Abstract:The objective of the study was to evaluate the effectiveness of the supraorbital "keyhole'' approach with endoscope assistance in surgical treatment of benign tumors around the sellar region. Thirty-five patients, including 19 pituitary tumors, 11 craniopharyngiomas and five tuberculum sellae meningiomas, were enrolled in this study. The tumors were resected through an endoscope-assisted supraorbital keyhole approach via a small skin incision within the eyebrow. Complete removal of the sellar region tumors was achieved in all 35 cases by endoscope-assisted supraorbital keyhole approach. Mean length of hospital stay after surgery was 10.2 days (range 5 - 17). There was no patient with evidence of residual or recurrent tumor during the follow-up period. There was no infection, bleeding, further vision impairment, oculomotor nerve injury or other cranial nerve injury symptom owing to surgery. Though some patients suffered from insipidus, hyperprolactinemia, subcutaneous edema or other postoperative complications, they eventually recovered with or without drug administration. The supraorbital "keyhole'' approach with endoscopic assistance in the surgical treatment of benign tumors around the sellar region is an ideal pattern.
Endoscopic Endonasal Intraconal Approach for Orbital Tumor Resection: Case Series and Systematic Review

Abstract:Intraorbital tumor could be approached by numerous surgical methods. The neuroendoscopic endonasal approach could provide a feasible corridor for indicated tumors. Herein we present a series of 6 consecutive intraorbital tumors from April 2018 to October 2020, which received endonasal endoscopic resection. Cadaveric dissection was performed for the intraconal approach, and the literature was also reviewed. Five tumors were located intraconally, while one extraconally. The pathology revealed 1 angioleiomyoma, 1 cavernous hemangioma, 1 pilocytic astrocytoma, 1 meningioma, and 2 schwannomas. Five of the six achieved gross total resection, including 3 tumors with lateral extension beyond the optic nerve. Preoperative visual deterioration was observed in 4 of the 6 patients, and all got improvement postoperatively. Transient oculomotor nerve palsy was presented in one patient postoperatively. No cerebrospinal fluid leakage, enophthalmos, or strabismus was observed. The median follow-up time is 27 months (11~41 months). At the 6-month follow-up, the visual acuity remained unchanged compared with that at discharge. Proptosis was resolved in 2 of the 3 patients; diplopia was improved in one patient. In conclusion, endoscopic endonasal intraconal approach could be suitable for selected pathological conditions, and for both medial or beyond medial extraconal and intraconal orbital tumors.

Microsurgical Resection of Sellar Tumors Via Lateral Supraorbital Approach:Clinical Analysis of 20 Cases

Abstract:Objective: To research the application of lateral supraorbital (LOS) approach for resecting sellar tumors. Methods: Clinical symptoms of 20 patients with surgery via LOS approach were analyzed retrospectively.According to the neuroradiological findings and the improvement of symptoms, and some score scale, the efficacy and safety of LOS were assessed for removing sellar tumors.There were tuberculum sellae meningioma (n=11), craniopharyngioma (n=4), epidermoid cysts (n=4) and pituitary adenoma (n=1). Results: Complete resection was achieved in seventeen patients(85%). The mean operating time was 144.0±54 minutes and the mean intraoperative blood loss was 96.5±51.1 ml.Four patients had post-operative fever, four had endocrine disorders, and one had seizure.No other complications were noted.Fifteen patients had visual function impairment before operation, seven of which were improved while one was aggravated after surgery.The median Karnofsky score (3-46 mon) was 95(0-100) and the Glasgow outcome scale (GOS) at follow-up (3-46 mon) was 4.7(0-5). In general, the outcomes were improved in 18 patients and the mortality was 2. Conclusion: The resection of tumors at sellar region via lateral supraorbital approach is effecient, simple and minimally invasive, which can make the removal of the tumors reliably and safely.It is worthy to be popularized clinically.
Endoscopic transorbital approach for skull base lesions: a report of 16 clinical cases

Abstract:Due to the deep location, complex anatomy, and adjacent vital neurovascular structures, skull base surgery is challenging and requires specific approaches. The emerging endoscopic transorbital approach (eTOA) technique provides a new approach to the orbital content, spheno-orbital region, lateral cavernous sinus, and Meckel's cave. In this study, the clinical utility and effectiveness of the eTOA are reported. Sixteen cases who underwent the eTOA were included in the current study. The patients were divided into 3 groups according to tumor location: Group A (intraorbital, 6 cases), group B (spheno-orbital, 7 cases), and group C (cavernous sinus, and Meckel's cave, 3 cases). The clinical data and surgical results were analyzed. Eight meningiomas, 2 hemangiomas, 1 low-grade glioma, 1 instance of inflammatory hyperplasia tissue, 1 Langerhans cell histiocytosis, 1 epidermoid cyst, 1 trigeminal schwannoma, and 1 bone fibrosis hyperplasia were observed. The mean tumor diameter was 2.4 cm. A single case in Group A and Group C underwent biopsy (12.5%), and 1 case of fibrous dysplasia in Group B underwent sufficient orbit decompression (6.25%). The remaining 13 cases underwent gross total tumor resection (81.25%). No cerebral-spinal fluid leak or infection occurred. And no cosmetic problems or significant complications were observed during the follow-up. As a minimally invasive technique, the eTOA has unique advantages for carefully selected skull base lesions because of its direct route, short working distance, and distinct attack angle.

Abstract:Autoinduction is a conserved mechanism of cell density-dependent gene regulation that occurs in a variety of gram-negative bacteria. Autoinducible luminescence in Vibrio fischeri requires a transcriptional activator, LuxR, while a LuxR homolog, LasR, activates elastase expression in Pseudomonas aeruginosa. Both LuxR and LasR require specific signal molecules, called autoinducers, for activity. We show here the activation in Escherichia coli of the V. fischeri luminescence (lux) operon by LasR and of the P. aeruginosa elastase gene (lasB) by LuxR when each is in the presence of its cognate autoinducer. Neither LuxR nor LasR showed appreciable activity with the heterologous V. fischeri or P. aeruginosa autoinducer. This supports the view that there is a direct interaction of each transcriptional activator with its proper autoinducer and suggests that there are conserved, autoinduction-related elements within the promoter regions of these genes.
Endoscopic Endonasal Approach for Optic Canal Dural Metastasis in a Patient with Progressive Visual Dysfunction: A Case Report

Abstract:To improve optic nerve function in a patient with progressive visual dysfunction, performing early decompressive and debulking surgery for a metastatic tumor located in the optic canal is essential. The endoscopic endonasal approach could be a practical and effective alternative for lesions in the inferomedial part of the optic canal. A 66-year-old man with a right visual eye field deficit had multiple lesions in the pineal gland, occipital lobe, and right inferomedial optic canal. The optic nerve was distorted by a tumor compressing against the falciform ligament. Although a systemic examination suggested the presence of primary lung cancer, the patient only complained of progressive visual impairment in the right eye. We planned surgery with endoscopic transethmoidal and transsphenoidal approaches to restore visual function and make a pathological diagnosis. During the procedure, we drilled the sella floor, tuberculum sellae, and optic canal and successfully removed the tumor underneath the dura mater. The patient's visual function improved rapidly following surgery, and no complications were observed, such as cerebrospinal fluid leakage. After confirming the pathological diagnosis, the patient subsequently received whole-brain radiotherapy. The endoscopic endonasal skull base approach to the optic canal region could be a practical alternative for treating symptomatic metastatic tumors.

Abstract:A middle-aged lady was referred to the Otolaryngology department for consideration of an endoscopic biopsy of a mass located in the right orbital apex. She had initially presented to the eye clinic with complaints of right proptosis and worsening vision. Upon examination, her right eye showed no perception of light, an amaurotic right pupil, and limited extraocular movement in all directions of gaze. Hertel's ophthalmometer revealed proptosis, and there was a loss of corneal sensation. Magnetic resonance imaging (MRI) of the orbits revealed an enhancing mass in the right orbital apex, extending into the cavernous sinus. She subsequently underwent an endoscopic trans-sphenoidal biopsy of the lesion. Histopathological analysis confirmed the pre- sence of squamous cell carcinoma (SCC). Further systemic evaluation did not reveal any other primary site or metastasis. Conside- ring the unresectable nature of the tumor and her poor candidacy for chemotherapy and radiotherapy, the patient was managed with best supportive care.

Abstract:Orbital Cavernous venous malformations (OCVMs) are well defined benign lesions. Due to their well encapsulated nature, they are easily dissectible from the surrounding orbital structures. After lymphoid tumours and idiopathic orbital inflammation, orbital Cavernous venous malformations are the third most common intraconal pathology [1] , [2] , [3] . These lesion have a well-defined contrast enhancement on CT and MRI [4] . A Middle aged female presented with history of protrusion of the left eye which was insidious onset and gradually progressive in nature. She had no history of visual disturbances or double vision. Imaging showed an intraconal lesion in the left eye which was inferior and medial to the optic nerve. Given the size and location of the tumour it was easily approachable via endoscopic endonasal transmaxillary transorbital approach [5] . We present a 2-dimensional video of endoscopic endonasal transmaxillary approach to intraorbital intraconal Cavernous venous malformation. Informed consent was taken from the patient. The patient has consented to publish any identifiable image or information pertaining to her. Histopathological examination revealed a cavernous hemangioma. Recovery was uneventful and proptosis resolved completely.

Abstract:Background Lesions situated within the orbit pose significant challenges in management due to the confined space they occupy and their proximity to critical anatomical structures. The objective of our study is to assess the feasibility of the ipsilateral endoscopic endonasal approach for orbital cavernous hemangiomas and to comprehend the surgical anatomy of the orbital apex and inferomedial orbital structures. Methods Thirteen patients with orbital cavernous hemangioma, who underwent surgery via the ipsilateral mononostril endoscopic endonasal approach between August 2018 and August 2023, were retrospectively evaluated. Demographic characteristics, clinical data, radiographic images, and clinical outcomes of the patients were collected from digital medical records. Results In the study, we included 13 patients (8 female, 5 male) with ages ranging from 25 to 54 years (mean 35.2 ± 8.3). The left orbit was more commonly affected (9 left, 4 right). The average postoperative follow-up duration was 22.2 months (range 6 months – 50 months). Among the 13 cases of orbital cavernous hemangioma, 1 (7.7%) was located in the extraconal compartment, while 12 (92.3%) were situated in the intraconal compartment. Complete surgical resection of the tumor was successfully achieved in all patients. Conclusions Our study highlights the potential advantages of employing a purely ipsilateral mononostril endoscopic endonasal approach for orbital surgery, particularly for accessing the orbital apex and managing medial and inferomedial orbital lesions. This technique holds promise for reducing morbidity and enhancing outcomes, especially when combined with careful patient selection, preoperative planning, and advanced endoscopic skills.

Abstract:Despite a decade and a half of digital imaging in radiology, most radiology teaching files remain film-based. The reasons include the high cost or unavailability of digital acquisition and display devices. In the past few years, a number of excellent exhibits that combine inexpensive microcomputers with radiologic education have been shown at national meetings [1-6]. Unfortunately, many of these methods require esoteric hardware, expensive or proprietary software, or special programming skills. We developed a simple method for creating and disseminating a teaching file of imaging studies obtained with digital techniques.

Abstract:Spheno-orbital meningiomas are challenging invasive tumors, involving different cranial regions, requiring multiple surgical approaches and often with an unfavorable biological behavior characterized by multiple recurrences and possible escape from standard treatments. 1 We present a case of a 57-year-old woman, already undergone a transcranial approach, an endoscopic transorbital surgery, and a radiosurgical treatment for a spheno-orbital meningioma. She presented a further tumoral recurrence, involving the orbit and the infratemporal and pterygopalatine fossa, and we proposed a combined fully endoscopic multistaged approach. The patient consented to the procedure and to the publication of his/her image. We adopted the endoscopic transorbital and the endoscopic transmaxillary-pterygoid corridors for the different portions of the tumor, followed by radiation therapy with carbon ions, achieving a large tumor resection with no progression at 12-month follow-up and without complications or neurological sequelae. This treatment strategy gave us the possibility to fully manage the tumoral extension with 2 innovative minimally invasive surgical procedures, which resulted well-tolerated and favored the prompt patient recovery and quality-of-life preservation, leaving the meningioma remnant to the external radiation therapy. 2-6 Our case emphasizes the possibilities given by the endoscopic approaches to manage at 360° the multiregional extensions of a spheno-orbital meningioma, demonstrating how such direct and extracranial corridors could allow the surgeon to remove these tumors, with no brain retraction or manipulation and limited functional or neurological sequelae. 7,8

Abstract:To study the feasibility of the modified osteotomy of transcranial orbitotomy in the treatment of intraorbital tumor.We treated 8 patients with intraorbital tumor during six years. By the bicoronary incision, all cases underwent double bone flap osteotomy on the frontal bone: the superior orbital rim bone flap and roof flap instead of single fronto-orbital bone flap in the conventional transcranial orbitotomy. After removal of bone flaps, intracranial and intraorbital operation was performed. Then, two bone flaps were reduced respectively and fixated with titanic micro-plates and nails. At last, the scalp flap was sutured.The operative field was very well exposed. It was found that the retrobulbar tumor was located at the superolateral, median and superonasal area respectively, which was coincided with the preoperative CT and MRI. The tumor included adenocarcinoma of the lacrimal gland, neurinoma, meningioma, and cavernous hemangioma. Four patients had blood transfusion during the operation. No other postoperative complications happened except 3 cases of diplopia and one case of blindness. After 3 to 6 months follow up, diplopia of the 3 cases gradually disappeared. Of the 8 cases, 6 reached the same visual acuity as the preoperative state. One decreased visual acuity and one lost light perception. There was well-balanced fronto-orbital appearance and eyeball position compared with the healthy side in all cases.This surgical method for the treatment of intraorbital tumor is safe with well-exposed operative field. It has advantages not only in simpleness and less trauma, but also in keeping orbital roof and anterior fossa intact and decreasing complications.

Abstract:Objective To investigate the fcasibility and efficacy of resection of orbital apex lesions through transnasal endoscopic approach.Methods From January 2009 to June 2013,18 patients with orbital apex lesions were analyzed retrospectively,of which the main symptoms were proptosis and vision loss.In this series,6 cases were orbital foreign body,4 cavernous hemangioma,3 trans-nasal-orbital mucinous cyst,2 inflammatory pseudotumor,1 cholesteatoma,1 neurofibroma,and 1 fibrodysplasia.All the patients underwent operation through ethmoid sinus with endoscopic techniques into orbit or orbital apex.Results All the foreign bodies were taken out by one operation.4 cavernous hemangiomas in orbital apex were totally removed,in two of them the postoperative visions were improved and another two unchanged.In 8 trans-nasal-orbital tumors,4 were totally removed,3 partially removed,of which the proptosis and eyeball movement were improved.All the patients were followed-up from 3 months to 4 years.1 inflammatory pseudotumor recurred and was removed again.There was no recurrence in the rest of cases,and the symptoms were disappeared.Conclusions In carefully selected cases,the lesions in orbit and orbital apex could be removed feasibly through transnasal endoscopic approach.

Abstract:Nasal natural killer/T-cell lymphoma (NK/TCL) is a rare form of malignant non-Hodgkin lymphoma (NHL) with a far more rare involvement of orbit. The orbital involvement has a highly variable clinical presentation. Here, we report one such case of a 40-year-old male patient who presented with swelling in the right upper and lower eyelids with the diminution of vision for 12 days. He had a history of blocked nose for two months. Clinical examination and CT scan of the orbit and paranasal sinuses suggested a diagnosis of right orbital cellulitis with pansinusitis. A combination of intravenous antibiotics was started, and functional endoscopic sinus surgery was done. Histopathology was a suggestive of nasal NK/TCLl NHL. After proper staging, the patient was given chemotherapy and radiotherapy. There was a complete resolution of mass with no recurrence over a follow-up of 10 months.
Modified Function-Preserving Endoscopic Endonasal Extracapsular Resection of a Large Orbital Apex Cavernous Hemangioma

Abstract:BACKGROUND AND IMPORTANCE: Various invasive oculoplastic procedures are commonly utilized to control the rectus muscles and widen the surgical corridor through the endoscopic endonasal removal of large orbital apex cavernous hemangiomas (OACHs). They require additional transconjunctival incision, rectus muscle insertional retraction, or muscle deinsertion at the globe that might not be safe and lead to prolonged postoperative extraocular muscle dysfunction. In this article, the authors described a modified 3-handed extracapsular technique for the resection of a large OACH without an additional procedure for rectus muscle control. The aim is to achieve a safe gross total tumor removal while minimizing the procedure-related complications. An intraoperative video is included, along with a stepwise cadaveric dissection relevant to the approach. CLINICAL PRESENTATION: A 71-year-old female presented with progressive left-sided blurred vision, binocular diplopia, and mild proptosis. Contrast-enhanced brain MRI revealed a large heterogeneous enhanced inferomedial intraconal mass in the left orbital apex, mostly consistent with cavernous hemangioma. Gross total tumor removal was achieved through a modified 3-handed endoscopic endonasal extracapsular approach. The diplopia was resolved, and significant visual improvement was achieved. Computed tomography scan demonstrated complete tumor removal, and histological examination confirmed the diagnosis. CONCLUSION: Endoscopic endonasal resection of large OACH can be feasibly performed by using a modified 3-handed extracapsular technique through the generous use of Q-tip swab applicators within the natural separation plane around the tumor capsule and a sequential traction-countertraction method. Subsequently, a gross total removal and optimal postoperative functional outcome are attainable through minimal rectus muscle fiber violation and intraconal fat manipulation.

Bilateral Lacrimal Gland Lymphoma - Case Presentation

Abstract:Lacrimal gland lymphomas are rare orbital tumors, constituting a minor fraction of all orbital and ocular adnexal malignancies. This case study presents an 83-year-old male with bilateral lacrimal gland tumors, more prominent in the left orbit, causing decreased visual acuity, red eye, excessive tearing, and diplopia. Initial ophthalmological evaluations and imaging suggested bilateral lacrimal gland lymphoma, confirmed by histopathology as diffuse large B-cell non-Hodgkin lymphoma of the MALT type. Due to the significant tumor size and risk of visual function loss, surgical intervention was performed, followed by corticosteroid therapy. Postoperatively, a marked improvement in symptoms and a reduction in tumor size were observed. This case underscores the importance of comprehensive diagnostic approaches, including clinical, imaging, and histopathological evaluations, highlighting the need for a multidisciplinary approach in managing rare orbital tumors like lacrimal gland lymphoma. The patient's postoperative and follow-up care included oncological management to monitor and ensure long-term disease control and patient well-being. Abstract:Introduction: Extramedullary plasmacytoma (EMP) is a disorder involving plasma cells, leading to the development of neoplastic masses outside the bone marrow. While EMP can occur in different body regions, its predominant manifestation is observed in the head and neck area. A 44-year-old female presented with nasal obstruction, left-sided proptosis, and infraorbital swelling. Rigid nasal endoscopy revealed a polypoidal mass filling both nasal cavities, and imaging showed a uniform density mass lesion involving bilateral sinuses extending into the left orbit. The lesion was resected endoscopically, with subsequent histopathological examination confirming the diagnosis of EMP. Ancillary studies ruled out multiple myeloma (MM). The intraorbital component of the tumour was successfully treated using primary radiotherapy, leveraging the tumours’ inherent radiosensitivity to minimize surgical morbidity.Conclusions: This case underscores the importance of including EMP in the differential diagnosis of sinonasal and other head and neck tumours. The favourable outcome achieved through a combination of surgery and radiotherapy highlights the significance of adopting a multidisciplinary approach in navigating challenging clinical scenarios, ensuring improved patient outcomes.

Abstract:A case is presented of a 43-year-old male with a chronic history of progressive nasal obstruction and epiphora. MRI confirmed a heterogeneous mass involving the middle and superior turbinates with T2 hyperintense and calcified components, with extension into the inferomedial orbit. Tissue biopsy revealed a grade 2 chondrosarcoma of the conventional subtype. Endonasal wide local resection of the lesion was performed with clear margins. The patient had no functional sequelae and will undergo routine surveillance.

Abstract:Background: Tuberculum sellae meningiomas (TSMs) are benign dural-based lesions of the anterior cranial fossa, which mainly present with impaired visual acuity/field deficits secondary to compression of the optic apparatus. Surgical management is recommended as the optimal strategy for large compressive TSMs, with goals of safe maximal resection, optic nerve decompression, and potential vision restoration. The philosophy of adapting keyhole approaches for such resections is commonly highlighted; however, it comes with notable criticism of encountering major blind spots during surgical resection and limited anatomical exposure. Adding angled endoscopes enhances the expanded panoramic view of the skull base and provides a synergistic modality to microsurgery for maximizing total resection and navigating the blind spots. Case description: This video case presentation aims to highlight the technical nuances of endoscope-assisted microscopic supraorbital craniotomy for TSM resection invading bilateral optic canals in a 66-year-old female presenting with progressive right eye vision loss (OD Hand motion). The video emphasizes traditional skull-base surgical principles of TSM resection through the optics of a keyhole approach augmented by endoscopic tumor removal. Gross total resection was achieved, the patient's vision improved to 20/25, and she was discharged home on postoperative day 2. Conclusion: The endoscope-assisted supraorbital craniotomy offers a safe surgical corridor for TSM, using a limited craniotomy with minimal brain retraction in appropriately selected individuals, particularly with larger tumors with greater lateral extension and above the planum.
