Abstract:Hormonal changes in pregnant and lactating women significantly affect bone metabolism and overall stress levels, positioning them as a unique group within the orthodontic population. Fluctuations in estrogen, progesterone, prolactin, and other hormones are closely linked to bone remodeling and the periodontal tissue's response to inflammation caused by dental plaque. Hormones such as thyrotropin, leptin, and melatonin also play crucial roles in pregnancy and bone remodeling, with potential implications for orthodontic tooth movement. Additionally, adverse personal behaviors and changes in dietary habits worsen periodontal conditions and complicate periodontal maintenance during orthodontic treatment. Notably, applying orthodontic force during pregnancy and lactation may trigger stress responses in the endocrine system, altering hormone levels. However, these changes do not appear to adversely affect the mother or fetus. This review comprehensively examines the interaction between hormone levels and orthodontic tooth movement in pregnant and lactating women, offering insights to guide clinical practice.

Levels of matrix metalloproteinases in saliva during orthodontic tooth movement

Abstract:Purpose: To determine the levels of matrix metalloproteinases (MMPs) in response to orthodontic tooth movement, in order to clarify the relationship between the change of MMPs and distance of tooth retraction. Materials and methods: In total, 11 patients who needed tooth extraction and 11 patients who didn't before being subjected to fixed orthodontic appliance therapy were assigned into two groups: The extraction group and the nonextraction group. The saliva samples were collected from each patient at four indicated time points, including the first visit time (T1), time before fitting the orthodontic appliance (T2), one hour after (T3) and eight weeks after fitting the appliance (T4). Salivary MMPs concentration was determined using multiplexed bead immunoassay. Alginate impressions were taken at T1 and T4. Results: In the non-extraction group, the concentration of MMP-8 and MMP-9 were significantly increased at T3. In the extraction group, the concentration of MMP-8, MMP-9, and MMP-12 increased during the orthodontic tooth movement process, reaching a peak at T3. In particular, orthodontic tooth movement was positively associated with MMP-8, MMP-9, and MMP-12 levels in saliva at T3 in the extraction group. Conclusions: The result of our present study suggested that orthodontic force could modulate MMPs levels in saliva. MMP-8, MMP-9 and MMP-12 in saliva may represent novel indicators of the degree of orthodontic tooth movement.

Material and Methods: Present study is based on PRISMA guidelines; all articles published in international databases such as PubMed, Scopus, Science Direct, and Embase between 2012 to May 2022 are included. 95% confidence interval (CI) for mean difference with fixed effect modal and inverse-variance were calculated. Data analysis was performed using STATA.V16 software. Results: In the initial review, duplicate studies were eliminated, abstracts of 175 studies were reviewed, two authors reviewed the full text of 21 studies, and finally, eleven studies were selected. The mean of CAL gain was 2.29 mm (MD, 95% CI -2.47 mm, -2.12 mm; p=0.00) (I2=91.81%; p=0.00; high heterogeneity). The mean difference of PPD changes was -1.93 mm (MD, 95% CI -2.07 mm, -1.80 mm; p=0.00) (I2=98.52%; p=0.00; high heterogeneity). Conclusion: Due to the limitations of the study and based on the meta-analysis, it is observed that orthodontic treatment is performed with higher success after reconstructive surgery with periodontal improvement.
Highly variable rate of orthodontic tooth movement measured by a novel 3D method correlates with gingival inflammation

Abstract:Objectives: Individual orthodontic treatment duration is hard to predict. Individual biological factors are amongst factors influencing individual rate of orthodontically induced tooth movement (OTM). The study aim is to determine the rate of OTM by a novel 3D method and investigate parameters that may predict the rate of tooth movement. Materials and methods: In this prospective cohort study, rate of OTM was determined from 90 three-dimensional intra-oral scans in 15 patients (aged 12-15) undergoing orthodontic treatment. For each patient, intra-oral scans were taken every week for up to 6 weeks (T0-T5). The teeth were segmented from the scans and the scans were superimposed on the palatal rugae. The rate of OTM was calculated for each tooth. Other parameters were gingival inflammation, contact-point displacement and the biological markers, matrix metalloproteinases (MMP), MMP-9 and MMP-2 in gingival crevicular fluid (GCF). Results: Our study showed a high variation in the rate of OTM, varying from 0.15 to 1.24 mm/week. Teeth in the anterior segment tended to move more compared with the posterior segment. The contact point displacement and gingival inflammation varied greatly amongst the patients. The MMPs measured did not correlate with tooth movement. However, the gingival inflammation index showed a significant correlation with OTM. Future studies should include other biological markers related to bone-remodeling. Conclusion: This novel and efficient 3D method is suitable for measuring OTM and showed large individual variation in rate of OTM. Clinical relevance: Patients show different rates of OTM. The rate of OTM in an individual patient can provide guidance in timing of follow-up appointments.

Abstract:This study aimed to evaluate the effects of the estrogen depression during orthodontic tooth movement on alveolar bone microarchitecture and periodontal ligament. Female Wistar rats were divided into two groups, one consisting of non‐ovariectomized animals subjected to orthodontic tooth movement, and one comprising ovariectomized animals subjected to orthodontic tooth movement. Micro‐CT assessment of bone volume to total volume (BV/TV), total porosity, trabecular thickness (Tb.Th), trabecular number (Tb.N), and trabecular separation (Tb.Sp) in the alveolar bone of the orthodontically moved tooth was performed. Histomorphometric analyses were made in the periodontal ligament, and immunoexpression of RANK, RANKL, OPG, and TUNEL were quantified. Orthodontic tooth movement in the group of ovariectomized rats was faster than in non‐ovariectomized animals. The alveolar bone area showed lower values of BV/TV and trabecular thickness, and higher bone porosity and trabeculae numbers in the ovariectomized rats. Histological analyses in the ovariectomized group revealed an increase in collagen fibers in the periodontal ligament. The apoptotic cell counts in the periodontal ligament were higher in the group of ovariectomized rats than in the sham‐operated rats. Ovariectomy resulted in an increase in tooth movement and alteration of the alveolar bone microstructure in the first 7 day of orthodontic tooth movement, and in the presence of apoptotic cells in the periodontal ligament.

Correlation of Leptin Concentration in Gingival Crevicular Fluid on the Rate of Orthodontic Tooth Movement - A Prospective Study

Abstract:Background: The evaluation of genetic markers in the gingival crevicular fluid (GCF) and their connection to tooth movement can be helpful. Aim: The objective of this research is to assess the relationship involving leptin levels and orthodontic tooth movement (OTM). Study setting and design: Thirty individuals reporting to the Department of Orthodontics participated in an observational prospective study. Material and method: Thirty orthodontic patients (15 male and 15 female) were included in the current study. The association of leptin concentration (GCF) was measured before and after orthodontic force was applied to move the mesially impacted canine distally (tooth movement distance on dental cast). Statistical analysis: With the help of SPSS software version 21, the data were statistically examined using the " t " test, one-way ANOVA, and Bonferroni correction in Chicago. The average degree of tooth movement was calculated using Pearson's correlation coefficient, and it was connected to the average leptin levels. Results: Average leptin levels in GCF elevated from baseline (T0, 285.315 picograms/micro litre) to 1 h after force application (T1, 384.683 pg/μL), then raised to a maximum at after 24 h (T2, 927.64 pg/μL), and decreased to the least amount after 15 days (T3, 29.26 pg/μL) and another time increase after two months (242.31 pg/μL). The degree of OTM was significantly correlated with the levels of leptin. Conclusion: While OTM was occurring, a biphasic fluctuation in the leptin level was noted. The amount of tooth movement was significantly correlated with the GCF leptin level.

Abstract:Abstract The structural process of bone and periodontal ligament (PDL) remodeling during long-term orthodontic tooth movement (OTM) has not been satisfactorily described yet. Although the mechanism of bone changes in the directly affected alveolar bone has been deeply investigated, detailed knowledge about specific mechanism of PDL remodeling and its interaction with alveolar bone during OTM is missing. This work aims to provide an accurate and user-independent analysis of the alveolar bone and PDL remodeling following a prolonged OTM treatment in mice. Orthodontic forces were applied using a Ni–Ti coil-spring in a split-mouth mice model. After 5 weeks both sides of maxillae were scanned by high-resolution micro-CT. Following a precise tooth movement estimation, an extensive 3D analysis of the alveolar bone adjacent to the first molar were performed to estimate the morphological and compositional parameters. Additionally, changes of PDL were characterized by using a novel 3D model approach. Bone loss and thinning, higher connectivity as well as lower bone mineral density were found in both studied regions. Also, a non-uniformly widened PDL with increased thickness was observed. The extended and novel methodology in this study provides a comprehensive insight about the alveolar bone and PDL remodeling process after a long-duration OTM.

Abstract:Objective This study aimed to investigate the responses of periodontal environment to hormone replacement therapy (HRT) in postmenopausal women with or without periodontitis. Background HRT is a common and effective strategy for controlling menopausal symptoms, while the changes of periodontal environment under it, particularly in postmenopausal women with periodontitis, remain unclear. Methods As a prospective cohort study, a total of 97 postmenopausal women receiving HRT were screened, including 47 with and 50 without periodontitis. Correspondingly, 97 women did not receiving HRT were screened as controls during the same period. The full‐mouth sulcus bleeding index (SBI), bleeding on probing (BOP), probing pocket depth (PPD), and clinical attachment level (CAL) were measured using periodontal probes. The levels of interleukin‐6 (IL‐6) and tumor necrosis factor α (TNF‐α) in the gingival crevicular fluid were measured using enzyme‐linked immunosorbent assay. In addition, cone beam computed tomography was performed to measure the alveolar bone height (ABH) and bone mineral density (BMD). Results In postmenopausal women without periodontitis, no significantly changes on periodontal parameters were observed after HRT. In women with stage II periodontitis, SBI, BOP, IL‐6, and TNF‐α were significant decreased after one year and two years of HRT. Compared to the controls, women with stage II periodontitis who underwent HRT had significantly lower CAL and ABH and higher BMD in the second year. The incidence of at least one site with CAL increase ≥1 mm between baseline and 2 years was significantly lower in the HRT group than in the control group in women with stage II periodontitis. In addition, HRT was significantly associated with a decrease in SBI, BOP, IL‐6, and TNF‐α in the first year and with a decrease in CAL, SBI, BOP, IL‐6, and ABH and an increase in BMD in the second year. Conclusions In postmenopausal women with stage II periodontitis, HRT is associated with the alleviation of inflammation within two years and the remission of alveolar bone loss in the second year. HRT appears to decrease the incidence of CAL increase ≥1 mm within 2 years in women with periodontitis by inhibiting inflammation and alveolar bone loss.

The Role of Bone and Root Resorption on the Biomechanical Behavior of Mandibular Anterior Teeth Subjected to Orthodontic Forces: A Finite Element Approach

Abstract:Aims: This study was conducted to systematically evaluate the biomechanical impact of varying degrees of root and bone resorption resulting from periodontitis and orthodontic tooth movement (OTM) on the mandibular anterior teeth. The objective was to determine whether these distinct resorption patterns exert a specific influence on tooth displacement and strain patterns. Methods: A finite element (FE) model of an idealized anterior mandible from the first premolar in the third to the fourth quadrant was developed without bone or root resorption and a constant periodontal ligament (PDL) thickness of 0.2 mm. Variations included three root resorption levels (0%, 20%, 50%) and three bone resorption types (circular 50%, circular 80%, vestibular 80%). Models ranged from 200,000 to 440,000 elements and 55,000 to 130,000 nodes. Orthodontic forces, namely root torque (5 Nmm), intrusion (0.2 N), and distalization (0.5 N) were applied for subsequent crown displacement and PDL strain analysis. Results: A total of 180 simulations were performed. Simulations showed that displacement was similar across different bone resorption conditions, irrespective of modeled root resorptions. Circumferential bone resorption increased tooth displacement, regardless of root resorption status. Vestibular bone resorption exhibited less increase in tooth displacement. However, when accompanied by root resorption, the combination exacerbated tooth displacement. Strains in the PDL clearly increased with a circumferential bone resorption of 80%. Conclusions: This study highlights the critical role of bone resorption in tooth displacement during OTM, particularly the challenges associated with circumferential resorption. Clinicians must consider both bone and root resorption for personalized medicine treatment of patients with severe periodontitis, in favor of low-force application strategies to optimize outcomes and minimize complications linked to excessive tooth displacement.

Abstract:Background: During orthodontic tooth movement (OTM), applied orthodontic forces cause an extensive remodeling of the extracellular matrix (ECM) in the periodontal ligament (PDL). This is mainly orchestrated by different types of matrix metalloproteinases (MMPs) and their tissue inhibitors of matrix metalloproteinases (TIMPs), which are both secreted by periodontal ligament (PDL) fibroblasts. Multiple in vitro and in vivo studies already investigated the influence of applied orthodontic forces on the expression of MMPs and TIMPs. The aim of this systematic review was to explore the expression levels of MMPs and TIMPs during OTM and the influence of specific orthodontic force-related parameters. Methods: Electronic article search was performed on PubMed and Web of Science until 31 January 2021. Screenings of titles, abstracts and full texts were performed according to PRISMA, whereas eligibility criteria were defined for in vitro and in vivo studies, respectively, according to the PICO schema. Risk of bias assessment for in vitro studies was verified by specific methodological and reporting criteria. For in vivo studies, risk of bias assessment was adapted from the Joanna Briggs Institute Critical Appraisal Checklist for analytical cross-sectional study. Results: Electronic article search identified 3266 records, from which 28 in vitro and 12 in vivo studies were included. The studies showed that orthodontic forces mainly caused increased MMPs and TIMPs expression levels, whereas the exact effect may depend on various intervention and sample parameters and subject characteristics. Conclusion: This systematic review revealed that orthodontic forces induce a significant effect on MMPs and TIMPs in the PDL. This connection may contribute to the controlled depletion and formation of the PDLs’ ECM at the compression and tension site, respectively, and finally to the highly regulated OTM.

Abstract:Abstract : We consider a class of dynamic resource allocation problems a specific example of which is the Weapon-Target Assignment problem. This problem is concerned with the optimal assignment of resources in a military engagement. These problems are, in general, NP-Complete, so our aim is to provide insight into the problem and its solution. We will provide analytical results for simple cases of the problem. We will also provide sub-optimal algorithms, together with bounds on the objective function, under certain assumptions. The battle scenario of the military engagement being modeled is as follows. The offense launches a number of weapons ( the targets) which are aimed at valuable assets of the defense. The defense has a number of defensive weapons each of which can engage at most one target. The outcome of such an engagement is stochastic. In the static scenario all weapons are fired simultaneously. In the dynamic scenario some weapons are assigned and fired and the outcomes of these engagements are observed before further assignments are made.
The role of Aktand RAFTK in beta1 integrin mediated survival of precursor B-acute lymphoblastic leukemia cells.

Abstract:In this study, we report that the related adhesion focal tyrosine kinase RAFTK, is an upstream kinase in beta1 integrin mediated activation of Akt. Stimulation through beta1 integrins by fibronectin reversed apoptosis induced by adriamycin. Inhibitors of phosphatidylinositol 3-kinase (PI3 kinase)/Akt (LY 294002), tyrosine kinases (Herbimycin-A) and the cytotoxic agent adriamycin induced apoptosis of REH cells. beta1 integrin ligation induced activation of Akt, and tyrosine phosphorylation of RAFTK and FAK, but not SYK in REH cells. This suggested that RAFTK and FAK activation might be linked to Akt activation. Evidence that RAFTK is a modulator of Akt came from phorbol myristic acetate (PMA) stimulation. RAFTK and Akt were activated but FAK was not. Using fibroblasts from FAK -/- mice, which express high levels of RAFTK, fibronectin plating enhanced Akt activation. Pretreatment of REH cells with a P13 kinase/Akt inhibitor LY 294002 did not inhibit RAFTK tyrosine phosphorylation showing that RAFTK is upstream of P13k/Akt. Further evidence for a link between RAFTK tyrosine phosphorylation and Akt activation was the observation that the p85 subunit of P13 kinase associated with RAFTK following integrin ligation in REH cells. These results suggest that RAFTK plays an anti-apoptotic role through the activation of Akt.

Abstract:INTRODUCTION:The purpose of this study was to evaluate the dynamic changes in the periodontal microstructure and the molar displacement pattern during orthodontic tooth movement in ovariectomized rats.METHODS:Twenty ovariectomized rats received either 100 or 30 g of orthodontic force to induce mesial movement of the maxillary left first molars over 14 days. Ten healthy rats underwent sham operations as controls. Periodontal ligament thickness, alveolar bone microstructural properties, and displacement of the molar were measured with 6 in-vivo microcomputed tomography scans for each sample.RESULTS:The ovariectomized rats that received 100 g of orthodontic force had obvious changes in periodontal ligament thickness at day 1 and poor periodontal ligament thickness recovery from days 5 through 14. The bone volume fraction increased and the trabecular separation decreased significantly in this group at day 3, and obvious bone loss was observed at day 14. Molar linear and angular movements were also higher in this group than in the other 2 groups.CONCLUSIONS:Relatively heavier force applications in ovariectomized rats resulted in poor periodontal ligament thickness recovery and local alveolar bone overcompression, and consequently induced undermining resorption and obvious alveolar bone loss; these led to high rates of tooth movement and molar inclination.
The Relationship Between Digit Independence and Digital Sesamoids in Humans and a Proposal of a New Digital Sesamoid Evolutionary Hypothesis

Abstract:Digital sesamoids are found in the metapodial‐phalangeal joints of most mammals and quadrupedal tetrapods, yet their functional significance is still unclear. During primate evolution, a slight decline in their frequency has been associated with brachiation in gibbons, followed by a quasi‐complete absence in orangutans then a slight resurgence occurred in gorillas and chimpanzees. Simultaneously, forearm muscles showed a progressive division in hominoid evolution towards a more “individualistic” musculature yielding more mobility and independence to some fingers. In humans, sesamoids are consistently observed in thumbs and big toes and frequently in other hypermobile digits such as the index and little fingers. Using a simple mathematical equation, this paper attempted to quantify a presumed association between hypermobile fingers and sesamoid frequency and distribution in humans. To this, an anatomic definition of digital independence has been formulated which includes three variables; (1) number and (2) frequency of independent flexor/extensor forearm muscles destined to a single finger, (3) and number of free/absent webspace. Results of previous meta‐analyses and means of big sample studies were used to evaluate the frequency of such muscles. The expected values obtained via this model were found to be very close to the observed (published) values of the ossified sesamoids in human hands, and that in terms of frequency and distribution. The findings in humans showed a quasi‐linear association between the degree of mobility and sesamoid frequency. The more the number of independent muscles destined to a finger, the more its metacarpo‐phalangeal joint is likely to bear sesamoids. Based on our results and on a new analysis of primates' forearm/hand muscles and sesamoid evolution, a new hypothesis is proposed to answer two questions; the evolution of digital sesamoid frequency in primates and its sesamoid distribution in human digits. It claims that the number/frequency of independent forearm/hand muscles, and particularly the independent extensors, is likely to be a major factor in sesamoid reversion in hominoids. The argument is based on the link between the metatarsal break induced by the digitigrade locomotion observed in quadrupedal mammals and tetrapods and the amount of extra‐extension of the metacarpo‐phalangeal joints conferred by the individualization of some forearm extensors. The same rationale yielded similar conclusions when applied to both the hand and foot. The manipulative function of the hand and the plantigrade locomotion of the foot required such extra‐extension in specific digits and consequently, a higher frequency of digital sesamoids was associated with these two different functions. The new evolutionary analysis suggests evolutionary pathways for both, the sesamoids of the hands and feet, and speculates that muscle individualization would have induced a very slow re‐acquisition of digital sesamoids when compared to their rapid decline after brachiation. It is the first hypothesis offering a plausible explication on the recent evolution of digital sesamoids; specifically, the progression of sesamoid frequency in African apes and its resurgence, prevalence and distribution in humans. Influence of Menstrual Cycle and Oral Contraceptive Phases on Strength Performance, Neuromuscular Fatigue and Perceived Exertion

Abstract:The aim of the study was to assess differences in strength performance, neuromuscular fatigue and perceived exertion across phases of the menstrual cycle (MC, early follicular [eFP], late follicular [lFP] and mid-luteal phase [mLP]) and oral contraceptives (OC, active pill phase [aPP] and non-active pill phase [nPP]). Secondly, in naturally menstruating women, the influence of fluctuating serum 17β-estradiol nd progesterone concentrations on these parameters were analysed. Thirty-four women (21 with a natural MC and 13 using OCs) completed three or two experimental sessions, respectively. Mean mean propulsive velocity (MPV mean ) and total number of repetitions (REP total ) were assessed during a power (3x8 at 60%1RM) and hypertrophy squat loading (3 sets to failure at 70%1RM), respectively. Change in bench press and squat MPV at 60%1RM in response to the loadings were used as surrogates for non-local and local fatigue, respectively. Total blood lactate accumulation (BLA A ) and markers of perceived exertion were assessed in each session. No significant differences between any of the MC or OC phases were observed for MPV mean , REP total , non-local and local fatigue and markers of perceived exertion (all p>0.050). A higher intraindividual 17β-estradiol concentration was significantly associated with a lower MPV mean (p=0.019). BLA A was significantly higher in lFP compared to mLP (p=0.019) and negatively associated with the intraindividual progesterone concentration (p=0.005). While 17β-estradiol may negatively influence the MPV, it appears that fluctuations of both sex hormones across the MC and OC phases are not prominent enough to induce significant nor practically relevant changes in the assessed parameters.

Abstract:Abstract Background Based on low-dose radiation Cone-bean computed tomography (CBCT) images, This study aims to establish a space coordinate system, which offers more precise and comparable evaluation on changes of maxillary third molars influenced by orthodontic treatment with premolar extraction in adults. The system suggests promising application prospect in future studies related to CBCT superimposition and evaluation for its feasibility and efficiency. Methods Forty-nine maxillary third molars from 27 patients (mean age, 20.78 years) were included. CBCT images were obtained before and after orthodontic treatment with premolars extracted (mean treatment duration, 31.47 months). The changes in the position, angulation, and rotation of the third molars were evaluated with a space coordinate system using four landmarks: anterior nasal spine (ANS), posterior nasal spine (PNS), left and right orbitales. Results After orthodontic treatment, the third molars moved forward (adjusted mean, 1.44 mm) ( p < 0.001) and downward (adjusted mean, 2.87 mm) ( p < 0.001) accompanied by outward rotation of the crowns (adjusted mean, 5.38°) ( p = 0.001), while changes in angulation were insignificant. Conclusions This was the first study to systematically investigate the spatial position change of maxillary third molars in adult patients who received orthodontic treatment with premolar extraction. During the process, maxillary third molars moved downward and forward accompanied by outward rotation of the crowns. Orthodontists should take tooth movement potential into consideration when making extraction plans.

Bite Force, Occlusal Contact and Pain in Orthodontic Patients during Fixed-Appliance Treatment

Abstract:Previously, bite force, occlusal contact and pain were investigated in orthodontic patients with moderate-to-severe malocclusion, but not in patients with minor malocclusion. The purpose of this study was to investigate changes in bite force, teeth in occlusal contact and pain in orthodontic patients with minor crowding before orthodontic treatment (T0), after bonding (T1), during treatment (T2), post-treatment (T3) and during retention (T4). In total, 27 patients (21 females, 6 males, median age 15.3 years) with neutral occlusion and normal craniofacial morphology were treated with non-extractions and fixed appliances. Differences in the registered data were analysed by a mixed linear model with repeated measures. Bite force and teeth in occlusal contact significantly decreased between T0 and T1 (p < 0.0001, respectively) and between T0 and T2 (p < 0.01, respectively). Bite force and teeth in occlusal contact significantly increased between T1 and T4 (p < 0.05, p < 0.0001, p < 0.001, respectively) and between T2 and T4 (p < 0.05, p < 0.0001, p < 0.01, respectively). No significant difference in pain was found. The results indicate that bite force and teeth in occlusal contact significantly decreased during treatment and reached baseline level at retention. The findings may prove valuable for informing orthodontic patients with minor malocclusion.
