Abstract:The American Journal of Sports Medicine, Ahead of Print. Background:Softball pitchers frequently pitch at high volumes. Previous research has demonstrated changes in mechanics, range of motion, and perceived levels of fatigue and pain at high workloads. To date, little research has assessed changes in kinetics at high workloads across pitch types to understand the injury risk.Purpose:To examine changes in peak kinetics of the shoulder, elbow, and wrist of the pitching arm throughout a simulated game and doubleheader inning.Study Design:Descriptive laboratory study.Methods:A total of 19 high school softball pitchers (mean age, 15.1 ± 1.5 years; mean height, 1.6 ± 0.2 m; mean weight, 76.3 ± 16.9 kg) participated. Pitchers threw 4 innings of 25 randomly assigned pitches to mimic a game's high pitch count. Participants then rested for 30 minutes before subsequently proceeding to pitch the first inning of a doubleheader. Each pitcher threw a fastball, drop ball, curveball, and changeup. Peak shoulder, elbow, and wrist kinetics were compared across the first, last, and doubleheader innings and pitch types.Results:Compared with the first inning, significant decreases in kinetics were observed at the shoulder, elbow, and wrist in the last (P < .016) and doubleheader (P < .016) innings, particularly for shoulder and elbow compression force during the drop ball and curveball pitch types and wrist net force for all pitch types but the changeup. Significant decreases in elbow and wrist kinetics were observed during the changeup between the last and doubleheader innings (P < .016). Furthermore, differences in kinetics were observed between pitch types (P < .008); notably, the changeup had reduced kinetics compared with the fastball and breaking ball pitch types (drop ball, curveball) across innings.Conclusion:Across innings, attenuations in select joint kinetics of the pitching arm occurred that were specific to the pitch type. Across pitch types, peak kinetics was often greater during the fastball, while the changeup displayed the lowest peak kinetics.Clinical Relevance:The joints examined in this study are common sites of overuse injuries in pitchers. This work adds to previous findings on decrements in neuromuscular function as well as self-reported fatigue and pain with tournament-style pitching that may increase the injury risk. Collectively, these findings support developing a protocol combining functional testing and player-reported outcomes to aid sports specialists' decisions for pitchers to continue to pitch or return to play, which may help prevent musculoskeletal injuries and time loss from sports participation.

Abstract:The American Journal of Sports Medicine, Ahead of Print. Background:Improper sequencing order of maximal joint and segment velocities has been identified as an important predictor for both throwing arm kinetics and ball velocity.Purpose:To investigate the intrapitcher variation of maximal segment velocities and the relationship to throwing arm kinetics and ball velocity in high school (HS) and professional (PRO) pitchers.Study Design:Descriptive laboratory study.Methods:HS (n = 59) and PRO (n = 338) pitchers, instructed to throw 8 to 12 fastball pitches, were evaluated with 3-dimensional motion capture (480 Hz). Maximal joint and segment velocities were calculated for each pitch, and the standard deviation of the maxima was calculated per pitcher. These standard deviations were used to classify pitchers as "low variance" or "high variance" for each segmental velocity subgroup, "overall low variance" or "overall high variance" based on cumulative segment velocity variation, or "population," with any pitcher eligible to be included in multiple subcategories. Maximal velocities and throwing arm kinetics were compared among the various subgroups.Results:The HS low-variance shoulder internal rotation velocity subgroup (4949 ± 642 deg/s) had significantly lower maximal shoulder internal rotation velocity compared with HS population (5774 ± 1057 deg/s) (P < .001); similar findings were observed for PROs (5269 ± 835 vs 5824 ± 1076 deg/s; P < .001), as well as lower shoulder superior force compared with the PRO population (14.8% ± 8.8% vs 17.8% ± 8.8% body weight; P = .001). The PRO low-variance lead knee extension velocity subgroup had significantly lower maximal lead knee extension velocity (216 ± 135 vs 258 ± 125 deg/s; P = .001) and shoulder distractive force (111.5% ± 14.4% vs 115.6% ± 15.9% body weight; P = .003) compared with the PRO population. The PRO overall low-variance subgroup had significantly lower shoulder distractive force (111.8% ± 14.1% vs 119.6% ± 15.5% body weight; P = .008) and elbow anterior force (40.6% ± 5.0% vs 43.6% ± 6.2% body weight; P = .008) compared with the PRO overall high-variance subgroup.Conclusion:HS and PRO pitchers with low variance for joint and segment velocities achieved significantly lower maximal velocities in the subgroup of interest, while preserving ball velocity. PRO pitchers with overall low variance among multiple maximal joint and segment velocities demonstrated decreased shoulder distractive and elbow anterior force.Clinical Relevance:PRO pitchers with low intrapitch variation in maximal joint and segment velocities may be viewed as kinetically conservative throwers. These pitchers with similarly maintained mechanics between pitches may have an increasingly regimented form that preserves kinetic forces about the throwing arm. The opposite may be true for PRO pitchers with increased variability in segmental velocities during their pitching motion, as they showed increased throwing arm kinetics including shoulder distractive and elbow anterior force compared with the overall low-variance group, theoretically increasing their risk of injury.

Abstract:501 instances of surgery during 1979 in 390 patients aged 70 years or older are reported. The total preoperative mortality rate was 5.4% of operations or 6.9% of patients. Surgery in this patient group was more frequent (1979 11%, 1956/57 8.3%) while the mortality rate had decreased significantly. Urgent operations were frequent (23%), with five times the mortality rate (15%) than for elective operations (2.6%). It is therefore desirable to avoid urgent interventions and operate under elective conditions, though this is not always possible in practice.

Abstract:Twenty-two college baseball players participated in a study designed to examine the effect of upper body strength training on the velocity of a thrown baseball. The treatment group received 8 weeks of strength training while the control group received no training during the fall portion of the preseason. Throwing velocity was measured for 19 players using a radar gun. Differences in mean throwing velocity were calculated for both groups, and overall significance (p < 0.05) for the interaction of group means was found. Post hoc analysis showed a significantly higher mean throwing velocity for the training group following 8 weeks of strength training. The implication is that college baseball players can improve throwing velocity via a structured strength training program. (C) 1998 National Strength and Conditioning Association

Background:Ulnar collateral ligament (UCL) injuries in youth pitchers continue to be concerning despite the institution of pitch count limits. Flexor-pronator mass fatigue can lead to diminished dynamic stability, resulting in greater stress on the UCL.Purpose/Hypothesis:To evaluate fatigue of the flexor-pronator mass by assessing changes in medial elbow laxity; noninvasively characterizing alterations in muscle glycogen; and identifying changes in subjective fatigue, strength, range of motion (ROM), pitching velocity, and accuracy with increasing pitches thrown by youth pitchers to their recommended 75-pitch count limit. It was hypothesized that, with increased pitches, medial elbow laxity would increase and that the glycogen content of the flexor-pronator mass would decrease.Study Design:Descriptive laboratory study.Methods:Healthy male pitchers aged 10 years (n = 22) threw 3 sets of 25 pitches with 12 minutes between sets (3 timepoints). Bilateral ulnohumeral joint gapping was measured by applying a standardized valgus force and utilizing ultrasound imaging. Relative changes in muscle glycogen in the bilateral flexor carpi radialis (FCR), and the flexor digitorum superficialis/flexor carpi ulnaris (FDS/FCU) muscles were measured with ultrasound software and recorded as fuel percentiles. Additional measures obtained included subjective fatigue, strength, ROM, velocity, and accuracy.Results:There were no differences in medial elbow joint-line gapping between the throwing and nonthrowing arms or between timepoints. The throwing arm demonstrated a significant decline in fuel percentile of the FCR from baseline to after 75 pitches (P = .05). There were no differences across timepoints for FDS/FCU fuel percentile values. Fatigue measurements for both arms were significantly higher at all timepoints compared with baseline (P≤ .03). Grip strength of the dominant arm after 75 pitches was decreased significantly compared with after 25 pitches (P = .02).Conclusion:Although an increase in medial elbow joint gapping was not demonstrated within the recommended 75 pitch count limit in 10-year-olds, a relative decrease in glycogen stores of the flexor-pronator mass did occur, as well as a decrease in grip strength, with increasing subjective fatigue.Clinical Relevance:This study provides a foundation for further objective testing of physiologic changes that occur with pitching to better guide pitch count limits and improve the safety of young athletes.

Changes in Elbow Stress and Ball Velocity During Reduced Effort Pitching: A Marker-Based Motion Capture Analysis

Abstract:Background: Baseball pitchers often participate in throwing programs that involve throwing at reduced effort levels to gradually increase the amount of stress experienced across the elbow. It is currently unknown how reduced effort pitching compares with maximum effort with respect to elbow stress and ball velocity. Purpose/hypothesis: The purpose was to determine the correlation between elbow stress and ball velocity with reduced effort pitching. We hypothesized that decreased perceived effort would disproportionately correlate with elbow stress and ball velocity. Study design: Descriptive laboratory study. Methods: Ten healthy male high school baseball pitchers threw 5 pitches from a regulation pitching mound at 3 effort levels: maximum effort, 75% effort, and 50% effort. Elbow stress, specifically elbow varus torque, was calculated for all pitches using a validated marker-based 3-dimensional motion capture system. Ball velocity was measured using a Doppler radar gun. Intrathrower variability was calculated for each effort level. Results: Elbow stress and ball velocity decreased with reduced effort throws (P < .001 and P = .003, respectively). However, the reductions in elbow stress and ball velocity were not proportional. At 75% effort throws, elbow stress measured 81% (intraclass correlation coefficient [ICC], 0.95), and ball velocity measured 90% (ICC, 0.80) of maximum, respectively. At 50% effort throws, elbow stress measured 75% (ICC, 0.93), and ball velocity measured 85% (ICC, 0.87) of maximum. Intrathrower reliability was excellent for elbow stress and ball velocity, with all ICCs ≥0.80. Conclusion: Pitching at a reduced effort level resulted in decreased elbow stress and ball velocity. However, for every 25% reduction in perceived effort, elbow stress decreased by a mean 13%, and ball velocity decreased 7.5%. When baseball pitchers attempt to throw at a reduced effort of maximum, throwing metrics do not decrease proportionately. Clinical relevance: While pitching at a reduced effort of maximum decreases elbow stress and ball velocity, the decrease is not proportional, subjecting the elbow to more stress than intended. This has significant clinical importance to pitchers, coaches, and medical professionals in the setting of injury prevention and return to sports.
EFFECTS OF HIGH VOLUME UPPER EXTREMITY PLYOMETRIC TRAINING ON THROWING VELOCITY AND FUNCTIONAL STRENGTH RATIOS OF THE SHOULDER ROTATORS IN COLLEGIATE BASEBALL PLAYERS

Abstract:To achieve maximal force output, clinicians and coaches have been experimenting with upper extremity plyometric exercises for years, without sufficient scientific validation of this training method. The goal of this study was to examine the effects of an 8-week course of high volume upper extremity plyometric training on the isokinetic strength and throwing velocity of a group of intercollegiate baseball players. Twenty-four Division I collegiate baseball players (age: 19.7 +/- 1.3 years; height: 183.9 +/- 5.9 cm; mass: 90.7 +/- 10.5 kg) were recruited to participate in this study. Throwing velocity, isokinetic peak torque, isokinetic functional strength ratios, and time to peak torque were measured pre- and posttraining. Subjects were rank-ordered according to concentric internal rotation (IR) strength and were assigned randomly to either the plyometric training group (PLY) or the control group (CON). Training consisted of 6 upper extremity plyometric exercises ("Ballistic Six") performed twice per week for 8 weeks. Subjects assigned to CON performed regular off-season strength and conditioning activities, but did not perform plyometric activities. PLY demonstrated significant increases (p < 0.05) in throwing velocity following 8 weeks of training when compared with CON (83.15 mph [pre] vs. 85.15 mph [post]). There were no statistically significant differences in any of the isokinetic strength measurements between PLY and CON groups pre- to posttraining. Statistically significant differences were seen within PLY for concentric IR and eccentric external rotation (ER) isokinetic strength at 180 degrees x s(-1) and 300 degrees x s(-1); and within CON for eccentric ER isokinetic strength at 300 degrees x s(-1) and concentric IR isokinetic strength at 180 degrees x s(-1). The Ballistic Six training protocol can be a beneficial supplement to a baseball athlete's off-season conditioning by improving functional performance and strengthening the rotator cuff musculature.

The influence of a simulated game on muscular strength in female high-school and collegiate softball pitchers

Abstract:Softball pitchers often pitch several games within a day and over consecutive days during a competitive season. High volumes of pitches thrown can decrease muscular strength, resulting in less proximal force generation and upper extremity compensation to maintain performance. Therefore, the purpose of this study was to assess upper and lower extremity muscular strength after pitching in a simulated game. Fourteen softball pitchers (17.9 ± 2.3 years, 166.4 ± 8.7 cm, 72.2 ± 12.6 kg) completed baseline isokinetic strength assessment for knee, hip, trunk and pitching elbow flexion and extension as well as trunk rotation. Seven days later, participants pitched a simulated game consisting of 105 fastballs prior to repeating all strength assessments. Changes in muscular strength were assessed using paired samples t-tests, with significance set a priori as p ≤ 0.05. Normalised (%BW) stride leg knee extension peak torque was significantly higher (p = 0.020) post-simulated game (75.1 ± 24.6%BW) as compared to baseline (64.0 ± 19.5%BW) and trunk flexion peak torque was significantly higher (p = 0.009) post-simulated game (84.8 ± 47.0%BW) as compared to baseline (63.5 ± 47.1%BW). This study showed an increase in knee extension and trunk flexion strength after an acute bout of pitching. The findings give insight into muscular changes following pitching which can assist in appropriate softball training and recovery.
Effect of a 6-Week Weighted Baseball Throwing Program on Pitch Velocity, Pitching Arm Biomechanics, Passive Range of Motion, and Injury Rates

Abstract:Background: Emphasis on enhancing baseball pitch velocity has become popular, especially through weighted-ball throwing. However, little is known about the physical effects or safety of these programs. The purpose of this study was to examine the effects of training with weighted baseballs on pitch velocity, passive range of motion (PROM), muscle strength, elbow torque, and injury rates. Hypothesis: A 6-week weighted ball training program would result in a change in pitching biomechanical and physical characteristics. Study Design: Randomized controlled trial. Level of Evidence: Level 1. Methods: During the baseball offseason, 38 healthy baseball pitchers were randomized into a control group and an experimental group. Pitch velocity, shoulder and elbow PROM, shoulder strength, elbow varus torque, and shoulder internal rotation velocity were measured in both groups. The experimental group then performed a 6-week weighted ball throwing program 3 times per week using balls ranging from 2 to 32 ounces while the control group only used a 5-ounce regulation baseball. Both groups performed a strength training program. Measurements were then repeated after the 6-week period. Injuries were tracked over the 6-week training program and the subsequent baseball season. The effect of training with a weighted ball program was assessed using 2-way repeated-measures analysis of variance at an a priori significance level of P < 0.05. Results: Mean age, height, mass, and pretesting throwing velocity were 15.3 ± 1.2 years (range, 13-18 years), 1.73 ± 0.28 m, 68.3 ± 11 kg, and 30.3 ± 0.7 m/s, respectively. Pitch velocity showed a statistically significant increase (3.3%) in the experimental group ( P < 0.001). There was a statistically significant increase of 4.3° of shoulder external rotation in the experimental group. The overall injury rate was 24% in the experimental group. Four participants in the experimental group suffered elbow injuries, 2 during the training program and 2 in the season after training. No pitchers in the control group were injured at any time during the study. Conclusion: Performing a 6-week weighted ball throwing program increased pitch velocity. However, the program resulted in increased shoulder external rotation PROM and increased injury rate. Clinical Relevance: Although weighted-ball training may increase pitch velocity, caution is warranted because of the notable increase in injuries and physical changes observed in this cohort.

Background:Individual maximum joint and segment angular velocities have shown positive associations with throwing arm kinetics and ball velocity in baseball pitchers.Purpose:To observe how cumulative maximum joint and segment angular velocities, irrespective of sequence, affect ball velocity and throwing arm kinetics in high school pitchers.Study Design:Descriptive laboratory study.Methods:High school (n = 55) pitchers threw 8 to 12 fastball pitches while being evaluated with 3-dimensional motion capture (480 Hz). Maximum joint and segment angular velocities (lead knee extension, pelvis rotation, trunk rotation, shoulder internal rotation, and forearm pronation) were calculated for each pitcher. Pitchers were classified as overall fast, overall slow, or high velocity for each joint or segment velocity subcategory, or as population, with any pitcher eligible to be included in multiple subcategories. Kinematic and kinetic parameters were compared among the various subgroups using t tests with post hoc regressions and multivariable regression models created to predict throwing arm kinetics and ball velocity, respectively.Results:The lead knee extension and pelvis rotation velocity subgroups achieved significantly higher normalized elbow varus torque (P = .016) and elbow flexion torque (P = .018) compared with population, with equivalent ball velocity (P = .118). For every 1-SD increase in maximum pelvis rotation velocity (87 deg/s), the normalized elbow distractive force increased by 4.7% body weight (BW) (B = 0.054; β = 0.290; P = .013). The overall fast group was older (mean ± standard deviation, 16.9 ± 1.4 vs 15.4 ± 0.9 years; P = .007), had 8.9-mph faster ball velocity (32.7 ± 3.1 vs 28.7 ± 2.3 m/s; P = .002), and had significantly higher shoulder internal rotation torque (63.1 ± 17.4 vs 43.6 ± 12.0 Nm; P = .005), elbow varus torque (61.8 ± 16.4 vs 41.6 ± 11.4 Nm; P = .002), and elbow flexion torque (46.4 ± 12.0 vs 29.5 ± 6.8 Nm; P < .001) compared with the overall slow group. A multiregression model for ball velocity based on maximum joint and segment angular velocities and anthropometrics predicted 53.0% of variance.Conclusion:High school pitchers with higher maximum joint and segment velocities, irrespective of sequence, demonstrated older age and faster ball velocity at the cost of increased throwing shoulder and elbow kinetics.Clinical Relevance:Pitchers and coaching staff should consider this trade-off between faster ball velocity and increasing throwing arm kinetics, an established risk factor for elbow injury.

Acute Effect of Pitching on Range of Motion, Strength, and Muscle Architecture

Abstract:Background: Acute adaptations in clinical measures of range of motion and strength have been found after baseball pitching; however, there is a lack of research concerning the physiological mechanism responsible for these changes. Adaptations in muscle architecture of the infraspinatus and teres minor may serve as the structural changes responsible for these clinical measure changes. Purpose: To longitudinally assess the acute changes in range of motion, strength, and muscle architecture of the infraspinatus and teres minor muscles in baseball pitchers after a simulated baseball game. Additionally, we examined the relationship between muscle architecture and changes in clinical measures of range of motion and strength. Study design: Controlled laboratory study. Methods: Ten healthy nonvarsity collegiate club baseball pitchers (mean ± SD; age, 20.1 ± 1.10 years) were examined pre-pitching, immediately after pitching, and each subsequent day for 5 days after pitching in a simulated baseball game. A digital inclinometer and handheld dynamometer were used to assess range of motion and strength, respectively. Diagnostic ultrasound was used to assess pennation angle and muscle thickness of the infraspinatus and teres minor at rest and at maximal contraction. Results: Internal rotation range of motion significantly decreased immediately and did not return to baseline until 4 days after pitching (P≤ .05). External rotation strength also immediately decreased and returned on the third day after pitching (P≤ .05). Moreover, the resting pennation angle of the superficial and deep portions of the infraspinatus increased immediately after pitching, with the superficial portion returning to baseline on day 4 and the deep portion returning on day 5 (P≤ .05). Furthermore, the pennation angle changes of the infraspinatus and thickness of the teres minor were predictive of the loss of internal rotation range of motion after pitching (R2 = 0.419; P≤ .05). Conclusion: This study found diminished internal rotation range of motion and external rotation strength after pitching, with alterations in muscle architecture of the infraspinatus. The pennation angle increase in the infraspinatus at rest is indicative of increased tension in the muscle, which was found to be the underlying mechanism for the clinical loss of internal rotation range of motion. This was demonstrated by the inverse relationship between internal rotation range of motion and the pennation angle of the superficial and deep fibers of the infraspinatus. Clinical relevance: Clinicians should consider recovery time after pitching to prevent chronic losses of shoulder range of motion and strength. Identification of the underlying mechanisms of range of motion loss after pitching allows clinicians to optimize recovery strategies in baseball pitchers.

Abstract:Orthopaedic Journal of Sports Medicine, Volume 12, Issue 11, November 2024. Background:The mechanism of ulnar collateral ligament (UCL) injury during pitching is excessive elbow varus torque (EVT). The EVT–ball velocity (T-V) relationship allows concurrent assessment of player performance and UCL injury risk. Modifiable physical capacities may underlie individual variation seen in the T-V relationship.Purpose:To identify physical performance characteristics that impact the T-V relationship during pitching.Study Design:Descriptive laboratory study.Methods:A total of 87 National Collegiate Athletic Association Division I pitchers participated. Pitching collection involved measurement of EVT and ball velocity during 5 maximal effort fastballs thrown to a catcher. Physical measures collected were the following: shoulder and hip passive range of motion (ROM) and strength, shoulder rate of torque development (RTD), grip strength, and lumbopelvic stability. Physical measures were entered into univariate linear mixed models with ball velocity as a covariate to predict EVT. Variable reduction for multivariate models involved selection of physical measures based on random forest–derived variable importance and univariate relationship significance, rendering a 27-variable pool. Multivariate linear mixed models predicting EVT, adjusting for physical measures and other physical characteristics, were then created using backward elimination.Results:In univariate analysis, for every 1 m/s (2.2 mph) increase in ball velocity, the mean EVT increased by 1.51 Nċm (95% CI, 0.66-2.37 Nċm; P = .001). In univariate analysis, hip abduction strength symmetry and bilateral lumbopelvic stability significantly increased EVT, while dominant-shoulder ROM, scaption RTD symmetry, and hip ROM significantly decreased EVT. Variables that increased EVT while controlling for ball velocity in the final model include grip strength symmetry, lead-leg lumbopelvic stability, and bodyweight. Increased dominant-shoulder internal rotation (IR) strength, dominant-shoulder flexion ROM, and scaption strength asymmetry decreased EVT as ball velocity increased.Conclusion:Several modifiable physical measures affected EVT in the univariate analysis. In our final model, when controlling for ball velocity, EVT increased with increased grip strength symmetry, lead-leg lumbopelvic stability, and bodyweight and decreased with increased dominant-shoulder IR strength, dominant-shoulder flexion ROM, and scaption strength asymmetry.Clinical Relevance:Defining the individual and multivariate effects of these physical capacities on EVT contextualizes their role in the T-V relationship and helps identify access points through which coaches and clinicians can optimize a pitcher's T-V relationship.

J Strength Cond Res 37(6): 1327-1338, 2023-Whole-body kinematics and kinetics impact pitching performance, and when coordination of momentum transfer is impacted, throwing-related injury risks increase. Although correlations between overhead throwing velocity and lower-body power measured by jump tests have been reported by previous studies, further research is needed to identify these relationships to better understand pitching mechanics and the validity and application of jump tests for the assessment of baseball pitchers. This review communicates effective whole-body pitching mechanics, including throwing arm, trunk, and pelvis, and lower body, as well as discusses the applicability of strength and power field tests to deepen training insights in establishing more efficient motor patterns. If both lower-body power and coordination of momentum transfer are optimized, baseball pitchers may experience reduced throwing arm stress. The overarching insight to be gained from this review article is that high performance in baseball pitching is multifactorial, and attention to both strength and skill coaching is paramount. As a result, the intersection of lower-body power testing and biomechanical analysis can play an important role in uniting strength and conditioning professionals, clinicians, pitching coaches, and biomechanics experts to advance pitching performance and health in the sport of baseball.
Baseball Pitchers’ Perceived Effort Does Not Match Actual Measured Effort During a Structured Long-Toss Throwing Program

Abstract:Background: During rehabilitation throwing programs, baseball players are commonly asked to throw at reduced levels of effort (ie, 50% effort, 75% effort, etc) to moderate stress to healing tissues. It is currently unknown how changes in players’ perceived exertion compares with changes in actual exertion during structured long-toss programs. Purpose: To determine whether decreased effort correlates with decreased throwing metrics, whether metrics decrease proportionally with reductions in perceived effort, and to quantify intrathrower variability. Study Design: Descriptive laboratory study. Methods: Sixty male high school and collegiate baseball pitchers participated in a structured throwing program. A motusBASEBALL sleeve was worn by all players, which measured elbow varus torque, arm velocity, arm slot, and shoulder rotation. Ball velocity was measured with a radar gun. Each pitcher threw 5 throws a distance of 120 ft with 3 efforts: maximum effort, 75% effort, and 50% effort. Throwing metrics were compared among the 3 levels of effort to see if each 25% decrease resulted in proportional decreases in elbow varus torque and ball velocity. Intrathrower variability was determined for each throwing metric at each degree of effort. Results: All throwing metrics decreased as players decreased their perceived effort ( P 0.75). Conclusion: For every 25% decrease in perceived effort, elbow varus torque only decreased 7% and velocity only decreased 11%. Thus, when players throw at what they perceive to be reduced effort, their actual throwing metrics do not decrease at the same rate as their perceived exertion. Clinical Relevance: Measured effort decreased with decreasing perceived effort, but these were not proportional. This has significant implications for physical therapists, physicians, trainers, coaches, and athletes to understand and monitor elbow stress during the rehabilitation process.

Background:Increases in peak shoulder distraction force and peak elbow valgus torque may influence throwing-arm pain and injury risk in youth pitchers.Purpose/Hypothesis:The purpose of this study was to determine whether shoulder distraction force and elbow valgus torque while accounting for anthropometrics and pitch velocity can predict the presence of pain in youth baseball pitchers. It was hypothesized that throwing-arm pain could be predicted using the concept of biomechanical efficiency, where a pitcher who is less efficient (having higher force or torque with the same pitch velocity) is more likely to experience pain.Study Design:Descriptive laboratory study.Methods:A total of 38 youth baseball pitchers (mean age, 13.3 ± 1.7 years) were divided into a pain group and pain-free group based on presence of throwing-arm pain as reported on a health history questionnaire. Each pitcher threw 3 maximal-effort fastballs to a catcher at regulation distance, and kinematics were measured using an electromagnetic motion-capture system (minimum 100 Hz). Height and weight as well as mean peak shoulder distraction force, peak elbow valgus torque, and pitch velocity across the 3 trials were evaluated. Logistic regression analyses determined whether shoulder distraction force or elbow valgus torque could predict the presence of throwing-arm pain when holding anthropometrics (body weight for shoulder distraction force; body weight and height for elbow valgus torque) and pitch velocity constant.Results:Shoulder distraction force significantly predicted the presence of throwing-arm pain after accounting for body weight and pitch velocity (χ2 = 9.49; P = .023). Specifically, for every 1-N increase in peak shoulder distraction force while holding body weight and pitch velocity constant, there was a 0.6% increased likelihood of experiencing throwing-arm pain. Elbow valgus torque could not predict the presence of pain when holding body weight, height, and pitch velocity constant.Conclusion:The models demonstrated that increases in peak shoulder distraction forces when holding pitch velocity and body weight constant increased a youth baseball pitcher's likelihood of experiencing throwing-arm pain. The study results support the concept of the biomechanical efficiency framework by offering evidence that maintaining pitch velocity with a lower joint load led to a lower likelihood of pain.Clinical Relevance:Results suggest that practicing efficient movement strategies can decrease the likelihood of experiencing throwing-arm pain while maintaining performance.

Abstract:Background: The posterior shoulder muscles play key roles in maintaining shoulder function in throwing. Arm fatigue has been identified as a risk factor for shoulder and elbow pain in youth baseball pitchers. However, endurance of the posterior shoulder muscles in overhead athletes is not routinely examined or conditioned. Hypothesis: Upper extremity muscular endurance can be improved in adolescent baseball players during a 20-week preseason training program. Secondarily, strength will be improved. Finally, these improvements will be associated with maintenance of range of motion. Study Design: Cohort study. Methods: Fourteen baseball players (age, 16 ± 2 years) attended 3 supervised training sessions per week for 20 weeks. Strengthening of the upper extremity was performed with a specific progression that utilized readily available equipment. Testing was completed at baseline and at 4, 8, and 20 weeks. The posterior shoulder endurance test was performed to assess muscular endurance. Glenohumeral internal and external rotation range of motion and strength were measured. Results: Posterior shoulder endurance improved from 30 ± 14 repetitions at baseline to 66 ± 26 at 4 weeks and 88 ± 36 at 20 weeks ( P < 0.05). Glenohumeral internal rotation range of motion and the glenohumeral internal/external rotation strength ratio remained similar over the course of the program. Conclusion: Implementation of a preseason training program effectively increased shoulder muscular endurance while maintaining strength ratios and range of motion throughout the 20-week program. Clinical Relevance: This program improved a key parameter known to be associated with shoulder function and injury risk. This study describes a simple clinical tool to assess muscular endurance of the posterior shoulder.

Abstract:The strength of the shoulder musculature involved with internal rotation and arm extension plays an important role in the overhead throwing motion for baseball athletes, both for throwing-related performance and injury risk. The maintenance of shoulder strength is a high priority for baseball athletes throughout a season; however, little is known in regards to the expected changes in strength throughout a season. To examine pre-post changes in shoulder strength, lower body power, and body composition among collegiate baseball players after the completion of a summer baseball league season. Amateur baseball players (n = 12; age: 20.9 ± 1.0 years.; height: 181.6 ± 5.6 cm; body mass: 86.4 ± 11.1 kg; BMI: 26.0 ± 2.6 kg/m2) participated in the current study. Pre- and post-competitive season, the participants completed shoulder strength assessments and body composition and countermovement vertical jump (CMJ) tests. An upper-body isometric test (athletic shoulder [ASH] test) was used to evaluate shoulder strength for each arm. Each subject completed maximal isometric contractions for both the throwing and non-throwing arms at four separate angles of abduction (180°, 'I'; 135°, 'Y'; 90°, 'T'; and -180°, 'A') while lying in a prone position. For shoulder strength, the primary dependent variable of interest was a composite measure that represented the average of the forces produced across all four positions of the ASH test (I, Y, T, A). For the ASH test composite measure, there was a trend toward a significant arm-by-time interaction effect (p = 0.08), as shoulder strength decreased by 9.03% for the throwing arm (ES = 0.72; 95% CI = [-0.27, -0.01]), compared to only 2.03% for the non-throwing arm (ES = 0.15; 95% CI = [-0.16, 0.09]), over the course of the season. The main effects of time (p = 0.16) and arm (p = 0.58) were not significant for the ASH test composite measure. There was no relationship between lower body power and throwing arm strength at baseline (r = 0.20, p = 0.56), and only a non-significant weak relationship at post-test (r = 0.28, p = 0.41). Throughout a season, baseball players may experience reductions in shoulder strength of the throwing arm with minimal changes in shoulder strength in the non-throwing arm.

Abstract:Background: The posterior scapular muscles eccentrically contract to disperse the high forces observed in the deceleration phase of pitching. Muscular adaptations often occur following chronic eccentric loading, however, no study has evaluated the adaptations of the posterior scapular muscles with regard to throwing and their relationship with humeral retroversion (HR) in professional pitchers. Hypothesis: Significant chronic adaptations in muscle thickness (MT) and strength of the trapezius and rhomboids would be observed in healthy professional baseball pitchers, and there would be a significant relationship between humeral adaptations (ie, HR) and posterior scapular muscle adaptations (ie, strength and MT). Study design: Cross-sectional; Level 3. Methods: A total of 28 healthy male professional baseball pitchers (age, 22 ± 2 years; mass, 95 ± 17 kg; height, 190 ± 7 cm) were included in the study. Bilateral isometric muscle strength of the upper trapezius (UT), middle trapezius, lower trapezius (LT), and rhomboids was measured during a maximum voluntary isometric contraction. Diagnostic ultrasound images of the UT, middle trapezius, LT, rhomboid major, and rhomboid minor muscles were collected bilaterally to measure MT. HR was also quantified bilaterally with ultrasound. Paired sample t tests were used to compare dominant and nondominant strength and MT. Pearson correlation coefficients were used to assess the relationship between HR, isometric strength, and MT. Results: A significantly increased MT of the LT was found on the dominant arm compared with the nondominant arm (5.4 ± 1.1 mm vs 4.4 ± 1.5 mm; P = 0.00). The Pearson correlation coefficient demonstrated a significant weak negative relationship between HR and rhomboid major MT (P = 0.03; R = -0.36), and a significant weak negative correlation between HR and middle trapezius isometric strength (P = 0.03; R = -0.37). Conclusion: LT thickness was greater in the throwing arm compared with the nonthrowing arm of pitchers, suggesting a positive adaptation of the LT. Interestingly, there was a weak negative relationship between HR and both rhomboid major MT and middle trapezius isometric strength. This negative relationship suggests that since increased HR leads to decreased internal rotation range of motion during deceleration, the scapula may be forced into anterior tilt and protraction, which can place excessive eccentric load on the rhomboid major and middle trapezius.
