Abstract:Analysis of volatile organic compounds (VOCs) in breath holds great promise for noninvasive diagnostic applications. However, concentrations of VOCs in breath may change quickly, and actual and previous uptakes of exogenous substances, especially in the clinical environment, represent crucial issues. We therefore adapted proton-transfer-reaction-time-of-flight-mass spectrometry for real time breath analysis in the clinical environment. For reasons of medical safety, a 6 m long heated silcosteel transfer line connected to a sterile mouth piece was used for breath sampling from spontaneously breathing volunteers and mechanically ventilated patients. A time resolution of 200 ms was applied. Breath from mechanically ventilated patients was analyzed immediately after cardiac surgery. Breath from 32 members of staff was analyzed in the post anesthetic care unit (PACU). In parallel, room air was measured continuously over 7 days. Detection limits for breath-resolved real time measurements were in the high pptV/low ppbV range. Assignment of signals to alveolar or inspiratory phases was done automatically by a matlab-based algorithm. Quickly and abruptly occurring changes of patients' clinical status could be monitored in terms of breath-to-breath variations of VOC (e.g. isoprene) concentrations. In the PACU, room air concentrations mirrored occupancy. Exhaled concentrations of sevoflurane strongly depended on background concentrations in all participants. In combination with an optimized inlet system, the high time and mass resolution of PTR-ToF-MS provides optimal conditions to trace quick changes of breath VOC profiles and to assess effects from the clinical environment.

Ziel der vorliegenden Studie war es, mithilfe von strukturierter Befunderhebung (" structured reporting ", SR) die Qualität jener Dokumentation und den Therapieentscheidungsprozess effizienter zu gestalten. Abstract:New 'omics'-technologies have the potential to better define airway disease in terms of pathophysiological and clinical phenotyping. The integration of electronic nose (eNose) technology with existing diagnostic tests, such as routine spirometry, can bring this technology to 'point-of-care'. We aimed to determine and optimize the technical performance and diagnostic accuracy of exhaled breath analysis linked to routine spirometry. Exhaled breath was collected in triplicate in healthy subjects by an eNose (SpiroNose) based on five identical metal oxide semiconductor sensor arrays (three arrays monitoring exhaled breath and two reference arrays monitoring ambient air) at the rear end of a pneumotachograph. First, the influence of flow, volume, humidity, temperature, environment, etc, was assessed. Secondly, a two-centre case-control study was performed using diagnostic and monitoring visits in day-to-day clinical care in patients with a (differential) diagnosis of asthma, chronic obstructive pulmonary disease (COPD) or lung cancer. Breathprint analysis involved signal processing, environment correction based on alveolar gradients and statistics based on principal component (PC) analysis, followed by discriminant analysis (Matlab2014/SPSS20). Expiratory flow showed a significant linear correlation with raw sensor deflections (R(2) = 0.84) in 60 healthy subjects (age 43 ± 11 years). No correlation was found between sensor readings and exhaled volume, humidity and temperature. Exhaled data after environment correction were highly reproducible for each sensor array (Cohen's Kappa 0.81-0.94). Thirty-seven asthmatics (41 ± 14.2 years), 31 COPD patients (66 ± 8.4 years), 31 lung cancer patients (63 ± 10.8 years) and 45 healthy controls (41 ± 12.5 years) entered the cross-sectional study. SpiroNose could adequately distinguish between controls, asthma, COPD and lung cancer patients with cross-validation values ranging between 78-88%. We have developed a standardized way to integrate eNose technology with spirometry. Signal processing techniques and environmental background correction ensured that the multiple sensor arrays within the SpiroNose provided repeatable and interchangeable results. SpiroNose discriminated controls and patients with asthma, COPD and lung cancer with promising accuracy, paving the route towards point-of-care exhaled breath diagnostics.

Abstract:Ziel der Studie In Deutschland besteht ein Bedarf an Interventionen zur Tabakentwöhnung, die niedrigschwellig verfügbar sind. Eine Lösung hierfür sind eHealth-Angebote; Forschung zu Benutzereindrücken und -zufriedenheit hinsichtlich dieser Interventionen ist allerdings selten. Die vorliegende Studie gewinnt detaillierte Erkenntnisse im Hinblick auf die Nutzererfahrungen von Raucher*innen mit dem onlinebasierten "TK-NichtraucherCoaching". Methodik 15 Nutzer*innen eines onlinebasierten, individualisierbaren und interaktiven Nichtraucher-Coachings nahmen an semistrukturierten Telefoninterviews teil. Die Gespräche wurden zur Erstellung eines Codesystems mittels einer deduktiv-induktiven qualitativen Inhaltsanalyse nach Kuckartz ausgewertet. Ergebnisse Es wurden 20 Hauptkategorien, 51 Subkategorien und 13 Sub-Subkategorien gebildet. Das finale Codesystem umfasste 199 positiv und 90 negativ bewertende Aussagen. Besonders die emotionale Unterstützung und die Motivierung durch das Programm, die Niederschwelligkeit des Angebots, die optische Gestaltung, die Inhalte der Vorbereitungsphase sowie die Erfolgsübersicht der zweiten Programmphase wurden von den Nutzer*innen gelobt. Gemischte Ergebnisse ergaben sich bezüglich der Individualisierungsmöglichkeiten des Programms, der Nützlichkeit und des Umfangs der vermittelten Informationen, der Unterstützung bei Entzugssymptomen und Rückfällen, der Bedienung und der technischen Umsetzung sowie der optionalen telefonischen Beratung. Schlussfolgerung Digitale Angebote können einer großen Zahl von Raucher*innen einen niederschwelligen Zugang zu Interventionen zur Tabakentwöhnung ermöglichen. Die hier untersuchte Intervention wurde überwiegend positiv bewertet. Durch die Erstellung eines ausführlichen Codesystems wurden umfassende Einblicke hinsichtlich Nutzereindrücken und Zufriedenheit der Teilnehmer*innen ermöglicht. Darauf basierend konnten zahlreiche Optimierungsmöglichkeiten für ein solches Angebot erarbeitet werden. Beispiele hierfür sind eine Überarbeitung der Menüführung, eine Flexibilisierung des Ablaufs, eine Umsetzung als Smartphone-App sowie die Bereitstellung von textbasierten Chats. Purpose In Germany, there is a need for low-threshold smoking cessation interventions. One solution to this are eHealth services, but research on user impressions and satisfaction with these interventions is rare. The present study aims to gain detailed insights into smokers' user experiences with the online-based "TK Smoking Cessation Coaching". Methods 15 users of an online-based, tailored and interactive non-smoking program participated in semi-structured telephone interviews. The latter were analyzed by means of a deductive-inductive qualitative content analysis according to Kuckartz in order to create a code system. Results 20 main categories, 51 subcategories and 13 sub-subcategories were created. The final code system included 199 positive and 90 critical statements. The emotional support and motivation provided by the program, the low-threshold nature of the offering, the visual design, the content of the preparation phase, and the overview of success in the second program phase were all positively evaluated. The results were more mixed with regard to the program's individualization options, the usefulness and scope of the information provided, the support in the event of withdrawal symptoms and relapses, the operation and technical implementation, and the optional telephone consultation. Abstract:Isothermal rebreathing has been proposed as an experimental technique for estimating the alveolar levels of hydrophilic volatile organic compounds (VOCs) in exhaled breath. Using the prototypic test compound acetone we demonstrate that the end-tidal breath profiles of such substances during isothermal rebreathing show characteristics that contradict the conventional pulmonary inert gas elimination theory due to Farhi. On the other hand, these profiles can reliably be captured by virtue of a previously developed mathematical model for the general exhalation kinetics of highly soluble, blood-borne VOCs, which explicitly takes into account airway gas exchange as major determinant of the observable breath output. This model allows for a mechanistic analysis of various rebreathing protocols suggested in the literature. In particular, it clarifies the discrepancies between in vitro and in vivo blood-breath ratios of hydrophilic VOCs and yields further quantitative insights into the physiological components of isothermal rebreathing.

Assessment of Inhalation Technique in Clinical and Functional Control of Asthma and Chronic Obstructive Pulmonary Disease

Abstract:Introduction: Chronic obstructive pulmonary disease and asthma affect almost 300 million individuals. Inhaled therapy is often associated with technical errors reducing efficacy and compliance. Objective: To evaluate the inhalation technique and its relation with clinical and functional control in asthma and chronic obstructive pulmonary disease. Material and methods: Analytical cross-sectional study including patients with asthma and chronic obstructive pulmonary disease treated with any type of inhaler device. Demographic data and inquiry about previous teaching of inhalation technique were collected in all participants. Inhalation technique was evaluated in: Step 1 - device activation; Step 2 - previous expiration; Step 3 - inspiration; Step 4 - end inspiratory pause. Clinical control was assessed from the questionnaires Asthma Control Test, Control of Allergic Rhinitis and Asthma Test, modified Medical Research Council and Chronic Obstructive Pulmonary Disease Assessment Test. Spirometric evaluation was performed in all participants. Results: From a total of 62 subjects, 74.19% made at least one error, mainly during step 2 (53.2%). Previous education on inhalation technique was associated with lower number of errors (p = 0.014). There was no association between number of errors and age (p = 0.321), years of diagnosis (p = 0.119) or spirometric evaluation (p > 0.05). In asthma an association was found between number of errors and Asthma Control Test (p = 0.032) and Control of Allergic Rhinitis and Asthma Test (p = 0.008). Discussion and conclusion: Teaching inhalation technique has a positive impact on its future performance. Most patients make mistakes, affecting clinical control in asthma, although in chronic obstructive pulmonary disease no relation was found. This is an ongoing work that aims to reevaluate inhalation technique after patients' education and its further impact.

Abstract:Clinical trials in COPD patients aim at achieving progress in diagnosis and treatment. Study results should be applicable to a large number of patients. However, an analysis of the methods and design of current and previous trials reveals considerable room for improvement. COPD is a complex disease with different clinical phenotypes. Genetic factors need to be evaluated systematically to allow appropriate stratification of patients. Frequently used endpoints such as the FEV1 that had previously been considered reliable have shown limitations in recent trials. Thus, researchers now aim to identify new surrogate parameters that are related to the prognosis of the disease, e. g., composite endpoints and biomarkers. Physical activity and capacity are becoming increasingly important for the evaluation of disease progression. The focus of pharmaceutical development is long acting bronchodilators and new anti-inflammatory drugs. The value of non-drug interventions will also be evaluated.

Messorte waren nicht nur das Labor, sondern auch ein Fahrzeugtunnel und eine Modellstraßenschlucht. Die Photokatalyse funktioniert zwar grundsätzlich, kann aber alleine nicht verhindern, dass die Schadstoffkonzentrationen in der Luft die Grenzwerte überschreiten.

Abstract:BackgroundAsthma patients are enrolled in multimodal pulmonary rehabilitation (PR) programs. However, available data for the effectiveness of PR in asthma are sparse. Therefore, the primary aim of this randomized control trial (RCT) is to evaluate short-term (end of rehabilitation) and intermediate-term effectiveness (3 months after rehabilitation) of PR for patients with asthma regarding asthma control (primary outcome) and other outcomes. Secondly, moderator effects of gender, age, baseline asthma control, quality of life, and anxiety will be examined. Thirdly, a longitudinal follow-up study will explore the course of the outcomes over one year and the annual costs.MethodsThe EPRA study is a single-center randomized controlled waiting-list trial in the Bad Reichenhall Clinic. Inclusion criteria include a referral diagnosis for uncontrolled asthma, no cognitive impairment and no very severe co-morbidities that indicate significantly greater illness morbidity than asthma alone. In the intervention group (IG), participants will start PR within 4 weeks after randomization; participants of the control group (CG) will start PR 20 weeks after randomization. Data will be assessed at randomization (T0), after 4 weeks (T1; IG: begin of PR), 7 weeks (T2; IG: end of PR), and 20 weeks (T3, CG: begin of PR). The primary outcome is asthma control at T2/T3. Secondary outcomes are health-related quality of life, functional exercise capacity, dyspnea, anxiety, depression, subjective self-management skills, illness perceptions, sick leave and subjective work ability. Outcomes will be analyzed with analysis of covariance, including baseline values of the respective outcomes as covariates. Healthcare costs will be analyzed with a gamma model with a log-link.A longitudinal follow-up study will generate additional data at 3/6/9/12 months after PR for both IG and CG. Latent change models will be used to analyze the course of the primary and secondary outcomes. Annual cost differences before and after rehabilitation will be compared by paired t-test.DiscussionThis RCT will determine the effectiveness of a complex inpatient PR for asthma patients concerning asthma control. Furthermore, important medical and economic information regarding the effectiveness of PR as part of the long-term management of patients with uncontrolled asthma will be generated.Trial registrationGerman Clinical Trials Register (DRKS00007740, May 15, 2015). Abstract:Hintergrund Reha-Zufriedenheit und Behandlungserfolg sind im Qualitätssicherungsprogramm der Deutschen Rentenversicherung als Qualitätskriterien für die medizinische Rehabilitation fest verankert. Im Bereich der Psychosomatischen Rehabilitation gibt es bislang kaum Arbeiten, in denen der relative Stellenwert von Behandlungsvariablen und von Hotel- und Serviceleistungen für die Vorhersagbarkeit dieser Kriterien überprüft wurde. Methodik In einer Sekundäranalyse wurden anonyme Befragungsdaten aus 25 psychosomatischen Kliniken/Fachabteilungen analysiert. Die Stichprobe umfasst etwa 50 000 stationäre Rehabilitand*innen der Jahre 2018–2022. Die Befragungen fanden zum Zweck der internen Qualitätssicherung mit einem weitgehend einheitlichen Fragebogen statt. Regressionsanalytisch überprüft wurde, in welchem Maße die beiden Ergebniskriterien aus 14 Prädiktoren (Bewertungen von Teilleistungsbereichen) und aus einem heuristischen Modell mit drei Variablenblöcken (KV=Kontrollvariablen, RB=Reha-Behandlung, HL=Hotelleistungen) vorhergesagt werden können. Ergebnisse Alle 14 Prädiktoren korrelieren signifikant mit beiden Kriterien und können etwa 67% (Kriterium Reha-Zufriedenheit) bzw. 40% (Kriterium Behandlungserfolg) erklären. In schrittweisen Regressionen tragen die Prädiktoren "Psychotherapeutische Betreuung" und "Nachsorgevorbereitung" am meisten zur Vorhersage der Kriterien bei. Hierarchische Regressionen zeigen, dass der inkrementelle Anteil des RB-Blocks an der Vorhersage beider Kriterien, also derjenige Anteil, der mit Sicherheit den Reha-Behandlungsvariablen zugeschrieben werden kann, um ein Vielfaches größer ist als derjenige der Hotelleistungen. Diskussion Die Daten weisen darauf hin, dass eine als hochwertig erlebte psychotherapeutische Betreuung und eine gute Nachsorgevorbereitung bezüglich der selbstberichteten Reha-Zufriedenheit und dem subjektiven Behandlungserfolg am wichtigsten sind. Die hierarchischen Regressionsanalysen stützen frühere Befunde, dass Merkmale der eigentlichen rehabilitativen Versorgung für die Rehabilitand*innen hinsichtlich beider Ergebniskriterien eine wesentlich größere Rolle spielen als Hotel- und Serviceleistungen. Es spricht einiges dafür, dass diese Befundlage auch für die psychosomatische Krankenhausbehandlung gültig ist. Background Rehab satisfaction and treatment success are firmly anchored in the German Pension Insurance's quality assurance program as quality criteria for medical rehab. In the field of psychosomatic rehab, there are hardly any studies in which the relative importance of treatment- and hotel-related characteristics for the prediction of these criteria has been investigated. Methods In a secondary analysis, anonymous survey data from 25 psychosomatic rehab clinics/specialist departments were analyzed. The sample comprises around 50,000 inpatient rehabilitants from 2018–2022. The surveys were conducted using a largely standardized questionnaire for the purpose of internal quality assurance. The extent to which the two outcome criteria can be predicted from 14 predictors (assessments of partial performance areas) and from a heuristic model with three blocks of variables (KV=control variables, RB=rehabilitation treatment, HL=hotel services) was examined using regression analysis. Results All 14 predictors correlate significantly with both criteria and can explain a total of around 67% (rehab satisfaction) resp. 40% (treatment success). In stepwise regressions, the predictors "psychotherapeutic care" and "preparing for the time after rehab" contribute most to the prediction of the criteria. Abstract:Hintergrund Verlegungen der oberen Atemwege stellen akute Notfälle dar. Ultima Ratio zur Sicherung der Atemwege ist die Durchführung einer Koniotomie nach Identifizierung des Lig. conicum. Hier kann die Sonografie unterstützen. Ziel dieser Studie war die Etablierung eines Trainings zur Vermittlung dieser Kompetenzen. Material Methoden Das Training bestand aus einer theoretischen Schulung mit anschließender Videopräsentation (je 10min) sowie einem praktischen Training (45min). Zur Messung der Vorerfahrungen, der Zufriedenheit mit dem Training sowie des subjektiven und objektiven Kompetenzlevels wurden vor (T1) und nach (T2) dem Training Evaluationen bearbeitet. Zu T2 wurde zudem eine praktische Prüfung von n=113 Teilnehmenden (TN) absolviert. Die Ergebnisse der praktischen Prüfung wurden durch einen standardisierten Bogen dokumentiert. Ergebnisse Der größte Teil der TN hatte weder eine Koniotomie gesehen (64,6%) noch eigenständig durchgeführt (79,6%). In der subjektiven Kompetenzeinschätzung konnte eine signifikante Verbesserung (T1 zu T2) gemessen werden (p<0,001). Das Trainingsprogramm wurde in allen abgefragten Items positiv bewertet. In den praktischen Prüfungen erreichten die TN durchschnittlich 89,2% der möglichen Punkte und benötigten für die Identifizierung des Lig. conicum im Mittelwert 101±23 Sekunden. Schlussfolgerung Strukturiertes Training zur sonografischen Identifizierung des Lig. conicum führt bereits in kurzer Zeit zu einer deutlichen Verbesserung der subjektiven Kompetenzen sowie zu einem hohen objektiven Kompetenzlevel. Zukünftig sollten solche Trainingsprogramme standardisiert in der Kopf-Hals-Ultraschallausbildung etabliert werden. Objective Upper airway obstructions are usually acute emergencies. Coniotomy is the last option to secure the airway and can be supported by sonography. The aim of this study was to establish a training program to teach these skills. Material and methods The training consisted of theoretical training with an additional video presentation (10 minutes each) and practical training (45 minutes). Evaluations were completed before (T1) and after (T2) the training to measure prior experience and satisfaction with the training as well as subjective and objective competence levels. At T2, a practical test was also completed by n=113 participants. A standardized evaluation form was used to document the results of the practical test. Results A large proportion of the participants had neither seen a coniotomy (64.6%) nor performed one independently (79.6%). Significant improvement (T1 to T2) was measured with regard to the subjective assessment of competence (p<0.001). The training received positive ratings for all items tested (scale ranges 1–2). During practical tests, the participants achieved an average of 89.2% of the possible points and needed a mean of 101 ±23 seconds to identify the conic ligament. Conclusion Structured training for sonographic identification of the conic ligament leads to significant improvement in the subjective assessment of competence and a high objective competence level in a short period of time. This type of training should be standardized in head and neck ultrasound training in the future. Abstract:Background: Early detection of respiratory overload is crucial to mechanically ventilated patients, especially during phases of spontaneous breathing. Although a diversity of methods and indices has been established, there is no highly specific approach to predict respiratory failure. This study aimed to evaluate acceleration sensors in abdominal and thoracic wall positions to detect alterations in breathing excursions in a setting of gradual increasing airway resistance. Methods: Twenty-nine healthy volunteers were committed to a standardized protocol of a two-minutes step-down spontaneous breathing on a 5 mm, 4 mm and then 3 mm orally placed endotracheal tube. Accelerator sensors in thoracic and abdominal wall position monitored breathing excursions. 15 participants passed the breathing protocol ("completed" group), 14 individuals cancelled the protocol due to subjective intolerance to the increasing airway resistance ("abandoned" group). Results: Gradual increased respiratory workload led to a significant decrease of acceleration in abdominal wall position in the "abandoned" group compared to the "completed" group (p < 0.001), while these gradual accelerating changes were not observed in thoracic wall position (p = 0.484). Thoracic acceleration sensors did not detect any time- and group-specific changes (p = 0.746). Conclusions: The abdominal wall position of the acceleration sensors may be a non-invasive, economical and practical approach to detect early breathing alterations prior to respiratory failure. Abstract:Ziel der Studie Als dysfunktionale Atemmuster (DAM) werden Abweichungen vom physiologischen Atemmuster bezeichnet. DAM scheinen mit einer geringeren Asthmakontrolle verbunden zu sein. Bisher ist unklar, welchen Einfluss eine stationäre Rehabilitation auf diese Problematik nehmen kann. Ziel dieser Arbeit ist es, den Effekt einer pneumologischen Rehabilitation (PR) auf DAM zu untersuchen. Methodik Die Daten basieren auf einer randomisierten kontrollieren Studie mit Wartekontrollgruppe. Die Interventionsgruppe (IG) erhielt 4 Wochen nach Antragsbewilligung eine PR, die Kontrollgruppe (KG) nach 5 Monaten. Dysfunktionale Atmung wurde mittels Nijmegen-Questionnaire (NQ) erfasst. Werte ≥ 23 Punkte weisen auf ein existentes DAM hin. Verglichen werden Werte zu Reha-Ende (T2) und nach drei Monaten (T3) (Kovarianzanalyse). Ergänzend wurde eine Moderatoranalyse durchgeführt, um zu prüfen, ob der Effekt der PR mit den Ausgangswerten des NQ-Score zusammenhängt. Ergebnisse Zwischen IG (n=202) und KG (n=210) finden sich signifikante Unterschiede im NQ-Score zu T2 (AMD=10,5; 95%CI [9; 12]; d=1,4; p<0,001) und zu T3 (AMD=5,8; 95%CI [4,3; 7,3]; d=0,8; p<0,001). Es zeigt sich ein Interaktionseffekt zwischen der Differenz im NQ-Score der Gruppen zu T2 und dem Ausgangswert zu T0 (b=5,6; 95%CI [2,2; 11,9]; p<0,001). Zu T3 ist dieser Interaktionseffekt nicht mehr nachweisbar (b=4,5; 95%CI [−3,1; 14,1]; p=807). Schlussfolgerung Die stationäre, multimodale und interdisziplinäre PR geht sowohl zum Entlassungszeitpunkt als auch 3 Monate danach mit einer signifikanten und klinisch relevanten Verbesserung der DAM einher. Kurzfristig profitieren Patienten mit bestehenden DAM mehr von der PR als Patienten ohne DAM. Purpose Dysfunctional breathing patterns (DAM) are deviations from physiologic breathing patterns. DAM seem to be associated with lower asthma control. To date, it is unclear what effect inpatient rehabilitation can have on this problem. The aim of this work is to investigate the effect of pulmonary rehabilitation (PR) on DAM. Methods The data are based on a randomized controlled trial with a waiting control group. The intervention group (IG) received PR 4 weeks after application approval and the control group (KG) after 5 months. Dysfunctional breathing was assessed by Nijmegen-Questionnaire (NQ). Values ≥ 23 points indicate an existing DAM. Values at the end of rehabilitation (T2) and after three months (T3) were compared (analysis of covariance). Supplemental moderator analysis was performed to examine whether the effect of PR was related to baseline NQ scores. Results Significant differences in NQ score are found between IG (n=202) and KG (n=210) at T2 (AMD=10.5; 95%CI [9; 12]; d=1.4; p<0.001) and at T3 (AMD=5.8; 95%CI [4.3; 7.3]; d=0.8; p<0.001). There is an interaction effect between the difference in NQ score between the groups at T2 and baseline at T0 (b=5.6; 95%CI [2.2; 11.9]; p<0.001). At T3, this interaction effect was no longer detectable (b=4.5; 95%CI [−3.1; 14.1]; p=807). Conclusion Inpatient, multimodality, and interdisciplinary PR is associated with significant and clinically relevant improvement in DAM both at discharge and 3 months later. Durch die Auswertung der Evaluationen von Lehrsektionen für Fachhochschulen der Polizei an verschiedenen rechtsmedizinischen Standorten sollte auch geprüft werden, ob abweichende Konzeptionen zu Unterschieden in der Wahrnehmung und Bewertung bei den Teilnehmern führen.

The impact of a dedicated training program for oral examiners at a medical school in Germany: a survey among participants from operative and non-operative disciplines

Abstract:Background: Oral examinations have been a crucial format in ancient and modern assessment to evaluate and guarantee quality of medical education and thereby to secure patient safety. To achieve a high level of quality in the oral part of the final examination of medical students, a training program for oral examiners at the Medical Faculty of Ulm (Germany) has been established since 2007.However, little is known about the attitude of the examiners in regard to the impact of this training program and of oral examinations as instruments to ensure patient safety. Methods: All 367 academic clinicians from operative and non-operative disciplines, attending the one-day examiner training program at the University of Ulm between 2007 and 2012 have been asked to answer an online survey (EvaSys 5.0). Focus of the survey was to find out in which respect the examiners profited from the trainings, if the training effects were discipline-dependent, and to which degree the oral examinations could contribute to patient safety. Statistical analysis was performed using the t-test for independent samples. Results were considered statistically significant when p < 0.05. Results: A total of 63 participants answered the survey, but in 4 cases the questionnaire was not fully completed (with single items missing). More than half of the study participants (n = 34/59; 58%) have experienced (at least sometimes or rarely) candidates that they deemed incompetent and perhaps even dangerous to the patients' health who nevertheless passed the oral exam successfully. The majority of participants were convinced that oral examinations using concrete clinical cases could significantly contribute to patient safety, if grading is based on clear criteria and if examinations as well as grading are performed more critically. The impact of the training program was rated significantly stronger by surgeons than by non-surgeons in several categories. These categories included "strengths and weaknesses of oral examinations", "reliability", "validity", "competence in grading", "critical grading", and "departmental improvements" concerning oral examinations. Conclusions: In respect to patient safety, it seems crucial to prevent incompetent candidates from passing the oral examination. The present study indicates the importance to continue and to develop our examiner trainings, with main emphasis on concrete clinical problems and a criteria-based critical grading system for oral examinations. Since the impact of the training was particularly high for colleagues from the operative disciplines, the training program should be offered especially in surgical departments.

Abstract:Der Bericht Abschatzung der grosenaufgelosten Partikelkonzentration und -zusammensetzung anhand wetterlagenorientierter experimenteller Messungen reprasentiert das REGKLAM-Produkt 2.2b. Auf der Grundlage von Experimenten wurde die heutige Situation analysiert und Szeanarien fur einen Temperaturanstiegt sowie fur eine Anderung der Anstromcharakteristik erarbeitet. Da die PM10-Massenkonzentrationen bereits heute Grenzwerte uberschreiten und im Rahmen der klimatischen Veranderung nicht mit einer wesentlichen Abnahme zu rechnen ist, bleibt es auch zukunftig eine wichtige Herausforderung, die Emissionen von Partikeln und deren Vorlaufersubstanzen sowohl in der Stadt als auch in der grosraumigen Umgebung zu vermeiden.
