper-extremity AVF creation surgery were recruited. Contrast-free dark-blood and phase-contrast magnetic resonance imaging (MRI) scans were performed using 3.0T scanners to obtain AVF lumen geometry and flow rates, respectively, at postoperative Day 1, Week 6, and Month 6. Arterial-venous anastomosis angle, nonplanarity, and tortuosity of the fistula were calculated based on lumen centerlines. AVFs were considered physiologically matured if, based on the Week 6 MRI data, the flow rate was at least 500 mL/min, and the minimum vein lumen diameter was at least 5 mm. Associations of these geometric parameters with AVF maturation and reintervention due to peri-anastomotic and mid-vein stenosis within one year were assessed.There were 111 patients with usable Day 1 scan, with the majority being upper-arm AVFs (n = 73). Compared to forearm AVFs, upper-arm AVFs had greater anastomosis angles (P < .001), larger deviations from a plane (nonplanarity, P = .002), and more prominent tortuosity (P = .038) at Day 1. These parameters significantly increased between Day 1 and Week 6 in upper-arm AVFs; in contrast, significant changes in these parameters in forearm AVFs were not observed. The rate of maturation was 54% and 86% for forearm and upper-arm AVFs, respectively, with none of the geometric parameters at Day 1 being associated with maturation for AVFs in either location. The rate of reintervention was 24% and 30% for forearm and upper-arm AVFs, respectively, with a larger nonplanarity angle at Day 1 being associated with less reintervention (30° ± 15° vs. 21° ± 10°, P = .034) in upper-arm AVFs only. This relationship was unchanged after adjusting for age, sex, race, dialysis status, or diabetes.Upper-arm fistulas have a larger anastomosis angle, are more nonplanar, and have more tortuous veins than forearm fistulas. For upper-arm fistulas, a larger nonplanarity angle is associated with a lower rate of reintervention within one year. Once confirmed, vascular surgeons may consider increasing the nonplanarity angle by incorporating a tension-free gentle curvature in the proximal segment of the mobilized vein to reduce reinterventions when creating an upper-arm fistula.

Abstract:The juxta-anastomotic stenosis of an arteriovenous fistula (AVF) is a significant clinical problem in hemodialysis patients with no effective treatment. Previous studies of AV anastomotic angles on hemodynamics and vascular wall injury were based on computational fluid dynamics (CFD) simulations using standardized AVF geometry, not the real-world patient images. The present study is the first CFD study to use angiographic images with patient-specific outcome information, i.e., the exact location of the AVF stenotic lesion. We conducted the CFD analysis utilizing patient-specific AVF geometric models to investigate hemodynamic parameters at different locations of an AVF, and the association between hemodynamic parameters and the anastomotic angle, particularly at the stenotic location. We analyzed 27 patients who used radio-cephalic AVF for hemodialysis and received an angiographic examination for juxta-anastomotic stenosis. The three-dimensional geometrical model of each patient's AVF was built using the angiographic images, in which the shape and the anastomotic angle of the AVF were depicted. CFD simulations of AVF hemodynamics were conducted to obtain blood flow parameters at different locations of an AVF. We found that at the location of the stenotic lesion, the AV angle was significantly correlated with access flow disturbance (<i>r</i> = 0.739; <i>p</i> < 0.001) and flow velocity (<i>r</i> = 0.563; <i>p</i> = 0.002). Furthermore, the receiver operating characteristic (ROC) curve analysis revealed that the AV angle determines the lesion's flow disturbance with a high area under the curve value of 0.878. The ROC analysis also identified a cut-off value of the AV angle as 46.5°, above or below which the access flow disturbance was significantly different. By applying CFD analysis to real-world patient images, the present study provides evidence that an anastomotic angle wider than 46.5° might lead to disturbed flow generation, demonstrating a reference angle to adopt during the anastomosis surgery.

Abstract:Abstract An arteriovenous fistula, created by artificially connecting segments of a patient’s vasculature, is the preferred way to gain access to the bloodstream for kidney dialysis. The increasing power and availability of supercomputing infrastructure means that it is becoming more realistic to use simulations to help identify the best type and location of a fistula for a specific patient. We describe a 3D fistula model that uses the lattice Boltzmann method to simultaneously resolve blood flow in patient-specific arteries and veins. The simulations conducted here, comprising vasculatures of the whole forearm, demonstrate qualified validation against clinical data. Ongoing research to further encompass complex biophysics on realistic time scales will permit the use of human-scale physiological models for basic and clinical medicine.

Abstract:The Journal of Vascular Access, Ahead of Print. Background:Arteriovenous fistula (AVF) failure is a prevalent concern for patients with end-stage kidney disease on hemodialysis. Recognizing the efficacy of ultrasound Doppler in post-operative AVF evaluation, this study sought to discern the predictive capabilities of various ultrasonographic and color-Doppler metrics for early AVF outcomes.Methods:This single-center, prospective cohort study spanned 1 year and, post ethical clearance, included all patients who underwent native AVF creation surgery and were subsequently referred for standard post-operative ultrasound Doppler assessment. Parameters such as fistula size, cephalic vein area and diameter, and AVF flow velocity and rates were assessed on post-operative day 2, week 2, and week 6. These initial findings were juxtaposed with later outcomes to determine unassisted AVF results.Results:Of the initial cohort of 40 patients, 75% encountered AVF failure, whereas 25% realized successful unassisted AVF maturation. A notable observation was the significant variance in AVF flow rates as early as post-operative day 2. A threshold of >246 ml/min was indicative of successful unassisted AVF maturation, leading to a sensitivity of 80% and a specificity of 70%. Although the cephalic vein diameter on post-operative day 2 lacked a robust association with AVF outcomes, a cut-off of >3.4 mm, when combined with flow rate testing, augmented the cumulative sensitivity to 92%.Conclusion:Ultrasound Doppler stands out as a valuable quantitative imaging modality, adept at prognosticating AVF outcomes from as early as post-operative day 2. In particular, a flow rate exceeding 246 ml/min and a cephalic vein diameter surpassing 3.4 mm are salient indicators for the early prediction of successful AVF outcomes.

Abstract:Objective: The aim of this study is to numerically evaluate thrombosis risk within occluded coronary arterial fistulas (CAF) with terminal aneurysms, and provide guidance in choosing occlusion positions, with clinical observations as reference.Method: Four patients with CAF were studied, with different occlusion positions in actual treatments. Hemodynamics simulations were conducted, with blood residue predicted using the blood stasis model. Three types of models (untreated model, aneurysm-reserved model and aneurysm-removed model) were studeid for each patient. Four metrics, i.e., proportion of high oscillatory shear index (OSI), area of high OSI, old blood volume fraction (OBVF)) and old blood volume (OBV) was obtained to distinguish the thrombosis risk of different treatments (proximal or distal occlusion), comparing with the follow-up CTA.Results: For all the postopertive models, the high OBVF, high OSI(>0.3) and low time-averaged wall shear stress (TAWSS) regions were mainly at the distal fistula, indicating these regions were prone to thrombosis. The regions where blood residue remains are roughly regions of high OSI, corresponding well with clinical observations. In contrast, TAWSS failed to distinguish the difference in thrombosis risk. Absolute values (area of high OSI, OBV) can better reflect the degree of thrombosis risk between treatment types compared with percentage values (proportion of high OSI, OBVF). By comparing with the actual clinical treatments and observations, the OBV is superior to the area of high OSI in determining treatment type.Conclusion: The OBV, a volumetric parameter for blood stasis, can better account for the CAF thrombosis and reflect the degree of blood stasis compared with OSI or TAWSS, is a more appropriate metric for thrombosis in the fistula. Together with morphological parameters, the OBV could guide clinicians to formulate more appropriate surgical plans, which is of great significance for the preoperative evaluation and treatment prognosis of CAF patients.
