Abstract:Many rivers in urbanised catchments in South Africa are polluted by raw sewage and effluent to an extent that their ecological function has been severely impaired. The Hennops and Jukskei Rivers lying in the Hartbeespoort Dam catchment are two of the worst impacted rivers in South Africa and are in need of rehabilitation. Passive sampling (Chemcatcher® with a HLB receiving phase) together with high-resolution tandem mass spectrometry–targeted screening was used to provide high sensitivity and selectivity for the identification of a wide range of emerging pollutants in these urban waters. Over 200 compounds, including pesticides, pharmaceuticals and personal care products, drugs of abuse and their metabolites were identified. Many substances (~ 180) being detected for the first time in surface water in South Africa. General medicines and psychotropic drugs were the two most frequently detected groups in the catchment. These accounted for 49% of the emerging pollutants found. Of the general medicines, antihypertensive agents, beta-blocking and cardiac drugs were the most abundant (28%) classes detected. The Hennops site, downstream of a dysfunctional wastewater treatment plant, was the most polluted with 123 substances detected. From the compounds detected, peak intensity–based prioritisation was used to identify the five most abundant pollutants, being in the order caffeine > lopinavir > sulfamethoxazole > cotinine > trimethoprim. This work provides the largest available high-quality dataset of emerging pollutants detected in South African urban waters. The data generated in this study provides a solid foundation for subsequent work to further characterise (suspect screening) and quantify (target analysis) these substances.

[A case of atypical type of Sturge-Weber syndrome demonstrated reversible change by MRI FLAIR method in ictus and in postictal state].

Abstract:We report a patient of atypical type of Sturge-Weber syndrome who demonstrated a reversible change by MRI FLAIR method in ictus and postictal state. A 5-year-old boy was admitted to our hospital because of severe headache, vomiting and loss of consciousness with his eyes conjugated to left for a few minutes. He had no facial nevus and other abnormal findings in physical examination. CT scan showed two small calcifications in the right occipital lobe. Postcontrast T 1-weighted image of MRI demonstrated a right parieto-occipital leptomeningeal enhancement. We diagnosed this case as an atypical type of Sturge-Weber syndrome. Although, on admission, FLAIR method showed the area of high signal intensity, after anticonvulsant therapy, those abnormal area disappeared. It is presumably detected by FLAIR method slight extravasation of plasma element in the surface of the brain due to regional hyperperfusion in ictus.
Target quantification of azole antifungals and retrospective screening of other emerging pollutants in wastewater effluent using UHPLC -QTOF-MS

Abstract:The information acquired by high resolution quadrupole-time of flight mass spectrometry (QTOF-MS) allows target analysis as well as retrospective screening for the presence of suspect or unknown emerging pollutants which were not included in the target analysis. Targeted quantification of eight azole antifungal drugs in wastewater effluent as well as new and relatively simple retrospective suspect and non-target screening strategy for emerging pollutants using UHPLC-QTOF-MS is described in this work. More than 300 (parent compounds and transformation products) and 150 accurate masses were included in the retrospective suspect and non-target screening, respectively. Tentative identification of suspects and unknowns was based on accurate masses, peak intensity, blank subtraction, isotopic pattern (mSigma value), compound annotation using data bases such as KEGG and CHEBI, and fragmentation pattern interpretation. In the targeted analysis, clotrimazole, fluconazole, itraconazole, ketoconazole and posaconazole were detected in the effluent wastewater sample, fluconazole being with highest average concentration (302.38 ng L-1). The retrospective screening resulted in the detection of 27 compounds that had not been included in the target analysis. The suspect compounds tentatively identified included atazanavir, citalopram, climbazole, bezafibrate estradiol, desmethylvenlafaxine, losartan carboxylic acid and cetirizine, of which citalopram, estradiol and cetirizine were confirmed using a standard. Carbamazepine, atrazine, efavirenz, lopinavir, fexofenadine and 5-methylbenzotriazole were among the compounds detected following the non-targeted screening approach, of which carbamazepine was confirmed using a standard. Given the detection of the target antifungals in the effluent, the findings are a call for a wide assessment of their occurrence in aquatic environments and their role in ecotoxicology as well as in selection of drug resistant fungi. The findings of this work further highlights the practical benefits obtained for the identification of a broader range of emerging pollutants in the environment when retrospective screening is applied to high resolution and high accuracy mass spectrometric data.
Identifying potential surface water sampling sites for emerging chemical pollutants in Gauteng Province, South Africa

Abstract:Emerging chemical pollutants (ECPs) are defined as new chemicals which do not have a regulatory status, but which may have an adverse effect on human health and the environment. The occurrence and concentrations of ECPs in South African water bodies are largely unknown, so monitoring is required in order to determine the potential threat that these ECPs may pose. Relevant surface water sampling sites in the Gauteng Province of South Africa were identified utilising a geographic information system (GIS). The sites were identified by identifying potential sources of ECPs, including hospitals and clinics, sewage treatment plants, and areas with high population densities or areas that were vulnerable from an environmental point of view. Buffers were drawn around these areas to identify the water sources which have the highest probability of containing ECPs. Areas along the Klip River were identified as having a high likelihood of containing both anthropogenic ECPs and agricultural ECPs. Additionally, sections of the Hennops River were likely to contain anthropogenic ECPs, while the Blesbokspruit area had a high likelihood of containing agricultural ECPs.

Abstract:The occurrence of 200 multiclass contaminants of emerging concern (CECs) encompassing 168 medicinal products and transformation products (TPs), 5 artificial sweeteners, 12 industrial chemicals, and 15 other compounds was investigated in influent and effluent wastewater samples collected during 7 consecutive days from 5 wastewater treatment plants (WWTPs) located in Cyprus. The methodology included a generic solid-phase extraction protocol using mixed-bed cartridges followed by Ultra-High Performance Liquid Chromatography coupled with Quadrupole-Time of Flight Mass Spectrometry (UHPLC-QTOF-MS) analysis. A total of 63 CECs were detected at least in one sample, with 52 and 55 out of the 200 compounds detected in influents and effluents, respectively. Ten (10) out of the 24 families of parent compounds and associated TPs were found in the wastewater samples (influent or effluent). 1-H-benzotriazole, carbamazepine, citalopram, lamotrigine, sucralose, tramadol, and venlafaxine (>80 % frequency of appearance in effluents) were assessed with respect to their bioavailability in soil as part of different scenarios of irrigation with reclaimed water following a qualitative approach. A high score of 12 (high probability) was predicted for 2 scenarios, a low score of 3 (rare occasions) for 2 scenarios, while the rest 28 scenarios had scores 5-8 (unlikely or limited possibility) and 9-11 (possibly). Retrospective screening was performed with the use of a target database of 2466 compounds and led to the detection of 158 additional compounds (medicinal products (65), medicinal products TPs (15), illicit drugs (7), illicit drugs TPs (3), industrial chemicals (11), plant protection products (25), plant protection products TPs (10), and various other compounds (22). This work aspires to showcase how the presence of CECs in wastewater could be investigated and assessed at WWTP level, including an expert-based methodology for assessing the soil bioavailability of CECs, with the aim to develop sustainable practices and enhance reclaimed water reuse.

Abstract:Efforts to regulate and monitor emerging contaminants are insufficient because new chemicals are continually brought to market, and many are unregulated and potentially harmful. Domestic wastewater treatment plants are not designed to remove micropollutants and are important sources of emerging contaminants in the aquatic environment. In this study, non-target screening, an unbiased method for analyzing compounds without prior information, was used to identify compounds that may be emitted in wastewater treatment plant effluent and should be monitored. Nine wastewater treatment plants using different treatment methods were studied, and a non-target screening data-processing method was used. The frequencies at which the contaminants were detected and contaminant persistence through the treatment processes were considered, and then the contaminants were prioritized. The predicted no-effect concentration of each prioritized contaminant was used to determine whether further analysis and monitoring of the contaminant was necessary. Quantitative analyses of five compounds (amantadine, atenolol, benzotriazole, diphenhydramine, and sulpiride) were performed using reference standards. Probable molecular formulae and structures were proposed for 17 contaminants, and the risks posed by the contaminants were estimated using predicted no-effect concentrations. The results provide valuable insights into how unregulated micropollutants can be identified and prioritized for monitoring in future studies.

Abstract:Globally, the environmental occurrence of Contaminants of Emerging Concern (CECs) including pharmaceuticals (PhACs), personal care products (PCPs) and modern polar pesticides has raised ecological and human health awareness. However, as the developed world races against time to establish regulatory measures to mitigate their effects, developing nations including Kenya are lagging behind, partly due to unavailability of adequate data. In this work, a multi-residue analysis of 86 CECs was carried out on 198 surface water and 18 effluent samples collected at 24 sites across the River Athi basin area, Kenya, in both dry and rainy seasons. Overall, 57 CECs comprising 31 PhACs (0.4 ng L −1 –142 μg L −1 ), 6 PCPs (0.7–570 ng L −1 ) and 20 pesticides (0.3 ng L −1 –8.3 μg L −1 ) were detected. The maximum loads varied from 217 g day −1 (PCPs) to 46 kg day −1 (PhACs). Individually, carbamazepine, nevirapine, sulfamethoxazole and DEET were the most ubiquitous CECs, with detection frequencies (DF) higher than 80 %. The highest concentrations were observed at river sites that are heavily impacted by informal settlements, highlighting the critical role of slums in urban rivers pollution. At least 8 CECs including acetamiprid, alachlor , atrazine, diuron, nevirapine and paracetamol show potential risk to algae, Daphnia magna and fish, as exemplified by Risk Quotients (RQ) up to 174. Similarly, potential risk of antibiotic resistant bacteria development is evident (RQ up to 64), being driven by metronidazole, sulfamethoxazole and trimethoprim. Ultimately, further studies on the occurrence and distribution of antibiotic resistant bacteria within the basin and among the communities consuming untreated river water for drinking is merited.

Abstract:Purpose of reviewTo review recent progress in the development and evaluation of rapid CD4 cell assays needed to treat HIV-infected individuals in resource-poor countries. Recent findingsField tests indicate that many simplified flow-based assays provide CD4 cell numbers comparable to the current standard, FACSCount, at a lower cost. Most are intended for use at centralized clinics, but some new assays are portable. The manual assays, Dynal T4 Quant and cyto-spheres, are useful when only a few tests are performed daily. Several new formats include a manual rosetting method that uses a unique anti-CD4 cell monoclonal antibody that does not bind to monocytes. Microchip digital imaging and immunochromatography assays are being developed with the goal of being less expensive, more portable and easier than current methods. Reagents that increase the stability of blood samples and clinical reagents in tropical climates are being field tested. SummaryThe availability of antiretroviral medications increases the need for CD4 cell assays to monitor disease progression and the efficacy of therapy in resource-poor countries. Simplified flow cytometry is needed in centralized clinics, but there is a critical need for even less expensive easier methodology that can be used in smaller laboratories and in rural villages.

Abstract:Pharmaceuticals are emerging contaminants in the aquatic environments. Their presence poses toxicological effects in humans and animals even at trace concentrations. This study investigated the presence of antibiotics, anti-epilepsy and anti-inflammatory drugs in river water of selected rivers in the Eastern Cape Province in South Africa. Enzyme-linked immunosorbent assay was used for screening of sulfamethoxazole and fluoroquinolones antibiotics. The samples were collected in upper-stream, middle-stream and lower-stream regions of the rivers and effluent of selected wastewater treatment plants. Pre-concentration of the samples was conducted using lyophilisation and extraction was conducted using solid phase extraction (SPE) on Waters Oasis hydrophilic-lipophilic-balanced cartridge. The percentage recovery after sample clean-up on SPE was 103% ± 6.9%. This was followed by ultra-performance liquid chromatography electrospray ionization tandem mass spectrometry. The detected analytes were sulfamethoxazole, erythromycin, clarithromycin and carbamazepine. Carbamazepine and erythromycin were detected in high concentrations ranging from 81.8 to 36,576.2 ng/L and 11.2 to 11,800 ng/L respectively, while clarithromycin and sulfamethoxazole were detected at moderate concentrations ranging from 4.8 to 3280.4 ng/L and 6.6 to 6968 ng/L, respectively. High concentrations of pharmaceuticals were detected on the lower-stream sites as compared to upper-stream sites.

Abstract:Landfill sites have been regarded as a significant source of chemicals of emerging concern (CECs) in groundwater. However, our understanding about the compositions of CECs in landfill leachate and adjacent groundwater is still very limited. Here we investigated the CECs in landfill leachates and groundwater of Guangzhou in South China by target, suspect and non-target analysis using high-resolution mass spectrometry (HRMS). A variety of CECs (n = 242), including pharmaceuticals (n = 64), pharmaceutical intermediates (n = 18), personal care products (n = 9), food additives (n = 18), industrial chemicals (n = 82, e.g., flame retardants, plasticizers, antioxidants and catalysts), pesticides (n = 26), transformation products (n = 8) and other organic compounds (n = 17) were (tentatively) identified by non-target and suspect screening. 142 CECs were quantitated with target analysis, and among them 37, 24 and 27 CECs were detected respectively in the raw leachate (272–1780 μg/L), treated leachate (0.25–0.81 μg/L) and groundwater (0.10–53.7 μg/L). The CECs in the raw leachates were efficiently removed with the removal efficiencies greater than 88.7%. Acesulfame, bisphenol F and ketoprofen were the most abundant compounds in both treated leachate and groundwater. The CECs in groundwater was found most likely to be originated from the landfill sites. Our results highlight the importance of non-target screening in identifying CECs, and reveal the contamination risk of groundwater by landfill leachate.

Abstract:Emerging and persistent contaminants (EPC) pose a significant challenge to water quality monitoring efforts. Effect-based monitoring (EBM) techniques provide an efficient and systematic approach in water quality monitoring, but they tend to be resource intensive. In this study, we investigated the EPC distribution for various land uses using target analysis (TA) and non-target screening (NTS) and in vitro bioassays, both individually and integrated, in the upper Ping River Catchment, northern Thailand. Our findings of NTS showed that urban areas were the most contaminated of all land use types, although agriculture sites had high unexpected pollution levels. We evaluated the reliability of NTS data by comparing it to TA and observed varying inconsistencies likely due to matrix interferences and isobaric compound interferences. Integrating NTS with in vitro bioassays for a thorough analysis posed challenges, primary due to a scarcity of concentration data for key compounds, and potentially additive or non-additive effects of mixture samples that could not be accounted for. While EBM approaches place emphasis on toxic sites, this study demonstrated the importance of considering non-bioactive sites that contain toxic compounds with antagonistic effects that may go undetected by traditional monitoring approaches. The present work emphasizes the importance of improving NTS workflows and ensuring high-quality EBM analyses in future water quality monitoring programs.

Abstract:The Lake Simcoe watershed in Ontario, Canada is an important recreational area and a recharge zone for groundwater resources. Lake Simcoe is a relatively shallow lotic system that has been impacted by urban development, recreation, industry and agriculture. As part of a watershed management plan, six wastewater treatment plants (WWTPs) located in this catchment basin were selected to measure the inputs of contaminants of emerging concern (CECs) of wastewater origin. These WWTPs were recently upgraded to tertiary treatment for phosphorus removal. Polar organic chemical integrative samplers (POCIS) and semipermeable membrane devices (SPMDs) were used to monitor for hydrophilic and hydrophobic CECs, respectively, in treated and untreated wastewater. The passive samplers were calibrated with performance reference compounds (PRCs) by measuring the loss of deuterated beta blocker drugs spiked into POCIS and the loss of PCB congeners spiked into SPMDs over the course of 14-day deployment periods. From the PRC data, field sampling rates of CECs were determined and applied to estimate time-weighted average (TWA) concentrations and mass loadings in mg/day/1000 members of the population serviced. In treated wastewater, TWA concentrations of an antibiotic, sulfamethoxazole, the prescription drugs, carbamazepine, naproxen and gemfibrozil, and the non-prescription drug, ibuprofen, were estimated to be in the low (<18 ng/L) range. The artificial sweeteners, sucralose and acesulfame, were particularly useful chemical tracers, with estimated TWA concentrations in treated wastewater ranging from 128 to 213 ng/L and 4 to 33 ng/L, respectively. The steroid hormones were detected only rarely in treated wastewater. Triclosan, triclocarban and the synthetic musks, HHCB and AHTN, were removed efficiently (>77 %), possibly because of the tertiary treatment technologies. Therefore, the mass loadings for these personal care products were all <5 mg/day/1000 people. Overall, this study indicates that tertiary treatment technologies designed for phosphorus removal do not entirely remove the target CECs.

Abstract:In a previous study, we identified several novel contaminants of emerging concern (CECs) using non-targeted screening of extracts prepared from passive samplers deployed in wastewater and surface waters in Hamilton Harbour. In the present study, we investigated whether several of these novel compounds are widely distributed in tributaries and coastal waters in other areas of the Great Lakes Basin. Extracts prepared from archived Polar Organic Chemical Integrated Samplers (POCIS) were analyzed for targeted CECs, which included four tire-derived compounds, two prescription pharmaceuticals (i.e., lamotrigine, formycin), a metabolite of a cough suppressant (i.e., dextrorphan) and a component of coal tar (i.e., lepidine). Five of these analytes were widely distributed in the basin. Concentrations in surface waters were estimated using sampling rates (R s ) predicted from physical-chemical properties. The estimated maximum time-weighted average concentrations for three compounds derived from tire-wear, hexamethoxymethylmelamine (HMMM), diphenylguanidine and 6PPD-quinone were 680, 48 and 17 ng/L, respectively. The distribution of these tire-wear compounds and lepidine indicated that they are transported into surface waters from urban runoff. Dextrorphan and lamotrigine were detected at the highest estimated concentrations near discharges of domestic wastewater. This study demonstrates an approach to identify and assess the sources of novel CECs as part of the development of an "Early Warning System" for potential threats to the Laurentian Great Lakes and their tributaries.

Tuberculosis in New South Wales

Abstract:Surveillance of tuberculosis in New South Wales depends on notification of cases by medical practitioners and careful follow‐up by public health nurses dedicated to the control of tuberculosis. Evaluation of tuberculosis surveillance data enables identification of patterns of infection, highlighting areas requiring specific interventions. In 1986, 290 cases of tuberculosis were notified to the Department of Health, New South Wales. The majority of patients (72.8%) were diagnosed as having pulmonary disease. The highest rates of infection were in people from Southeast Asian countries. Whereas 5.2% of cases were identified when the patients entered Australia, a substantial proportion of diagnoses (23.1%) were made in people who had been resident in Australia for 10 or more years. This article highlights the need to continue efforts to improve tuberculosis surveillance and control programmes in New South Wales.
Computerized axial tomography in the evaluation of gynecologic pelvic masses.

Abstract:The Authors retrospectively evaluate the advantages and limitations of CAT in 57 patients presenting suspected gynecologic pelvic masses. CAT showed great accuracy in identifying the presence or absence of the mass (only 3 false negatives and no false positive) whereas it diagnosed the nature of the lesion correctly only in 19 cases. In 41 cases the CAT provided clinically significant information adding to those of the gynecologic examination. Despite some limitations, CAT has proven to be a useful and accurate method to diagnose and evaluate suspected gynecologic masses before the surgical intervention.

Abstract:The unintended release of chemicals to the environment has led to global concern on water quality prompting widespread research on the occurrence of these compounds in water. While increasing information on organic micropollutants (OMPs) in European water resources is available, there is still limited information on the occurrence of OMPs in African water systems. In this study, a multi-residue analysis covering 428 chemicals using liquid chromatography coupled to high resolution mass spectrometry (LC-HRMS) was performed on water samples collected from 48 surface water sites within the Lake Victoria South Basin, Kenya. A total of 75 compounds including pharmaceuticals, personal care products (PPCPs), pesticides, and industrial chemicals were detected and an additional three compounds (nevirapine, lamivudine and adenosine) were identified through suspect screening. Four compounds including diphenhydramine, simazine, triethylphosphate and acetyl-sulfamethoxazole (A-SMX) were detected in >80% of the sites showing their ubiquitous nature in the study area. Individual compound concentrations were detected up to 24 μg L-1. Concentrations above 1 μg L-1 were also reported for triethylcitrate, N-ethyl-o-toluenesulfonamide, hexazinone, nevirapine, adenosine and carbendazim. While crustaceans were potentially the taxon at risk for acute toxicity (toxic unit (TU) up to 2) with diazinon driving this risk, lower but substantial acute risk (TU 0.5) was observed for algae. Chronic risks were observed in 11 sites for algae (TU > 0.02) and in 5 sites for fish (TU > 0.01). A total of 16 compounds were prioritized based on frequency and extent of the exceedance of thresholds for acute and chronic risks to algae, crustaceans and fish and another 7 compounds prioritized by applying lowest Predicted No-Effect Concentrations (PNEC). Based on these indicators, this study provides candidate priority compounds for monitoring, assessment and abatement in western Kenya.
Quantitative Determination and Environmental Risk Assessment of 102 Chemicals of Emerging Concern in Wastewater-Impacted Rivers Using Rapid Direct-Injection Liquid Chromatography—Tandem Mass Spectrometry

Abstract:The rapid source identification and environmental risk assessment (ERA) of hundreds of chemicals of emerging concern (CECs) in river water represent a significant analytical challenge. Herein, a potential solution involving a rapid direct-injection liquid chromatography–tandem mass spectrometry method for the quantitative determination of 102 CECs (151 qualitatively) in river water is presented and applied across six rivers in Germany and Switzerland at high spatial resolution. The method required an injection volume of only 10 μL of filtered sample, with a runtime of 5.5 min including re-equilibration with >10 datapoints per peak per transition (mostly 2 per compound), and 36 stable isotope-labelled standards. Performance was excellent from the low ng/L to μg/L concentration level, with 260 injections possible in any 24 h period. The method was applied in three separate campaigns focusing on the ERA of rivers impacted by wastewater effluent discharges (1 urban area in the Basel city region with 4 rivers, as well as 1 semi-rural and 1 rural area, each focusing on 1 river). Between 25 and 40 compounds were quantified directly in each campaign, and in all cases small tributary rivers showed higher CEC concentrations (e.g., up to ~4000 ng/L in total in the R. Schwarzach, Bavaria, Germany). The source of selected CECs could also be identified and differentiated from other sources at pre- and post- wastewater treatment plant effluent discharge points, as well as the effect of dilution downstream, which occurred over very short distances in all cases. Lastly, ERA for 41 CECs was performed at specific impacted sites, with risk quotients (RQs) at 1 or more sites estimated as high risk (RQ > 10) for 1 pharmaceutical (diclofenac), medium risk (RQ of 1–10) for 3 CECs (carbamazepine, venlafaxine, and sulfamethoxazole), and low risk (RQ = 0.1–1.0) for 7 CECs (i.e., RQ > 0.1 for 11 CECs in total). The application of high-throughput methods like this could enable a better understanding of the risks of CECs, especially in low flow/volume tributary rivers at scale and with high resolution.

Abstract:Pharmaceuticals in aquatic environments pose threats to aquatic organisms because of their continuous release and potential accumulation. Monitoring methods for these contaminants are inadequate, with targeted analyses falling short in assessing water quality's impact on biota. The present study advocates for integrated strategies combining suspect and targeted chemical analyses with molecular biomarker approaches to better understand the risks posed by complex chemical mixtures to nontarget organisms. The research aimed to integrate chemical analysis and transcriptome changes in fathead minnows to prioritize contaminants, assess their effects, and apply this strategy in Wascana Creek, Canada. Analysis revealed higher pharmaceutical concentrations downstream of a wastewater‐treatment plant, with clozapine being the most abundant in fathead minnows, showing notable bioavailability from water and sediment sources. Considering the importance of bioaccumulation factor and biota–sediment accumulation factor in risk assessment, these coefficients were calculated based on field data collected during spring, summer, and fall seasons in 2021. Bioaccumulation was classified as very bioaccumulative with values >5000 L kg–1, suggesting the ability of pharmaceuticals to accumulate in aquatic organisms. The study highlighted the intricate relationship between nutrient availability, water quality, and key pathways affected by pharmaceuticals, personal care products, and rubber components. Prioritization of these chemicals was done through suspect analysis, supported by identifying perturbed pathways (specifically signaling and cellular processes) using transcriptomic analysis in exposed fish. This strategy not only aids in environmental risk assessment but also serves as a practical model for other watersheds, streamlining risk‐assessment processes to identify environmental hazards and work toward reducing risks from contaminants of emerging concern. Abstract:A diversity of contaminants of emerging concern (CECs) are present in wastewater effluent, posing potential threats to receiving waters. It is urgent for a holistic assessment of the occurrence and risk of CECs related to wastewater treatment plants (WWTP) on national and regional scales. A data mining-based risk prioritization method was developed to collect the reported contaminants and their respective concentrations in municipal and industrial WWTPs and their receiving waters across China over the past 20 years. A total of 10,781 chemicals were reported in 8336 publications, of which 1037 contaminants were reported with environmental concentrations. While contaminant categories varied across WWTP types (municipal vs. industrial) and regions, pharmaceuticals and cyclic hydrocarbons were the most studied CECs. Contaminant composition in receiving water was closer to that in municipal than industrial WWTPs. Publications on legacy pesticides and polycyclic aromatic hydrocarbons in WWTP decreased recently compared to the past, while pharmaceuticals and perfluorochemicals have received increasing attention, showing a changing concern over time. Detection frequency, concentration, removal efficiency, and toxicity data were integrated for assessing potential risks and prioritizing CECs on national and regional scales using an environmental health prioritization index (EHPi) approach. Among 666 contaminants in municipal WWTP effluent, trichlorfon and perfluorooctanesulfonic acid were with the highest EHPi scores, while 17ɑ-ethinylestradiol and bisphenol A had the highest EHPi scores among 304 contaminants in industrial WWTPs. The prioritized contaminants varied across regions, suggesting a need for tailoring regional measures of wastewater treatment and control.
