Abstract:Detecting consciousness in clinically unresponsive patients remains a significant challenge. Existing studies demonstrate that electroencephalography (EEG) can detect brain responses in behaviorally unresponsive patients, indicating potential for consciousness detection. However, most of this evidence is based on chronic patients, and there is a lack of studies focusing on acute coma cases. This study aims to detect signs of residual consciousness in patients with acute coma by using bedside EEG and electromyography (EMG) during an auditory oddball paradigm. We recruited patients with acute brain injury (either traumatic brain injury or cardiac arrest) who were admitted to the intensive care unit within two weeks after injury, with a Glasgow Coma Scale (GCS) score of 8 or below. Auditory stimuli included the patients' own names and other common names (referred to as standard names), spoken by the patients' relatives, delivered under two conditions: passive listening (where patients were instructed that sounds would be played) and active listening (where patients were asked to move hands when heard their own names). Brain and muscle activity were recorded using EEG and EMG during the auditory paradigm. Event-related potentials (ERP) and EMG spectra were analyzed and compared between responses to the subject's own name and other standard names in both passive and active listening conditions. A total of 22 patients were included in the final analysis. Subjects exhibited enhanced ERP responses when exposed to their own names, particularly during the active listening task. Compared to standard names or passive listening, distinct differences in brain network connectivity and increased EMG responses were detected during active listening to their own names. These findings suggest the presence of residual consciousness, offering the potential for assessing consciousness in behaviorally unresponsive patients.

Abstract:When listening to an unknown language, listeners could learn the transitional probability between syllables and group frequently co-occurred syllables into a whole unit. Such statistical learning ability has been demonstrated for both pre-verbal infants and adults, even during passive listening. Here, we investigated whether statistical learning occurred in patients in minimally conscious state (MCS) and patients emerged from the minimally conscious state (EMCS) using electroencephalography (EEG). We presented to participants an isochronous sequence of syllables, which were composed of either 2-word real phrases or 2-word artificial phrases that were defined by the transitional probability between words. An inter-trial phase coherence (ITPC) analysis revealed that the phrase-rate EEG response was weakened in EMCS patients compared with healthy individuals, and was even more severely weakened in MCS patients. Although weak, the phrase-rate response or its harmonics remained statistically significant in MCS patients, suggesting that the statistical learning ability was preserved in MCS patients. The word-rate response was also weakened with a decreased level of consciousness. The harmonics of the word-rate response, however,were more salient in MCS than EMCS patients in the alpha and beta bands. Together with previous studies, the current results suggest that MCS patients retain residual learning ability, which can potentially be harnessed to induce neural plasticity, and that different frequency bands are differentially related to the consciousness level.
Neural Tracking of Sound Rhythms Correlates with Diagnosis, Severity, and Prognosis of Disorders of Consciousness

Abstract:Effective diagnosis and prognosis of patients with disorders of consciousness (DOC) provides a basis for family counseling, decision-making, and the design of rehabilitation programs. However, effective and objective bedside evaluation is a challenging problem. In this study, we explored electroencephalography (EEG) response tracking sound rhythms as potential neural markers for DOC evaluation. We analyzed the responses to natural speech and tones modulated at 2 and 41 Hz. At the population level, patients with positive outcomes (DOC-P) showed higher cortical synchronization to modulated tones at 41 Hz compared with patients with negative outcomes (DOC-N). At the individual level, phase coherence to modulated tones at 41 Hz was significantly correlated with Coma Recovery Scale-Revised (CRS-R) and Glasgow Outcome Scale-Extended (GOS-E) scores. Furthermore, SVM classifiers, trained using phase coherences in higher frequency bands or combination of the low frequency aSSR and speech tracking responses, performed very well in diagnosis and prognosis of DOC. These findings show that EEG response to auditory rhythms is a potential tool for diagnosis, severity, and prognosis of DOC.
Phase Property of Envelope-Tracking EEG Response is Preserved in Patients with Disorders of Consciousness.

Abstract:When listening to speech, the low-frequency cortical response below 10 Hz can track the speech envelope. Previous studies have demonstrated that the phase lag between speech envelope and cortical response can reflect the mechanism by which the envelope-tracking response is generated. Here, we analyze whether the mechanism to generate the envelope-tracking response is modulated by the level of consciousness, by studying how the stimulus-response phase lag is modulated by the disorder of consciousness (DoC). It is observed that DoC patients in general show less reliable neural tracking of speech. Nevertheless, the stimulus-response phase lag changes linearly with frequency between 3.5 and 8 Hz, for DoC patients who show reliable cortical tracking to speech, regardless of the consciousness state. The mean phase lag is also consistent across these DoC patients. These results suggest that the envelope-tracking response to speech can be generated by an automatic process that is barely modulated by the consciousness state.
Cortical responses to auditory stimulation predict the prognosis of patients with disorders of consciousness

Abstract:Objective: This study aimed to assess the prognosis of patients with disorders of consciousness (DoC) using auditory stimulation with electroencephalogram (EEG) recordings. Methods: We enrolled 72 patients with DoC in the study, which involved subjecting patients to auditory stimulation while EEG responses were recorded. Coma Recovery Scale-Revised (CRS-R) scores and Glasgow Outcome Scale (GOS) were determined for each patient and followed up for three months. A frequency spectrum analysis was performed on the EEG recordings. Finally, the power spectral density (PSD) index was used to predict the prognosis of patients with DoC based on a support vector machine (SVM) model. Results: Power spectral analyses revealed that the cortical response to auditory stimulation showed a decreasing trend with decreasing consciousness levels. Auditory stimulation-induced changes in absolute PSD at the delta and theta bands were positively correlated with the CRS-R and GOS scores. Furthermore, these cortical responses to auditory stimulation had a good ability to discriminate between good and poor prognoses of patients with DoC. Conclusions: Auditory stimulation-induced changes in the PSD were highly predictive of DoC outcomes. Significance: Our findings showed that cortical responses to auditory stimulation may be an important electrophysiological indicator of prognosis in patients with DoC. & COPY; 2023 International Federation of Clinical Neurophysiology. Sleep Disrupts High-Level Speech Parsing Despite Significant Basic Auditory Processing

Abstract:The extent to which the sleeping brain processes sensory information remains unclear. This is particularly true for continuous and complex stimuli such as speech, in which information is organized into hierarchically embedded structures. Recently, novel metrics for assessing the neural representation of continuous speech have been developed using noninvasive brain recordings that have thus far only been tested during wakefulness. Here we investigated, for the first time, the sleeping brain's capacity to process continuous speech at different hierarchical levels using a newly developed Concurrent Hierarchical Tracking (CHT) approach that allows monitoring the neural representation and processing-depth of continuous speech online. Speech sequences were compiled with syllables, words, phrases, and sentences occurring at fixed time intervals such that different linguistic levels correspond to distinct frequencies. This enabled us to distinguish their neural signatures in brain activity. We compared the neural tracking of intelligible versus unintelligible (scrambled and foreign) speech across states of wakefulness and sleep using high-density EEG in humans. We found that neural tracking of stimulus acoustics was comparable across wakefulness and sleep and similar across all conditions regardless of speech intelligibility. In contrast, neural tracking of higher-order linguistic constructs (words, phrases, and sentences) was only observed for intelligible speech during wakefulness and could not be detected at all during nonrapid eye movement or rapid eye movement sleep. These results suggest that, whereas low-level auditory processing is relatively preserved during sleep, higher-level hierarchical linguistic parsing is severely disrupted, thereby revealing the capacity and limits of language processing during sleep.SIGNIFICANCE STATEMENT Despite the persistence of some sensory processing during sleep, it is unclear whether high-level cognitive processes such as speech parsing are also preserved. We used a novel approach for studying the depth of speech processing across wakefulness and sleep while tracking neuronal activity with EEG. We found that responses to the auditory sound stream remained intact; however, the sleeping brain did not show signs of hierarchical parsing of the continuous stream of syllables into words, phrases, and sentences. The results suggest that sleep imposes a functional barrier between basic sensory processing and high-level cognitive processing. This paradigm also holds promise for studying residual cognitive abilities in a wide array of unresponsive states.

Abstract:When listening to speech, cortical activity can track mentally constructed linguistic units such as words, phrases, and sentences. Recent studies have also shown that the neural responses to mentally constructed linguistic units can predict the outcome of patients with disorders of consciousness (DoC). In healthy individuals, cortical tracking of linguistic units can be driven by both long-term linguistic knowledge and online learning of the transitional probability between syllables. Here, we investigated whether statistical learning could occur in patients in the minimally conscious state (MCS) and patients emerged from the MCS (EMCS) using electroencephalography (EEG). In Experiment 1, we presented to participants an isochronous sequence of syllables, which were composed of either 4 real disyllabic words or 4 reversed disyllabic words. An inter-trial phase coherence analysis revealed that the patient groups showed similar word tracking responses to real and reversed words. In Experiment 2, we presented trisyllabic artificial words that were defined by the transitional probability between words, and a significant word-rate EEG response was observed for MCS patients. These results suggested that statistical learning can occur with a minimal conscious level. The residual statistical learning ability in MCS patients could potentially be harnessed to induce neural plasticity.

Abstract:Assessing residual consciousness and cognitive abilities in unresponsive patients is a major clinical concern and a challenge for cognitive neuroscience. Although neuroimaging studies have demonstrated a potential for informing diagnosis and prognosis in unresponsive patients, these methods involve sophisticated brain imaging technologies, which limit their clinical application. In this study, we adopted a new language paradigm that elicited rhythmic brain responses tracking the single-word, phrase and sentence rhythms in speech, to examine whether bedside electroencephalography (EEG) recordings can help inform diagnosis and prognosis. EEG-derived neural signals, including both speech-tracking responses and temporal dynamics of global brain states, were associated with behavioral diagnosis of consciousness. Crucially, multiple EEG measures in the language paradigm were robust to predict future outcomes in individual patients. Thus, EEG-based language assessment provides a new and reliable approach to objectively characterize and predict states of consciousness and to longitudinally track individual patients' language processing abilities at the bedside.

The Changes of Central Auditory Processing Function and Its Electroencephalogram in Patients with Mild Cognitive Impairment and Early Alzheimer’s Disease

Abstract:Background: The aim of this study is to assess the central auditory processing (CAP) function and its electroencephalogram (EEG) in patients with mild cognitive impairment (MCI) and the early stage of Alzheimer's disease (AD).Methods: In this study, 25 patients with early AD, 22 patients with MCI, and 22 matched healthy controls (HC) were included. After cognitive assessment, binaural processing function was assessed using the staggered spondaic word (SSW) test, and auditory working memory was assessed by auditory n-back paradigm, while EEG was recorded. Patients' behavioral indicators, event-related potentials (ERPs) components, and function connection (FC) were compared be-tween groups and the related factors were analyzed. Results: The difference of the accuracy of behavioral tests for the three groups of subjects was significant and all the behavioral indicators were positively correlated with cognitive function scores. Intergroup differences in amplitude (p < 0.05) and latency (p < 0.01) were significant for P3 in the 1-back paradigm. In the SSW test, AD and MCI patients showed reduced connectivity between the left frontal lobe and the whole brain in the 8-band, while in the n-back paradigm, patients with MCI and early AD showed reduced association of frontal leads with central and parietal leads in the 8-band.Conclusions: Patients with MCI and early AD have reduced CAP functions including binaural processing function and auditory working memory functions. This reduction is significantly associated with reduced cognitive function, and is reflected in different patterns of changes in ERP as well as functional connectivity in the brain.
EEG Correlates of Language Function in Traumatic Disorders of Consciousness

Abstract:Background/objective: Behavioral examinations may fail to detect language function in patients with severe traumatic brain injury (TBI) due to confounds such as having an endotracheal tube. We investigated whether resting and stimulus-evoked electroencephalography (EEG) methods detect the presence of language function in patients with severe TBI. Methods: Four EEG measures were assessed: (1) resting background (applying Forgacs' criteria), (2) reactivity to speech, (3) background and reactivity (applying Synek's criteria); and (4) an automated support vector machine (classifier for speech versus rest). Cohen's kappa measured agreement between the four EEG measures and evidence of language function on a behavioral coma recovery scale-revised (CRS-R) and composite (CRS-R or functional MRI) reference standard. Sensitivity and specificity of each EEG measure were calculated against the reference standards. Results: We enrolled 17 adult patients with severe TBI (mean ± SD age 27.0 ± 7.0 years; median [range] 11.5 [2-1173] days post-injury) and 16 healthy subjects (age 28.5 ± 7.8 years). The classifier, followed by Forgacs' criteria for resting background, demonstrated the highest agreement with the behavioral reference standard. Only Synek's criteria for background and reactivity showed significant agreement with the composite reference standard. The classifier and resting background showed balanced sensitivity and specificity for behavioral (sensitivity = 84.6% and 80.8%; specificity = 57.1% for both) and composite reference standards (sensitivity = 79.3% and 75.9%, specificity = 50% for both). Conclusions: Methods applying an automated classifier, resting background, or resting background with reactivity may identify severe TBI patients with preserved language function. Automated classifier methods may enable unbiased and efficient assessment of larger populations or serial timepoints, while qualitative visual methods may be practical in community settings.

Abstract:Importance: Despite the apparent absence of external signs of consciousness, a significant small proportion of patients with disorders of consciousness can respond to commands by willfully modulating their brain activity, even respond to yes or no questions, by performing mental imagery tasks. However, little is known about the mental life of such responsive patients, for example, with regard to whether they can have coherent thoughts or selectively maintain attention to specific events in their environment. The ability to selectively pay attention would provide evidence of a patient's preserved cognition and a method for brain-based communication, thus far untested with functional magnetic resonance imaging in this patient group. Objective: To test whether selective auditory attention can be used to detect conscious awareness and communicate with behaviorally nonresponsive patients. Design, setting, and participants: Case study performed in 3 patients with severe brain injury, 2 diagnosed as being in a minimally conscious state and 1 as being in a vegetative state. The patients constituted a convenience sample. Main outcomes and measures: Functional magnetic resonance imaging data were acquired as the patients were asked to selectively attend to auditory stimuli, thereby conveying their ability to follow commands and communicate. Results: All patients demonstrated command following according to instructions. Two patients (1 in a minimally conscious state and 1 in a vegetative state) were also able to guide their attention to repeatedly communicate correct answers to binary (yes or no) questions. Conclusions and relevance: To our knowledge, we show for the first time with functional magnetic resonance imaging that behaviorally nonresponsive patients can use selective auditory attention to convey their ability to follow commands and communicate. One patient in a minimally conscious state was able to use attention to establish functional communication in the scanner, despite his inability to produce any communication responses in repeated bedside examinations. More important, 1 patient, who had been in a vegetative state for 12 years before the scanning and subsequent to it, was able to use attention to correctly communicate answers to several binary questions. The technique may be useful in establishing basic communication with patients who appear unresponsive to bedside examinations and cannot respond with existing neuroimaging methods.
Covert Speech Comprehension Predicts Recovery From Acute Unresponsive States

Abstract:Objective: Patients with traumatic brain injury who fail to obey commands after sedation-washout pose one of the most significant challenges for neurological prognostication. Reducing prognostic uncertainty will lead to more appropriate care decisions and ensure provision of limited rehabilitation resources to those most likely to benefit. Bedside markers of covert residual cognition, including speech comprehension, may reduce this uncertainty. Methods: We recruited 28 patients with acute traumatic brain injury who were 2 to 7 days sedation-free and failed to obey commands. Patients heard streams of isochronous monosyllabic words that built meaningful phrases and sentences while their brain activity via electroencephalography (EEG) was recorded. In healthy individuals, EEG activity only synchronizes with the rhythm of phrases and sentences when listeners consciously comprehend the speech. This approach therefore provides a measure of residual speech comprehension in unresponsive patients. Results: Seventeen and 16 patients were available for assessment with the Glasgow Outcome Scale Extended (GOSE) at 3 months and 6 months, respectively. Outcome significantly correlated with the strength of patients' acute cortical tracking of phrases and sentences (r > 0.6, p < 0.007), quantified by inter-trial phase coherence. Linear regressions revealed that the strength of this comprehension response (beta = 0.603, p = 0.006) significantly improved the accuracy of prognoses relative to clinical characteristics alone (eg, Glasgow Coma Scale [GCS], computed tomography [CT] grade). Interpretation: A simple, passive, auditory EEG protocol improves prognostic accuracy in a critical period of clinical decision making. Unlike other approaches to probing covert cognition for prognostication, this approach is entirely passive and therefore less susceptible to cognitive deficits, increasing the number of patients who may benefit. Hemispheric and executive influences on low-level language processing after traumatic brain injury

Abstract:OBJECTIVE: The purpose of this study was to examine whether minor high-level language deficits found after traumatic brain injury (TBI) might be due to low-level language processing issues or executive control influences. A possible mechanism was also investigated.METHOD: Nineteen age- and education-matched healthy controls (16 M, 3 F) and 19 persons who had experienced a complicated mild, moderate or severe TBI between 1-3 years prior (16 M, 3 F; mean GCS = 9.44) participated in two computerized behavioural experiments utilizing two paradigms standard in the psycholinguistic literature (priming with lexical decision and verb generation), which included trials of greater and lesser executive demand.RESULTS: Response time and accuracy differences were found in both experiments, indicating deficits in single-word processing for the patient group. Disproportionate difficulty was found for trials which included an executive component. Right visual field (left hemisphere) preferences were found to be stronger in the TBI group than in controls.CONCLUSIONS: Results suggest that persons with TBI may have difficulties in processing single words alone, especially under conditions of increased executive demand, and that atypical patterns of hemispheric recruitment may be associated with these difficulties.

Neural substrates and behavioral relevance of speech envelope tracking: evidence from post-stroke aphasia

Abstract:Neural tracking of the low-frequency temporal envelope of speech has emerged as a prominent tool to investigate the neural mechanisms of natural speech processing in the brain. However, there is ongoing debate regarding the functional role of neural envelope tracking. In this context, our study aims to offer a novel perspective by investigating the critical brain areas and behavioral skills required for neural envelope tracking in aphasia, a language disorder characterized by impaired neural envelope tracking. We analyzed an EEG dataset of 39 individuals with post-stroke aphasia suffering a left-hemispheric stroke who listened to natural speech. Our analysis involved lesion mapping, where left lesioned brain voxels served as binary features to predict neural envelope tracking measures. We also examined the behavioral correlates of receptive language, naming, and auditory processing (via rise time discrimination task) skills. The lesion mapping analysis revealed that lesions in language areas, such as the middle temporal gyrus, supramarginal gyrus and angular gyrus, were associated with poorer neural envelope tracking. Additionally, neural tracking was related to auditory processing skills and language (receptive and naming) skills. However, the effects on language skills were less robust, possibly due to ceiling effects in the language scores. Our ﬁndings highlight the importance of central brain areas implicated in language understanding, extending beyond the primary auditory cortex, and emphasize the role of intact auditory processing and language abilities in effectively processing the temporal envelope of speech. Collectively, these ﬁndings underscore the signiﬁcance of neural envelope tracking beyond mere audibility and acoustic processes.

Detecting post-stroke aphasia using EEG-based neural envelope tracking of natural speech

Abstract:[Objective]. After a stroke, one-third of patients suffer from aphasia, a language disorder that impairs communication ability. The standard behavioral tests used to diagnose aphasia are time-consuming and have low ecological validity. Neural tracking of the speech envelope is a promising tool for investigating brain responses to natural speech. The speech envelope is crucial for speech understanding, encompassing cues for processing linguistic units. In this study, we aimed to test the potential of the neural envelope tracking technique for detecting language impairments in individuals with aphasia (IWA). [Approach]. We recorded EEG from 27 IWA in the chronic phase after stroke and 22 controls while they listened to a story. We quantified neural envelope tracking in a broadband frequency range as well as in the delta, theta, alpha, beta, and gamma frequency bands using mutual information analysis. Besides group differences in neural tracking measures, we also tested its suitability for detecting aphasia using a Support Vector Machine (SVM) classifier. We further investigated the required recording length for the SVM to detect aphasia and to obtain reliable outcomes. [Results]. IWA displayed decreased neural envelope tracking compared to controls in the broad, delta, theta, and gamma band. Neural tracking in these frequency bands effectively captured aphasia at the individual level (SVM accuracy 84%, AUC 88%). High-accuracy and reliable detection could be obtained with 5-7 minutes of recording time. [Significance]. Our study shows that neural tracking of speech is an effective biomarker for aphasia. We demonstrated its potential as a diagnostic tool with high reliability, individual-level detection of aphasia, and time-efficient assessment. This work represents a significant step towards more automatic, objective, and ecologically valid assessments of language impairments in aphasia.

Abstract:One out of three stroke-patients develop language processing impairment known as aphasia. The need for ecological validity of the existing diagnostic tools motivates research on biomarkers, such as stimulus-evoked brain responses. With the aim of enhancing the physiological interpretation of the latter, we used EEG to investigate how functional brain network patterns associated with the neural response to natural speech are affected in persons with post-stroke chronic aphasia. EEG was recorded from 24 healthy controls and 40 persons with aphasia while they listened to a story. Stimulus-evoked brain responses at all scalp regions were measured as neural envelope tracking in the delta (0.5-4 Hz), theta (4-8 Hz) and low-gamma bands (30-49 Hz) using mutual information. Functional connectivity between neural-tracking signals was measured, and the Network-Based Statistics toolbox was used to: 1) assess the added value of the neural tracking vs EEG time series, 2) test between-group differences and 3) investigate any association with language performance in aphasia. Graph theory was also used to investigate topological alterations in aphasia. Functional connectivity was higher when assessed from neural tracking compared to EEG time series. Persons with aphasia showed weaker low-gamma-band left-hemispheric connectivity, and graph theory-based results showed a greater network segregation and higher region-specific node strength. Aphasia also exhibited a correlation between delta-band connectivity within the left pre-frontal region and language performance. We demonstrated the added value of neural tracking when investigating functional brain networks associated with natural speech processing in post-stroke aphasia. Its higher sensitivity to language-related brain circuits favours its use as informative biomarker for the assessment of aphasia.

Abstract:Background: Patients with brain injury who are unresponsive to commands may perform cognitive tasks that are detected on functional magnetic resonance imaging (fMRI) and electroencephalography (EEG). This phenomenon, known as cognitive motor dissociation, has not been systematically studied in a large cohort of persons with disorders of consciousness. Methods: In this prospective cohort study conducted at six international centers, we collected clinical, behavioral, and task-based fMRI and EEG data from a convenience sample of 353 adults with disorders of consciousness. We assessed the response to commands on task-based fMRI or EEG in participants without an observable response to verbal commands (i.e., those with a behavioral diagnosis of coma, vegetative state, or minimally conscious state-minus) and in participants with an observable response to verbal commands. The presence or absence of an observable response to commands was assessed with the use of the Coma Recovery Scale-Revised (CRS-R). Results: Data from fMRI only or EEG only were available for 65% of the participants, and data from both fMRI and EEG were available for 35%. The median age of the participants was 37.9 years, the median time between brain injury and assessment with the CRS-R was 7.9 months (25% of the participants were assessed with the CRS-R within 28 days after injury), and brain trauma was an etiologic factor in 50%. We detected cognitive motor dissociation in 60 of the 241 participants (25%) without an observable response to commands, of whom 11 had been assessed with the use of fMRI only, 13 with the use of EEG only, and 36 with the use of both techniques. Cognitive motor dissociation was associated with younger age, longer time since injury, and brain trauma as an etiologic factor. In contrast, responses on task-based fMRI or EEG occurred in 43 of 112 participants (38%) with an observable response to verbal commands. Conclusions: Approximately one in four participants without an observable response to commands performed a cognitive task on fMRI or EEG as compared with one in three participants with an observable response to commands. Abstract:Objective. Clinical assessments of individuals with Cognitive-Motor Dissociation (CMD) following brain injury are challenging and prone to errors. This prompts investigation of objective, movement-independent neurophysiological markers using electroencephalography (EEG) based Brain Computer Interface (BCI) technology. The current pilot study involving adults with prolonged disorders of consciousness (PDoC) investigated the combination of Motor Imagery BCI (MI BCI) training and auditory evoked Event Related Potentials (ERPs) using an oddball paradigm to produce complementary biomarkers to improve evaluation of awareness in PDoC . Approach. EEG data (16 channels) were collected from participants with Unresponsive Wakefulness Syndrome (UWS, n = 2), Minimally Conscious State (MCS, n = 3), and Locked-In Syndrome (LIS, n = 4). The MI-BCI involved assessing sensorimotor rhythm modulation, motor-imagery training with and without auditory feedback, and motor-imagery responses to closed questions over 12 sessions each lasting 1 hour. The oddball protocol was also deployed in 2 to 3 of those sessions, with approximately 10 days between first and last, featuring auditory stimuli, comprising two 5 minute sets of standard, deviant beeps plus novel sounds, in a structured ratio. We expected those with the lowest levels of awareness would have reduced ERP components, with highest latencies to peak, as well as lowest accuracy in the motor imagery BCI protocol, and that trends across these metrics would be observed across the three patient groups based on their clinical diagnoses. Main results. Significant differences in mean N1 component latencies and mean MI Decoding Accuracies (DA, for significant runs) occurred between groups, with shorter N1 latencies for the LIS and MCS groups than for the UWS group (LIS vs. UWS and MCS vs. UWS, p < 0.001), and higher DA for the LIS group compared to MCS and UWS (p < 0.001). Mean DA were found to have a significant negative correlation with mean N1 latencies (two-tailed, p = 0.017). Significance. The results indicate that neurophysiological markers from the concomitant application of an MI-BCI and auditory oddball paradigm can augment standard clinical assessments by providing objective measures that produce robust evidence of awareness in people with PDoC.

Abstract:OBJECTIVE: To characterize the language impairments that occur in brain tumor patients using a cognitively oriented theoretical framework.METHOD: Forty-nine preoperative brain tumor patients completed a new testing protocol (the BLAST) which assesses 8 well documented, "core" cognitive skills required for language: auditory word recognition, accessing semantic knowledge, lexical selection, phonological encoding, verbal short-term memory, goal-driven language selection, verb retrieval, and articulatory-motor planning. Patients were unselected with respect to lesion location.RESULTS: A surprising 65% of patients scored below controls on at least 1 core skill. Patients with left temporal tumors, as a group, had lower scores than the remaining patients on phonological encoding, accessing semantic knowledge and verbal short-term memory (STM). Those with left frontal tumors had the lowest scores on articulatory-motor planning. These findings are broadly consistent with previous studies examining the anatomical substrates of our "core" cognitive processes.CONCLUSIONS: We conclude that selective impairments in key language skills are common in brain tumor patients, but many of these are not adequately assessed on conventional aphasia assessments.
