Abstract:B0 field inhomogeneity is a long-lasting issue for Cardiac MRI (CMR) in high-field (3T and above) scanners. The inhomogeneous B0 fields can lead to corrupted image quality, prolonged scan time, and false diagnosis. B0 shimming is the most straightforward way to improve the B0 homogeneity. However, today's standard cardiac shimming protocol requires manual selection of a shim volume, which often falsely includes regions with large B0 deviation (e.g., liver, fat, and chest wall). The flawed shim field compromises the reliability of high-field CMR protocols, which significantly reduces the scan efficiency and hinders its wider clinical adoption. This study aims to develop a dual-channel deep learning model that can reliably contour the cardiac region for B0 shim without human interaction and under variable imaging protocols. By utilizing both the magnitude and phase information, the model achieved a high segmentation accuracy in the B0 field maps compared to the conventional single-channel methods (Dice score: 2D-mag = 0.866, 3D-mag = 0.907, and 3D-mag-phase = 0.938, all p < 0.05). Furthermore, it shows better generalizability against the common variations in MRI imaging parameters and enables significantly improved B0 shim compared to the standard method (SD(B0Shim): Proposed = 15 ± 11% vs. Standard = 6 ± 12%, p < 0.05). The proposed autonomous model can boost the reliability of cardiac shimming at 3T and serve as the foundation for more reliable and efficient high-field CMR imaging in clinical routines.

Abstract:Background and objective The sheer volume of data generated by population imaging studies is unparalleled by current capabilities to extract objective and quantitative cardiac phenotypes; subjective and time-consuming manual image analysis remains the gold standard. Automated image analytics to compute quantitative imaging biomarkers of cardiac function are desperately needed. Data volumes and their variability pose a challenge to most state-of-the-art methods for endo- and epicardial contours, which lack robustness when applied to very large datasets. Our aim is to develop an analysis pipeline for the automatic quantification of cardiac function from cine magnetic resonance imaging data. Method This work adopt 4,638 cardiac MRI cases coming from UK Biobank with ground truth available for left and RV contours. A hybrid and robust algorithm is proposed to improve the accuracy of automatic left and right ventricle segmentation by harnessing the localization accuracy of deep learning and the morphological accuracy of 3D-ASM (three-dimensional active shape models). The contributions of this paper are three-fold. First, a fully automatic method is proposed for left and right ventricle initialization and cardiac MRI segmentation by taking full advantage of spatiotemporal constraint. Second, a deeply supervised network is introduced to train and segment the heart. Third, the 3D-ASM image search procedure is improved by combining image intensity models with convolutional neural network (CNN) derived distance maps improving endo- and epicardial edge localization. Results The proposed architecture outperformed the state of the art for cardiac MRI segmentation from UK Biobank. The statistics of RV landmarks detection errors for Triscuspid valve and RV apex are 4.17 mm and 5.58 mm separately. The overlap metric, mean contour distance, Hausdorff distance and cardiac functional parameters are calculated for the LV (Left Ventricle) and RV (Right Ventricle) contour segmentation. Bland–Altman analysis for clinical parameters shows that the results from our automated image analysis pipelines are in good agreement with results from expert manual analysis. Conclusions Our hybrid scheme combines deep learning and statistical shape modeling for automatic segmentation of the LV/RV from cardiac MRI datasets is effective and robust and can compute cardiac functional indexes from population imaging.

Abstract:Accurate segmentation of heart structures imaged by cardiac MR is key for the quantitative analysis of pathology. High-resolution 3D MR sequences enable whole-heart structural imaging but are time-consuming, expensive to acquire and they often require long breath holds that are not suitable for patients. Consequently, multiplanar breath-hold 2D cine sequences are standard practice but are disadvantaged by lack of whole-heart coverage and low through-plane resolution. To address this, we propose a conditional variational autoencoder architecture able to learn a generative model of 3D high-resolution left ventricular (LV) segmentations which is conditioned on three 2D LV segmentations of one short-axis and two long-axis images. By only employing these three 2D segmentations, our model can efficiently reconstruct the 3D high-resolution LV segmentation of a subject. When evaluated on 400 unseen healthy volunteers, our model yielded an average Dice score of $87.92 \pm 0.15$ and outperformed competing architectures.

Abstract:Accurate and robust estimation of the three-dimensional left ventricular geometry and deformation has important clinical implications for better diagnosis and understanding of ischemic heart diseases. So far, most image analysis efforts have performed the shape recovery and the motion tracking tasks in separate steps, typically in sequential fashion. In this paper, we present a continuum biomechanics model based framework that performs simultaneous segmentation and motion analysis of the left ventricle from 3D image sequences, achieved through the tracking of the spatiotemporal evolution of a 3D active region model driven by imaging data, statistical priors of the left ventricular boundaries, and cyclic motion models of the myocardial tissue elements. Experiments on 3D canine and human MR image sequences have shown the superiority of the strategy.

Abstract:The accurate reconstruction of the right heart geometry and motion from time-resolved medical images enhances diagnostic tools based on image visualization as well as the analysis of cardiac blood dynamics through computational methods. Due to the peculiarity of the right heart morphology and motion, commonly used segmentation and/or reconstruction techniques, which only employ Short-Axis cine-MRI, lack accuracy in relevant regions of the right heart, like the ventricular base and the outflow tract. Moreover, the reconstruction procedure is time-consuming and, in the case of the generation of computational domains, requires a lot of manual intervention. This paper presents a new method for the accurate and efficient reconstruction of the right heart geometry and motion from time-resolved MRI. In particular, the proposed method makes use of surface morphing to merge information coming from multi-series cine-MRI (such as Short/Long-Axis and 2/3/4 Chambers acquisitions) and to reconstruct important cardiac features. It also automatically provides the complete cardiac contraction and relaxation motion by exploiting a suitable image registration technique. The method is applied both to a healthy and a pathological (tetralogy of Fallot) case, and yields more accurate results than standard procedures. The proposed method is also employed to provide significant input for computational fluid dynamics. The corresponding numerical results demonstrate the reliability of our approach in the computation of clinically relevant blood dynamics quantities.

Abstract:Quantitative assessment of the shape and motion variability of the heart has important implications for the diagnosis and treatment of cardiac diseases. In this paper, we present a unified methodology which simultaneously recovers the shape and motion of the left ventricle, including the endo-, epi-, and mid-wall myocardium. The left ventricle is modeled as an isotropic linear elastic material, and represented by volumetric mesh constructed from the Delaunay triangulation of the sampling points. Specifically, the evolution forces imposed on the myocardium are individually constructed for each nodal point through the integration of the data-driven edginess measures, the prior spatial distributions of the myocardial tissues, the temporal coherence of the image-derived salient features, and the cyclic motion characteristics of the heart. The dense displacement field can then be estimated when the total energy of the elastic body is minimized at equilibrium. Experiments on 3D human magnetic resonance image sequences of healthy and pathological subjects show the accuracy and robustness of the strategy

Abstract:The efficient construction of anatomical models is one of the major challenges of patient-specific in-silico models of the human heart. Current methods frequently rely on linear statistical models, allowing no advanced topological changes, or requiring medical image segmentation followed by a meshing pipeline, which strongly depends on image resolution, quality, and modality. These approaches are therefore limited in their transferability to other imaging domains. In this work, the cardiac shape is reconstructed by means of three-dimensional deep signed distance functions with Lipschitz regularity. For this purpose, the shapes of cardiac MRI reconstructions are learned to model the spatial relation of multiple chambers. We demonstrate that this approach is also capable of reconstructing anatomical models from partial data, such as point clouds from a single ventricle, or modalities different from the trained MRI, such as the electroanatomical mapping (EAM).

While application to cardiac magnetic resonance (CMR) has demonstrated excellent results, automated analysis of 3D echocardiography (3D-echo) remains challenging, due to the lower signal-to-noise ratio (SNR), signal dropout, and greater interobserver variability in manual annotations. As 3D-echo is becoming increasingly widespread, robust analysis methods will substantially benefit patient evaluation. We sought to leverage the high SNR of CMR to provide training data for a convolutional neural network (CNN) capable of analysing 3D-echo. We imaged 73 participants (53 healthy volunteers, 20 patients with non-ischaemic cardiac disease) under both CMR and 3D-echo (<1 hour between scans). 3D models of the left ventricle (LV) were independently constructed from CMR and 3D-echo, and used to spatially align the image volumes using least squares fitting to a cardiac template. The resultant transformation was used to map the CMR mesh to the 3D-echo image. Alignment of mesh and image was verified through volume slicing and visual inspection (Fig. 1) for 120 paired datasets (including 47 rescans) each at end-diastole and end-systole. 100 datasets (80 for training, 20 for validation) were used to train a shallow CNN for mesh extraction from 3D-echo, optimised with a composite loss function consisting of normalised Euclidian distance (for 290 mesh points) and volume. Data augmentation was applied in the form of rotations and tilts (<15 degrees) about the long axis. The network was tested on the remaining 20 datasets (different participants) of varying image quality (Tab. I). For comparison, corresponding LV measurements from conventional manual analysis of 3D-echo and associated interobserver variability (for two observers) were also estimated. Initial results indicate that the use of embedded CMR meshes as training data for 3D-echo analysis is a promising alternative to manual analysis, with improved accuracy and precision compared with conventional methods. Further optimisations and a larger dataset are expected to improve network performance. Abstract:In cardiac cine imaging, acquiring high-quality data is challenging and time-consuming due to the artifacts generated by the heart's continuous movement. Volumetric, fully isotropic data acquisition with high temporal resolution is, to date, intractable due to MR physics constraints. To assess whole-heart movement under minimal acquisition time, we propose a deep learning model that reconstructs the volumetric shape of multiple cardiac chambers from a limited number of input slices while simultaneously optimizing the slice acquisition orientation for this task. We mimic the current clinical protocols for cardiac imaging and compare the shape reconstruction quality of standard clinical views and optimized views. In our experiments, we show that the jointly trained model achieves accurate high-resolution multi-chamber shape reconstruction with errors of mode=text, retain-explicit-plus 80 , indicating its effectiveness in both simulated cardiac cine MRI and clinical cardiac MRI with a wide range of pathological shape variations.

Abstract:Abstract Background Measurement of cardiac structure and function from images (e.g. volumes, mass and derived parameters such as left ventricular (LV) ejection fraction [LVEF]) guides care for millions. This is best assessed using cardiovascular magnetic resonance (CMR), but image analysis is currently performed by individual clinicians, which introduces error. We sought to develop a machine learning algorithm for volumetric analysis of CMR images with demonstrably better precision than human analysis. Methods A fully automated machine learning algorithm was trained on 1923 scans (10 scanner models, 13 institutions, 9 clinical conditions, 60,000 contours) and used to segment the LV blood volume and myocardium. Performance was quantified by measuring precision on an independent multi-site validation dataset with multiple pathologies with n = 109 patients, scanned twice. This dataset was augmented with a further 1277 patients scanned as part of routine clinical care to allow qualitative assessment of generalization ability by identifying mis-segmentations. Machine learning algorithm (‘machine’) performance was compared to three clinicians (‘human’) and a commercial tool (cvi42, Circle Cardiovascular Imaging). Findings Machine analysis was quicker (20 s per patient) than human (13 min). Overall machine mis-segmentation rate was 1 in 479 images for the combined dataset, occurring mostly in rare pathologies not encountered in training. Without correcting these mis-segmentations, machine analysis had superior precision to three clinicians (e.g. scan-rescan coefficients of variation of human vs machine: LVEF 6.0% vs 4.2%, LV mass 4.8% vs. 3.6%; both P < 0.05), translating to a 46% reduction in required trial sample size using an LVEF endpoint. Conclusion We present a fully automated algorithm for measuring LV structure and global systolic function that betters human performance for speed and precision.

Joint image reconstruction and segmentation of real-time cardiac MRI in free-breathing using a model based on disentangled representation learning

Abstract:A joint image reconstruction and segmentation approach based on disentangled representation learning was trained to enable cardiac cine MR imaging in real-time and under free-breathing. An exploratory feasibility study tested the proposed method in undersampled real-time acquisitions based on an in-house developed spiral bSSFP pulse sequence in eight healthy participants and five patients with intermittent atrial fibrillation. Images and predicted LV segmentations were compared to the reference standard of ECG-gated segmented Cartesian cine in repeated breath-holds and corresponding manual segmentation. On a 5-point Likert scale, image quality of the real-time breath-hold approach and Cartesian cine was comparable in healthy participants (RT-BH: 1.99 $\pm$ .98, Cartesian: 1.94 $\pm$ .86, p=.052), but slightly inferior in free-breathing (RT-FB: 2.40 $\pm$ .98, p<.001). In patients with arrhythmia, image quality from both real-time approaches was favourable (RT-BH: 2.10 $\pm$ 1.28, p<.001, RT-FB: 2.40 $\pm$ 1.13, p<.001, Cartesian: 2.68 $\pm$ 1.13). Intra-observer reliability was good (ICC=.77, 95%-confidence interval [.75, .79], p<.001). In functional analysis, a positive bias was observed for ejection fractions derived from the proposed model compared to the clinical reference standard (RT-BH mean EF: 58.5 $\pm$ 5.6%, bias: +3.47%, 95%-confidence interval [-.86, 7.79%], RT-FB mean: 57.9 $\pm$ 10.6%, bias: +1.45%, [-3.02, 5.91%], Cartesian mean: 54.9 $\pm$ 6.7%). The introduced real-time MR imaging technique is capable of acquiring high-quality cardiac cine data in 1-2 minutes without the need for ECG gating and breath-holds. It thus offers a promising alternative to the current clinical practice of segmented acquisition, with shorter scan times, higher patient comfort and increased robustness to arrhythmia and patient incompliance.

Multi-class point cloud completion networks for 3D cardiac anatomy reconstruction from cine magnetic resonance images

Abstract:Cine magnetic resonance imaging (MRI) is the current gold standard for the assessment of cardiac anatomy and function. However, it typically only acquires a set of two-dimensional (2D) slices of the underlying three-dimensional (3D) anatomy of the heart, thus limiting the understanding and analysis of both healthy and pathological cardiac morphology and physiology. In this paper, we propose a novel fully automatic surface reconstruction pipeline capable of reconstructing multi-class 3D cardiac anatomy meshes from raw cine MRI acquisitions. Its key component is a multi-class point cloud completion network (PCCN) capable of correcting both the sparsity and misalignment issues of the 3D reconstruction task in a unified model. We first evaluate the PCCN on a large synthetic dataset of biventricular anatomies and observe Chamfer distances between reconstructed and gold standard anatomies below or similar to the underlying image resolution for multiple levels of slice misalignment. Furthermore, we find a reduction in reconstruction error compared to a benchmark 3D U-Net by 32% and 24% in terms of Hausdorff distance and mean surface distance, respectively. We then apply the PCCN as part of our automated reconstruction pipeline to 1000 subjects from the UK Biobank study in a cross-domain transfer setting and demonstrate its ability to reconstruct accurate and topologically plausible biventricular heart meshes with clinical metrics comparable to the previous literature. Finally, we investigate the robustness of our proposed approach and observe its capacity to successfully handle multiple common outlier conditions.

Deep Conditional Shape Models for 3D cardiac image segmentation

Abstract:Delineation of anatomical structures is often the first step of many medical image analysis workflows. While convolutional neural networks achieve high performance, these do not incorporate anatomical shape information. We introduce a novel segmentation algorithm that uses Deep Conditional Shape models (DCSMs) as a core component. Using deep implicit shape representations, the algorithm learns a modality-agnostic shape model that can generate the signed distance functions for any anatomy of interest. To fit the generated shape to the image, the shape model is conditioned on anatomic landmarks that can be automatically detected or provided by the user. Finally, we add a modality-dependent, lightweight refinement network to capture any fine details not represented by the implicit function. The proposed DCSM framework is evaluated on the problem of cardiac left ventricle (LV) segmentation from multiple 3D modalities (contrast-enhanced CT, non-contrasted CT, 3D echocardiography-3DE). We demonstrate that the automatic DCSM outperforms the baseline for non-contrasted CT without the local refinement, and with the refinement for contrasted CT and 3DE, especially with significant improvement in the Hausdorff distance. The semi-automatic DCSM with user-input landmarks, while only trained on contrasted CT, achieves greater than 92% Dice for all modalities. Both automatic DCSM with refinement and semi-automatic DCSM achieve equivalent or better performance compared to inter-user variability for these modalities.

Abstract:Introduction Cardiac computed tomography (CCT) images provide vast spatial and temporal information. Segmentation of the structure of interest is necessary to provide quantitative information. However, manual segmentation of cardiac substructures is highly time-consuming. Objective This study aims to develop a robust deep-learning segmentation model for cardiac substructure segmentation in 3D CCT images. Method We initially developed a 3D U-Net architecture, incorporating extensive augmentation, for the segmentation of the Left Ventricle Myocardium (LVM), the blood cavities of the Left and Right Ventricles (LV, RV), and the Left and Right Atria (LA, RA). This model utilized CT images from 20 patients who had available manual segmentation. Subsequently, we applied this initial model to datasets from two centers, including different generations of scanners. We then visually inspected the segmentation, selected the highest-quality segmentations, and modify it if necessary, and excluded the low-quality data. We continued fine-tuning the model with the new dataset until we reached a count of 456 patients. As there were multiple images from one patient during this model's development, we generated datasets to develop a new model using the entire new dataset to avoid any data leakage in the reported metrics. The model was developed using data from Center 1 (5-fold cross-validation) and then evaluated on an external test set from Center 2 (126 patients) and also 4D images from Center 1. Various metrics were calculated to assess the model's performance. Results In the internal test set, utilizing 5-fold CV, the model demonstrated high segmentation accuracy, achieving an overall Dice coefficient of 0.98, with individual scores of 0.96 for LVM, 0.98 for LV, 0.98 for RV, 0.99 for RA, and 0.98 for LA. We assessed the automatic quantification capability of the segmentation model from 256 patients, resulting in an R2 value greater than 0.92 for both LV end-diastolic and systolic volumes. In contrast, the model maintained robust performance on the external dataset, achieving an overall Dice coefficient of 0.96, with individual scores of 0.95 for LVM, 0.98 for LV, 0.97 for RV, 0.98 for RA, and 0.94 for LA. Conclusion We provided a large cohort of ground truth segmentations for different cardiac CT substructures using a human-in-the-loop strategy. Subsequently, we developed a deep learning model for highly accurate automated segmentation of these substructures. This model allows for fast and fully automatic segmentation and quantification of 3D and 4D cardiac CT images, from which various quantitative metrics can be extracted from different cardiac substructures.

Automated biventricular quantification in patients with repaired tetralogy of Fallot using a 3D deep learning segmentation model

Abstract:Background: Deep learning is the state-of-the-art approach for automated segmentation of the left ventricle (LV) and right ventricle (RV) in cardiac magnetic resonance (CMR) images. However, these models have been mostly trained and validated using CMR datasets of structurally normal hearts or cases with acquired cardiac disease, and are therefore not well-suited to handle cases with congenital cardiac disease such as tetralogy of Fallot (TOF). We aimed to develop and validate a dedicated model with improved performance for LV and RV cavity and myocardium quantification in patients with repaired TOF. Methods: We trained a 3D convolutional neural network (CNN) with 5-fold cross-validation using manually delineated end-diastolic (ED) and end-systolic (ES) short-axis image stacks obtained from either a public dataset containing patients with no or acquired cardiac pathology (n=100), an institutional dataset of TOF patients (n=96), or both datasets mixed. Our method allows for missing labels in the training images to accommodate for different ED and ES phases for LV and RV as is commonly the case in TOF. The best performing model was applied to all frames of a separate test set of TOF cases (n=36) and ED and ES phases were automatically determined for LV and RV separately. The model was evaluated against the performance of a commercial software (suiteHEART®, NeoSoft, Pewaukee, Wisconsin, US). Results: Training on the mixture of both datasets yielded the best agreement with the manual ground truth for the TOF cases, achieving a median DICE similarity coefficient of (93.8%, 89.8%) for LV cavity and of (92.9%, 90.9%) for RV cavity at (ED, ES) respectively, and of 80.9% and 61.8% for LV and RV myocardium at ED. The offset in automated ED and ES frame selection was 0.56 and 0.89 frames on average for LV and RV respectively. No statistically significant differences were found between our model and the commercial software for LV quantification (two-sided Wilcoxon signed rank test, p<5%), while RV quantification was significantly improved with our model achieving a mean absolute error of 12ml for RV cavity compared to 36ml for the commercial software. Conclusion: We developed and validated a fully automatic segmentation and quantification approach for LV and RV, including RV mass, in patients with repaired TOF. Compared to a commercial software, our approach is superior for RV quantification indicating its potential in clinical practice.
DMCVR: Morphology-Guided Diffusion Model for 3D Cardiac Volume Reconstruction

Abstract:Accurate 3D cardiac reconstruction from cine magnetic resonance imaging (cMRI) is crucial for improved cardiovascular disease diagnosis and understanding of the heart's motion. However, current cardiac MRI-based reconstruction technology used in clinical settings is 2D with limited through-plane resolution, resulting in low-quality reconstructed cardiac volumes. To better reconstruct 3D cardiac volumes from sparse 2D image stacks, we propose a morphology-guided diffusion model for 3D cardiac volume reconstruction, DMCVR, that synthesizes high-resolution 2D images and corresponding 3D reconstructed volumes. Our method outperforms previous approaches by conditioning the cardiac morphology on the generative model, eliminating the time-consuming iterative optimization process of the latent code, and improving generation quality. The learned latent spaces provide global semantics, local cardiac morphology and details of each 2D cMRI slice with highly interpretable value to reconstruct 3D cardiac shape. Our experiments show that DMCVR is highly effective in several aspects, such as 2D generation and 3D reconstruction performance. With DMCVR, we can produce high-resolution 3D cardiac MRI reconstructions, surpassing current techniques. Our proposed framework has great potential for improving the accuracy of cardiac disease diagnosis and treatment planning. 4D Myocardium Reconstruction with Decoupled Motion and Shape Model

Abstract:Estimating the shape and motion state of the myocardium is essential in diagnosing cardiovascular diseases.However, cine magnetic resonance (CMR) imaging is dominated by 2D slices, whose large slice spacing challenges inter-slice shape reconstruction and motion <a class="link-external link-http" href="http://acquisition.To" rel="external noopener nofollow">this http URL</a> address this problem, we propose a 4D reconstruction method that decouples motion and shape, which can predict the inter-/intra- shape and motion estimation from a given sparse point cloud sequence obtained from limited slices. Our framework comprises a neural motion model and an end-diastolic (ED) shape model. The implicit ED shape model can learn a continuous boundary and encourage the motion model to predict without the supervision of ground truth deformation, and the motion model enables canonical input of the shape model by deforming any point from any phase to the ED phase. Additionally, the constructed ED-space enables pre-training of the shape model, thereby guiding the motion model and addressing the issue of data scarcity. We propose the first 4D myocardial dataset as we know and verify our method on the proposed, public, and cross-modal datasets, showing superior reconstruction performance and enabling various clinical applications.

Fully-automatic left ventricular segmentation from long-axis cardiac cine MR scans

Abstract:With an increasing number of large-scale population-based cardiac magnetic resonance (CMR) imaging studies being conducted nowadays, there comes the mammoth task of image annotation and image analysis. Such population-based studies would greatly benefit from automated pipelines, with an efficient CMR image analysis workflow. The purpose of this work is to investigate the feasibility of using a fully-automatic pipeline to segment the left ventricular endocardium and epicardium simultaneously on two orthogonal (vertical and horizontal) long-axis cardiac cine MRI scans. The pipeline is based on a multi-atlas-based segmentation approach and a spatio-temporal registration approach. The performance of the method was assessed by: (i) comparing the automatic segmentations to those obtained manually at both the end-diastolic and end-systolic phase, (ii) comparing the automatically obtained clinical parameters, including end-diastolic volume, end-systolic volume, stroke volume and ejection fraction, with those defined manually and (iii) by the accuracy of classifying subjects to the appropriate risk category based on the estimated ejection fraction. Automatic segmentation of the left ventricular endocardium was achieved with a Dice similarity coefficient (DSC) of 0.93 on the end-diastolic phase for both the vertical and horizontal long-axis scan; on the end-systolic phase the DSC was 0.88 and 0.85, respectively. For the epicardium, a DSC of 0.94 and 0.95 was obtained on the end-diastolic vertical and horizontal long-axis scans; on the end-systolic phase the DSC was 0.90 and 0.88, respectively. With respect to the clinical volumetric parameters, Pearson correlation coefficient (R) of 0.97 was obtained for the end-diastolic volume, 0.95 for end-systolic volume, 0.87 for stroke volume and 0.84 for ejection fraction. Risk category classification based on ejection fraction showed that 80% of the subjects were assigned to the correct risk category and only one subject (< 1%) was more than one risk category off. We conclude that the proposed automatic pipeline presents a viable and cost-effective alternative for manual annotation.

Abstract:Purpose To develop a reconstruction method for highly accelerated cardiac cine MRI with high spatiotemporal resolution and low temporal blurring, and to demonstrate accurate estimation of ventricular volumes and myocardial strain in healthy subjects and in patients. Methods The proposed method, called CineVN, employs a spatiotemporal Variational Network combined with conjugate gradient descent for optimized data consistency and improved image quality. The method is first evaluated on retrospectively undersampled cine MRI data in terms of image quality. Then, prospectively accelerated data are acquired in 18 healthy subjects both segmented over two heartbeats per slice as well as in real time with 1.6 mm isotropic resolution. Ventricular volumes and strain parameters are computed and compared to a compressed sensing reconstruction and to a conventional reference cine MRI acquisition. Lastly, the method is demonstrated in 46 patients and ventricular volumes and strain parameters are evaluated. Results CineVN outperformed compressed sensing in image quality metrics on retrospectively undersampled data. Functional parameters and myocardial strain were the most accurate for CineVN compared to two state‐of‐the‐art compressed sensing methods. Conclusion Deep learning‐based reconstruction using our proposed method enables accurate evaluation of cardiac function in real‐time cine MRI with high spatiotemporal resolution. This has the potential to improve cardiac imaging particularly for patients with arrhythmia or impaired breath‐hold capability.

Abstract:Segmentation of anatomical structures on 2D images of cardiac exams is necessary for performing 3D volumetric analysis, enabling the computation of parameters for diagnosing cardiovascular disease. In this work, we present robust algorithms to automatically segment cardiac imaging data and generate a volumetric anatomical reconstruction of a patient-specific heart model by propagating active contour output within a patient stack through a self-supervised learning model. Contour initializations are automatically generated, then output segmentations on sparse image slices are transferred and merged across a stack of images within the same heart data set during the segmentation process. We demonstrate whole-heart segmentation and compare the results with ground truth manual annotations. Additionally, we provide a framework to represent segmented heart data in the form of implicit surfaces, allowing interpolation operations to generate intermediary models of heart sections and volumes throughout the cardiac cycle and to estimate ejection fraction.
