Abstract:As an effective adjunct to central refraction,peripheral refraction can affect retinal shape and image quality. Defocus in the peripheral retina can induce accommodation, but the principle of this reaction is still not clear.Experimental data shows peripheral refraction is able to control the progression of emmetropization, and clinical studies also indicate the association between peripheral refraction and the development of myopia. It is important to do further study on the correlation between peripheral refraction and myopia progression.

Abstract:Myopia is the most common refractive error in the world, and its’ prevalence continually increases. The potential pathological and visual complications of progressive myopia have inspired researchers to study the sources of myopia, axial elongation, and explore modalities to arrest progression. Considerable attention has been given over the past few years to the myopia risk factor known as hyperopic peripheral blur, the focus of this review. The primary theories currently believed to be the cause of myopia, the parameters considered to contribute and influence the effect of peripheral blur, such as the surface retinal area or depth of blur will be discussed. The currently available optical devices designed to provide peripheral myopic defocus will be discussed, including bifocal and progressive addition ophthalmic lenses, peripheral defocus single vision ophthalmic lenses, orthokeratology lenses, and bifocal or multifocal center distance soft lenses, as well as their effectivity as mentioned in the literature to date.

Abstract:Myopia is a leading cause of visual impairment in young people worldwide. It sometimes increases the risk of blindness and reduces life quality. Previous reports have revealed the treatment effects of defocus-incorporated multiple segments (DIMS) and topical atropine (ATP) on myopia control. However, no study has evaluated these two interventions together. In this retrospective study, we aimed to determine whether the combination of DIMS lenses and 0.01% ATP can slow the progression of myopia compared with DIMS lenses or single vision (SV) lenses alone. We included 107 children with myopia who were treated with DIMS and 0.01% ATP combination (DIMS + ATP group), DIMS monotherapy (DIMS group), or a control group (SV group). We compared treatment effects among three groups in axial length and myopia progression. After a 1-year follow-up, the DIMS + ATP group showed a smaller change in axial length and myopia progression than the DIMS and SV groups (P < 0.05). Hence, combination treatment with DIMS and 0.01% ATP might be a better choice for children with myopia.

Abstract:Exposing the retina to a simultaneous myopic defocus is an optical method that has shown a promising effect in slowing the progression of myopia. Optical treatments applying a simultaneous defocus are available in the form of soft contact lenses or multifocal lenses originally designed to correct presbyopia. Orthokeratology is another optical method that slows down the progression of myopia. With orthokeratology, it is hypothesized that a change in peripheral refraction could slow the progression of myopia. We aimed to measure the accommodation response between monofocal and multifocal contact lenses in young subjects. Additionally, we performed a ray-tracing simulation to visualize the quality of the retinal image and the refractive status in the retinal periphery. The accommodation and pupil size measurements were performed on 29 participants aged 24.03 ± 2.73 years with a refractive error (spherical equivalent) of −1.78 ± 1.06 D. With the multifocal lens in situ, our participants showed less accommodation in comparison to the monofocal contact lens (mean difference, 0.576 ± 0.36 D, p > 0.001) when focusing on a near target at 40 cm. Pupil size became smaller in both contact lens groups during an accommodation of 0.29 ± 0.69 mm, p ≤ 0.001 and 0.39 ± 0.46 mm, p ≤ 0.001 for monofocal and multifocal contact lenses, respectively. The ray-tracing model showed a degradation for central and peripheral vision with the multifocal contact lens. The peripheral refraction was relatively myopic in both contact lens conditions up to 30°. Even if the accommodation ability is without fault, parts of simultaneous myopic defocus are used for the near task. The peripheral refraction in the ray-tracing model was not different between the two contact lenses. This is contrary to the proposed hypothesis that myopic peripheral refraction slows down the progression of myopia in current optical methods.

Abstract:Objective To investigate the changes in the peripheral defocus of the human eye under accommodation and the correlation between defocus and refractive power and to investigate the possible mechanism of the development of myopia.Methods Thirty-three subjects were enrolled,ranging in age from 22 to 27 years with all average age of(25.03±l.33)years.Emmetropia,low myopia,moderate myopia group were 10,10,13,spherical equivalent was (-0.25 ±0.26)D,(-1.75±0.55)D,(-4.15±0.92)D.Trial lenses were used to correct myopia,Measurements at the fovea,and at eccentricities of 30° in the temporal and nasal hemispheres of the horizontal visual field at distances of 3m,40cm were obtained using Zemike aberration coefficients calculated with a WASCA wavefront analyzer.Only the right eye of each subject was tested.Repeated measures ANOVA were analysis of variance accommodation increases peripheral aberrations trends,as well as the distant and near vision,peripheral retina 30° different refractive aberration differences between groups.Results The defocus at all retinal positions increased with accommodation were statistically significant(P ＜0.001).The significant differences were found at distance between refractive error groups at nasal 30° with the defocus amount emmetropia(0.640±0.670μm)and low myopia group(0.298±0.462μm)were greater than moderate myopia group(-0.599±0.833μm),(P ＜0.001,P =0.004).As for near,defocus amount emmetropia(l.734±0.589μm)than moderate myopia group(0.868±l.061μm),and was statistically significant(P=0.019).At temporal 30°,as far and near vision it was no difference among the three groups.The focus was correlated with diopter at nasal 30° as far and near vision(as far slope of 0.274 μm/D,r2=0.572,P =0.001; as near slope 0.170 μm/D,r2=0.345,P =0.049).Conclusions The defocus toward positive values with accommodation and that it is myopic shift with accommodation.Peripheral 30° hyperopic defocus amount increased with the increase in myopia.

Abstract:Receiver operating characteristic (ROC) curves were used to quantitatively assess the ability of individual detectors in a 32-detector 133Xe inhalation system to discriminate between two populations over the range of regional cerebral blood flow (rCBF) values. These populations were clinically evaluated as normal (age 63.1 +/- 13.1, n = 23) and presumed Alzheimer's disease (age 72.7 +/- 7.0, n = 82). Summary statistics showed that for homologous detectors the average value of blood flow in the normal group was greater than the flow value in the group of subjects with Alzheimer's disease. Conclusions drawn from single values of flow or mean hemispheric flow can lead to erroneous conclusions about hemisphere asymmetries. However, the dynamic relationship between the correct identifications (true positives) compared with incorrect identifications (false positives) of Alzheimer's disease at each detector varies over the range of blood flow values, and quantitative characterization of this relationship in terms of an ROC curve provides more insight into the structure of the data. Detectors approximating the speech, auditory and association cortex were most effective in discriminating between groups. Frontal detectors were marginally useful diagnostically.

Abstract:Background. The change in refraction caused by accommodation inevitably affects the peripheral defocus state and thus may influence the effect of retinal peripheral myopic defocus measures in myopia control. This study investigated accommodation changes in different peripheral retinas under cycloplegia to help improve myopia control. Methods. Fifty-six eyes of fifty-six myopic subjects were recruited for this prospective study. The center and peripheral retina refractions were measured using multispectral refractive topography. The subjects were divided into low-to-moderate myopia group (range: −1.25 D to −6.00 D) and high myopia group (range: −6.25 D to −9.75 D) according to spherical equivalent (SE). The compound tropicamide (0.5% tropicamide and 0.5% phenylephrine) was used to relax the accommodation. The difference between cycloplegia and non-cycloplegia peripheral retinal refraction was analyzed using the t-test. The correlation between eccentricity and changes in peripheral refraction was analyzed using Pearson's correlation analysis. Results. The manifest refraction of the retina significantly decreased with an increase in eccentricity after cycloplegia. The annular refraction difference value at 50°–53° (ARDV 50–53) showed the largest refraction decrease of 1.31 D compared with the central retinal refraction decrease of 0.84 D. The inferior quadrantal refraction difference value had the least change compared to the other quadrants. The relative peripheral refraction (RPR) changes in refraction difference value (RDV) at 15° (RDV-15), RDV-30, and RDV-45 were less than 0.15 D. When the range of annulus narrowed to 5°, the narrower annulus showed faster change with eccentricity increase in ARDV 30–35, ARDV 35–40, ARDV 40–45, ARDV 45–50, and ARDV 50–53. The RPR was highly correlated with eccentricity (R = 0.938 and P < 0.001 ). The high myopia group had a greater hyperopic shift in the periphery than the low-to-moderate group after cycloplegia. Conclusions. Peripheral refraction showed a significant hyperopic shift after cycloplegia with an increase in eccentricity. The RPR became more hyperopic than the central refraction. The high myopia group showed more hyperopic shifts in the peripheral region. Accommodation should be taken into consideration in peripheral defocus treatment.

Abstract:To discuss the effectiveness of optical reduction of peripheral hyperopic defocus in preventing progression of myopia. The relationship between reducing peripheral hyperopic defocus and slowing progression of myopia has been discussed with regard to glasses, contact lenses, and orthokeratology. Previous studies on optical reduction of peripheral hyperopic defocus mainly in Japan will be presented. We performed a prospective, randomized, double‐blind clinical trial using a questionnaire to compare myopic progression between a single‐vision lens and a lens that reduces peripheral hyperopic defocus. This study is part of a multicenter trial in Japan and the results of our institute will be presented. Many clinical trials have suggested that reducing peripheral hyperopic defocus possibly retards progression of myopia. Outdoor activity is an evidence‐based environmental factor that protects against myopia; our data also show the importance of outdoor activities for preventing progression of myopia. Not only reducing peripheral hyperopic defocus but also environmental factors such as outdoor activity is important to retard myopia. A greater number of eyes should be evaluated in future studies.

Myopia Control Effect Is Influenced by Baseline Relative Peripheral Refraction in Children Wearing Defocus Incorporated Multiple Segments (DIMS) Spectacle Lenses

Abstract:The aim of this study is to investigate if baseline relative peripheral refraction (RPR) influences the myopia control effects in Chinese myopic children wearing Defocus Incorporated Multiple Segments (DIMS) lenses. Peripheral refraction at 10°, 20°, and 30° nasal (10 N, 20 N, 30 N) and temporal (10 T, 20 T, 30 T) retina were measured at six-month intervals for children who participated in a 2-year randomized controlled trial. The relationship between the baseline peripheral refractions and myopia progression and axial length changes were analysed. A total of 79 children and 81 children in the DIMS and single vision (SV) group were investigated, respectively. In the DIMS group, more baseline myopic RPR spherical equivalent (SE) was associated with more myopic progression (10 N: r = 0.36, p = 0.001; 20 N: r = 0.35, p = 0.001) and greater axial elongation (10 N: r = −0.34, p = 0.001; 20 N: r = −0.29, p = 0.006) after adjusting for co-factors. In the SV group, baseline RPR had association with only myopia progression (10 N: r = 0.37, p = 0.001; 20 N: r = 0.36, p = 0.001; 30 N: r = 0.35, p = 0.002) but not with axial elongation after Bonferroni correction (p > 0.008). No statistically significant relationship was found between temporal retina and myopia progression or axial elongation in both groups. Children with baseline myopic RPR had statistically significant more myopia progression (mean difference around −0.40 D) and more axial elongation (mean difference 0.15 mm) when compared with the children having baseline hyperopic RPR in the DIMS group but not in the SV group. In conclusion, the baseline RPR profile may not influence future myopia progression or axial elongation for the SV lens wearers. However, DIMS lenses slowed down myopia progression and was better in myopia control for the children with baseline hyperopic RPR than the children with myopic RPR. This may partially explain why myopia control effects vary among myopic children. Customised myopic defocus for individuals may optimise myopia control effects, and further research to determine the optimal dosage, with consideration of peripheral retinal profile, is warranted.

Abstract:Myopia is an important public health problem due to its prevalence and significant public health cost. Elevating levels of myopia increase the risk of vision impairment, and therefore, high myopia has become one of the main causes of untreatable vision loss throughout the world due to its irreversible complications. At present, many options for slowing progression of myopia have already been proposed and evaluated such as progressive addition of executive bifocal spectacle lenses, peripheral defocusing lenses, overnight orthokeratology, pharmacological agents such as atropine eye drops, and multifocal soft contact lenses (MFSCLs). Use of MFSCLs has especially increased in recent years due to the growing demand to slow myopia progression during patient's adolescent growth period to avoid pathological myopia in adulthood. Compared with the other traditional methods of controlling myopia, MFSCLs allow myopic patients to better maintain their clear visual quality and slow myopia progression. In this manuscript, we aim to review the basics of myopia, recent advances in contact lenses to control myopia with emphasis on MFSCLs, define the elements for proper MFSCL fittings (such as pupil size, aberrations, accommodation and centering), discuss the potential rebound effect after discontinuation of contact lenses, and future directions for improvements of contact lenses for the control of myopia.

Abstract:Myopia has become an increasing health problem worldwide, particularly in some East Asian countries. This has led to many attempts to slow its progression in children. Many of these attempts have involved optical interventions, including blue- and red-light therapies, more outdoor time (increased illumination), orthokeratology (the overnight flattening of the central cornea to make optical correction unnecessary during the day), spectacle lenses, and contact lenses. The theoretical justifications of the last two include (1) that children who go on to develop myopia or already have myopia have poor accommodation response 1 so that the consequent hyperopic defocus with near vision tasks stimulates eye growth 2 and (2) that a relative hyperopic peripheral refraction is the cause of eye growth. The second of these theories is the basis for many of the available treatment options and is supported by animal studies. 3 The foundation of this theory is dubious, 4,5 however, and there is clinical evidence that relative peripheral hyperopia is purely a consequence of the development of myopia and not the other way around. 6–8 – 8 Nevertheless, correction of the relative peripheral hyperopia may still be of benefit, and recent lenses designed in accordance with the theory have established clinical benefits in slowing myopia progression. 9

Abstract:The IEEE International Conference on Software Maintenance (ICSM) is the premiere international venue in software maintenance and evolution, where participants from academia, government, and industry meet and share ideas and experiences for solving critical software maintenance problems. In 2011, the 27th edition of ICSM was held at the Williamsburg Lodge, in the heart of Williamsburg, VA, USA. Williamsburg, once the capital of England’s oldest, wealthiest, and most populous North American colony, is one of the most important American historical landmarks. ICSM 2011 was held from September 25 to 30, 2011. The conference was preceded by two colocated events, the 11th IEEE International Working Conference on Source Code Analysis and Manipulation (SCAM) and the International Workshop on the Maintenance and Evolution of Service-Oriented and Cloud-Based Systems (MESOCA). It was followed by two other co-located events, the 13th IEEE International Symposium on Web Systems Evolution (WSE) and the 6th IEEE International Workshop on Visualizing Software for Understanding and Analysis (VISSOFT). ICSM 2011 attracted 127 submissions, each of which was reviewed by at least three members of the ICSM Program Committee. Following a week and a half of online discussion supported by EasyChair, 36 papers were selected for publication and presentation in the Technical Program of the conference. Consensus was aimed for and reached in almost all cases. The topics of accepted papers ranged from Reverse Engineering and Program Comprehension to Impact Analysis, Traceability, Migration and Evolution, Refactoring, Software Clones, Linguistic Analysis, and Regression Testing. The conference program included the ICSM Doctoral Symposium, Tutorials, Tool Demonstrations, Posters, the Industry track, and the Early Research Achievements track. This special section of the Journal of Software: Evolution and Process contains extended versions of the papers selected by the ICSM 2011 Program Committee among the best papers presented at the conference. These extended versions have additionally undergone the rigorous JSEP journal review process: they were reviewed by three anonymous referees under the supervision and coordination of the guest editors. We are proud to present you the three excellent papers that are the results of this process. Paper [1], ‘An empirical study of faults in late propagation clone genealogies’, by Liliane Barbour, Foutse Khomh, and Ying Zou, investigates the history of clone evolution over time, focusing on the phenomenon of late propagation. Late propagation occurs whenever one of two clones diverges from the other, as a result of code evolution, while later such changes are reconciled, so as to remove the divergence. The authors have defined eight types of late propagation, and they have evaluated empirically the relationship between fault proneness and late propagation of various type. They have also investigated whether reconciling changes are fault-fixing changes. This paper contributes to our knowledge of the risks associated with clones and with their evolution over time. Paper [2], ‘Evaluating test-to-code traceability recovery methods through controlled experiments’, by Abdallah Qusef, Gabriele Bavota, Rocco Oliveto, Andrea De Lucia, and David Binkley, presents a combination of two controlled experiments comparing the effectiveness of four different approaches to recovering traceability links between test cases and code. Traceability addresses the problem of identifying links between software artifacts of different types, and test-to-code traceability addresses the issue of identifying precisely which sections of code are actually tested by a given test case. In the experiments presented in this paper, the effectiveness of the four approaches on three large Java systems is compared using a manual consensus of three PhD student programmers as a reference answer. Both accuracy and practical usefulness to programmers are explored, demonstrating that the authors’ own SCOTCH method can be significantly more effective than existing techniques. Abstract:AIM:To characterize peripheral refraction and its relationship with myopia development in a selected group of male teenage Chinese students. METHODS:This 2-year prospective cohort study randomly enrolled 85 non-myopic boys (age, 14-16y) from the Experimental Class of Air Force in China. Cycloplegic peripheral refraction was examined at 0°, ±10°, and ±20° along the horizontal visual field in the right eye at the baseline and 2-year follow-up. RESULTS:The incidence of myopia at the 2-year follow-up was 15.29% (13/85). The baseline central refraction (CR) and peripheral refraction at ±10° were significantly lower in students who developed myopia than in those who did not (P<0.05). Relative peripheral refraction (RPR) did not differ between students with and without myopia (P>0.05). At the 2-year follow-up, the RPR at ±10° and 20° nasal was significantly more hyperopic in the myopic group than in the non-myopic group. Multiple linear regression analysis indicated that the change in CR was significantly correlated with the changes in RPR at 20° nasal, 10° nasal, and 20° temporal. Multivariate Logistic regression analysis indicated that the baseline CR [odds ratio (OR): 0.092, 95% confidence interval (CI): 0.012-0.688, P=0.020] and the baseline RPR at 10° nasal (OR: 0.182, 95%CI: 0.042-0.799, P=0.024) were significantly correlated with incident myopia (Omnibus test, χ 2=10.20, P=0.006). CONCLUSION:CR change is significantly correlated with changes in RPR, and students who develop myopia have more relative peripheral hyperopia. More baseline CR and relative peripheral hyperopia at 10° nasal are protective of myopia onset.

Abstract:The prevalence of myopia is growing at an alarming rate and is associated with axial elongation of the eye. The cause of this undesirable physiological change involves multiple factors. When the magnitude of myopia approaches high levels, this accompanying mechanical effect increases the risk of developing other clinical conditions associated with permanent vision loss. Prior work has investigated how we may halt or reverse this process of axial elongation associated with myopic progression when we expose the eye to a peripheral myopic defocus stimulus. Specifically, the known, short-term response to myopic defocus stimulation is promising and demonstrates the possibility of establishing more permanent effects by regulating the axial length of the eye with specific defocus stimulation. However, how to directly convert these known, short-term effects into more long-term, permanent changes to effectively prevent these unfavourable physiological and refractive changes over time is yet to be understood. Here, we show for the first time that we can produce sustained, long-term reductions in axial length and refractive endpoints with cumulative short-term exposure to specific myopic defocus stimuli using a novel optical design that incorporates an augmented reality optical system. We believe that this technology will have the potential to improve the quality of vision in mankind.

Abstract:Myopia is one of the most common eye diseases causing visual impairment and blindness, and the high prevalence in adolescents remains a major public health concern. Based on clinical studies using optical defocus to regulate ocular growth and refractive changes through visual feedback, we hypothesize that early wearing of peripheral myopic defocusing spectacles in children with high risk of myopia may slow the process of emmetropization and even prevent the onset of myopia by inducing more peripheral myopic defocus. The aim of this study is to investigate whether the wearing of peripheral focus-out glasses can be effective in delaying emmetropization in non-myopic children aged 6–8 years.

Abstract:Purpose To evaluate the short‐term effects of three myopia‐control lenses, which impose peripheral myopic defocus while providing clear central vision, on central and peripheral visual performance in myopic children. Methods Twenty‐one myopic children were enrolled in the study. Central visual performance was assessed using the quick contrast sensitivity function. Peripheral visual performance was evaluated by measuring peripheral contrast threshold and global motion perception, while subjects maintained fixation through the central portion of the lens. Single‐vision spectacle lenses (SVL), spectacle lenses with highly aspherical lenslets (HAL) and defocus‐incorporated soft contact (DISC) lenses were evaluated in random order, followed by orthokeratology (OK) lenses. Abstract:Progressive myopia remains one of the urgent problems in modern ophthalmology. Optical methods for controlling progressive myopia suggest the formation of peripheral myopic defocus, which has a stabilizing effect on the axial length of the eye and, as a result, the degree of myopia. One option for optical control of myopia is defocus soft contact lenses. Purpose : to evaluate the growth of axial length and changes in the spherical equivalent of clinical refraction in users of defocus (bifocal) contact lenses and monofocal glasses expressed in diopters, against the background of the correction of progressive myopia with defocus (bifocal) contact lenses and monofocal glasses. Materials and methods. The study involved 256 patients with bilateral mild to moderate and high myopia in the range from (–)0.5 to (–) 7.75 D in spherical equivalent and an annual progression gradient from 0.65 to 1.5 D in the age categories 7–10, 11–14, 15–18 years old. Depending on the degree of myopia and the method of correction, the patients were divided into main and control groups. For correction in the main groups, defocused (with an addition of +4.0 D) soft contact lenses (SCLs) were used. In the control groups monofocal glasses were used. All patients underwent a standard ophthalmological examination. Clinical refraction data and axial length of the eye were used as criteria for evaluating the results. The follow-up period was 3, 6, 12 months. Results . After 12 months of wearing bifocal soft contact lenses, there was a statistically significant slowdown in the growth of the axial length and the spherical equivalent of refraction Higher rates of progression of myopia were in young children (7–10 years). Myopia at the age of 15–18 years is characterized by a decrease in the rate of progression with any type of optical correction and in most cases does not require the appointment of correction with the formation of peripheral myopic defocus. Conclusion. The results obtained indicate the effectiveness of the use of bifocal SCLs in the control of progressive mild, moderate and high myopia. The main stabilization factor is the formation of peripheral myopic defocus on the retina, followed by a slowdown in the growth of the axial length of the eye and the degree of myopia (spherical equivalent of clinical refraction).
