Preliminary Study on Evaluating Adrenal Function in Nephrotic Syndrome Patients by Low-Dose Rapid ACTH1-39 Stimulation Tes

Abstract:Objective Recently, a lot of papers have been published in abroad which recommended low-dose rapid ACTH1-24 stimulation test instead of large-dose ACTH1-24stimulation test when evaluating adrenocortical insufficiency. There was no similar research report in China. This study used ACTH1-39to replace ACTH1-24 for performing low-dose rapid ACTH stimulation test. A self control design was also used to compare the results of low-dose rapid ACTH stimulation test with conventional 2-hour ACTH stimulation test. Methods Two kinds of ACTH stimulation tests were performed in 18 patients with nephritic syndrome on alternate days. On day 1, low-dose rapid ACTH stimulation test was performed. 1 U ACTH was injected as an intravenous bolus after collection of a sample at 0 min. Another sample was drawn at 25 min for cortisol measurement. On day 3, conventional 2-hour ACTH stimulation test was performed. 25 U ACTH was injected as an intravenous drip for 2 hours after collection of a sample at 0 min. The other sample was attained at 2 hour for cortisol measurement. ACTH1-24 was replaced by ACTH1-39 in both low-dose rapid ACTH stimulation test and conventional 2-hour ACTH stimulation test. Student's test and Kappa analysis were used for statistical analysis.Results The results of two kinds of tests were completely equal in 10 cases who took prednisone daily or prior treatment. 8 cases took prednisone every other day, 5 cases of them showed the same results in the two tests. Only 3 cases showed different results in the two tests, who took prednisone more than 1 mg·kg-1 every other day. The low-dose rapid ACTH stimulation test revealed a diminished adrenocortical reserve, while the traditional 2-hour ACTH stimulation test was normal. Kappa analysis of paired results showed good concordance. Conclusions ACTH1-39 could substitute ACTH1-24 to perform low-dose rapid ACTH stimulation test. The two kinds of ACTH stimulation tests had good correlation. Compared with the traditional 2-hour ACTH stimulation test,the low-dose rapid ACTH stimulation test was more convenient and could be performed in out-patient clinic. So the low-dose rapid ACTH stimulation test may replace the conventional 2-hour ACTH stimulation test for evaluating adrenocortical function.
Detecting the interferences in adrenocorticotropic hormone measurement - three cases reinforcing the efficiency of the complementary clinical and laboratory audit

Abstract:Accurate measurement of adrenocorticotropic hormone (ACTH) is crucial in the evaluation of pituitary and adrenal disorders. Although great progress has been achieved in ACTH measurement with immunometric assays, interference may occur and adversely affect the clinical management. The report contributes to compiling the evidence on the clinical challenges with the management of the interferences in the ACTH measurement by presenting three cases: two with clinically overt hypercortisolism and discrepant ACTH concentrations within the reference interval; the third case describes the falsely elevated ACTH in a patient with secondary adrenal insufficiency. In all patients, the results obtained with the two immunometric platforms, chemiluminescence (CLIA) immunoassay (Siemens, Immulite) and electrochemiluminescence (ECLIA) immunoassay (Roche, Cobas), were discordant. Serial dilution of plasma samples revealed nonlinearity. After polyethylene glycol (PEG) precipitation recoveries were less than 22%, 26%, and 3%, respectively, supporting interference. Moreover, a decrease in ACTH concentration after incubation in a heterophile antibody-blocking tube was observed in the second case. In the first case, misinterpretation of ACTH led to inferior petrosal sinus sampling (IPSS), whereas timely detection of assay interference prevented further investigations in other cases. Increasing awareness regarding ACTH interference and comprehensive approach in evaluation could allow timely detection, helping to prevent unnecessary testing and perplexing clinical outcomes.

Abstract:Objective To evaluate the value of inferior petrosal sinus sampling in ACTH-dependent Cushing's syndrome with unknown origin.Method IPSS was carried out for the diagnosis of 20 cases of ACTH dependent Cushing's syndrome who had not been identified of diagnosis after a series of dexamethasone suppression tests and radiological examinations from Jan,2008 to Jan 2009.The ratio of plasma ACTH concentration between inferior petrosal sinus and peripheral veins was assayed.The sensitivity and specificity of diagnosis of the Cushing's disease were estimated.Results The basal ratio of ACTH concentration between inferior petrosal sinus and peripheral veins was ＞2 in 18 cases.13 cases underwent transphenoidal surgery with pathological diagnosis of pituitary ACTH adenoma;1 patient chose γ knife treatment and 4 patients chose somatostatin therapy with relieve of the disease.The basal ratio of ACTH concentration between inferior petrosal sinus and peripheral veins was ＜2 in 2 cases. One pulmonary node was removed by surgery with pathological diagnosis of carcinoid in 1 patient.One transphenoidal surgery was still undergone according to the clinical features in the other patient with pathological diagnosis of pituitary ACTH adenoma. The sensitivity and specify of inferior petrosal sinus sampling for the diagnosis of Cushing's disease in this study were 93% and 100% respectively.Conclusion Inferior petrosal sinus sampling is a safe technique with infrequent complications. It has great value in the differential diagnosis of ACTH-dependent Cushing's disease with unknown origin.

Abstract:INTRODUCTION: The accurate interpretation of the cosyntropin (adrenocorticotropic hormone [ACTH]) stimulation test requires method- and assay-specific cutoffs of the level of cortisol. Compared with a historical cutoff (18 μg/dL) for polyclonal antibody-based immunoassays, lower thresholds were proposed for the Roche Elecsys II assay, which uses a monoclonal antibody. However, cutoffs for other commonly adopted, monoclonal antibody-based cortisol assays were not yet available. Here, we established the thresholds for the level of cortisol specific to the Abbott Architect immunoassay by comparing the measurements of the level of cortisol using 3 immunoassays.METHODS: The ACTH stimulation test was performed in patients with suspected adrenal insufficiency (n = 50). The serum cortisol level was measured using the Abbott Architect, Roche Elecsys II, and Siemens Centaur assays. The results of the Abbott assay were also compared with those of liquid chromatography-tandem mass spectrometry. The receiver operating characteristic analysis was performed to derive new diagnostic thresholds for the Abbott assay using the polyclonal antibody-based Siemens assay as the reference method.RESULTS: The concentrations of cortisol measured using the Abbott assay were similar to those measured using liquid chromatography-tandem mass spectrometry and the Roche Elecsys II assay but significantly lower than those measured using the Siemens assay. The optimized threshold for cortisol using the Abbott assay was 14.6 μg/dL at 60 minutes after stimulation (sensitivity, 92%; specificity, 96%) and 13.2 μg/dL at 30 minutes after stimulation (sensitivity, 100%; specificity, 89%).CONCLUSION: We recommend a threshold of 14.6 μg/dL for the level of cortisol at 60 minutes after ACTH stimulation for the Abbott assay. In comparison with the historical threshold of 18 μg/dL, the application of the new cutoff may significantly decrease false-positive results due to ACTH stimulation testing. The use of assay-specific cutoffs will be essential for reducing misclassification and overtreatment in patients with suspected adrenal insufficiency.

While the Endocrine Society recommends an 18 ug/dL cutoff, lower thresholds (14.6 ug/dL) were proposed for the Roche Elecsys II assay in 2021, which employs a monoclonal antibody. However, cutoff values for the new Siemens Atellica platform, utilizing polyclonal antibody-based cortisol assays, remain undetermined. This study aimed to evaluate cortisol thresholds specific to the Siemens Atellica immunoassay by comparing cortisol measurements with the Roche Elecsys II assay.MethodsThe ACTH stimulation test was conducted in patients with suspected adrenal insufficiency (n=28). Serum cortisol levels were measured using the Siemens Atellica and Roche Elecsys II assays. Atellica assay results were also compared with those obtained via liquid chromatography tandem mass spectrometry (LC-MS/MS).ResultsCortisol concentrations measured with the Atellica assay exhibited approximately 25-30% higher bias compared to LC-MS/MS and the Roche Elecsys II assay. Passing Bablok regression analysis revealed a strong correlation between the Atellica and LC-MS/MS assays (R 2 = 0.9312, slope = 1.26, intercept = 0.34). However, the correlation between Atellica and the Roche Elecsys II assay was weaker compared to LC-MS/MS (R 2 = 0.7049 at 0 min, slope = 1.28, intercept = 0.19; R 2 = 0.8066 at 30 min, slope = 1.28, intercept = 0.59; R 2 = 0.8584 at 60 min, slope = 1.17, intercept = 2.03). The optimized cortisol threshold using the Atellica assay was determined to be 18 ug/dL at 30 minutes after stimulation (sensitivity, 100%; specificity, 90%), using the proposed 14.6 ug/dL cutoff at 30 minutes on Roche Elecsys II as the standard.ConclusionsEmploying assay-specific cutoffs is crucial for reducing misclassification and overtreatment in patients with suspected adrenal insufficiency. We recommend a cortisol threshold of 18 ug/dL at 30 minutes after ACTH stimulation for the Atellica assay, consistent with the Endocrine Society's recommendation.

Abstract:BACKGROUND: Analysis of plasma adrenocorticotropic hormone concentration [ACTH] aids diagnosis of pituitary pars intermedia dysfunction (PPID). Comparisons of the validated chemiluminescent-immunoassay (CI) and immunofluorescent (IF) assays are limited.OBJECTIVES: To compare the results of [ACTH] analysis by CI and IF methods of samples collected in autumn and spring and assess cross-reactivity.STUDY DESIGN: Method comparison.METHODS: Plasma from nonlaminitic ponies was analysed concurrently using the IF and CI methods in autumn and the following spring. Diagnostic thresholds for the IF method were derived using ROC curves and Youden indices to correspond with CI thresholds. Assay specificity was assessed using commercially available ACTH fragments and degradation products of endogenous ACTH.RESULTS: CI and IF methods yielded different results (P<0.001); mean differences (CI-IF), (95% confidence intervals): Autumn (n=99) 38.6 (30.6-46.5) pg/ml, Spring (n=88) 5.1 (3.9-6.3) pg/ml. The association between CI and IF results differed in autumn and spring, consistent with seasonally dependent cross-reactivity or interference. Good (κ=0.66-0.74) agreement was obtained for binary interpretation in spring between IF and CI using thresholds of >24 and >29 pg/ml respectively and in autumn between IF and CI using thresholds of >27 and >47 pg/ml respectively or >33 and >77 pg/ml respectively. Of 88 ponies with both spring and autumn samples, 56 (64%) exceeded a published autumn CI threshold (>47 pg/ml), of which 39 (70%) were below the equivalent threshold (<29 pg/ml) the following spring without treatment. The CI assay showed apparent increases in [ACTH] following addition of CLIP (ACTH 18-39). Degradation of ACTH during storage affected the assays differently.MAIN LIMITATIONS: Limited numbers of PPID cases were included. Immunoreactivity of commercially available peptides may differ from their endogenous equivalents.CONCLUSIONS: The methods yielded different absolute values but the agreement for binary classification was good. Abstract:Background The 0.25 mg short synacthen test is used to assess recovery from hypothalamic–pituitary–adrenal suppression due to chronic glucocorticoid administration. We assessed the potential role of salivary cortisol and cortisone in predicting hypothalamic–pituitary–adrenal function using the short synacthen test as the gold standard test. Method Between 09:00 and 10:30, salivary and blood samples were collected just prior to a short synacthen test to assess hypothalamic–pituitary–adrenal axis recovery in patients previously treated with oral glucocorticoids. The cut-off for a normal short synacthen test was a 30-min cortisol ≥450 nmol/L. Results Fifty-six short synacthen tests were performed on 47 patients. Of these, 15 were normal. The area under receiver operating characteristic curves for serum cortisol, salivary cortisone and salivary cortisol were 0.772, 0.785 and 0.770, respectively. From the receiver operating characteristic analysis, the cut-offs for baseline serum cortisol (≥365 nmol/L) and salivary cortisone (≥37.2 nmol) predicted hypothalamic–pituitary–adrenal axis recovery with 100% specificity in 26.7% of pass short synacthen tests, whereas salivary cortisol predicted none. Baseline serum cortisol (≤170 nmol/L), salivary cortisone (≤9.42 nmol/L) and salivary cortisol (≤1.92 nmol/L) predicted hypothalamic–pituitary–adrenal suppression with 100% sensitivity in 58.5%, 53.7% and 51.2% of failed short synacthen tests, respectively. Using these cut-offs, baseline serum cortisol, salivary cortisone and salivary cortisol could reduce the need for short synacthen tests by 50%, 46% and 37%, respectively. Conclusion Although marginally inferior to early morning serum cortisol, early morning salivary cortisone may be used as a first-line test for assessing hypothalamic–pituitary–adrenal function. We plan to incorporate salivary cortisone into a home-based patient pathway to identify patients with hypothalamic–pituitary–adrenal recovery, continuing hypothalamic–pituitary–adrenal suppression and those who require a short synacthen test.
Stability of adrenocorticotropic hormone in whole blood samples: effects of storage conditions

Abstract:Introduction: Adrenocorticotropic hormone (ACTH) is a peptide secreted by pituitary gland that plays an important role in regulating cortisol secretion. Its determination is difficult because of instability in whole blood. Several factors that influence ACTH stability in blood before analysis have been identified: temperature, hemolysis, time to centrifugation and presence of protease inhibitors. Published results on ACTH whole blood stability seem contradictory. Materials and methods: We performed a stability study in 10 healthy volunteers. Three different conditions were tested: ethylenediaminetetraacetic acid (EDTA) at 4 °C, EDTA + aprotinin at 4 °C, EDTA + aprotinin at room temperature. Stability was evaluated for 8 hours. Adrenocorticotropic hormone measurements and hemolysis index were performed respectively on Cobas e602 and c701 (Roche Diagnostics, Mannheim, Germany). We compared percentage deviations with total change limit using a threshold of 7.5%. Results: We showed that ACTH is stable 8 hours with EDTA at 4 °C, 4 hours with EDTA + aprotinin at 4 °C and 2 hours with EDTA + aprotinin at 22 °C. Conclusions: Aprotinin does not appear to give ACTH greater stability but can be used without exceeding 4 hours at 4 °C. Refrigerated pouch transport also seems to be more appropriate for ACTH in whole blood.
Dynamic testing for differential diagnosis of ACTH-dependent Cushing Syndrome: a systematic review and meta-analysis.

Abstract:OBJECTIVE Diagnostic accuracy of testing currently used for the differential diagnosis of Cushing's disease (CD) vs ectopic -ACTH secretion (EAS) is difficult to interpret. The present study aimed to identify and evaluate the diagnostic accuracy of corticotropin-releasing hormone (CRH) test, desmopressin test, and high-dose dexamethasone suppression test (HDDST) when used to establish a CD or EAS diagnosis. DESIGN This study is a systematic review of the literature and meta-analysis. METHODS MEDLINE, OVID, and Web of Science databases were searched for articles published between the years 1990-2021. Articles included described at least one test(s) (CRH, desmopressin, or HDDST) and the diagnostic reference standard(s) (histopathology, petrosal sinus sampling, surgical remission, imaging, and long-term follow-up) used to establish a CD or EAS diagnosis. RESULTS Sixty-two studies were included: 43 reported the use of HDDST, 32 CRH test, and 21 desmopressin test. CRH test was found to have the highest sensitivity in detecting CD (ACTH 86.9%, 95% confidence interval [CI] 82.1-90.6, cortisol 86.2%, 95%CI 78.3-91.5) and the highest specificity in detecting EAS (ACTH 93.9%, 95%CI 87-98.3, cortisol 89.4%, 95%CI 82.8-93.7). This resulted in a high diagnostic odds ratio (58, 95%CI 43.25-77.47), large area under the curve, and a receiver-operating characteristic (ROC) of 0.934. The diagnostic accuracy of HDDST and desmopressin test(s) was lower than that of the CRH test. CONCLUSION The meta-analysis indicates that a patient with a positive ACTH response after a CRH test is highly likely to have CD. Further studies analysing role of dynamic testing in addition to imaging are needed.

Abstract:Objective: To investigate the hypothalamic-pituitary-adrenal (HPA) axis via the insulin-tolerance test (ITT), standard-dose (250 microg) ACTH test (SDT) and low-dose (1 microg) ACTH test (LDT) in patients with ankylosing spondylitis (AS). ACTH stimulation tests were carried out by using 1 microg and 250 microg i.v. ACTH as a bolus injection, and blood samples were drawn at 0, 30 and 60 min. ITT was performed by using intravenous (i.v.) soluble insulin, and serum glucose and cortisol levels were measured before and after 30, 60, 90 and 120 min. All of the tests were performed consecutively with 3-day intervals, after an overnight fast. Results: There were no significant differences between the patients with AS (mean age 36.9+/-6.7 years) and the healthy subjects (mean age 37.4+/-5.7 years) in terms of age. The basal cortisol levels in the AS group measured during LDT, SDT and ITT (556+/-204 nmol/l; 524+/-169 nmol/l; 418+/-232 nmol/l, respectively) were comparable to the values of the control group (572+/-199 nmol/l; 520+/-182 nmol/l; 424+/-194 nmol/l, respectively). There were also no significant differences in peak cortisol responses between the two groups using LDT (patients 1025+/-339 nmol/l; controls 844+/-236 nmol/l), SDT (patients 1082+/-243 nmol/l; controls 1120+/-131 nmol/l) and ITT (patients 834+/-256 nmol/; controls 820+/-239 nmol/). Conclusion: In contrast with findings in other inflammatory diseases such as rheumatoid arthritis and polymyalgia rheumatica, the present data indicate that there is no apparent abnormality of the HPA axis activity in patients with AS.

Abstract:Abstract Biochemical function of adrenal masses is currently based on 1mg post-overnight dexamethasone suppression test (pDST). Several approaches are recently developed, in order to reduce false positive/negative samples, only in retrospective series. They are based on the correlation of some different parameters, i.e. late-night salivary cortisol (LNSC) vs serum and salivary cortisol pDST; LNSC vs serum and salivary cortisol and serum dexamethasone pDST; LNSC and cortisone vs serum cortisol and salivary cortisol and cortisone pDST. Although these findings offer a better diagnostic performance, several conditions are still disappointed. No information is traceable about the harvest time of diurnal salivary and serum samples and no study include neither the levels of salivary nor urinary dexamethasone pDST. Aim of our study is to combine all these strategies in order to avoid the underestimated biases and obtain more precise information about the true “cortisol condition” of the patients. To reach this purpose we assess both cortisol and dexamethasone concentrations in several samples: saliva at 11PM before the drug administration, diurnal saliva and serum at 8AM and also the urine collection from 11PM to 8AM. Analytes levels are measured using a validated liquid chromatography-tandem mass spectrometry method. In this study we included 20 subjects without morphological adrenal alteration (MRI assessment), dyslipidemia, hypertension and impaired glucose tolerance (healthy controls) and 20 patients with adrenal incidentaloma showing different cortisol levels ranging from normal to ACTH-independent hypercortisolism. In both series, LNSC were similar to salivary cortisol pDST, even if they were greater in the patients with adrenal incidentalomas and subclinical cortisol secretion. Serum dexamethasone levels were in reference ranges, while salivary and urinary dexamethasone found in these matrices require additional sample numbers in order to establish appropriate cut-offs. Our preliminary results suggest that the combination of these findings could represent an improvement to assess the individual cortisol status.

Abstract:Adrenal insufficiency (AI) is potentially life-threatening and accurate diagnosis is crucial. The first-line diagnostic test, the ACTH stimulation test, measures serum total cortisol. However, this is affected in states of altered albumin or cortisol-binding globulin levels, limiting reliability. Salivary cortisol reflects free bioactive cortisol levels and is a promising alternative. However, few studies are available, and heterogenous methodologies limit applicability. This study prospectively recruited 42 outpatients undergoing evaluation for AI, excluding participants with altered cortisol-binding states. Serum (immunoassay) and salivary (LC-MS/MS) cortisol levels were sampled at baseline, 30-minutes and 60-minutes following 250mcg Synacthen administration. AI was defined as a peak serum cortisol level <500nmol/l in accordance with guidelines. The study recruited 21 (50%) participants with AI, and 21 without AI. There were no significant differences in baseline characteristics, blood pressure, or sodium levels between groups. Following Synacthen stimulation, serum and salivary cortisol levels showed good correlation at all timepoints (R2 = 0.74, P<0.001), at peak levels (R2 = 0.72, P<0.001), and at 60-minutes (R2=0.72, P<0.001). A salivary cortisol cut-off of 16.0 nmol/L had a sensitivity of 90.5% and a specificity of 76.2% for the diagnosis of AI. This study demonstrates good correlation between serum and salivary cortisol levels during the 250mcg Synacthen test. A peak salivary cortisol cut-off of 16.0 nmol/L can be used for the diagnosis of AI. It is a less invasive alternative to evaluate patients with suspected AI. Its potential utility in the diagnosis of AI in patients with altered cortisol-binding states should be further studied.

Abstract:Abstract Introduction: The short synacthen test (SST) is commonly used to assess adrenal function. Accurate timing and appropriate holding of exogenous steroids are essential to ensure correct interpretation of results. Aims & Methods: We reviewed all SSTs performed on inpatients in our hospital over a 1-year period, in order to determine accuracy of testing. Results: 42 patients (Male 15, Female 27), with mean age 68 years (range 43–90), underwent SST. The majority (39/42; 93%) of tests were requested by internal medicine physicians. The indications for testing were; suspected adrenal insufficiency (18), HPA axis suppression (9), fatigue (7), hyponatremia (5), suspected pituitary disease (2) and vomiting (1). 7 (44%) of the 16 patients taking steroids did not have medication appropriately held. 31 (74%) patients did not have serum ACTH measured prior to the test. 28 (66%) tests were not started at the correct time. Only 10 (24%) of the 30 minute samples were completed within the 25-35min sample window. The mean time between the 0min and 30min samples was 42mins (median 62mins; range 0-209mins). 12 (29%) tests involved an unnecessary 60min sample. 8 (19%) tests had no interpretation of results documented in the medical notes. 4 (10%) patients underwent repeat testing, necessitated by an incorrect first test. Discussion: The vast majority of inpatient SSTs (33/42;79%) were performed suboptimally, with the most common errors pertaining to incorrect timing of the test, inaccurate sampling and inappropriate pre-test steroid administration. Considering these errors, some results may have been interpreted incorrectly. Repeat tests were recognised as required in 10% of patients, with associated inconvenience, cost and discomfort. Improved training and guidelines for performing SSTs should be available to hospital staff to ensure more accurate application of the test.

Abstract:Abstract Introduction High dose (250 mcg) ACTH stimulation test is the gold standard dynamic testing to diagnose primary adrenal insufficiency (PAI). While the current guidelines do not elaborate on the preferable route of administration of corticotropin (intravenous or intramuscular), most published studies have been conducted on healthy people using the intravenous route. We present a patient with non-alcoholic steatohepatitis cirrhosis and hypotension who was thought to have suboptimal response with intramuscular ACTH stimulation test. Case 62-year-old male with cirrhosis, presented with dizziness, and was found to be hypotensive. Lab tests showed normal cell counts, electrolytes, renal function and lactic acid. His albumin was 2.6 g/dL. His blood pressure did not improve with intravenous fluids. Workup initiated for adrenal insufficiency showed 8 am cortisol level of 7. 0 mcg/dL, and ACTH level of 26 pg/mL. He underwent ACTH stimulation test with 250 mcg intramuscular cosyntropin, after which the 30-minute cortisol level was 13.7 mcg/dL and the 60-minute level was 15.1 mcg/dL (level immediately before the test 7.2 mcg/dL). Due to the patient not having signs of adrenal insufficiency and low albumin level due to liver disease, the cortisol response was considered adequate. Discussion Per the current Endocrine Society Clinical Practice Guidelines, the standard method of performing the ACTH stimulation test involves measuring baseline cortisol level, administration of 250 mcg of cosyntropin, and then measuring cortisol level 30 and 60 minutes post administration. Traditionally, an appropriate response is characterized by peak cortisol level above 18 mcg/dL, with lower values indicative of AI. While the responses can vary due to multitude of factors including time of testing etc., the individual response to the intravenous versus intramuscular injections is similar. However, the cut-off points between the two administration methods may differ. In a study on 64 healthy adult men who were administered 250 mcg cosyntropin intramuscularly, Longui et al. demonstrated lower cut-off values of 12.6 mcg/dL and 16.3 mcg/dL at 30 and 60 minutes respectively. In addition, with the introduction of newer monoclonal antibody and LC-MS/MS assays having greater specificity for cortisol, a study by Jarvosky et. al. recommends a new lower cut-off of 14-15 mcg/dL. Furthermore, several studies have shown higher prevalence of low total cortisol levels in cirrhosis and other liver disease leading to overdiagnosis of PAI, due to deficiency of cortisol binding globulin and albumin. In our patient, we hypothesize that the combination of all these factors could have caused a falsely low cortisol peak level without any underlying PAI. Our case highlights the need for additional studies on patients with underlying liver disease and hypoalbuminemia who undergo testing with intramuscular cosyntropin administration, and analysis with the new assays available. Abstract:Abstract Background There has been debate regarding the appropriate cortisol cutoffs during the cosyntropin stimulation test (CST) when the newer cortisol assays are used. Recent studies have recommended cortisol cutoffs lower than previously recommended. We aimed to evaluate the cortisol levels during the standard dose CST in patients evaluated for adrenal insufficiency and found to have a normal HPA axis using the Elecsys ® Cortisol II (Roche Diagnostics)assay. METHODS: This was a retrospective chart review study of adult patients who had the CST performed within an academic ambulatory setting from January 2018 to December 2020. They had ICD-10 codes of pituitary gland disorders in their problem list or diagnoses at the time of data extraction. Only patients with a normal HPA axis were included in the analysis, which was defined based on evaluation by an endocrinologist, restrictive exclusion criteria, and a lack of glucocorticoid administration for at least 6 months after CST. Exclusion criteria included oral glucocorticoid use within 6 months before the CST or 3 months for other routes, albumin < 2.5 g/dL, liver disease, pituitary insult within 6 weeks before the CST, estrogen and opioid use within 3 months before the CST, and pregnancy. The results are reported in the median (range). Results Out of 305 patients, 63 (20.6) met the study criteria. The median age was 54.7 (27.6-89.1) years; 32 (51%) were females, and 27 (43%) were white. The duration of follow-up after the CST was 13.9 (6.3-43.9) months. Cortisol values at 30- and 60-minutes post-CST were available for 43 (68.2%), and 61 patients (96.8%), respectively. The cortisol levels at 30- and 60-minutes after the CST were 21.7 (15.7-29.1) μg/dL and 24.4 (17.9-35.8) μg/dL, respectively. The lowest cortisol levels at 30 and 60 minutes after the CST for patients with either normal TSH or gonadal axis (n=47) or in whom both axes were normal (n=18) were similar to the entire cohort. Conclusion Our study supports using a lower cortisol cutoff value at 30 minutes after the CST using the Roche Elecsys ® Cortisol II assay. The lowest cortisol level in our cohort was 15.7 ug/dL. However, the same pass criteria cannot be used at 60 minutes. Therefore, it is essential to consider the time of cortisol draw after the CST. Presentation: No date and time listed

Abstract:The evaluation of chronic activity of the hypothalamic-pituitary-adrenal (HPA) axis is critical for determining the impact of chronic stressful situations. However, current methods have important limitations. The potential use of hair glucocorticoids as a noninvasive retrospective biomarker of long-term HPA activity is gaining acceptance in humans and wild animals. However, there is no study examining hair corticosterone (HC) in laboratory animals. The present study validates a method for measuring HC in rats and demonstrates that it properly reflects chronic HPA activity. The HC concentration was similar in male and female rats, despite higher total plasma corticosterone levels in females, tentatively suggesting that it reflects free rather than total plasma corticosterone. Exposure of male rats to 2 different chronic stress protocols (chronic immobilization and chronic unpredictable stress) resulted in similarly higher HC levels compared to controls (1.8-fold). HC also increased after a mild chronic stressor (30 min daily restraint). Chronic administration of 2 different doses of a long-acting ACTH preparation dramatically increased HC (3.1- and 21.5-fold, respectively), demonstrating that a ceiling effect in HC accumulation is unlikely under other more natural conditions. Finally, adrenalectomy significantly reduced HC. In conclusion, HC measurement in rats appears appropriate to evaluate integrated chronic changes in circulating corticosterone.
