Abstract:DIDMOAD is usually considered an autosomal recessive condition, with wide phenotypic variation, but the possibility of mitochondrial mutations occurring in this condition has been considered. A 19 year old man presented with long standing diabetes mellitus, optic atrophy, and grand mal seizures. Further investigations showed unilateral sensorineural hearing loss and the most common mitochondrial DNA mutation associated with Leber's hereditary optic neuropathy, which was inherited from his mother. This suggests the DIDMOAD phenotype is a mitochondrial disorder in some cases and is likely to have a heterogeneous aetiology.
Motivators and Barriers to Career Choices in Community Medicine Among Medical Students in South Punjab, Pakistan: A Cross-Sectional Survey

Abstract:Background Community medicine plays a vital role in public health, yet research on medical students' career choices in this field is limited, especially in underserved areas like South Punjab, Pakistan. This study explores the factors that influence undergraduate medical students' interest in pursuing a career in community medicine. Methodology A cross-sectional survey was conducted with 305 fourth- and final-year undergraduate medical students from various colleges in South Punjab. Data were collected via a structured online questionnaire, focusing on students' interests, motivations, barriers, and perceptions related to community medicine. Results The survey found that 40% of students showed interest in community medicine, with higher interest among females. Key motivators included preventive healthcare and public health initiatives, while financial constraints and unclear career progression were significant barriers. A gap was noted between students' interest and their perceived preparedness to address public health challenges. Conclusion Female students showed greater interest in community medicine and were more likely to recommend it. Bridging the gap between interest and perceived preparedness requires enhancing practical experiences, increasing the visibility of community medicine successes, and addressing financial and career progression concerns. Implementing these strategies can help attract and retain students in community medicine and improve public health outcomes.

Abstract:Background — The mental health of medical students has long been a concern. Symptoms of depression and burnout are common among medical students, but the overlap of symptoms of both diseases with risk factors remains unclear. The goal of this study was to determine the prevalence of burnout and depression symptoms’ overlap in medical students. Methods — This cross-sectional study was conducted among students of the clinical course (3rd, 4th and 5th years). A multistage stratified random sampling design was used to select 284students that were offered a validated and structured questionnaire that included a modified Oldenburg Burnout Inventory and Patient Health Questionnaire-9 to assess their emotional burnout and depression, respectively. Chi-square and multinomial regression analyses were done to identify overlapping symptoms of burnout and depression with association of risk factors with dependent variable. Results — Out of 284 students, 13% had symptoms of severe depression, 24% had high burnout symptoms, and 32% had overlapping symptoms of burnout and depression. In regression analysis, higher academic year more than 2 times (5th year, OR 2.03, 95% CI 1.42-7.96, p-value 0.004) and living at dormitory more than 3 times (OR 3.97, 95% CI 1.97-5.01, p-value 0.003) were more likely associated with more extensive overlap of burnout and depression symptoms. Conclusion — There is a high prevalence of burnout and depression symptoms among medical students, with a strong association between the two disorders. Early burnout detection and psychiatric therapies given to affected students and these disorders management training at medical school, may help reduce negative consequences of these conditions.

Material and Method: This study involved quantitative (online questionnaire) and qualitative (semi-structured interview) approaches. An online questionnaire was distributed among (n = 308) students. The questionnaire contained 30 questions covering the following three domains: economy, vocational and personal. A total of 25 individual semi-structured interviews were conducted with purposive sampling of radiography students in seven universities (public and private) across the western region of Saudi Arabia. Interview responses were coded, and main themes were extracted based on Miles and Huberman's framework. Results: The findings demonstrated that radiography was the first option as profession for 44% of the study participants. Several factors that study participants considered important were in the realm of patient care, helping patients, radiographer–patient relations, science-based profession, and the desire to work in the healthcare system. A few participants (14%) reported that they are planning to change their profession to another medical speciality. Four themes were identified from the interviews: 1) Profession decision-making, 2) changing career", 3) difficulties and challenges, and 4) recommended radiography as a profession. Conclusion: The results of the study support the need to bridge the gap between high school, universities, and employment through a collaborative network to assist students in exploring their career path by providing sufficient information and experience. In addition, there are opportunities to study these specialities abroad through the scholarship programme provided by Ministry of Education. 3 The radiography course is offered as full-time on-campus study (minor portion of the course can be delivered through blackboard). There is a good collaboration between Hospitals of Ministry of Health, Armed force, national guard, security force and King Faisal Specialist Hospital & Research Center to provide access for clinical practice and training for the students. Awareness of Interventional Radiology Among Medical Students

Abstract:Background: Interventional radiology (IR) plays a crucial role in modern medicine, offering minimally invasive treatments for various conditions. Despite its growing significance, the awareness and understanding of IR among medical students are often limited. Objective: This research aims to evaluate the level of awareness of IR among medical students at the University of Gezira in Sudan Methodology: This study is a descriptive cross-sectional study that involved 174 medical students from batches 39 and 40. Data was collected through a structured questionnaire designed to assess participants' awareness and perception of IR. The questionnaire was carried out through Google Forms platform, and the data was analyzed using IBM SPSS Statistics for Windows, Version 25 (Released 2017; IBM Corp., Armonk, New York, United States). The chi-square test was used to compare qualitative data, and a p-value of <0.05 was considered significant. Results: Females were 97 (55.7%) of the participants, while males were 77 (44.3%). The educational level found that 85 (48.9%) are finalists (fifth year) and 89 (51.1%) have graduated. A total of 75 (43.1%) of the medical students considered themselves having poor knowledge about IR. A total of 163 (93%) have confirmed that mandatory radiology course is important to be added to the curriculum of the medical school. Furthermore, 149 (85.6%) are familiar with the concept of angioplasty, and 60 (34.5%) have seen patients treated by an interventional radiologist. Most of the students are not considering IR as a career mainly due to the lack of knowledge or fear from radiological exposure. Conclusion: Medical students' awareness and perception of IR is inadequate which may adversely affect their consideration of pursuing it as a career path. Educational interventions aimed at enhancing medical students' knowledge and interest in IR may be needed to improve patient outcomes and tackle healthcare challenges.

Abstract:BACKGROUND: A low neurosurgeon-to-patient ratio persists in many developing nations including Pakistan. We aimed to investigate the perceptions of medical students regarding neurosurgery as a first step toward recognizing this problem and potentially suggesting ways to solve it.METHODS: A questionnaire was designed comprising 3 sections: 1) demographics, 2) possible challenges and perceptions regarding neurosurgery as a profession, and 3) general perceptions about neurosurgery. Statistical analysis was conducted, and a P value < 0.05 was considered significant.RESULTS: Of 387 responses received, 44.4% of male respondents and 50.6% of female respondents revealed intent to consider opting for neurosurgery as a profession. Regression analysis revealed inadequate dexterity (P = 0.001) and inability to carry out private practice (P = 0.002) were responsible for increased likelihood of opting out of neurosurgery by medical students.CONCLUSIONS: This study identified the perceptions that may influence the decision to pursue neurosurgical training among physicians early in their careers. Interventions including availability of day care facilities for children and introduction of curricula to promote neurosurgical knowledge in clinical training are expected to encourage the decision to pursue neurosurgery among medical students in Pakistan.

Abstract:Objective: To test medical students’ perceptions of the impact of artificial intelligence (AI) on radiology and the influence of these perceptions on their choice of radiology as a lifetime career. Methods: A cross-sectional multicenter survey of medical students in Saudi Arabia was conducted in April 2019. Results: Of the 476 respondents, 34 considered radiology their first specialty choice, 26 considered it their second choice, and 65 considered it their third choice. Only 31% believed that AI would replace radiologists in their lifetime, while 44.8% believed that AI would minimize the number of radiologists needed in the future. Approximately 50% believed they had a good understanding of AI; however, when knowledge of AI was tested using five questions, on average, only 22% of the questions were answered correctly. Among the respondents who ranked radiology as their first choice, 58.8% were anxious about the uncertain impact of AI on radiology. The number of respondents who ranked radiology as one of their top three choices increased by 14 when AI was not a consideration. Radiology conferences and the opinions of radiologists had the most influence on the respondents’ preferences for radiology. Conclusion: The worry that AI might displace radiologists in the future had a negative influence on medical students’ consideration of radiology as a career. Academic radiologists are encouraged to educate their students about AI and its potential impact when students are considering radiology as a lifetime career choice. Advances in knowledge: Rapid advances of AI in radiology will certainly impact the specialty, the concern of AI impact on radiology had negative influence in our participants and investing in AI education and is highly recommended.

Abstract:Abstract Background Breast cancer is the most common form of cancer in women and second most common cancer after lung cancer. The prognosis of breast cancer depends on early detection and intervention which in turn relies upon awareness. Health workers in all communities are responsible for educating the population at risk. This study evaluates the knowledge regarding breast cancer, personal judgment of confidence, perceived barriers to help seeking and breast self-examination practices among Pakistani female medical students and studies the impact of clinical training on the studied variables. Methods Descriptive cross-sectional study design using self-administered validated questionnaires (BCAM-Breast Cancer Awareness Measure designed by Cancer Research UK) was employed. Female medical students enrolled in clinical and pre-clinical classes of King Edward Medical University, Lahore were targeted and questioned regarding symptoms, risk factors and their practices regarding breast cancer. Possible barriers to seeking help were also studied. Result The mean number of symptoms correctly identified was 6.7 ± 3.2 (5.5 ± 3.2 for pre-clinical and 7.8 ± 2.9 for clinical students) and for risk factors it was 4.3 ± 2.1 (3.7 ± 2.1 for pre-clinical and 4.7 ± 2.2 for clinical students). The difference in the level of perception of two groups was found to be significant ( p < 0.001 for symptoms and p < 0.001 for risk factors). 38.7% of the subjects responded that they check their breasts rarely, 33.1% were fairly confident while 8.6% were very confident about detecting a change in their breast, 50.0% never noticed a change in their breast, and 77.4% will contact a doctor within a week or less of finding a change in their breast. Confidence about detecting a change significantly improved ( p < 0.001) after the start of clinical training. Conclusion This study suggests that clinical training may have improved knowledge of female medical students regarding breast cancer; yet the knowledge related to the symptoms and risk factors of breast cancer and frequency of breast self-examination of female medical students is less than anticipated.

Grooming Future Physician-scientists: Evaluating the Impact of Research Motivations, Practices, and Perceived Barriers Towards the Uptake of an Academic Career Among Medical Students

Abstract:PURPOSE: To evaluate the research trends and underlying motivations that shape intentions for the future uptake of an academic career among medical students. Further, to investigate the barriers and sought-after interventions which may optimise research outcomes in a resource-limited setting.METHODS: A cross-sectional study was conducted among 294 undergraduate (UG) medical students in Karachi, Pakistan. A self-administered questionnaire was employed to assess current research practices and future intentions, and to evaluate related motivations, barriers, and sought-after interventions.RESULTS: Almost two-thirds of medical students reported some form of involvement in medical research and expressed positive attitudes towards the same. However, intentions to pursue research at a professional level not only remained low (19.7%) but were found to decrease with each passing year of study (p<0.01). The most commonly expressed motivation for pursuing research was "admission into a residency program" (71.8%), and was associated with a decreased likelihood of pursuing research professionally. The most cited barriers to conducting UG research were a "lack of time" (72.4%), "lack of supervisors" (50.3%) and a "lack of opportunities in the university" (48.3%). A dichotomy in sought-after interventions was observed among research-naïve and research-experienced students.CONCLUSIONS: Despite promising trends in UG medical research, the intentions for uptake of an academic career remain low. Research practices driven by career enhancement alone may be detrimental. Interventions to increase research output must promote the capacity building of research-naïve students and facilitate the ongoing practices of research-experienced students.

Abstract:Aims and Objectives Studies investigating Radiology Teaching within the undergraduate population suggest that medical students do not feel adequately trained in radiological scan interpretation. This study aims to address this shortfall by designing and evaluating a longitudinal teaching series based upon the nationally recognised RCR Curriculum. By doing this, a framework can be provided for other centres to reproduce and develop their own teaching programme. Methods Participants were pre-clinical and clinical medical students across the United Kingdom. The course consisted of 2 sessions per anatomical system across 6 systems, one covering anatomy and the other pathology. These were Thorax, Abdomen, Musculoskeletal (MSK), Central Nervous System (CNS), Paediatric and Trauma Radiology. Surveys were filled before and after each session with questions on level of training, confidence in interpreting radiological scans out of 10 and outlook on Radiology as a career. Results Analysis of 59 pre-lecture and 60 post-lecture feedback forms showed an overall increase in confidence in scan interpretation, from 4/10 points (3-5) which increased to 7/10 points (6-8) (p<0.001). In those who filled both forms, the increase was 51%, from 4.3 ± 1 points to 6.5 ± 1.3 points (p=0.04). Conclusion The results show a promising increase in participants' subjective confidence in interpretation, which can help medical students post-graduation. With most participants reporting their exposure to radiology in medical school being inadequate, this course can serve as a base for formal teaching which educational institutions can refine upon.

Exploring the career maze: An investigation of career intentions of medical graduates in Pakistan: A qualitative study

Abstract:Background & Objectives: Pakistan faces a number of challenges in medical education. While there is an increase in the number of medical schools across the country, there is a dearth of practicing doctors in rural areas as well as a shortage of specialists in various fields specially in surgical specialties. Similarly, the number of doctors migrating overseas is also increasing due to security concerns. This requires investigation of the factors that influence career intentions of medical graduates in Pakistan. As there is no validated instrument available within Pakistani context, this qualitative study was designed to examine medical graduates’ reasons for their career intentions in light of Ajzen’s Theory of Planned Behavior (TPB). Methods: Five focus group discussions were conducted in two cities of Pakistan i.e. Karachi and Hyderabad during November – December 2012. These were then transcribed and were coded into the three primary attributes of TPB i.e. behavioral, normative and control beliefs by comparing similarities and differences. Results: The results suggest that there is a variation in the factors that influence the career intentions of the medical students. In addition a number of new themes were identified which have not been reported earlier in studies elsewhere and are specific to this region. This needs further examination by stakeholders for intervention. Conclusion: The analysis of data from the focus groups confirms the theoretical framework and identifies a range of influencing factors, at different stages of education and practice. Abstract:Background: Cardiovascular diseases (CVD) are a major cause of premature deaths globally. The gap between knowledge and behavior regarding modifiable risk factors related to these diseases has been noticed in medical students and is associated with a high number of morbidity and mortality. The aim of this study was to assess knowledge and behavior regarding cardiovascular diseases risk factors among newly enrolled MBBS students. Subjects and methods: This cross-sectional study was conducted from May to September 2021. A total of 140 first-year MBBS students participated including 80 females and 60 males. Data were collected through a self-administered questionnaire having 17 questions and two sections, one for knowledge assessment and the other for the behavior of participants regarding CVD risk factors. Results: Major risk factors for heart disease identified by students included obesity (99.3%), high cholesterol (98.6%), hypertension (97.9%), lack of physical activity (95.7%), smoking (82.1%), older age (89.3%), family history of heart disease (73.6%) and diabetes (78.6%). Pertaining to personal history, 10% were smokers, 32.1% regularly consumed soft drinks and 79.3% reported having any physical activities. Significant differences were found between male and female students regarding knowledge of CVD risk factors such as smoking (p-value = 0.003). Similarly, significant differences were observed between male and female students heart health behavior such as cholesterol checkup (31.7% vs. 16.2%, p-value = 0.032), use of soft drinks (48.3% vs 20%, p-value = 0.001) and physical activity (90% vs. 71.2%, p-value = 0.007). Conclusion: First-year medical students have good knowledge of CVD risk factors but the behavior, regarding practice to avoid these risk factors, needs improvement. It was also observed by comparison that male students have more knowledge of heart disease risk factors as compared to female students.

Abstract:Introduction Point-of-care ultrasound (POCUS) has become integral across medical specialties globally, addressing clinical queries, guiding procedures, and bridging the gap between physical examination and advanced imaging. Early ultrasound training for medical students enhances clinical decision-making and reduces diagnostic errors. Aims To evaluate the knowledge and attitude of senior medical students towards POCUS and to assess knowledge gaps and difficulties encountered by senior medical students to assist in the development of future curricula. Methodology This is an observational, cross-sectional approach to evaluate knowledge, attitude, and practice of POCUS among senior medical students in the Kingdom of Saudi Arabia. The study was conducted from January to September 2023. An electronic questionnaire was distributed through online platforms utilizing medical school databases across various regions. The survey encompassed sociodemographics, training methods, diagnostic indications, and participants' self-reported proficiency and attitudes toward POCUS. The data was chiefly collected using the Likert scale. Descriptive statistics were used to describe the quantitative and categorical variables. Bivariate and multivariate analyses were used to examine correlations. Results A total of 359 senior medical students completed the survey. Most responders were females (57.9%) with the predominating age group being ≤ 24 years (83.6%). The students predominantly were from the Central region of Saudi Arabia (75.5%). Ultrasound training varied among responders; 31.5% received formal courses (median duration: two hours) and 23.4% informal courses (median duration: four hours). Around 17.3% practiced POCUS self-teaching (median duration: four hours). A total of 3.6% had formal POCUS accreditation. A gargantuan 82.2% never used POCUS in their attached hospital for a variety of reasons. Multivariable logistic binary regression analysis showed a positive correlation between students' self-teaching of POCUS and their perceived difficulty performing an ultrasound examination for patients in daily practice. Discussion A comparable study was done at King Saud bin Abdulaziz University for Health Sciences (KSAU-HS) in 2022 surveying 229 senior medical students by Rajendram et al. In their study, 21.4% completed formal courses and 12.7% took informal courses. While many students in our study were not exposed to POCUS (82.2%), KSAU-HS reported a higher percentage reaching 94.8%. A study by Russel et al. demonstrated more than half of 154 surveyed medical schools in the United States have implemented POCUS into their students' curriculum. Conclusion POCUS stands as a valuable skill that can enhance the educational journey of undergraduate medical students. Considering that a significant number of participants haven't yet taken formal medical school courses suggests a lack of awareness about its significance in the medical field. Offering additional courses with practical components could enhance the proficiency, confidence, and outlook of medical students toward POCUS.

Abstract:Objective: To evaluate the awareness, attitude and knowledge of cardiopulmonary resuscitation (CPR) among university-enrolled medical and non-medical undergraduate students of Pakistan. Methods: Cross-sectional online survey-based study was conducted across institutes in Pakistan from December, 2022 to January, 2023. The study involved university-enrolled undergraduate students across the country. The structured questionnaire was disseminated via Google forms. For statistical analysis, SPSS version 20 was used to analyze the data by applying independent sample t-tests and ANOVA. Results: A total of 249 responses were received. After the exclusion of two responses, the overall awareness score of participants was found to be 2.49 ± 1.33, attitude score of 4.09 ± 1.74, and knowledge score of 3.51 ± 2.13. Female respondents, medical students, unmarried (single), private institutes, and respondents with educated parents achieved relatively higher scores. The overall difference in awareness scores among different regions of Pakistan was also significant (p <0.05). Gender, region, and parental literacy rate also showed effects on participants' basic life support (BLS) and CPR knowledge (p <0.05). Conclusions: Overall knowledge and awareness were unsatisfactory and inadequate in university-enrolled undergraduate students, with no one getting a complete score on very basic knowledge questions. Abstract:EDUCTIONISM HAS BEEN the driving obsesR sion of many biologists over the last decades. This obsession has camed us far through many of biology’s thorniest problems, including the search for the origins of cancer. Only a decade ago, few could have perceived even the outlines of our current corpus of results and insights. I will review my own travels through this field over this decade, in large part because they illustrate how many disparate fields of basic biological research have impinged on and pushed forward the work of those struggling to divine the origins of human cancer. Many molecular biologists kept a healthy distance from cancer research in the early and mid-1970s. The reasons were clear and oft-repeated. Molecular biology and genetics offered powerful tools for a clear understanding of single cell micro-organisms, but cancer seemed to represent a process at the organismic level, thereby confronting the experimentalist with a prospect of endless, unfathomable complexity. The cancer problem could not be reduced to a set of small, separable, easily studied pieces. Additionally frustrating for those steeped in microbial genetics was the strongly phenomenonologic vein of cancer research: the multiple aspects of tumors and cancer cells were being described in everincreasing detail, but these descriptions yielded only consequences, end results, and few insights into causal mechanisms. By cataloguing consequences, researchers were confined to the periphery of the cancer process. The engines working at the core remained obscure. By the early 1970s, we had one clear idea about where to look for cancer’s origins. A half century of work with Drosophila had shown that x-rays, which were known to induce human cancer, also created genetic lesions in the fly genome. Abstract:Background Medical students often face challenges in choosing a career path due to limited exposure to specialized fields like neurosurgery. Understanding their perceptions and experiences is crucial in addressing the gaps in neurosurgical education and inspiring future neurosurgeons. Methods A cross-sectional study was conducted involving 461 medical students, utilizing convenience sampling. Data collection employed a validated self-administered tool. Statistical analysis in SPSS Version 25 included t-tests, chi-square tests, comparing scores based on age, gender, year of study, and exposure to the formal neurosurgical rotations in their institute. Significance value was set at p < 0.05. Results In the study of 461 medical students, 79.8% identified with the 19-23 age group, and 63.8% affirmed neurosurgery exposure. Medical students' perceptions included 167(36.3%) students found neurosurgery teaching sufficient, 164 (35.6%) disagreed on obtaining neurosurgical history is difficult, 224 (48.6%) agreed on neurosurgical disease complexity, and 250 (54.2%) found these diseases challenging and interesting. A majority of 183 (39.7%) respondents agreed that neurosurgical diseases had poor outcomes. Regarding training of the neurosurgical surgery 205 (44.5%) participants strongly agreed on its length, and 215 (46.7%) consented on extensive operating hours. However, 167 (36.3%) strongly disagreed about ample job prospects in Pakistan. Conclusion Enhancing neurosurgery education with quality, consistency, and adaptability is essential to bridge gaps and inspire future neurosurgeons. These findings guide improvements in educational programs, preparing a skilled workforce to meet evolving healthcare demands.

Why Medical Students Pursue Radiology: A Current Longitudinal Survey on Motivations and Controversial Issues in Radiology

Abstract:Rationale and objectives: Radiology is an increasingly competitive specialty. Various current factors influence medical students' decision to pursue a radiology career, including artificial intelligence (AI), remote reading, and COVID-19. This study seeks to determine the decision-making factors of all alumni from our medical school who matched into a radiology residency, and to gather opinions on emerging radiology topics. Materials and methods: A survey querying decision-making factors and opinions on current radiology topics was distributed to all alumni from our medical school (first graduating class in 2011) who previously matched into a diagnostic or interventional radiology residency program (n = 57). Wilcoxon Rank-Sum and Fisher's Exact tests were used to determine statistical significance. Results: Forty-three of fifty-seven responses were received (75% response rate). The most influential factor that sparked respondents' interest in radiology was a radiology elective (25/43, 58%). Students who will finish radiology training in 2023 or later were more likely to be influenced by a mentor (15/23, 65%) than those who finished radiology training before 2023 (5/20, 25%) (p = 0.04). Respondents reported a 1.6/5 concern about AI negatively impacting their future career in radiology. There was 1.7/5 concern about performing radiology procedures on patients during the COVID-19 pandemic. Respondents predicted that remote reading would have a 3.2/5 positive impact on helping them achieve their preferred lifestyle. Job satisfaction among attending radiologists is rated at 4.3/5. Conclusion: Radiology electives had the greatest influence in piquing students' interest in radiology, while mentorship is assuming increasing influence. AI is perceived as a relatively minimal threat to negatively impact radiologists' jobs. Respondents had little concern about performing radiology procedures during the COVID-19 pandemic. Remote reading is viewed as having a moderately positive impact on lifestyle. Responding radiologists enjoy notably high job satisfaction.

Abstract:Background and aims: Increased use of digital devices in the modern era has led to the development of digital eye strain (DES) or computer vision syndrome in their users. This can result in the development of various ocular and visual symptoms among them. In this study, the authors aimed to view the prevalence of digital eye strain among radiology physicians in Pakistan and their associated risk factors. Materials and methods: A cross-sectional study was conducted to evaluate occupational DES among radiology physicians in Pakistan. The data collection was done using the convenience sampling technique, and the data were analyzed using IBM SPSS for Windows, Version 25.0. Results: Out of the 247 respondents, 33.6% were males and 66.4% were females. 41.7% of them were between 30 and 40 years of age and 51.8% of them were radiology residents. 52.2% of the participants had a refractive error and were using a corrective lens. The majority of the radiologists in our study (84.2%) preferred picture archiving and communication system (PACS) over films and 82.2% of them reported having breaks of less than 15 min. Major symptoms reported by the participants were tired or heavy eyes (69.6%) and headache (69.3%). The proportion of developing DES was higher in females [P=0.001, adjusted odds ratio (aOR)=2.94], radiology residents (P=0.031, aOR=3.29), and working hours of more than 4 h per day (P<0.001, aOR=0.04). Conclusion: With recent advances in the field of radiology in Pakistan, the frequency of developing DES among radiologists is increasing. Being a female, having long working hours, and having noticeable flickers on the digital screens were among the significant factors in developing DES among radiologists.

Abstract:Objectives: To assess the perceptions of dental students regarding their clinical learning environment in an urban setting. Method: The descriptive, cross-sectional study was conducted from March 2020 to May 2021 after approval from the ethics review committee of Islamic International Dental College, Islamabad, Pakistan, and comprised clinical year students, house officers and postgraduate trainees from 6 dental teaching hospitals of Rawalpindi and Islamabad. Data was collected using a pre-validated instrument assessing the dental clinical learning environment. Data was analysed using SPSS 24. Results: Of the 1030 students approached, 561(54.4%) responded. Of them, 448(80%) were girls, 234(41.7%) were 3rd year students, 110(19.6%) were 4th year students, 120(21.4%) were house officers and 97(17.3%) were postgraduate trainees. Female students had a better patient attitude and were more confident in their abilities to keep up with their peers (p<0.05). The students were content with the performance of their clinical teachers, with a mean score of 70.99+/-16.0, while the lowest score of 54.67+/-22.9 was for clinical infrastructure and materials. Students of Islamabad pointed out the lack of clinical materials and maintenance of equipment but noted better research opportunities compared to their Rawalpindi counterparts (p<0.05). Conclusion: The clinical learning environment for Islamabad and Rawalpindi cities individually was good. Overall, students were satisfied with their clinical teachers, learning and training experience. Dental materials, infrastructure and maintenance factors scored the lowest. Abstract:We sought to evaluate the impact of crossing strategy on the incidence of periprocedural myocardial infarction (PMI) during chronic total occlusion (CTO) percutaneous coronary intervention (PCI). Background: The optimal technique for crossing coronary CTOs remains controversial. Methods: We retrospectively examined the incidence of PMI among 184 consecutive patients who underwent CTO PCI at our institution between 2012 and 2015. Creatine kinase‐myocardial band fraction (CK‐MB) and troponin were measured before and after PCI in all patients. PMI was defined as CK‐MB increase ≥3× upper limit of normal (ULN). Results: Mean age was 65 ± 8 years, 98% of patients were men, 57% had diabetes mellitus, 36% were current smokers, 38% had prior heart failure, 31% had prior coronary artery bypass graft surgery (CABG), and 55% had prior PCI. The retrograde approach was used in 38% of cases. As compared with antegrade wire escalation and antegrade dissection/re‐entry, use of the retrograde approach was associated with higher J‐CTO (Multicenter CTO Registry of Japan) scores (P < 0.0001), higher frequency of moderate or severe calcification (P = 0.0061), longer CTO length (P < 0.0001), more frequent proximal cap ambiguity (P < 0.0001), and lower technical (P = 0.0007) and procedural (P = 0.0014) success. The frequency of PMI for the antegrade‐only and retrograde cases was 10% and 33%, respectively (P = 0.0001). On multivariate analysis, use of the retrograde approach and moderate/severe calcification were independently associated with higher incidence of PMI. Conclusions: As compared with antegrade‐only crossing techniques, the retrograde approach is used in patients with more complex anatomy but may carry higher risk for PMI. © 2016 Wiley Periodicals, Inc.
