Abstract:The neurophysiological basis of the association between interhemispheric connectivity and speech comprehension processing remains unclear. This prospective study examined regional cerebral blood flow (CBF), homotopic functional connectivity, and neurovascular coupling, and their effects on comprehension performance in post-stroke aphasia. Multimodal imaging data (including data from functional magnetic resonance imaging and arterial spin labeling imaging) of 19 patients with post-stroke aphasia and 22 healthy volunteers were collected. CBF, voxel-mirrored homotopic connectivity (VMHC), CBF-VMHC correlation, and CBF/VMHC ratio maps were calculated. Between-group comparisons were performed to identify neurovascular changes, and correlation analyses were conducted to examine their relationship with the comprehension domain. The correlation between CBF and VMHC of the global gray matter decreased in patients with post-stroke aphasia. The total speech comprehension score was significantly associated with VMHC in the peri-Wernicke area [posterior superior temporal sulcus (pSTS): r = 0.748, p = 0.001; rostroventral area 39: r = 0.641, p = 0.008]. The decreased CBF/VMHC ratio was also mainly associated with the peri-Wernicke temporoparietal areas. Additionally, a negative relationship between the mean CBF/VMHC ratio of the cingulate gyrus subregion and sentence-level comprehension was observed (r = -0.658, p = 0.006). These findings indicate the contribution of peri-Wernicke homotopic functional connectivity to speech comprehension and reveal that abnormal neurovascular coupling of the cingulate gyrus subregion may underly comprehension deficits in patients with post-stroke aphasia.
Auditory perpetual dysfunction in patients with CCI, with implications for therapy

Abstract:Injury to the head, as from impact, can result in damaged function of the auditory receptive (peripheral) or perceptual (central) system, or both. The altered function may be clearly apparent, as in a loss of hearing, or may be exposed only through specially devised tests. At the Loewenstein Rehabilitation Hospital tests were designed or adapted to evaluate processing in the auditory periphery, the brain stem and the cortex. With speech as the the stimulus the tasks explore binaural interaction, cerebral hemisphere dominance and suppression, and cognitive efficiency in understanding degraded speech messages. Other stimuli, such as pure tones and musical patterns are employed to reveal dysfunction on various levels of the system. This report concentrates on tests which require the cerebral hemispheres to listen competitively. Results are given for earphone listening and for two new types of selective listening tasks in the sound field (loudspeaker) mode.
Central Auditory Processing of Temporal and Spectral-Variance Cues in Cochlear Implant Listeners

Abstract:Cochlear implant (CI) listeners have difficulty understanding speech in complex listening environments. This deficit is thought to be largely due to peripheral encoding problems arising from current spread, which results in wide peripheral filters. In normal hearing (NH) listeners, central processing contributes to segregation of speech from competing sounds. We tested the hypothesis that basic central processing abilities are retained in post-lingually deaf CI listeners, but processing is hampered by degraded input from the periphery. In eight CI listeners, we measured auditory nerve compound action potentials to characterize peripheral filters. Then, we measured psychophysical detection thresholds in the presence of multi-electrode maskers placed either inside (peripheral masking) or outside (central masking) the peripheral filter. This was intended to distinguish peripheral from central contributions to signal detection. Introduction of temporal asynchrony between the signal and masker improved signal detection in both peripheral and central masking conditions for all CI listeners. Randomly varying components of the masker created spectral-variance cues, which seemed to benefit only two out of eight CI listeners. Contrastingly, the spectral-variance cues improved signal detection in all five NH listeners who listened to our CI simulation. Together these results indicate that widened peripheral filters significantly hamper central processing of spectral-variance cues but not of temporal cues in post-lingually deaf CI listeners. As indicated by two CI listeners in our study, however, post-lingually deaf CI listeners may retain some central processing abilities similar to NH listeners.

Abstract:In the present paper we report on speech perception in left (n=16) and right (n=10) frontal lobe nonaphasic patients compared with healthy controls (n=26). The patients were inpatients of the University Clinic of Neurology, Innsbruck, Austria, who participated in a prospective neuropsychological study dealing with several aspects of lateralized functioning. The patients were tested with dichotic presentations of consonant-vowel syllables, which allows for specific probing of speech perception in the left and right hemisphere, respectively. The task was to report which syllable they heard best on each trial, emphasizing single answers on each trial. There were 36 dichotic trials with pairs of CV syllables made up of the six stop consonants /p/, /t/, /k/, /b/, /d/, /g/, all paired with the vowel /a/. There were always two different syllables presented on each trial, one in each ear. The results showed significant differences between the left and right lesioned patients in their performance on the dichotic listening test. While the right lesioned patients and the control subjects had a normal and expected right ear advantage, the left lesioned patients performed almost at random with regard to the right and left ear stimulus. The right lesioned patients and the control group showed a right ear advantage of the same magnitude, although the overall performance was somewhat impaired in the right lesioned group compared to the healthy control group. The left lesioned patients showed no ear advantage at all, and particularly their right ear scores were impaired compared to both the control group and the right lesioned patient group. The results are discussed in terms of the role played by the left frontal cortex in speech perception and language asymmetry.

Abstract:Background: Stroke survivors may suffer from a range of hearing impairments that may restrict their participation in postacute rehabilitation programs. Hearing impairment may have a significant impact on listening, linguistic skills, and overall communication of the affected stroke patient. However, no studies sought to systematically characterize auditory function of stroke patients in detail, to establish the different types of hearing impairments in this cohort of patients. Such information would be clinically useful in understanding and addressing the hearing needs of stroke survivors. Purpose: The present study aimed to characterize and classify the hearing impairments, using a detailed audiological assessment test battery, in order to determine the level of clinical need and inform appropriate rehabilitation for this patient population. Research Design: A case–control study. Study Sample: Forty-two recruited stroke patients who were discharged from a stroke unit and 40 control participants matched for age. Data Collection and Analysis: All participants underwent pure-tone audiometry and immittance measurements including acoustic reflex threshold, transient-evoked otoacoustic emissions, auditory-evoked brainstem response, and a central auditory processing assessment battery, performed in a single session. Hearing impairments were classified as peripheral hearing loss (cochlear and neural type), central auditory processing disorder (CAPD), and as a combination of CAPD and peripheral hearing loss. Results: Overall mean hearing thresholds were not significantly different between the control and stroke groups. The most common type of hearing impairment in stroke patients was the combination type, “peripheral and CAPD,” in the 61- to 80-yr-old subgroup (in 55%), and auditory processing deficits in 18- to 60-yr-olds (in 40%), which were both significantly higher than in controls. Conclusions: This is the first study to examine hearing function in detail in stroke patients. Given the importance of hearing for the efficiency of communication, it is essential to identify hearing impairments and differentiate peripheral and central deficits to define an appropriate intervention plan.

Abstract:Background: In central auditory disorders caused by damage of the cerebral hemispheres, there are cortical deafness and auditory agnosia. Although clinical cases of cortical deafness have been reported, little is known about the hearing problems and localized lesions associated with cortical deafness. Aims/objectives: The aims of our research are to elucidate lesion sites associated with cortical deafness and to clarify why patients with cerebral lesions are not aware of any sound at all. Materials and methods: Three patients diagnosed as having total loss of hearing participated in this study. We conducted pure-tone audiometry, speech audiometry, distortion product otoacoustic emission (DPOAE), auditory brainstem response (ABR), and brain magnetic resonance imaging (MRI) to diagnose cortical deafness with aphasia tests of these patients. Results: Our studies showed that waves VI and VII as well as waves I to V have normal peak latencies in ABRs in all three patients. In brain MRI, we found complete damage of proximal parts of bilateral auditory radiations in the three patients. Conclusions: We propose 'subcortical deafness' as a subtype of auditory agnosia.

Abstract:The authors tested functional hemispheric asymmetry through word dichotic listening and lateralized lexical decision tasks in tinnitus patients and controls stimulated by a continuous tinnitus-like noise to test the influence of a tinnitus-like external stimulation. A classic right-ear advantage was shown in the auditory task for all but right-ear tinnitus patients, who performed as equally badly when the stimuli were presented to the right and left ears. Concerning the visual task, all participants demonstrated the expected right visual field advantage for word stimuli. Moreover, those who submitted to external stimulation demonstrated normal asymmetric patterns. These data suggest a specific effect of tinnitus on central processing and provide evidence for a functional reorganization induced by this auditory phantom perception.

Abstract:The disorders of perceived auditory lateralization after unilateral damage of the cerebral hemispheres were investigated by dichotic presentation of pure tones, lateralization being obtained by interaural intensity differences. Observations were made on 107 unilateral brain-damaged patients and 30 normal subjects who were required to localize the perceived position of fused sound images. The images were generated by dichotic stimuli delivered through headphones and the patients were asked to point the perceived position. A clear-cut hemispheric asymmetry was found. Whereas left brain-damaged patients and right brain-damaged patients without visual field defects were not impaired in this task in comparison with controls, the performance of right brain-damaged patients with visual field defects was defective, with a systematic directional error towards the right side, that is, the side ipsilateral to the damaged hemisphere. The defect of auditory lateralization involved both the half-space contralateral to the lesion and the ipsilateral one. Different hypotheses are discussed in order to explain the present results. It is argued that they cannot easily be interpreted in terms of defective processing of sensory information and the proposal is made that lesions of the right hemisphere may produce a distortion of the internal representation of egocentric space to the right, yielding a systematic error of auditory lateralization are discussed.

Abstract:Acute ischemic stroke, characterized by a localized reduction in blood flow to specific areas of the brain, has been shown to affect binaural auditory perception. In a previous study conducted during the acute phase of ischemic stroke, two tasks of binaural hearing were performed: binaural tone-in-noise detection, and lateralization of stimuli with interaural time- or level differences. Various lesion-specific, as well as individual, differences in binaural performance between patients in the acute phase of stroke and a control group were demonstrated. For the current study, we re-invited the same group of patients, whereupon a subgroup repeated the experiments during the subacute and chronic phases of stroke. Similar to the initial study, this subgroup consisted of patients with lesions in different locations, including cortical and subcortical areas. At the group level, the results from the tone-in-noise detection experiment remained consistent across the three measurement phases, as did the number of deviations from normal performance in the lateralization task. However, the performance in the lateralization task exhibited variations over time among individual patients. Some patients demonstrated improvements in their lateralization abilities, indicating recovery, whereas others' lateralization performance deteriorated during the later stages of stroke. Notably, our analyses did not reveal consistent patterns for patients with similar lesion locations. These findings suggest that recovery processes are more individual than the acute effects of stroke on binaural perception. Individual impairments in binaural hearing abilities after the acute phase of ischemic stroke have been demonstrated and should therefore also be targeted in rehabilitation programs.

Abstract:Objective: To analyze auditory verbal memory impairment after isolated stroke lesions in subcerebral structure. Methods: Auditory verbal memory function was examined in 25 normal elderly and 83 patients at 20th day after their admission to hospital for acute cerebrovascular disease with non-aphasiaand solitary lesions as identified by cranial CT or MRI. Results: Test findings suggested patients with stroke of internal capsule-basal ganglia, cerebellum and pons lesion have processing impairments of recent memory and subjective organization which are related to concentration and arousal levels. Conclusion: Stroke of subcortical regions such as lesions of thalamus,internal capsule-basal ganglia,cerebellum and pons may induce memory impairments with unique impaired cognitive coding modalities.

Abstract:Objective To explore into the relationship between the focus of temporal lobe and the performance of aphasia.Methods Two aphasia cases caused by HSE were tested by means of aphasia battery of Chinese(ABC)test and imaging analysis.Results Patient 1,whose focus was mainly at the posterior superior temporal area,presented quite different symptoms from patient 2,whose focus was at the anterior superior temporal area and middle temporal gyrus.The former could not be classified into subtypes,while the latter could be determined as a typical Wernicke's aphasia.Conclusions An aphasic subjected to the damage at the posterior superior temporal area has comparatively minor difficulty in listening comprehension;the damages at the anterior superior temporal area and middle temporal gyrus may lead to serious damage to listening comprehension;different temporal areas play different roles in the formation of language.

Abstract:Hearing is multifaceted and the relative contributions of peripheral and central hearing loss are rarely considered together in the context of dementia. Here, we assessed peripheral (as measured with pure-tone audiometry) and central (as measured with dichotic listening) hearing in 19 patients with typical amnestic Alzheimer's disease (tAD), 10 patients with logopenic variant primary progressive aphasia (lvPPA), 11 patients with nonfluent/agrammatic variant PPA (nfvPPA), 15 patients with semantic variant PPA (svPPA), and 28 healthy age-matched individuals. Participants also underwent neuropsychological assessment and magnetic resonance image scanning, allowing us to use voxel-based morphometry to assess associations between hearing scores and grey matter volume. Dichotic listening was impaired in all patient groups relative to healthy controls. In the combined patient (but not healthy control) cohort, dichotic listening scores were significantly correlated with measures of global cognitive functioning and speech-based neuropsychological tasks. Pure-tone audiometry scores were not significantly elevated in any patient group relative to the healthy control group, and no significant correlations were observed between peripheral hearing and neuropsychological task performance in either the combined patient or healthy control cohorts. Neuroanatomically, dichotic listening performance was associated with grey matter volume in a bilateral fronto-temporo-parietal network over the combined patient cohort, but no correlates were identified for pure-tone audiometry. Our findings highlight the importance of speech parsing mechanisms beyond elementary sound detection in driving cognitive test performance, underline the importance of assessing central hearing alongside peripheral hearing in people with dementia, and further delineate the complex auditory profiles of neurodegenerative dementias.
A Comparison Study on Cognitive Function in Patients with Single Subcortical Lesion Stroke of Four Different Areas

Abstract:OBJECTIVE:Comparing the characteristics of cognitive impairment of patients with single subcortical lesion stroke of four different areas, we are to explore the cognitive function of the thalamus and basal ganglia and this is help for early identification of vascular cognitive impairment (VCI).METHODS:63 patients with single subcortical lesion stroke (including 14 left thalamic stoke group, 17 left basal ganglia stroke group, 15 right thalamic stroke group, 17 right basal ganglia stroke group) and 34 healthy subjects participated in the current study, whose age, sex and education were matched. A comprehensive neuropsychological battery was used for evaluation.RESULTS:Compared to the normal control group, there was an overall decline of cognitive functions in patients with single subcortical lesion stroke in memory, attention/executive function, language, and visuospatial ability (P < 0.05). The scores of the left thalamic stroke group were worse than the other three stroke groups in language (BNT16.6 ± 2.6), auditory verbal learning test-immediate recall (12.8 ± 4.4), auditory verbal learning test-delayed recall (2.4 ± 2.3), listening recognition (19.1 ± 3.1), structure delayed recall (9.1 ± 4.7) and symbol digit modalities test-recall (0.9 ± 1.1) (P < 0.05). However, the left basal ganglia stroke group did better in tests manipulated by the right hand [including Trial making test (part A) score (75 ± 22), Trail making test (part B) score (204 ± 81), Clock drawing test (23.5 ± 4.6), Symbol digit modalities test (24 ± 9)] than other three stroke group, as good as the normal group (P < 0.05).CONCLUSIONS:Single subcortical stroke patients may have general, non-selective cognitive impairment. But, different stroke areas have their own characteristics. The scores of the left thalamic stroke group were worse than the other three stroke groups.

Functional magnetic resonance imaging of auditory cortex in sudden sensorineural hearing loss

Abstract:Objective To characterize pure tone-induced activation in central auditory functional areas in patients with sudden sensorineural hearing loss (SSNHL) using blood oxygen level dependent functional MRI (BOLD-fMRI). Methods A total of 13 subjects with SSNHL and 12 normal hearing volunteers were studied. A 1000 Hz pure tone at 127dB was presented to the right ear or binaurally. Cortical activation was recorded using functional imaging and analyzed using the SPM 2 software. Results 1. Monaural right side stimulation resulted in activation in bilateral superior temporal gyrus,.left occipital lobe, left superior frontal gyrus,.left cerebellum, bilateral hippocampus and paracentral lobule in normal hearing subjects, and in bilateral superior temporal gyrus,.right occipital lobe, left hippocampus and left calacarine fissure in SSNHL patients. 2. Binaural acoustic stimulation induced activation in bilateral middle temporal gyrus, right cingulate gyrus and left corpus callosum in normal subjects and in bilateral middle temporal gyrus,.right superior temporal gyrus, right cerebellum, right inferior frontal gyrus and right precuneus in SSNHL patients. Stronger activation was seen in the left cerebellum, and left corpus callosum in healthy subjects than in SSNHL subjects. Activation in right superior temporal gyrus, right inferior frontal gyrus and right cerebellum was seen only in SSNHL subjects, not in normal individuals. Conclusion There appears to be a directional preponderance change in activation by monaural sound stimulation in patients with SSNHL, indicating reorganization of the auditory cortex. Auditory cortical plasticity may help normalize auditory function in SSNHL patients. Activation of occipital lobes in SSNHL subjects may suggest cross-modal plasticity in these individuals.

Abstract:The dichotic listening task is commonly used in a wide range of neuropsychological issues for studying interhemispheric asymmetry and interhemispheric interaction mechanisms. The article aims to determine the specific vector of auditory speech asymmetry for efferent motor and acoustic mnestic aphasia and its correlation with the dynamics of speech recovery. The paper studied the frequency of the right and left vectors of auditory speech asymmetry in aphasics with different time post-onset and structural characteristics of lesions. Also, the paper analyses the influence of the ear advantage side on the efficiency index of dichotic listening and quantitative indicators of speech. Material. Patients with acoustic mnestic (n = 52) and efferent motor aphasia (N = 58) of moderate and mild severity. Methods. The dichotic listening task: 16 paired series of 4 monosyllabic words in each. Results. Time post-onset, as well as lesion size, does not influence directly auditory speech laterality in aphasics. The frequency of occurrence of the positive and negative signs of the coefficient of the right ear has no pronounced differences between the different lesion sizes and time post-onset in aphasia. The left hemisphere lesion marks the impairment of the reciprocity in hemispheric interaction. The left ear advantage influences positively the dichotic listening efficiency index. Aphasia recovery does not have a direct conjugacy with the side of lesion or ear advantage. At the same time, the left ear advantage in acoustic mnestic aphasia creates a more pronounced “positive shift” in speech scores than in efferent motor aphasia. Conclusion. Abstract:We report the case of a 65-year-old man WDK, who experienced selective loss of timbre perception for keyboard and percussion instruments following a right temporal stroke comprising the anterior superior and medial gyrus and parts of the insular region. Formerly an avid listener to music, the sound of an orchestra appeared to be "flat" to him. WDK and a matched control subject underwent a detailed neuropsychological test battery covering basic auditory function (audiometry and just notable difference for pitch shifts), specific auditory function (recognition of environmental sounds), specific musical functions like discrimination of pitch, interval, contour, rhythm and metre, recognition of familiar melodies, emotional responsiveness, perception of timbre and auditory spectral analysis. While WDK displayed no deficits in primary auditory function, the test battery revealed a selective impairment in the perception of timbre of keyboard and percussion instruments which could be traced to a deficit in discriminating rapid changes of the spectral composition of a tone. Hence, these findings demonstrate the existence of highly specific neural networks underlying isolated aspects of timbre perception in the right anterior temporal lobe.

Abstract:Analogous to the visual system, auditory information has been proposed to be processed in two largely segregated streams: an anteroventral ("what") pathway mainly subserving sound identification and a posterodorsal ("where") stream mainly subserving sound localization. Despite the popularity of this assumption, the degree of separation of spatial and non-spatial auditory information processing in cortex is still under discussion. In the present study, a statistical approach was implemented to investigate potential behavioral dissociations for spatial and non-spatial auditory processing in stroke patients, and voxel-wise lesion analyses were used to uncover their neural correlates. The results generally provided support for anatomically and functionally segregated auditory networks. However, some degree of anatomo-functional overlap between "what" and "where" aspects of processing was found in the superior pars opercularis of right inferior frontal gyrus (Brodmann area 44), suggesting the potential existence of a shared target area of both auditory streams in this region. Moreover, beyond the typically defined posterodorsal stream (i.e., posterior superior temporal gyrus, inferior parietal lobule, and superior frontal sulcus), occipital lesions were found to be associated with sound localization deficits. These results, indicating anatomically and functionally complex cortical networks for spatial and non-spatial auditory processing, are roughly consistent with the dual-pathway model of auditory processing in its original form, but argue for the need to refine and extend this widely accepted hypothesis.
