Abstract:Introduction: Many military service members and civilians suffer from lower extremity trauma. Despite recent advancements in lower limb bracing technology, it remains unclear whether these newer advanced braces offer improved comfort and functionality compared to conventional options. The IDEO (Intrepid Dynamic Exoskeletal Orthosis), a type of "advanced" orthosis was developed to assist in maintaining high functional performance in patients who have experienced high-energy lower extremity trauma and underwent limb salvage surgeries. Methods: A cross-sector multi-site initiative was completed to study the efficacy of advanced ankle foot orthoses (AFO) for lower limb trauma and injury compared to a conventional AFO. Following fitting, training, and accommodation, the subjects were assessed in each AFO system for mobility, self-reported function, safety and pain, and preference. Results: They preferred the advanced over the conventional AFO and the mobility and exertion perception improved with the advanced AFO with no difference in pain or overall health status scores. Discussion: Thus, an advanced AFO is an option for trauma affecting the lower limb. Long-term studies are required to better understand the accommodation and learning process of using an advanced AFO.

Abstract:The expression of cytokeratins in the epithelium of the middle ear and external auditory meatus of the rat was studied on cryosections of ethylenediaminetetraacetic acid–decalcified specimens by use of a panel of monoclonal antibodies. The normal middle ear epithelium revealed a complex cytokeratin profile, including regional differences. The induction of sterile middle ear effusions resulted in increased cytokeratin expression. Infective effusions were accompanied by both quantitative and qualitative changes in the cytokeratin expression patterns. The difference observed between the cytokeratin profiles of external meatal skin and those of middle ear epithelium may form a useful tool for research into cholesteatoma development.
Effects of carbon versus plastic ankle foot orthoses on gait outcomes and energy cost in patients with chronic stroke

Abstract:Objective: To compare the walking performances of hemiplegic subjects with chronic stroke under 3 conditions: with a new standard carbon fibre ankle foot orthosis (C-AFO), with a personal custom-made plastic AFO (P-AFO), and without any orthosis (No-AFO). Design: Randomized, controlled crossover design. Patients: Fifteen chronic patients with stroke (3 women and 12 men, 59 [10] years, 13 [15] years since injury). Methods: Patients performed 3 randomized sessions (with C-AFO, P-AFO, no-AFO), consisting of a 6-min walk test (6MWT) with VO2 measurement and a clinical gait analysis. Energy cost (Cw), walking speed, spatio-temporal, kinetic, and kinematic variables were measured. Results: No significant differences were found between the C-AFO and P-AFO conditions. Distance and walking speed in the 6MWT increased by 12% and 10% (p < 0.001) and stride width decreased by -8.7% and -13% (p < 0.0001) with P-AFO and C-AFO compared with the No-AFO condition. Cw decreased by 15% (p < 0.002), stride length increased by 10% (p < 0.01), step length on affected leg increased by 8% (p < 0.01), step length on contralateral leg by 13% (p < 0.01), and swing time on the contralateral leg increased by 6% (p < 0.01) with both AFO compared with the No-AFO condition. Conclusion: The use of an off-the-shelf composite AFO (after a short habituation period) in patients with chronic stroke immediately improved energy cost and gait outcomes to the same extent as their usual custom-made AFO.

Abstract:Background: In children with unilateral spastic cerebral palsy (USCP), ankle-foot orthoses (AFOs) are widely used to correct common gait deviations such as a drop-foot pattern. Most studies on this topic have investigated specific time points while omitting other parts of the gait cycle. Objectives: This study investigated the separate effects of prefabricated carbon fiber AFOs and custom-made hinged AFOs compared with barefoot walking in children with USCP with a drop-foot gait pattern using statistical parametric mapping. Study design: Retrospective, cross-sectional, repeated measures study. Methods: Twenty ambulatory children (9.9 ± 2.5 years) with USCP and a drop-foot gait pattern were included. Kinematics, kinetics, and spatiotemporal parameters assessed during 3-dimensional gait analysis were compared between barefoot and AFO walking. Statistical parametric mapping was used to compare joint angles and moment waveforms. Kinematics, kinetics and spatiotemporal parameters assessed during 3-dimensional gait analysis were compared between barefoot and AFO walking for each AFO type but not between the 2 AFO types. Results: Compared with barefoot walking, there was a steeper sole angle at initial contact, corresponding to a heel strike pattern, and an increased ankle dorsiflexion in swing with the use of both AFOs. The ankle plantar flexion moment during loading response increased. Ankle power generation during pre-swing decreased in the carbon fiber AFO group when walking with AFOs. Conclusions: Both AFOs were beneficial for improving a drop-foot gait pattern in these small patient groups and can, therefore, be recommended to treat this gait deviation in patients with unilateral cerebral palsy. However, the reduction in ankle power generation during push-off and additional goals targeted by AFOs, such as correction of structural or flexible foot deformities, should be considered for prescription.

Abstract:Background An ankle-foot orthosis (AFO) with plantar flexion resistance (PFR) can improve the first rocker function during gait, but the incremental changes in the resistive moment on balance and gait have not been well identified. Objectives To investigate the effect of changing the PFR moment of dynamic AFO (DAFO) on measures of the center of pressure (COP) and clinical gait outcomes in individuals with post-stroke hemiparesis. Method In this randomized repeated measure study of 36 stroke individuals, the customized DAFO using foot drop ankle units set in three PFR situations (low, medium, and high) was evaluated. The balance parameters for COP measures were investigated by HUMAC® Balance & Tilt System. Gait parameters and ankle kinematics were recorded using the 3D motion analysis through force platform and optoelectronic system. The comparison was made using a parametric ANOVA test and the P value was set at 0.05 for statistical significance. Results Significant differences were observed for COP average velocity (1.30 ± 0.64, 1.10 ± 0.05, and 1.37 ± 0.43), COP path length (43.3 ± 4.6, 33.4 ± 4.3, and 36.3 ± 5.4), walking velocity (11.0 ± 3.1, 13.2 ± 4.4, and 9.9 ± 3.5), and cadence (31.5 ± 2.0, 33.0 ± 3.1, and 29.0 ± 1.6) respectively for low, medium and high PFR settings (P < 0.05). Except for the COP path length and cadence, posthoc multiple comparisons revealed significant differences between low and medium (P < 0.05) and medium and high (P < 0.05) PFR grades. PFR with medium resistance demonstrated near-normal maximal peak ankle dorsiflexion (mean deviation of 8 degrees, P < 0.05). Conclusion Medium PFR grade should be encouraged since it can enhance balance parameters like path length and average velocity of COP, increase cadence and average velocity during gait, and improve maximal peak ankle dorsiflexion.

Abstract:Background Ankle-foot orthoses (AFOs) are orthopaedic devices often prescribed to treat foot drop. For patients who are not satisfied with off-the-shelf solutions, custom AFOs personalized to the patient's lower limb anatomy are required. Dynamic AFOs provide stability while allowing for physiological ankle mobility in the stance phase of walking. Research question Can a morphology-based dynamic custom AFO made of fiberglass-reinforced polyamide restore a quasi-normal gait pattern and improve comfort in patients with foot drop? Methods In this pilot study, the legs and feet of ten foot drop patients (age=64.9 ± 11.4 years; BMI=26.2 ± 2.1 kg/m 2 ) were scanned using a Kinect-based 3D scanner. A custom AFO was designed and produced for each patient using a fiberglass-reinforced polyamide through selective laser sintering. To assess kinematics, skin markers were placed on relevant bony landmarks according to a validated protocol. Each patient was instructed to walk at a self-selected comfortable speed under three conditions: wearing the custom AFO, wearing an off-the-shelf orthosis (Codivilla spring), and without any AFO (shod condition). Muscle activation in the tibialis anterior, gastrocnemius , rectus femoris and biceps femoris muscles in both legs was recorded using wireless sEMG sensors. The comfort and of each AFO was evaluated using a Visual Analogue Scale. Results The custom AFO resulted in significant increase of stride length and walking speed compared to the shod condition. Except for the hip joint, which exhibited greater maximum flexion and reduced range of motion, the kinematic parameters of all other joints were similar to those observed in a healthy control population. Furthermore, the custom AFO received significantly higher comfort scores compared to the Codivilla spring. Significance This study has provided evidence supporting the effectiveness of custom orthotic solutions in restoring lower limb kinematics and improving the perceived comfort in foot drop patients compared to off-the-shelf solutions.

Abstract:Rigid ankle foot orthosis (RAFO) is a commonly prescribed rehabilitation treatment to immobilize the ankle during recovery after an ankle injury. However, its use affects propulsion, which impacts gait speed, toe clearance, and metabolic cost of walking. Propulsion mechanics generate propulsive force that depends on two critical components, i.e., trailing limb angle (TLA) and ankle moment during push-off. This study aimed to investigate the immediate training effect of center of pressure (CoP) biofeedback on propulsion mechanism in healthy adults wearing RAFO during unconstrained overground walking. We hypothesized that shifting the CoP at heel strike in the target limb (limb with RAFO) will prompt adjustments in foot placement relative to the body's center of mass (CoM), thereby altering TLA and propulsive force during push-off. A wearable haptic biofeedback system was developed to provide CoP shift information, and six healthy subjects, simulating an immobile ankle with RAFO, participated in the study. Subjects walked under three conditions: without RAFO, with RAFO but without feedback, and with RAFO while receiving feedback (anterior or posterior CoP shift). Results revealed that walking with RAFO with posterior CoP shift feedback significantly increased peak propulsive force (PPF) (by ~13%) and peak TLA (by ~17%) compared to walking with RAFO but without feedback in the target limb. Conversely, anterior CoP shift feedback produced opposite effects. Interestingly, feedback does not affect the ankle moment. The findings suggest the potential incorporation of a CoP-guided biofeedback training system in the clinical rehabilitation of individuals with an immobile ankle wearing RAFO.

Abstract:Background: Passive-dynamic ankle-foot orthoses (PD-AFOs) are often prescribed to address plantar flexor weakness during gait, which is commonly observed after stroke. However, limited evidence is available to inform the prescription guidelines of PD-AFO bending stiffness. This study assessed the extent to which PD-AFOs customized to match an individual's level of plantar flexor weakness influence walking function, as compared to No AFO and their standard of care (SOC) AFO. Methods: Mechanical cost-of-transport, self-selected walking speed, and key biomechanical variables were measured while individuals greater than six months post-stroke walked with No AFO, with their SOC AFO, and with a stiffness-customized PD-AFO. Outcomes were compared across these conditions using a repeated measures ANOVA or Friedman test (depending on normality) for group-level analysis and simulation modeling analysis for individual-level analysis. Results: Twenty participants completed study activities. Mechanical cost-of-transport and self-selected walking speed improved with the stiffness-customized PD-AFOs compared to No AFO and SOC AFO. However, this did not result in a consistent improvement in other biomechanical variables toward typical values. In line with the heterogeneous nature of the post-stroke population, the response to the PD-AFO was highly variable. Conclusions: Stiffness-customized PD-AFOs can improve the mechanical cost-of-transport and self-selected walking speed in many individuals post-stroke, as compared to No AFO and participants' standard of care AFO. This work provides initial efficacy data for stiffness-customized PD-AFOs in individuals post-stroke and lays the foundation for future studies to enable consistently effective prescription of PD-AFOs for patients post-stroke in clinical practice. Abstract:Superconductivity of nanosized Pb-island structures whose radius is 0.8 to 2.5 times their coherence length was studied under magnetic fields using low-temperature scanning tunneling microscopy and spectroscopy. Spatial profiles of superconductivity were obtained by conductance measurements at zero-bias voltage. Critical magnetic fields for vortex penetration and expulsion and for superconductivity breaking were measured for each island. The critical fields depending on the lateral size of the islands and existence of the minimum lateral size for vortex formation were observed.
Biomechanical effects of three types of foot orthoses in individuals with posterior tibial tendon dysfunction

Abstract:Background: Posterior tibial tendon dysfunction (PTTD) is characterized by degeneration of this tendon leading to a flattening of the medial longitudinal arch of the foot. Foot orthoses (FOs) can be used as a treatment option, but their biomechanical effects on individuals with PTTD are not yet fully understood. Research question: The aim of this study was to investigate the effects of three types of FOs on gait biomechanics in individuals with PTTD. Methods: Fourteen individuals were recruited with painful stage 1 or 2 PTTD based on Johnson and Strom's classification. Quantitative gait analysis of the affected limb was performed in four conditions: shoes only (Shoe), prefabricated FO (PFO), neutral custom FO (CFO) and custom varus FO (CVFO) with a 5° medial wedge and a 4 mm medial heel skive. A curve analysis, using 1D statistical parametric mapping, was undertaken to assess differences in lower limb joint motion, joint moments and muscle activity over the stance phase of gait across conditions. Results: Decreased hindfoot eversion angles, decreased ankle inversion moments and increased ankle eversion moments were observed with custom FOs compared to the Shoe and PFO conditions (p < 0.001). CFOs and CVFOs induced an increased knee abduction moment compared to Shoe (p < 0.001). No changes in hip kinematics and kinetics or in EMG activity of tested muscles were observed between conditions. Significance: Custom orthoses may be more suitable than PFOs to decrease the pathological biomechanical outcomes observed in PTTD. Decreased ankle inversion moments during the stance phase could explain why custom orthoses are effective at reducing pain in PTTD patients. However, clinicians should be careful when prescribing custom orthoses for PTTD since unwanted collateral biomechanical effects can be observed at the knee.

Abstract:The primary goal of rehabilitation for individuals with lower limb amputation, particularly those with unilateral transfemoral amputation (uTFA), is to restore their ability to walk independently. Effective control of the center of pressure (COP) during gait is vital for maintaining balance and stability, yet it poses a significant challenge for individuals with uTFA. This study aims to study the COP during gait in individuals with uTFA and elucidate their unique compensatory strategies. This study involved 12 uTFA participants and age-matched non-disabled controls, with gait and COP trajectory data collected using an instrumented treadmill. Gait and COP parameters between the control limb (CL), prosthetic limb (PL), and intact limb (IL) were compared. Notably, the mediolateral displacement of COP in PL exhibited significant lateral displacement compared to the CL from 30% to 60% of the stance. In 20% to 45% of the stance, the COP forward speed of PL was significantly higher than that of the IL. Furthermore, during the initial 20% of the stance, the vertical ground reaction force of PL was significantly lower than that of IL. Additionally, individuals with uTFA exhibited a distinct gait pattern with altered duration of loading response, single limb support, pre-swing and swing phases, and step time. These findings indicate the adaptability of individuals with uTFA in weight transfer, balance control, and pressure distribution on gait stability. In conclusion, this study provides valuable insights into the unique gait dynamics and balance strategies of uTFA patients, highlighting the importance of optimizing prosthetic design, alignment procedures, and rehabilitation programs to enhance gait patterns and reduce the risk of injuries due to compensatory movements.

Abstract:Spinal cord injuries frequently impair the ability to walk. Powered lower limb exoskeletons offer a promising solution to restore walking ability. However, they are currently restricted to even ground. We hypothesized that compliant exoskeleton knees could decrease required effort to maneuver on uneven terrain, and increase gait velocity and stability. We describe a case study of a motor-complete spinal cord injury user (AIS A, Th12) walking with a powered exoskeleton on even and uneven ground over multiple sessions after extensive training. Measurements with compliant or rigid exoskeleton knee joints were performed on three different days for each configuration. Body motion and crutch ground interaction forces were recorded to assess gait performance. We observed higher walking speeds with a compliant exoskeleton knee configuration (mean: 0.116 m/s on uneven and 0.145 m/s on even ground) compared to a rigid configuration (mean: 0.083 m/s and 0.100 m/s). Crutch force impulse was significantly reduced in the compliant configuration. Lastly, gait was more symmetric when the knee joints were compliant. In conclusion, compliant exoskeleton knee joints can help maneuver uneven ground faster and with less user effort than rigid joints. Based on our findings, exoskeleton designers should consider introducing compliance into their design to increase gait robustness and performance, and render exoskeletons more suitable for daily life use.
Deformable foot orthoses redistribute power from the ankle to the distal foot during walking

Abstract:Recently, carbon fiber plates, or orthoses, have been incorporated into footwear to improve running performance, presumably through improved energy storage and return. However, few studies have explored the energetic effects these orthoses have on the distal foot, have utilized such orthoses in walking, and none have sought to specifically harness metatarsophalangeal joint deformation to store and return energy to the ankle-foot complex. To address these gaps, we developed and tested a deformable carbon fiber foot orthosis aiming to harness foot energetics and quantify the resulting effects on ankle energetics during walking in healthy adults. Eight subjects walked under three conditions: barefoot (BF), with minimalist shoes (SH), and with bilateral, deformable foot orthoses in the minimalist shoes (ORTH). Ankle and distal foot energetics, foot-to-floor and ankle angle, stance time, step length, and max center of pressure (COP) position were calculated. When walking with the orthoses, subjects showed 263.6% increase in positive distal foot work along with a 31.9% decrease in ankle work and little to no change in the overall ankle-foot complex work. Step length, stance time, and max anterior COP position significantly increased with orthosis use. No statistical or visual differences were found between BF and SH conditions indicating that our findings were due to the foot orthoses. These results suggest this foot orthosis redistributes power from the ankle to the distal foot for healthy adults, reducing the energetic demand on the ankle. These results lay the foundation for designing orthotics and footwear to improve ankle-foot energetics for clinical populations.

Abstract:Background: Adequate research is not reported so far to underline the influence of commonly used polycentric knee joints on gait performance of subjects with trans-femoral amputation. Objective: The intent of this investigation is to analyze prosthetic gait of unilateral traumatic trans-femoral amputees with polycentric four-bar linkage knee and compare it with normal subjects for evaluating any asymmetry in gait performance. Methods: Objective three-dimensional gait analysis of 15 subjects [mean (age): 36.4 (10.7) years] were performed in gait lab through force plate and optoelectronic devices to measure temporal-spatial parameters, kinematic and kinetic performances. Gait patterns of amputees were compared with those of 15 individuals with normal gait to analyze distinct functionalities of existing polycentric knee. Results: Asymmetry in gait was observed between amputees and normal subjects for all variables concerned ([Formula: see text]). Amputee gait was with significantly lesser velocity, cadence with shorter step and stride length. There was significantly less hip, knee and pelvic motions, however, pelvic obliquity and rotation did not show significant difference from the normal subjects. The vertical component of the ground reaction force differed significantly between prosthetic and intact limb [49.7 (8.5)% and 90.4 (7.4)% body weight] and also from normal subjects [107.5 (2.4)% body weight] during stance ([Formula: see text]). Interpretation and Conclusion: This difference may be attributed to nonproportionate loading of limbs and mechanical adaptations for counteracting deficiencies of prosthetic side. Abstract:Background: Undesirable lower limb gait deviations have previously been reported when wearing orthotic walker boots; therefore, there is a need to optimize orthotic walker boot designs to facilitate normal gait. Objective: This study explored the biomechanical effects of 2 designs of orthotic walker boot on the lower limb and trunk compared with usual footwear. Study design: A repeated measures analysis of variance was used to evaluate selected kinematic and kinetic variables under different walking conditions. Methods: Sixteen healthy participants walked in 3 conditions using: Walker A (Airselect Elite, Enovis), Walker B (Townsend XLR8 Series Walker, Thuasne, France), and a usual shoe . A 10-camera motion analysis system and 4 force plates were used to collect kinematic and kinetic data. Results: Gait speed was significantly slower in both orthotic walker boots, and there was significantly decreased ankle range of motion, which is their primary function. Significant deviations in normal knee and hip kinematics and kinetics, shank-to-vertical angle, and pelvic and trunk movements were noted with both walker boots, with the greatest deviations from the shoe condition observed in Walker B. Recline and incline shank angular velocities showed the greatest differences in Walker B, which could be associated with adverse knee joint moments and a significantly greater perceived ease of walking in Walker A. Conclusions: Orthotic walker boot design significantly affects walking mechanics. Orthotic walkers with greater forefoot rocker profiles and inclined vertical shank angles may at least partly mitigate known gait deviations when wearing orthotic walkers.

Abstract:In the new electricity tariff market, as an important part of the smart grid, V2G (vehicle-to-grid) has been obtained widespread concern and in-depth study. In this paper, taking the scenario of year 2020 in the urban district of Chongqing, China for example, the size of EVs (electric vehicles) and EV charging facilities are estimated, and V2G system's net revenue are calculated. The results show that earnings per V2G electric car is still too small, and it's not enough to stimulate electric car owners to participate in V2G mode. The main reason is that the high cost per kWh of EV battery and relatively small peak-valley electricity price ratio.
