Abstract:Orbital tumours encompass a wide range of benign and malignant space occupying lesions that may arise primarily from the orbit or have spread from other sites in the body. They are rare with an incidence of 1 in every 100 000 and may lead to devastating complications of which mechanical compression causing optic neuropathy is the most important.1 Multiple orbital tumours of the same orbit are even rarer with most reported cases being benign homologous tumours such as cavernous haemangiomas or myxofibrosarcomas.2 3 A 58-year-old ethnic Han Chinese male presented in December 2021 with a 1-month history of right eye proptosis. He had no clinical sign of optic neuropathy. Computed tomography of the orbit revealed right axial proptosis and two separate lesions in the right orbit. One lesion appeared infiltrative and measured 2.2 × 1.5 × 1.7 cm 3 (anteroposterior × transverse × longitudinal) at the extraconal space with retrobulbar extension between the lamina papyracea and the medial rectus. The other was an encapsulated mass with regular border located in the superolateral intraconal region measuring 1.9 × 1.7 × 2.0 cm 3 and abutting the optic nerve. Both lesions enhanced mildly with intravenous contrast (Fig 1). Blood results revealed normal thyroid function and immunoglobulin G4 and white blood cell levels. Based on the distinguishing radiological features of each lesion, we performed a two-stage surgery for theranostic reasons. An incisional biopsy of the medial infiltrative lesion was performed first through an anterior orbitotomy via an upper lid skin crease approach. Frozen section of the medial yellow jelly-like mass revealed atypical lymphoid cells with enlarged vesicular nuclei and amphophilic cytoplasm, highly suspicious of lymphoproliferative malignancy. We then performed a complete excision of the vascular encapsulated intraconal lesion using cryotherapy via a lateral orbitotomy. Formal histopathology reports revealed the first medial infiltrative lesion to be consistent with diffuse large B-cell lymphoma with positive immunostaining for CD20, BCL2, BCL6, MUM1, and CMYC1 (Fig 2). The second intraconal lesion was consistent with cavernous haemangioma (Fig 3).

Abstract:We herein report a 76-year-old male who presented to the ophthalmology clinic after complaining from excessive lacrimation for ~6 months prior to presentation. He reports previously undergoing a nasolacrimal recanalization procedure in attempts to resolve his symptoms, but reported no improvement. On examination, a mass on the nasal conjunctiva around the medial canthus of the right eye was noted and subsequently excised. The findings support the diagnosis of low-grade follicular lymphoma. The patient was referred to radiation oncology for radiation therapy and received a total dose of 2400 cGy. Most ocular adnexal lymphomas are B-cell in origin, with follicular lymphomas being one of the rarest forms of such lymphomas. The most common translocation reported in over 85% of follicular lymphomas of the ocular adnexa is t(14; 18) (q32; q21). Traditional treatment options typically include a mix of chemical, surgical, and radio-oncological interventions.
Eyelid atypical fibroxanthoma: a rare challenging entity

Abstract:Atypical fibroxanthoma (AFX) is a rare neoplasm, with a limited number of cases reported in the periocular region. In this case report, we detail a 63-year-old woman who presented with a polypoid, exophytic lesion on her right upper eyelid that had been progressing for a year. The lesion was meticulously excised with security margins and reconstructed using a glabellar flap. Following a thorough microscopic and immunohistochemical analysis, AFX was diagnosed. Despite its sometimes clinical and histological benign appearance, AFX is classified as a malignant neoplasm; however, it carries an excellent prognosis with low metastasis and recurrence rates. Complete excision with safety margins is essential and an adequate post-operative surveillance is recommended. Owing to its rarity, ophthalmologists should remain vigilant and include AFX in their differential diagnosis, as the tumor's benign appearance may lead to misdiagnosis of this malignant entity.

Abstract:Backgrounds/aims: The authors describe the case of a 79-year-old Caucasian woman who presented an ocular adnexal lesion as the first clinical manifestation of a systemic follicular lymphoma, highlighting the clinicopathological features of this rare entity and its potential to be misdiagnosed as marginal zone lymphoma of the mucosa-associated lymphoid tissue. Methods: Conjunctival impression cytology was performed for a rapid initial diagnosis of lymphoma, and subsequent histopathological and immunohistochemical studies were carried out for its confirmation and to identify the best therapeutic regimen. Results: After the initial presentation and diagnosis, she was submitted to complete clinical evaluation; confluent retroperitoneal lymphadenopathy was detected through abdominal computed tomography, characterizing clinical stage III. Conclusion: Awareness of this lymphoma is important when making a diagnosis of ocular adnexal lymphoid neoplasms for its appropriate evaluation and management.

Abstract:Case report: A case is presented of an 85 year-old Caucasian female with lymphoma that recurred in the orbit (secondary ocular adnexal lymphoma). The orbital tumour was a diffuse large B-cell lymphoma according to the REAL classification (Revised European-American Lymphoma Classification). Discussion: Orbital lymphomas are predominantly B-cell proliferations of a variety of histological types, and most are low-grade tumours. Patients are usually middle-aged or elderly, and it is slightly more common in women. A palpable mass, proptosis and blepharoptosis are the most common signs of presentation.

Abstract:Background: Atypical fibroxanthoma (AFX) is a pleomorphic tumor that most commonly presents in the elderly and arises on sun-exposed skin. Methods: We report a case of a 24-year-old white male with xeroderma pigmentosum (XP), who developed an AFX of the conjunctiva, and review the literature. Results: The patient presented with a rapidly growing conjunctival mass which was surgically excised and found to be AFX. The patient has done well with no recurrence. A review of the literature was performed, and we found that only one other case has been published so far. Conclusion: Patients with XP may present with unusual tumors, including nonepithelial soft-tissue tumors. AFX is a rapidly growing tumor which may rarely present on the ocular surface.

Abstract:We report a clinical case of orbital lymphoma masquerading as orbital cellulitis. Orbital lymphoma and lymphoma of the orbital adnexa are relatively rare types of lymphoma. They represent the malignant end of a spectrum of lymphoproliferative lesions that occur in the orbit, conjunctiva and eyelid. Diffuse large B-cell lymphomas (DLBCL) with variable clinical presentations have a relatively poor prognosis with rapid visual loss. Therefore, thorough evaluation and assessment with a multi-specialty effort are warranted for earlier diagnosis and initiation of treatment to save life and prevent sight loss.

Abstract:Purpose: To report a case of diffuse large B-cell lymphoma of the conjunctival fornix.Case summary: A 49-year-old man visited our clinic with redness and foreign body sensation in the right inferior conjunctival fornix that had begun 20 days previously. The patient had no pain, no fever, weight loss, and no past history of chronic disease. On slit lamp biomicroscopic examination, a large, salmon-colored, raised, well-defined, mass was detected in the inferior bulbar and fornix conjunctiva. We performed excision biopsy. The soft tissue lesion was 17 × 5 × 4 mm in size. Microscopic examination identified an area of small, mature mononuclear lymphocytes and an area of lymphocytes, such as large lymphoblasts with divided nuclei and conspicuous nucleoli. On immunohistochemical staining, the diffuse large B-cell lymphoma areas were positive for CD20 and Bcl-6, negative for CD3, CD5, and CD23, and the Ki-67 positive rate was relatively high. Finally, the patient was diagnosed with diffuse large B-cell lymphoma accompanying mucosal-associated lymphoid tissue lymphoma. There were no findings suggestive of metastasis invasion from other organs. The patient underwent immunochemotherapy after excisional biopsy. No recurrence has occurred over 1-year follow-up.Conclusions: Although rare, diffuse large B-cell lymphoma should be considered in the differential diagnosis of conjunctival lymphomatous lesions.

Abstract:Rationale: Eosinophilic angiocentric fibrosis (EAF) is considered to be a kind of benign IgG4-related disease, and it is more often found in the nasal cavity. We present a pretty rare case of orbital EAF that is unlike any other reported case for this case is an IgG4 negative orbital EAF and successfully treated by the fronto orbitozygomatic approach surgery. Patient concerns: This is a 68-year-old man from a rural area of Inner Mongolia Autonomous Region, went to our hospital for a 2-month history of vision loss with a local hospital orbital computer tomography which showed that there was a lesion in his left orbit. The inspection of the patient revealed that the patient left eye was protruding outward and the left eyelid unable to complete open or close. And his left eyeball movement had difficulty in all directions. Postoperative pathology diagnosed that this was a case of IgG4-negative EAF case. Diagnoses: Orbital EAF. Interventions: Surgical radical resection and postoperative glucocorticoid therapy. Outcomes: After surgery, the left eye vision of this patient increased to 0.6 tested in the standard logarithmic visual acuity chart. And his left eyeball movement dysfunction and eyeball outward protruding get a partially relief. Lessons: EAF occurring in the orbit is a very rare disease and immunohistochemical results of EAF can be IgG4 negative.

Abstract:BACKGROUND Anaplastic lymphoma kinase (ALK)-positive large B-cell lymphoma (LBCL) is an aggressive and rare variant of diffuse LBCL. Herein, we report an uncommon case of stage IE extranodal ALK-positive LBCL initially originating in the bulbar conjunctiva. CASE SUMMARY A 63-year-old woman presented with a mass in the left bulbar conjunctiva that had persisted for six months, accompanied by swelling and pain that had persisted for 3 d. Eye examination revealed an 8 mm slightly elevated pink mass in the lower conjunctival sac of the left eye. Microscopically, the tumor was composed of large immunoblastic and plasmablastic large lymphoid cells with scattered anaplastic or multinucleated large cells. Immunophenotypically, the neoplastic cells were positive for ALK, CD10, CD138, Kappa, MUM1, BOB.1, OCT-2, CD4, CD45, EMA, CD79a, CD38, and AE1/AE3, and negative for CD20, PAX5, Lambda, BCL6, CD30 and all other T-cell antigens. The results of gene rearrangement tests showed monoclonal IGH/IGK/IGL and TCRD rearrangements. Fluorescence in situ hybridization studies did not reveal any BCL2, BCL6 or MYC rearrangements. Furthermore, Epstein-Barr virus was not detected by in situ hybridization in the lesions. Based on the histopathological and imaging examinations, the neoplasm was classified as stage IE ALK-positive LBCL. No further treatments were administered. At the 6, 15, and 21 mo postoperative follow-up visits, the patient was in good condition, without obvious discomfort. This case represents the first example of primary extranodal ALK-positive LBCL presenting as a bulbar conjunctival mass, which is extremely rare and shares morphological and immunohistochemical features with a variety of other neoplasms that can result in misdiagnosis. CONCLUSION Awareness of the condition presented in this case report is necessary for early and accurate diagnosis and appropriate treatment.

Abstract:Ocular adnexa region (OAR) primary lymphomas are uncommon, accounting for 1-2% of non-Hodgkin lymphomas and 8% of extranodal lymphomas. Extranodal marginal zone lymphoma (EMZL) originates from several epithelial tissues, including the stomach, salivary gland, lung, small intestine, thyroid gland, and ocular adnexa region. Here, we report a 66-year-old female patient who was diagnosed with EMZL of OAR. In consideration of the possible side effect of radiotherapy, such as conjunctivitis, visual acuity impairment, and even retinal complications, she received six cycles of triweekly targeted chemotherapy with rituximab, cyclophosphamide, vincristine, and prednisone (R-CVP) without radiotherapy. Then, she remained in complete remission up to the present day.

Abstract:A 68-year-old male presented with a one-month history of progressive proptosis and vision loss in the left eye. Examination of the left eye showed visual acuity of NLP, marked relative proptosis of 10 mm, and complete external ophthalmoplegia. CT orbits showed an extensive left orbital lesion with proptosis. Urgent orbital biopsy was undertaken. Intraoperatively, the patient developed new atrial flutter and fever. Bloodwork revealed metabolic derangements suggestive of tumor lysis syndrome. Systemic evaluation revealed a large tumor burden involving the retroperitoneal space. Histopathology of the orbital specimen showed non-germinal center diffuse large B-cell lymphoma. The patient passed away 3 days postoperatively due to rapidly progressive multisystem organ failure. Our case demonstrates an unusually aggressive presentation of DLBCL in which orbital mass was the first presentation of spontaneous tumor lysis syndrome owing to large systemic tumor burden.
The digital divide, inclusion and access for disabled people in IT supported emergency response systems: A UK and EU-based analysis

Abstract:This short insights paper examines IT Supported Emergency Response systems from the perspective of the digital divide and access for disabled people. It is argued that inclusive design is often overlooked in the development of emergency systems and this can lead to increased marginalisation of certain societal groups, such as disabled people and older people. Given the importance of equality of provision in relation to emergency responses there is a need to examine accessibility policy in this area to determine whether it enables the equal participation of all. This is important both in relation to the ability to access key information in an emergency situation and to participate in crowdsourced data generation to help in the targeting of resources, both at the time of an emergency and in subsequent planning. This insights paper takes both a practical and theoretical approach and focuses upon key policies emanating from the EU and UK.

Abstract:Objective: The aim of this case report is to assess the clinicopathological characteristics and differential diagnosis of orbital granular cell tumor (GCT). Methods: Clinical and imaging data of a rare case of orbital GCT involving the superior rectus muscle were collected. Its clinical characteristics, imaging, and histopathological features were observed. Results: A 36-year-old female patient presented with a 2-year history of left eye proptosis. Magnetic resonance imaging (MRI) enhancement suggested a space-occupying lesion in the left superior rectus muscle region. On T1-weighted and T2-weighted MRI, the tumor was isointense to gray matter and significantly enhanced on the enhanced scan. Microscopic examination revealed that most tumor cells exhibited diffuse growth with unclear boundaries, and some cells were arranged in small nests. The tumor cells were large, with abundant, coarse eosinophilic granules in the cytoplasm. Occasional cells contained larger round eosinophilic droplets in the cytoplasm. Focal areas showed foamy cells, small and central round or oval nuclei with occasional nuclear enlargement and mild atypia, inconspicuous nucleoli, rare mitoses, and low proliferative activity. Immunohistochemistry results were Vimentin (+), S-100 (+), CD68 (+), Ki67 (2%+), Inhibin-a (−), CK (−), SMA (−), and Desmin (−). The pathological examination of a specimen harvested from the mass corresponded to a GCT. Conclusion: Orbital GCT is rare and should be considered in the differential diagnosis of orbital tumors. It is essential to distinguish it from thyroid-associated ophthalmopathy, inflammatory pseudotumor, and myohemangioma. Definitive diagnosis requires a comprehensive analysis of clinical, histopathological, and immunohistochemical findings. Surgical excision is the primary treatment for orbital GCTs. For patients with incomplete tumor resection, close follow-up is necessary. Proton beam radiation therapy can be considered to prevent recurrence or metastasis if needed.

Abstract:Chronic lymphocytic leukemia is characterized by the accumulation of mature CD5-positive B-cells in the lymphoid organs.1 Extranodal involvement occurs in up to 10% of cases and can arise in various tissues, including the orbit. Less than 400 cases of orbital lymphoma are diagnosed per year in the United States, typically manifesting as a form of B-cell non-Hodgkin lymphoma, with extranodal marginal zone B-cell lymphoma being the most common subtype. Orbital lymphoma typically presents with proptosis and a palpable mass; however, patients may also have a relatively benign examination. Here, we present a 76-year-old man with symmetric dermatochalasis and marked fat prolapse of all four lids, who was incidentally diagnosed with secondary orbital lymphoma in all four eyelids during a cosmetic four lid blepharoplasty. His history was significant for RAI Stage 0 chronic lymphocytic leukemia diagnosed 15 years before consultation. Orbital lymphoma presenting as orbital fat prolapse has only been reported a few times in the literature. To our knowledge, this is the first case of secondary orbital lymphoma in all four eyelids found incidentally during an aesthetic four lid blepharoplasty.
