Abstract:Objective To explore the factors raising the effective ratio of cardiopulmonary resuscitation (CPR) in cardiac arrest (CA).Methods A retrospective analysis was done on the clinical data of CPR in 87 CA patients.The clinical data included the age,the basic disease before CA,the starting time of electric defibrillation and CPR,the duration of CPR,the starting time of artificial respiration.Results In 87 cases,49 were successfully resuscitated(56.3%),while 38 were unsuccessful(43.7%).There were significant differences between the patients who were successfully resuscitated and those who were unsuccessful in the distribution of etiological factor,the age,the starting time of electric defibrillation and CPR,the duration of CPR (P0.05),but the starting time of artificial respiration were similar(P0.05).Adrenalin of 1mg was used at first,then the dosage of adrenalin was added,which was applied to 21 patients who were successfully resuscitated,and to 9 patients who were not.Conclusion The major factors affecting the effective rate of CPR are the primary disease,the age,the starting time of electric defibrillation and CPR and the duration of CPR;Building the artificial circulation immediately and the effective combination of the dosage and the time of using adrenaline are the key measures to raise the effective rate of cardiopulmonary resuscitation.
[Prognostic value of hemoglobin-to-red cell distribution width ratio in patients with cardiopulmonary resuscitation after out-of-hospital cardiac arrest]

Abstract:Objective: To investigate the prognostic value of hemoglobin-to-red cell distribution width ratio (HRR) in patients with cardiopulmonary resuscitation (CPR) after out-of-hospital cardiac arrest (OHCA). Methods: A retrospective study was conducted. Patients aged ≥ 18 years with OHCA who were transferred to intensive care unit (ICU) after successful CPR from the emergency room of the First Affiliated Hospital of Zhengzhou University from August 2016 to February 2022 were enrolled. General clinical data, initial vital signs, acute physiology and chronic health evaluation II (APACHE II), Glasgow coma scale (GCS), first laboratory indicators after admission to ICU [including white blood cell count (WBC), red blood cell count (RBC), hemoglobin (Hb), pH value, lactic acid (Lac), 6-hour lactic acid clearance (LCR), red cell distribution width (RDW), HRR], length of ICU stay were collected. According to whether the patients died in hospital, the patients were divided into survival group and death group. Binary Logistic regression was used to analyze the independent factors influencing the prognosis of patients after CPR. Receiver operator characteristic curve (ROC curve) was drawn to analyze the predictive value of independent influencing factors for the prognosis of patients after CPR. Results: A total of 122 patients were enrolled after OHCA CPR, of which 88 died in hospital, the in-hospital mortality was 72.13%. There were no significant differences in age, past medical history, initial vital signs and WBC in ICU between the two groups. Compared with the death group, the survival group had higher GCS score, RBC, Hb, pH value, 6-hour LCR, HRR, lower APACHE II score, Lac, RDW level, and longer length of ICU stay. Multivariate Logistic regression analysis showed that APACHE II score, GCS score, 6-hour LCR, HRR, length of ICU stay were independent factors influencing the prognosis of patients after CPR [APACHE II score: odds ratio (OR) = 0.784, 95% confidence interval (95%CI) was 0.683-0.901, P = 0.001; GCS score: OR = 1.390, 95%CI was 1.059-1.823, P = 0.018; 6-hour LCR: OR = 1.039, 95%CI was 1.015-1.064, P = 0.001; HRR: OR = 2.047, 95%CI was 1.383-3.029, P < 0.001; length of ICU stay: OR = 1.128, 95%CI was 1.046-1.216, P = 0.002]. ROC curve analysis showed that HRR, 6-hour LCR and APACHE II score could predict the prognosis of patients after CPR. The sensitivity was 85.3% and the specificity was 54.5% when the area under the ROC curve (AUC) of HRR was 0.731, and the cut-off value was 8.555. The sensitivity was 88.2% and the specificity was 46.6%, when the AUC of 6-hour LCR was 0.701, and the cut-off value was 28.947%. The sensitivity was 73.9% and the specificity was 79.4% when the AUC of APACHE II score was 0.848, the cut-off value was 22.000. The predictive value of the combination of HRR and 6-hour LCR was higher than that of a single index. The sensitivity was 79.3% and the specificity was 76.1%, when the AUC was 0.796, the cut-off value was 0.296. Conclusions: HRR, 6-hour LCR and APACHE II score have high prognostic value in patients with OHCA after CPR. HRR < 8.555, 6-hour LCR < 28.947% and APACHE II score > 22.000 indicated poor prognosis.
A model for predicting return of spontaneous circulation and neurological outcomes in adults after in-hospital cardiac arrest: development and evaluation

Abstract:Introduction: In-hospital CA (IHCA) is associated with rates of high incidence, low return of spontaneous circulation (ROSC), low survival to discharge, and poor neurological outcomes. We aimed to construct and evaluate prediction models for non-return of spontaneous circulation (non-ROSC) and poor neurological outcomes 12 months after ROSC (PNO-12). Methods: We retrospectively analyzed baseline and clinical data from patients experiencing cardiac arrest (CA) in a big academic hospital of Jilin University in China. Patients experiencing CA between September 1, 2019 and December 31, 2020 were categorized into the ROSC and non-ROSC groups. Patients maintaining ROSC >20 min were divided into the good and PNO-12 subgroups. Results: Univariate and multivariate logistic regression identified independent factors associated with non-ROSC and PNO-12. Two nomogram prediction models were constructed and evaluated. Of 2,129 patients with IHCA, 851 were included in the study. Multivariate logistic regression analysis revealed that male sex, age >80 years, CPR duration >23 min, and total dose of adrenaline >3 mg were significant risk factors for non-ROSC. Before CA, combined arrhythmia, initial defibrillation rhythm, and advanced airway management (mainly as endotracheal intubation) also influenced outcomes. The area under the receiver operating characteristic curve in the prediction model was 0.904 (C-index: 0.901). Respiratory failure, shock, CA in the monitoring area, advanced airway management, and noradrenaline administration were independent risk factors for PNO-12. The AUC was 0.912 (C-index: 0.918). Conclusions: Prediction models based on IHCA data could be helpful to reduce mortality rates and improve prognosis.

Abstract:Objective To evaluate the status of implementation of the chain of survival and the gap between the guideline's recommendations and clinical practice as well as to analyze the factors influencing the prognosis of cardiac arrest (CA) patients.Methods A retrospective analysis of CA in adult patients admitted to Emergency Department of Peking University Third Hospital from January 2012 to December 2013 was carried out.The epidemiology,clinical features,implementations of the chain of survival and outcome were compared between out-of-hospital cardiac arrest (OHCA) and in-hospital cardiac arrest (IHCA) patients,with regard to the analysis of the predictors for survival and neurological outcome.Results A total of 414 patients with 69.8％ male and average age of (61.7 ± 18.0) years were divided into two groups,OHCA group (n =190) and IHCA group (n =224).Cardiogenic cause was found in 30％ of CA patients.There were 27.5％ patients with restoration of spontaneous circulation (ROSC),8.2％ patients discharged in survival and 3.1％ patients with good neurologic outcome (CPC =1 and 2).There were higher proportion of medical responders arriving to CA patients within 5 minutes after onset (99.1％ vs.10.5％,P ＜0.01),bystander carrying out cardiopulmonary resuscitation (100％ vs.15.3％,P ＜0.01),CPR initiated in 5 minutes (98.7％ vs.11.1％,P ＜ 0.01),defibrillation performed in 5 minutes (87.5％ vs.12.5％,P ＜ 0.01) in IHCA group compared with OHCA.There were no statistical differences in epinephrine administration and epinephrine dose,and targeted temperature management between two groups.There were higher proportion of ROSC (37.1％ vs.16.3％,P ＜ 0.05),higher percentage of survivals discharged (31.0％ vs.22.6％,P =0.002) and good neurologic outcome with CPC =1 or 2 (48.1％ vs.0.0％,P =0.029) in IHCA group compared with OHCA.Location of CA occurred and initial arrhythmia rectifiable with defibrillation treatment after ROSC were the favorable predictors for assessing the percentages of ROSC and survivals discharged.In contrast,male and age over 65 years were the unfavorable predictors of ROSC.Conclusions Improvement in outcome of victims with CA is required in every link of the chain of survival,especially in prehospital rescue act,bystander carrying out CPR,defibrillation,and therapeutic hypothermia in unconscious patients after resuscitation.The effective implementation of chain of survival concept can improve the prognosis of CA patients.
A Comparison of Prognostic Factors in a Large Cohort of In-Hospital and Out-of-Hospital Cardiac Arrest Patients

Abstract:We investigated independent factors predicting neurological outcome and death, comparing in-hospital (IHCA) and out-of-hospital cardiac arrest (OHCA) patients. The study was conducted in the mixed 34-bed Intensive Care Department at the Hôpital Universitaire de Bruxelles (HUB), Belgium. All adult consecutive cardiac arrest (CA) survivors were included between 2004 and 2022. For all patients, demographic data, medical comorbidities, CA baseline characteristics, treatments received during Intensive Care Unit stay, in-hospital major complications, and neurological outcome at three months after CA, using the Cerebral Performance Category (CPC) scale, were collected. In the multivariable analysis, in the IHCA group (n = 540), time to return of spontaneous circulation (ROSC), older age, unwitnessed CA, higher lactate on admission, asystole as initial rhythm, a non-cardiac cause of CA, the occurrence of shock, the occurrence of acute kidney injury (AKI), and the presence of previous neurological disease and of liver cirrhosis were independent predictors of an unfavorable neurological outcome. Among patients with OHCA (n = 567), time to ROSC, older age, higher lactate level on admission, unwitnessed CA, asystole or pulseless electrical activity (PEA) as initial rhythm, the occurrence of shock, a non-cardiac cause of CA, and a previous neurological disease were independent predictors of an unfavorable neurological outcome. To conclude, in our large cohort of mixed IHCA and OHCA patients, we observed numerous factors independently associated with a poor neurological outcome, with minimal differences between the two groups, reflecting the greater vulnerability of hospitalized patients.

Abstract:BACKGROUND: The outcome of cardiopulmonary resuscitation (CPR) may depend on a variety of factors related to patient status or resuscitation management. To evaluate the factors influencing the outcome of CPR after cardiac arrest (CA) will be conducive to improve the effectiveness of resuscitation. Therefore, a study was designed to assess these factors in the emergency department (ED) of a city hospital.METHODS: A CPR registry conforming to the Utstein-style template was conducted in the ED of the First Affiliated Hospital of Wenzhou Medical College from January 2005 to December 2011. The outcomes of CPR were compared in various factors groups. The primary outcomes were rated to return of spontaneous circulation (ROSC), 24-hour survival, survival to discharge and discharge with favorable neurological outcomes. Univariate analysis and multivariable logistic regression analysis were performed to evaluate factors associated with survival.RESULTS: A total of 725 patients were analyzed in the study. Of these patients, 187 (25.8%) had ROSC, 100 (13.8%) survived for 24 hours, 48 (6.6%) survived to discharge, and 23 (3.2%) survived to discharge with favorable neurologic outcomes. A logistic regression analysis demonstrated that the independent predictors of ROSC included traumatic etiology, first monitored rhythms, CPR duration, and total adrenaline dose. The independent predictors of 24-hour survival included traumatic etiology, cardiac etiology, first monitored rhythm and CPR duration. Previous status, cardiac etiology, first monitored rhythms and CPR duration were included in independent predictors of survival to discharge and neurologically favorable survival to discharge.CONCLUSIONS: Shockable rhythms, CPR duration ≤15 minutes and total adrenaline dose ≤5 mg were favorable predictors of ROSC, whereas traumatic etiology was unfavorable. Cardiac etiology, shockable rhythms and CPR duration ≤15 minutes were favorable predictors of 24-hour survival, whereas traumatic etiology was unfavorable. Cardiac etiology, shockable rhythms, CPR duration ≤15 minutes were favorable predictors of survival to discharge and neurologically favorable survival to discharge, but previous terminal illness or multiple organ failure (MOF) was unfavorable.

Aetiology and predictors of outcome in non-shockable in-hospital cardiac arrest: A retrospective cohort study from the Swedish Registry for Cardiopulmonary Resuscitation

Abstract:Background: Non-shockable in-hospital cardiac arrest (IHCA) is a condition with diverse aetiology, predictive factors, and outcome. This study aimed to compare IHCA with initial asystole or pulseless electrical activity (PEA), focusing specifically on their aetiologies and the significance of predictive factors. Methods: Using the Swedish Registry of Cardiopulmonary Resuscitation, adult non-shockable IHCA cases from 2018 to 2022 (n = 5788) were analysed. Exposure was initial rhythm, while survival to hospital discharge was the primary outcome. A random forest model with 28 variables was used to generate permutation-based variable importance for outcome prediction. Results: Overall, 60% of patients (n = 3486) were male and the median age was 75 years (IQR 67-81). The most frequent arrest location (46%) was on general wards. Comorbidities were present in 79% of cases and the most prevalent comorbidity was heart failure (33%). Initial rhythm was PEA in 47% (n = 2702) of patients, and asystole in 53% (n = 3086). The most frequent aetiologies in both PEA and asystole were cardiac ischemia (24% vs. 19%, absolute difference [AD]: 5.4%; 95% confidence interval [CI] 3.0% to 7.7%), and respiratory failure (14% vs. 13%, no significant difference). Survival was higher in asystole (24%) than in PEA (17%) (AD: 7.3%; 95% CI 5.2% to 9.4%). Cardiopulmonary resuscitation (CPR) durations were longer in PEA, 18 vs 15 min (AD 4.9 min, 95% CI 4.0-5.9 min). The duration of CPR was the single most important predictor of survival across all subgroup and sensitivity analyses. Aetiology ranked as the second most important predictor in most analyses, except in the asystole subgroup where responsiveness at cardiac arrest team arrival took precedence. Conclusions: In this nationwide registry study of non-shockable IHCA comparing asystole to PEA, cardiac ischemia and respiratory failure were the predominant aetiologies. Duration of CPR was the most important predictor of survival, followed by aetiology. Asystole was associated with higher survival compared to PEA, possibly due to shorter CPR durations and a larger proportion of reversible aetiologies.
Risk factors of prognosis in patients with non-traumatic out-of-hospital cardiac arrest and development of a nomogram prediction model

Abstract:Objective:To analyze risk factors for death at 28 days after return of spontaneous circulation (ROSC) in adult non-traumatic out-of-hospital cardiac arrest (OHCA) patients and to develop a nomogram model.Methods:A total of 134 patients with non-traumatic OHCA and ROSC admitted to the emergency rescue room at the Fourth Affiliated Hospital, Zhejiang University School of Medicine from October 22, 2015 to June 30, 2022 were included. According to their death status within 28 days after ROSC, all patients were divided into a survival group (45 cases) and a death group (89 cases). Clinical data of the two groups were collected and univariate analysis was performed. Independent variables were screened by optimal subset regression analysis with minimum Bayesian information criterion (BIC) values. Multivariate logistic regression analysis was used to identify independent risk factors for death at 28 days after ROSC, and a nomogram prediction model was constructed. The receiver operating characteristic (ROC) curve, calibration curve and decision curve analysis were used to evaluate the nomogram model. Finally, the area under the curve (AUC) of acute physiology and chronic health evaluation (APACHE) Ⅱ score and the nomogram model was compared.Results:Multivariate logistic regression showed that blood potassium [odds ratio (OR) = 1.454, 95% confidence interval (CI) (1.007, 2.098), P = 0.046], creatine kinase isoenzyme MB (CK-MB) [OR = 1.024, 95%CI (1.008, 1.041), P = 0.004], uric acid [OR = 1.005, 95%CI (1.001, 1.010), P = 0.013] and adrenaline dosage [OR = 1.282, 95%CI (1.080, 1.522), P = 0.004] were independent risk factors for death at 28 days after ROSC in non-traumatic OHCA patients, and the shockable rhythm was a protective factor [OR = 0.054, 95%CI (0.014, 0.211), P < 0.001]. Based on the above risk factors, a nomogram was constructed for predicting mortality risk of OHCA patients. The AUC of the nomogram was 0.872 [95%CI (0.812, 0.932), P < 0.001]. The calibration curve showed that the predicted and actual probabilities were consistent, and the decision curve analysis showed that the nomogram had a good clinical benefit. In addition, the AUC and 95%CI of APACHEⅡ score was 0.773 (0.688, 0.858), with P < 0.001, and the AUC of the nomogram model was significantly better than that of the APACHEⅡ score (Z = 1.994, P = 0.046).Conclusions:The shockable rhythm, the dosage of adrenaline, and the first blood potassium, blood uric acid and CK-MB after admission to the emergency department are independent influencing factors for 28-day mortality in non-traumatic OHCA patients after ROSC. The prediction model based on the above factors has a good clinical practical value.
Development of prognostic models for predicting 90-day neurological function and mortality after cardiac arrest

Abstract:Background The survivors of cardiac arrest experienced vary extent of hypoxic ischemic brain injury causing mortality and long-term neurologic disability. However, there is still a need to develop robust and reliable prognostic models that can accurately predict these outcomes. Objectives To establish reliable models for predicting 90-day neurological function and mortality in adult ICU patients recovering from cardiac arrest. Methods We enrolled patients who had recovered from cardiac arrest at Binhaiwan Central Hospital of Dongguan, from January 2018 to July 2021. The study's primary outcome was 90-day neurological function, assessed and divided into two categories using the Cerebral Performance Category (CPC) scale: either good (CPC 1–2) or poor (CPC 3–5). The secondary outcome was 90-day mortality. We analyzed the relationships between risk factors and outcomes individually. A total of four models were developed: two multivariable logistic regression models (models 1 and 2) for predicting neurological function, and two Cox regression models (models 3 and 4) for predicting mortality. Models 2 and 4 included new neurological biomarkers as predictor variables, while models 1 and 3 excluded. We evaluated calibration, discrimination, clinical utility, and relative performance to establish superiority between the models. Results Model 1 incorporates variables such as gender, site of cardiopulmonary resuscitation (CPR), total CPR time, and acute physiology and chronic health evaluation II (APACHE II), while model 2 includes gender, site of CPR, APACHE II, and serum level of serum level of ubiquitin carboxy-terminal hydrolase L1 (UCH-L1). Model 2 outperforms model 1, showcasing a superior area under the receiver operating characteristic curve (AUC) of 0.97 compared to 0.83. Additionally, model 2 exhibits improved accuracy, sensitivity, and specificity. The decision curve analysis confirms the net benefit of model 2. Similarly, models 3 and 4 are designed to predict 90-day mortality. Model 3 incorporates the variables site of CPR, total CPR time, and APACHE II, while model 4 includes APACHE II, total CPR time, and serum level of UCH-L1. Model 4 outperforms model 3, showcasing an AUC of 0.926 and a C-index of 0.830. The clinical decision curve analysis also confirms the net benefit of model 4. Conclusions By integrating new neurological biomarkers, we have successfully developed enhanced models that can predict 90-day neurological function and mortality outcomes more accurately.

Abstract:Background: Out-of-hospital cardiac arrest (OHCA) is a critical health issue with survival influenced by multiple factors. This study analyzed resuscitation outcomes at the County Clinical Emergency Hospital of Sibiu, Romania, during pre-COVID-19 and pandemic periods. Methods: A retrospective analysis of 508 OHCA patients (2017–2020) assessed the return of spontaneous circulation (ROSC) as the primary endpoint. Statistical methods included decision tree analysis, logistic regression and ROC curve analysis to evaluate the predictive value of adrenaline dose and patient factors. Results: The mortality rate was 68.7%, with non-shockable rhythms predominant among fatalities. Rural patients, though younger, had lower ROSC rates than urban counterparts. Logistic regression showed that lower adrenaline doses (≤4 mg, OR 11.835 [95% CI: 6.726–20.27]; 4–6 mg, OR 2.990 [95% CI: 1.773–5.042]) were associated with better ROSC outcomes. Conclusions: A multivariable model (AUC = 0.773) incorporating demographics and pandemic status outperformed adrenaline dose alone (AUC = 0.711).

Blood glucose increases early after stroke onset: a study on serial measurements of blood glucose in acute stroke

Abstract:The aim of this study was to evaluate if blood glucose levels change within the first 12 h after stroke onset and to investigate if the degree of change is related to stroke severity, type of stroke, or prognosis. This was a retrospective, descriptive trial based on 445 stroke patients with two blood glucose tests within 12 h of stroke onset and no history of diabetes mellitus. Blood glucose increased in the first 12 h after stroke onset; in mild to moderate stroke from 5.8 to 6.1 mmol/l (P < 0.001) and in severe stroke from 6.2 to 6.7 mmol/l (P < 0.001). In patients who died within 7 days of stroke onset, blood glucose increased from 6.8 to 7.1 mmol/l (P < 0.001). In conclusion, blood glucose increases after the onset of acute stroke and the increase is related to the severity of the stroke.

Abstract:Background: Cardiac etiologies arrest accounts for almost half of all in-hospital cardiac arrest (IHCA), and previous studies have shown that the location of IHCA is an important factor affecting patient outcomes. The aim was to compare the characteristics, causes and outcomes of cardiovascular disease in patients suffering IHCA from different departments of Fuwai hospital in Beijing, China. Methods: We included patients who were resuscitated after IHCA at Fuwai hospital between March 2017 and August 2022. We categorized the departments where cardiac arrest occurred as cardiac surgical or non-surgical units. Independent predictors of in-hospital survival were assessed by logistic regression. Results: A total of 119 patients with IHCA were analysed, 58 (48.7%) patients with cardiac arrest were in non-surgical units, and 61 (51.3%) were in cardiac surgical units. In non-surgical units, acute myocardial infarction/cardiogenic shock (48.3%) was the main cause of IHCA. Cardiac arrest in cardiac surgical units occurred mainly in patients who were planning or had undergone complex aortic replacement (32.8%). Shockable rhythms (ventricular fibrillation/ventricular tachycardia) were observed in approximately one-third of all initial rhythms in both units. Patients who suffered cardiac arrest in cardiac surgical units were more likely to return to spontaneous circulation (59.0% vs. 24.1%) and survive to hospital discharge (40.0% vs. 10.2%). On multivariable regression analysis, IHCA in cardiac surgical units (OR 5.39, 95% CI 1.90-15.26) and a shorter duration of resuscitation efforts (≤ 30 min) (OR 6.76, 95% CI 2.27-20.09) were associated with greater survival rate at discharge. Conclusion: IHCA occurring in cardiac surgical units and a duration of resuscitation efforts less than 30 min were associated with potentially increased rates of survival to discharge.
Evaluation of the spectral transmittance of UV-absorbing disposable contact lenses.

Abstract:PURPOSE Increasing evidence suggesting deleterious effects of ultraviolet radiation (UVR) on the eye has prompted manufacturers to develop UV-absorbing disposable contact lenses. The spectral transmittances of a sample of these lenses were measured in order to determine if they provide adequate protection from UVR. METHODS Four types of UV-absorbing disposable lenses were studied (Acuvue [Vistakon, Jacksonville, FL], Precision UV [Wesley-Jessen, Des Plaines, IL], Specialty Sport [Specialty UltraVision, Sunnyvale, CA], and Surevue [Vistakon]). For comparison, a lens without UV-absorbing properties (Acuvue), a conventional soft lens (Permaflex UV [Cooper Vision, San Jose, CA]) and an RGP lens (Boston ES [Polymer Technology, Rochester, NY]) were also evaluated. The tested lens was placed in a silica wet cell filled with saline. Spectral transmittance was measured over the 200-800 nm waveband. RESULTS Each disposable lens exhibited a transmittance window in the 240-320 nm waveband. Computing indices of protection factor and safe exposure duration allowed for the assessment of the level of protection afforded by each lens. Among disposable lenses studied, the Surevue performed best with a transmittance of less than 1% in the waveband 280-348 nm and the longest safe exposure duration, while the Specialty Sport had the highesttransmittance of UVB (290-315 nm). CONCLUSION All UV-absorbing lenses studied provided a better level of protection from UVR exposure than lenses without UV absorber. These results will help the eyecare practitioner in prescribing the most appropriate UV-blocking lens for each patient.
Cardiac Arrest Survival Postresuscitation In-Hospital (CASPRI) Score Predicts Neurological Favorable Survival in Emergency Department Cardiac Arrest

Abstract:Background: This study was conducted to identify the predictive factors for survival and favorable neurological outcome in patients with emergency department cardiac arrest (EDCA). Methods: ED patients who suffered from in-hospital cardiac arrest (IHCA) from July 2014 to June 2019 were enrolled. The electronic medical records were retrieved and data were extracted according to the IHCA Utstein-style guidelines. Results: The cardiac arrest survival post-resuscitation in-hospital (CASPRI) score was associated with survival, and the CASPRI scores were lower in the survival group. Three components of the CASPRI score were associated with favorable neurological survival, and the CASPRI scores were lower in the favorable neurological survival group of patients who were successfully resuscitated. The independent predictors of survival were presence of hypotension/shock, metabolic illnesses, short resuscitation time, receiving coronary angiography, and TTM. Receiving coronary angiography and low CASPRI score independently predicted favorable neurological survival in resuscitated patients. The performance of a low CASPRI score for predicting favorable neurological survival was fair, with an AUROCC of 0.77. Conclusions: The CASPRI score can be used to predict survival and neurological status of patients with EDCA. Post-cardiac arrest care may be beneficial for IHCA, especially in patients with EDCA.

Characteristics and Outcome of Pediatric In-Hospital Cardiopulmonary Resuscitation

Abstract:Objective To investigate the present situation and outcome of cardiopulmonary resuscitation (CPR) for pediatric in-hospital cardiopulmonary arrest and to analyze the prognostic risk factors.Methods Data were collected from October 2008 till October 2011 using Ustein style.Patients older than 28 days who received CPR were evaluated.Returning of spontaneous circulation(ROSC) more than 24 hours was the primary outcome.Neurological outcome was assessed by pediatric cerebral performance categories half year after discharge.Results Of the 36 patients,15 (41.7％) achieved sustained ROSC.Seven (19.4％) patients survived to discharge.Single factor analysis indicated that the beginning heart rhythm,defibrillation and original disease were significantly different between the two groups(P ＜0.05).The beginning heart rote of the patient in ROSC ＞ 24 h group was mostly sinus bradycardia.Patients who need defibrillation had bad prognosis.Patients with heart disease had a lower rate of ROSC ＞ 24 h.At half year follow-up study,4 patients had 1 or 2 score,1 patient had 4 and 1 had 5 score in the pediatric cerebral performance categories scales.Condusion The successful rate of CPR in our hospital was the same as that in developed country.The beginning heart rhythm,defibrillation and original disease were associated with the outcome.Most of the patients who survived to discharge had a good neurological outcome.
Endoscopic endonasal resection of craniopharyngiomas: a case series and review of the literature

Abstract:Abstract: Background: Transcranial approaches were historically the first established routes for craniopharyngiomas resection. Transcranial approaches commonly entail some degree of brain retraction as well as manipulation of neurovascular structures, located between the surgeon and the pathogenic region. A more direct route to the sellar -suprasellar region is provided by the transsphenoidal approach. Introduction of extended approaches offered a safe alternative technique for reaching suprasellar craniopharyngiomas. The next stage of development was incorporation of the endoscope in the extended approach which increased the field of view and illumination. There is a limited body of literature on the outcome of endoscopic endonasal approach in resection of craniopharyngiomas. Therefore, the present study aims to assess the outcome of endoscopic endonasal approach in resection of craniopharyngiomas. Methods: Eight patients underwent surgery at Loghman-Hakim and Day General Hospitals (Tehran, Iran). One patient had recurrent lesion. The gross-total resection (GTR) was attempted in 6 surgeries. Indications for intended subtotal resection included advanced age, medical comorbidities, preservation of pituitary function, and hypothalamic invasion. Results: The average size of tumor diameter was 2.8 cm. GTR in 6 cases and near-total resection in 2 cases were achieved. The average follow-up period was 30 months during which one recurrence in near resection cases was observed. Vision was improved by 80%. Four cases developed diabetes insipidus (unrelated to diabetes mellitus) and 3 cases developed panhypopituitarism postoperatively. Furthermore, postoperative cerebrospinal fluid (CSF) leaks occurred in 2 patients, one of them needed reoperation for CSF leak management. In addition, postoperative bacterial meningitis occurred in one patient. Conclusions: Endoscopic, endonasal surgery for craniopharyngioma can be accompanied by high rates of GTR with low rates of CSF leak. Keywords: Craniopharyngioma, Endonasal, Endoscopic
The Potential Risk Factors for Mortality in Patients After In-Hospital Cardiac Arrest: A Multicenter Study

Abstract:Background and Purpose: In-hospital cardiac arrest (IHCA) has high mortality rate, which needs more research. This multi-center study aims to evaluate potential risk factors for mortality in patients after IHCA. Methods: Data for this study retrospectively enrolled IHCA patients from 14 regional hospitals, two district hospitals, and five medical centers between 2013 June and 2018 December. The study enrolled 5,306 patients and there were 2,871 patients in subgroup of intensive care unit (ICU) and emergency room (ER), and 1,894 patients in subgroup of general wards. Results: As for overall IHCA patients, odds ratio (OR) for mortality was higher in older patients (OR = 1.69; 95% CI:1.33–2.14), those treated with ventilator (OR = 1.79; 95% CI:1.36–2.38) and vasoactive agents (OR = 1.88; 95% CI:1.45–2.46). Whereas, better survival was reported in IHCA patients with initial rhythm as ventricular tachycardia (OR = 0.32; 95% CI: 0.21–0.50) and ventricular fibrillation (OR = 0.26; 95% CI: 0.16–0.42). With regard to ICU and ER subgroup, there was no mortality difference among different nursing shifts, whereas for patients in general wards, overnight shift (OR = 1.83; 95% CI: 1.07–3.11) leads to poor outcome. Conclusion: For IHCA patients, old age, receiving ventilator support and vasoactive agents reported poor survival. Overnight shift had poor survival for IHCA patients in general wards, despite no significance in overall and ICU/ER subgroups.

Abstract:Aim To assess the ability of clinical examination, biomarkers, electrophysiology and brain imaging, individually or in combination to predict good neurological outcomes at 6 months after CA. Methods This was a retrospective analysis of the Korean Hypothermia Network Prospective Registry 1.0, which included adult out-of-hospital cardiac arrest (OHCA) patients (≥18 years). Good outcome predictors were defined as both pupillary light reflex (PLR) and corneal reflex (CR) at admission, Glasgow Coma Scale Motor score (GCS-M) >3 at admission, neuron-specific enolase (NSE) 4 μV, continuous background without discharges on electroencephalogram (EEG), and absence of anoxic injury on brain CT and diffusion-weighted imaging (DWI). Results A total of 1327 subjects were included in the final analysis, and their median age was 59 years; among them, 412 subjects had a good neurological outcome at 6 months. GCS-M >3 at admission had the highest specificity of 96.7% (95% CI 95.3–97.8), and normal brain DWI had the highest sensitivity of 96.3% (95% CI 92.9–98.4). When the two predictors were combined, the sensitivities tended to decrease (ranging from 2.7–81.1%), and the specificities tended to increase, ranging from81.3–100%. Through the explorative variation of the 2021 European Resuscitation Council (ERC) and the European Society of Intensive Care Medicine (ESICM) prognostication strategy algorithms, good outcomes were predicted, with a specificity of 83.2% and a sensitivity of 83.5% in patients by the algorithm. Conclusions Clinical examination, biomarker, electrophysiology, and brain imaging predicted good outcomes at 6 months after CA. When the two predictors were combined, the specificity further improved. With the 2021 ERC/ESICM guidelines, the number of indeterminate patients and the uncertainty of prognostication can be reduced by using a good outcome prediction algorithm.

Abstract:OBJECTIVE:To provide guidance to implement effective perioperative cardiopulmonary resuscitation.METHODS:A total of 69 perioperative cardiac arrest patients who received Cardiopulmonary resuscitation according to the 2005 guidelines in Sir Run Run Shaw Hospital from January 2005 to June 2012 were included (including emergency operation). According to the results of cardiopulmonary resuscitation, divide these 69 patients in to two groups, Successful resuscitation group (27cases) and failed resuscitation group (42cases). Compare the age, gender, ASA physical status, BMI, type of anesthesia, time that cardiac arrest occurred, cardiac arrest type, amount of bleeding, CPR duration, hypotension duration, defibrillation time, combined cardiovascular disease between successful resuscitation group and failed resuscitation group, calculate the measurement data for the chi-square test and logistic regression, derive factors that affect the success rate of cardiopulmonary resuscitation.RESULTS:A total of 69 perioperative cardiac arrest patients, according to Logistic regression, ASA IV-V (P = 0.029,OR = 1.956, CI = 1.226-4.809), amount of bleeding more than 1500 ml(P = 0.023,OR = 1.615, CI = 1.245-2.313), CPR duration more than 30 min(P = 0.003,OR = 3.54, CI = 3.313-5.007), hypotension duration more than 30 min(P = 0.002,OR = 2.782, CI = 1.791-4.322), the type of cardiac arrest is pulseless electrical activity(P = 0.032,OR = 1.419, CI = 0.146-1.583).CONCLUSION:ASA IV-V, amount of bleeding more than 1500 ml, CPR duration more than 30 min, hypotension duration more than 30 min and pulseless electrical activity are the Independent risk factors of cardiopulmonary resuscitation.

Abstract:Characteristics of patients presenting with out-of-hospital cardiac arrest (OHCA) selected for coronary angiography (CA) and factors predicting in-hospital mortality remain unclear. We assessed clinical characteristics associated with undertaking CA in patients presenting with OHCA and shockable rhythm (CA group). Predictors of in-hospital mortality were evaluated with multivariable analysis. Of 1,552 patients presenting with cardiac arrest between 2014 and 2018 to 2 health services in Victoria, Australia, 213 patients with OHCA and shockable rhythm were stratified according to CA status. The CA group had shorter cardiopulmonary resuscitation duration (17 vs 25 minutes) and time to return of spontaneous circulation (17 vs 26 minutes) but higher proportion of ST-elevation on electrocardiogram (48% vs 24%) (all p <0.01). In-hospital mortality was 38% (n = 81) for the overall cohort, 32% (n = 54) in the CA group, and 61% (n = 27) in the no-CA group. Predictors of in-hospital mortality included non-selection for CA (odds ratio 4.5, 95% confidence interval 1.5 to 14), adrenaline support (3.9, 1.3 to 12), arrest at home (2.7, 1.1 to 6.6), longer time to defibrillation (2.5, 1.5 to 4.2 per 5-minute increase), lower blood pH (2.1, 1.4 to 3.2 per 0.1 decrease), lower albumin (2.0, 1.2 to 3.3 per 5 g/L decrease), higher Acute Physiology and Chronic Health Evaluation II score (1.7, 1.0 to 3.0 per 5-point increase), and advanced age (1.4, 1.0 to 2.0 per 10-year increase) (all p ≤0.05). In conclusion, non-selection for CA, concomitant cardiogenic shock requiring inotropic support, poor initial resuscitation (arrest at home, longer time to defibrillation and lower pH), greater burden of co-morbidities (higher Acute Physiology and Chronic Health Evaluation II score and lower albumin), and advanced age were key adverse prognostic indicators among patients with OHCA and shockable rhythm.
