Abstract:Our role as aesthetic surgeons demands individualized surgical planning that maximizes patient input and understanding. The value of such shared decision-making (SDM) in aesthetic surgery is becoming increasingly appreciated. This is particularly true for potential patients seeking surgical rejuvenation of the face, where the volume of "educational" information available on the internet, and through various social medial channels, may be overwhelming and even misleading. Presented is a "3-Level approach to facelift planning" named for the facial subregions targeted. This novel paradigm maximizes SDM with its simplicity and reproducibility, serves as an invaluable educational tool for patients, novice and seasoned surgeons alike, and facilitates communication between senior surgeons through its descriptive standardization.

Abstract:Background: Minimally invasive and noninvasive facial aesthetic treatments are increasingly popular, and a greater understanding of patient perspectives on facial aesthetic priorities is needed. Objectives: The authors surveyed facial aesthetic concerns, desires, and treatment goals of aesthetically conscious men and women, and physicians, in 18 countries. Methods: This was a global, internet-based survey on desired appearance and experiences with, or interest in, facial aesthetic treatments. Eligible respondents were aesthetically conscious adults (21-75 years). Eligible aesthetic physicians were required to see ≥30 patients per month for aesthetic reasons, have 2 to 30 years of experience in clinical practice, and spend ≥70% of their time in direct patient care. Results: A total of 14,584 aesthetically conscious adults (mean age, 41 years; 70% women) and 1315 aesthetic physicians (mean age, 45 years; 68% men) completed the survey. Most respondents (68%) reported that aesthetic procedures should be sought in their 30s to 40s; physicians recommended patients seek treatment earlier. Respondents expressed greatest concern over crow's feet lines, forehead lines, facial skin issues, hair-related concerns, and under-eye bags or dark circles; in contrast, physicians tended to underestimate concerns about under-eye bags or dark circles, mid-face volume deficits, and skin quality. Although both physicians and respondents cited cost as a major barrier to seeking aesthetic treatments, respondents also emphasized safety, fear of injections or procedure-related pain, and concern about unnatural-looking outcomes. Conclusions: This global survey provides valuable insight into facial aesthetic concerns and perspectives that may be implemented in patient education and consultations to improve patient satisfaction following aesthetic treatments.

Abstract:BACKGROUND: Aging affects the 3-dimensional structure of all the facial tissues: Bones, muscles, ligaments, adipose tissue, and skin. AIM: To customize minimally invasive treatments for facial rejuvenation, we present a standardized holistic approach characterized by at-home treatments in associations with outpatient procedures. METHODS: Forty-four patients underwent 3-dimensional stepwise facial rejuvenation and were evaluated prospectively. Each patient received a customized treatment plan based on a clinical examination and consultation. Treatment outcomes were evaluated from patient photographs with and skin analysis was performed with an A-One Smart automated skin analysis system. RESULTS: The mean age of the patients was 41.7 years and the approximate mean duration of treatment was 160 days. Patients applied cosmeceuticals such as retinoic acid. Outpatient procedures included the delivery of botulinum toxin or dermal fillers, thread lifting, chemical peels, etc. Upon treatment completion, significant improvements were noted in multiple domains: Skin elasticity and hydration increased, areas of hyper-pigmentation were less extensive, and there were fewer visible wrinkles and pores. CONCLUSION: Outcomes of the present article suggest how important is to customize facial anti-aging treatments. Nonsurgical treatments carried out progressively, involving the patient to perform at-home treatments in associations with outpatient procedures, let to achieve facial improvements in terms of increased skin elasticity and hydration, reduction of hyperpigmentation, wrinkles, and pores.

Abstract:The topic of facial rejuvenation should be expected to be a recurring theme in a journal named Facial Plastic Surgery . I invite you to ponder that statement. The reason for this periodic reappearance of this topic is because of the amazing aspect of current human experience that is continually benefited from ongoing advances in science and medicine. These advances have not only resulted in the elimination of many illnesses and maladies but have also prolonged the active human lifespan. Peoples' health is considerably better than it was just a few decades ago. These advances continue and among such are frank endeavors to further longevity. As a result of this, as evidenced by the patients entering our practices, there is an enlarging and more sophisticated group of patients seeking physical and physiological rejuvenation. Their desire for rejuvenation of one's facial appearance is indeed part of the evolution of patients' desire for good health, youthfulness, longevity, and vitality. The field of facial rejuvenation has undergone parallel advances, like other aspect of medicine, with changes in surgical techniques and new technologies. Patients' growing desire for rejuvenation has been catalyzed by the internet and social media. Patients are continually presented with not only what is possible, but what is possible for them personally. The intermittent recurrence of the topic of facial rejuvenation in Facial Plastic Surgery , therefore, is not only understandable but necessary. As pertinent advances and practices change, it is essential that these changes are communicated to the clinicians who are involved in facial rejuvenation. Abstract:This article addresses some critical aspects of the relationship between aesthetic medicine (AM) and ethics and proposes a possible deontological ethical line to pursue based on current practices. The role of AM has always been controversial and suffers from unclear practical and moral boundaries, even within academic settings, since it aims to improve the appearance of individuals, not to cure a disease. Today, it is essential and pertinent to discuss these issues, as AM specialists are dealing with a growing and increasingly demanding patient population that has undergone profound evolution in recent years. Current challenges within the field of AM include a lack of global uniformity concerning the education of AM specialists, an increasing number of physicians practicing AM with diverse training backgrounds, the spread of AM being practiced outside of medical practice or hospital settings, and the influence of social media where the success is modelled and dictated by the identification of a youthful appearance). By the field of action enriched by technologies that aim not only at enhancement per se but also at the preservation and regeneration of tissues, it is necessary to establish an active multidisciplinary discussion on the definition of shared ethical limits. This discussion would allow AM to fully reclaim its identity as a specialty that aims to improve patient well-being whilst maintaining respect for patient aesthetic harmony, the expertise of specialists who practice AM, the essential role of safety, and awareness of the importance of a confidential doctor−patient relationship.

Nasopharyngeal carcinoma risk by histologic type in central China: Impact of smoking, alcohol and family history

Abstract:The role of cigarette smoking, alcohol use, family history of cancer and the interaction of cigarettes and family history in the etiology of Nasopharyngeal carcinoma (NPC) in general and within each histologic type are unclear. We conducted a case‐control study among 1,044 Han Chinese patients with NPC and 1,095 Han Chinese cancer‐free control subjects. Logistic regression was used to analyze the association between histologic type of NPC and cigarette smoking, alcohol drinking and family history. The results indicated that NPC was significantly associated with cigarette smoking [adjusted odds ratio (OR) = 2.97, 95% confidence interval (CI), 2.38–3.70], and the association exhibited a dose‐response relationship for intensity, duration, and cumulative consumption of cigarettes (ptrend < 0.0001 for intensity, duration and cumulative consumption of cigarettes). Positive family history of cancer led to a significant 12‐fold elevated risk of NPC (adjusted OR = 12.95, 95% CI, 7.12–23.54) and acted jointly with cigarettes in contributing to NPC risk (adjusted OR = 56.68, 95% CI, 17.25–186.19). The association of NPC risk with cigarettes was stronger for nonkeratinizing carcinoma than for keratinizing squamous cell carcinoma (KSCC), whereas family history was more closely associated with KSCC. NPC risk was not associated with alcohol consumption. Our study demonstrated that cigarette smoking and family history of cancer could serve independently and jointly as risk factors for etiology of NPC and might affect the risk of histology‐specific NPC differently. This knowledge may help facilitate comprehension of NPC etiology in general as well as within each histologic type, and thereby improve prevention efforts.

Abstract:Throughout the process of building a new medical spa, it is important to standardize and implement safe, evidenced-based practice standards. As medical spas and aesthetic services become more prevalent and popular, it is more important than ever for providers to ensure that patient safety is a top priority. Integrating a structured medical model into the aesthetic consultation and treatment ensures that a mutual decision-making process is shared between the provider and the patient. Together, the provider and the patient can determine whether the treatment will improve the patient's overall health and well-being. The patient is given the opportunity to fully understand and choose which procedure will best address their overall psychological health, aesthetic concern, and anatomical diagnosis. In establishing agreement with the patient on a common goal, the provider can first assess the risks and benefits, and then determine which intervention or treatment will provide the patient with the greatest benefit. When providers establish and adhere to an easy-to-use, decision-making algorithm, patients are empowered in the decision-making process. This helps build a working, trusting relationship between the patient and the provider, which leads to greater patient satisfaction and patient loyalty. Sharing this medical model algorithm will greatly benefit other aesthetic providers by helping to create a standardized evaluation process in a medically based aesthetic practice.

Abstract:Isoflavones occurring in soybean seed have been thought to have a positive effect on the health of humans, especially in cancer prevention. This positive impact may be a result of their antioxidant properties, estrogenic activity, or a combination of mechanisms. The purpose of this study was to quantify and compare different levels of isoflavones in various Kansas soybean cultivars. Seeds of four different cultivars grown in 2001 at three different locations were analyzed. There were significant differences in total and individual amounts of isoflavones across both cultivars and locations. These results demonstrated a large variation in isoflavone content in different cultivars grown in different locations, suggesting that selection of cultivar and growing location are important parameters in optimizing the isoflavone content of soybeans grown in Kansas.
A Retrospective Observational Study of an Innovative Protocol for Regenerative Aesthetics

Abstract:Background/Objectives: This retrospective observational study sought to determine the efficacy and safety of an innovative combined treatment protocol using guided Superficial Enhanced Fluid Fat Injection (SEFFI) and calcium hydroxylapatite (CaHA) in facial rejuvenation. Methods: A total of 158 patients (149 females and 9 males) underwent the combined treatment of guided SEFFI and diluted/hyperdiluted CaHA. The study evaluated treatment outcomes at 30, 90, and 150 days post-treatment using the Global Aesthetic Improvement Scale (GAIS) and three-dimensional photogrammetric analysis. Results: The combined treatment demonstrated consistent enhancement in skin quality and facial volume across temporal, malar, zygomatic, and jawline regions. At 90 days post-treatment, substantial improvements were observed, with the GAIS scores reflecting significant enhancements in both skin quality and volume, which were sustained or slightly improved by 150 days. Minor complications, predominantly ecchymosis at the injection sites, resolved within a week, confirming the treatments' safety. Conclusions: The integration of guided SEFFI and CaHA resulted in significant improvements in skin quality and facial volume with minimal complications. Further research is recommended to consolidate these findings and explore long-term outcomes.

Bridging the gaps in patient education through aesthetic-based education: A concept analysis study

Abstract:Background: Aesthetics is a branch of philosophy that has been entered into the philosophy of education for a long time. Reviewing the concept of aesthetics in education, we can see the components that can cover a number of challenges in the field of patient education. Focusing on the conceptual dimensions obtained from the analysis of this concept, the aim is matching the resulting conceptual categories with the gaps in the field of patient education. Materials and methods: Using a scoping review, we reviewed the literature dealt with different dimensions of aesthetic-based education. Walker and Avant's concept analysis approach was used to inductively analyze the content obtained from the review of literature. The data were divided into three general groups of antecedents, attributes, and consequences of the aesthetic-based education concept, and the final conceptual model was defined. Interdisciplinary comparisons between educational and medical sciences were made to match the concept of aesthetic education with the field of patient education to cover its issues. During the scope review process of the texts, after screening the articles, 17 articles selected to enter the concept analysis stage. Result: Concept analysis phase showed that the main conceptual categories of antecedents of aesthetic-based education are aesthetic-based intrapersonal competencies and aesthetic capacities of educational setting. The attributes include aesthetic-based educational content and teaching methods including unity in diversity, combination with art and being based on empathy. The consequences include subliminal learning and constraint-free learning. These conceptual dimensions can cover four important issues in patient education including patient satisfaction, pedagogical competences of health educators, patient centeredness, and empathetic relationship. Conclusion: Applying aesthetic concept in the patient education process can bridge part of the gaps in this process. So, this study can be an introduction to future innovative models based on aesthetic paradigm in the field of patient education.
Volumetric Perceptions in Midfacial Aging with Altered Priorities for Rejuvenation

Abstract:The history of facial rejuvenation surgery has largely involved the manipulation of facial soft tissues under tension within the two-dimensional confinements of a facial plane. However, the youthful human face is not planar; it presents as a complex geometric solid with a curvilinear profile on the oblique view that forms an architectural ogee. Restoration of this shape by sculptural manipulation of the facial soft tissues is deemed the highest priority in midfacial rejuvenation, whereas improvement of the periorbital and perioral environments is deemed second. Effacement of the nasolabial fold is relegated to a third level of importance in this philosophy of altered priorities for midfacial rejuvenation. The importance of early patient photographs is stressed in operative planning to direct facial changes toward a past intrinsic facial personality. Postoperative results are presented of patients who have undergone facial rejuvenation by a new midfacial technique that targets these reordered goals. Comparisons with photographs taken earlier in life and traditional preoperative photographs can then be made.

Abstract:Aesthetic plastic surgery is a most unusual and unique subspecialty. Unlike other medical and surgical specialties driven primarily by objective medical diagnosis and outcome, aesthetic surgery is patient driven; it represents a special exercise in professional competence by adapting objective medical standards to a mostly subjective field. Given the subjectivity of facial beauty and of the projected aesthetic intervention outcome, in addition to be able to determine beauty of the final result as precisely and objectively as possible, it is also essential to be able to assess patient's satisfaction. However, lack of standardized facial measurements and clear definition of aesthetic outcome and beauty are still major obstacles preventing real change in the consultation dynamics to help better serve patients' expectations. Because of the tremendous increase in demand for all types of facial aesthetic procedures, an objective understanding of aesthetics and beauty is becoming of utmost importance. Recognition of the importance of science and evidence-based medicine is long overdue in our specialty. This study aims at identifying the most recently available evidence-based measures to quantitatively assess beauty and measure outcome of rhytidoplasty that can be useful in everyday aesthetic practice.

Abstract:Nowadays, there are many nonsurgical treatment modalities for facial and body rejuvenation [1]. If we do not take into account invasive procedures such as chemical peelings, infiltrative therapies (fillers and revitalizing), needling, or dermabrasion, it is possible to identify two main classes of physical treatments: resurfacing procedures (laser, IPL, etc.) and therapies finalized to improve the deep dermis, subcutaneous tissue, and muscles (massage, radiofrequency, and in some occasions carboxytherapy). This chapter focuses on these noninvasive methods that are gaining more interest in the scientific community, playing a key role in facial rejuvenation. Sometimes they may be considered as a good alternative to invasive procedures, especially in those patients who refuse or do not tolerate them or, on the other hand, as preparatory or complementary treatments to be used in association to surgical procedures. Finally, it is meaningful to underline the importance of such procedures for a good prevention and careful management of the patient’ skin, in order to avoid or delay as much as possible more invasive and debilitating procedures.

Three agents with specific properties are available in this perspective: a polycaprolactone (PCL)-based collagen stimulator, a poly-L-lactic acid (PLLA)- and a poly-glycolic acid (PLGA)-based resorbable suspension suture with a 3D-cone technology, and a cross-linked hyaluronic acid (HA). Objective: To develop the first practice guidelines on rejuvenation treatment of the face and the neck using combinations of these agents, whether associated or not with other widely used MIPs such as botulinum neurotoxins or energy-based devices. Methods: A multi-disciplinary, multi-national board of plastic surgeons and dermatologists convened to develop guidelines using a predefined consensus method. The consensus was defined as ≥ 83% agreement rate between participants. Results: Practice guidelines and algorithms, describing optimal procedure sequence and spacing, are proposed for the treatment of upper-, mid-, lower-face and neck, combining the PCL collagen stimulator, the PLLA/PLGA suspension sutures, and the cross-linked HA, whether associated or not with other MIPs. Conclusion: These new guidelines provide general support to optimal management strategies. Individual treatment plans should be adapted according to the physician's individual competence and the patient's preferences. Abstract:BackgroundIn the field of cosmetic medicine, patient satisfaction is an important and common indicator used to measure the efficacy of the treatment. However, it is insufficient to prove objectively that the benefit of the specific factors involved in the cosmetic outcomes. The practitioner should be aware of these assessment tools, in particular in case of demanding or litigious patients.ObjectiveThe aim of this review was to establish a list and discuss the subjective and objective methods used to assess facial aesthetic rejuvenation treatments.MethodsA systematic literature search was performed using the Pubmed search engine. Studies published over the last 5 years, i.e. between January 2010 and January 2015 were considered for review. The following keywords were used: “aesthetic treatment”, “facial rejuvenation”, and “subjective evaluation” or “objective evaluation”.ResultsOf the 446 articles identified by the search strategy, 47 articles focused specifically on facial rejuvenation and on the efficacy of aesthetic medical treatments were retrieved for review. Thirty-seven articles used quantitative methods to assess aesthetic treatment outcomes and only 12 used subjective methods. The different assessment methods were listed according to the tools used and treatment indications.ConclusionThis review will help in choosing adequate methods to assess facial rejuvenation medical treatment. It is important to combine these tools adequately to improve the assessment. Abstract:BACKGROUND: Individuals seeking aesthetic treatment have concerns regarding multiple facial areas.OBJECTIVES: Assess the aesthetic impact and satisfaction achieved with a multimodal approach to aesthetic treatment using a combination of minimally invasive treatments.METHODS: Prospective, multicenter, rater-blinded, 4-month HARMONY study evaluated patient satisfaction and aesthetic impact of a combination of fillers (VYC-20L, HYC-24L, and HYC-24L+), onabotulinumtoxinA, and bimatoprost. Males and females aged 35 to 65 years received on-label, staged treatment with fillers, as needed per investigator assessment, on day 1, with touch ups allowed on day 14. Bimatoprost was self-administered once daily for 17 weeks. OnabotulinumtoxinA was injected into glabellar lines, crow's feet lines, or both at month 3. Primary effectiveness measure was mean change from baseline on the FACE-Q 10-item Satisfaction with Facial Appearance Overall Scale.RESULTS: Of 100 patients treated, 93 underwent at least the 4-month posttreatment assessment and were assessed for efficacy. The FACE-Q Satisfaction with Facial Appearance Overall Scale total score increased from baseline (41.2) to month 4 (72.9; P < 0.00001; effect size, 2.7). Improvement following multimodal treatment was observed on FACE-Q individual items. Self-perceived age decreased from 0.2 years older than actual age at baseline to 4.6 years younger at month 4. Nearly all patients (99%) rated themselves as improved or much improved on the Global Aesthetic Improvement Scale. Investigator assessments also demonstrated improvement. Mild to moderate adverse events occurred in 42 patients.CONCLUSIONS: Minimally invasive, multimodal treatment resulted in improvements in FACE-Q scores and perceived age, indicating a high degree of patient satisfaction and a younger facial appearance.LEVEL OF EVIDENCE 4:

Abstract:Background: You only get one opportunity to make a first impression. In today's era, that first impression is frequently a digital one. The authors' old digital brand "face" was stale and not a true representation of how they view themselves as a practice. In an evolving arena of competition and surgical scope, the authors felt compelled to engage in rebranding their practice. Methods: This article details the steps the authors took to launch a new website, generate collateral branded material, and execute a social media marketing plan. The authors attempt to keep the outline general enough to be applicable to the range of practice types of the Journal 's readership, and present relevant results of the process. Results: Samples of "creative" products are shown. Quantifiable outcomes were direct website traffic (91% increase), website sessions (82% increase), unique users (55% increase), page views (118% increase), and time spent browsing (100% increase). The authors experienced a 21% increase in new patient volume and a similar increase in total cases performed. Conclusions: This article outlines steps the authors took to rebrand their practice in the face of current challenges in the plastic surgery landscape and how prospective patients seek surgeons. Benchmarking the steps of a successful branding process is crucial and informative to developing and executing a plan. Although there are many potential contributors to the growth of a practice, the impact of our branding appears to be a significant factor.
The McCollough Facial Rejuvenation System: Expanding the Scope of a Condition-Specific Algorithm

Abstract:The ideal facial rejuvenation algorithm is comprised of an appropriate combination of procedures, thoughtfully chosen from an assortment of reliable alternatives, that when skillfully performed provide both short- and long-term enhancement to the undesirable conditions of aging that exists at the time of treatment. In 2010, the senior author published the first scientific article in which a condition-specific classification system and a treatment plan algorithm were applied to the discipline of facial rejuvenation. In the landmark article, the senior author reviewed his surgical experience of more than 5000 face-lifts and grouped patients into five major categories (or stages), based upon the extent of aging identified in various regions of the face and neck and the procedures performed to correct them. The criteria (that have now been suggested on a facial aging worksheet) were recorded in a data blank comprised of a first-generation worksheet. Once the data were collected--and using algorithmic charts for each region and/or facial feature--the most appropriate plan of action for a given patient was created. The sole objective in sharing the senior author's methodology was to launch a scholarly discussion among physicians and surgeons involved in the various disciplines that provide rejuvenation procedures on the face, head, and neck. From such a debate would, hopefully, emerge a definitive algorithmic system--one based squarely on the venerable ethics of medicine, coupled with the appropriate application of and skillful performance of the fundamental principles of surgery. A single, science-based system would restore order to a noble discipline, currently being challenged by narcissism, gimmickry, and commercialization. The implementation of a system rooted in universal truths would require its advocates to agree upon a common "language," the implementation of which allows aesthetically focused surgeons to share both new ideas and time-tested experiences. More importantly, a condition-specific system matches each potential patient's problems--at every age--with the appropriate facial rejuvenation treatment plan, restoring the ideals of science and art to the profession. Initially provided in a consumer information book devised to assist patients with understanding the advantages of personalized treatment plans, the senior author later shared his practices and evolving system with colleagues attending conventions, seminars, and courses. Only after he was convinced that his system could be of benefit to physicians and surgeons from a variety of backgrounds was it offered to the peer-reviewed medical literature. Clearly, a plethora of techniques and materials are available for facial rejuvenation; however, only the ones deemed to be worthy of consideration were included. In practice--and in this presentation--the authors expanded the scope of the previously published article and offer a user-friendly, condition-specific worksheet and algorithmic tables designed to make it easier for surgeons to select the right combinations of procedures--at the right time in a patient's life. Although imitations potentiate an environment of disharmony, the authors remain committed to enabling the evolution of a single facial rejuvenation classification system, one that--with the input of like-minded scholars--could restore needed order to a branch of the medical profession that, in recent years, seems to have lost its focus.
