Abstract:Background: Cardiovascular Disease (CVD) is considered as the main cause of death worldwide. Identifying the links among CVDs risk factors can help decrease CVD-related deaths. Aim: To assess the prevalence of risk factors for CVD and their relationships among the Tabuk City population in Saudi Arabia. Methods: A cross-sectional design was used; 432 participants in the Tabuk region were included in this study. Results: The prevalence of diabetes mellitus (DM) was 5.6%, the prevalence of hypertension (HTN) was 11.1% and obesity and overweight together were 69.9%. Mean Body Mass Index (BMI), HTN, and DM increased with age. There was a correlation between BMI with HTN (r=.200, p<.001), BMI and DM (r=.149, p<.001) and DM and HTN (r=.366, p<.001). Conclusion: Public awareness may help in reducing the prevalence of CVD.

Abstract:Background Atherosclerotic cardiovascular disease (ASCVD), heart failure (HF), chronic kidney disease (CKD), and obesity are associated with increased morbidity and mortality in patients with type 2 diabetes mellitus (T2DM). Nonetheless, their prevalence among patients with T2DM in Saudi Arabia (SA) remains unknown. As current guidelines recommend, these comorbidities require adding certain antidiabetic agents with cardiorenal benefits. However, the prescribers' adherence to these recommendations remains unclear. Methods A two-center retrospective cross-sectional study was conducted including adult patients (≥18 years) with T2DM admitted to hospital or seen at outpatient clinics between January and December 2020. Patients were classified into two groups based on the presence or absence of ASCVD. Patients with no prior ASCVD history were further classified based on the 10-year ASCVD risk estimation. Endpoints of interest included the prevalence of ASCVD, HF, CKD, and obesity in patients with T2DM. We also evaluated the characteristics of the utilized antidiabetic agents, statin, and aspirin therapies.. Results Of the 1,218 included patients with T2DM, the majority were female (57.0 %), and aged 45–64 years (53.0 %) with a mean age of 59.3 ± 13.1 years. Hypertension and dyslipidemia were the most prevalent comorbidities (67.7 % and 69.0 %, respectively). Among all patients, 18.6 % had an established ASCVD and the prevalence of HF, CKD, and obesity were 5.1 %, 8.7 %, and 58.3 %, respectively. The most common types of ASCVD witnessed were revascularization (42.3 %), myocardial infarction (36.6 %), and stroke (33.9 %); with an increased prevalence of ASCVD as the age increases (52.8 % at age ≥ 65 years). In the non‐ASCVD group, the 10-year ASCVD risk was intermediate or high in 62.7 % of these patients. The rates of utilization of guidelines-recommended therapies were 83.6 % for metformin, 9.4 % for GLP-1 RA, 10.8 % for SGLT2i, 35.2 % for aspirin alone or in combination with clopidogrel, and 79.7 % for statin therapy. Conclusions ASCVD, HF, CKD, and obesity are common complications in patients with T2DM in SA, with low overall utilization of the recommended guidelines-recommended medical therapies. Multimodal strategies should be utilized to assess T2DM and its complications, and to improve prescribers' adherence to guidelines-recommended therapies.

Abstract:Introduction & Objectives: Diabetic retinopathy(DR) is associated with significant visual impairment and quality of life in people living with diabetes, however, studies on the prevalence and risk factors from Saudi Arabia have been inconsistent, hence the aim of this was to define the prevalence, risk factors and predictors of DR in a large sample of subjects with type 1 diabetes (T1DM) and T2DM. Methods: This was a cross-sectional study that included subjects with both T2DM and T1DM (if duration > 5 years) who attended the diabetes clinics at King Fahad Medical City in Riyadh between January 2022 and December 2022. Data are presented as Mean ± SD. Results: The study sample included 1399 subjects of whom 566(40.5%) had T1DM and 833 (59.5 %) subjects had T2DM. Mean Hba1c for T1DM subjects was 8.8 ± 3.0 %, mean diabetes duration 10.2 ±5.6 years, and mean Time in Range (TIR) was 42.7 ± 18 % (N=354 subjects). Mean HbA1c for T2DM subjects was 7.8 ± 1.5 %, mean diabetes duration was 14.1 ± 8.8 years and mean TIR was 52.7 ± 25 (N=139). The prevalence of DR in the overall study population was 20.0 % (280/1399) with the majority ( 86.5 %) in the Mild and Moderate Non-Proliferative DR stages. The prevalence of DR in T1DM & T2DM subjects was 12.7 % and 24.8 %, respectively. Subjects with T1DM & DR were significantly older and had longer diabetes duration while subjects with T2DM & DR were older, had longer diabetes duration, higher Systolic BP, higher urinary albumin/Creatinine ratio( UACR) and lower estimated GFR. Logistic regression showed that in T1DM subjects, longer diabetes duration, lower TIR and higher glucose variability were independently associated while, longer diabetes duration and higher UACR were independently associated with DR in T2DM subjects. Conclusion: In a large sample of Saudi subjects with diabetes, the prevalence of DR was 20 %. Abstract:This systematic review aims to investigate the published literature on vitamin D deficiency (VDD) as a risk factor for developing type 2 diabetes mellitus (T2DM) or affecting the glycemic state of patients in Saudi Arabia. PubMed, Scopus, Web of Science, ScienceDirect, and the Cochrane Library were systematically searched to include the relevant literature. Rayyan QCRI (Rayyan Systems Inc., Cambridge, MA) was used throughout this systematic approach. Eleven studies were included with a total of 4229 patients. Three studies demonstrated that VDD was a significant risk factor for developing T2DM, and one reported that it increased insulin resistance. However, two studies found that VDD did not affect the incidence of T2DM and did not affect the insulin sensitivity or glycated hemoglobin (HbA1c) levels in patients with T2DM. This systematic review demonstrated that VDD significantly increases the risk of developing T2DM and negatively affects the glycemic state of patients with T2DM among Saudi patients. Due to the many populations examined, vitamin D chemical compositions, doses, and supplementation periods, interventional research has produced contradictory and ambiguous results. Additional research is necessary, particularly in individuals with a high risk of developing diabetes (impaired fasting glucose and/or glucose tolerance, possibly without obesity). These individuals may be the primary benefactors of vitamin D's benefits in preventing T2DM, according to the hypothesized mechanism of action for the vitamin.

Abstract:This study aimed to assess the prevalence of thyroid dysfunction, as measured by hormone levels, in Saudi women with type 2 diabetes mellitus (T2DM). The study will also assess thyroid hormones and leptin, angiopoietin like 8 (ANGPTL8), obesity, and cardiovascular diseases (CVD) in T2D patients. A total of 250 women aged 40 to 60 years with T2DM were retrospectively studied between 2021 and 2022. This research examined medical records for T2DM patients. In this investigation, no T2DM patients had thyroid autoantibodies in their medical records. These patients were chosen for their FT4 and TSH values. All participants were Saudi females with T2DM, aged 54.5 years. Of the 250 participants, 32% had hypothyroidism, 14.8% had hyperthyroidism, and 40.8% (102) had no thyroid disease. Hypothyroidism (7.8 ± 0.67 mmol/L) exhibited greater fasting blood glucose (FBG) levels than hyperthyroidism (7.1 ± 0.64 mmol/L) (P < .05). Hypothyroid and hyperthyroid females had significant differences in high density lipoprotein-cholestrol (HDL-C), triglycerides, triglyceride glucose (TyG) index, body mass index (BMI), waist circumstance (WC), high-sensitivity C-reactive protein (hs-CRP), leptin, ANGPTL8, insulin resistance (IR), and insulin levels (P < .05). Pearson's correlation test showed that T2DM patients' HDL-C levels were favorably but negatively correlated with leptin and ANGPTL8 levels. In hypothyroidism, thyroid stimulation hormone (TSH) is favorably linked with glycated hemoglobin (HbA1c), triglyscride (TG), TyG index, BMI, WC, leptin, ANGPTL8, hs-CRP, and IR. T2DM is linked to thyroid malfunction, notably hypothyroidism, which correlates positively with TSH. TSH variations due to increasing leptin, ANGPTL8, and TyG index may enhance the risk of insulin resistance diseases, such as obesity and CVD, in Saudi females with T2DM.
Depression and Anxiety Screening of Patients With Type II Diabetes Mellitus Attending Primary Healthcare Centers in Saudi Arabia

Abstract:Background: Diabetes mellitus is a chronic condition affecting both physical and mental health. High blood sugar levels can lead to serious complications if not managed properly. Individuals with diabetes are also at a higher risk of developing mental health disorders like depression and anxiety. Objective: The objective was to assess the prevalence of depression and anxiety among patients with type II diabetes attending primary healthcare centers in Makkah, Saudi Arabia. Methods: An analytical cross-sectional study design was employed, targeting adults with type II diabetes attending primary healthcare centers in Makkah. A total sample size of 412 participants was selected through systematic random sampling. Data were collected using validated Arabic versions of the patient health questionnaire (PHQ-9) and the general anxiety disorder (GAD-7) questionnaire, along with demographic information. Statistical analysis was conducted with non-parametric tests used to evaluate the significance of outcome variables. Results: The median age of participants was 47 years, with 237 (57.5%) being male and 373 (90.5%) Saudi nationals. Neuropathy was the most prevalent complication, affecting 181 (43.9%) of participants. The median GAD-7 score was 5, and the median PHQ-9 score was 7. Mild to moderate depression was observed in 224 (54.4%), and 194 (47%) had mild-to-moderate anxiety. Significant associations were found between mental health outcomes and several factors: nationality (non-Saudis showed higher depression and anxiety scores, p = 0.010 and p = 0.020, respectively), marital status (divorced participants had the highest mean ranks for both conditions, p = 0.003 for depression and p < 0.001 for anxiety), and monthly income (p = 0.007 for depression and p = 0.008 for anxiety). Additionally, the presence of diabetes complications was strongly associated with higher depression and anxiety scores (p < 0.001 for both). Conclusion: The findings indicate a high prevalence of depression and anxiety among patients with type II diabetes in Makkah. These mental health issues are significantly associated with socio-demographic factors and diabetes complications. Integrated medical and psychological care is needed to improve the quality of life and treatment outcomes for diabetic patients.
Iliopsoas Tenotomy at the Lesser Trochanter Versus at the Pelvic Brim in Ambulatory Children With Cerebral Palsy

Abstract:Background: Progressive hip flexion deformity is a common problem in ambulatory children with spastic cerebral palsy, causing static and dynamic deformity. The iliopsoas muscle is recognized as a major deforming force in the development of this problem. Many clinicians address this problem by lengthening the iliopsoas, either in an intramuscular location at the pelvic brim or by complete tenotomy at the lesser trochanter. The goal of this study was to compare the outcomes of patients with ambulatory cerebral palsy who had intramuscular lengthening at the pelvic brim to those who underwent complete release of the iliopsoas tendon at the level of the lesser trochanter. Methods: Twenty patients were included in the study, 11 of whom had iliopsoas release at the lesser trochanter (group 1) and 9 of whom had intramuscular lengthening at the pelvic brim (group 2). All patients had physical examinations, plus kinematic and kinetic analyses in our gait laboratory before and 1 year after surgery. Results: Hip flexion contracture was decreased significantly only in group 1, although there was a trend of decrease in group 2. There was a significant increase in maximum hip extension in terminal stance and a reciprocal decrease in maximum swing phase hip flexion ingroup 1, with a similar trend that did not reach significance in group2. Stride length increased significantly in both groups. There was no significant change in power generation of hip flexion during the swing phase in either group. Conclusions: We found improved static and dynamic parameters of hip extension after iliopsoas lengthening and did not detect any adverse kinematic or kinetic change in hip function after surgery. The improvement was more robust in the group who underwent release at the lesser trochanter. Because there are no adverse effects of iliopsoas release from the lesser trochanter and the improvement in hip extension is greater, this approach should be considered in ambulatory patients with spastic diplegia when a hip flexor weakening procedure is considered. Abstract:Aim: Vitamin D deficiency is linked with type 2 diabetes mellitus (T2DM) and the occurrence of complications in patients with type 2 diabetes mellitus. None of the studies have focused on the association between vitamin D levels in patients with type 2 diabetes mellitus and diabetic nephropathy (DN) in the Makkah region, Saudi Arabia. Hence, the purpose of this study is to investigate the relationship of vitamin D with kidney disease in patients with T2DM in the Makkah region, of Saudi Arabia. Materials and methods: This descriptive cross-sectional study was conducted at different hospitals in the Makkah region on T2DM patients from 2021 to 2022. In total, 328 patients with confirmed diabetes were enrolled in this study. T2DM patients over the aged>18 to 92 years were included in the study. General laboratory characteristics of the study population were measured, including fasting blood sugar, HbA1C (Glycated hemoglobin), vitamin D, kidney function (BUN-Blood urea nitrogen and creatinine), and lipid profiles (cholesterol, triglycerides, LDL-Low density lipoprotein, and HDL-High density lipoprotein). Results: 46.6% (n=153) of participants had normal serum vitamin D levels. Insufficient and deficient serum vitamin D level were observed in 43.9% (n=144) and 9.5% (n=31) of participants, respectively. Of the participants, 25.9% (n=85) had good glycemic control (<7.0%). Moderate and poor glycemic control were observed in 39.9% (n=131) and 34.1% (n=112) of the participants, respectively. A significant negative correlation (p<0.5) was found between vitamin D levels and kidney function test results (blood urea nitrogen and serum creatinine levels). An inverse relationship was observed between HbA1c levels and vitamin D deficiency. Conclusion: Nephropathy is more likely to develop in people with type 2 diabetes mellitus and vitamin D deficiency.
Knowledge, Attitude, and Practice of Diabetes Mellitus Among the Saudi Population in Riyadh, Saudi Arabia: A Quantitative Study

Abstract:Aim To investigate the knowledge, attitudes, and practices of diabetes among Saudi adults in Riyadh. Methods A questionnaire-based study was carried out in September 2019. A previously validated questionnaire was used to assess participants' knowledge. Results The study sample included 3,208 total participants. Of these, 53% were females and 47% were males. About 53.5% of the participants had good knowledge scores. The great majority of respondents did not know whether metformin could cause kidney damage (n = 2651, 82.6%) and more than half did not know whether long-term drug use could cause organ failure (n = 2073, 64.6%) and whether insulin could cause harmful effects (n = 1836, 57.2%). Results showed that 91.3% of the respondents stated that they would seek treatment if they or one of their family members got diabetes mellitus (DM). Approximately 50% of the participants (49.9%) regularly exercised. More than half (68%) of the respondents had never checked their blood glucose levels on an annual basis. More than half of the respondents tried to avoid refined sugar. Conclusion The majority of the participants had never checked their blood glucose levels. In addition, one-third of the participants believed that the use of complementary medicine could actually control diabetes.
The prevalence and determinants of diabetes mellitus and thyroid disorder comorbidity in Tabari cohort population

Abstract:Studies have shown that the co-occurrence of diabetes mellitus (DM) and thyroid dysfunction (TD) exacerbates diabetes complications and imposes a financial burden on the healthcare system. Therefore, this study aimed to investigate the prevalence of TD-DM comorbidity and its associated risk factors. This cross-sectional study was conducted on enrollment phase data of the TABARI cohort population which consisted of 10,255 adults aged between 35 to 70 years old residing in Sari, Mazandaran, Iran from 2015 to 2017. A total of 9939 out of 10,255 individuals (96.92%) entered the study. The prevalence of TD among T2DM patients was 13.2%. The prevalence of T2DM among patients with TD was 9.2%. Furthermore, the prevalence of TD-DM comorbidity in the overall population was 2.2%. Logistic regression analysis revealed that the odds of TD-DM comorbidity was significantly higher in women (OR 2.85; 95% CI 1.58-5.11), in the age group of 60-70 years (OR 9.62; 95% CI 3.69-25.10), in smokers (OR 2.32; 95% CI 1.19-4.52), in individuals with high waist circumference (WC) (OR 2.22; 95% CI 1.32-3.75), in individuals with low high-density lipoprotein (HDL) (OR 1.60; 95% CI 1.20-2.14), in individuals with high total cholesterol (TC) (OR 1.71; 95% CI 1.21-2.41), in individuals with high triglycerides (TG) (OR 1.79; 95% CI 1.27-2.51), and significantly lower in individuals with higher physical activity (PA) (OR 0.67; 95% CI 0.49-0.93). The present study demonstrated a prevalence of 2.2% in patients with both TD and T2DM. Additionally, female gender, older age, smoking, high WC, low HDL, high TC, high TG, and low PA were predictors of TD-DM comorbidity.

Abstract:Cryptococcus neoformans is a pathogenic fungus, distinguished by an elaborate polysaccharide capsule that is essential for its virulence. As part of an effort to understand the biosynthesis of this important structure, we initiated purification of an alpha-1,3-mannosyltransferase with appropriate specificity for a role in building the main capsule polysaccharide, glucuronoxylomannan. A pool of proteins that was 5,000-fold enriched in this activity included several polypeptides, which acted potentially as the catalytic protein. These were analyzed using sequence information and double-stranded RNA interference. Interference that targeted a sequence corresponding to part of a 46 kDa protein in the enriched fraction abolished the activity of interest and reduced the capsule on the affected cells. This gene was cloned and expressed in active form in Saccharomyces cerevisiae to confirm function, and was termed CMT1, for cryptococcal mannosyltransferase 1. CMT1 has no confirmed homologs in GenBank other than CAP59, a cryptococcal gene encoding a protein of unknown function that is required for capsule synthesis and virulence. The Cmt1p protein also co-purifies with a homolog of CAP64, a gene whose product has similarly been implicated in capsule synthesis and virulence. A strain disrupted in CMT1 was generated in C. neoformans; this had no effect on virulence in an animal model of cryptococcosis.
Thyroid Disorders among Type 2 Diabetes Mellitus: A Cross-sectional Study from a District Hospital in West Bengal

Abstract:Introduction: The coexistence of diabetes mellitus (DM) (both type 1 and type 2) and thyroid dysfunction, two common endocrine problems, is scientifically predictable. The prevalence of thyroid disorders among type 2 diabetics (more prevalent in India) was studied among patients visiting the outpatient department (OPD) of a district hospital in West Bengal to assess its relation with different characteristics of type 2 DM. Materials and methods: A total of 120 patients suffering from type 2 DM (already diagnosed and on treatment) were randomly selected from the OPD (irrespective of their glycemic status). The thyroid status of all those patients was assessed. All diabetic patients were studied with a predesigned schedule and lab investigations for the prevalence of thyroid dysfunction and its association with pertinent variables from January to December 2019. Results: This study found a 28.3% prevalence of thyroid dysfunction among diabetics. It was significantly associated with poor glycemic control [rising hemoglobin A1c (HbA1c) level] (actual p-value for HbA1c vs abnormal thyroid status = 2.4 E-21) but not with other variables, including the duration of diabetes. Conclusion: Screening for thyroid dysfunction among diabetic persons should be routine, and strict glycemic control is essential.

Abstract:The prevalence of anemia in adults with diabetes is of growing importance due to its impact on overall health and the management of diabetes-related complications. This study aimed to determine the prevalence of anemia among adult patients with diabetes at King Abdulaziz University Hospital in Jeddah, Saudi Arabia. A retrospective study was done on 1208 patients with diabetes >18 years who attended the study setting from 2010 to 2022. Data about patients' demographics, body mass index, glycated hemoglobin (HbA1c; %), hemoglobin (Hb), serum ferritin, iron, mean corpuscular Hb, mean corpuscular volume, free thyroxine and triiodothyronine (T3), and serum thyroid–stimulating hormone (TSH) were collected. Of patients, 86.6% had anemia with a prevalence of 30.2%, 47.6%, and 22.2% for mild, moderate, and severe anemias, respectively. The prevalence of anemia was significantly higher among females, those with high serum ferritin, normal serum iron or normal serum T3, lower mean HbA1c level (%), lower serum iron or T3, and higher serum ferritin or TSH. A significant positive correlation was found between Hb level and HbA1c level (%), serum iron, free T3, and body mass index. A significant negative correlation was found between Hb level and mean corpuscular volume, serum ferritin, and serum TSH. Being female, having high serum ferritin, lower mean free T3, and a high TSH were risk factors for anemia. The prevalence of severe anemia was significantly higher among patients with uncontrolled diabetes mellitus. A high prevalence of anemia was found among studied diabetics. Anemia screening should be included in the routine assessment of patients with diabetes. A multidisciplinary approach involving endocrinologists, hematologists, and dietitians is recommended to ensure holistic care and address all aspects of the patient's health. In addition, further research should be supported to better understand the mechanisms linking diabetes and anemia and to establish evidence-based guidelines for managing anemia in diabetics.

Abstract:OBJECTIVES: To assess the proportion and grades of retinopathy and its risk factors in diabetes type 2 patients.MATERIALS AND METHODS: This was a cross-sectional study of 401 type 2 diabetic patients. A questionnaire and checklist were used to collect the data. Retinopathy was diagnosed and graded by fundus photographs and slit lamp examination. The duration of diabetes, age of patients, age at onset of diabetes, body mass index, hemoglobin A1c level, blood pressure, and complications were noted.RESULTS: The mean age of male and female patients was 54.93 and 54.25 years; 57.6% were males. The mean age of onset and mean duration of diabetes were 43.91 and 13.4 years, respectively. The proportion of retinopathy was 36.4%. Grades of retinopathy were: Mild 57.5%, moderate 19.9%, severe nonproliferative 11%, and proliferative retinopathy 11.6%; 7.2% of patients had maculopathy. Retinopathy was significantly associated with older age, younger age at onset, longer duration of disease, poorly controlled blood sugar, hypertension, insulin use; the presence of neuropathy and nephropathy appeared as a significant risk. Younger age at onset, longer duration, and insulin use appeared as the strongest predictors for diabetic retinopathy.CONCLUSIONS: More than a third (36.4%) of the diabetic patients attending a diabetic center had retinopathy. The control of the risk factors may reduce both prevalence and consequences of retinopathy.

Abstract:Diabetes mellitus is a common disease in Saudi Arabia and globally, with some studies reporting about 30% prevalence among the Saudi population. Diabetic retinopathy (DR) is the leading cause of blindness among working-age patients, and it has a prevalence of 34.6% among diabetics worldwide. The objectives of this review were to examine the prevalence of DR in Saudi Arabia, describe the major associated risk factors, and raise awareness about the disease among diabetics. The prevalence of DR in Saudi Arabia has risen substantially in recent decades. Significant risk factors associated with DR include older age, longer duration of diabetes, poor glycemic control, and hypertension. Awareness and education about diabetes are associated with better outcomes and fewer complications.

Abstract:Background: Diabetes is becoming more prevalent in undeveloped countries, what is known about self-care practice is insignificant, like self-monitoring of blood glucose, balanced diet, Medication adherence, and exercise. For people newly diagnosed with type 2 diabetes, exercise is usually one of the first control measures recommended. These practices are considered as the basis of diabetes management. Objctive: The study goal is to assess the awareness of type 2 diabetic patients about the importance of exercise and diet on diabetes type 2. Methods: A cross-sectional observational survey study that was carried out in western Province, Saudi Arabia. A questionnaires were used to ask about the background information and patients' demographics, self-monitoring of blood glucose, eating and drinking habits, exercise habits, and beliefs about the effectiveness of diet and exercise on blood glucose. Results: A total of 568 type 2 of diabetic patients. The average age of the participants was 51.89 ± 12.82 years, 53.5% were females, 89.8% were educated, 81.2% were married, and 30.1% were from Jeddah city. 30.3 % of patients did not practice healthy habits, and 43 % did not exercise, even though 90 % believed that exercise and diet had a positive impact on glucose level in the blood.Male patients showed considerably a higher percentage of those who continued to engage in unhealthy habits such as "smoking, eating fast food, eating sugars, or drinking soft drinks. Conclusion: A significant proportion of Saudi diabetic patients did not follow a healthy diet and exercise on a regular basis. Health policymakers should conduct effective health education sessions for diabetic patients on the importance of a healthy diet and exercise in diabetes management.

Abstract:BACKGROUND: Due to rising rates of morbidity and mortality associated with type 2 diabetes mellitus (T2DM), Saudi Arabia is ranked second in the Middle East and seventh overall among nations with the greatest incidence of diabetes mellitus (DM). Significant sleep abnormalities have been linked to difficulties in managing blood sugar levels, suggesting a link between sleep disorders and diabetes. This study aimed to find out how common sleep disturbances were among patients with T2DM in Makkah, Saudi Arabia.METHODS: This descriptive cross-sectional study was conducted between June and August 2022 in Makkah City, Saudi Arabia. Patients with T2DM who visited primary healthcare facilities in Makkah during the study's duration were included in the study. To evaluate sleep quality, the Pittsburgh Sleep Quality Index in Arabic was employed. Patients who met the inclusion criteria were given an interview questionnaire to fill out.RESULTS: In total, 355 patients with T2DM were enrolled in this study. The patients' median age was 49.24 years. Other than DM, a majority of them (58.9%) had chronic illnesses, with hypertension (64.5%) and cardiovascular disease (65.5%) as the most prevalent comorbidities. Only 22% of the patients had controlled diabetes. Of the individuals who had sleep disorders, 63.7% stated having poor sleep quality.CONCLUSION: Sleep problems are a common occurrence in patients with T2DM. Additionally, compared to people with other chronic disorders, people with endocrine diseases had poorer sleep quality. Hence, the duration of diabetes has an impact on sleep quality.

Abstract:Diabetes, a prevalent chronic metabolic disorder, presents a considerable global health challenge. It is marked by impaired insulin production or reduced insulin sensitivity, leading to elevated blood glucose levels that have detrimental effects on multiple organs and systems. This condition increases the risk of developing severe comorbidities, including cerebrovascular accidents, ischemic heart diseases, and renal diseases. [ 1 ] According to the International Diabetes Federation [ 2 ] globally more than half a billion people aged 20 to 79 years are currently living with diabetes, which constitutes more than 10% of this age group, while 1.2 million children and adolescents have type 1 diabetes. While premature death from main noncommunicable diseases is declining between the years 2000 to 2016, those from diabetes in the same period witnessed a 5% increase. [ 3 ] In the Kingdom of Saudi Arabia, where 4.3 million are living with the condition, [ 2 ] diabetes is a serious problem to patients, their families, and the healthcare system. [ 4 ] More than half of diabetics in Africa, South–East Asia and the Western Pacific are undiagnosed. Generally, half of adults suffering from diabetes are not diagnosed. [ 3 ] Addressing the complexities of diabetes management and understanding its impact on various health aspects are crucial steps towards combating this significant public health issue and improving the overall well-being of those affected.

Abstract:Background Diabetes mellitus is a disease whose dramatic increase in prevalence worldwide poses a global health crisis. Saudi Arabia has the seventh highest rate of diabetes in the world and the second highest rate in the Middle East. The incidence of diabetes has risen due to insufficient knowledge, attitude, and practice surrounding the disease. Saudis' quality of life has decreased due to the recent rise in diabetes-related mortality and illnesses. Hence, leading healthy lives requires patients to have positive attitudes and self-awareness, which will eventually contribute to reducing diabetes mellites-related complications. Methods A cross-sectional study among patients with type 2 diabetes mellites (T2DM) in the Jazan region of Saudi Arabia was conducted to assess Saudi adults' knowledge, attitude, and practice (KAP) regarding the disease. The patients participated in the study by filling in a questionnaire. Data analysis was carried out using R software, version 4.0.5 (R Studio: Integrated Development for R, Boston, MA) and the analysis included general sample demographics. KAP scores were the dependent variables. Following the sample description, regression analysis was performed to examine the adjusted relationships between KAP factors and independent variables. The impact of all independent variables on KAP variables was examined using multiple linear regression analysis. A p-value of less than 0.05 was considered significant, and the beta coefficient was applied to estimate the associations between the independent variables and the KAP variables. Results A total of 424 participants were involved in this study, with 194 male participants and 230 female participants. The mean duration of having T2DM was 7.63 ± 7.19 years. More than half of the study participants had a degree (58.49%), and nearly half were employed (42.45%). Married participants had higher knowledge and attitude scores than single and divorced or widowed participants (9.19 ± 3.38, 80.14 ± 14.72, respectively). In comparison, divorced or widowed participants had higher practice scores than single and married participants (16.35 ± 7.42). Participants with higher education attainment had higher knowledge and attitude scores than those with no degree (9.54 ± 3.39, 80.58 ± 17.57, respectively), whereas participants with no degree demonstrated higher practice scores (16.85 ± 8.3) than those with degrees. In examining the study participants' KAP scores, we found knowledge to be insufficient in 51.2% and sufficient in 48.8% of the participants. Of the participants, 7.8% were found to have negative attitude levels and 92.2% were found to have positive attitude levels. Finally, study participants' practice levels were found to be negative in 24.8% and positive in 75.2% of participants. Conclusions This study revealed that patients have gaps in their knowledge, attitude, and practice in regard to T2DM. In this study, associations and correlations were established between KAP scores and the sociodemographic characteristics of the patients. The findings of this study could be helpful to policymakers, decision-makers, health care professionals, and patient advocacy groups in developing interventions to improve the health outcomes in T2DM patients.
