Abstract:B0 field inhomogeneity is a long-lasting issue for Cardiac MRI (CMR) in high-field (3T and above) scanners. The inhomogeneous B0 fields can lead to corrupted image quality, prolonged scan time, and false diagnosis. B0 shimming is the most straightforward way to improve the B0 homogeneity. However, today's standard cardiac shimming protocol requires manual selection of a shim volume, which often falsely includes regions with large B0 deviation (e.g., liver, fat, and chest wall). The flawed shim field compromises the reliability of high-field CMR protocols, which significantly reduces the scan efficiency and hinders its wider clinical adoption. This study aims to develop a dual-channel deep learning model that can reliably contour the cardiac region for B0 shim without human interaction and under variable imaging protocols. By utilizing both the magnitude and phase information, the model achieved a high segmentation accuracy in the B0 field maps compared to the conventional single-channel methods (Dice score: 2D-mag = 0.866, 3D-mag = 0.907, and 3D-mag-phase = 0.938, all p < 0.05). Furthermore, it shows better generalizability against the common variations in MRI imaging parameters and enables significantly improved B0 shim compared to the standard method (SD(B0Shim): Proposed = 15 ± 11% vs. Standard = 6 ± 12%, p < 0.05). The proposed autonomous model can boost the reliability of cardiac shimming at 3T and serve as the foundation for more reliable and efficient high-field CMR imaging in clinical routines.
Reconstruction of Cardiac Cine MRI Using Motion-Guided Deformable Alignment and Multi-Resolution Fusion

Abstract:Cardiac cine magnetic resonance imaging (MRI) is one of the important means to assess cardiac functions and vascular abnormalities. Mitigating artifacts arising during image reconstruction and accelerating cardiac cine MRI acquisition to obtain high-quality images is important. A novel end-to-end deep learning network is developed to improve cardiac cine MRI reconstruction. First, a U-Net is adopted to obtain the initial reconstructed images in k-space. Further to remove the motion artifacts, the motion-guided deformable alignment (MGDA) module with second-order bidirectional propagation is introduced to align the adjacent cine MRI frames by maximizing spatial-temporal information to alleviate motion artifacts. Finally, the multi-resolution fusion (MRF) module is designed to correct the blur and artifacts generated from alignment operation and obtain the last high-quality reconstructed cardiac images. At an 8$\times$ acceleration rate, the numerical measurements on the ACDC dataset are structural similarity index (SSIM) of 78.40%$\pm$.57%, peak signal-to-noise ratio (PSNR) of 30.46$\pm$1.22dB, and normalized mean squared error (NMSE) of 0.0468$\pm$0.0075. On the ACMRI dataset, the results are SSIM of 87.65%$\pm$4.20%, PSNR of 30.04$\pm$1.18dB, and NMSE of 0.0473$\pm$0.0072. The proposed method exhibits high-quality results with richer details and fewer artifacts for cardiac cine MRI reconstruction on different accelerations.

DMCVR: Morphology-Guided Diffusion Model for 3D Cardiac Volume Reconstruction

Abstract:Accurate 3D cardiac reconstruction from cine magnetic resonance imaging (cMRI) is crucial for improved cardiovascular disease diagnosis and understanding of the heart's motion. However, current cardiac MRI-based reconstruction technology used in clinical settings is 2D with limited through-plane resolution, resulting in low-quality reconstructed cardiac volumes. To better reconstruct 3D cardiac volumes from sparse 2D image stacks, we propose a morphology-guided diffusion model for 3D cardiac volume reconstruction, DMCVR, that synthesizes high-resolution 2D images and corresponding 3D reconstructed volumes. Our method outperforms previous approaches by conditioning the cardiac morphology on the generative model, eliminating the time-consuming iterative optimization process of the latent code, and improving generation quality. The learned latent spaces provide global semantics, local cardiac morphology and details of each 2D cMRI slice with highly interpretable value to reconstruct 3D cardiac shape. Our experiments show that DMCVR is highly effective in several aspects, such as 2D generation and 3D reconstruction performance. With DMCVR, we can produce high-resolution 3D cardiac MRI reconstructions, surpassing current techniques. Our proposed framework has great potential for improving the accuracy of cardiac disease diagnosis and treatment planning. Abstract:Accurate and robust estimation of the three-dimensional left ventricular geometry and deformation has important clinical implications for better diagnosis and understanding of ischemic heart diseases. So far, most image analysis efforts have performed the shape recovery and the motion tracking tasks in separate steps, typically in sequential fashion. In this paper, we present a continuum biomechanics model based framework that performs simultaneous segmentation and motion analysis of the left ventricle from 3D image sequences, achieved through the tracking of the spatiotemporal evolution of a 3D active region model driven by imaging data, statistical priors of the left ventricular boundaries, and cyclic motion models of the myocardial tissue elements. Experiments on 3D canine and human MR image sequences have shown the superiority of the strategy.

Abstract:Feature tracking Cardiac Magnetic Resonance (CMR) has recently emerged as an area of interest for quantification of regional cardiac function from balanced, steady state free precession (SSFP) cine sequences. However, currently available techniques lack full automation, limiting reproducibility. We propose a fully automated technique whereby a CMR image sequence is first segmented with a deep, fully convolutional neural network (CNN) architecture, and quadratic basis splines are fitted simultaneously across all cardiac frames using least squares optimization. Experiments are performed using data from 42 patients with hypertrophic cardiomyopathy (HCM) and 21 healthy control subjects. In terms of segmentation, we compared state-of-the-art CNN frameworks, U-Net and dilated convolution architectures, with and without temporal context, using cross validation with three folds. Performance relative to expert manual segmentation was similar across all networks: pixel accuracy was ~97%, intersection-over-union (IoU) across all classes was ~87%, and IoU across foreground classes only was ~85%. Endocardial left ventricular circumferential strain calculated from the proposed pipeline was significantly different in control and disease subjects (-25.3% vs -29.1%, p = 0.006), in agreement with the current clinical literature.

Abstract:Delineating anatomical structures for cardiac magnetic resonance imaging (CMRI) is crucial for various medical applications such as medical diagnoses, treatment, and pathological studies. CMRI segmentation, which aims to automatically and accurately segment the heart structures, is highly beneficial for cardiologists. However, it is non-trivial to perfectly segment the ventricles, especially for the heart apex slices, considering their small sizes compared to the input images. For example, the endocardium in the Sunnybrook dataset only occupies 4% of the entire image by average. During the training process, these target pixels, or other hard samples, are buried by the massive backgrounds that make the model mostly receive optimization signals from easy samples. In this paper, we propose a focal loss constrained residual network (FR-Net) to tackle the problem. In order to mitigate the fact that the gradients of the hard samples can be easily overwhelmed by the easy samples, we use a pixel-wise re-weighting strategy to balance the gradients. Furthermore, considering focal loss constraints for each pixel independently, we propose an alternative training fashion that trains the model with focal loss and dice loss alternatively. The segmentation model can not only benefit from the pixel-wise focal loss but also from the region-wise dice loss to comprehensively optimize the model. We conducted thorough experiments on the Sunnybrook dataset, CMRI dataset, right ventricle dataset, and ACDC dataset to verify the effectiveness of the proposed method.
Efficient and Phase-aware Video Super-resolution for Cardiac MRI

Abstract:Cardiac Magnetic Resonance Imaging (CMR) is widely used since it can illustrate the structure and function of heart in a non-invasive and painless way. However, it is time-consuming and high-cost to acquire the high-quality scans due to the hardware limitation. To this end, we propose a novel end-to-end trainable network to solve CMR video super-resolution problem without the hardware upgrade and the scanning protocol modifications. We incorporate the cardiac knowledge into our model to assist in utilizing the temporal information. Specifically, we formulate the cardiac knowledge as the periodic function, which is tailored to meet the cyclic characteristic of CMR. In addition, the proposed residual of residual learning scheme facilitates the network to learn the LR-HR mapping in a progressive refinement fashion. This mechanism enables the network to have the adaptive capability by adjusting refinement iterations depending on the difficulty of the task. Extensive experimental results on large-scale datasets demonstrate the superiority of the proposed method compared with numerous state-of-the-art methods.

Abstract:BACKGROUND: Cardiovascular magnetic resonance (CMR) is an important imaging modality for the assessment and management of adult patients with congenital heart disease (CHD). However, conventional techniques for three-dimensional (3D) whole-heart acquisition involve long and unpredictable scan times and methods that accelerate scans via k-space undersampling often rely on long iterative reconstructions. Deep-learning-based reconstruction methods have recently attracted much interest due to their capacity to provide fast reconstructions while often outperforming existing state-of-the-art methods. In this study, we sought to adapt and validate a non-rigid motion-corrected model-based deep learning (MoCo-MoDL) reconstruction framework for 3D whole-heart MRI in a CHD patient cohort.METHODS: The previously proposed deep-learning reconstruction framework MoCo-MoDL, which incorporates a non-rigid motion-estimation network and a denoising regularization network within an unrolled iterative reconstruction, was trained in an end-to-end manner using 39 CHD patient datasets. Once trained, the framework was evaluated in eight CHD patient datasets acquired with seven-fold prospective undersampling. Reconstruction quality was compared with the state-of-the-art non-rigid motion-corrected patch-based low-rank reconstruction method (NR-PROST) and against reference images (acquired with three-or-four-fold undersampling and reconstructed with NR-PROST).RESULTS: Seven-fold undersampled scan times were 2.1 ± 0.3 minutes and reconstruction times were ∼30 seconds, approximately 240 times faster than an NR-PROST reconstruction. Image quality comparable to the reference images was achieved using the proposed MoCo-MoDL framework, with no statistically significant differences found in any of the assessed quantitative or qualitative image quality measures. Additionally, expert image quality scores indicated the MoCo-MoDL reconstructions were consistently of a higher quality than the NR-PROST reconstructions of the same data, with the differences in 12 of the 22 scores measured for individual vascular structures found to be statistically significant.CONCLUSION: The MoCo-MoDL framework was applied to an adult CHD patient cohort, achieving good quality 3D whole-heart images from ∼2-minute scans with reconstruction times of ∼30 seconds.

Improved 3D Whole Heart Geometry from Sparse CMR Slices

Abstract:Cardiac magnetic resonance (CMR) imaging and computed tomography (CT) are two common non-invasive imaging methods for assessing patients with cardiovascular disease. CMR typically acquires multiple sparse 2D slices, with unavoidable respiratory motion artefacts between slices, whereas CT acquires isotropic dense data but uses ionising radiation. In this study, we explore the combination of Slice Shifting Algorithm (SSA), Spatial Transformer Network (STN), and Label Transformer Network (LTN) to: 1) correct respiratory motion between segmented slices, and 2) transform sparse segmentation data into dense segmentation. All combinations were validated using synthetic motion-corrupted CMR slice segmentation generated from CT in 1699 cases, where the dense CT serves as the ground truth. In 199 testing cases, SSA-LTN achieved the best results for Dice score and Huasdorff distance (94.0% and 4.7 mm respectively, average over 5 labels) but gave topological errors in 8 cases. STN was effective as a plug-in tool for correcting all topological errors with minimal impact on overall performance (93.5% and 5.0 mm respectively). SSA also proves to be a valuable plug-in tool, enhancing performance over both STN-based and LTN-based models. Abstract:The accurate measurement of 3D cardiac function is an important task in the analysis of cardiac magnetic resonance (MR) images. However, short-axis image acquisitions with thick slices are commonly used in clinical practice due to constraints of acquisition time, signal-to-noise ratio and patient compliance. In this situation, the estimation of a high-resolution image can provide an approximation of the underlaying 3D measurements. In this paper, we develop a novel algorithm for the estimation of high-resolution cardiac MR images from single short-axis cardiac MR image stacks. First, we propose to use a novel approximate global search approach to find patch correspondence between the short-axis MR image and a set of atlases. Then, we propose an innovative super-resolution model which does not require explicit motion estimation. Finally, we build an expectation-maximization framework to optimize the model. We validate the proposed approach using images from 19 subjects with 200 atlases and show that the proposed algorithm significantly outperforms conventional interpolation such as linear or B-spline interpolation. In addition, we show that the super-resolved images can be used for the reproducible estimation of 3D cardiac functional indices.
4D Myocardium Reconstruction with Decoupled Motion and Shape Model

Abstract:Estimating the shape and motion state of the myocardium is essential in diagnosing cardiovascular diseases.However, cine magnetic resonance (CMR) imaging is dominated by 2D slices, whose large slice spacing challenges inter-slice shape reconstruction and motion <a class="link-external link-http" href="http://acquisition.To" rel="external noopener nofollow">this http URL</a> address this problem, we propose a 4D reconstruction method that decouples motion and shape, which can predict the inter-/intra- shape and motion estimation from a given sparse point cloud sequence obtained from limited slices. Our framework comprises a neural motion model and an end-diastolic (ED) shape model. The implicit ED shape model can learn a continuous boundary and encourage the motion model to predict without the supervision of ground truth deformation, and the motion model enables canonical input of the shape model by deforming any point from any phase to the ED phase. Additionally, the constructed ED-space enables pre-training of the shape model, thereby guiding the motion model and addressing the issue of data scarcity. We propose the first 4D myocardial dataset as we know and verify our method on the proposed, public, and cross-modal datasets, showing superior reconstruction performance and enabling various clinical applications.

Abstract:Background and Objective High-resolution (HR) MR images provide rich structural detail to assist physicians in clinical diagnosis and treatment plan. However, it is arduous to acquire HR MRI due to equipment limitations, scanning time or patient comfort. Instead, HR MRI could be obtained through a number of computer assisted post-processing methods that have proven to be effective and reliable. This paper aims to develop a converlutional neural network (CNN) based super-resolution reconstruction framework for low-resolution (LR) T2w images. Method In this paper, we propose a novel multi-modal HR MRI generation framework based on deep learning techniques. Specifically, we construct a CNN based on multi-resolution analysis to learn an end-to-end mapping between LR T2w and HR T2w, where HR T1w is fed into the network to offer detailed a priori information to help generate HR T2w. Furthermore, a low-frequency filtering module is introduced to filter out the interference from HR-T1w during high-frequency information extraction. Based on the idea of multi-resolution analysis, detailed features extracted from HR T1w and LR T2w are fused at two scales in the network and then HR T2w is reconstructed by upsampling and dense connectivity module. Results Extensive quantitative and qualitative evaluations demonstrate that the proposed method enhances the recovered HR T2w details and outperforms other state-of-the-art methods. In addition, the experimental results also suggest that our network has a lightweight structure and favorable generalisation performance. Conclusion The results show that the proposed method is capable of reconstructing HR T2w with higher accuracy. Meanwhile, the super-resolution reconstruction results on other dataset illustrate the excellent generalization ability of the method.

Improving coronary artery imaging in single source CT with cardiac motion correction using attention and spatial transformer based neural networks

Abstract:Motion artifact is a major challenge in cardiac CT which hampers accurate delineation of key anatomic (e.g. coronary lumen) and pathological features (e.g. stenosis). Conventional motion correction techniques are limited on patients with high / irregular heart rate, due to simplified modeling of CT systems and cardiac motion. Emerging deep learning based cardiac motion correction techniques have demonstrated the potential of further quality improvement. Yet, many methods require CT projection data or advanced motion simulation tools that are not readily available. We aim to develop an image-domain motion-correction method, using convolutional neural network (CNN) integrated with customized attention and spatial transformer techniques. Forty cardiac CT exams acquired from a clinical dual-source CT system were retrospectively collected to generate training (n=26) and testing (n=14) sets. Dual-source data uniquely allow image reconstruction with different temporal resolutions from the same patient scan. Slow temporal resolution (140ms; equivalent to single-source CT (SSCT) half scan) and fast temporal resolution (75ms; dual source) images were reconstructed to generate paired samples of motion-corrupted and reference images. The combinations of 2 training-inference strategies and 3 CNNs were evaluated: strategy #1 - whole-heart images in training / inference; strategy #2 - vessel patches in training / inference; CNN #1 - attention only; CNN #2 - spatial-transformer (STN) only; CNN #3 - attention & STN synergy. Testing data showed that CNN #3 with strategy #2 provided relatively better performance: improving vessel delineation, increasing structural similarity index from 0.85 to 0.91, and reducing mean CT number error of lumen by 71.0%. Our method could improve the image quality in cardiac exams with SSCT.
Whole Heart 3D+T Representation Learning Through Sparse 2D Cardiac MR Images

Abstract:Cardiac Magnetic Resonance (CMR) imaging serves as the gold-standard for evaluating cardiac morphology and function. Typically, a multi-view CMR stack, covering short-axis (SA) and 2/3/4-chamber long-axis (LA) views, is acquired for a thorough cardiac assessment. However, efficiently streamlining the complex, high-dimensional 3D+T CMR data and distilling compact, coherent representation remains a challenge. In this work, we introduce a whole-heart self-supervised learning framework that utilizes masked imaging modeling to automatically uncover the correlations between spatial and temporal patches throughout the cardiac stacks. This process facilitates the generation of meaningful and well-clustered heart representations without relying on the traditionally required, and often costly, labeled data. The learned heart representation can be directly used for various downstream tasks. Furthermore, our method demonstrates remarkable robustness, ensuring consistent representations even when certain CMR planes are missing/flawed. We train our model on 14,000 unlabeled CMR data from UK BioBank and evaluate it on 1,000 annotated data. The proposed method demonstrates superior performance to baselines in tasks that demand comprehensive 3D+T cardiac information, e.g. cardiac phenotype (ejection fraction and ventricle volume) prediction and multi-plane/multi-frame CMR segmentation, highlighting its effectiveness in extracting comprehensive cardiac features that are both anatomically and pathologically relevant.

Abstract:Diffusion Tensor Cardiac Magnetic Resonance (DT-CMR) is the only in vivo method to non-invasively examine the microstructure of the human heart. Current research in DT-CMR aims to improve the understanding of how the cardiac microstructure relates to the macroscopic function of the healthy heart as well as how microstructural dysfunction contributes to disease. To get the final DT-CMR metrics, we need to acquire diffusion weighted images of at least 6 directions. However, due to DWI's low signal-to-noise ratio, the standard voxel size is quite big on the scale for microstructures. In this study, we explored the potential of deep-learning-based methods in improving the image quality volumetrically (x4 in all dimensions). This study proposed a novel framework to enable volumetric super-resolution, with an additional model input of high-resolution b0 DWI. We demonstrated that the additional input could offer higher super-resolved image quality. Going beyond, the model is also able to super-resolve DWIs of unseen b-values, proving the model framework's generalizability for cardiac DWI superresolution. In conclusion, we would then recommend giving the model a high-resolution reference image as an additional input to the low-resolution image for training and inference to guide all super-resolution frameworks for parametric imaging where a reference image is available.

Abstract:Cardiac Magnetic Resonance imaging (CMR) is the gold standard for assessing cardiac function. Segmenting the left ventricle (LV), right ventricle (RV), and LV myocardium (MYO) in CMR images is crucial but time-consuming. Deep learning-based segmentation methods have emerged as effective tools for automating this process. However, CMR images present additional challenges due to irregular and varying heart shapes, particularly in basal and apical slices. In this study, we propose a classifier-guided two-stage network with an all-slice fusion transformer to enhance CMR segmentation accuracy, particularly in basal and apical slices. Our method was evaluated on extensive clinical datasets and demonstrated better performance in terms of Dice score compared to previous CNN-based and transformer-based models. Moreover, our method produces visually appealing segmentation shapes resembling human annotations and avoids common issues like holes or fragments in other models' segmentations.

Abstract:Background and objective The sheer volume of data generated by population imaging studies is unparalleled by current capabilities to extract objective and quantitative cardiac phenotypes; subjective and time-consuming manual image analysis remains the gold standard. Automated image analytics to compute quantitative imaging biomarkers of cardiac function are desperately needed. Data volumes and their variability pose a challenge to most state-of-the-art methods for endo- and epicardial contours, which lack robustness when applied to very large datasets. Our aim is to develop an analysis pipeline for the automatic quantification of cardiac function from cine magnetic resonance imaging data. Method This work adopt 4,638 cardiac MRI cases coming from UK Biobank with ground truth available for left and RV contours. A hybrid and robust algorithm is proposed to improve the accuracy of automatic left and right ventricle segmentation by harnessing the localization accuracy of deep learning and the morphological accuracy of 3D-ASM (three-dimensional active shape models). The contributions of this paper are three-fold. First, a fully automatic method is proposed for left and right ventricle initialization and cardiac MRI segmentation by taking full advantage of spatiotemporal constraint. Second, a deeply supervised network is introduced to train and segment the heart. Third, the 3D-ASM image search procedure is improved by combining image intensity models with convolutional neural network (CNN) derived distance maps improving endo- and epicardial edge localization. Results The proposed architecture outperformed the state of the art for cardiac MRI segmentation from UK Biobank. The statistics of RV landmarks detection errors for Triscuspid valve and RV apex are 4.17 mm and 5.58 mm separately. The overlap metric, mean contour distance, Hausdorff distance and cardiac functional parameters are calculated for the LV (Left Ventricle) and RV (Right Ventricle) contour segmentation. Bland–Altman analysis for clinical parameters shows that the results from our automated image analysis pipelines are in good agreement with results from expert manual analysis. Conclusions Our hybrid scheme combines deep learning and statistical shape modeling for automatic segmentation of the LV/RV from cardiac MRI datasets is effective and robust and can compute cardiac functional indexes from population imaging.

Abstract:Automated segmentation of Cardiac Magnetic Resonance (CMR) plays a pivotal role in efficiently assessing cardiac function, offering rapid clinical evaluations that benefit both healthcare practitioners and patients. While recent research has primarily focused on delineating structures in the short-axis orientation, less attention has been given to long-axis representations, mainly due to the complex nature of structures in this orientation. Performing pixel-wise segmentation of the left ventricular (LV) myocardium and the four cardiac chambers in 2-D steady-state free precession (SSFP) cine sequences is a crucial preprocessing stage for various analyses. However, the challenge lies in the significant variability in contrast, appearance, orientation, and positioning of the heart across different patients, clinical views, scanners, and imaging protocols. Consequently, achieving fully automatic semantic segmentation in this context is notoriously challenging. In recent years, several deep learning models have been proposed to accurately quantify and diagnose cardiac pathologies. These automated tools heavily rely on the accurate segmentation of cardiac structures in magnetic resonance images (MRI). Hence, there is a need for new methods to handle such structures' geometrical and textural complexities. We proposed 2D and 3D two-stage self-supervised deep learning segmentation hybrid transformer and CNN-based architectures for 4CH whole heart segmentation. Accurate segmentation of the ventricles and atria in 4CH views is crucial for analyzing heart health and reconstructing four-chamber meshes, which are essential for estimating various parameters to assess overall heart condition. Our proposed method outperformed state-of-the-art techniques, demonstrating superior performance in this domain.
