Abstract:Summary The irises of seven chickens with lesions of spontaneous ocular leukosis were examined by light and electron microscopy. Histologically, the abnormal contents of the irises included necrotic striated musculature, mononuclear cells, and granulocytes. Observed by electron microscopy, viral particles were embedded in the basement membrane of striated muscle cells, in membrane-bound vesicles in muscle cells, in proximity to pigmented epithelial cells, and in the lumen and basement membrane of endothelial cells of capillaries of the iris. Necrosis of striated muscle cells occurred where large masses of viral particles were observed extracellularly and in vesicles in the cell. Particles budded from the cell membrane and the membrane surrounding vesicles. It appeared that depigmentation of the irises of chickens with ocular leukosis was correlated with necrotizing changes of the striated muscle cells in the irises.
Early Inspiratory Effort Assessment by Esophageal Manometry Predicts Noninvasive Ventilation Outcome in De Novo Respiratory Failure. A Pilot Study

Abstract:Rationale: The role of inspiratory effort still has to be determined as a potential predictor of noninvasive mechanical ventilation (NIV) failure in acute hypoxic de novo respiratory failure. Objectives: To explore the hypothesis that inspiratory effort might be a major determinant of NIV failure in these patients. Methods: Thirty consecutive patients with acute hypoxic de novo respiratory failure admitted to a single center and candidates for a 24-hour NIV trial were enrolled. Clinical features, tidal change in esophageal pressure (ΔPes), tidal change in dynamic transpulmonary pressure (ΔPl), expiratory Vt, and respiratory rate were recorded on admission and 2–4 to 12–24 hours after NIV start and were tested for correlation with outcomes. Measurements and Main Results: ΔPes and ΔPes/ΔPl ratio were significantly lower 2 hours after NIV start in patients who successfully completed the NIV trial (n = 18) compared with those who needed endotracheal intubation (n = 12) (median [interquartile range], 11 [8–15] cm H2O vs. 31.5 [30–36] cm H2O; P < 0.0001), whereas other variables differed later. ΔPes was not related to other predictors of NIV failure at baseline. NIV-induced reduction in ΔPes of 10 cm H2O or more after 2 hours of treatment was strongly associated with avoidance of intubation and represented the most accurate predictor of treatment success (odds ratio, 15; 95% confidence interval, 2.8–110; P = 0.001 and area under the curve, 0.97; 95% confidence interval, 0.91–1; P < 0.0001). Conclusions: The magnitude of inspiratory effort relief as assessed by ΔPes variation within the first 2 hours of NIV was an early and accurate predictor of NIV outcome at 24 hours. Performance of non-invasive respiratory function indices in predicting clinical outcomes in patients hospitalized for COVID-19 pneumonia in medical and sub-intensive wards: a retrospective cohort study

Abstract:Coronavirus disease 2019 (COVID-19) is a newly recognized infectious disease which can lead to acute respiratory distress syndrome requiring ventilatory support and intensive care unit admission. The aim of our study is to evaluate the performance of two non-invasive respiratory function indices (the ROX index and the SatO2/FiO2 ratio), as compared to the traditional PaO2/FiO2 ratio, in predicting a clinically relevant composite outcome (death or intubation) in hospitalized patients for COVID-19 pneumonia. Four hospital centers in Northern Italy conducted an observational retrospective cohort study during the first wave of COVID-19 pandemic. Four hundred and fifty-six patients with COVID-19 pneumonia admitted to medical or sub-intensive wards were enrolled. Clinical, laboratory, and respiratory parameters, for the calculation of different indices, were measured at hospital admission. In medical wards (Verona and Padua) the PaO2/FiO2 ratio, ROX index and SatO2/FiO2 ratio were able to predict intubation or death with good accuracy (AUROC for the PaO2/FiO2 ratio, ROX index and SatO2/FiO2 ratio of 75%, 75% and 74%, respectively). Regarding sub-intensive wards (Milan and Mantua), none of the three respiratory function indices was significantly associated with the composite outcome. In patients admitted to medical wards for COVID-19 pneumonia, the ROX index and the SatO2/FiO2 ratio demonstrated not only good performance in predicting intubation or death, but their accuracy was comparable to that of the PaO2/FiO2 ratio. In this setting, where repeated arterial blood gas tests are not always feasible, they could be considered a reliable alternative to the invasive PaO2/FiO2 ratio.

Abstract:Abstract Background Monitoring respiratory effort in ventilated patients is important to balance lung and diaphragm protection. Esophageal manometry remains the gold standard for monitoring respiratory effort but is invasive and requires expertise for its measurement and interpretation. Airway pressures during occlusion maneuvers may provide an alternative, although pediatric data are limited. We sought to determine the correlation between change in esophageal pressure during tidal breathing (∆Pes) and airway pressure measured during three airway occlusion maneuvers: (1) expiratory occlusion pressure (Pocc), (2) airway occlusion pressure (P0.1), and (3) respiratory muscle pressure index (PMI) in children. We also sought to explore pediatric threshold values for these pressures to detect excessive or insufficient respiratory effort. Methods Secondary analysis of physiologic data from children between 1 month and 18 years of age with acute respiratory distress syndrome enrolled in an ongoing randomized clinical trial testing a lung and diaphragm protective ventilation strategy (REDvent, R01HL124666). ∆Pes, Pocc, P0.1, and PMI were measured. Repeated measure correlations were used to investigate correlation coefficients between ∆Pes and the three measures, and linear regression equations were generated to identify potential therapeutic thresholds. Results There were 653 inspiratory and 713 expiratory holds from 97 patients. Pocc had the strongest correlation with ∆Pes ( r = 0.68), followed by PMI ( r = 0.60) and P0.1 ( r = 0.42). ∆Pes could be reliably estimated using the regression equation ∆Pes = 0.66 $$\times$$ × Pocc ( R 2 = 0.82), with Pocc cut-points having high specificity and moderate sensitivity to detect respective ∆Pes thresholds for high and low respiratory effort. There were minimal differences in the relationship between Pocc and ∆Pes based on age (infant, child, adolescent) or mode of ventilation (SIMV versus Pressure Support), although these differences were more apparent with P0.1 and PMI. Conclusions Airway occlusion maneuvers may be appropriate alternatives to esophageal pressure measurement to estimate the inspiratory effort in children, and Pocc represents the most promising target. Predictive Factors of Oxygen Therapy Failure in Patients with COVID-19 in the Emergency Department

Abstract:Background: Most patients with coronavirus disease 2019 (COVID-19) pneumonia require oxygen therapy, including standard oxygen therapy and a high-flow nasal cannula (HFNC), in the Emergency Department (ED), and some patients develop respiratory failure. In the COVID-19 pandemic, the intensive care unit (ICU) was overburdening. Therefore, prioritizing patients who require intensive care is important. This study aimed to find predictors and develop a model to predict patients at risk of requiring an invasive mechanical ventilator (IMV) in the ED. Methods: We performed a retrospective, single-center, observational study. Patients aged ≥18 years who were diagnosed with COVID-19 and required oxygen therapy in the ED were enrolled. Cox regression and Harrell's C-statistic were used to identifying predictors of requiring IMV. The predictive model was developed by calculated coefficients and the ventilator-free survival probability. The predictive model was internally validated using the bootstrapping method. Results: We enrolled 333 patients, and 97 (29.1%) had required IMV. Most 66 (68.0%) failure cases were initial oxygen therapy with HFNC. Respiratory rate-oxygenation (ROX) index, interleukin-6 (IL-6) concentrations ≥20 pg/mL, the SOFA (Sequential Organ Failure Assessment) score without a respiratory score, and the patient's age were independent risk factors of requiring IMV. These factors were used to develop the predictive model. ROX index and the predictive model at 2 hours showed a good performance to predict oxygen therapy failure; the c-statistic was 0.814 (95% confidence level [CI] 0.767-0.861) and 0.901 (95% CI 0.873-0.928), respectively. ROX index ≤5.1 and the predictive model score ≥8 indicated a high probability of requiring IMV. Conclusion: The COVID-19 pandemic was limited resources, ROX index, IL-6 ≥20 pg/mL, the SOFA score without a respiratory score, and the patient's age can be used to predict oxygen therapy failure. Moreover, the predictive model is good at discriminating patients at risk of requiring IMV and close monitoring.

Abstract:BACKGROUND: We hypothesize that oxygen consumption (<(V)over dot>. O-2) estimation in patients with respiratory symptoms is inaccurate and can be improved by considering arterial blood gases or spirometric variables. METHODS: For this retrospective study, we included consecutive subjects who underwent cardiopulmonary exercise testing. Resting <(V) over dot>. O-2 was determined using breath-bybreath testing methodology. Using a training cohort (n = 336), we developed 3 models to predict <(V) over dot>. O-2. In a validation group (n = 114), we compared our models with 7 available formulae. RESULTS: Our first model (<(V) over dot>. O-2 = -184.99 + 189.64 x body surface area [BSA, m(2)] + 1.49 x heart rate [beats/min] + 51.51 x F-IO2 [21% = 0; 30% = 1] + 30.62 x gender [male = 1; female = 0]) showed an R-2 of 0.5. Our second model (<(V) over dot>. O-2 = -208.06 + 188.67 x BSA = 1.38 x heart rate + 35.6 x gender + 2.06 x breathing frequency [breaths/min]) showed an R-2 of 0.49. The best R-2 (0.68) was obtained with our last model, which included minute ventilation (<(V) over dot>. O-2 = -142.92 + 0.52 x heart rate + 126.84 x BSA + 14.68 x minute ventilation [L]). In the validation cohort, these 3 models performed better than other available equations, but had wide limits of agreement, particularly in older individuals with shorter stature, higher heart rate, and lower maximum voluntary ventilation. CONCLUSIONS: We developed more accurate formulae to predict resting <(V) over dot>. O-2 in subjects with respiratory symptoms; however, equations had wide limits of agreement, particularly in certain groups of subjects. Arterial blood gases and spirometric variables did not significantly improve the predictive equations.

Abstract:The COVID-19 pandemic has had a major impact on health care services, leading to a breakdown in public and private health systems worldwide. A major challenge was the scarcity of mechanical ventilators, which resulted in the use of anaesthesia devices for this purpose. However, they are quite different from mechanical ventilators used in Intensive Care Units and some adaptations, such as the use of high flow to reduce CO2 rebreathing, were necessary to ensure patient safety. The objective of this study was to present a mathematical formula and develop a tool that can be used to adjust the flow of oxygen and air in flow metres of anaesthesia devices that do not have oxygen analysers or these analysers are not operational. A literature review was conducted using the main health databases and libraries as research sources: PubMed, Virtual Health Library (VHL), SciELO, and Cochrane. The review included studies published in English, Spanish, and Portuguese. Animal studies were excluded. A total of 11 references were included to support this article.

Abstract:Breathing effort is important to quantify to understand mechanisms underlying central and obstructive sleep apnea, respiratory-related arousals, and the timing and effectiveness of invasive or non-invasive mechanically assisted ventilation. Current quantitative methods to evaluate breathing effort rely on inspiratory esophageal or epiglottic pressure swings or changes in diaphragm electromyographic (EMG) activity, where units are problematic to interpret and compare between individuals and to measured ventilation. This paper derives a novel method to quantify breathing effort in units directly comparable to measured ventilation by applying respiratory mechanics first principles to convert continuous transpulmonary pressure measurements into "attempted" airflow expected to have arisen without upper airway obstruction. The method was evaluated using data from eleven subjects undergoing overnight polysomnography, including 6 obese patients with severe obstructive sleep apnea (OSA), including one who also had frequent central events, and 5 healthy-weight controls. Classic respiratory mechanics showed excellent fits of airflow and volume to transpulmonary pressures during wake periods of stable unobstructed breathing (mean ± SD r2 = 0.94 ± 0.03), with significantly higher respiratory system resistance in patients compared to healthy controls (11.2 ± 3.3 vs 7.1 ± 1.9 cmH 2 O·l -1 ·sec, P=0.032). Subsequent estimates of attempted airflow from transpulmonary pressure changes clearly highlighted periods of acute and prolonged upper airway obstruction, including within the first few breaths following sleep onset in patients. This novel technique provides unique quantitative insights into the complex and dynamically changing inter-relationships between breathing effort and achieved airflow during periods of obstructed breathing in sleep.

Abstract:Background: Noninvasive monitoring of maximal inspiratory and expiratory flows (MIF and MEF, respectively) by electrical impedance tomography (EIT) might enable early recognition of changes in the mechanical properties of the respiratory system due to new conditions or in response to treatments. We aimed to validate EIT-based measures of MIF and MEF against spirometry in intubated hypoxemic patients during controlled ventilation and spontaneous breathing. Moreover, regional distribution of maximal airflows might interact with lung pathology and increase the risk of additional ventilation injury. Thus, we also aimed to describe the effects of mechanical ventilation settings on regional MIF and MEF. Methods: We performed a new analysis of data from two prospective, randomized, crossover studies. We included intubated patients admitted to the intensive care unit with acute hypoxemic respiratory failure (AHRF) and acute respiratory distress syndrome (ARDS) undergoing pressure support ventilation (PSV, n = 10) and volume-controlled ventilation (VCV, n = 20). We measured MIF and MEF by spirometry and EIT during six different combinations of ventilation settings: higher vs. lower support during PSV and higher vs. lower positive end-expiratory pressure (PEEP) during both PSV and VCV. Regional airflows were assessed by EIT in dependent and non-dependent lung regions, too. Results: MIF and MEF measured by EIT were tightly correlated with those measured by spirometry during all conditions (range of R2 0.629-0.776 and R2 0.606-0.772, respectively, p < 0.05 for all), with clinically acceptable limits of agreement. Higher PEEP significantly improved homogeneity in the regional distribution of MIF and MEF during volume-controlled ventilation, by increasing airflows in the dependent lung regions and lowering them in the non-dependent ones. Conclusions: EIT provides accurate noninvasive monitoring of MIF and MEF. The present study also generates the hypothesis that EIT could guide PSV and PEEP settings aimed to increase homogeneity of distending and deflating regional airflows.
Measuring Breathing Pattern in Patients With Chronic Obstructive Pulmonary Disease by Electrical Impedance Tomography

Abstract:Background and ObjectiveThe measurement of breathing pattern in patients with chronic obstructive pulmonary disease (COPD) by electrical impedance tomography (EIT) requires the use of a mathematical calibration model incorporating not only anthropometric characteristics (previously evaluated in healthy individuals) but probably functional alterations associated with COPD as well. The aim of this study was to analyze the association between EIT measurements and spirometry parameters, static lung volumes, and carbon monoxide diffusing capacity (DLCO) in a group of male patients to develop a calibration equation for converting EIT signals into volume signals.Materials and MethodsWe measured forced vital capacity (FVC), forced expiratory volume in 1 second (FEV1), FEV1/FVC, residual volume, total lung capacity, DLCO, carbon monoxide transfer coefficient (KCO) and standard anthropometric parameters in 28 patients with a FEV1/FVC ratio of <70%. We then compared tidal volume measurements from a previously validated EIT unit and a standard pneumotachometer.ResultsThe mean (SD) lung function results were FVC, 72 (16%); FEV1, 43% (14%); FEV1/FVC, 42% (9%); residual volume, 161% (44%); total lung capacity, 112% (17%); DLCO, 58% (17%); and KCO, 75% (25%). Mean (SD) tidal volumes measured by the pneumotachometer and the EIT unit were 0.697 (0.181) L and 0.515 (0.223) L, respectively (P<.001). Significant associations were found between EIT measurements and CO transfer parameters. The mathematical model developed to adjust for the differences between the 2 measurements (R2=0.568; P<.001) was compensation factor=1.81 – 0.82 × height (m) – 0.004×KCO (%).ConclusionsThe measurement of breathing pattern by EIT in patients with COPD requires the use of a previously calculated calibration equation that incorporates not only individual anthropometric characteristics but gas exchange parameters as well.

Abstract:There is no universally accepted method for positive end expiratory pressure (PEEP) titration approach for patients on spontaneous mechanical ventilation (SMV). Electrical impedance tomography (EIT) guided PEEP-titration has shown promising results in controlled mechanical ventilation (CMV), current implemented algorithm for PEEP titration (based on regional compliance measurements) is not applicable in SMV. Regional peak flow (RPF, defined as the highest inspiratory flow rate based on EIT at a certain PEEP level) is a new method for quantifying regional lung mechanics designed for SMV. The objective is to study whether RPF by EIT is a feasible method for PEEP titration during SMV. Single EIT measurements were performed in COVID-19 ARDS patients on SMV. Clinical (i.e., tidal volume, airway occlusion pressure, end-tidal CO 2 ) and mechanical (cyclic alveolar recruitment, recruitment, cumulative overdistension (OD), cumulative collapse (CL), pendelluft, and PEEP) outcomes were determined by EIT at several pre-defined PEEP thresholds (1–10% CL and the intersection of the OD and CL curves) and outcomes at all thresholds were compared to the outcomes at baseline PEEP. In total, 25 patients were included. No significant and clinically relevant differences were found between thresholds for tidal volume, end-tidal CO 2 , and P0.1 compared to baseline PEEP; cyclic alveolar recruitment rates changed by -3.9% to -37.9% across thresholds; recruitment rates ranged from − 49.4% to + 79.2%; cumulative overdistension changed from − 75.9% to + 373.4% across thresholds; cumulative collapse changed from 0% to -94.3%; PEEP levels from 10 up to 14 cmH 2 O were observed across thresholds compared to baseline PEEP of 10 cmH 2 O. A threshold of approximately 5% cumulative collapse yields the optimum compromise between all clinical and mechanical outcomes. EIT-guided PEEP titration by the RPF approach is feasible and is linked to improved overall lung mechanics) during SMV using a threshold of approximately 5% CL. However, the long-term clinical safety and effect of this approach remain to be determined.

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Background</h3>Bedside measures of patient effort are essential to properly titrate the level of pressure support ventilation. We investigated whether the tidal swing in oesophageal (ΔPes) and transdiaphragmatic pressure (ΔPdi), and ultrasonographic changes in diaphragm (TFdi) and parasternal intercostal (TFic) thickening are reliable estimates of respiratory effort. The effect of diaphragm dysfunction was also considered.<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3>Twenty-one critically ill patients were enrolled: age 73 (14) yr, BMI 27 (7) kg m−2, and Pao2/Fio2 33.3 (9.2) kPa. A three-level pressure support trial was performed: baseline, 25% (PS-medium), and 50% reduction (PS-low). We recorded the oesophageal and transdiaphragmatic pressure–time products (PTPs), work of breathing (WOB), and diaphragm and intercostal ultrasonography. Diaphragm dysfunction was defined by the Gilbert index.<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3>Pressure support was 9.0 (1.6) cm H2O at baseline, 6.7 (1.3) (PS-medium), and 4.4 (1.0) (PS-low). ΔPes was significantly associated with the oesophageal PTP (R2=0.868; P<0.001) and the WOB (R2=0.683; P<0.001). ΔPdi was significantly associated with the transdiaphragmatic PTP (R2=0.820; P<0.001). TFdi was only weakly correlated with the oesophageal PTP (R2=0.326; P<0.001), and the correlation improved after excluding patients with diaphragm dysfunction (R2=0.887; P<0.001). TFdi was higher and TFic lower in patients without diaphragm dysfunction: 33.6 (18.2)% vs 13.2 (9.2)% and 2.1 (1.7)% vs 12.7 (9.1)%; P<0.0001.<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusions</h3>ΔPes and ΔPdi are adequate estimates of inspiratory effort. Diaphragm ultrasonography is a reliable indicator of inspiratory effort in the absence of diaphragm dysfunction. Abstract:Background This study aimed to evaluate the effectiveness of inhalation therapy in patients with chronic airway diseases via the use of a new multiparametric inhalation assessment device. Methods A multiparametric inhalation evaluation device (PF810, UBREATH, Zhejiang, China) that could simulate common inhalation devices with 6 different levels (0-V) of resistance was used in this study. The device was considered suitable if the three parameters of peak inspiratory flow rate (PIFR), effective inspiratory time (EIT), and breath-hold time (BHT) after inspiration met the minimum requirements. Results A total of 4,559 tests were performed. The qualification rates of 0-V resistance gear from low to high were 3.38% (I), 8.42% (0), 15.31% (II), 16.71% (III), 20.27% (IV), and 46.91% (V). The COPD patients in the 3 experimental groups had the lowest percentages of isolates classified as resistant 0, III, and V, which were 5.65%, 11.93%, and 40.43%, respectively. The lowest percentage was 39.67% (V) for insufficient EIT and 18.40% (V) for BHT less than 5 s after inspiration. The results of 149 subjects who had used the inhalation device showed that the VIE and EIT at 0 levels were significantly greater than those before training (Z= -5.651, -5.646, P < 0.001). The VIE and EIT at I-III and V significantly increased after training (all P < 0.05). Conclusions Patients using portable inhaler devices do not always inhale with adequate flow patterns. The multiparametric inhalation assessment device may be useful in outpatient settings.

Abstract:Background: Case finding and diagnosis of COPD still remain challenging in primary healthcare settings. Aim: Develop a stand-alone solution based on respiratory oscillometry that is suitable for use in primary care and helps in case finding by providing disease specific information. Methods: 1) We developed a prototype using a 3D printed pneumotach and airflow channel including a digital differential pressure sensor. Signal generation, hardware control, signal processing, data collection and GUI were programmed using a single-board computer (Raspberry Pi 4) and a touchscreen display. 2) Technical comparison was performed with a commercial respiratory oscillometry device (Resmon Pro v3.0, Restech, Milan, Italy) using resistant test loads (0 to 14 hPa.s.L-1) to simulate differing amounts of airway restriction. 3) Measurements were performed in adult volunteers (± COPD) recruited at the outpatient area of the hospital. Anthropomorphic and clinical data were collected incl spirometry parameters. Linear regression and Bland-Altman analysis were used to evaluate bias between the devices. Results: Measurements were obtained from healthy controls (N=8) and individuals with COPD of different severities (N=11). Resistant testloads showed good correlation at 5 and 10Hz (slope=1, intercept=0.1/0.2). A bias was detected in clinical testing at 5Hz and 19Hz (slopes=0.75-0.9). Conclusion: Proof of concept study of BREAZ prototype shows non-inferiority over a commercial oscillometry device using mechanical resistant testloads up to 14 hPa.s.L-1 as well as in detecting resistance and reactance changes in patients with COPD.

Abstract:This report summarizes current physiological and technical knowledge on esophageal pressure (Pes) measurements in patients receiving mechanical ventilation. The respiratory changes in Pes are representative of changes in pleural pressure. The difference between airway pressure (Paw) and Pes is a valid estimate of transpulmonary pressure. Pes helps determine what fraction of Paw is applied to overcome lung and chest wall elastance. Pes is usually measured via a catheter with an air-filled thin-walled latex balloon inserted nasally or orally. To validate Pes measurement, a dynamic occlusion test measures the ratio of change in Pes to change in Paw during inspiratory efforts against a closed airway. A ratio close to unity indicates that the system provides a valid measurement. Provided transpulmonary pressure is the lung-distending pressure, and that chest wall elastance may vary among individuals, a physiologically based ventilator strategy should take the transpulmonary pressure into account. For monitoring purposes, clinicians rely mostly on Paw and flow waveforms. However, these measurements may mask profound patient-ventilator asynchrony and do not allow respiratory muscle effort assessment. Pes also permits the measurement of transmural vascular pressures during both passive and active breathing. Pes measurements have enhanced our understanding of the pathophysiology of acute lung injury, patient-ventilator interaction, and weaning failure. The use of Pes for positive end-expiratory pressure titration may help improve oxygenation and compliance. Pes measurements make it feasible to individualize the level of muscle effort during mechanical ventilation and weaning. The time is now right to apply the knowledge obtained with Pes to improve the management of critically ill and ventilator-dependent patients.

Abstract:Objective: This paper proposes an initial proof-of-concept validation for a method that estimates the respiratory effort during pressure support ventilation, based solely on the signals provided by a standard commercial ventilator. The study is carried out on a test bench with an Active Servo Lung 5000 mechanical simulator. Methods: The asymptotic behavior of the respiratory mechanics during expiration is exploited to analyze its identifiability (the fact that two sets of respiratory mechanics parameters necessarily generate distinct signals). Furthermore, we propose a method of estimation of the patient's respiratory muscle pressure, based on the degree of smoothness of a signal constructed from the airway pressure and flow measurements. Results: The proposed method was validated on a commercial ventilator, under a wide range of respiratory mechanics parameters and respiratory efforts. Conclusion: Our approach provides a non-invasive estimation of the respiratory effort, which allows for accurately detecting over- and under-assistance. Significance: Compared to existing muscle pressure estimation methods, our approach clarifies the conditions under which the respiratory effort of a patient can be estimated and offers a simple way to compute it in real time, without using invasive devices or occlusion-based techniques.

Abstract:Background: Positive end-expiratory pressure (PEEP) benefits in acute respiratory distress syndrome are driven by lung dynamic strain reduction. This depends on the variable extent of alveolar recruitment. The recruitment-to-inflation ratio estimates recruitability across a 10-cm H2O PEEP range through a simplified maneuver. Whether recruitability is uniform or not across this range is unknown. The hypotheses of this study are that the recruitment-to-inflation ratio represents an accurate estimate of PEEP-induced changes in dynamic strain, but may show nonuniform behavior across the conventionally tested PEEP range (15 to 5 cm H2O). Methods: Twenty patients with moderate-to-severe COVID-19 acute respiratory distress syndrome underwent a decremental PEEP trial (PEEP 15 to 13 to 10 to 8 to 5 cm H2O). Respiratory mechanics and end-expiratory lung volume by nitrogen dilution were measured the end of each step. Gas exchange, recruited volume, recruitment-to-inflation ratio, and changes in dynamic, static, and total strain were computed between 15 and 5 cm H2O (global recruitment-to-inflation ratio) and within narrower PEEP ranges (granular recruitment-to-inflation ratio). Results: Between 15 and 5 cm H2O, median [interquartile range] global recruitment-to-inflation ratio was 1.27 [0.40 to 1.69] and displayed a linear correlation with PEEP-induced dynamic strain reduction (r = -0.94; P < 0.001). Intraindividual recruitment-to-inflation ratio variability within the narrower ranges was high (85% [70 to 109]). The relationship between granular recruitment-to-inflation ratio and PEEP was mathematically described by a nonlinear, quadratic equation (R2 = 0.96). Granular recruitment-to-inflation ratio across the narrower PEEP ranges itself had a linear correlation with PEEP-induced reduction in dynamic strain (r = -0.89; P < 0.001). Conclusions: Both global and granular recruitment-to-inflation ratio accurately estimate PEEP-induced changes in lung dynamic strain. However, the effect of 10 cm H2O of PEEP on lung strain may be nonuniform. Granular recruitment-to-inflation ratio assessment within narrower PEEP ranges guided by end-expiratory lung volume measurement may aid more precise PEEP selection, especially when the recruitment-to-inflation ratio obtained with the simplified maneuver between PEEP 15 and 5 cm H2O yields intermediate values that are difficult to interpret for a proper choice between a high and low PEEP strategy.

Abstract:Background: There is a need of novel paradigms to interpret cardiopulmonary exercise testing (CPET) which properly considers the neurobiological underpinnings of exertional dyspnoea. Aim: We tested a conceptual framework which relates complementary metrics of demand-capacity imbalance of the respiratory system with the severity of dyspnoea as exercise intensifies. Methods: An AI-based software ( DyVe-X ) determined the overall burden (i.e., considering all CPET data points) of sex- and age-adjusted metrics of "excessive ventilation" ( dynamic ventilatory reserve (VR dyn )), "constrained ventilation" ( dynamic inspiratory reserve based on tidal volume/inspiratory capacity (IR dyn1 ) or inspiratory reserve volume/total lung capacity (IR dyn2 )), and dyspnoea (Borg 0-10) in 359 men and women with mild to end-stage COPD Results: Severe decrements (<25 th centile) in VR dyn , IR dyn1 , and IR dyn2 showed high positive predictive values for "intense" dyspnoea-work rate (>75 th centile); p <0.001)). Binary logistic regression, showed that IR dyn2 added information to the simpler IR dyn1 and VR dyn to predict "intense" dyspnoea-work rate (odds ratio (95% CI)= 3.26 (1.09-9.74), 21.6 (4.7-92.9), and 10.4 (3.03-35.6) respectively; 86.1 % correct). Of note, IR dyn1 and IR dyn2 – but not VR dyn - were highly predictive of "intense" dyspnea-ventilation (22.8 (7.37-70.7) and 9.6 (3.1-29.7) respectively; 89.1 % correct), i.e., "constrained ventilation". Conclusions: By combining indexes of "excessive" and "constrained" ventilation throughout incremental CPET, DyVe-X may provide unique insights into the physiological underpinnings of exertional dyspnoea in individual patients.

Abstract:The ability of COVID-19 to compromise the respiratory system has generated a substantial proportion of critically ill patients in need of invasive mechanical ventilation (IMV). The objective of this paper was to analyze the prognostic ability of the pulse oximetry saturation/fraction of inspired oxygen ratio (SpO2/FiO2) and the ratio of SpO2/FiO2 to the respiratory rate-ROX index-as predictors of IMV in an emergency department in confirmed COVID-19 patients. A multicenter, retrospective cohort study was carried out in four provinces of Spain between March and November 2020. The discriminative power of the predictive variable was assessed through a prediction model trained using a derivation sub-cohort and evaluated by the area under the curve (AUC) of the receiver operating characteristic (ROC) on the validation sub-cohort. A total of 2040 patients were included in the study. The IMV rate was 10.1%, with an in-hospital mortality rate of 35.3%. The performance of the SpO2/FiO2 ratio was better than the ROX index-AUC = 0.801 (95% CI 0.746-0.855) and AUC = 0.725 (95% CI 0.652-0.798), respectively. In fact, a direct comparison between AUCs resulted in significant differences (p = 0.001). SpO2 to FiO2 ratio is a simple and promising non-invasive tool for predicting risk of IMV in patients infected with COVID-19, and it is realizable in emergency departments.

Abstract:Objective: Dyspnea is one of the most common symptoms for many pulmonary diseases including COVID-19. Clinical assessment of dyspnea is mainly performed by subjective self-report, which has limited accuracy and is challenging for continuous monitoring. The objective of this research study is to determine if dyspnea progression in COVID patients can be assessed using a non-invasive wearable sensor and if the findings are comparable to a learning model of physiologically induced dyspnea on healthy subjects. Methods: Non-invasive wearable respiratory sensors were employed to retrieve continuous respiratory characteristics with user comfort and convenience. Overnight (~16h) respiratory waveforms were collected on 12 COVID-19 patients, and a benchmark on 13 healthy subjects with exertion-induced dyspnea were also performed for blind comparison. The learning model was built from the respiratory features with self report on 32 healthy subjects under exertion and airway blockage. Results: High similarity between dyspnea on COVID patients and physiologically induced dyspnea on healthy subjects was established. COVID patients have consistently high objective dyspnea scores in comparison with normal breathing of healthy subjects. We also exhibited continuous dyspnea scoring capability for 12-16 hours on patients. Conclusion: This paper validates the viability to use our objective dyspnea scoring for clinical dyspnea assessment on COVID patients. Significance: The proposed system can help the identification of dyspneic exacerbation in conditions such as COVID, leading to early intervention and possibly improving their outcome. This approach can be potentially applied to other pulmonary disorders such as asthma, emphysema, and pneumonia.
