Abstract:Objective To investigate the analgesic effect of parecoxib sodium and oxycodone for postoperative pain after total knee arthroplasty. Methods Sixty patients undergoing elective TKA were randomly divided into 2 groups( n =30 each) : parecoxib group( group A),parecoxib and oxycodone group( group B). All the patients were received parecoxib sodium for analgesia at the first 24h after surgery. From then on,if the visual analogue score( VAS) > 5 the supplements( parecoxib sodium,oxycodone) would be infused in turn till the patient felt comfortable or the analgesic effect reached the clinical maximum. The VAS and Ramsay sedation score( RSS) were recorded at 24,32, 40,48,and 72 hours after surgery. Adverse effects related to analgesia and the maximum tolerable degree of passive exercise were investigated and compared between the two groups. Results Compared with group A,the resting and exercising VAS was decreased in group B at 24,32,40,48,and 72 hours after surgery,while the sedation score increased( P < 0. 05). And the maximum tolerable degree of passive exercise was higher in group B at the 48 and 72 hours after surgery( P < 0. 05). No serious adverse effects occurred between the two groups. Conclusions Intravenous injection of parecoxib sodium combined with oral oxycodone is consistent trend of multimodal analgesia. It can realize the basic goal of postoperative analgesia in patients undergoing total knee arthroplasty.
Additional femoral nerve block analgesia does not reduce the chronic pain after total knee arthroplasty: A retrospective study in patients with knee osteoarthritis.

Abstract:Femoral nerve block analgesia was deemed to the gold standard for acute pain management after total knee arthroplasty (TKA). But effect on chronic pain management is not investigated fully. We conducted a retrospective study to explore the effect of single-injection femoral nerve block on postsurgical chronic pain. All medical records of patients undertaking TKA between January, 2013 and June, 2014 were reviewed via the Docare anesthesia database. Patients who administrated with the self-controlled intravenous analgesia were assigned to group P. Patients who received a single-injection femoral never block combined with patient self-controlled intravenous analgesia were assigned to group N+P. The visual analog scale (VAS) score before surgery, the first postoperative day (POD 1), POD 2, 3 months, 6 months, and 12 months after surgery were extracted from medical records. Pain score was compared over these 2 groups to investigate treatment outcomes. In all, 470 patients met the selection criteria for group P and 266 patients met the selection criteria for group N+P. Compared with group P, the VAS score decreased significantly in group N+P at POD 1 (P<.001), and the same was observed at POD 2 (P<.001); the moderate to severe pain incidence rate decreased significantly in group N+P at POD 1 (P<.01) and POD 2 (motion, P<.001). The rescued anesthesia rate reduced significantly in group N+P in POD 1 (P=.001), whereas no difference was found in POD 2 (P=.864). No difference was found at 3, 6, and 12 months after surgery (all P>.05). The single-injection femoral nerve block could relieve the acute postsurgical pain in a short period of time. But no evidence was found that it could reduce the chronic pain between 3 and 12 months after TKA.

Abstract:BACKGROUND The PROSPECT (PROcedure SPEcific Postoperative Pain ManagemenT) Working Group is a global collaboration of surgeons and anaesthesiologists formulating procedure-specific recommendations for pain management after common operations. Total knee arthroplasty (TKA) is associated with significant postoperative pain that is difficult to treat. Nevertheless, pain control is essential for rehabilitation and to enhance recovery. OBJECTIVE To evaluate the available literature and develop recommendations for optimal pain management after unilateral primary TKA. DESIGN A narrative review based on published systematic reviews, using modified PROSPECT methodology. DATA SOURCES A literature search was performed in EMBASE, MEDLINE, PubMed and Cochrane Databases, between January 2014 and December 2020, for systematic reviews and meta-analyses evaluating analgesic interventions for pain management in patients undergoing TKA. ELIGIBILITY CRITERIA Each randomised controlled trial (RCT) included in the selected systematic reviews was critically evaluated and included only if met the PROSPECT requirements. Included studies were evaluated for clinically relevant differences in pain scores, use of nonopioid analgesics, such as paracetamol and nonsteroidal anti-inflammatory drugs and current clinical relevance. RESULTS A total of 151 systematic reviews were analysed, 106 RCTs met PROSPECT criteria. Paracetamol and nonsteroidal anti-inflammatory or cyclo-oxygenase-2-specific inhibitors are recommended. This should be combined with a single shot adductor canal block and peri-articular local infiltration analgesia together with a single intra-operative dose of intravenous dexamethasone. Intrathecal morphine (100 μg) may be considered in hospitalised patients only in rare situations when both adductor canal block and local infiltration analgesia are not possible. Opioids should be reserved as rescue analgesics in the postoperative period. Analgesic interventions that could not be recommended were also identified. CONCLUSION The present review identified an optimal analgesic regimen for unilateral primary TKA. Future studies to evaluate enhanced recovery programs and specific challenging patient groups are needed.

Abstract:INTRODUCTION: The aim of this systematic review was to document efficacy, safety and quality of evidence of analgesic interventions after total knee arthroplasty (TKA).METHODS: This PRISMA-compliant and PROSPERO-registered review includes all-language randomized controlled trials of medication-based analgesic interventions after TKA. Bias was evaluated according to Cochrane methodology. Outcomes were opioid consumption (primary), pain scores at rest and during mobilization, adverse events, and length of stay. Interventions investigated in three or more trials were meta-analysed. Outcomes were evaluated using forest plots, Grading of Recommendations Assessment, Development and Evaluation (GRADE), L'Abbe Plots and trial sequential analysis.RESULTS: The included 113 trials, investigating 37 different analgesic interventions, were characterized by unclear/high risk of bias, low assay sensitivity and considerable differences in pain assessment tools, basic analgesic regimens, and reporting of adverse events. In meta-analyses single and continuous femoral nerve block (FNB), intrathecal morphine, local infiltration analgesia, intraarticular injection of local anaesthetics, non-steroidal anti-inflammatory drugs, and gabapentinoids demonstrated significant analgesic effects. The 24-hour morphine-sparing effects ranged from 4.2 mg (CI: 1.3, 7.2; intraarticular local anaesthetics), to 16.6 mg (CI: 11.2, 22; single FNB). Pain relieving effects at rest at 6 hours ranged from 4 mm (CI: -10, 2; gabapentinoids), to 19 mm (CI: 8, 31; single FNB), and at 24 hours from 3 mm (CI: -2, 8; gabapentinoids), to 16 mm (CI: 8, 23; continuous FNB). GRADE-rated quality of evidence was generally low.CONCLUSION: A low quality of evidence, small sample sizes and heterogeneity of trial designs prohibit designation of an optimal procedure-specific analgesic regimen after TKA.

Abstract:Background: Several local regional anesthesia regimes have been described in the literature to reduce post-surgical pain following total knee arthroplasty (TKA), but it is unclear which regime has the best analgetic effect combined with the best motor function. The aim of this study was to determine if patients with infiltration between the popliteal artery and capsule of the posterior knee (IPACK) combined with an adductor canal block (SACB) had less pain, better motor function, and less opioid consumption after TKA than patients with a femoral nerve block (FNB) combined with a popliteal sciatic nerve block (PSB). Methods: In a retrospective cohort analysis, 342 patients following primary TKA were examined; 175 patients were treated with an IPACK combined with a SACB, and 167 patients with a femoral FNB combined with a PSB. The outcome parameters postoperative pain (visual analogue scale (VAS) for mobilization and at rest, functional recovery, opioid consumption, hospital discharge, and complications were analyzed and compared between both groups. Results: The IPACK/SACB group had a higher postoperative need for opioids despite higher doses of ropivacaine compared to the FNB/PSB group, accompanied by higher VAS scores. Patients' satisfaction was equal between the groups. Both groups showed comparable mobilization rates and walking distances following TKA. Conclusions: IPACK/SACB showed equal results compared to FNB/PSB for mobilization rates and patients' satisfaction following TKA without a reduction in opioid consumption.

Abstract:ABSTRACT Using the simian immunodeficiency virus/human immunodeficiency virus (SHIV)-macaque model of AIDS, we had shown in a previous report that a live, nonpathogenic strain of SHIV, further attenuated by deletion of the vpu gene and inoculated orally into adult macaques, had effectively prevented AIDS following vaginal inoculation with pathogenic SHIVKU. Examination of lymph nodes from the animals at 18 weeks postchallenge had shown that all six animals were persistently infected with challenge virus. We report here on a 2-year follow-up study on the nature of the persistent infections in these animals. DNA of the vaccine virus was present in the lymph nodes at all time points tested, as far as 135 weeks postchallenge. In contrast, the DNA of SHIVKU became undetectable in one animal by week 55 and in three others by week 63. These four macaques have remained negative for SHIVKU DNA as far as the last time point examined at week 135. Quantification of the total viral DNA concentration in lymph nodes during the observation period showed a steady decline. All animals developed neutralizing antibody and cytotoxic-T-lymphocyte responses to SHIVKU that persisted throughout the observation period. Vaccine-like viruses were isolated from two animals, and a SHIVKU-like virus was isolated from one of the two macaques that remained positive for SHIVKU DNA. There was no evidence of recombination between the vaccine and the challenge viruses. Thus, immunization with the live vaccine not only prevented disease but also contributed to the steady decline in the virus burdens in the animals.

Abstract:Post-operative knee pain management has become a challenge to provide early relief and pain-free postoperative care to the patient. The major objectives of post-operative analgesic treatment are to reduce opioid requirements, post-operative pain, and adverse events related to opioid intake. This narrative review aimed to document post-operative analgesia techniques after total knee arthroplasty (TKA). The traditional approach involved high-dose opioid-based regimen, though opioid is considered strong analgesic, but are associated with a number of unwanted side effects to seek for alternative techniques. The role of sciatic nerve block in TKA pain is doubtful. Femoral Nerve Block (FNB) is still considered as the gold standard; however, FNB is associated with quadriceps weakness and risk of fall and sciatic block with foot drop. To overcome these drawback more distal nerve block techniques has evolved, namely saphenous nerve block in adductor canal, selective tibial which are claimed to provide comparable analgesia to that of femoral and sciatic nerve block. The combination of pre-emptive and multi-modal analgesia and technically well-delivered regional nerve blocks and postoperative physical therapy are an essential component which not only minimize the side effects of traditional opioid-based analgesia but also speed up functional recovery, increases patient satisfaction, and reduces the overall length of hospitalization and cost.
Comparing Efficacy of Different Methods for Postoperative Analgesia in Patients Undergoing Total Knee Arthroplasty: Several Issues Need Special Attention

Abstract:By a randomized, double-blind, placebo, parallel, and controlled study including 80 patients who underwent unilateral primary total knee arthroplasty (TKA), Li et al[1] assessed the benefits of combining the femoral triangle block (FTB) and the interspace between the popliteal artery and the capsule of the posterior knee (IPACK) block by comparing with intraarticular cocktail analgesic mixture local injection (ICAMLA). They showed that a combination of FTB and IPACK block significantly increased postoperative quadriceps strength, with an improved early postoperative pain control and reduced consumptions of intraoperative anesthetics and postoperative morphine for pain control. Given that the use of nonopioid or opioid-sparing multimodal analgesia protocol to improve postoperative pain control is being emphasized in current practice of enhanced recovery after surgery (ERAS) protocols,[2] their findings have clinical implications. Other than the limitations described in discussion, however, there are several issues in method and results of this study that need further clarification and discussion. First, this study did not include a sample size evaluation, though it is a crucial content in designing a randomized controlled trial for preventing the type I and type II statistical errors.[3] Furthermore, the authors did not clearly describe the expected minimal clinically important difference of primary end point between the groups. Because of these limitations, we argue that primary findings of this small sample study must be interpreted with caution. Second, the visual analogue scale (VAS) scores of resting and active pain at nearly all time points after surgery were significantly higher in patients receiving the ICAMLA compared with those receiving a combination of FTB and IPACK block. We would like to remind the readers that for patients undergoing TKA, the recommended minimal clinically important difference of postoperative pain score is 1.5 at rest state and 1.8 during motion on a 0–10 pain scale.[4] In this study, other than VAS scores of resting pain at postoperative 2 hours, the between-group differences in mean VAS scores of resting and active pain at all other time points did not exceed the above recommended minimal clinically important differences. Furthermore, a VAS pain score of more than 3 is generally considered as an unsatisfactory postoperative pain control.[2] We noted that mean VAS scores of active pain postoperatively until the 12-hour mark in the two groups were more than 4.5, indicating that significant proportion of patients experienced moderate to severe active pain. This is evidently not conducive to early postoperative activity and functional exercise, which are important components of ERAS protocols for patients with TKA.[5] Most importantly, this study did not assess and compare patient satisfaction with postoperative pain control. In this case, it is difficult for readers to determine whether improvement of postoperative pain control with a combination of FTB and IPACK should be considered as being clinically important. Third, total morphine consumption for postoperative pain control was significantly decreased in patients receiving a combination of FTB and IPACK block compared with those receiving ICAMLA. However, the between-group difference of mean morphine consumptions did not exceed the recommended minimal clinically important difference, i.e., an absolute reduction of 10 mg intravenous morphine in 24 hours.[4] Thus, we questioned the clinical significance of this small opioid sparing with a combination of FTB and IPACK block. Finally, this study assessed the range of keen motion, quadriceps strength, the occurrence of postoperative complications and length of hospital stay, but it did not evaluate the quality of postoperative recovery, as performed in other studies assessing postoperative benefits of nerve blocks in patients undergoing TKA.[6] [7] As a quality outcome of ERAS protocols, the quality of postoperative recovery is very important for determining efficacy and clinical availability of an intervention for postoperative pain control[2] and is very easily measured by a comprehensive QoR-15 score ranging from 0 to 150, with a higher score indicating an improved quality of postoperative recovery.[8] Thus, we believe that this study would have provided more useful data regarding clinical values of a combination of FTB and IPACK block in patients with TKA, if the design had included the assessment on the quality of recovery. Abstract:Effective postoperative analgesia is of great significance for postoperative rehabilitation. This meta-analysis aimed to investigate the efficacy of corticosteroid on pain following total joint arthroplasty.PubMed (1996-December 2020), Embase (1996-December 2020), and the Cochrane Library (CENTRAL, December 2020) were searched and a total of 11 randomized controlled trials met our inclusion criteria.Eleven randomized controlled trials met the inclusion criteria. Pooled data indicated the corticosteroid group was effective compared to the control group in terms of the visual analogue scale at rest (P < .05) and movement (P < .05), the total morphine equivalent consumption (P < .05), and the length of stay (P < .05), without increasing the risk of periprosthetic joint infection (P = .74) and the length of stay (P = .32).Compared to the control group, intraoperative corticosteroid was benefit to the pain management in total joint arthroplasty.
PROSPECT guideline for total hip arthroplasty: a systematic review and procedure‐specific postoperative pain management recommendations

Abstract:<p>The aim of this systematic review was to develop recommendations for the management of postoperative pain after primary elective total hip arthroplasty, updating the previous procedure-specific postoperative pain management (PROSPECT) guidelines published in 2005 and updated in July 2010. Randomised controlled trials and meta-analyses published between July 2010 and December 2019 assessing postoperative pain using analgesic, anaesthetic, surgical or other interventions were identified from MEDLINE, Embase and Cochrane databases. Five hundred and twenty studies were initially identified, of which 108 randomised trials and 21 meta-analyses met the inclusion criteria. Peri-operative interventions that improved postoperative pain include: paracetamol; cyclo-oxygenase-2-selective inhibitors; non-steroidal anti-inflammatory drugs; and intravenous dexamethasone. In addition, peripheral nerve blocks (femoral nerve block; lumbar plexus block; fascia iliaca block), single-shot local infiltration analgesia, intrathecal morphine and epidural analgesia also improved pain. Limited or inconsistent evidence was found for all other approaches evaluated. Surgical and anaesthetic techniques appear to have a minor impact on postoperative pain, and thus their choice should be based on criteria other than pain. In summary, the analgesic regimen for total hip arthroplasty should include pre-operative or intra-operative paracetamol and cyclo-oxygenase-2-selective inhibitors or non-steroidal anti-inflammatory drugs, continued postoperatively with opioids used as rescue analgesics. In addition, intra-operative intravenous dexamethasone 8–10 mg is recommended. Regional analgesic techniques such as fascia iliaca block or local infiltration analgesia are recommended, especially if there are contra-indications to basic analgesics and/or in patients with high expected postoperative pain. Epidural analgesia, femoral nerve block, lumbar plexus block and gabapentinoid administration are not recommended as the adverse effects outweigh the benefits. Although intrathecal morphine 0.1 mg can be used, the PROSPECT group emphasises the risks and side-effects associated with its use and provides evidence that adequate analgesia may be achieved with basic analgesics and regional techniques without intrathecal morphine.</p>

Opioid-Free Forefoot Surgery vs Traditional Perioperative Opiate Regimen: A Randomized Controlled Trial

Abstract:Background: In response to the opioid epidemic, the use of multimodal pain management in orthopaedic surgery is increasing. Efforts to decrease opioid prescribing and opioid consumption among foot and ankle surgical patients are needed. The purpose of this study was to compare the efficacy and adverse events between 2 multimodal pain management pathways for forefoot surgical patients: standard opioid-containing (OC) and opioid-free (OF). Methods: This is a single-center noninferior randomized controlled trial of 51 patients undergoing forefoot surgery allocated to one of 2 perioperative pain management treatments: opioid-free, multimodal (OF, n=27 patients), or traditional opioid-containing (OC, n=24 patients). Patient characteristics, creatine markers, pain (numeric rating scale [NRS]), general health (Veterans Rand 12-Item Health Survey [VR-12]), and depression were measured preoperatively. Postoperatively, pain was measured at 24-hour, 2-week, and 6-week time points. Satisfaction with pain control, complications, and general health were measured at 2 and 6 weeks. Results: The OF group is statistically noninferior to the OC group and reported lower median pain scores at 24 hours (2 [IQR 0, 3] vs 6 [IQR 3.5, 7]; p<.0001) and 2 weeks (2 [IQR 1, 4] vs 4 [IQR 0, 3]; p=.018]. By 6 weeks, pain levels were similar between groups. More than 85% of all patients reported satisfaction with pain level at 2 weeks, which increased to >90% at 6 weeks. The VR-12 scores were similar between groups across all time points. At 2 weeks, 8 patients in each group reported constipation. By 6 weeks, all but 2 OC patients reported resolution. No other adverse events of postoperative wound complications, readmissions, medication reactions, thrombosis, or persistent pain were documented. Conclusion: In forefoot surgery, the opioid-free pain management protocol was statistically noninferior to the opioid-containing protocol in reducing postoperative pain. Abstract:Total knee arthroplasty (TKA) is one of the most common surgeries performed to relieve joint pain in patients with end‐stage osteoarthritis or rheumatic arthritis of the knee. However, TKA is followed by moderate to severe postoperative pain that affects postoperative rehabilitation, patient satisfaction, and overall outcomes. Historically, opioids have been widely used for perioperative pain management of TKA. However, opioids are associated with undesirable adverse effects, such as nausea, respiratory depression, and retention of urine, which limit their application in daily clinical practice. The aim of this review was to discuss the current postoperative pain management regimens for TKA. Our review of the literature demonstrated that multimodal analgesia is considered the optimal regimen for perioperative pain management of TKA and improves clinical outcomes and patient satisfaction, through a combination of several types of medications and delivery routes, including preemptive analgesia, neuraxial anesthesia, peripheral nerve blockade, patient‐controlled analgesia and local infiltration analgesia, and oral opioid/nonopioid medications. Multimodal analgesia provides superior pain relief, promotes recovery of the knee, and reduces opioid consumption and related adverse effects in patients undergoing TKA.

Comparison of Femoral Nerve Catheter, Adductor Canal Catheter, and Periarticular Liposomal Bupivacaine Infiltration for Postoperative Analgesia After Primary Total Knee Arthroplasty

Abstract:Background: Peripheral nerve catheters are used to provide analgesia after total knee arthroplasty (TKA) and have been shown to decrease pain and opioid use, to facilitate participation with physical therapy (PT), and to hasten discharge. More recently, pericapsular infiltration using liposomal bupivacaine (LB) has been employed as an alternative analgesic approach. Methods: This retrospective study compared outcomes for three analgesic approaches: femoral nerve catheter (FNC), adductor canal catheter (ACC), and intraoperative LB infiltration. The primary outcome was numeric rating scale (NRS) pain scores at 24 hours. Secondary outcomes included pain scores at 12, 36, and 48 hours, time-to-first opioid, cumulative opioid use, distance walked, and time-to-discharge. Results: Pain scores at 24 hours were significantly lower in both the ACC and FNC cohorts when compared to the LB cohort (3.1 versus 4.6 [P = .017] and 2.4 versus 4.6 [P < .0001]). The ACC and FNC groups did not differ significantly at that timepoint (P = .27). Similar comparisons were found at 12 and 36 hours, while at 48 hours the FNC group was superior. Time to first opioid and opioid consumption favored the ACC and FNC groups. Walking distance favored the ACC group. Both the ACC and LB groups had a faster time-to-discharge than the FNC group. Conclusion: Both ACCs and FNCs provided superior analgesia at 24 hours compared to LB, while being equivalent to each other. Pain scores at 12 hours and 36 hours as well as opioid consumption through 48 hours mirrored this finding. Although various differences were found between groups in terms of time-to-first analgesic, walking distance and time-to-discharge, the ACC approach appeared to optimally balance analgesia, ambulation, and time-to-discharge.

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Background</h3><p>This study aimed to explore the efficacy of two unique combinations of nerve blocks on postoperative pain and functional outcome after total knee arthroplasty (TKA).</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>Patients scheduled for TKA were randomized to receive a combination of adductor canal block (ACB) + infiltration between the popliteal artery and capsule of the posterior knee block (IPACK) + sham obturator nerve block (ONB) + sham lateral femoral cutaneous nerve block (LFCNB) (control group), or a combination of ACB + IPACK + ONB + sham LFCNB (triple nerve block group), or a combination of ACB + IPACK + ONB + LFCNB (quadruple nerve block group). All patients received local infiltration analgesia. Primary outcome was postoperative morphine consumption. Secondary outcomes were the time until first rescue analgesia, postoperative pain assessed on the visual analogue scale (VAS), QoR-15 score, functional recovery of knee and postoperative complications.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>Compared to the control group, the triple and quadruple nerve block groups showed significantly lower postoperative morphine consumption (17.2±9.7 mg <em>vs.</em> 11.2±7.0 mg <em>vs</em>. 11.4±6.4 mg, <em>p</em> = 0.001). These two groups also showed significantly longer time until first rescue analgesia (<em>p</em> = 0.007 and 0.010, respectively, analyzed with Kaplan-Meier method), significantly lower VAS scores on postoperative day 1 (<em>p</em> < 0.01), significantly better QoR-15 scores on postoperative days 1 and 2 (p < 0.001), and significantly better functional recovery of knee including range of motion (<em>p</em> = 0.002 and 0.001 on postoperative days 1 and 2) and daily ambulation distance (<em>p</em> <0.001 and <em>p</em> = 0.004 on postoperative days 1 and 2). However, the absolute change in morphine consumption, VAS scores and QoR-15 scores did not exceed the reported minimal clinically important differences (MCIDs) (morphine consumption: 10 mg; VAS scores: 1.5 at rest and 1.8 during movement; QoR-15 scores: 8.0). The MCIDs of other outcomes has not been reported in literature. The triple and quadruple nerve block groups showed no significant differences in these outcomes between each other. The three groups did not show a significant difference in complication rates.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusion</h3><p>Adding ONB or ONB + LFCNB to ACB + IPACK can statistically reduce morphine consumption, improve early pain relief and functional recovery. However, the absolute change in morphine consumption, VAS scores and QoR-15 scores did not exceed the MCIDs. Based on our findings and considering the sample size of this study, there is not enough clinical evidence to support the triple or quadruple nerve block use within a multimodal analgesic pathway after TKA.</p>

Abstract:Since there is no consensus on the optimal perioperative analgesic method, the present article provides a brief and practical synthesis on current multimodal pre–postsurgery analgesia protocols for primary TKA (excluding intraoperative anesthetic techniques) reported especially in recent meta-analyses and reviews. The focus is not only on the traditional aims (pain scores and opioid sparing), but also the functional recovery and patient-reported outcomes. Multimodal analgesia (pre-emptive analgesia, local infiltration analgesia, peripheral nerve blocks, and specific systemic drugs) is considered the optimal regimen for perioperative pain management of TKA.

Abstract:Total knee arthroplasty (TKA) often involves significant postoperative pain, necessitating effective analgesia. This meta‐analysis compares the analgesic efficacy of local infiltration anaesthesia (LIA) and femoral nerve block (FNB) in managing postoperative wound pain following TKA. Adhering to Preferred Reporting Items for Systematic Reviews and Meta‐Analyses (PRISMA) guidelines, this meta‐analysis was structured around the PICO framework, assessing studies that directly compared LIA and FNB in TKA patients. A comprehensive search across PubMed, Embase, Web of Science and the Cochrane Library was conducted without time restrictions. Studies were included based on specific criteria such as participant demographics, study design and outcomes like pain scores and opioid consumption. Quality assessment utilized the Cochrane Collaboration's risk of bias tool. The statistical approach was determined based on heterogeneity, with the choice of fixed‐ or random‐effects models guided by the I2 statistic. Sensitivity analysis and evaluation of publication bias using funnel plots and Egger's linear regression test were also conducted. From an initial pool of 1275 articles, eight studies met the inclusion criteria. These studies conducted in various countries from 2007 to 2016. The meta‐analysis showed no significant difference in resting and movement‐related Visual Analogue Scale scores post‐TKA between the LIA and FNB groups. However, LIA was associated with significantly lower opioid consumption. The quality assessment revealed a low risk of bias in most studies, and the sensitivity analysis confirmed the stability of these findings. There was no significant publication bias detected. Both LIA and FNB are effective in controlling postoperative pain in TKA patients, but LIA offers the advantage of lower opioid consumption. Its simplicity, cost‐effectiveness and opioid‐sparing nature make LIA the recommended choice for postoperative analgesia in knee replacement surgeries.

Abstract:Four of five geographical isolates of Heterodera glycines from Indiana classified as Race 3 using standard differentials showed many differences when classified using another group of differentials comprised of five soybean breeding lines and cultivars. Two isolates from northern Indiana produced cysts on more of the differentials tested than did three isolates from southern Indiana, suggesting that potential resistant lines should be tested on a range of H. glycines populations originating from the areas for which cultivars are being developed.
