Abstract:Aims Previous studies have shown the COVID‐19 pandemic was associated with reductions in volume across a spectrum of non‐SARS‐CoV‐2 hospitalizations. In the present study, we examine the impact of the pandemic on patient safety and quality of care. Design This is a retrospective population‐based study of discharge abstracts. Methods We applied a set of nationally validated indicators for measuring the quality of inpatient care to hospitalizations in Ontario, Canada between January 2010 and December 2022. We measured 90‐day mortality after selected types of higher risk admissions (such as cancer surgery and cardiovascular emergency) and the rate of patient harm events (such as delirium, pressure injuries and hospital‐acquired infections) occurring during the hospital stay. Results A total 13,876,377 hospitalization episodes were captured. Compared with the pre‐pandemic period, and independent of SARS‐CoV‐2 infection, the pandemic period was associated with higher rates of mortality after bladder cancer resection (adjusted risk ratio [aRR] 1.20 (1.07–1.34)) and open repair for abdominal aortic aneurysm (aRR 1.45 (1.06–1.99)). The pandemic was also associated with higher rates of delirium (adjusted odds ratio [aOR] 1.04 (1.02–1.06)), venous thromboembolism (aOR 1.10 (1.06–1.13)), pressure injuries (aOR 1.28 (1.24–1.33)), aspiration pneumonitis (aOR 1.15 (1.12–1.18)), urinary tract infections (aOR 1.02 (1.01–1.04)), Clostridiodes difficile infection (aOR 1.05 (1.02–1.09)), pneumothorax (aOR 1.08 (1.03–1.13)), and use of restraints (aOR 1.12 (1.10–1.14)), but was associated with lower rates of viral gastroenteritis (aOR 0.22 (0.18–0.28)). During the pandemic, SARS‐CoV‐2‐positive admissions were associated with a higher likelihood of various harm events. Conclusion The COVID‐19 pandemic was associated with higher rates of patient harm for a wide range of non‐SARS‐CoV‐2 inpatient populations. Impact Understanding which quality measures are improving or deteriorating can help health systems prioritize quality improvement initiatives. Nurses and junior doctors were observed on two COVID-19 wards and one non-COVID-19 ward using a semi-structured observation schedule. Survey data were collected from nurses and junior doctors that had worked on the COVID-19 ward during the pandemic. Patient data were collected from Web Patient Administration System during the period of March-August 2021 and compared to March-August 2022. Results: The results of this study indicated that the workloads of nurses and junior doctors changed on the COVID-19 wards. Nurses on the COVID-19 ward spent less time on administration, communication and documentation compared to nurses working on the non-COVID-19 ward. For junior doctors, less time was being spent on direct care activities and administrative work compared to the non-COVID ward. On the COVID-19 wards, patients were older and had a shorter length of stay compared to the year before. Five themes were identified from the surveys, staffing shortages resulting in higher workloads and the need for overtime, unforeseen time to undertake tasks, lack of support and physical health and patient care compromised. Conclusion: The study highlights the need for systemic changes to staffing shortages and elevated workloads to improve the compromised mental and physical health caused during the pandemic and to retain the workforce for future sustainability. Implications: As we collectively reflect on the lessons learned from this unprecedented period, it is imperative to address the challenges experienced proactively, fostering a healthcare environment that prioritises the well-being of its front line heroes and, by extension, the quality of patient care. Abstract:(1) Background: While the association between nurse mental health and quality and safety of patient care delivery was well documented pre-pandemic, fewer research studies have examined this relationship in the context of COVID-19. This study examines the impact of various mental health symptoms experienced by nurses on quality and safety before and during the COVID-19 pandemic; (2) Methods: A secondary analysis of cross-sectional survey data from 4729 and 3585 nurses in one Canadian province between December 2019 and June-July 2020 was conducted. Data were analyzed using between group difference tests and logistic regression; (3) Results: Compared to pre-COVID-19, during COVID-19 nurses reported a higher safety grade, a greater likelihood of recommending their units for care and lower quality of nursing care. Most mental health symptoms were higher during COVID-19 and higher levels of mental health symptoms were correlated with lower ratings of quality and safety both pre- and during COVID-19; (4) Conclusion: Mental health symptoms have implications for nurses’ quality and safety of patient care delivery, with the association between mental health symptoms and quality and safety following a dose–response relationship before and during COVID-19. These findings suggest that it is worthwhile for nurse mental health symptoms to be included as hospital level performance metrics.

Abstract:Background The COVID-19 pandemic had a considerable impact on US healthcare systems, straining hospital resources, staff, and operations. However, a comprehensive assessment of the impact on healthcare associated infections (HAIs) across different hospitals with varying level of infectious disease (ID) physician expertise, resources, and infrastructure is lacking. Methods This retrospective longitudinal multi-center cohort study included central-line-associated bloodstream infections (CLABSIs), catheter-associated urinary tract infections (CAUTIs), C. difficile infections (CDIs), and ventilator-associated events (VAEs) from 53 hospitals (academic and community) in Southeastern United States from January 1, 2018 to March 31, 2021. Segmented negative binomial regression generalized estimating equations models estimated changes in monthly incidence rates in the baseline (01/2018 – 02/2020) compared to the pandemic period (03/2020 – 03/2021, further divided into three pandemic phases). Results CLABSIs and VAEs increased by 24% and 34% respectively during the pandemic period. VAEs increased in all phases of the pandemic, while CLABSIs increased in later phases of the pandemic. CDI trend increased by 4.2% per month in the pandemic period. On stratifying the analysis by hospital characteristics, the impact of the pandemic on healthcare-associated infections was more significant in smaller sized and community hospitals. CAUTIs did not change significantly during the pandemic across all hospital types. Conclusions CLABSIs, VAEs, and CDIs increased significantly during the pandemic, especially in smaller community hospitals, most of which lack ID physician expertise. Future efforts should focus on better understanding challenges faced by community hospitals, strengthening infection prevention infrastructure, and expanding the ID workforce, particularly to community hospitals.

Abstract:A molecular-dynamics study on equiatomic NaPb, KPb, RbPb, and CsPb liquid alloys has been carried out to supplement earlier neutron-diffraction measurements. The alkali-metal atoms are assumed to be monovalent, positively charged ions while the Pb atoms are arranged in regular tetrahedra, (Pb4)4−, so-called Zintl ions. A simplified version of the Tosi-Fumi potential has been used to represent the interatomic forces, except for the Pb-Pb interactions within a (Pb4)4− ion, which are approximated by a harmonic potential. The structure factors calculated from this molecular-ionic model prove to be in excellent agreement with the measured ones.
Polycyclic aromatic hydrocarbons (PAHs) in yerba mate (Ilex paraguariensis) from the Argentinean market

Abstract:Polycyclic aromatic hydrocarbons (PAHs) occurrence in 50 samples marketed in the main supermarkets from Argentina was surveyed. A high performance liquid chromatography (HPLC) method was applied with fluorescence detection (FLD) and UV—VIS diodes array detector (DAD) for the analysis of 16 PAHs in “yerba mate” (Ilex paraguariensis), with recoveries higher than 89% and limits of detection and quantification lower than that found by other methodologies in previous studies. Contamination expressed as the sum of 16 analysed PAHs ranged between 224.6 and 4449.5 μg kg−1 on dry mass. The contamination expressed as PAH4 (sum of benzo(a)pyrene, chrysene, benzo(a)anthracene and benzo(b)fluoranthene) varied between 8.3 and 512.4 μg kg−1. The correlation coefficient for PAH2 (sum of benzo(a)pyrene and chrysene) and PAH4 groups was 0.99, for PAH2 and PAH8 (sum of benzo(a)pyrene, chrysene, benzo(a)anthracene, benzo(b)fluoranthene, benzo(k)fluoranthene, benzo(g,h,i)perylene, dibenzo(a,h)anthracene and indeno(1,2,3cd) pyrene) 0.97 and for PAH4 and PAH8 0.98.
Determinants of burnout in Canadian health care workers during the COVID-19 pandemic

Abstract:Background: Health care workers (HCWs) are among the most vulnerable groups to experience burnout during the coronavirus (COVID-19) pandemic. Understanding the risk and protective factors of burnout is crucial in guiding the development of interventions; however, the understanding of burnout determinants in the Canadian HCW population remains limited. Objective: Identify risk and protective factors associated with burnout in Canadian HCWs during the COVID-19 pandemic and evaluate organizational factors as moderators in the relationship between COVID-19 contact and burnout. Methods: Data were drawn from an online longitudinal survey of Canadian HCWs collected between 26 June 2020 and 31 December 2020. Participants completed questions pertaining to their well-being, burnout, workplace support and concerns relating to the COVID-19 pandemic. Baseline data from 1029 HCWs were included in the analysis. Independent samples t-tests and multiple linear regression were used to evaluate factors associated with burnout scores. Results: HCWs in contact with COVID-19 patients showed significantly higher likelihood of probable burnout than HCWs not directly providing care to COVID-19 patients. Fewer years of work experience was associated with a higher likelihood of probable burnout, whereas stronger workplace support, organizational leadership, supervisory leadership, and a favourable ethical climate were associated with a decreased likelihood of probable burnout. Workplace support, organizational leadership, supervisory leadership, and ethical climate did not moderate the associations between contact with COVID-19 patients and burnout. Conclusions: Our findings suggest that HCWs who worked directly with COVID-19 patients, had fewer years of work experience, and perceived poor workplace support, organizational leadership, supervisory leadership and ethical climate were at higher risk of burnout. Ensuring reasonable work hours, adequate support from management, and fostering an ethical work environment are potential organizational-level strategies to maintain HCWs' well-being. Antecedentes: Los trabajadores de la salud (HCWs en su sigla en inglés) están entre los grupos más vulnerables a experimentar burnout durante la pandemia del coronavirus (COVID-19). Es crucial entender los factores de riesgo y protectores del burnout para guiar el desarrollo de intervenciones; sin embargo, el entendimiento de los determinantes del burnout en la población de HCW canadienses permanece limitado. Objetivo: Identificar los factores de riesgo y protectores asociados con el burnout en los HCWs canadienses durante la pandemia del COVID-19 y evaluar los factores organizacionales como moderadores en la relación entre el contacto del COVID-19 y el burnout. Métodos: Los datos fueron obtenidos de una encuesta longitudinal en línea de los HCWs canadienses recolectadas entre el 26 de junio de 2020 y el 31 de diciembre de 2020. Burnout Among Hospitalists During the Early COVID-19 Pandemic: a National Mixed Methods Survey Study

Abstract:BACKGROUND : Hospitalist physician stress was exacerbated by the pandemic, yet there have been no large scale studies of contributing factors. Objective: Assess remediable components of burnout in hospitalists. Participants, study design and measures: In this Coping with COVID study, we focused on assessment of stress factors among 1022 hospital-based clinicians surveyed between April to December 2020. We assessed variables previously associated with burnout (anxiety/depression due to COVID-19, work overload, fear of exposure or transmission, mission/purpose, childcare stress and feeling valued) on 4 point Likert scales, with results dichotomized with the top two categories meaning "present"; burnout was assessed with the Mini Z single item measure (top 3 choices = burnout). Quantitative analyses utilized multilevel logistic regression; qualitative analysis used inductive and deductive methods. These data informed a conceptual model. Key results: Of 58,408 HCWs (median response rate 32%), 1022 were hospital-based clinicians (906 (89%) physicians; 449 (44%) female; 469 (46%) White); 46% of these hospital-based clinicians reported burnout. Work overload was associated with almost 5 times the odds of burnout (OR 4.9, 95% CIs 3.67, 6.85, p < 0.001), and those with anxiety or depression had 4 times the odds of burnout (OR 4.2, CIs 3.21, 7.12, p < 0.001), while those feeling valued had half the burnout odds (OR 0.43, CIs 0.31, 0.61, p < 0.001). Regression models estimated 42% of burnout variance was explained by these variables. In open-ended comments, leadership support was helpful, with "great leadership" represented by transparency, regular updates, and opportunities to ask questions. Conclusions: In this national study of hospital medicine, 2 variables were significantly related to burnout (workload and mental health) while two variables (feeling valued and leadership) were likely mitigators. These variables merit further investigation as means of reducing burnout in hospital medicine.
Structural Factors Contributing to Compassion Fatigue, Burnout, and Secondary Traumatic Stress Among Hospital-Based Healthcare Professionals During the COVID-19 Pandemic

Abstract:High levels of burnout among healthcare providers (HCPs) have been a widely documented phenomenon, which have been exacerbated during the COVID-19 pandemic. In the United States, qualitative studies that are inclusive of HCPs in diverse professional roles have been limited. Therefore, we utilized a qualitative-quantitative design to examine professional quality of life in terms of compassion fatigue, burnout, and secondary traumatic stress among hospital-based HCPs, including social workers, hospitalists, residents, and palliative care team members during COVID-19. HCPs (n = 26) participated in virtual semi-structured focus groups or individual interviews and online surveys (n = 30) including the Professional Quality of Life (ProQOL) Scale. While ProQOL scores indicated low levels of compassion fatigue, burnout, and secondary traumatic stress, thematic analysis of our qualitative data included rich descriptions of compassion fatigue, burnout, and secondary traumatic stress. Safety concerns and value misalignment characterized structural stressors perceived to contribute to HCP compassion fatigue, burnout, and secondary traumatic stress. The discrepancy between our qualitative and quantitative findings may be indication that modifications to current screenings are warranted. These findings also suggest a need to identify and implement structural and policy changes that increase HCPs' physical and emotional safety and promote better alignment of institutional interests with HCP values.
Impact of Hospital Strain on Excess Deaths During the COVID-19 Pandemic — United States, July 2020–July 2021

Abstract:Surges in COVID-19 cases have stressed hospital systems, negatively affected health care and public health infrastructures, and degraded national critical functions (1,2). Resource limitations, such as available hospital space, staffing, and supplies led some facilities to adopt crisis standards of care, the most extreme operating condition for hospitals, in which the focus of medical decision-making shifted from achieving the best outcomes for individual patients to addressing the immediate care needs of larger groups of patients (3). When hospitals deviated from conventional standards of care, many preventive and elective procedures were suspended, leading to the progression of serious conditions among some persons who would have benefitted from earlier diagnosis and intervention (4). During March-May 2020, U.S. emergency department visits declined by 23% for heart attacks, 20% for strokes, and 10% for diabetic emergencies (5). The Cybersecurity & Infrastructure Security Agency (CISA) COVID Task Force* examined the relationship between hospital strain and excess deaths during July 4, 2020-July 10, 2021, to assess the impact of COVID-19 surges on hospital system operations and potential effects on other critical infrastructure sectors and national critical functions. The study period included the months during which the highly transmissible SARS-CoV-2 B.1.617.2 (Delta) variant became predominant in the United States.<sup>†</sup> The negative binomial regression model used to calculate estimated deaths predicted that, if intensive care unit (ICU) bed use nationwide reached 75% capacity an estimated 12,000 additional excess deaths would occur nationally over the next 2 weeks. As hospitals exceed 100% ICU bed capacity, 80,000 excess deaths would be expected in the following 2 weeks. This analysis indicates the importance of controlling case growth and subsequent hospitalizations before severe strain. State, local, tribal, and territorial leaders could evaluate ways to reduce strain on public health and health care infrastructures, including implementing interventions to reduce overall disease prevalence such as vaccination and other prevention strategies, as well as ways to expand or enhance capacity during times of high disease prevalence.

Abstract:Journal of Health Services Research &Policy, Ahead of Print. ObjectivesThis study sought to describe feelings and perceptions of burnout and moral distress experienced by health care providers in the Canadian province of Saskatchewan during the COVID-19 pandemic.MethodsThis study was part of a larger mixed methods project, and we here report on the qualitative results relating to burnout and moral distress experienced by medical doctors, registered nurses and respiratory therapists. We used an exploratory, qualitative descriptive design involving one-one-one interviews with 24 health care providers. Interview data were analysed using a reflexive thematic analysis approach.ResultsWe identified three overarching themes each for health care provider burnout and moral distress. Interviews revealed that providers experienced burnout through (i) increased expectations and (ii) unfavourable work environments, which led most of them to recognise (iii) a need to step back. Regarding moral distress, key themes were: (i) a sense of compromised care, (ii) feelings of bumping heads with authorities and patient families, and (iii) seeing patients make difficult decisions.ConclusionOur study found that medical doctors, registered nurses and respiratory therapists working during the COVID-19 pandemic experienced and continue to experience significant burnout and moral distress. This was often driven by both institution- and system-level factors. There is a need for sustained investment to build and support a motivated health care workforce to prepare for future pandemics and health emergencies.

In 1996, a large outbreak (including 270 patients) of acute gastroenteritis occurred in Fukui. No known enteropathogens were detected in fecal samples of the patients, but P. alcalifaciens was detected from 7 of the 18 samples. The isolates were a clonal by pulsed-field gel electrophoresis. The specific antibody against the isolated strain was elevated in the patients' sera. The isolates showed invasiveness into Caco-2 cells and fluid-accumulation in rabbit ileal loops. These results indicate that P. alcalifaciens can be one of the causative agents of gastroenteritis or food-poisoning.
Assessing the impact of the COVID-19 pandemic on the mental health-related hospitalization rate of youth in Canada: an interrupted time series analysis

Abstract:Introduction: This study evaluated the effect of the COVID-19 pandemic on temporal trends in mental health and addiction-related inpatient hospitalization rates among youth (aged 10-17 years) in Canadian provinces and territories (excluding Quebec) from 1 April 2018 to 5 March 2022. Methods: We conducted an interrupted time series analysis across three periods: T0 (pre-pandemic: 1 April 2018 to 15 March 2020); T1 (early pandemic: 15 March 2020 to 5 July 2020); and T2 (later pandemic: 6 July 2020 to 5 March 2022). Results: Pre-pandemic mental health and addiction-related hospitalization rates had significant regional variability, with weekly rates from 6.27 to 85.59 events per 100 000 persons in Manitoba and the territories combined, respectively. During T1, the national (excluding Quebec) weekly hospitalization rate decreased from a pre-pandemic level of 12.82 (95% CI: 12.14 to 13.50) to 5.11 (95% CI: 3.80 to 6.41) events per 100 000 persons. There was no statistically significant change in the mental health and addiction- related hospitalization rate across provinces and territories in T2 compared to T0. However, there was a significant increase in the rate of self-harm-related hospitalizations among females Canada-wide and in most provinces during this period. Conclusion: Although several Canadian studies have reported increases in mental health and addiction-related outpatient and emergency department visits among youth during the COVID-19 pandemic, this did not correspond to an increase in the inpatient hospital burden, with the notable exception of self-harm among young females.
Lessons in Quality and Safety From Hospitals' COVID-19 Burdens

Abstract:Metersky et al 1 present compelling evidence that adverse events (AEs), including medication errors and hospital-acquired infections (HAIs), increased among patients both with and without COVID-19 during weeks in which hospitals experienced greater COVID-19 admission burdens during the COVID-19 pandemic. The risk of AEs increased 9% with each 10% increase in COVID-19 burden, defined as the daily mean number of COVID-19 inpatients per 100 hospital beds. The investigation by Metersky et al 1 improved on the methods of existing work through its use of hospital-specific COVID-19 admissions data (rather than regional or aggregate hospital data) from the US Department of Health and Human Services (HHS) and its reliance on systematically adjudicated AEs from the Agency for Healthcare Research and Quality. The authors concluded with the recommendation to build more resilient systems of care to maintain patient safety during times of surge capacity. Indeed, these conclusions are well supported by the authors' approach and analysis, and this represents a public health call to action in the US to avoid excess morbidity and mortality in the future. However, still absent from the peer-reviewed literature is a discussion of the importance of hospital quality performance reporting and benchmarking in times of crisis as a mechanism to ensure safety. Hospitals in the US were effectively relieved of their obligations to provide high-quality pandemic-era care when pay-for-performance programs, 2 rankings, 3 and ratings 4 excluded months of pandemic-era data from their methods. Though ostensibly made with benign intent, these decisions to exclude pandemic-era quality-reporting data were not patient centered and may not have been methodologically necessary. 5 ,6 The findings of Metersky et al 1 demonstrate that quality and safety data are needed more than ever during times of crisis. This would allow patients and payers to understand which hospitals have resilient systems in order to make informed decisions about where they choose to receive and fund health care. For example, while Metersky et al 1 found only small differences in fall rates for patients without COVID-19 across hospitals with varying COVID-19 burden, a dose-response association was observed between increasing COVID-19 burdens and HAIs. We may therefore infer differences in systems and processes for fall prevention that allowed for greater resilience than systems and processes for prevention of HAIs. Logically, we could then consider whether exclusion of pandemic-era falls data from hospital-quality reporting was necessary. Exploring these nuances will be critical to identify where resilience in safety is lacking and to build resilience for the future. This may require the creation of pandemic-specific resilience action plans and self-reporting requirements by accrediting bodies, analogous to recent Joint Commission requirements for the reduction of health care disparities. Individual hospitals almost certainly varied in the specific domains of quality that were most and least resilient during the pandemic. Each hospital's least resilient system may represent low-hanging fruit or a starting point for building resilience. To date, however, finding these opportunities to build resilience has been a difficult task, in part due to the exclusion of pandemic-era benchmarking data. Ask chief quality officers to select any 3 among the hundreds of publicly reported quality measures (30-day readmissions, 30-day mortality, falls, HAIs, pressure ulcers, and so on) at which they believe their institution performed the worst during the pandemic, and you will likely get dozens of different answers. For many, the most truthful answer may be "I do not know," which hinders the ability to prioritize improvement. For example, if you knew that the 30-day hospital-wide readmissions rate had dropped from the 90th percentile of national performance to the 20th percentile of performance during the pandemic, there would be an important opportunity to consider which aspects of resilience specific to readmissions at your institution required improvement. With hospital-specific COVID-19 admissions data from HHS coupled with patient-level Medicare or other electronic health record or claims data, it is still possible to recreate the unreported pandemic-era quality outcomes benchmarks to identify resilient hospitals, systems, and processes during surge capacity. In fact, the Centers for Medicare & Medicaid Services' FY 2025 Inpatient Prospective Payment System Proposed Rule will extend the COVID-19 admissions data to include data for all respiratory illnesses. This will allow for the inclusion of future pandemic-era data in quality reporting using risk adjustment for hospitals' respiratory illness admission burdens rather than excluding months or years -Abstract Truncated-

Abstract:Aim: To describe and compare the prevalence of assaults and aggressive patient behavior among frontline staff in behavioral health (BH), medical-surgical (MS), and emergency department (ED) settings and examine the impact on staff health, work stress, work engagement, and intent to leave their position. Background: Patient verbal and physical assaults have significant staff consequences, including decreased work productivity, increased burnout, job dissatisfaction, absenteeism, turnover, and intentions to leave. Methods: Using a descriptive cross-sectional design, data were collected from a sample of 432 frontline staff working in ED, BH, and MS settings across 3 healthcare systems. Results: The majority of frontline staff (74%) reported experiencing verbal aggression often/frequently, significantly impacting their mental health, work engagement, stress levels, and intent to leave. All 3 specialty groups reported a significant increase in verbal/psychological assaults and physical assaults since the pandemic's onset. Conclusion: The COVID-19 pandemic had a significant impact on assaultive/aggressive behaviors. Nurse leaders must strategize on methods to decrease the normalization of violence against healthcare workers and support research aimed at evidence-based interventions to reduce such incidences of violence and ensure the well-being of healthcare workers.
Missed opportunities in hospital quality measurement during the COVID-19 pandemic: a retrospective investigation of US hospitals CMS Star Ratings and 30-day mortality during the early pandemic

Abstract:Objectives In the USA and UK, pandemic-era outcome data have been excluded from hospital rankings and pay-for-performance programmes. We assessed the relationship between US hospitals' pre-pandemic Centers for Medicare and Medicaid Services (CMS) Overall Hospital Star ratings and early pandemic 30-day mortality among both patients with COVID and non-COVID to understand whether pre-existing structures, processes and outcomes related to quality enabled greater pandemic resiliency. Design and data source A retrospective, claim-based data study using the 100% Inpatient Standard Analytic File and Medicare Beneficiary Summary File including all US Medicare Fee-for-Service inpatient encounters from 1 April 2020 to 30 November 2020 linked with the CMS Hospital Star Ratings using six-digit CMS provider IDs. Outcome measure The outcome was risk-adjusted 30-day mortality. We used multivariate logistic regression adjusting for age, sex, Elixhauser mortality index, US Census Region, month, hospital-specific January 2020 CMS Star rating (1–5 stars), COVID diagnosis (U07.1) and COVID diagnosisxCMS Star Rating interaction. Results We included 4 473 390 Medicare encounters from 2533 hospitals, with 92 896 (28.2%) mortalities among COVID-19 encounters and 387 029 (9.3%) mortalities among non-COVID encounters. There was significantly greater odds of mortality as CMS Star Ratings decreased, with 18% (95% CI 15% to 22%; p<0.0001), 33% (95% CI 30% to 37%; p<0.0001), 38% (95% CI 34% to 42%; p<0.0001) and 60% (95% CI 55% to 66%; p<0.0001), greater odds of COVID mortality comparing 4-star, 3-star, 2-star and 1-star hospitals (respectively) to 5-star hospitals. Among non-COVID encounters, there were 17% (95% CI 16% to 19%; p<0.0001), 24% (95% CI 23% to 26%; p<0.0001), 32% (95% CI 30% to 33%; p<0.0001) and 40% (95% CI 38% to 42%; p<0.0001) greater odds of mortality at 4-star, 3-star, 2-star and 1-star hospitals (respectively) as compared with 5-star hospitals. Conclusion Our results support a need to further understand how quality outcomes were maintained during the pandemic. Valuable insights can be gained by including the reporting of risk-adjusted pandemic era hospital quality outcomes for high and low performing hospitals.

Abstract:Importance Unprecedented increases in hospital occupancy rates during COVID-19 surges in 2020 caused concern over hospital care quality for patients without COVID-19. Objective To examine changes in hospital nonsurgical care quality for patients without COVID-19 during periods of high and low COVID-19 admissions. Design, Setting, and Participants This cross-sectional study used data from the 2019 and 2020 Agency for Healthcare Research and Quality's Healthcare Cost and Utilization Project State Inpatient Databases. Data were obtained for all nonfederal, acute care hospitals in 36 states with admissions in 2019 and 2020, and patients without a diagnosis of COVID-19 or pneumonia who were at risk for selected quality indicators were included. The data analysis was performed between January 1, 2023, and March 15, 2024. Exposure Each hospital and week in 2020 was categorized based on the number of COVID-19 admissions per 100 beds: less than 1.0, 1.0 to 4.9, 5.0 to 9.9, 10.0 to 14.9, and 15.0 or greater. Main Outcomes and Measures The main outcomes were rates of adverse outcomes for selected quality indicators, including pressure ulcers and in-hospital mortality for acute myocardial infarction, heart failure, acute stroke, gastrointestinal hemorrhage, hip fracture, and percutaneous coronary intervention. Changes in 2020 compared with 2019 were calculated for each level of the weekly COVID-19 admission rate, adjusting for case-mix and hospital-month fixed effects. Changes during weeks with high COVID-19 admissions (≥15 per 100 beds) were compared with changes during weeks with low COVID-19 admissions (<1 per 100 beds). Results The analysis included 19 111 629 discharges (50.3% female; mean [SD] age, 63.0 [18.0] years) from 3283 hospitals in 36 states. In weeks 18 to 48 of 2020, 35 851 hospital-weeks (36.7%) had low COVID-19 admission rates, and 8094 (8.3%) had high rates. Quality indicators for patients without COVID-19 significantly worsened in 2020 during weeks with high vs low COVID-19 admissions. Pressure ulcer rates increased by 0.09 per 1000 admissions (95% CI, 0.01-0.17 per 1000 admissions; relative change, 24.3%), heart failure mortality increased by 0.40 per 100 admissions (95% CI, 0.18-0.63 per 100 admissions; relative change, 21.1%), hip fracture mortality increased by 0.40 per 100 admissions (95% CI, 0.04-0.77 per 100 admissions; relative change, 29.4%), and a weighted mean of mortality for the selected indicators increased by 0.30 per 100 admissions (95% CI, 0.14-0.45 per 100 admissions; relative change, 10.6%). Conclusions and Relevance In this cross-sectional study, COVID-19 surges were associated with declines in hospital quality, highlighting the importance of identifying and implementing strategies to maintain care quality during periods of high hospital use.
