Abstract:In this study, we examined the synthesis, and characterization of TiO2/poly(N-maleoylglycine-co-acrylic acid) (TiO2/poly(MG-co-AA)) nanocomposite. The nanocomposite was prepared by the dispersion of TiO2 nanotubes in a water solution of the polymer (3% w/w) and then it was lyophilized. The nanocomposite was characterized by FTIR and Raman spectroscopies. The incorporation of TiO2 nanotubes to the polymeric matrix was determined by transmission electron microscopy (TEM) from which TiO2 nanotubes in the inner of copolymer with diameters ranging between 90 and 100 nm were observed. The overall morphology of the previously synthetized nanotubes was determined by scanning electron microscopy (SEM). For the nanocomposite system, the morphology was studied by atomic force microscopy (AFM) from which a grain shape structure was observed. This process resulted in a new nanocompsite material with an average grain diameter estimated by SEM and AFM ranging between 210-240 nm. It was observed that the nanotubes were homogeneously dispersed within the polymeric matrix. The prepared material could be suitable in the design of electronic devices and additionally could have potential applications as biomaterial.
Pharyngeal Characteristics of Male Patients with Clefts in Mixed Dentition

Abstract:Objective To explore the pharyngeal characteristics of patients with clefts in mixed dentition.Methods Fifty-two male patients with unilateral cleft lip and palate (UCLP) in mixed dentition and fifty-two controls were selected for the current study,cephalometric analysis were applied to measure the pharyngeal structures.Results The TB-TPPW and the V-LPW value were 10.44 mm±3.23 mm and 13.82 mm±2.82 mm respectively in the UCLP group while the TB-TPPW and the.V-LPW value were 13.78 mm±3.47 mm and 15.63 mm±2.79 mm respectively in the noncleft group,Significant differences were found in TB-TPPW and V-LPW between UCLP and control groups (P=0.00).The mandibular plane angle (MP-SN) of the UCLP group and the control were 39.78°±5.22° and 33.41°±4.35°,respectively.Significant differences were found in MP-FH between UCLP and control groups(P=0.00).There are no significant difference of the length of soft palate,the dimension of velopharyngeal and the need ratio between the two groups.Conclusions Compared with the non-cleft patients,the UCLP patients demonstrate many substantial characteristics of the patients with obstructive sleep apnea; Velopharyngeal incompetence is not only related to the length of soft palate and velopharyngeal width.

Abstract:Introduction: Our objective was to compare the craniofacial hard and soft tissue characteristics between children with operated unilateral cleft lip and palate (UCLP) and children with noncleft lip and palate (non-CLP) with a Class III skeletal relationship. Methods: The study sample consisted of 30 subjects (18 boys, 12 girls; mean age, 10.21 years) affected by UCLP and 30 non-CLP subjects (17 boys, 13 girls; mean age, 10.19 years) as the control group. All subjects were in the mixed dentition with a Class III skeletal relationship. Cone-beam computed tomography-synthesized cephalograms were traced and evaluated, and craniofacial hard and soft tissue morphologies were compared between the UCLP and non-CLP groups. Results: Maxillary length and gonial angle were 2.66 mm shorter and 3.67 degrees greater, respectively, in the UCLP group than those in the non-CLP group. The SNA and SNB angles describing the sagittal positions of the maxilla and mandible, respectively, relative to the cranial base were significantly smaller in the UCLP group (P < 0.001 and P = 0.003, respectively). However, the 2 groups had similar sagittal intermaxillary relationships with similar ANB angles (P = 0.669). In the vertical dimension, the mandibular plane angle and the growth direction vector were significantly greater in the UCLP group (P = 0.007 and P < 0.001, respectively). Lastly, the UCLP group had a more concave soft tissue profile, manifested by a reduced facial convexity angle, as well as an acute nasolabial angle and a more protruded lower lip. Conclusions: Although the 2 groups had similar sagittal intermaxillary relationships, patients in the UCLP group had more retrusive maxillary and mandibular positions relative to the cranial base and more severe vertical discrepancies. Additionally, the soft tissue profiles of patients affected by UCLP were more concave, and the compensatory adaptation was less satisfactory.

Abstract:To study the relationship between velopharyngeal morphology and velopharyngeal function in operated cleft palate patients with velopharyngeal competence (VPC) with levator veli palatini retropositioning according to Sommerlad.Three groups were included in the study. The experimental group comprised 18 incomplete cleft patients (group T1) repaired with VPC repaired with levator veli palatini retropositioning according to Sommerlad and 14 incomplete cleft patients (group T2) with velopharyngeal incompetence (VPI) repaired with Langenbeck's technique. The control group was composed of 13 normal adults. The outcome of the velopharyngeal function by nasopharyngoscopy and lateral cephalogram was assessed. Skeletal landmarks and measurements were derived from the tracing of lateral cephalograms. The measurements included velar length, pharyngeal depth, and adequate ratio of velar length to pharyngeal depth. The cranial base, cervical vertebrae, posterior nasal spine, and the position of the posterior pharyngeal wall (PPW) in the pharyngeal triangle were also analyzed. All data were subjected to student's t-test of statistical significance.All 18 subjects in group T1 obtained complete velopharyngeal closure. Velopharyngeal closure in seven, five, and two subjects in group T2 was 70%, 50% to 70%, and less than 50%, respectively, according to the results of nasopharyngoscopy. The lateral velar length (25.7 mm + 2.3 mm) in group T1 was similar to that of the control group (29.9 mm + 2.7 mm) (P > 0.05). The pharyngeal depth in group T1 was shorter than that in the other two groups, and the adequate ratio (1.43 + 0.26) was similar to that in the normal group (1.45 + 0.26). Group T2 had a significantly short velar length (22.9 mm + 2.3 mm) and normal pharyngeal depth, resulting in a small length/depth ratio (0.95 + 0.14) than that in group T1 and the control group. PPW in the pharyngeal triangle was positioned superiorly in group T2 compared with the control group.The velopharyngeal morphology of operated cleft palate patients with VPC with levator veli palatini retropositioning according to Sommerlad was found to be similar to that of the normal control group. VPI cleft palate patients are characterized by a shorter palate, smaller adequate ratio (< 1.0), slightly counterclockwise-rotated pharyngeal triangle, and superiorly positioned PPW.
Craniofacial Morphology of Patients with Isolated Cleft Palate in the Mixed Dentition after Palatoplasty

Abstract:OBJECTIVE:To evaluate the craniofacial morphology of patients with isolated cleft palate in the mixed dentition after palatoplasty.METHODS:Twenty patients (7 to 11 years old) with isolated cleft palate in the mixed dentition were selected. The control group comprised 35 age-matched non-cleft children without orthodontic treatment. Cephalometric analysis was carried out to evaluate craniofacial morphology.RESULTS:The mean values of total cranial base length, maxillary depth, mandibular effective length in cleft patients were 86.48 mm, 44.79 mm and 65.45 mm, respectively. Those in control group were 91.27 mm, 48.84 mm and 70.49 mm, respectively (P < 0.001).CONCLUSIONS:Underdeveloped maxilla and class III skeletal profile were presented in patients with isolated cleft palate.

Abstract:The aim of this study was to visualize bony defects of the palate and vomer in submucous cleft palate patients (SMCP) by three-dimensional (3D) computed tomography (CT) reconstruction and to classify the range of bony defects. Forty-eight consecutive non-operated SMCP patients were included. Diagnosis was based on the presence of at least one of three classical signs of SMCP: bifid uvula, a translucent zone in the midline of the soft palate, and a palpable 'V' notch on the posterior border of the bony palate. Patients were imaged using spiral CT. 3D reconstruction models were created of the palate and vomer. The sagittal extent of the bony cleft in SMCP was classified into four types: type I, no V-shaped hard palate cleft (8.3%); type II, cleft involving the partial palate (43.8%); type III, cleft involving the complete palate and extending to the incisive foramen (43.8%); type IV, cleft involving the complete palate and the alveolar bone (4.2%). The extent of the vomer defect was classified into three types: type A, vomer completely fused with the palate (8.3%); type B, vomer partially fused with the palate (43.8%); type C, vomer not fused with the palate up to the incisive foramen (47.9%). Significant variability in hard palate defects in SMCP is the rule rather than the exception. The association of velopharyngeal insufficiency with anatomical malformations may be complex.
Comparative CBCT Analysis of Maxillofacial Skeletal Structures in Patients with Unilateral Cleft Lip and Palate and Non-Cleft Individuals

Abstract:Background: The aim of this study was to evaluate and compare the maxillofacial structures of individuals with unilateral cleft lip and palate (UCLP) and healthy controls using cone beam computed tomography (CBCT). Methods: The study included a total of 90 subjects, comprising 45 randomly selected individuals with UCLP (30 males and 15 females, mean age 14.69 ± 3.95 years) in the study group and 45 healthy individuals (30 males and 15 females, mean age 14.46 ± 3.65 years) in the control group. Maxillofacial measurements were taken in three different planes and categorized into five groups, namely vertical, facial, cranial, maxillary, and mandibular. In the statistical comparison between groups, the significance level was determined as p 0.05). Upper anterior face height and posterior face height in the UCLP group were found to be significantly shorter than the control group (p < 0.05). Midface width and depth were inadequate in the UCLP group (p < 0.05). Anterior and posterior cranial base lengths were significantly shorter in individuals with UCLP (p < 0.05). Nasal width and interorbital width were significantly greater in the UCLP group (p < 0.05). In addition, maxillary width, maxillary length, and mandibular width were significantly shorter in the UCLP group than in the control group (p < 0.05). Conclusions: While the control group exhibited generally longer measurements in all three dimensions compared to the study group, the skeletal structures adjacent to the cleft demonstrated the most notable developmental deficiency.

Abstract:Objective: To evaluate the development of bony nasal septum of cleft lip and palate patients. Methods: This retrospective study included 157 patients aged over 14 years with cleft lip and palate (CLP) and 54 normal people. The three-dimensional computed tomography reconstruction images of the posterior and inferior part of the bony nasal septum were measured. We evaluated the development of the bony nasal septum. Results: There were 39 patients with cleft lip only (CLO), 38 patients with cleft palate only (CPO), 80 patients with CLP. The L1, L2, and S of CLO and/or CPO patients were smaller than the normal control group. The L1 and S of cleft palate group and CLP group were smaller than cleft lip group, there was no significant difference for L2 between the three groups. Compared with the postoperative cleft palate group, the S of preoperative group was significantly bigger, the L2 was bigger without significant difference; but compared with the normal group, L1 and S of preoperative group was significantly smaller, L2 was not significantly different. Conclusions: The patients with cleft palate only were most affected. There was not only decrease of the fusion of maxilla and vomer, but also growth inhibition of the sutura between vomer and nasal septum cartilages, vertical plate of ethmoid bone in patients with cleft lip and palate. The adverse effects of the cleft palate operation to bony nasal septal development mainly lie in anterior inferior direction.

Abstract:Objective Experimental and clinical findings suggest that the vomer is involved in facial development and might contribute to the short and retrusive maxilla in cleft patients. The aim of this study was to investigate the relationship between vomer development and maxillary growth in unoperated submucous cleft palate (SMCP) patients. Design Retrospective cohort study. Participants Thirty unoperated SMCP patients were included. The criteria for clinical diagnosis were: bifid uvula, a translucent zone in the midline of the soft palate, and a touchable “V” notch on the posterior border of the bony palate. There were 19 female and 11 male patients, with an age range from 3 to 25 years. Interventions All patients were imaged using spiral computed tomography (CT) while in centric occlusion before the surgeries. Main Outcome Measures Three-dimensional (3D) reconstruction models were created, and dentoalveolar relationships were rated by three experienced doctors according to the GOSLON score principles. The patients then were divided into three groups: 1 - normal occlusion, 2 - edge-to-edge bite, and 3 - crossbite. The vomer-palate fusion rate was calculated on 3D CT images and represented the vomer development. Results The sagittal extent of the palatal cleft and the malformation of vomer in SMCP were greatly varied. The vomer-palate fusion rate in the crossbite group (occlusal score = 3) was significantly lower than that in the normal occlusion group (P .027). Conclusions Our findings suggest that correlation exists between vomer development and sagittal maxillary growth in unoperated SMCP patients.

Abstract:OBJECTIVE:To investigate the insertion position of the levator veli palatini in the soft palate with SMCP comparing normal group and find out reliance diagnosis methods for SMCP.METHODS:Experimental group: lateral skull radiography film were taken in 50 patients with SMCP at rest and "i" phonation. Five parameters were measured which are relative length of soft palate and hard palate; levatorveli palatini insertion in the soft palate; elongation and angle of soft palate when lifting up on phonation.CONTROL GROUP:Ten sets (rest and pronouncing "i") of lateral skull radiography were used as control group. X-rays were taken for ten normal subjects without speech disorder or other abnormal physical situations. All data were subjected to the t-test of statistical significance.RESULTS:The difference of measures between patients with SMCP and normal control was significant (P <or= 0.05).CONCLUSION:It was obvious that both absolute and relative length of soft palate are short and levator muscle insert into soft palate arterially in patients with SMCP comparing with normal group, which caused VPI and become one of the criterias for SMCP.
Simultaneous Monitoring of Force, Acceleration and Electromyogram during Computer-Controlled Infusion of Atracurium in Sheep

Abstract:Five sheep were anaesthetised with sodium thiopentone, nitrous oxide and oxygen, and the lungs ventilated artificially. A computer-controlled infusion of atracurium employing feedback from isometric force measurement was set to maintain 15, 50 and 85% depth of neuromuscular block. Each was held constant for 30 minutes, while isometric force and electromyogram were measured on the left forelimb and acceleration on the right. The acceleration transducer records at the three levels of block were practically identical with those from force measurements: mean differences (SD) between simultaneous measurements being 3.6(5.4)%. EMG value was 10.1 (12.0)% different from force measurement. Correlation between simultaneous force and either acceleration or EMG was excellent (r = 0.986 and 0.938 respectively). In clinical practice all three techniques measure block comparably. In the sheep forelimb, EMG was a less reliable method of measuring block.
Three-dimensional Analysis of Naso-Maxillary Complex Symmetry in Patients with Unilateral Complete Cleft Lip and Palate

Abstract:Objective To evaluate the symmetry of naso-maxillary complex in patients with unilateral complete cleft lip and palate with no palatoplasty history before 17 years old.Methods 9 patients (4 males and 5 females) with unilateral complete cleft lip and palate (UCLP) were selected.They all underwent lip repair before 2.5 years old and had no history of palatoplasty before 17 years.They received spiral CT scan and 3D reconstruction of their naso-maxillary complex.22 variables were measured in the reconstructed images and the symmetry of the complex were analyzed.Results The distances of NC-HP and PF-MSP in cleft side were 18.01 mm and 19.72 mm,which were 13.35 mm and 17.31 mm bigger than that in the noncleft side(P＜0.05).The distances of NC-CP and U3-HP in cleft side were 59.64 mm and 39.55 mm,which were 64.13 mm and 39.70 mm shorter than that in the non-cleft side (P＜0.05).The measurements showed a collapsed and sunken nasal cavity,a wider posterior hard palate and elevated canine in cleft side.There were also deviations of the middle structures.Crista galli,nasal septum,and the mid-point of upper central incisors deviated to cleft side,while skeletal anterior nasal spine deviated to noncleft side.Conclusions Most asymmetries are found in nasal cavity and the dentoalveolar area near the cleft.No significant asymmetry is found in upper cranialfacial strutures,nor in regions not in the adjacency of the cleft.
Comparison of Velopharyngeal Morphology of Two Palatoplasty Techniques in Patients with Hard and Soft Cleft Palate

Abstract:PurposeThe study aims to compare the velopharyngeal morphology of hard and soft cleft palate (HSCP) patients after Furlow and Sommerlad palatoplasty.Patients and methodsA total of 51 patients (20 cases in Furlow palatoplasty group, 16 cases in Sommerlad palatoplasty group and 15 normal children in the control group) were included in our study. Velopharyngeal function and speech outcomes of patients with HSCP who had either Furlow palatoplasty or Sommerlad palatoplasty for cleft palate repair were evaluated by perceptual speech assessment (PSA), lateral cephalometric radiographs and nasopharyngoscopy. To assess velopharyngeal morphology of patients treated with two techqiques, we analyzed measurements such as velar length, pharyngeal depth, and the Adequate ratio (the ratio of velar length to pharyngeal depth). Furthermore, skeletal landmarks including cranial base, cervical vertebrae, posterior nasal spine which were defined as the pharyngeal triangle were measured. Finally, the position of the point U relative to the pharyngeal triangle were compared.ResultsVelopharyngeal closure (VPC) rate in Furlow palatoplasty group accounted for 90%, while that in Sommerlad palatoplasty group was 81.3%. PSA of the former group was significantly better than that of the latter group (P < 0.05). Velar length, pharyngeal depth and the Adequate ratio (1.37 ± 0.14 vs. 1.41 ± 0.15) were comparable between the Furlow group and control group (P > 0.05), while Sommerlad group had a shorter velar length, deeper pharyngeal depth and a smaller Adequate ratio (1.20 ± 0.18) compared to the above two groups (P < 0.05). Furhermore, the point U of Sommerlad group in the pharyngeal triangle was higher than that of the other two groups.ConclusionsIn the treatment modality of patients with HSCP, both Furlow palatoplasty and Sommerlad palatoplasty seem to be effective. Furlow palatoplasty appears to have velopharyngeal morphology similar to normal control group., while Sommerlad group shows a shorter velar length, deeper pharyngeal depth and a smaller Adequate ratio

Abstract:Objective:To evaluate the vomer development of cleft palate patients.Methods:38 patients over 1 4 years(averaged 23.4 years)of age with cleft palate and 76 controls of normal people(aged 22.8 year on average)were included.The 3D computed tomo-graphy reconstruction images of the bony nasal septum were measured.The development of the vomer was evaluated by comparing the L1 (the length of the lower edge of the vomer),L2 (the length from nasal spine to the point of the sella)and S (the approximate area of vomer)among deferent groups.Results:The L1 ,L2,and S of cleft palate patients were smaller than those of the controls(P <0. 05).Compared with the postoperative cleft cases,the S and L2 of preoperative cases were bigger(P <0.05).Conclusion:The vomer development is adversely affected by cleft palate.Not only the vomer-palate fusion is lower,but also the sutura between vomer and na-sal septum cartilage and ethmoid bone are short.And the latter is greatly influenced by surgical trauma.

Abstract:The Cleft Palate Craniofacial Journal, Ahead of Print. ObjectiveThis study aimed to use cone beam computed tomography (CBCT) scans to compare the morphometric characteristics of the greater palatine foramen (GPF) in unilateral and bilateral cleft lip/palate patients (CL/P) and non-cleft (NC) individuals.Method and materialOne hundred patients with CL/P (78 unilateral and 22 bilateral) and 100 non-cleft individuals had their GPF morphometrically assessed in CBCT images. The antero-posterior width and medio-lateral width of the GPF, the distance between the right and left GPFs, the mesio-distal location of the GPF with respect to the molar teeth, the distance between the GPF and the hamulus of the medial pterygoid plate, and the distance between the GPF and the palatine alveolar ridge were measured on these images. The Kruskal–Wallis and Chi-square tests were employed to ascertain if there were any notable disparities across the 3 groups.ResultsThe GPF's antero-posterior width and medio-lateral width, and distance from the hamulus and palatine alveolar ridge were considerably smaller in the unilateral cleft lip/palate (UCL/P) and bilateral cleft lip/palate (BCL/P) patients than in the NC individuals. However, no significant difference was observed between the BCL/P and UCL/P groups in terms of these parameters. The distance between GPFs and the location of the GPF did not exhibit any significant differences among the 3 groups.ConclusionCertain morphometric parameters of the GPF were significantly different in CL/P patients. Abstract:Secondary deformities to the midfacial area in adolescence and adulthood are common sequelae after primary cleft lip and palate repair and lead to significant functional, aesthetic and psychosocial challenges. Treatment for patients with normal occlusion or treated malocclusion and remaining midfacial hypoplasia includes autologous and alloplastic augmentation of the maxilla. Because of the great variation of deformities, accurate evaluation is essential for appropriate treatment planning. To provide insights in the altered midfacial anatomy in cleft patients, a retrospective computed tomography morphologic analysis of the anterior projection of the maxilla in the paranasal region, soft tissue thickness, and relation of soft tissue to bony landmarks was performed in adolescent, unilateral, non-syndromic complete cleft and palate patients. The anterior projection of the maxilla was very variable with the cleft side bone surface being more anterior relative to the non-cleft side in some patients. Soft tissue thickness was not significantly different between cleft and non-cleft side. The lateral distance from the most anterior point of the maxilla to subalare was significantly smaller on the cleft side as compared with the non-cleft side. The authors' study demonstrates that in cleft lip and palate patients midfacial anatomy with regards to maxilla projection at the pyriform aperture is very variable making accurate evaluation of the bone morphology and an individualized treatment plan a prerequisite for successful correction of nasomaxillary deformities.

Abstract:This study aimed to assess and quantify the morphologic characteristics of the lips and the lower third of the face in cleft and noncleft patients, utilizing three-dimensional (3D) stereophotogrammetry. Sixty patients were included in the study, comprising 30 unilateral cleft lip and palate patients (G1, 24 female, 6 male; aged 20 to 60 y, mean age 44.0±12.0 y) and 30 noncleft patients (G2, 23 female, 7 male; aged 20 to 59 y, mean age 43.5±12.0 y). Anthropometric landmarks were identified on the facial surface. Three-dimensional stereophotogrammetry was employed to capture images. Statistical analysis was conducted to compare the groups, with a significance level set at 0.05. The comparative analysis revealed statistically significant differences in 5 linear and 6 angular measurements. Linear measurements such as philtrum width, upper and lower cutaneous lip height, mandibular ramus length, and midfacial depth exhibited significant differences between cleft and noncleft patients. Similarly, angular measurements, including upper lip angle, Cupid’s bow angle, lower/medium face convexity, lip seal, nasolabial angle, and left gonial angle, displayed statistically significant disparities. These findings underscore the ongoing surgical challenges in the comprehensive rehabilitation of patients with clefts, highlighting the critical need for continued advancements in treatment strategies.

An Observational Study to Evaluate Association Between Velopharyngeal Anatomy and Speech Outcomes in Adult Patients With Severe Velopharyngeal Insufficiency

Abstract:Objective: By measuring velopharyngeal structure and evaluating speech intelligibility, to explore and observe the association between velopharyngeal anatomy and speech outcomes in these patients. Methods: Thirty-one adult patients with velopharyngeal insufficiency after the primary palatoplasty aged 18 to 35 years (mean 22.03 years) were enrolled as the study group. The patients had significant hypernasality and audible nasal emission. The degree of velopharyngeal closure assessed by electronic nasopharyngeal fiberoptic endoscopy was grade III. Cephalometric analysis was performed on lateral cephalograms to measure velopharyngeal structure, including hard palate length (ANS-PNS), velar length (PNS-U), pharyngeal depth (PNS-PPW), and oropharyngeal airway space (U-MPW). Their speech intelligibility was evaluated through the Mandarin Chinese speech intelligibility test, and each speech sample was examined by 2 speech and language pathologists. The results were assessed with the SPSS 23.0 software package, and regression analysis was used to examine the relationship between velopharyngeal structure and speech outcomes. Results: A significant negative correlation was confirmed between speech intelligibility and pharyngeal depth. Pharyngeal depth also showed a linear relationship with speech intelligibility, and there was no significant correlation between speech intelligibility and other measures (hard palate length, velar length, oropharyngeal airway space). Conclusions: In the velopharyngeal anatomy, only pharyngeal depth was associated with speech intelligibility in adult patients with severe velopharyngeal insufficiency, this is consistent with our clinical observation. It suggests that appropriate reduction of pharyngeal depth during palatopharyngoplasty may have a good effect on the speech recovery in patients with cleft palate and patients with velopharyngeal insufficiency after palatorrhaphy.
