Abstract:The aim of this study is to capture the enduring fascination with youth and transformation. These themes, epitomized by the myth of Medea, resonate in the context of modern plastic surgery and cosmetic enhancements. Book 7 of Metamorphosis was read and analyzed. Medea is a sorceress and daughter of King Aeëtes. Her magical abilities are highlighted in 2 notable stories: (1) her rejuvenation of Aeson and (2) her attempt to rejuvenate herself. Aeson, the father of Jason, had grown old and frail. Medea, eager to assist Jason's father, employed her magical abilities to restore Aeson's youth. Medea concocted a powerful potion that required a variety of rare ingredients and intricate rituals. This process involved herbs, incantations, and a cauldron for boiling the mixture. To prove her magical prowess to the onlookers, Medea performed a demonstration using an old ram. She dismembered the ram, boiled the pieces in her cauldron with her magical brew, and from the pot emerged a young lamb. Encouraged by this success, she then applied the same magical process to Aeson, rejuvenating him and restoring his vigor. Medea, having endured considerable personal loss and turmoil, resolved to use her magical abilities to restore her youth. She collected the required ingredients, conducted the rituals, and either drank the potion or bathed in it, which rejuvenated her. This act demonstrates her determination to transcend the constraints of mortality. These stories speak to the timeless human quest for youth and immortality while also exploring the themes of power, knowledge, and ethical boundaries.

Abstract:The topic of facial rejuvenation should be expected to be a recurring theme in a journal named Facial Plastic Surgery . I invite you to ponder that statement. The reason for this periodic reappearance of this topic is because of the amazing aspect of current human experience that is continually benefited from ongoing advances in science and medicine. These advances have not only resulted in the elimination of many illnesses and maladies but have also prolonged the active human lifespan. Peoples' health is considerably better than it was just a few decades ago. These advances continue and among such are frank endeavors to further longevity. As a result of this, as evidenced by the patients entering our practices, there is an enlarging and more sophisticated group of patients seeking physical and physiological rejuvenation. Their desire for rejuvenation of one's facial appearance is indeed part of the evolution of patients' desire for good health, youthfulness, longevity, and vitality. The field of facial rejuvenation has undergone parallel advances, like other aspect of medicine, with changes in surgical techniques and new technologies. Patients' growing desire for rejuvenation has been catalyzed by the internet and social media. Patients are continually presented with not only what is possible, but what is possible for them personally. The intermittent recurrence of the topic of facial rejuvenation in Facial Plastic Surgery , therefore, is not only understandable but necessary. As pertinent advances and practices change, it is essential that these changes are communicated to the clinicians who are involved in facial rejuvenation. That is what we present here in the Facial Rejuvenation: Comprehensive 2025 Experts' Update. I have been most privileged to assemble this group of leading experts who are immersed in the practice of facial rejuvenation and keenly represent the benefits of newer surgical techniques and technologies. To lift or not to lift? The dilemma of an aging face in Erica Jong's later works

Abstract:Resistance to physical decay and the desire to keep youthful looks are not new in human history. Today, however, anti-aging discourse intensifies the longing to reshape one's body and stop the ravages of time. Aging is often portrayed as a personal accountability, a sign of moral responsibility, and even disease, which affects older women disproportionately. In order to adjust to contemporary beauty standards, many older women decide to undergo various rejuvenation procedures. However, few studies on the outcomes of such cosmetic interventions in relation to aging, acceptance and self-perception exist. This article addresses American writer Erica Jong's concerns about the aging body and surgical anti-aging interventions. As spokeswoman of the American post-war generation, the author shows how facial rejuvenation procedures generate conflicting feelings, and reminds her readers that older women continue to be doubly marginalized in today's youth- and beauty-driven society.
Volumetric Perceptions in Midfacial Aging with Altered Priorities for Rejuvenation

Abstract:The history of facial rejuvenation surgery has largely involved the manipulation of facial soft tissues under tension within the two-dimensional confinements of a facial plane. However, the youthful human face is not planar; it presents as a complex geometric solid with a curvilinear profile on the oblique view that forms an architectural ogee. Restoration of this shape by sculptural manipulation of the facial soft tissues is deemed the highest priority in midfacial rejuvenation, whereas improvement of the periorbital and perioral environments is deemed second. Effacement of the nasolabial fold is relegated to a third level of importance in this philosophy of altered priorities for midfacial rejuvenation. The importance of early patient photographs is stressed in operative planning to direct facial changes toward a past intrinsic facial personality. Postoperative results are presented of patients who have undergone facial rejuvenation by a new midfacial technique that targets these reordered goals. Comparisons with photographs taken earlier in life and traditional preoperative photographs can then be made.

Abstract:Facelift surgery, also known as rhytidectomy, is a popular cosmetic procedure aimed at reversing the signs of facial aging. While facelift surgery is more commonly performed in women, an increasing number of men are seeking this procedure to maintain a youthful appearance. The long recovery period conflicts the overall primary outcome anticipated by men undergoing rhytidectomy, i.e., appear younger, youthful, and ready to reaffirm their value in the working environment. Anterior direct neck lift, blepharoplasty (upper and lower eyelid blepharoplasty), and lipofilling are three techniques that have been shown to be effective in improving facial rejuvenation outcomes in elderly male patients. However, there is little to no evidence of the safety and efficacy of these techniques when used in combination. By combining these techniques, we hypothesized that we could achieve outcomes comparable to traditional facelift surgery, but with a lower risk of complications and a shorter recovery time. We conducted a retrospective case series study of 18 male patients who underwent facial rejuvenation surgery using the combination of anterior direct neck lift, blepharoplasty (upper and lower eyelid blepharoplasty), and lipofilling at our institution between 2018 and 2021. All 18 patients completed the study, with an average follow-up of 12 months. No major complications were observed, and all patients reported a high level of satisfaction with their outcomes.

Abstract:Diabetes mellitus exerts a sex-dependent effect on hepatic drug metabolism in the rat and it has been suggested that this is due to a reduction in serum androgen levels. This study shows that the effect of diabetes is only seen in the presence of androgen and that testosterone can reverse the effect of diabetes on drug metabolism. Diabetes, however, does not consistently cause a reduction in serum testosterone. Diabetes and androgens, therefore, are postulated to interact in their effects on drug metabolism by action on a common mediator. It is suggested that this mediator is growth hormone, which is known to be affected by the androgens and insulin and to be involved in sex differences in drug metabolism in the rat.

Abstract:The aim of this paper is to review portraits from the Renaissance to the Romantic eras, created before the invention of photography, and to discuss their usefulness to plastic surgeons. Before photography and film were invented, portraits played a critical role in documenting individuals, conveying power, and immortalizing people's appearance and identity. Portraits were not merely simple representations of a person's physical appearance; they played a crucial role in conveying the subject's inner qualities and social position, as well as the cultural values of the era. Artists often idealized or embellished certain features to align with the cultural and aesthetic ideals of their time. Portraits of the Renaissance to Romantic eras could have a significant impact on plastic surgeons' understanding of beauty and aesthetics. A plastic surgeon drawing on Renaissance ideals might prioritize symmetry and balanced facial features, which continue to be regarded as attractive in contemporary beauty standards. Beauty standards during these times favored curvaceous and lively figures, potentially inspiring modern plastic surgeons to focus on creating more dramatic or unique appearances. A plastic surgeon inspired by Romantic art might value uniqueness and avoid rigid beauty standards, focusing instead on enhancing a person's natural traits rather than conforming to a singular "ideal" appearance. Plastic surgeons could gain a richer, more classical perspective on aesthetics by studying historical art, potentially leading to more personalized and balanced outcomes. However, integrating these classical ideals into modern practice must also consider the diverse and varying definitions of beauty prevalent in today's multicultural society.

Presented is a "3-Level approach to facelift planning" named for the facial subregions targeted. This novel paradigm maximizes SDM with its simplicity and reproducibility, serves as an invaluable educational tool for patients, novice and seasoned surgeons alike, and facilitates communication between senior surgeons through its descriptive standardization.

Abstract:Abstract Background Facial aesthetic surgery is one of plastic and reconstructive surgery’s most sought-after and evolving areas. The growing diversity in both local and worldwide populations compels us to reevaluate our previous approaches and underscores the significance of departing from a singular aesthetic standard. Thankfully, the historical practice of stereotypical racial profiling has become less prevalent in the literature. Regrettably, however, the comparative terminology persists, characterizing non-Caucasian races as deviating from the “norm,” which typically refers to the White/Caucasian race. Additionally, there is a lack of contemporary literature comparing the distinctive considerations relevant to rhytidectomy across multiple racial populations. Objectives To succinctly outline the current body of literature examining the impact of race on facial aging and rhytidectomy, laying the groundwork for subsequent discussion on the relationship between race and facial aesthetic surgery. Methods A comprehensive literature review was conducted using PubMed (National Institutes of Health, Bethesda, MD), identifying all studies published before 2023 discussing facial aging and rhytidectomy, specifically in non-Caucasian patients. Results A total of 22 publications were identified. Asians were the most described group with twelve publications, followed by Black/African with five, and Hispanic/LatinX with four. Conclusions In today's context, the aesthetic surgeon must possess the knowledge and readiness to provide culturally sensitive, patient-centered care guided by subtle anatomical nuances, informed by anecdotal recommendations, and ultimately, aimed at effectively addressing aging concerns within diverse populations.

Abstract:The aim of this study is to analyze the process of metamorphosis of a tall and ugly woman through plastic surgery as depicted in The Life and Loves of a She-Devil (1983) of Fay Weldon. The process of plastic surgeries and the psychology of the protagonist of this novel were analyzed. A highly unattractive woman (Ruth, 185 cm) takes revenge on her husband (Bobbo, 175 cm) and his attractive lover (Mary Fisher, 160 cm). To transform her appearance, Ruth remodels her jaw, has her teeth extracted (2-jaw surgery or facial contouring surgery), straightens and trims her nose (corrective rhinoplasty), flattens her cheekbones (reduction malarplasty), reduces her breast size (reduction mammoplasty), and shortens both her arms and legs (limb reduction surgery). Following the series of surgeries, Ruth ultimately made herself look identical to Mary Fisher, her husband’s lover. She believes her inadequacies stem from her body’s failure to meet societal beauty standards, prompting her decision to “remake” herself. She declares, “I will be what I want, not what God ordained.” Ruth’s aspiration is not to acquire a new face but to attain the face of Mary Fisher. When confronted with pain and harm, Ruth does not waver but acknowledges it. “Of course it hurts. Andersen’s little mermaid wanted legs instead of a tail. And, like her, I welcome it. I don’t complain.” In this study, we derived insights into why patients who want to change their entire bodies undergo plastic surgery, what these patients really want, and patients’ attitudes toward the pain caused by surgical procedures.

Abstract:Facial aging reflects the dynamic, cumulative effects of time on the skin, soft tissues, and deep structural components of the face, and is a complex synergy of skin textural changes and loss of facial volume. Many of the facial manifestations of aging reflect the combined effects of gravity, progressive bone resorption, decreased tissue elasticity, and redistribution of subcutaneous fullness. A convenient method for assessing the morphological effects of aging is to divide the face into the upper third (forehead and brows), middle third (midface and nose), and lower third (chin, jawline, and neck). The midface is an important factor in facial aesthetics because perceptions of facial attractiveness are largely founded on the synergy of the eyes, nose, lips, and cheek bones (central facial triangle). For aesthetic purposes, this area should be considered from a 3-dimensional rather than a 2-dimensional perspective, and restoration of a youthful 3-dimensional facial topography should be regarded as the primary goal in facial rejuvenation. Recent years have seen a significant increase in the number of nonsurgical procedures performed for facial rejuvenation. Patients seeking alternatives to surgical procedures include those who require restoration of lost facial volume, those who wish to enhance normal facial features, and those who want to correct facial asymmetry. Important factors in selecting a nonsurgical treatment option include the advantages of an immediate cosmetic result and a short recovery time.

Abstract:Rejuvenative therapy of the lower face has traditionally been surgical in nature, with office-based treatments such as soft tissue fillers relegated to "second tier" status. However, traditional rhytidectomy does not significantly affect the perioral complex and leaves the central lower face unaltered and unimproved. If it is left untreated, there is a clear disparity between the rejuvenated neck and aged perioral area. Soft tissue augmentation of the perioral area can provide rapid aesthetic improvement. Careful analysis of this area and appropriate treatment can harmonize these areas and produce a globally aesthetic result.

Abstract:The aged neck is the most common motivator for patients seeking facial rejuvenation. Unlike the deflated midface, surgical treatment is still the gold standard for optimal outcomes. It is our view that the majority etiology of both the aged neck and deflated midface is gravity’s effects on the su-perficial soft tissue envelope, leading to soft tissue redundancy. It is also our view that the deep-plane rhytidectomy is supported by anatomy, embryology, and clinical evidence as the best meth-odology to treat soft tissue redundancy without producing the aesthetic and longevity issues asso-ciated with SMAS techniques. As one of the early pioneers in deep-plane rhytidectomy, the lead author will define contemporary advances in deep-plane technique such as dissection entry point, flap design, and flap fixation concepts that allow aggressive approach to treating both the aged neck and deflated midface.

Abstract:Objective. This review aims to summarize different kinds of applications of minimally invasive surgery in improving facial aging to provide a comprehensive and accurate introduction on the issue of esthetic treatment of facial skin. Overview. In the twentieth century, facial rejuvenation has become a new beauty trend. Facial cosmetology has entered a period of antiaging and rejuvenation therapies and microplastic surgery. The pursuit of beauty has promoted the development of minimally invasive plastic surgery. This review introduces the possible causes of facial aging and its related topics with a focus on facial injectable drugs, such as botulinum toxin, main filler materials (hyaluronic acid, calcium hydroxyapatite, poly L-lactic acid, collagen, autologous fat, and polymethyl methacrylate), and some current antiwrinkle technologies, such as thread lift and radiofrequency rhytidectomy. Conclusions. Despite the difference in mechanisms of action, each technique can address facial aging involving the loss of collagen, displacement and enlargement of fat, and muscle relaxation. Combinations of these treatments can provide patients with reasonable, comprehensive, and personalized treatment plans.

Abstract:ABSTRACT The convergence of two seemingly distinct discourses - those of medical science and planning - in the period of urban reconstruction at the close of the Second World War is discussed. It is suggested that planners in 1940s Britain conceptualized urban reconstruction by employing the visual imagery and rhetoric of contemporary 'medical aesthetics'. The convergence of these ideas was purposeful: medical knowledge and its newly emerging aesthetics transplanted itself easily into the imaginative realms of urban reconstruction so that planners and architects alike could visualize the social, moral and architectural improvements to be made to the city in terms of diagnosis, cure and treatment. Reconstruction was not simply building cities anew; it also involved a vision of post-war life in its full cultural complexity.

Abstract:Background: We aimed to track the global trend of cosmetic treatment outcomes for facial rejuvenation and the differences in modalities used in East Asian and Western populations. Methods: Articles reporting on facial rejuvenation procedures (invasive/noninvasive) were identified in PubMed from 2013 to March 2023 and bibliometrically analyzed for type of published document, citation frequency, authors with most articles, author's affiliation, and frequency distribution of keywords. Results: From 553 articles, most were published in 2021 (n = 86, 15.6%). Western countries (n = 323, 58.4%) contributed more than East Asian (n = 230, 41.6%), with more invasive interventions (n = 355, 64.2%) than noninvasive techniques (n = 198, 35.8%). Numbers of invasive techniques in West versus East Asian countries were 225 (40.7%) versus 135 (24.4%). Main indications were the reduction of facial wrinkles and antiageing treatments. Hyaluronic acid, fillers, and botulinum toxin were the main hotspots for invasive treatments, whereas laser, platelet-rich plasma, and radiofrequency were for noninvasive treatments. Nasolabial folds (13.4%) and glabellar lines (12.4%) were the top research hotspots in the East Asian and Western regions. Common adverse events were pain, erythema, swelling, and bruising. Approximately, 89.3% of publications were from single countries, whereas 10.7% of publications were from international collaborations. Most articles (n = 387; 69.95%) presented their findings using level II evidence. Dermatological surgery (IF = 2.914) had the greatest number of publications (n = 109; 19.71%). Conclusions: The main hotspots were antiaging and youthfulness. This study provides a trend and a new perspective on the future research directions in the field of facial rejuvenation.

Abstract:The extended deep plane facelift is a powerful technique to correct aging of the midface and neck. After many years of superficial muscular aponeurotic system lift techniques, the senior author transitioned to an extended deep plane facelift for all patients. The primary catalyst for this shift in practice was the pursuit of superior rejuvenation of the midface. Consistent uniform elevation of the deep plane with complete ligament release and management of the soft tissue flap were the most significant challenges in the early adoption period. Navigating the transition was facilitated by consultation with experienced colleagues and frequent cadaver dissections. This manuscript details the authors' current technique. Complications and recovery from this technique are similar to those reported with historical techniques and are minimized with proper preparation, precision, and perioperative management. In our experience the results from this procedure are extremely reproducible, durable, and natural, and patients are overwhelmingly extremely satisfied.
