Abstract:Endovascular aneurysm repair (EVAR) is a minimally invasive technique widely used in abdominal aortic aneurysm treatment. The most common complications after EVAR remain endoleaks, type 2 endoleak (T2EL) being the most prevalent. T2EL detection and surveillance require imaging techniques such as computed tomography angiography (CTA), contrast-enhanced magnetic resonance angiography (CE-MRA), Doppler ultrasound, or digital subtraction angiography (DSA). However, these modalities are associated with numerous limitations, including exposure to ionizing radiation, contrast media administration, or operator dependency, as in the case of ultrasonography. A non-contrast-enhanced MRA could be a substitute non-invasive method for endoleak monitoring. Our case report describes an 83-year-old female patient with type 2 endoleak and enlarging aneurysm sac detected on a CT scan. Despite the enormous aneurysm size, the patient underwent endovascular treatment owing to multiple comorbidities. Due to challenging feeding vessel anatomy, catheterization of the aneurysmal sac was impossible. Attempted polymerization of the aneurysmal sac with Glubran-2 partially sealed the sac and obliterated the feeders' inflow. Unfortunately, a non-targeted embolization resulted in the loss of patency of the right feeder and adjacent communicating branch. The patient underwent follow-up imaging that included non-contrast-enhanced as well as contrast-enhanced MRA. The examination revealed the presence of a small residual endoleak, a freshly formed thrombus, and areas of old thrombi. The NCE-MR appeared to be a valuable tool in endoleak detection and provided a detailed clot morphology.

Detection of Endoleaks after Endovascular Repair of Abdominal Aortic Aneurysm: Value of Unenhanced and Delayed Helical CT Acquisitions

Abstract:PURPOSE: To assess unenhanced and delayed phase computed tomographic (CT) images combined with arterial phase images for detecting endoleaks after endovascular treatment for abdominal aortic aneurysm (AAA).MATERIALS AND METHODS: CT scans were retrospectively evaluated for the presence of endoleaks after endovascular treatment of AAAs in 33 patients with endoleak (positive group) and 40 patients without evidence of endoleak or aneurysm enlargement (negative group). All patients underwent unenhanced and biphasic contrast material-enhanced CT. The CT scans were reviewed in the following combinations: (a) arterial phase and unenhanced scans (uniphasic/unenhanced set), (b) arterial and delayed phase scans only (biphasic set), and (c) arterial and delayed phase scans with unenhanced scans (complete set). Each set was reviewed by two radiologists blinded to the diagnosis of endoleak. Findings were recorded as positive, negative, or indeterminate for endoleak.RESULTS: Within the positive group, endoleaks were diagnosed with the uniphasic/unenhanced, biphasic, and complete image sets in 30 (91%), 32 (97%), and 33 (100%) patients, respectively. With the uniphasic/unenhanced set, three (9%) endoleaks (seen only on delayed phase images) were missed. With the biphasic set, one (3%) endoleak was interpreted as indeterminate. Within the negative group, uniphasic/unenhanced, biphasic, and complete image sets were negative for endoleaks in 100%, 80%, and 100% of patients, respectively. With the biphasic set, results were indeterminate in 20% of cases.CONCLUSION: A delayed CT acquisition enables detection of additional endoleaks, while an unenhanced acquisition helps eliminate indeterminate results. Thus, both acquisitions contribute to accurate diagnosis of endoleaks when combined with an arterial phase acquisition.

Abstract:Background The aim of this study was to assess the feasibility and safety of dynamic volumetric computed tomography angiography ( DV ‐ CTA ) for endoleaks detected but not classified by conventional CTA in patients after endovascular aortic repair. Methods and Results From January 2016 to October 2017, 24 patients with endoleaks with aneurysm sac enlargement detected but not classified by conventional CTA were randomly assigned to the conventional CTA group and the DV ‐ CTA group for further evaluation. The amount of contrast agent, radiation dosage, and changes in creatinine during the operation were compared between the 2 groups. Reintervention was performed according to the endoleak classification followed by the 6‐ and 12‐month follow‐up. The accuracy of classifying endoleaks by DV ‐ CTA was comparable to that by digital subtraction angiography. Additionally, the total amount of contrast agent and the radiation dosage in the DV ‐ CTA group during the operation were diminished by 14.0% ( P =0.007) and 12.1% ( P =0.004), respectively, compared with those in the conventional CTA group. No contrast‐induced nephropathy was observed. All endoleaks were treated instantly after identification. No endoleaks were found in any of the patients during follow‐up. Conclusions DV ‐ CTA could replace digital subtraction angiography as an alternative method for the classification of endoleaks that cannot be differentiated by conventional CTA . Additionally, the amount of contrast agent and the total radiation dosage were substantially reduced, which improved safety among operators and patients.

Abstract:Aim: To evaluate contrast-enhanced ultrasound (CEUS) as an effective alternative to CT-angiography (CTA) for endoleak detection and aneurismal sac diameter measurement in the follow-up after endovascular abdominal aortic aneurysm repair (EVAR). Methods: From January 2006 to December 2010, 395 patients underwent EVAR follow-up with both CTA and CEUS. The diameter of the aneurismal sac and the presence of endoleaks were evaluated in all the 395 paired examinations. Results: Bland-Altman plots showed a good agreement in aneurismal sac diameter evaluation between the two imaging modalities. The mean diameter was 54.93 mm (standard deviation (SD) ±12.57) with CEUS and 56.01 mm (SD ± 13.23) with CTA. The mean difference in aneurismal sac diameter was -1.08 mm ± 3.3543 (95% confidence interval (CI), -0.75 to -1.41), in favour of CTA. The number of observed agreement in endoleak detection was 359/395 (90.89%). The two modalities detected the same type I and type III endoleaks. McNemar's χ(2) test confirmed that CTA and CEUS are equivalent in endoleak detection. Conclusions: CEUS demonstrated to be as accurate as CTA in endoleak detection and abdominal aortic aneurysm diameter measurements during EVAR follow-up, without carrying the risks of radiation exposure or nephrotoxicity. Even if it cannot be proposed as the sole imaging modality during follow-up, our analysis suggests that it should have a major role.

Abstract:Journal of Endovascular Therapy, Ahead of Print. Introduction:Endoleaks represent one of the main complications after endovascular aortic repair (EVAR) and can lead to increased re-intervention rates and secondary rupture. Serial lifelong surveillance is required and traditionally involves cross-sectional imaging with manual axial measurements. Artificial intelligence (AI)-based imaging analysis has been developed and may provide a more precise and faster assessment. This study aims to evaluate the ability of an AI-based software to assess post-EVAR morphological changes over time, detect endoleaks, and associate them with EVAR-related adverse events.Methods:Patients who underwent EVAR at a tertiary hospital from January 2017 to March 2020 with at least 2 follow-up computed tomography angiography (CTA) were analyzed using PRAEVAorta 2 (Nurea). The software was compared to the ground truth provided by human experts using Sensitivity (Se), Specificity (Sp), Negative Predictive Value (NPV), and Positive Predictive Value (PPV). Endovascular aortic repair–related adverse events were defined as aneurysm-related death, rupture, endoleak, limb occlusion, and EVAR-related re-interventions.Results:Fifty-six patients were included with a median imaging follow-up of 27 months (interquartile range [IQR]: 20-40). There were no significant differences overtime in the evolution of maximum aneurysm diameters (55.62 mm [IQR: 52.33-59.25] vs 54.34 mm [IQR: 46.13-59.47]; p=0.2162) or volumes (130.4 cm3 [IQR: 113.8-171.7] vs 125.4 cm3 [IQR: 96.3-169.1]; p=0.1131) despite a −13.47% decrease in the volume of thrombus (p=0.0216). PRAEVAorta achieved a Se of 89.47% (95% confidence interval [CI]: 80.58 to 94.57), a Sp of 91.25% (95% CI: 83.02 to 95.70), a PPV of 90.67% (95% CI: 81.97 to 95.41), and an NPV of 90.12% (95% CI: 81.70 to 94.91) in detecting endoleaks. Endovascular aortic repair–related adverse events were associated with global volume modifications with an area under the curve (AUC) of 0.7806 vs 0.7277 for maximum diameter. The same trend was observed for endoleaks (AUC of 0.7086 vs 0.6711).Conclusions:The AI-based software PRAEVAorta enabled a detailed anatomic characterization of aortic remodeling post-EVAR and showed its potential interest for automatic detection of endoleaks during follow-up. The association of aortic aneurysmal volume with EVAR-related adverse events and endoleaks was more robust compared with maximum diameter.Clinical ImpactThe integration of PRAEVAorta AI software into clinical practice promises a transformative shift in post-EVAR surveillance. By offering precise and rapid detection of endoleaks and comprehensive anatomic assessments, clinicians can expect enhanced diagnostic accuracy and streamlined patient management. This innovation reduces reliance on manual measurements, potentially reducing interpretation errors and shortening evaluation times. Ultimately, PRAEVAorta's capabilities hold the potential to optimize patient care, leading to more timely interventions and improved outcomes in endovascular aortic repair.

Abstract:The objective was to assess the diagnostic accuracy of microvascular flow imaging ultrasound (MVUS) for the detection of endoleak after endovascular aortic aneurysm repair. A systematic search of the literature published until January 2024 was performed according to the Preferred Reporting Items for Systematic reviews and Meta-Analyses of Diagnostic Test Accuracy (PRISMA-DTA) guidelines. The pooled rates of sensitivity, specificity, and diagnostic odds ratio of MVUS in endoleak detection with computed tomography angiography as the reference standard were estimated using univariate random-effect analysis with 95% confidence intervals. Three studies were considered eligible for inclusion. Superb micro-vascular imaging (SMI) was used as an MVUS technique in all cases. The total number of paired scans was 209. Sensitivity and specificity of the individual studies ranged 0.75-1.00 and 0.93-1.00, respectively. Pooled sensitivity and specificity of the SMI method was 0.91 (CI: 0.82-0.96) and 0.98 (CI: 0.94-1.00), respectively. The pooled diagnostic odds ratio was 635 (95% CI: 128-3140). The findings of the present study support the use of the MVUS for endoleak detection. However, further research is warranted to explore the broader application of MVUS, providing a more comprehensive understanding and establishing robust criteria for its role in clinical practice.
[Ventricular septal perforation due to weak blunt chest trauma. A case report].

Abstract:Traumatic ventricular septal perforation (VSP) is a rare type of heart injury. This case report describes a 72-year-old Japanese woman who got VSP secondary to weak blunt chest trauma on a train. A two-dimensional color doppler echocardiography and cardiac catheterization study revealed a VSP at muscular portion near the apex. On the 48th day following injury, she was performed a patch closure through left ventriculotomy under cardiopulmonary bypass. The patient was discharged uneventfully.
BMI-Adapted Double Low-Dose Dual-Source Aortic CT for Endoleak Detection after Endovascular Repair: A Prospective Intra-Individual Diagnostic Accuracy Study

Abstract:Purpose: To assess the diagnostic accuracy of BMI-adapted, low-radiation and low-iodine dose, dual-source aortic CT for endoleak detection in non-obese and obese patients following endovascular aortic repair. Methods: In this prospective single-center study, patients referred for follow-up CT after endovascular repair with a history of at least one standard triphasic (native, arterial and delayed phase) routine CT protocol were enrolled. Patients were divided into two groups and allocated to a BMI-adapted (group A, BMI < 30 kg/m2; group B, BMI ≥ 30 kg/m2) double low-dose CT (DLCT) protocol comprising single-energy arterial and dual-energy delayed phase series with virtual non-contrast (VNC) reconstructions. An in-patient comparison of the DLCT and routine CT protocol as reference standard was performed regarding differences in diagnostic accuracy, radiation dose, and image quality. Results: Seventy-five patients were included in the study (mean age 73 ± 8 years, 63 (84%) male). Endoleaks were diagnosed in 20 (26.7%) patients, 11 of 53 (20.8%) in group A and 9 of 22 (40.9%) in group B. Two radiologists achieved an overall diagnostic accuracy of 98.7% and 97.3% for endoleak detection, with 100% in group A and 95.5% and 90.9% in group B. All examinations were diagnostic. The DLCT protocol reduced the effective dose from 10.0 ± 3.6 mSv to 6.1 ± 1.5 mSv (p < 0.001) and the total iodine dose from 31.5 g to 14.5 g in group A and to 17.4 g in group B. Conclusion: Optimized double low-dose dual-source aortic CT with VNC, arterial and delayed phase images demonstrated high diagnostic accuracy for endoleak detection and significant radiation and iodine dose reductions in both obese and non-obese patients compared to the reference standard of triple phase, standard radiation and iodine dose aortic CT.

Fluoroscopy in colonoscopy. Who is using it and why?

Abstract:Use of fluoroscopy during colonoscopy has never been broadly assessed. A survey of 1,864 members of the Society of American Gastrointestinal Endoscopic Surgeons and the American Society of Colon and Rectal Surgeons was carried out to quantify the use of fluoroscopy and to elicit impressions regarding its capabilities, indications, and usefulness. After the establishment of the responding colonoscopist's training, experience, and other background data, impressions of fluoroscopy's role in many issues were obtained using a graded response system. Concluding the two-page survey were open-ended questions addressing the respondent's indications, contraindications, and rationale for using or not using fluoroscopy. Six hundred thirty-one colonoscopists responded. Seventy-five per cent never use fluoroscopy; the most frequently cited reasons were lack of need and inaccessibility of fluoroscopy. For many colonoscopists, fluoroscopy is unavailable (22%) or available outside of the usual endoscopy setting (44%). Fluoroscopy is used by 25 per cent of colonoscopists. Almost three quarters of this group have the capability of performing fluoroscopy in their colonoscopy unit. The indications for fluoroscopy varied with the frequency of its use; frequent users employ fluoroscopy to treat loops, to confirm cecal intubation, and to locate the instrument tip precisely. Infrequent users employ fluoroscopy to apply the sigmoid straightening overtube or because of prior failed colonoscopy. Impressions regarding the impact of fluoroscopy on learning, completing, and safeguarding colonoscopy were obtained. Most colonoscopists are satisfied without using fluoroscopy, although 65 per cent of nonusers believe it would improve colonoscopy performance if it were used. Ninety-two per cent of frequent users of fluoroscopy reported that they would feel significantly impaired without having the capability to perform fluoroscopy.(ABSTRACT TRUNCATED AT 250 WORDS)
THE SYSTEMATIC POSITION OF THE GRAY CATBIRD ( DUMETELLA CAROLZNENSZS ) AS INDICATED BY ITS NASAL MITES ( ACARINA : DERMANYSSIDAE , RHINONYSSINAE )

Abstract:The Gray Catbird (Dumetella carolinensis) is presently considered to be a mimic thrush (Mimidae: mockingbirds and thrashers) (A.O.U. 1957). However, some of its characteristics suggest an affinity with true thrushes (Turdidae : thrushes, solitaires, and bluebirds). Coues (1927) noted the resemblance of Gray Catbird eggs to those of the Robin (Turdus migratorius) . Similarities in nest construction by the Gray Catbird, Veery (Hylocichla fuscesens) , and the Wood Thrush (H. mustelina) have also been described (Nickel 1965) . The catbird’ s maxillo-palatines and other skeletal characters resemble those of the true thrushes and it has been stated that the catbird is nearer to the true thrushes than is any other mimid (Ridgway 1907:182). Indeed, a review of the synonymies reveals that the Gray Catbird has upon occasion been placed in the genus Turdus (Hellmayr 1934). Beecher (1953) found the jaw muscles, bill, palate, and tongue of the mimic and true thrushes to exhibit only minor variations. He dissected the catbird but apparently did not find evidence of a close relationship with the true thrushes. McCabe and Deutsch (1952) found the electrophoretic patterns of egg white proteins of the Gray Catbird, Brown Thrasher (Toxostoma rufum), Robin, and Eastern Bluebird (Sialia sialis) to be similar but drew no conclusion as to the relationship of the catbird and the true thrushes. Clark (1972) recently noted that mockingbirds and thrashers have a single large scute at the base of the two outer toes whereas Dumetella and the other genera of mimids have divided scutes. Divided scutes were also found in the Turdidae and several other families of passerines. The foregoing observations suggest that the catbird may have affinities with both the Mimidae and Turdidae. To assess the parasite evidence relating to this question we surveyed the literature on rhinonyssine mites for records of infections from mimic thrushes, true thrushes, and related groups. Specimens of these mites were studied from Pence’ s collection. This evidence was evaluated with respect to the current hypotheses regarding the biology and systematics of the mites.

Abstract:Background: Endoleaks are common complications after endovascular aneurysm repair (EVAR) for abdominal aortic aneurysm (AAA). Computed tomographic angiography (CTA)/digital subtraction angiography (DSA) is considered the gold standard for evaluating contrast-enhanced ultrasound (CEUS) accuracy in the detection and classification of endoleaks. In recent years, CEUS has been widely used in this field. This study aimed to analyze the accuracy of CEUS in the detection and classification of endoleaks after EVAR. Methods: The data of 98 patients who underwent abdominal aorta CEUS from November 2017 to September 2023 in the ultrasound (US) department of Beijing Hospital were retrospectively analyzed. All the patients underwent EVAR of AAA before CEUS and CTA/DSA, and had complete clinical data. The CEUS and CTA/DSA results were compared to detect endoleaks and categorize the specific types of endoleaks. Results: Among the 98 patients, 74 were male and 24 were female. The patients had an average age of 74.8±9.8 years (range, 43-90 years). Among the 98 patients, 37 (37.8%) endoleaks were detected by CEUS, of which 8 were type Ia, 2 were type Ib, 15 were type II, 7 were type III, 2 were type IV, 2 were type Ia combined with type III, and 1 was type II combined with type III. In addition, among these 98 patients, 39 (39.8%) endoleaks were detected by CTA/DSA, of which 8 were type Ia, 3 were type Ib, 18 were type II, 6 were type III, 2 were type Ia combined with type III, 1 was type II combined with type III, and 1 was type Ib combined with type II. The sensitivity and specificity of CEUS in the detection of endoleaks were 92.3% and 98.3%, respectively. CEUS and CTA/DSA had similar diagnostic efficacy and good consistency in the detection and classification of endoleaks (Kappa value: 0.914, P<0.01). Conclusions: CEUS has high sensitivity and specificity in the detection and classification of endoleaks following EVAR, and its diagnostic efficacy is similar to that of CTA/DSA. In addition, US is safe, non-invasive and repeatable, and thus is worthy of extensive clinical application.

Abstract:Background Persistent type 2 endoleaks (T2EL) require lifelong surveillance to avoid potentially life-threatening complications. Purpose To evaluate the performance of radiomic features (RF) derived from computed tomography angiography (CTA), for differentiating aggressive from benign T2ELs after endovascular aneurysm repair (EVAR). Material and Methods A prospective study was performed on patients who underwent EVAR from January 2018 to January 2020. Analysis was performed in patients who were diagnosed with T2EL based on the CTA of the first postoperative month and were followed at six months and one year. Patients were divided into two groups according to the change of aneurysm sac dimensions. Segmentation of T2ELs was performed and RF were extracted. Feature selection for subsequent machine-learning analysis was evaluated by means of artificial intelligence. Two support vector machines (SVM) classifiers were developed to predict the aneurysm sac dimension changes at one year, utilizing RF from T2EL at one- and six-month CTA scans, respectively. Results Among the 944 initial RF of T2EL, 58 and 51 robust RF from the one- and six-month CTA scans, respectively, were used for the machine-learning model development. The SVM classifier trained on one-month signatures was able to predict sac expansion at one year with an area under curve (AUC) of 89.3%, presenting 78.6% specificity and 100% sensitivity. Similarly, the SVM classifier developed with six-month radiomics data showed an AUC of 95.5%, specificity of 90.9%, and sensitivity of 100%. Conclusion Machine-learning algorithms utilizing CTA-derived RF may predict aggressive T2ELs leading to aneurysm sac expansion after EVAR.

Dose-Response of Ketorolac as an Adjunct to Patient-Controlled Analgesia Morphine in Patients After Spinal Fusion Surgery

Abstract:This randomized, blind study was designed to determine the appropriate dose of ketorolac (a drug used as a supplement to opioids) to administer to patients who have undergone spinal stabilization surgery.The ketorolac was administered every 6 h, in addition to patient-controlled analgesia (PCA) with morphine, to 70 inpatients undergoing spine stabilization by one surgeon. The study was performed to determine the analgesic efficacy and incidence of side effects with different doses of ketorolac. The patients were divided into seven groups. They were given either IV saline (control group) or IV ketorolac (5, 7.5, 10, 12.5, 15, or 30 mg) every 6 h. The outcomes measured included pain scores, 24-h morphine usage, level of sedation, and side effect profile six times during the first 24 h postoperatively. The total dose of morphine was significantly larger in the control and 5 mg ketorolac groups than in the other five groups. Morphine consumption was similar in all groups receiving >or=to7.5 mg of ketorolac. The pain scores were significantly higher in the control group than in some of the larger dose groups at three of the study intervals. The 5 mg group had higher pain scores than the other groups at most of the time intervals studied. There were no significant differences in pain scores among the other five groups. Sedation scores were higher (i.e., patients were more sedated) in the control group than in the other six groups at three of the time periods. We conclude that the administration of ketorolac 7.5 mg every 6 h has a morphine-sparing effect equivalent to that of larger doses in patients undergoing spine stabilization surgery. Using larger doses of ketorolac did not result in less somnolence, lower morphine use, or less pain. We recommend that ketorolac 7.5 mg be given every 6 h to patients undergoing spinal fusion surgery in addition to PCA morphine. Implications: Using smaller doses of ketorolac (e.g., 7.5 mg every 6 h) as a supplement to morphine patient-controlled analgesia is as effective as larger doses in patients who have undergone spine stabilization surgery. Abstract:PurposeTo evaluate the value of metal artifact reduction (MAR) post-processing and iodine MD images in fast kV-switching dual-energy computed tomography (DECT) in patients after endovascular aortic repair (EVAR).Materials and methodsTwenty-four consecutive EVAR patients (age 76 ± 9 years, 7/24 (29%) with coils, 9/24 (37.5%) with 10 endoleaks) who underwent DECT angiography were included in this HIPAA-compliant, IRB-approved retrospective study. Monochromatic reconstructions included 55, 60, 65, 70, and 75 keV with and without MAR and iodine MD images. Near field, far field, and vessel artifacts were assessed subjectively (1 = none; 5 = severe) and objectively by measuring noise and contrast-to-noise ratio. Visibility of endoleak was evaluated (1 = optimal; 5 = not visible).ResultsMAR objectively decreased artifacts from EVAR stents in the near field (60.7 ± 25.4 HU vs. 70.1 ± 34.2; p = .002) and subjectively increased near field (3.2 ± 0.9 vs. 2.8 ± 0.6; p < .001), far field (2.2 ± 0.6 vs. 1.6 ± 0.6; p < .001), and vessel (3.1 ± 1.1 vs. 2.5 ± 0.9; p < .001) artifacts. Near-field artifacts from coils were reduced by the MAR objectively (72.4 ± 24.8 vs. 182.7 ± 57.3 HU; p < .001) and subjectively (4.5 ± 0.5 vs. 4.9 ± 0.4; p = .02). CNR of standard reconstructions was optimal at 60 keV (38.3 ± 16.8). Reconstructions without MAR and iodine MD images provided improved endoleak visualization in 6/10 (60%) of cases (median 1 for both) compared to MAR (median 3) (p < 0.001). However, MAR improved visualization in 1/10 (10%) cases due to endoleak location adjacent to a coil.ConclusionDECT with MAR reduced artifacts from coils and improved endoleak visualization in 1/10 (10%) cases due to location adjacent to a coil. However, MAR impaired endoleak visualization in 6/10 (60%) cases and should be reviewed combined with 60 keV standard reconstructions and iodine MD images.

Abstract:Background Multiple studies have reported the clinical usefulness of silent magnetic resonance angiography (MRA) in the follow-up of endovascularly-treated aneurysms. However, most previous studies were retrospective or with small sample sizes. The objective of this study was to prospectively evaluate the diagnostic performance of silent MRA in the follow-up of intracranial aneurysms treated by different interventional approaches. Methods Patients with endovascularly-treated intracranial aneurysms and followed by silent MRA and digital subtraction angiography (DSA) were enrolled. The visualization of treated sites on silent MRA was rated on a 5-point scale. The aneurysm occlusion status was evaluated using the Raymond Scale and a simplified two-grade scale. Results A total of 155 patients with 175 treated aneurysms were enrolled. The average score for the visualization of treated sites was 3.92±0.94, and 93.7% (164/175) had a score ≥3. In the subgroup analysis, except for the simple coiling group which had an obviously higher score (4.95±0.21), there was no significant difference among the stent-assisted coiling group (3.51±0.77), flow diversion group (3.74±0.80), and flow diversion with coiling group (3.40±1.17). Regarding aneurysm occlusion status, silent MRA and DSA were discordant for only one aneurysm using the Raymond Scale, and the inter-modality consistency was almost perfect (κ=0.992, 95% CI 0.977 to 1.000). Conclusions Silent MRA showed an excellent diagnostic performance in the follow-up of endovascularly-treated intracranial aneurysms, and may be an ideal option for repeated examinations.

Abstract:To assess the image quality parameters of dual-energy computed tomography angiography (DECTA) 40-, and 60 keV virtual monoenergetic images (VMIs) combined with deep learning-based image reconstruction model (DLM) and iterative reconstructions (IR). CT scans of 28 post EVAR patients were enrolled. The 60 s delayed phase of DECTA was evaluated. Objective [noise, contrast-to-noise ratio (CNR), signal-to-noise ratio (SNR)] and subjective (overall image quality and endoleak conspicuity – 3 blinded readers assessment) image quality analyses were performed. The following reconstructions were evaluated: VMI 40, 60 keV VMI; IR VMI 40, 60 keV; DLM VMI 40, 60 keV. The noise level of the DLM VMI images was approximately 50% lower than that of VMI reconstruction. The highest CNR and SNR values were measured in VMI DLM images. The mean CNR in endoleak in 40 keV was accounted for as 1.83 ± 1.2; 2.07 ± 2.02; 3.6 ± 3.26 in VMI, VMI IR, and VMI DLM, respectively. The DLM algorithm significantly reduced noise and increased lesion conspicuity, resulting in higher objective and subjective image quality compared to other reconstruction techniques. The application of DLM algorithms to low-energy VMIs significantly enhances the diagnostic value of DECTA in evaluating endoleaks. DLM reconstructions surpass traditional VMIs and IR in terms of image quality.
