Abstract:This systematic review aims to investigate the published literature on vitamin D deficiency (VDD) as a risk factor for developing type 2 diabetes mellitus (T2DM) or affecting the glycemic state of patients in Saudi Arabia. PubMed, Scopus, Web of Science, ScienceDirect, and the Cochrane Library were systematically searched to include the relevant literature. Rayyan QCRI (Rayyan Systems Inc., Cambridge, MA) was used throughout this systematic approach. Eleven studies were included with a total of 4229 patients. Three studies demonstrated that VDD was a significant risk factor for developing T2DM, and one reported that it increased insulin resistance. However, two studies found that VDD did not affect the incidence of T2DM and did not affect the insulin sensitivity or glycated hemoglobin (HbA1c) levels in patients with T2DM. This systematic review demonstrated that VDD significantly increases the risk of developing T2DM and negatively affects the glycemic state of patients with T2DM among Saudi patients. Due to the many populations examined, vitamin D chemical compositions, doses, and supplementation periods, interventional research has produced contradictory and ambiguous results. Additional research is necessary, particularly in individuals with a high risk of developing diabetes (impaired fasting glucose and/or glucose tolerance, possibly without obesity). These individuals may be the primary benefactors of vitamin D's benefits in preventing T2DM, according to the hypothesized mechanism of action for the vitamin.

Abstract:Summary Using hemoglobin markers to identify donor- and host-type erythrocytes, we have demonstrated that cells in the leukocyte fraction of leukemoid blood are capable of transplanting and giving rise to mature erythrocytes of donor type in lethally irradiated mice. Autoradiograms showed that several cell types in leukemoid blood incorporate H3-thymidine in vitro. We wish to thank Mr. W. D. Gude of the Biology Division for making the autoradiographs.
Association between standardized vitamin 25(OH)D and dyslipidemia: a community-based study in Riyadh, Saudi Arabia

Abstract:Background: Vitamin D deficiency associated with dyslipidemia can contribute towards cardiovascular diseases. Previous studies have found that Saudi Arabia has a high burden of vitamin D deficiency and cardiovascular disease risk factors. We aimed to explore the relationship between vitamin D deficiency and dyslipidemia, including total cholesterol, low-density lipids, high-density lipids (HDL), and triglycerides (TG) in apparently healthy Saudi male and female participants aged 30-75 years. Methods: A cross-sectional study was conducted on 1717 apparently healthy Saudi participants from 18 primary health care centers in Riyadh. Data collectors conducted the interviews, took anthropometric measurements, and collected the blood samples. Serum 25-hydroxyvitamin vitamin D (25(OH)D) levels were measured using an electrochemiluminescence assay method. Lipid panel was measured by a fully automated analyzer using enzymatic methods. Results: Multivariable logistic regression analysis revealed that the adjusted odds ratio (ORA) of low level of HDL cholesterol in association with 25(OH)D deficiency was 2.1 times higher in males (ORA = 2.1; 95% CI = 1.1, 3.9) and 1.3 times higher in females (ORA = 1.3; 95% CI = 0.9, 1.9). A significant excess odds ratio of high levels of TG in association with 25(OH) D deficiency was observed in females (ORA = 3.0; 95% CI = 1.1, 7.9) but not in males. Conclusion: Vitamin D deficiency is highly prevalent in Saudi Arabia. Low levels of HDL cholesterol in men and high TG levels in women are associated with vitamin D deficiency. The results emphasize the importance of treating vitamin D deficiency in the general population.
Association of Glycosylated Hemoglobin Levels With Vitamin D Status

Abstract:Background: The discovery of vitamin D is one of medicine's great achievements. Despite all the positive evidence emerging about the beneficial effect of vitamin D, we still find many are vitamin D deficient. The purposes of this study were to examine the association between serum 25-hydroxyvitamin D (25(OH)D) and glycosylated hemoglobin (HbA1c) levels, to test the hypothesis that lower 25(OH)D levels are associated with poorer glucose control in diabetes mellitus (DM) patients and to investigate the effect of vitamin D supplementation on HbA1c levels. Methods: This was a prospective observational cohort study of all patients with type 1 and type 2 diabetes (above 12 years) who attended the outpatient clinics of a tertiary center in Riyadh. HbA1c and vitamin D levels were recorded prior to supplementation and after 9 months of supplementation with vitamin D. All patients were divided into four groups according to their vitamin D level and an association between 25(OH)D and HbA1c was tested. Results: Results showed that 73.1% of the patients had 25(OH)D levels < 50 nmol/L. We observed lowering of HbA1c after vitamin D supplementation (from mean HbA1c of 10.55 to 7.70). We found HbA1c to be inversely related to serum vitamin D levels (r = -0.14 (P < 0.0000002) before supplementation and -0.16 (P < 0.000001) after supplementation with vitamin D). Conclusions: Advising patients with higher HbA1c to test their vitamin D level and correct any deficiency will result in better blood glucose control and benefit the patient's overall health.

Abstract:Two copper salts (Cu(CF3CO2)2 and IMesCuCl) were identified as earth-abundant, inexpensive, but effective metal catalysts together with diazo malonate for chemo-/regioselective and stereospecific deoxygenation of various epoxides with tolerance of common functional groups (alkene, ketone, ester, p-methoxybenzyl, benzyl, tert-butyldimethylsilyl, and triisopropylsilyl). In particular, the unprecedented regioselectivity allowed for the first time monodeoxygenation of diepoxides to alkenyl epoxides. Density functional theory mechanistic studies showed that the deoxygenation occurred by collapsing the free ylide, unfavoring the possible intuitive pathway via cycloreversion of possible oxetane.
Impact of serum levels of vitamin D on lipid profiles, glycemic indices, and insulin resistance in obese type 2 diabetes patients: An observational study

Abstract:Background: Diabetes patients have a higher chance of developing dyslipidemia and increased release of free fatty acids, which participate in developing insulin-resistant fat cells. On the other hand, vitamin D insufficiency is linked to the evolution of type 2 diabetes mellitus (T2DM). This study examines the impact of vitamin D serum levels on lipid profiles and insulin resistance concerning glycemic indices in obese T2DM patients. Methods: During the data collecting stage, 47 diabetes patients were chosen from the out-patient clinic. The control individuals were selected from the general population and were equivalent to the matching patients, with a total of 43 healthy participants. After an overnight fast, a venous blood sample was collected from each individual to test insulin and vitamin D3 levels using particular ELISA kits. In addition, by colorimetric test, serum was used to estimate total cholesterol, triglyceride, and high-density lipoprotein cholesterol. Aside from that, fasting serum glucose levels were measured (FSG). Results: Fasting serum glucose (FSG), homeostatic model assessment-insulin resistance (HOMA-IR), total cholesterol, and triglycerides, all of these values were significantly elevated in people with diabetes as compared to controls (p-value <0.05) when the serum level of vitamin D was markedly low. In contrast, insulin and high-density lipoprotein values had decreased significantly in the diabetic population compared to controls (p-value <0.05) and were not correlated to vitamin D levels. Conclusions: Diabetes patients had higher FSG, HOMA-IR, hemoglobin A1c (HbA1c), fasting insulin, triglycerides, total cholesterol to high-density lipoprotein cholesterol ratios (TC: HDL-C), triglyceride to high-density lipoprotein cholesterol ratios (TG: HDL-C), and low-density lipoprotein cholesterol to high-density lipoprotein cholesterol ratios (LDL-C: HDL). In obese diabetic individuals, vitamin D levels had a significant impact on total cholesterol, LDL-C, and the TC:HDL-C ratio.

Abstract:Aim: Vitamin D deficiency is linked with type 2 diabetes mellitus (T2DM) and the occurrence of complications in patients with type 2 diabetes mellitus. None of the studies have focused on the association between vitamin D levels in patients with type 2 diabetes mellitus and diabetic nephropathy (DN) in the Makkah region, Saudi Arabia. Hence, the purpose of this study is to investigate the relationship of vitamin D with kidney disease in patients with T2DM in the Makkah region, of Saudi Arabia. Materials and methods: This descriptive cross-sectional study was conducted at different hospitals in the Makkah region on T2DM patients from 2021 to 2022. In total, 328 patients with confirmed diabetes were enrolled in this study. T2DM patients over the aged>18 to 92 years were included in the study. General laboratory characteristics of the study population were measured, including fasting blood sugar, HbA1C (Glycated hemoglobin), vitamin D, kidney function (BUN-Blood urea nitrogen and creatinine), and lipid profiles (cholesterol, triglycerides, LDL-Low density lipoprotein, and HDL-High density lipoprotein). Results: 46.6% (n=153) of participants had normal serum vitamin D levels. Insufficient and deficient serum vitamin D level were observed in 43.9% (n=144) and 9.5% (n=31) of participants, respectively. Of the participants, 25.9% (n=85) had good glycemic control (<7.0%). Moderate and poor glycemic control were observed in 39.9% (n=131) and 34.1% (n=112) of the participants, respectively. A significant negative correlation (p<0.5) was found between vitamin D levels and kidney function test results (blood urea nitrogen and serum creatinine levels). An inverse relationship was observed between HbA1c levels and vitamin D deficiency. Conclusion: Nephropathy is more likely to develop in people with type 2 diabetes mellitus and vitamin D deficiency.

Abstract:Introduction: Metabolic syndrome includes a range of disorders that increase the risk of cardiovascular disease and diabetes mellitus. In this study, we examined the serum level of vitamin D3 in diabetic individuals with metabolic syndrome compared with non-diabetic individuals without metabolic syndrome and the association of serum vitamin D3 levels with metabolic syndrome and atherogenic factor (LDL/HDL). Material and Methods: In a case-control study, we included 110 women with metabolic syndrome according to ATP III criteria and 127 healthy women as a control group. Serum concentration of total cholesterol, LDL-C, FBS, HDL-C and serum triglyceride determined by enzymatic method and colorimetric and, serum level 25-(OH) vitamin D determined by ELISA. Results: It was found that the two healthy and metabolic groups were significantly different in terms of total cholesterol levels, LDL and triglyceride levels, HDL, VLDL, FBS, atherogenic index (LDL/HDL) and vitamin D levels (p<0.05). All participants in the control group and the patient and the whole study population were divided into two categories of insufficient and sufficient based on their measured serum concentrations of 25-(OH) vitamin D. There was a significant difference between the group with insufficient levels of vitamin D in comparison with the group with sufficient levels of vitamin D in terms of total cholesterol, LDL and triglyceride levels, HDL, VLDL, FBS and atherogenic index (LDL/HDL) (p=0.000). Conclusion: The present results showed that there is a significant relationship between level 25-(OH) D and atherogenic index (LDL/HDL) and the incidence of metabolic syndrome.

Abstract:Background: Vitamin D may play important role in the pathogenesis of several components of metabolic syndrome (MS). The aim of this study was to observe the association of vitamin D with MS and its components in Bangladeshi adults with prediabetes. Methods: This cross-sectional study was done among 117 newly detected nonpregnant adults with prediabetes [age (years): 36.30±9.99; BMI (kg/ m2): 28.89±4.35, mean±SD; M/F: 23/94] based on American Diabetes Association 2018 criteria. Metabolic syndrome was diagnosed by any three of five criteria: central obesity; elevated blood pressure, fasting blood glucose & triglyceride (TG) and lower HDL cholesterol. Glucose was measured by glucose oxidase, lipid by glycerol phosphate dehydrogenase-peroxidase and vitamin D by high performance liquid chromatography method. Vitamin D <20 ng/ml was considered as deficiency (VDD). Results: Among all the variables only TG was significantly higher in patients with VDD than those without VDD (320 ng/ml) [163.0 (135.50, 224.50) vs. 143.50 (101.25, 190.0), median (IQR), p=0.048]. There were no significant correlations [p=NS for all] and associations of vitamin D with MS or its components in linear and logistic regression [p=NS for all]. Abstract:Introduction: Several studies have suggested a benefic impact of vitamin D supplementation on glycemic control and insulin resistance among patients with type 2 diabetes mellitus. The aims of our study were to assess vitamin D status in individuals with type 2 diabetes mellitus and to investigate the effects of vitamin D supplementation on glycemic measures in patients having vitamin D deficiency. Methods: We conducted a comparative prospective study involved 100 Tunisian patients with type 2 diabetes followed at the National Institute of Nutrition and Food Technology of Tunis. Glycemic control and insulin resistance were evaluated in the beginning of the study and three months after supplementation. Results: Baseline mean 25-Hydroxy vitamin D (25(OH)D) level was 17.5±9.8 ng/ml. Vitamin D status was deficient in 60%, insufficient in 26% and sufficient in 14% patients. After vitamin D supplementation, mean serum 25(OH)D concentration increased significantly (p˂10-3). We observed a negative correlation between the variation of plasma 25(OH)D level and the waist circumference's variation (r=-0.266 and p=0.018). This correlation persisted after adjustment for therapeutic management. Vitamin D supplementation did significantly improve neither glycemic control nor insulin resistance parameters. Conclusion: Vitamin D deficiency is frequent in patients with type 2 diabetes mellitus. The metabolic effects of supplementation are controversial, hence the need of expanding studies to better demonstrate these effects.
Vitamin D and Risk for Type 2 Diabetes in People With Prediabetes : A Systematic Review and Meta-analysis of Individual Participant Data From 3 Randomized Clinical Trials

Abstract:Background: The role of vitamin D in people who are at risk for type 2 diabetes remains unclear. Purpose: To evaluate whether administration of vitamin D decreases risk for diabetes among people with prediabetes. Data sources: PubMed, Embase, and ClinicalTrials.gov from database inception through 9 December 2022. Study selection: Eligible trials that were specifically designed and conducted to test the effects of oral vitamin D versus placebo on new-onset diabetes in adults with prediabetes. Data extraction: The primary outcome was time to event for new-onset diabetes. Secondary outcomes were regression to normal glucose regulation and adverse events. Prespecified analyses (both unadjusted and adjusted for key baseline variables) were conducted according to the intention-to-treat principle. Data synthesis: Three randomized trials were included, which tested cholecalciferol, 20 000 IU (500 mcg) weekly; cholecalciferol, 4000 IU (100 mcg) daily; or eldecalcitol, 0.75 mcg daily, versus matching placebos. Trials were at low risk of bias. Vitamin D reduced risk for diabetes by 15% (hazard ratio, 0.85 [95% CI, 0.75 to 0.96]) in adjusted analyses, with a 3-year absolute risk reduction of 3.3% (CI, 0.6% to 6.0%). The effect of vitamin D did not differ in prespecified subgroups. Among participants assigned to the vitamin D group who maintained an intratrial mean serum 25-hydroxyvitamin D level of at least 125 nmol/L (≥50 ng/mL) compared with 50 to 74 nmol/L (20 to 29 ng/mL) during follow-up, cholecalciferol reduced risk for diabetes by 76% (hazard ratio, 0.24 [CI, 0.16 to 0.36]), with a 3-year absolute risk reduction of 18.1% (CI, 11.7% to 24.6%). Vitamin D increased the likelihood of regression to normal glucose regulation by 30% (rate ratio, 1.30 [CI, 1.16 to 1.46]). There was no evidence of difference in the rate ratios for adverse events (kidney stones: 1.17 [CI, 0.69 to 1.99]; hypercalcemia: 2.34 [CI, 0.83 to 6.66]; hypercalciuria: 1.65 [CI, 0.83 to 3.28]; death: 0.85 [CI, 0.31 to 2.36]). Limitations: Studies of people with prediabetes do not apply to the general population. Trials may not have been powered for safety outcomes. Prevalence of vitamin D deficiency in urban south Indians with different grades of glucose tolerance

Abstract:The present study assessed the prevalence of vitamin D deficiency in an urban south Indian population in individuals with different grades of glucose tolerance. A total of 1500 individuals (900 normal glucose tolerance (NGT), 300 prediabetes and 300 with type 2 diabetes mellitus (T2DM)) who were not on vitamin D supplementation were randomly selected from the Chennai Urban Rural Epidemiological Study follow-up study. Anthropometric, clinical examination and biochemical investigations (25-hydroxyvitamin D (25(OH)D), insulin, glycated Hb (HbA1c) and serum lipids) were measured. Vitamin D deficiency was defined as serum 25(OH)D < 20·0 ng/ml, insufficiency as 20-29·9 ng/ml and sufficiency as ≥30 ng/ml. Of the 1500 individuals studied, 45 % were males and the mean age was 46 (sd 12) years. Vitamin D levels lowered with increasing degrees of glucose tolerance (NGT: 21 (sd 11); prediabetes: 19 (sd 10); T2DM: 18 (sd 11) ng/ml, P < 0·001). The overall prevalence of vitamin D deficiency was 55 % and was significantly higher among individuals with T2DM (63 %) followed by prediabetes (58 %) and NGT (51 %) (Pfor trend < 0·001). Women had 1·6 times the risk of vitamin D deficiency compared with men (unadjusted OR 1·6 (95 % CI 1·3, 2·0) and adjusted OR 1·6 (95 % CI 1·2, 1·9)). However, there was no increasing trend observed with increasing age. The prevalence of abdominal obesity (66 v. 49 %), generalised obesity (80 v. 64 %), the metabolic syndrome (45 v. 37 %) and insulin resistance (38 v. 27 %) was significantly higher in those with vitamin D deficiency compared with those without. This study shows that vitamin D deficiency is highly prevalent in this urban south Indian population and was higher among individuals with T2DM and prediabetes compared with those with NGT.

Abstract:Background Vitamin D deficiency has been associated with metabolic syndrome and its related outcomes, including type 2 diabetes mellitus, cardiovascular disease, myocardial infarction, and stroke. However, studies in Indian populations have provided conflicting results. Methods This single-center cross-sectional study was conducted in a tertiary care hospital in north India to determine the prevalence of vitamin D deficiency in patients with metabolic syndrome and to study the correlations of individual components of metabolic syndrome with 25 hydroxy vitamin D levels. The study included 235 patients aged between 30 to 70 years who met the criteria for metabolic syndrome. Patients with diabetes, hypothyroidism, Cushing's, and other disorders affecting vitamin D status, on supplements of vitamin D or anti-dyslipidaemic drugs were excluded. Information regarding socio-demographic characteristics, medical history, and anthropometric measurements were collected. Blood samples were collected to assess vitamin D levels. Results The prevalence of vitamin D deficiency (<20 ng/ml) was 76% among the study population. There was a significant negative correlation between vitamin D levels and diastolic blood pressure (Spearman's rho: -0.134, 95% CI: -0.82,-0.260, p=0.040), fasting blood glucose (Spearman's rho: -0.142, 95% CI: -0.101,-0.269, p=0.029), A weak correlation was also found between vitamin D3 levels and total cholesterol (Spearman's rho: -0.246, 95% CI: -0.119,-0.367, p<0.001), triglyceride levels (Spearman's rho: -0.246, 95% CI: -0.118,-0.370, p<0.001) and low-density lipoprotein (LDL) levels (Spearman's rho: -0.229, 95% CI: -0.102,-0.351, p<0.001). Conclusion The study findings suggest that vitamin D deficiency is prevalent among patients with metabolic syndrome in north India. There is a significant negative correlation between vitamin D levels and some components of metabolic syndrome. This highlights the need for further research to understand the underlying mechanisms and potential benefits of vitamin D supplementation in this population. Identification of high-risk individuals for hypovitaminosis D can aid in streamlining treatment guidelines and preventing unnecessary prescription of investigations in developing countries.

Abstract:INTRODUCTIONLow vitamin D levels have been associated with type 2 diabetes mellitus and non-alcoholic fatty liver disease. Insulin resistance, inflammation and oxidative stress have been suggested to be responsible for the development of NAFLD. These mechanisms are linked to vitamin D’s anti-inflammatory effect. Vitamin D deficiency is highly prevalent among patients with T2DM and NAFLD and is an independent risk factor for developing NAFLD in patients with T2DM. METHODOLOGYThe study aimed to determine the prevalence and associated risk factors of vitamin D deficiency in patients with NAFLD and T2DM. We conducted a cross-sectional study in patients with T2DM (n=110). The patients were divided into 2 groups: NAFLD (n=86) and no NAFLD (n = 24). The patients within the NAFLD group were further divided into 2 groups (vitamin D deficient and non-deficient). Serum total 25 (OH) D3 was analysed using electrochemiluminescence immunoassay (Roche), and deficiency was defined as a level of <50 nmol/L. Diagnosis of NAFLD was based on the abdominal ultrasound performed by 2 experienced radiologists. Steatosis was defined as increased liver echogenicity. RESULTThe prevalence of vitamin D deficiency in patients with T2DM and NAFLD was 52.3% (45/86), as compared to only 33.8% (8/24) in those without NAFLD. Vitamin D level was significantly lower in patients with NAFLD as compared to those without NAFLD (51.53 ± 19.68 vs 60.61 ± 20.25, p <0.05). There was no difference in the age (57.74 ± 8.42 vs 57.96 ± 7.97 years, p = 0.91), gender (male 55.7% vs 70.8%, p = 0.18), BMI (28.99 ± 3.80 vs 28.24 ± 3.74 kg/m2 , p = 0.40), and diabetes duration (12.43 ± 7.92 vs 10.42 ± 6.51, p = 0.25), between the 2 groups. Multivariate analysis demonstrated that HbA1c (AOR 1.89; 95%CI 1.15-3.09; p = 0.01) and vitamin D deficiency were the independent risk factors for NAFLD (AOR 3.15 95%CI 1.10-9.04; p = 0.03), after adjustment for age, gender, diabetes duration and eGFR. CONCLUSIONThis study demonstrated a high prevalence of vitamin D Deficiency in patients with NAFLD and T2DM. In patients with T2DM, those with vitamin D deficiency were three times more likely to develop NAFLD. Vitamin D levels of <50 nmol/L and higher HbA1c were the two independent risk factors for developing NAFLD.

Abstract:Objective: To determine the burden of vitamin D deficiency and its determinants and to assess the relationship of 25 hydroxycholecalciferol (25-OHD) levels with biochemical parameters linked to health outcomes in women with Type 2 Diabetes Mellitus (T2DM). Material and methods: This was a hospital based cross-sectional study in the diabetes out-patient department clinic of a major tertiary care hospital in Delhi, India. Adult women with T2DM on treatment for at least 6 months were included in this study. The women who have been given Vitamin D supplementation during the past 6 months were excluded. We assessed Serum 25-OHD, HbA1c, lipid profile and fasting plasma glucose in the patients through standardized laboratory methods. Results: One hundred women with T2DM were enrolled of which 22 (22%) had good glycemic control (HbA1c < 7%). Vitamin D deficiency was seen among 77 (77%) and insufficiency among 16 (16%) of the recruited subjects. Younger age group (31-45 years) and illiteracy was significantly associated with vitamin D deficiency (p < 0.05). No association was found between Vitamin D deficiency and HbA1c levels. Conclusion: Vitamin D deficiency is highly prevalent among women with T2DM. Illiteracy and young age were major determinants of vitamin D deficiency indicating they need special attention and Vitamin D supplementation.

Abstract:Vitamin D's role extends beyond classical calcium and phosphate homeostasis to encompass a pivotal influence on immune modulation and metabolic health. The mechanisms by which vitamin D exerts these effects involve its conversion to hormonally active calcitriol, which binds intracellular vitamin D receptors, initiating various downstream cascades. In this review, we tease out the evidence showing the relationship between vitamin D deficiency and prediabetes within the context of subclinical inflammation, with a special focus on the novel monocyte-to-HDL ratio (MHR), a novel inflammatory marker reflecting subclinical inflammation. This was based on a thorough literature review using reputable databases covering the period from 1980 to 2024. In light of this, we discuss calcitriol's anti-inflammatory effects and consequently link vitamin D deficiency to both overt and subclinical inflammation. Additionally, the utility of several biomarkers, notably MHR, in investigating this association is also discussed. We further reviewed the role of vitamin D deficiency in precipitating prediabetes and type 2 diabetes mellitus (T2DM) via insulin resistance, decreased insulin synthesis and secretion, and subclinical inflammation. Taken together, this mini review highlights that vitamin D deficiency is significantly associated with subclinical inflammation, playing a critical role in the development of prediabetes and the progression to T2DM. Addressing vitamin D deficiency through appropriate interventions may serve as a preventative measure against the development of prediabetes and T2DM.

It is also predicted to become the leading cause of mortality by the year 2035. [ 1 ] Vitamin D deficiency (VDD) is also fairly prevalent, contributing to multiple disorders such as rickets, osteomalacia, osteoporosis, and male infertility. [ 2 , 3 ] VDD is a worldwide concern that impacts individuals of all age groups, regardless of gender. Additionally, vitamin D has the potential to enhance insulin sensitivity by promoting greater responsiveness of insulin in facilitating glucose transport, thus reducing the risk of developing DM. [ 4 ]
