Abstract:This study was designed to evaluate and compare the usefulness of clear aligners and conventional appliances on Oral Health-Related Quality of Life (OHRQoL) in pediatric population. Emphasis was placed on the relative benefits and implications of employing clear aligners owing to their escalating prevalence and acceptability. The study participants were divided into four groups: Clear Aligner Group (CAG), Conventional Appliance Group (ConAG), Malocclusion Control Group (MCG), and Normal Control Group (NCG). Parameters including sociodemographic indicators and daily routines were assessed. OHRQoL was evaluated via the Child Perceptions Questionnaire (CPQ). Psychological conditions were assessed through the Depression, Anxiety and Stress Scale (DASS). Statistical differences were found between the four groups regarding CPQ subscales and total scores (p < 0.05). CAG was better than ConAG (p < 0.05) regarding the scores of functional limitations, emotional and social well-being, and total score, however no significant difference was discovered in the oral symptoms scores (p = 0.62). Moreover, all the treatment groups had worse OHRQoL compared to NCG (p < 0.05). Malocclusions and their treatments did not increase the psychological distress as per the DASS results. A novel correlation between the excessive tooth brushing and reduced OHRQoL was also observed (p < 0.05). The study herein emphasized the benefits of clear aligners in children and adolescents with OHRQoL. It was highlighted that the clear aligners had potential and were preferred for the adolescent orthodontic treatment.

Abstract:Clear aligner therapy has emerged as a popular alternative to traditional fixed orthodontic treatment, particularly among adult patients seeking aesthetic and comfortable options. These aligners, made of clear thermoformed plastic, offer advantages such as improved aesthetics, comfort, oral hygiene, and periodontal health compared to fixed appliances. Recent studies have shown that clear aligners can be equally effective as fixed appliances, if not more so, for treating mild to moderate malocclusions, with shorter treatment durations, fewer appointments, and reduced emergency visits. While clear aligners may not be as effective for complex cases requiring additional techniques, advancements in technology have expanded their applicability, allowing for the incorporation of methods used in traditional braces. However, more extensive research is needed to fully understand their impact on oral health and the oral microbiome, spanning all treatment phases from initiation to maintenance. Additionally, various brands of clear aligners with different materials and designs have entered the market, offering alternatives to the widely known Invisalign™, though scientific literature on these alternatives remains limited. This article discusses about the efficacy of clear aligners as compared to fixed appliances in orthodontic treatment.

Abstract:BACKGROUND Clear aligners are orthodontic devices that are transparent, a plastic used to correct malaligned teeth. Here patient wears a series of customized clear, removable aligners that gradually move the teeth to the desired position. The clear aligner system is a modern adaptation of the systems described since the middle of the 20th century, therefore there were different devices and philosophies that have led to its creation and the system has evolved a lot over the decades. Clear aligner therapy has been a part of the orthodontic practice for years, but, popularity was increased since the introduction of Invisalign appliances (Align Technology) in 1998. There are almost 27 different clear aligner products currently on offer for orthodontic treatment. Nowadays, more people prefer clear aligner treatment because it is aesthetically superior to brackets and lingual orthodontics. The superiority of clear aligners lies in their aesthetics. The optical properties of the clear aligner material play a major role in aesthetics. The rising demand among adult patients for “invisible” orthodontic treatment has led to an exponential growth in the clear aligner market. Indeed, these aligners have a low aesthetic impact, as well as being able to effectively and progressively guide the teeth into their programmed positions. They are also removable and therefore do not hamper oral hygiene maintenance, in turn reducing the risk of white spots, caries, gingivitis and periodontal disease. All the materials do not possess the same chemical composition. The properties change before and after wear. In this article, we bring out the different materials used for the manufacture of clear aligners and their various properties. Tooth colored brackets and wires gained popularity for a few decades but gradually declined owing to its own disadvantages. Orthodontists have given a new dimension by shifting from the labial to the lingual so as to give rise to the concept of Invisible braces or Lingual Orthodontics. Abstract:Techniques such as polyclonal B cell activation with pokeweed mitogen (PWM) and polymerase chain reaction (PCR) analysis have documented the existence of simian immunodeficiency virus (SIV)-and human immunodeficiency virus type 1-seronegative but infected humans and nonhuman primates. To establish whether blood from such seronegative but PWM- and PCR-positive monkeys can transmit infection, naive macaques were transfused with whole blood (n = 2) or cultured cells and supernatant fluid (n = 2) from two seronegative but PWM- and PCR-positive sooty mangabeys. After transfusion, three of the four recipients seroconverted, and peripheral blood mononuclear cells from all four recipients secreted SIV-reactive antibodies upon polyclonal activation in vitro and were SIV-positive by PCR that used highly specific gag primer pairs and probe. In addition, CD8+ cells from all four recipients markedly inhibited replication of SIV in autologous cells in vitro. These data suggest caution in the sole use of serologic tests for the detection of retroviral infection and document the ability of such blood samples to transmit infection.

Abstract:Mixed dentition represents a critical phase in the oral development of pediatric patients, characterized by the simultaneous presence of primary and permanent teeth. This article proposes a comprehensive systematic review of the application of aligners as an innovative methodology in managing mixed dentition. The primary objective is to explore the efficacy, safety, and acceptability of this emerging orthodontic technology in the evolving age group. This systematic review focuses on randomized controlled trials, cohorts, and observational studies investigating the use of aligners in patients with mixed dentition. Clinical, radiographic, and psychosocial parameters will be considered to assess the overall impact of aligner therapy in this critical phase of dental development. An in-depth analysis of such data aims to provide a comprehensive overview of the potential of this technology in pediatric orthodontics. Expected outcomes may contribute to outlining practical guidelines and targeted therapeutic strategies for orthodontists involved in managing mixed dentition. Furthermore, this article aims to identify gaps in the current research and suggest future directions for studies exploring the use of transparent aligners in patients with mixed dentition, thereby contributing to the ongoing evolution of evidence-based orthodontic practices.

Abstract:This is a frequently asked question. The answer depends on various factors, with cooperation being among the most essential. As a parent, you must ask yourself if you can rely on your child to wear aligners correctly for most of the day. If the answer to this question is yes, then clear aligners may be a better choice than traditional braces. There are three major benefits of clear aligners in children: 1. Greater comfort, aligners are less voluminous than traditional braces and more likely to prevent irritation, mouth ulcers and other discomfort that derives from the movement of the orthodontic wire or braces. 2. Better hygiene, as their removal during meals and tooth cleaning manoeuvres makes home oral hygiene easier. 3. Better aesthetics, thanks to the transparency of the plastic replacing the metal. However, a certain degree of caution is advised to those who are considering this treatment option. Although aligners allow us to start resolving certain tooth malpositions (which can cause periodontal and aesthetic problems or increase the severity of damage in the event of trauma), and to improve alignment and minor crowding at an early age, it must not be forgotten that the intervening period for changing the definitive position of individual teeth is still the period of permanent teeth. It is important to consider whether a child has a dental or skeletal problem. In cases where a child's problem is exclusively dental, the aligner may be one of the tools used. In cases of skeletal problems, the literature advises against the aligner, as it is not the right instrument at this stage of development. The literature in some cases refers to aligners as the universal remedy, valid for every malocclusion, presenting them as a panacea for all orthodontic problems of very young patients. It is necessary to remain open to the innovations that the market offers us and that can improve our daily clinic, and aligners can be a valid support in this regard, but, as dentists, we should always keep a critical and open eye on treatments with an evidence-based rationale. As with all other orthodontic appliances, aligners must be evaluated on a case-by-case basis, keeping in mind that they are one of the tools in the hands of the clinician. When the clinical situation makes it possible and when both the child's requirements and the parent's expectations are fulfilled, it is certainly worth considering clear aligners as a viable route in the family-child's orthodontic treatment, making it as simple and convenient as possible. Lastly, we should remember the concept of efficiency regarding an orthodontic treatment, which depends on its cost/benefit ratio, with the former being understood in a biological sense and in terms of its impact on the life of the patient and their family with respect to duration, cooperation required, discomfort induced by the equipment used and frequency of check-ups. The latter should obviously be considered in terms of results obtained in relation to the pre-established treatment goals. A consideration that we should always keep in mind: let us never forget that is the orthodontist who treats the patient, not the appliance!

Abstract:Clear aligners have revolutionized orthodontic treatment by offering an esthetically driven treatment modality to patients of all ages. Over the past two decades, aligners have been used to treat malocclusions in millions of patients worldwide. The inception of aligner therapy goes back to the 1940s, yet the protocols to fabricate aligners have been continuously evolved. CAD/CAM driven protocol was the latest approach which drastically changed the scalability of aligner fabrication—i.e., aligner mass production manufacturing. 3D printing technology has been adopted in various sectors including dentistry mostly because of the ability to create complex geometric structures at high accuracy while reducing labor and material costs—for the most part. The integration of 3D printing in dentistry has been across, starting in orthodontics and oral surgery and expanding in periodontics, prosthodontics, and oral implantology. Continuous progress in material development has led to improved mechanical properties, biocompatibility, and overall quality of aligners. Consequently, aligners have become less invasive, more cost-effective, and deliver outcomes comparable to existing treatment options. The promise of 3D printed aligners lies in their ability to treat malocclusions effectively while providing esthetic benefits to patients by remaining virtually invisible throughout the treatment process. Herein, this review aims to provide a comprehensive summary of studies regarding direct-3D printing of clear aligners up to the present, outlining all essential properties required in 3D-printed clear aligners and the challenges that need to be addressed. Additionally, the review proposes implementation methods to further enhance the effectiveness of the treatment outcome.

Abstract:Design Systematic reviewStudy population Orthodontic patients undergoing treatment with removable thermoplastic retainers compared with conventional fixed orthodontic appliances. The mean age of patients was between 15 and 33 years. Data sources Six electronic databases including: Scopus, Web-of-Science, PubMed, Cochrane, Clinical Trials and Grey Literature were searched in addition to a search of references in eligible studies with no restriction of language. Out of 559 studies, 55 were eligible. Only four articles were included; three non-randomised and one randomised controlled trial (RCT) involving 96-182 patients. The studies included were completed within 2005-2016.Study selection Clinical trials involving orthodontic patients undergoing treatment using thermoplastic removeable aligners, namely Invisalign, in comparison to conventional fixed orthodontic appliances of either self-ligating or tip-edge bracket systems were included.Data extraction and synthesis Two members of the research team independently selected articles which fulfilled the inclusion criteria and compared results prior to data collection. A third party examiner was available to resolve disagreements. The study quality was analysed for presence of bias; three non-randomised studies were evaluated and scored using the Methodological Index for Non-Randomised Studies (MINORS), one study was considered high methodological quality, two were considered moderate. The RCT was assessed using the Cochrane Collaboration Common Scheme for Bias and was deemed to be low risk of bias. Results The results suggest that Invisalign can provide successful alignment, however, it poses difficulties in achieving occlusal contacts, buccolingual inclination and vertical movement of teeth. One study found that Invisalign cases are more susceptible to relapse, which can be explained through the tipping mechanism compared to bodily movement, of conventional fixed appliances. Furthermore some studies show that although using aligners can result in overall shorter treatment time the final occlusion was not favourable. The outcome of the RCT, showed that aligners can be successful in treating class I malocclusions in extraction cases.Conclusions This systematic review found that the use of clear aligners made no significant difference to treatment time compared with conventional appliances. Additionally, removable aligners were effective in crowding cases but had limitations in achieving favourable outcomes in the anterior-posterior position and final occlusion, as well as being more likely to relapse post treatment.

Abstract:The aim of this study was to evaluate the efficacy and efficiency of orthodontic treatment using clear aligner therapy (CAT). This efficiency was measured using the Peer Assessment Rating (PAR) index, the American Board of Orthodontics (ABO) index, or the similarity between the final ClinCheck and the final scanned models. A search was done electronically between 1998 and 2021 using the Cochrane Library, PubMed, and Google Scholar databases. Three reviewers individually rated the articles. The ROBINS tool and the Cochrane risk of bias tool were used to evaluate the quality of observational research and randomized controlled trials, respectively. The degree of certainty for each selected outcome was evaluated using the grading of recommendations assessment, development, and evaluation (GRADE) approach.<i> </i>Six studies with a total of 166 participants were considered after the full texts of 61 potential reports were reviewed. This research included in the review covered the period from 1998 to 2021 in retrospect. According to the current systematic review and meta-analysis, transparent aligners had a successful outcome. In mild to moderate cases, aligner treatment appears to have a significant advantage in terms of efficiency (treatment time); nonetheless, insufficient evidence of efficacy was observed based on multiple cross-sectional investigations. When compared to traditional brackets, clear aligners provided a more stable course of treatment.

Abstract:Improper diagnosis and design, misusing orthodontic technique, relapse after orthodontic treatment, and poor patient compliance may lead to treatment failure, requiring orthodontic retreatment. While in such cases, patients usually have already had their teeth extracted, or are with periodontal, joint, and other dental problems, which often require multidisciplinary treatment. The retreatment of orthodontic patients is difficult, and the treatment options are limited. The characteristics of clear aligner treatment, such as comfortable and beautiful appearance, high degree of digital precision, and personalized tooth arrangement to simulate the final outcome, make it popular in the orthodontic retreatment. This article discusses and summarizes the details of orthodontic design, case monitoring, and the application of clear aligner in orthodontic retreatment of failed cases. Before starting treatment, the difficulty assessment tool (clear aligner treatment complexity assessment tool) can be used to evaluate the difficulty of clear aligner treatment. Orthodontists are recommended to treat orthodontic patients with clear aligners within their ability to avoid treatment failure. The key to the success of the extraction case treatment with clear aligner is to make the correct diagnosis and select the right cases. In the treatment planning, attention should be paid to the restoration of anterior tooth torque, making good use of molar distalization to obtain the space, vertical control, and improving posterior tooth relationship. Proper selection of cases and reasonable design in clear aligner treatment are expected to obtain tooth alignment, good intercuspation, normal overbite and overjet, periodontal health, parallel roots, and to achieve the goal of aesthetic, functional, stable, and healthy orthodontic treatment.

Lingual orthodontic appliances brought paradigm shift, but could not got prevalent because of tedious bracket positioning, insufficient training of orthodontists and cost. Chemical insult to the enamel-a common problem to all bracket system, gained attention towards non adhesive orthodontics. Clear thermoplastic aligner trays, customized to individual orthodontic needs are fetching adult orthodontic patients seeking treatment. Clear aligners do have some limitations and inconveniences. The benefits of clear aligners generally outweigh the drawbacks.

Abstract:Clear aligners have gained popularity in orthodontic treatment as a comfortable and aesthetically pleasing alternative to traditional braces. The use of clear aligners has increased due to their advantages, including improved aesthetics, easy removability, and minimal soft tissue irritation. The treatment process involves sequential aligners custom-made for each patient, using computer-aided design and manufacturing technology. Clear aligners employ controlled forces to gradually move teeth into proper alignment. However, clear aligners have limitations, such as the need for patient compliance and their effectiveness in addressing complex malocclusions. Clinical considerations include thorough assessment, regular follow-up visits, and patient education. Treatment outcomes vary depending on the specific malocclusion characteristics. Clear aligners have demonstrated benefits in treating mild malocclusions, spacing issues, and certain types of malocclusion corrections. Treatment duration may be shorter compared to fixed appliances in mild cases. Stability and periodontal health can be influenced by clear aligner therapy, which has effects on plaque levels and gingival inflammation. Root resorption is a concern, but aligners have shown a lower prevalence compared to fixed appliances. Compliance, eruption of permanent teeth, and limitations in visualizing hard and soft tissues are important considerations. [Re-evaluating the clinical implications of clear aligners and lingual appliances - a retrospective and prospective overview]

Abstract:Clear aligners and lingual appliances share common traits of physical invisibility, and therefore, clinical popularity. The marked differentiations between these two modalities, however, lie in treatment mechanisms and indications. This article summarized the accomplishments made and the challenges aroused in these two orthodontic mechanisms, advocating optimal selection of indications and proper use of these two appliances. Cautions are raised against excessive use of clear aligners by ignoring indication inspections.

Abstract:Clear Aligner Treatment (CAT) is an orthodontic technique used to align teeth with removable and scarcely visible appliances. Conventionally, these are produced through the process of thermoforming. The inherent disadvantages of thermoforming include increased surface roughness leading to plaque accumulation, compromised biomechanics due to a reduction in force delivered and flexure of the aligner, and altered mechanical properties, such as increased opacity, water sorption, and hardness. Direct Three-dimensional (3D)-printed aligners, or Direct Printed Aligners (DPA), introduce a new frontier to aligner technology and are a recent addition to the ever-improving field of orthodontics. Through 3D printing, the various disadvantages of thermoformed aligners like surface roughness, extent and definition of aligner borders, undercuts, and differential thickness of the aligner can be controlled to enhance the accuracy of aligner fit with lesser reliance on attachments. 3D printing of aligners is more environmentally friendly since there is no subtractive process for thermoforming or post-processing of the TA. Various methods of 3D printing, such as selective laser melting, selective laser sintering, Stereolithography (SLA), and Digital Light Processing (DLP), can be applied to the printing of clear aligners. Challenges in printing primarily involve maintaining transparency and designing support during the printing process. The present review aimed to include a detailed description of all aspects of direct 3D-printed aligners.

Abstract:Introduction Orthodontic improvements, mainly through the previous decade, have led to a substantial rise in patients’ esthetic needs. Traditional orthodontic procedures have been linked with a compromise in facial appearance, which increases concern among patients pursuing orthodontic treatment. Methods PubMed, MEDLINE, Google Scholar, and Cochrane Library of Systematic Reviews databases were searched using appropriate entry terms and Boolean operators. Search strategy was conducted till June 2022 for systematic reviews and meta-analyses. Publications were systematically assessed for eligibility, and two observers independently graded the methodologic quality using AMSTAR-2 tool. For each meta-analysis, we estimated the summary effect size by the use of random-effects and fixed-effects models and the 95% confidence interval. We estimated the between-study heterogeneity expressed by I 2 (defined as large for I 2 ≥50%), evidence of small-study effects (i.e., large studies had significantly more conservative results than smaller studies), and evidence of excess significance bias (i.e., more studies than expected with significant results). Results Overall, 16 unique systematic reviews and meta-analyses following the eligibility criteria were selected for umbrella review. The results of qualitative analysis of studies revealed that 4 studies had a critically low risk of bias, 11 showed low risk, and 1 study had moderate risk. The results of the meta-analysis favored clear aligners with respect to oral hygiene and root resorption parameters. When tooth movements were considered, the results favored conventional orthodontic treatment. Conclusion Clear aligner therapy (CAT) has significant advantages with regard to efficiency (treatment time, chair time) in mild-moderate malocclusion cases. The overall accuracy of CAT when compared to fixed orthodontic treatment was found to be 55-82%.

Abstract:Background: Align technology has developed greatly over past few years. Patients tended to prefer clear aligners over conventional brackets because of the superior comfort and esthetics, while the effectiveness of clear aligners was still controversial. The aim of this systematic review was to verify whether the treatment effectiveness of clear aligners was similar to the conventional fixed appliances. Methods: A comprehensive search of the Pubmed, Web of Science, Embase, Scopus, and Cochrane Central Register of Controlled Clinical Trials Register databases for studies published through to August 20, 2018 was conducted. Comparative clinical studies assessing the effectiveness of clear aligners compared with braces were included. Results: Eight papers were included in this study. Two of the included papers were randomized controlled trials and six were cohort studies. Clear aligners might not be as effective as braces in producing adequate occlusal contacts, controlling teeth torque, increasing transverse width and retention. While no statistically significant difference was found between two groups in Objective Grading System score (WMD = 8.38, 95% CI [- 0.17, 16.93]; P = 0.05). On the other hand, patients treated with clear aligners had a statistically significant shorter treatment duration than with braces (WMD = - 6.31, 95% CI [- 8.37, - 4.24]; P < 0.001). Conclusion: Both clear aligners and braces were effective in treating malocclusion. Clear aligners had advantage in segmented movement of teeth and shortened treatment duration, but were not as effective as braces in producing adequate occlusal contacts, controlling teeth torque, and retention.

Abstract:Expeditious strides in the fields of biomaterials, computer-aided design, and manufacturing have catapulted clear aligner therapy (CAT) to become a comprehensive orthodontic treatment modality. The efficiency of achieving planned tooth movement with clear aligners is a significant consideration while setting up the final treatment goals, as well as calculating treatment times and costs based on the available evidence. Contemporary research outcomes confirm that one of the most commonly reported clinical concerns with CAT is the discrepancy between the prescribed outcome in the digital treatment plan and the clinically achieved outcome from a given series of aligners. Inaccurate prediction of tooth movements may not only lead to a prolonged duration of aligner treatment with an additional need for refinement strategies; but it may also cause other concerns, such as patient burnout and increased potential for relapse. The authors of this paper have elucidated some of the critical elements that may help address this discrepancy between digitally prescribed and clinical outcomes based on an evidence-based approach with regard to the predictability and accuracy of CAT. A strong diagnostic acumen, judicious case selection, solid biomechanical understanding of various types of orthodontic tooth movements, a research framework that keeps pace with technological and material developments and provides evidence-based knowledge of the limitations of CAT; and above all, the ability of the clinician to continually innovate as per different clinical scenarios, all contribute to attaining treatment predictability, efficacy, and efficiency with CAT.
