Abstract:Paper packaging made with recycled paperboard is used to pack various consumer goods that can include amongst others, electronics, toys, food, cosmetics, and stationery. Chemical profiling of the various paper recycling grades used in the manufacture of recycled paperboard was undertaken to investigate possible sources of contaminants and their propagation in the paper recycling chain. Pre-consumer, retail and post-consumer paper-based materials were collected at papermills, corrugators, grocery stores, household waste, solid waste disposal sites and recycling facilities. In the GC-MS analysis, phthalates, long-chain aliphatic compounds, and fatty acids were the most commonly detected compounds whilst phthalates and bisphenols featured most prevalently in the LC-MS analysis. The factors that were identified as likely contributors to the detection of the different chemical compounds included the presence of wood derivatives, the use of certain chemical additives during manufacturing, and exposure of paper to contaminants from consumers, other goods and the environment. Waste mingling, recovery, sorting and reprocessing into recycled paper were also shown to influence the chemical profile of paper materials. Sparse partial least squares-discriminate analysis indicated that newspaper and office paper had unique chemical constituents, whilst cartons were shown to have higher variability. By looking at key stages of paper recycling, this study showed that the possible persistence and transformation of chemical compounds in additives must be evaluated when considering the recyclability of paper-based materials. Further, it highlighted that different separation approaches may be required to reduce contaminant exposure opportunities in post-consumer paper materials.

Abstract:Abstract Globally, per- and polyfluoroalkyl substances (PFAS)–related research on paper products has focused on food packaging with less consideration on the presence of PFAS at different stages of the paper recycling chain. This study analysed the prevalence of PFAS in paper grades used for the manufacture of recycled paperboard. The presence of PFAS was attributed to the use of PFAS-containing additives, consumer usage, exposure to packed goods as well as contamination during mingling, sorting, collection, and recovery of paper recycling material. Q Orbitrap mass spectrometry was used to analyse the paper samples after accelerated solvent extraction and solid phase extraction. The distribution and possible propagation of 22 PFAS were determined in pre-consumer, retail and post-consumer paper products. Post-consumer samples had the highest combined average concentration (ΣPFAS) at 213 ng/g, while the ΣPFAS in retail (159 ng/g) and pre-consumer samples (121 ng/g) was detected at lower concentrations. This study showed that waste collection and recycling protocols may influence PFAS propagation and that measures must be developed to minimise and possibly eliminate exposure opportunities.

Abstract:Non-targeted analysis and suspect screening of per- and polyfluoroalkyl substances (PFAS) in various matrices have gained traction with advancements in accurate mass analytical instruments. This study employed ultra-high performance liquid chromatography coupled to quadrupole orbitrap high-resolution mass spectrometry for PFAS suspect screening of paper grades used in the paper recycling chain. The samples were prepared using two extraction techniques; selective accelerated solvent extraction with weak anionic exchange solid-phase extraction and non-selective ultrasonic-assisted extraction. A suspect screening protocol was established to tentatively identify suspected PFAS against spectral databases using a systematic approach of peak filtering and study-specific thresholds for reporting, linked to a confidence level. The possible prevalence of previously unreported PFAS in several paper materials across the various collection sites in the paper recycling chain was inferred by the common detection of short-chain polyfluoroalkyl ketones and diketones in the paper recycling chain. The suspect screening tentatively identified 41 unique PFAS, with 3 common to both pre-treatment techniques. The detection of unique PFAS by the two sample pre-treatment techniques highlighted the significance of both selective and non-selective extraction in PFAS screening endeavours. Further, it showed the importance of understanding the acquisition mechanisms employed in mass spectrometry where data-dependent acquisition triggered fragmentation in certain identified compounds, and not in others. The tentatively identified PFAS indicated that there were several previously unreported PFAS in the paper recycling chain and that additional studies were required to investigate their abundance, possible persistence, bioaccumulation and toxicity, in relation to their functional groups and carbon chains.

Abstract:Purpose We analyzed and compared the distributions of 13 target pharmaceuticals in different water samples from the Hangzhou metropolitan area and Linan County, Southeast China. Methods Sampling was conducted in five hospitals, two wastewater treatment plants (WWTPs), and Qiantang River. Samples were concentrated by solid-phase extraction and PPCP concentrations were determined by UPLC-MS/MS. Results and discussion Trimethoprim, erythromycin A dihydrate, norfloxacin, ofloxacin, diclofenac sodium, and atenolol were the most frequently detected pharmaceuticals in hospital effluents. Most of the pharmaceutical concentrations in hospital effluents were higher than those in the WWTP influents. Although both WWTPs adopt the anaerobic–aerobic–anoxic treatment process, the removal rates for pharmaceuticals, such as trimethoprim and diclofenac sodium, were completely different. Meanwhile, erythromycin A dihydrate, ofloxacin, penicillin-G, cephalexin, cefazolin, ibuprofen, and diclofenac sodium were detected in Qiantang River. Conclusions These results indicate that hospitals are more concentrated sources of pharmaceuticals than WWTPs, and the WWTPs are not the only route of entry of pharmaceuticals into aquatic environments in these two regions.
Temporal Analysis of Pharmaceuticals as Emerging Contaminants in Surface Water and Wastewater Samples: A Case Study

Abstract:Pharmaceuticals in the environment are a global concern, with studies in all continents highlighting their widespread occurrence and potential ecological impacts, revealing their presence, fate, and associated risks in aquatic ecosystems. Despite typically occurring at low concentrations (ranging from ng/L to µg/L), advancements in analytical methods and more sensitive equipment have enabled the detection of a higher number of pharmaceuticals. In this study, surface and wastewater samples were extracted using solid phase extraction and analyzed using ultra-high-performance liquid chromatography with tandem mass spectrometry. Among the therapeutic classes investigated, nonsteroidal anti-inflammatory drugs/analgesics, antibiotics, and psychiatric drugs showed a higher number of detected pharmaceuticals. Concentrations ranged from below method detection limit (<MDL) to 3.20 µg/L (caffeine) and <MDL to 639 µg/L (hydroxyibuprofen) in 2018, and from <MDL to 0.848 µg/L (diclofenac) and <MDL to 53.0 µg/L (caffeine) in 2019 for river water and wastewater samples. Temporal analysis showed an increase in the sum of pharmaceutical concentrations over the study years, highlighting the importance of monitoring pharmaceuticals in the environment and their potential accumulation over time.

Abstract:Pharmaceuticals are emerging contaminants in the aquatic environments. Their presence poses toxicological effects in humans and animals even at trace concentrations. This study investigated the presence of antibiotics, anti-epilepsy and anti-inflammatory drugs in river water of selected rivers in the Eastern Cape Province in South Africa. Enzyme-linked immunosorbent assay was used for screening of sulfamethoxazole and fluoroquinolones antibiotics. The samples were collected in upper-stream, middle-stream and lower-stream regions of the rivers and effluent of selected wastewater treatment plants. Pre-concentration of the samples was conducted using lyophilisation and extraction was conducted using solid phase extraction (SPE) on Waters Oasis hydrophilic-lipophilic-balanced cartridge. The percentage recovery after sample clean-up on SPE was 103% ± 6.9%. This was followed by ultra-performance liquid chromatography electrospray ionization tandem mass spectrometry. The detected analytes were sulfamethoxazole, erythromycin, clarithromycin and carbamazepine. Carbamazepine and erythromycin were detected in high concentrations ranging from 81.8 to 36,576.2 ng/L and 11.2 to 11,800 ng/L respectively, while clarithromycin and sulfamethoxazole were detected at moderate concentrations ranging from 4.8 to 3280.4 ng/L and 6.6 to 6968 ng/L, respectively. High concentrations of pharmaceuticals were detected on the lower-stream sites as compared to upper-stream sites.

Abstract:The carbon footprint of pharmaceuticals through manufacturing, distribution, the incineration of unwanted pharmaceuticals as well as the packaging of pharmaceutical waste is an emerging and enormous challenge. Pharmaceuticals are major contributors to water pollution in aquatic environments that include surface water and groundwater. These pollutants arise not only from waste products but also from pharmaceutical products that have not been properly disposed of. The continuous exposure to unspecified sub-therapeutic doses of antibiotics presents risks to humans and other animals. Due to their extensive use and incomplete elimination, antibiotics have been detected in various environmental waters. The persistence of antibiotics in the environment and chronic exposure of organisms to these chemical stressors has also proven to have ecotoxicological effects. The prevailing emergence of antimicrobial resistance amongst bacteria is an area of primary concern, especially with regard to the release of antibiotics into the environment. Resistance is the acquired ability of bacterial populations to render an antibiotic ineffective as a result of a change in bacterial DNA which occurs when bacteria are subjected to an antibiotic concentration that will not kill them. A sub-lethal concentration possibly exerts a selective pressure that can result in the development of antimicrobial resistance in bacteria. It is clear that there is a need for extensive research to improve regulations and guidance on pharmaceutical waste management, pharmaceutical take-back programmes and consumer awareness.

Abstract:Exposure of active pharmaceutical compounds (APCs) to the environment during human use is of potential importance in the emergence of drug resistance, changing soil microbiota and their residual effect on living organisms. Thus, this study aimed to assess the extent of exposure of APCs in the hydrologic cycle in and around New Delhi. This study analyzed the presence of 28 drugs from different classes in the surface water (river Yamuna) and aquifers collected from 48 places in Delhi (within the radius of 40 km). The collected water samples were quantified for APCs content using LC-MS/MS. This study revealed that aquifers are extensively affected in most areas based on the accumulation of APCs in water resources to the levels > 0.01 μg/L. Interestingly, a geographical plot of total APCs studied indicated clustering in aquifers with such high levels closer to an unscientific landfill. This 30-year-old un-segregated landfill is found to drain leachate into surface water that had high APCs. This study further revealed that apart from therapeutic usage, the main source of ecological exposure could be due to the disposal of unused and expired pharmaceutical compounds into landfills. For the first time, this study revealed the existence of antimicrobial agents and other APCs in the aquifers of Delhi with levels > 0.1 μg/L, which is a matter of serious concern in terms of multi-drug resistance and other environmental perils. This study warrants the enforcement of regulations for the disposal of unused/expired APCs in high-density population areas.

Abstract:ABSTRACT The occurrence of pharmaceuticals in various wastewater treatment plants (WWTPs) and their discharge into the surface water are existing global challenges. However, such challenges are more detrimental in developing countries due to the aging infrastructure and its vandalism influenced by poverty, resulting in the illegal breakdown of the WWTPs. This study investigated the presence of efavirenz, ibuprofen, naproxen, sulfamethoxazole, and trimethoprim in WWTPs. Ultrasound-assisted extraction (UAE) followed by solid-phase extraction (SPE) was used to extract these pharmaceuticals in sludge samples, with wastewater extracted with only the latter. This was followed with analysis using a high-performance liquid chromatography-photo-diode array detection system. Recoveries found after spiking the samples with analytes at different concentrations ranged from 56 to 117%. An antibiotic, sulfamethoxazole, was among the prominent drugs in untreated wastewater with its concentration reaching 77 ug L-1. All the analytes were detected in sludge samples, with naproxen having the highest concentration of 13.35 ng g-1. The pharmaceutical with the lowest removal efficiency (2-12%) in WWTPs was efavirenz, while other drugs were fairly removed from wastewater. Overall, the findings of this study indicate the dysfunctionality of selected WWTPs in the Eastern Cape province of South Africa due to the release of high amounts of pharmaceuticals into the surface water which can be detrimental to humans, animals, and aquatic life.

Abstract:In this work, environmental occurrence and risk assessment of pharmaceuticals, personal care products (PPCPs), and stimulants are presented. A quantitative technique is described for ultrasonic-assisted solid-phase extraction (SPE) followed by GC-MS after derivatization of PPCPs; propylparaben, triclosan, carbamazepine, chloramphenicol, and stimulant caffeine. Ultrasonic-assisted extraction together with centrifugation were used to extract sediment samples collected from the Mgeni and Msunduzi rivers. An SPE procedure was used for cleanup and to concentrate selected compounds from diluted aqueous extracts. The final extracts were derivatized and analyzed with GC-MS in selected ion monitoring (SIM) mode. The recoveries of the analytes ranged from 66 to 108%. The method detection limits were (0.08–1.82 ng g −1 for solid and 0.08–10 μg L −1 for liquid) and quantification limits (0.42–5.51 ng g −1 for solid and 0.25–25 μg L −1 for liquid). The optimized method was applied in the evaluation of two rivers over 3 months in KwaZulu-Natal, South Africa. All targeted compounds were present in the environment at concentration levels between not detected to 174 ng g −1 and not detected to 30 μg L −1 for solids and aqueous environmental samples, respectively. A comparison of predicted no environmental effect concentration (PNECs) with measured environmental concentration (MECs) showed that these PPCPs present a high ecological risk to the receiving environment (agricultural lands and households). Our work is close to reality because we used MECs as opposed to using predicted environmental concentration (PECs) values, which are normally calculated from consumption, production of compound per year, and various estimated factors.
Diagnosing Down-the-Drain Disposal of Unused Pharmaceuticals at a River Catchment Level: Unrecognized Sources of Environmental Contamination That Require Nontechnological Solutions

Abstract:Down-the-drain disposal of pharmaceuticals remains an overlooked and unrecognized source of environmental contamination that requires nontechnological "at-source" solutions. Monitoring of 31 pharmaceuticals over 7 days in five wastewater treatment plants (WWTPs) serving five cities in South-West UK revealed down-the-drain codisposal of six pharmaceuticals to three WWTPs (carbamazepine and propranolol in city A, sildenafil in city B, and diltiazem, capecitabine, and sertraline in city D), with a one-off record codisposal of estimated 253 pills = 40 g of carbamazepine and estimated 96 pills = 4 g of propranolol in city A accounting for their 10- and 3-fold respective increases in wastewater daily loads. Direct disposal of pharmaceuticals was found to affect the efficiency of wastewater treatment with much higher pharmaceutical removal (decrease in daily load) during "down-the-drain disposal" days. This is due to lack of conjugated glucuronide metabolites that are cleaved during "consumption-only" days, with the release of a parent pharmaceutical counterbalancing its removal. Higher removal of pharmaceuticals during down-the-drain disposal days reduced pharmaceutical loads reaching receiving environment, albeit with significant levels remaining. The estimated daily loads in receiving water downstream from a discharge point accounted for 13.8 ± 3.4 and 2.1 ± 0.2 g day-1 of carbamazepine and propranolol, respectively, during consumption-only days and peaked at 20.9 g day-1 (carbamazepine) and 4.6 g day-1 (propranolol) during down-the-drain disposal days. Actions are needed to reduce down-the-drain disposal of pharmaceuticals. Our recent work indicated that down-the-drain disposal of pharmaceuticals doubled since the last study in 2005, which may be due to the lack of information and messaging that informs people to dispose of unused medicines at pharmacies. Media campaigns that inform the public of how to safely dispose of medicines are key to improving rates of return and reducing pharmaceutical waste in the environment. The environment is a key motivator for returning unused medicines to a pharmacy and so messaging should highlight environmental risks associated with improper disposal.

Abstract:The occurrence of 15 pharmaceuticals and personal care products in the influent and effluent from the wastewater treatment plant (WWTP) and its receiving water in Beijing, China were determined. Results from the present study confirmed that caffeine, N , N -diethyl- m -toluamide and chloramphenicol were removed at a high rate (>70 % efficiency). In contrast, removal efficiency of the other 12 compounds was quite poor (ranged from −40 % to 58 %). Some compounds in the receiving river were present at higher concentrations compared to those in the WWTP effluent, indicating that sources other than treated effluents are present. The risk to the aquatic environment was estimated by a ratio of measured environmental concentration and predicted no-effect concentration. For those compounds found in the effluent and surface water, mefenamic acid, trimethoprim and gemfibrozil may pose a medium risk to aquatic environment.

Abstract:The application of biosolids, manure, and slurry onto agricultural soils and the growing use of treated wastewater in agriculture result in the introduction of human and veterinary pharmaceuticals to the environment. Once in the soil environment, pharmaceuticals may be taken up by crops, resulting in consequent human exposure to pharmaceutical residues. The potential side effects of pharmaceuticals administered in human medicine are widely documented; however, far less is known regarding the risks that arise from incidental dietary exposure. The aim of this study was to evaluate human exposure to pharmaceutical residues in crops and assess the associated risk to health for a range of pharmaceuticals frequently detected in soils. Estimated concentrations of carbamazepine, oxytetracycline, sulfamethoxazole, trimethoprim, and tetracycline in soil were used in conjunction with plant uptake and crop consumption data to estimate daily exposures to each compound. Exposure concentrations were compared to Acceptable Daily Intakes (ADIs) to determine the level of risk. Generally, exposure concentrations were lower than ADIs. The exceptions were carbamazepine, and trimethoprim and sulfamethoxazole under conservative, worst-case scenarios, where a potential risk to human health was predicted. Future research therefore needs to prioritize investigation into the health effects following exposure to these compounds from consumption of contaminated crops.

Oncolytic Viruses in Cancer Treatment: A Review

Abstract:Importance Oncolytic viruses (OVs) are emerging as important agents in cancer treatment. Oncolytic viruses offer the attractive therapeutic combination of tumor-specific cell lysis together with immune stimulation, therefore acting as potential in situ tumor vaccines. Moreover, OVs can be engineered for optimization of tumor selectivity and enhanced immune stimulation and can be readily combined with other agents. The effectiveness of OVs has been demonstrated in many preclinical studies and recently in humans, with US Food and Drug Administration approval of the oncolytic herpesvirus talimogene laherparepvec in advanced melanoma, a major breakthrough for the field. Thus, the OV approach to cancer therapy is becoming more interesting for scientists, clinicians, and the public. The main purpose of this review is to give a basic overview of OVs in clinical development and provide a description of the current status of clinical trials. Observations In 2016 approximately 40 clinical trials are recruiting patients, using a range of OVs in multiple cancer types. There are also many more trials in the planning stages. Therefore, we are now in the most active period of clinical OV studies in the history of the field. There are several OVs currently being tested with many additional engineered derivatives. In OV clinical trials, there are a number of specific areas that should be considered, including viral pharmacokinetics and pharmacodynamics, potential toxic effects, and monitoring of the patients’ immune status. Clinical development of OVs is increasingly focused on their immune stimulatory properties, which may work synergistically with immune checkpoint inhibitors and other strategies in the treatment of human cancer. Conclusions and Relevance Oncolytic viruses are an active area of clinical research. The ability of these agents to harness antitumor immunity appears to be key for their success. Combinatorial studies with immune checkpoint blockade have started and the results are awaited with great interest.

Abstract:Pharmaceuticals are released to the environment after human and animal consumption, which partly comes from accumulation in sewage sludge during wastewater treatment. This paper developed a method for the determination of N,N-diethyl-meta-toluamide (DEET) and 14 pharmaceuticals belonging to multiple therapeutic classes such as antibiotic, anti-inflammatory, antilipidemic, anti-hypertensive, and anticonvulsant in dewatered sludge. Pressurized liquid extraction using methanol/McIlvaine buffer (volume ratio, 1:1), associated with HPLC-MS/MS was proven to be effective for extraction and quantification of pharmaceuticals and DEET in dewatered sludge. Optimization procedures, including extraction method, extraction solvents and clean-up treatment, were carried out by simulating naturally aged samples to provide a more realistic extraction than previous methods, which were often done by spiking the standard solution before extraction without any aging procedures. The internal standard quantification method using six isotopically labelled compounds was applied to improve the poor absolute recoveries induced by severe matrix effects to obtain better relative recoveries (70-120%). Good relative standard deviations (lower than 19.0% for 7 repeats) and the limit of quantification (0.6-19.4 mu g/kg) indicated that the developed method was reliable and sensitive to extract and quantify the studied pharmaceuticals in dewatered sludge. For samples collected from eight sewage treatment plants in Beijing, China, it was found that trimethoprim, caffeine, ketoprofen and ibuprofen were the most prominent contaminants, with the median concentrations reaching 97.6, 179.4, 268.0 and 153.0 mu g/kg, respectively. The level of ketoprofen, ibuprofen, mefenamic acid, cabamazepine and diclofenac were similar to results reported from Spain, whilst the levels of other compounds were similar to those found in Germany, Canada and the US.
Risk factors for mortality in smear-negative tuberculosis suspects: a cohort study in Harare, Zimbabwe.

Abstract:OBJECTIVE To investigate mortality rates and risk factors for death among smear-negative tuberculosis (TB) suspects. DESIGN Cohort study nested within a cluster-randomised trial of community-based active case finding. Smear-negative TB suspects were followed for 12 months, with home tracing where necessary. We calculated mortality rates and used regression analysis to investigate the relationship between clinical characteristics and death. RESULTS Between February 2006 and June 2007, 1195 smear-negative TB suspects were followed for 1136.8 person-years. Human immunodeficiency virus (HIV) prevalence was 63.3%. During follow-up, 139 participants died (11.6%) and mortality rates remained high throughout; 119 (16.5%) HIV-positive individuals and 13 (3.1%) HIV-negative individuals died (HR = 5.8, 95%CI 3.3-10.4, P < 0.001). Advanced immunosuppression was the main risk factor for death among HIV-positive participants, with CD4 count < 50 cells/μ l associated with a 13-fold increased risk of death. Antiretroviral treatment (ART) was initiated by only 106 (14.7%), with long delays in accessing care. CONCLUSION HIV-positive smear-negative TB suspects are at high and sustained risk of death. Current guidelines for the management of HIV-infected TB suspects are limited, and this study adds to evidence that specific policies are required to promote earlier HIV and TB diagnosis and reduce delays in ART initiation.

TrAC Trends in Analytical Chemistry

Abstract:Microplastics (MPs) are emerging toxicants which have been detected in varying environments. Despite MPs adverse effects, reports on MPs detection from human clinical samples are only a few. This is due to several reasons such as inefficiency of current MPs detection techniques to detect them from human clinical samples, lack of understanding about the MPs toxicity to human organs and ethical regulations that restricts study with human placental exposure to MPs. This review gives a comprehensive outlook on the major sources MPs sources and routes into human system and their human toxicity mechanisms. Further an in-depth discussion on the significance and limitations of various MPs detection methods is elaborated in the review. Challenges in current research framework for detection of MPs from human clinical samples and the possible future directions in this imperative research domain are also focused in this review.
