Abstract:Background Skin cancer is one of the most common forms worldwide, with a significant increase in incidence over the last few decades. Early and accurate detection of this type of cancer can result in better prognoses and less invasive treatments for patients. With advances in Artificial Intelligence (AI), tools have emerged that can facilitate diagnosis and classify dermatological images, complementing traditional clinical assessments and being applicable where there is a shortage of specialists. Its adoption requires analysis of efficacy, safety, and ethical considerations, as well as considering the genetic and ethnic diversity of patients. Objective The systematic review aims to examine research on the detection, classification, and assessment of skin cancer images in clinical settings. Methods We conducted a systematic literature search on PubMed, Scopus, Embase, and Web of Science, encompassing studies published until April 4th, 2023. Study selection, data extraction, and critical appraisal were carried out by two independent reviewers. Results were subsequently presented through a narrative synthesis. Results Through the search, 760 studies were identified in four databases, from which only 18 studies were selected, focusing on developing, implementing, and validating systems to detect, diagnose, and classify skin cancer in clinical settings. This review covers descriptive analysis, data scenarios, data processing and techniques, study results and perspectives, and physician diversity, accessibility, and participation. Conclusion The application of artificial intelligence in dermatology has the potential to revolutionize early detection of skin cancer. However, it is imperative to validate and collaborate with healthcare professionals to ensure its clinical effectiveness and safety.

Abstract:In recent years, there has been growing interest in the use of computer-assisted technology for early detection of skin cancer through the analysis of dermatoscopic images. However, the accuracy illustrated behind the state-of-the-art approaches depends on several factors, such as the quality of the images and the interpretation of the results by medical experts. This systematic review aims to critically assess the efficacy and challenges of this research field in order to explain the usability and limitations and highlight potential future lines of work for the scientific and clinical community. In this study, the analysis was carried out over 45 contemporary studies extracted from databases such as Web of Science and Scopus. Several computer vision techniques related to image and video processing for early skin cancer diagnosis were identified. In this context, the focus behind the process included the algorithms employed, result accuracy, and validation metrics. Thus, the results yielded significant advancements in cancer detection using deep learning and machine learning algorithms. Lastly, this review establishes a foundation for future research, highlighting potential contributions and opportunities to improve the effectiveness of skin cancer detection through machine learning.

Artificial Intelligence Applied to Non-Invasive Imaging Modalities in Identification of Nonmelanoma Skin Cancer: A Systematic Review

Abstract:Background: The objective of this study is to systematically analyze the current state of the literature regarding novel artificial intelligence (AI) machine learning models utilized in non-invasive imaging for the early detection of nonmelanoma skin cancers. Furthermore, we aimed to assess their potential clinical relevance by evaluating the accuracy, sensitivity, and specificity of each algorithm and assessing for the risk of bias. Methods: Two reviewers screened the MEDLINE, Cochrane, PubMed, and Embase databases for peer-reviewed studies that focused on AI-based skin cancer classification involving nonmelanoma skin cancers and were published between 2018 and 2023. The search terms included skin neoplasms, nonmelanoma, basal-cell carcinoma, squamous-cell carcinoma, diagnostic techniques and procedures, artificial intelligence, algorithms, computer systems, dermoscopy, reflectance confocal microscopy, and optical coherence tomography. Based on the search results, only studies that directly answered the review objectives were included and the efficacy measures for each were recorded. A QUADAS-2 risk assessment for bias in included studies was then conducted. Results: A total of 44 studies were included in our review; 40 utilizing dermoscopy, 3 using reflectance confocal microscopy (RCM), and 1 for hyperspectral epidermal imaging (HEI). The average accuracy of AI algorithms applied to all imaging modalities combined was 86.80%, with the same average for dermoscopy. Only one of the three studies applying AI to RCM measured accuracy, with a result of 87%. Accuracy was not measured in regard to AI based HEI interpretation. Conclusion: AI algorithms exhibited an overall favorable performance in the diagnosis of nonmelanoma skin cancer via noninvasive imaging techniques. Ultimately, further research is needed to isolate pooled diagnostic accuracy for nonmelanoma skin cancers as many testing datasets also include melanoma and other pigmented lesions.

Abstract:Recently, there has been great interest in developing Artificial Intelligence (AI) enabled computer-aided diagnostics solutions for the diagnosis of skin cancer. With the increasing incidence of skin cancers, low awareness among a growing population, and a lack of adequate clinical expertise and services, there is an immediate need for AI systems to assist clinicians in this domain. A large number of skin lesion datasets are available publicly, and researchers have developed AI-based image classification solutions, particularly deep learning algorithms, to distinguish malignant skin lesions from benign lesions in different image modalities such as dermoscopic, clinical, and histopathology images. Despite the various claims of AI systems achieving higher accuracy than dermatologists in the classification of different skin lesions, these AI systems are still in the very early stages of clinical application in terms of being ready to aid clinicians in the diagnosis of skin cancers. In this review, we discuss advancements in the digital image-based AI solutions for the diagnosis of skin cancer, along with some challenges and future opportunities to improve these AI systems to support dermatologists and enhance their ability to diagnose skin cancer.
Artificial Intelligence for the Classification of Pigmented Skin Lesions in Populations with Skin of Color: A Systematic Review

Abstract:Background: While skin cancers are less prevalent in people with skin of color, they are more often diagnosed at later stages and have a poorer prognosis. The use of artificial intelligence (AI) models can potentially improve early detection of skin cancers; however, the lack of skin color diversity in training datasets may only widen the pre-existing racial discrepancies in dermatology. Objective: The aim of this study was to systematically review the technique, quality, accuracy, and implications of studies using AI models trained or tested in populations with skin of color for classification of pigmented skin lesions. Methods: PubMed was used to identify any studies describing AI models for classification of pigmented skin lesions. Only studies that used training datasets with at least 10% of images from people with skin of color were eligible. Outcomes on study population, design of AI model, accuracy, and quality of the studies were reviewed. Results: Twenty-two eligible articles were identified. The majority of studies were trained on datasets obtained from Chinese (7/22), Korean (5/22), and Japanese populations (3/22). Seven studies used diverse datasets containing Fitzpatrick skin type I-III in combination with at least 10% from black Americans, Native Americans, Pacific Islanders, or Fitzpatrick IV-VI. AI models producing binary outcomes (e.g., benign vs. malignant) reported an accuracy ranging from 70% to 99.7%. Accuracy of AI models reporting multiclass outcomes (e.g., specific lesion diagnosis) was lower, ranging from 43% to 93%. Reader studies, where dermatologists' classification is compared with AI model outcomes, reported similar accuracy in one study, higher AI accuracy in three studies, and higher clinician accuracy in two studies. A quality review revealed that dataset description and variety, benchmarking, public evaluation, and healthcare application were frequently not addressed. Conclusions: While this review provides promising evidence of accurate AI models in populations with skin of color, the majority of the studies reviewed were obtained from East Asian populations and therefore provide insufficient evidence to comment on the overall accuracy of AI models for darker skin types. Large discrepancies remain in the number of AI models developed in populations with skin of color (particularly Fitzpatrick type IV-VI) compared with those of largely European ancestry. A lack of publicly available datasets from diverse populations is likely a contributing factor, as is the inadequate reporting of patient-level metadata relating to skin color in training datasets.

Abstract:Skin cancer is the most common type of cancer worldwide and poses a significant public health challenge. Its visible nature often leads individuals to seek medical attention, highlighting the importance of early detection for better patient outcomes. In recent years, Artificial Intelligence (AI) has shown promise in improving the detection and diagnosis of skin cancer, offering the potential to enhance clinical outcomes. A systematic review was conducted, involving a comprehensive literature search to identify studies focused on AI techniques in detecting, diagnosing, or treating skin cancer. Strict inclusion and exclusion criteria were applied to assess the eligibility of scientific articles, resulting in the selection of nine relevant studies. These studies were analyzed to address predefined research questions about the effectiveness of AI in diagnosing skin cancer. The review found that AI-assisted clinicians achieved higher sensitivity and specificity in diagnosing skin cancer than those without assistance. Various AI algorithms demonstrated high sensitivity in detecting skin cancers, highlighting their potential to support primary care clinicians in evaluating suspicious lesions. The analysis also highlighted the effectiveness of smartphone applications designed for skin cancer risk assessment, which could facilitate self-examinations and enhance early detection rates. Despite these promising findings, the field of AI in skin cancer diagnosis is still in its early stages. Challenges remain, including developing robust algorithms, addressing data quality issues, and improving the interpretability of AI-generated results. Collaboration between AI developers and healthcare professionals is crucial to ensure these tools' clinical effectiveness and safety. The review emphasizes the need for continued validation of AI technologies and their integration into clinical practice to improve patient outcomes and alleviate the burden on healthcare systems.
Analysis of Artificial Intelligence-Based Approaches Applied to Non-Invasive Imaging for Early Detection of Melanoma: A Systematic Review

Abstract:Background: Melanoma, the deadliest form of skin cancer, poses a significant public health challenge worldwide. Early detection is crucial for improved patient outcomes. Non-invasive skin imaging techniques allow for improved diagnostic accuracy; however, their use is often limited due to the need for skilled practitioners trained to interpret images in a standardized fashion. Recent innovations in artificial intelligence (AI)-based techniques for skin lesion image interpretation show potential for the use of AI in the early detection of melanoma. Objective: The aim of this study was to evaluate the current state of AI-based techniques used in combination with non-invasive diagnostic imaging modalities including reflectance confocal microscopy (RCM), optical coherence tomography (OCT), and dermoscopy. We also aimed to determine whether the application of AI-based techniques can lead to improved diagnostic accuracy of melanoma. Methods: A systematic search was conducted via the Medline/PubMed, Cochrane, and Embase databases for eligible publications between 2018 and 2022. Screening methods adhered to the 2020 version of the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guidelines. Included studies utilized AI-based algorithms for melanoma detection and directly addressed the review objectives. Results: We retrieved 40 papers amongst the three databases. All studies directly comparing the performance of AI-based techniques with dermatologists reported the superior or equivalent performance of AI-based techniques in improving the detection of melanoma. In studies directly comparing algorithm performance on dermoscopy images to dermatologists, AI-based algorithms achieved a higher ROC (>80%) in the detection of melanoma. In these comparative studies using dermoscopic images, the mean algorithm sensitivity was 83.01% and the mean algorithm specificity was 85.58%. Studies evaluating machine learning in conjunction with OCT boasted accuracy of 95%, while studies evaluating RCM reported a mean accuracy rate of 82.72%. Conclusions: Our results demonstrate the robust potential of AI-based techniques to improve diagnostic accuracy and patient outcomes through the early identification of melanoma. Further studies are needed to assess the generalizability of these AI-based techniques across different populations and skin types, improve standardization in image processing, and further compare the performance of AI-based techniques with board-certified dermatologists to evaluate clinical applicability.

Abstract:Skin cancer is a major global public health concern. Early diagnosis of skin cancer is essential for better patient outcomes and lower mortality rates. Artificial intelligence (AI) has the potential to increase the precision of skin cancer prediction and help doctors with diagnosis. This short communication reviews the recent advances and future directions in AI-powered skin cancer prediction. A comprehensive assessment of the literature is conducted to evaluate papers published between 2016 and 2022. The proposed research methodology explored Google Scholar, Scopus, and Web of Science databases to identify relevant studies according to proposed research questions. A total of 780 studies were found, of which 62 were from Scopus and 20 were from Web of Science These studies were selected for study as per the research question constraints. The study examined how well AI could distinguish between benign and malignant lesions and diagnose skin cancer, including melanoma and non- melanoma skin cancers. The analysis showed that deep learning algorithms showed good rates of sensitivity and specificity in predicting skin cancer, suggesting that they could be a useful tool for dermatologists. However, the small sample sizes and lack of real-world testing limited the generalizability of these findings. Additional research is needed to assess how well AI works on people with various skin tones under different environmental conditions. Future research should also focus on developing AI algorithms that can be used to diagnose other types of skin cancer, such as basal cell carcinoma and squamous cell carcinoma.

Abstract:Abstract Background Medical research, driven by advancing technologies like Artificial Intelligence (AI), is transforming healthcare. Dermatology, known for its visual nature, benefits from AI, especially in dermatopathology with digitized slides. This review explores into AI's role, challenges, opportunities, and future potential in enhancing dermatopathological diagnosis and care. Materials and Methodology Adhering to PRISMA and Cochrane Handbook standards, this systematic review explored AI's function in dermatopathology. It employed an interdisciplinary method, encompassing diverse study types and comprehensive database searches. Inclusion criteria encompassed peer-reviewed articles from 2000 to 2023, with a focus on practical AI use in dermatopathology. Results Numerous studies have investigated AI's potential in dermatopathology. We reviewed 112 papers. Notable applications include AI classifying histopathological images of nevi and melanomas, although challenges exist regarding subtype differentiation and generalizability. AI achieved high accuracy in melanoma recognition from formalin-fixed paraffin-embedded samples but faced limitations due to small datasets. Deep learning algorithms showed diagnostic accuracy for specific skin conditions, but challenges persisted, such as small sample sizes and the need for prospective validation. Conclusion This systematic review underscores AI's potential in enhancing dermatopathology for better diagnosis and patient care. Addressing challenges like limited datasets and potential biases is essential. Future directions involve expanding datasets, conducting validation studies, promoting interdisciplinary collaboration, and creating patient-centred AI tools to enhance dermatopathology's accuracy, accessibility, and patient-focused care.

Abstract:Abstract Background Melanoma is one of the most aggressive types of cancer that has become a world-class problem. According to the World Health Organization estimates, 132,000 cases of the disease and 66,000 deaths from malignant melanoma and other forms of skin cancer are reported annually worldwide ( https://apps.who.int/gho/data/?theme=main ) and those numbers continue to grow. In our opinion, due to the increasing incidence of the disease, it is necessary to find new, easy to use and sensitive methods for the early diagnosis of melanoma in a large number of people around the world. Over the last decade, neural networks show highly sensitive, specific, and accurate results. Objective This study presents a review of PubMed papers including requests «melanoma neural network» and «melanoma neural network dermatoscopy». We review recent researches and discuss their opportunities acceptable in clinical practice. Methods We searched the PubMed database for systematic reviews and original research papers on the requests «melanoma neural network» and «melanoma neural network dermatoscopy» published in English. Only papers that reported results, progress and outcomes are included in this review. Results We found 11 papers that match our requests that observed convolutional and deep-learning neural networks combined with fuzzy clustering or World Cup Optimization algorithms in analyzing dermatoscopic images. All of them require an ABCD (asymmetry, border, color, and differential structures) algorithm and its derivates (in combination with ABCD algorithm or separately). Also, they require a large dataset of dermatoscopic images and optimized estimation parameters to provide high specificity, accuracy and sensitivity. Conclusions According to the analyzed papers, neural networks show higher specificity, accuracy and sensitivity than dermatologists. Neural networks are able to evaluate features that might be unavailable to the naked human eye. Despite that, we need more datasets to confirm those statements. Nowadays machine learning becomes a helpful tool in early diagnosing skin diseases, especially melanoma.

[The Rise of Artificial Intelligence - High Prediction Accuracy in Early Detection of Pigmented Melanoma]

Abstract:The incidence of malignant melanoma is increasing worldwide. If detected early, melanoma is highly treatable, so early detection is vital.Skin cancer early detection has improved significantly in recent decades, for example by the introduction of screening in 2008 and dermoscopy. Nevertheless, in particular visual detection of early melanomas remains challenging because they show many morphological overlaps with nevi. Hence, there continues to be a high medical need to further develop methods for early skin cancer detection in order to be able to reliably diagnosemelanomas at a very early stage.Routine diagnostics for melanoma detection include visual whole body inspection, often supplemented by dermoscopy, which can significantly increase the diagnostic accuracy of experienced dermatologists. A procedure that is additionally offered in some practices and clinics is wholebody photography combined with digital dermoscopy for the early detection of malignant melanoma, especially for monitoring high-risk patients.In recent decades, numerous noninvasive adjunctive diagnostic techniques were developed for the examination of suspicious pigmented moles, that may have the potential to allow improved and, in some cases, automated evaluation of these lesions. First, confocal laser microscopy should be mentioned here, as well as electrical impedance spectroscopy, multiphoton laser tomography, multispectral analysis, Raman spectroscopy or optical coherence tomography. These diagnostic techniques usually focus on high sensitivity to avoid malignant melanoma being overlooked. However, this usually implies lower specificity, which may lead to unnecessary excision of benign lesions in screening. Also, some of the procedures are time-consuming and costly, which also limits their applicability in skin cancer screening. In the near future, the use of artificial intelligence might change skin cancer diagnostics in many ways. The most promising approach may be the analysis of routine macroscopic and dermoscopic images by artificial intelligence.For the classification of pigmented skin lesions based on macroscopic and dermoscopic images, artificial intelligence, especially in form of neural networks, has achieved comparable diagnostic accuracies to dermatologists under experimental conditions in numerous studies. In particular, it achieved high accuracies in the binary melanoma/nevus classification task, but it also performed comparably well to dermatologists in multiclass differentiation of various skin diseases. However, proof of the basic applicability and utility of such systems in clinical practice is still pending. Prerequisites that remain to be established to enable translation of such diagnostic systems into dermatological routine are means that allow users to comprehend the system's decisions as well as a uniformly high performance of the algorithms on image data from other hospitals and practices.At present, hints are accumulating that computer-aided diagnosis systems could provide their greatest benefit as assistance systems, since studies indicate that a combination of human and machine achieves the best results. Diagnostic systems based on artificial intelligence are capable of detecting morphological characteristics quickly, quantitatively, objectively and reproducibly, and could thus provide a more objective analytical basis - in addition to medical experience.
Performance of Commercial Dermatoscopic Systems That Incorporate Artificial Intelligence for the Identification of Melanoma in General Practice: A Systematic Review

Abstract:Background: Cutaneous melanoma remains an increasing global public health burden, particularly in fair-skinned populations. Advancing technologies, particularly artificial intelligence (AI), may provide an additional tool for clinicians to help detect malignancies with a more accurate success rate. This systematic review aimed to report the performance metrics of commercially available convolutional neural networks (CNNs) tasked with detecting MM. Methods: A systematic literature search was performed using CINAHL, Medline, Scopus, ScienceDirect and Web of Science databases. Results: A total of 16 articles reporting MM were included in this review. The combined number of melanomas detected was 1160, and non-melanoma lesions were 33,010. The performance of market-approved technology and clinician performance for classifying melanoma was highly heterogeneous, with sensitivity ranging from 16.4 to 100.0%, specificity between 40.0 and 98.3% and accuracy between 44.0 and 92.0%. Less heterogeneity was observed when clinicians worked in unison with AI, with sensitivity ranging between 83.3 and 100.0%, specificity between 83.7 and 87.3%, and accuracy between 86.4 and 86.9%. Conclusion: Instead of focusing on the performance of AI versus clinicians for classifying melanoma, more consistent performance has been obtained when clinicians' work is supported by AI, facilitating management decisions and improving health outcomes.

Abstract:Objective: Artificial intelligence (AI), with its potential to diagnose skin cancer, has the potential to revolutionize future medical and dermatological practices. However, the current knowledge regarding the utilization of AI in skin cancer diagnosis remains somewhat limited, necessitating further research. This study employs visual bibliometric analysis to consolidate and present insights into the evolution and deployment of AI in the context of skin cancer. Through this analysis, we aim to shed light on the research developments, focal areas of interest, and emerging trends within AI and its application to skin cancer diagnosis. Methods: On July 14, 2023, articles and reviews about the application of AI in skin cancer, spanning the years from 1900 to 2023, were selected from the Web of Science Core Collection. Co-authorship, co-citation, and co-occurrence analyses of countries, institutions, authors, references, and keywords within this field were conducted using a combination of tools, including CiteSpace V (version 6.2. R3), VOSviewer (version 1.6.18), SCImago, Microsoft Excel 2019, and R 4.2.3. Results: A total of 512 papers matching the search terms and inclusion/exclusion criteria were published between 1991 and 2023. The United States leads in publications with 149, followed by India with 61. Germany holds eight positions among the top 10 institutions, while the United States has two. The most prevalent journals cited were Cancer , the European Journal of Cancer , and Sensors . The most frequently cited keywords include "skin cancer", "classification", "artificial intelligence", and "deep learning". Conclusions: Research into the application of AI in skin cancer is rapidly expanding, and an increasing number of scholars are dedicating their efforts to this field. With the advancement of AI technology, new opportunities have arisen to enhance the accuracy of skin imaging diagnosis, treatment based on big data, and prognosis prediction. However, at present, the majority of AI research in the field of skin cancer diagnosis is still in the feasibility study stage. It has not yet made significant progress toward practical implementation in clinical settings. To make substantial strides in this field, there is a need to enhance collaboration between countries and institutions. Despite the potential benefits of AI in skin cancer research, numerous challenges remain to be addressed, including developing robust algorithms, resolving data quality issues, and enhancing results interpretability. Consequently, sustained efforts are essential to surmount these obstacles and facilitate the practical application of AI in skin cancer research.

Abstract:Skin cancer poses a serious risk to one's health and can only be effectively treated with early detection. Early identification is critical since skin cancer has a higher fatality rate, and it expands gradually to different areas of the body. The rapid growth of automated diagnosis frameworks has led to the combination of diverse machine learning, deep learning, and computer vision algorithms for detecting clinical samples and atypical skin lesion specimens. Automated methods for recognizing skin cancer that use deep learning techniques are discussed in this article: convolutional neural networks, and, in general, artificial neural networks. The recognition of symmetries is a key point in dealing with the skin cancer image datasets; hence, in developing the appropriate architecture of neural networks, as it can improve the performance and release capacities of the network. The current study emphasizes the need for an automated method to identify skin lesions to reduce the amount of time and effort required for the diagnostic process, as well as the novel aspect of using algorithms based on deep learning for skin lesion detection. The analysis concludes with underlying research directions for the future, which will assist in better addressing the difficulties encountered in human skin cancer recognition. By highlighting the drawbacks and advantages of prior techniques, the authors hope to establish a standard for future analysis in the domain of human skin lesion diagnostics.

Abstract:Skin cancer is one of the most dangerous diseases in the world. Correctly classifying skin lesions at an early stage could aid clinical decision-making by providing an accurate disease diagnosis, potentially increasing the chances of cure before cancer spreads. However, achieving automatic skin cancer classification is difficult because the majority of skin disease images used for training are imbalanced and in short supply; meanwhile, the model’s cross-domain adaptability and robustness are also critical challenges. Recently, many deep learning-based methods have been widely used in skin cancer classification to solve the above issues and achieve satisfactory results. Nonetheless, reviews that include the abovementioned frontier problems in skin cancer classification are still scarce. Therefore, in this article, we provide a comprehensive overview of the latest deep learning-based algorithms for skin cancer classification. We begin with an overview of three types of dermatological images, followed by a list of publicly available datasets relating to skin cancers. After that, we review the successful applications of typical convolutional neural networks for skin cancer classification. As a highlight of this paper, we next summarize several frontier problems, including data imbalance, data limitation, domain adaptation, model robustness, and model efficiency, followed by corresponding solutions in the skin cancer classification task. Finally, by summarizing different deep learning-based methods to solve the frontier challenges in skin cancer classification, we can conclude that the general development direction of these approaches is structured, lightweight, and multimodal. Besides, for readers’ convenience, we have summarized our findings in figures and tables. Considering the growing popularity of deep learning, there are still many issues to overcome as well as chances to pursue in the future.

Artificial intelligence for nonmelanoma skin cancer

Abstract:Nonmelanoma skin cancers (NMSCs) are among the top five most common cancers globally. NMSC is an area with great potential for novel application of diagnostic tools including artificial intelligence (AI). In this scoping review, we aimed to describe the applications of AI in the diagnosis and treatment of NMSC. Twenty-nine publications described AI applications to dermatopathology including lesion classification and margin assessment. Twenty-five publications discussed AI use in clinical image analysis, showing that algorithms are not superior to dermatologists and may rely on unbalanced, nonrepresentative, and nontransparent training data sets. Sixteen publications described the use of AI in cutaneous surgery for NMSC including use in margin assessment during excisions and Mohs surgery, as well as predicting procedural complexity. Eleven publications discussed spectroscopy, confocal microscopy, thermography, and the AI algorithms that analyze and interpret their data. Ten publications pertained to AI applications for the discovery and use of NMSC biomarkers. Eight publications discussed the use of smartphones and AI, specifically how they enable clinicians and patients to have increased access to instant dermatologic assessments but with varying accuracies. Five publications discussed large language models and NMSC, including how they may facilitate or hinder patient education and medical decision-making. Three publications pertaining to the skin of color and AI for NMSC discussed concerns regarding limited diverse data sets for the training of convolutional neural networks. AI demonstrates tremendous potential to improve diagnosis, patient and clinician education, and management of NMSC. Despite excitement regarding AI, data sets are often not transparently reported, may include low-quality images, and may not include diverse skin types, limiting generalizability. AI may serve as a tool to increase access to dermatology services for patients in rural areas and save health care dollars. These benefits can only be achieved, however, with consideration of potential ethical costs.

Abstract:Skin cancer is among the most common and lethal cancer types, with the number of cases increasing dramatically worldwide. If not diagnosed in the nascent stages, it can lead to metastases, resulting in high mortality rates. Skin cancer can be cured if detected early. Consequently, timely and accurate diagnosis of such cancers is currently a key research objective. Various machine learning technologies have been employed in computer-aided diagnosis of skin cancer detection and malignancy classification. Machine learning is a subfield of artificial intelligence (AI) involving models and algorithms which can learn from data and generate predictions on previously unseen data. The traditional biopsy method is applied to diagnose skin cancer, which is a tedious and expensive procedure. Alternatively, machine learning algorithms for cancer diagnosis can aid in its early detection, lowering the workload of specialists while simultaneously enhancing skin lesion diagnostics. This article presents a critical review of select state-of-the-art machine learning techniques used to detect skin cancer. Several studies have been collected, and an analysis of the performance of k-nearest neighbors, support vector machine, and convolutional neural networks algorithms on benchmark datasets was conducted. The shortcomings and disadvantages of each algorithm are briefly discussed. Challenges in detecting skin cancer are highlighted and the scope for future research is proposed.

Abstract:Journal of Cutaneous Medicine and Surgery, Ahead of Print. Background:Artificial intelligence (AI) in skin cancer is a promising research field to assist physicians and to provide support to patients remotely. Physicians' awareness to new developments in AI research is important to define the best practices and scope of integrating AI-enabled technologies within a clinical setting.Objectives:To analyze the characteristics and trends of AI skin cancer publications from dermatology journals.Methods:AI skin cancer publications were retrieved in June 2022 from the Web of Science. Publications were screened by title, abstract, and keywords to assess eligibility. Publications were fully reviewed. Publications were divided between nonmelanoma skin cancer (NMSC), melanoma, and skin cancer studies. The primary measured outcome was the number of citations. The secondary measured outcomes were articles' general characteristics and features related to AI.Results:A total of 168 articles were included: 25 on NMSC, 77 on melanoma, and 66 on skin cancer. The most common types of skin cancers were melanoma (134, 79.8%), basal cell carcinoma (61, 36.3%), and squamous cell carcinoma (45, 26.9%). All articles were published between 2000 and 2022, with 49 (29.2%) of them being published in 2021. Original studies that developed or assessed an algorithm predominantly used supervised learning (66, 97.0%) and deep neural networks (42, 67.7%). The most used imaging modalities were standard dermoscopy (76, 45.2%) and clinical images (39, 23.2%).Conclusions:Most publications focused on developing or assessing screening technologies with mainly deep neural network algorithms. This indicates the eminent need for dermatologists to label or annotate images used by novel AI systems.
