Abstract:Purpose: To evaluate the agreement and repeatability between a new commercially available binocular open-field wavefront autorefractor, as part of the Eye Refract system, and a monocular closed-field wavefront autorefractor (VX110). Methods: A cross-sectional, randomized, and single-masked study was performed. Ninety-nine eyes of 99 healthy participants (37.22 ± 18.04 years, range 8 to 69 years) were randomly analyzed. Three measurements with the Eye Refract and the VX110 were taken on three different days, under noncycloplegic conditions. Mean spherical equivalent (MSE), cylindrical vectors (J0 and J45), and binocular corrected distance visual acuity (BCDVA) were compared between both autorefractors. An intersession repeatability analysis was done considering the values of repeatability (S r ) and its 95% limit (r). Results: The VX110 showed more negative values (P < 0.001) in terms of MSE in comparison with the Eye Refract (0.20 D). Regarding cylindrical vectors, J45 showed statistically significant differences (P=0.001) between both wavefront autorefractors, but they were not clinically relevant (<0.05 D). In BCDVA, there were no statistically significant differences (P=0.667) between both wavefront autorefractors. Additionally, the Eye Refract was more repeatable than the VX110 in terms of both MSE (S r EYE REFRACT = 0.21 D, S r VX110 = 0.53 D) and J0 (S r EYE REFRACT = 0.12 D, S r VX110 = 0.35 D). Conclusions: The Eye Refract provided enough accuracy and reliability to estimate refractive errors in different age groups, achieving better results than the VX110. Therefore, the Eye Refract proved to be a useful autorefractor to be incorporated into clinical practice.
Correlation analysis between myopic refraction and ocular components

Abstract:Objective To investigate the relationship between myopic refraction and ocular components.Methods Retinoscopy was performed in 533 myopic subjects (1018 eyes).K value was measured by corneal mapping,and A/B ultrasound scan to get AL,ACD and LT.VCD,AnL,RLT/AL and RAL/R were calculated.Results In myopia,AL was not associated with LT (r =0.028,P =0.364); SEQ and AL,ACD,LT,K,VCD,RAL/R,RLT/AL were all significantly correlated (P ＜0.01),and better associated with RAL/R,A1 and VCD (r =-0.815;-0.624;-0.619); ACD,LT and AnL were not statistically different between myopia groups (ANOVA:P =0.153; 0.063; 0.625),nonetheless,all the other parameter did (P ＜0.01).Conclusions Myopic severity can be featured by the significant difference of posterior segment measurement,the anterior segment is comparatively stable and statistical insignificant.
An Investigation on the Consistency Between Autorefraction and Subjective Refraction

Abstract:Purpose To observe the consistence in results between autorefraction and subjective referaction.Methods Subjects aged 7-22 years were performed optometry with autorefraction and subjective refrection.Comparisions of spherical lens dioptre,cylinder dioptre and astigmatic axis obtained with the two methods were made.Results Significant difference in spherical lens dioptre(P<0.05)was found in high hyperopia and middle myopia subjects.And in high myopia,significant difference in cylinder dioptre was identified(P<0.05).Significant difference in the length of astigmatic axis existed in low myopia and hypempia(P<0.05).Conclusion Although autorefraction has advantage of quick optometry,it cannot take over subjective refraction.The examined results with autorefraction cannot be used as last prescription for refraction.(Chin J Ophthalmol and Otorhinolaryngol.2007,7:35-36)

Abstract:Abstract Background: The evaluation of refraction is indispensable in ophthalmic clinics, generally requiring a refractor or retinoscopy under cycloplegia. Retinal fundus photographs (RFPs) supply a wealth of information related to the human eye and might provide a new approach that is more convenient and objective. Here, we aimed to develop and validate a fusion model-based intelligent retinoscopy system (FMIRS) to identify ocular refraction via RFPs and compare with the cycloplegic refraction. In this population-based comparative study, we retrospectively collected 11,973 RFPs from May 1, 2020 to November 20, 2021. The FMIRS was constructed, and the performance of the regression models of sphere and cylinder was evaluated. The accuracy, sensitivity, specificity, area under the receiver operating characteristic curve, and F1-score were used to evaluate the classification model of the cylinder axis. Results: Overall, 11,973 images were included. For sphere and cylinder, the mean absolute error values between the FMIRS and cycloplegic refraction were 0.50 D and 0.31 D, representing an increase of 29.41% and 26.67%, respectively, when compared with those of the single models. The correlation coefficients (r) were 0.949 and 0.807, respectively. For axis analysis, the accuracy, specificity, sensitivity, and area under the curve value of the classification model were 0.89, 0.941, 0.882, and 0.814, respectively, and the F1-score was 0.88. Conclusions: The FMIRS successfully identified ocular refraction accurately in sphere, cylinder, and axis, and it showed good agreement with the cycloplegic refraction. The RFPs can not only provide comprehensive fundus information but also the refraction state of the eye, emphasising their potential clinical value.
Comparing ocular biometry and autorefraction measurements from the Myopia Master with the IOLMaster 700 and the Huvitz HRK-8000A autorefractor

Abstract:Purpose: To compare axial length (AL) and corneal radius (CR) measured with the Oculus Myopia Master and the Zeiss IOLMaster 700, and cycloplegic refractive error measured with the Myopia Master and the Huvitz Auto Ref/Keratometer (HRK-8000A). Methods: The study included both eyes of 74 participants (16 male), with a mean (SD) age of 22.8 (3.7) years. The parameters indicated were measured under cycloplegia with these instruments: Myopia Master (AL, CR and refractive error), IOLMaster 700 (AL and CR) and HRK-8000A (refractive error and CR). Bland-Altman plots with mixed effects 95% limits of agreement (LoA) and corresponding 95% confidence intervals were used to assess the agreement in ocular biometry between the Myopia Master and the IOLMaster 700, and for refractive error between the Myopia Master and the HRK-8000A. Results: The analysis included 139 eyes, of which 52 were myopic (spherical equivalent refractive error, SER ≤ -0.50 D), 32 emmetropic and 55 hyperopic (SER ≥ 0.50 D). The 95% LoA for AL between the Myopia Master and IOLMaster 700 was -0.097 to 0.089 mm. There was no mean difference in AL [mean (SD) = -0.004 (0.047) mm, p = 0.34]. There was a significant difference in mean CR, with that measured with the Myopia Master being flatter than that found with the IOLMaster 700 [0.035 (0.028) mm, p < 0.001]. The 95% LoA for CR was -0.02 to 0.09 mm. Compared with HRK-8000A, the Myopia Master measured a significantly more negative SER [-0.19 (0.33) D, p < 0.001], with 95% LoA of -0.86 to 0.46 D. Conclusion: The LoA for measurements of SER, CR and AL when comparing the Myopia Master with the HRK-8000A and the IOLMaster 700 were wider than deemed acceptable for making direct comparisons. This indicates that the instruments cannot be used interchangeably in clinical practice or research.

Abstract:Purpose To investigate the influence of phenylephrine and tropicamide on anterior segment biometry with ultralong scan depth optical coherence tomography (UL-OCT) during accommodation. Methods In this study, 20 left eyes of healthy volunteers with a mean ± standard deviation age of 31.05 ± 5.84 years and a mean refraction of −1.16 ± 1.11 diopters (range 0∼−3.0 D) were imaged using UL-OCT after instillation of artificial tears, phenylephrine, and tropicamide in three follow-up trials, respectively. At each follow-up trial, two repeated measurements were performed at states of relax and 5D accommodative stimulation. The dimensional parameters included central corneal thickness (CCT), anterior chamber depth (ACD), pupil diameter (PD), lens thickness (LT), and horizontal radii of the lens anterior and posterior surface curvatures (LAC and LPC). Results Tropicamide led to larger pupil, deeper ACD, thinner LT, and flatter crystalline lens surface (P < 0.05). Phenylephrine induced an increase in PD (P < 0.05), while no significant changes were seen in ACD, LT, LAC, and LPC (P > 0.05). CCT did not change after both phenylephrine and tropicamide instillation in this study (P > 0.05). Tropicamide induced the loss of accommodation and phenylephrine achieved pupil dilation without affecting the accommodation. PD, ACD decreased, LT increased significantly and the anterior and posterior surface of the lens in a 6.294 mm of diameter optical zone became steeper during accommodation after administration of phenylephrine (P < 0.05). Conclusion The anterior segment physiology changed after tropicamide instillation. Besides, tropicamide induced the loss of accommodation and phenylephrine preserved the accommodation with a larger pupil. And, the anterior and posterior surface of lens in a 6.294 mm of diameter optical zone became steeper during the accommodation.

Abstract:Purpose To compare scanning-slit retinoscopy automated refraction spherical equivalent (ARSE) to subjective manifest refraction spherical equivalent (MRSE) in normal eyes with four different types of intraocular lenses (IOLs). Design Retrospective cross-sectional study Methods 279 pseudophakic eyes that underwent lens extraction at a private center with implantation of either a wavefront shaped IOL (Acrysof® Vivity, DFT015), a nonapodized diffractive trifocal IOL (Acrysof® Panoptix), or a monofocal IOL with negative spherical aberration (Tecnis ZCBOO) or aberration-free (Envista Mx60E). Patients who had an automated refraction measurement with retinoscopy refractometer and aberrometer (NIDEK OPD Scan III, Nidek Technologies) and subjective refraction data 1-2 months postoperatively were included in the study. Main outcome measured was the difference between automated refraction and subjective refraction. Results Sixty-one eyes implanted with a DFT015 IOL, 78 eyes with a TFNT00 IOL, 40 with a ZCBOO IOL and 100 with a Mx60E IOL were enrolled in this study. Statistically significant myopic results were observed when using ARSE compared to MRSE in the DFT015 (-0.95±0.64, -0.33±0.65, p < .001), TFNT00 (-0.43±0.36, -0.2±0.35, p <.001), ZCBOO (-0.81±0.63, -0.4±0.69, p = .008) and Mx60E (-0.75±0.65, -0.45±0.52; p <0.05) IOL groups. The absolute difference between the 2 methods was statistically significant in the DFT015 IOL group (0.65±0.49; p< 0.05). Conclusion Automated refraction yields myopic results in pseudophakic eyes with monofocal and presbyopia correcting IOLs and may be used with caution. Manifest refraction "push-plus" technique should be used in all pseudophakic eyes to avoid over-minus prescriptions, especially in patient with residual refractive error following cataract surgery.

Comparison of Subjective and Wavefront-Measured Refractions by InnovEyes Platform in Myopic Patients

Abstract:PURPOSE:To compare the subjective refraction and Innov-Eyes (Alcon Laboratories, Inc) wavefront-measured refraction in patients with myopia and astigmatism before refractive surgery. METHODS:Consecutive myopic patients planning to receive refractive surgery with ages between 18 and 50 years old were retrospectively enrolled. The subjective and wavefront-measured refraction (InnovEyes platform) were compared, and the influential factors for the measurement difference were analyzed. RESULTS:Of the 500 eyes enrolled, the mean age was 29.3 ± 6.8 years. Mean wavefront-measured refraction was 0.58 ± 0.62 diopters (D) more myopic and 0.10 ± 0.27 D more astigmatic than subjective refraction (P < .001). Wavefront-measured refraction exhibited excellent consistency and significant correlation with subjective refraction. Linear regression demonstrated that age (P = .008) and wavefront-measured sphere standard deviation (P < .001) were positively correlated with spherical measurement difference, and total quality score (P < .001) and accommodative response (binocular cross-cylinder) (P = .011) demonstrated a negative correlation with spherical and cylindrical measurement difference. The patients with low wavefront measurement quality had significantly greater spherical differences than those with high-quality measurement (P < .001). The patients with accommodative lead had greater spherical equivalent differences than other participants (P = .047). CONCLUSIONS:Wavefront-measured refraction had excellent consistency with subjective refraction, although it measured more myopia and astigmatism than subjective refraction. The spherical measurement difference was greater in older patients and those with accommodative lead. More stable and higher quality repeated measurements will yield closer results between wavefront-measured and subjective refraction. [J Refract Surg. 2024;40(11):e836-e844.].

Abstract:AIM: To compare the consistency of two autorefractors (Tianle RM-9000 and Topcon KR-800) for school-age myopia children, and to provide a basis for largescale data analysis and comparison. METHODS: The refractive error in 909 subjects (age 4–18y) were measured using both autorefractors without cycloplegia. The data were analyzed using Fourier decomposition and the correlation coefficients, intraclass correlation coefficients (ICC), and Bland-Altman limits of agreement (LoA) for each parameter were calculated. RESULTS: There was a strong correlation between the spherical equivalent (SE), sphere diopter (DS), and cylinder diopter (DC) readings of the Tianle RM-9000 and those of the Topcon KR-800, with correlation coefficient values of 0.98, 0.98 and 0.83 and ICC values of 0.99, 0.99 and 0.93, respectively. However, the correlation coefficients and ICC values of J0 and J45 were unreliable (R=-0.004, -0.034; both ICC<0.10). Bland-Altman analysis revealed that SE, DS, and DC measured by the Tianle RM-9000 were significantly biased toward myopia compared with the Topcon KR-800, and the mean differences were -0.072, -0.026, -0.091 D, respectively (all P<0.01). The minimum absolute value of the difference within the 95% LoA for SE, DS, and DC was 0.63 D, 0.50 D, 0.62 D, respectively; all these values were in the clinically acceptable range. For J0 and J45, the mean differences were close to zero (P=0.43, 0.84); however, the 95% LoA were relatively wide (J0 SD: 0.53; 95%CI: -1.00, 1.10; J45 SD: 0.52; 95%CI: -1.00, 1.00). CONCLUSION: The two autorefractors are consistent with each other, as the differences in SE, DS, and DC were within the clinically acceptable range. Readers can compare the data measured by either device in different studies and use the two devices in the same study to generate a dataset that can be analyzed together. However, the J0 and J45 vectors are unreliable and should not be used to assess astigmatism.

Abstract:To measure by ultralong scan depth optical coherence tomography (UL-OCT) dimensional changes in the anterior segment of human eyes during accommodation.Evaluation of diagnostic test or technology.Forty-one right eyes of healthy subjects with a mean age of 34 years (range, 22-41 years) and a mean refraction of -2.5 ± 2.6 diopters were imaged in 2 repeated measurements at minimal and maximal accommodations.A specially adapted and designed UL-OCT instrument was used to image from the front surface of the cornea to the back surface of the crystalline lens. Custom software corrected the optical distortion of the images and yielded the biometric measurements. The coefficient of repeatability and the intraclass correlation coefficient were calculated to evaluate the repeatability and reliability.Anterior segment parameters and associated repeatability and reliability upon accommodation. The dimensional results included central corneal thickness (CCT), anterior chamber depth and width (ACD, ACW), pupil diameter (PD), lens thickness (LT), anterior segment length (ASL = ACD + LT), lens central position (LCP = ACD + 1/2LT), and horizontal radii of the lens anterior and posterior surface curvatures (LAC, LPC).Repeated measurements of each variable within each accommodative state did not differ significantly (P>0.05). The coefficients of repeatability (CORs) and intraclass correlation coefficients for CCT, ACW, ACD, LT, LCP, and ASL were excellent (1.2%- 3.59% and 0.998-0.877, respectively). They were higher for PD (18.90%-21.63% and 0.880-0.874, respectively) and moderate for LAC and LPC (34.86%-42.72% and 0.669-0.251, respectively) in the 2 accommodative states. Compared with minimal accommodation, PD, ACD, LAC, LPC, and LCP decreased and LT and ASL increased significantly at maximal accommodation (P<0.05), whereas CCT and ACW did not change (P>0.05).The UL-OCT measured changes in anterior segment dimensions during accommodation with good repeatability and reliability. During accommodation, the back surface of the lens became steeper as the lens moved forward.The authors have no proprietary or commercial interest in any of the materials discussed in this article.
Correlation among auto-refractor, wavefront aberration and subjective manual refraction

Abstract:Three optometry methods which include auto-refractor, wavefront aberrometer and subjective manual refraction were studied and compared in measuring low order aberrations of 60 people's 117 normal eyes. Paired t-test and linear regression were used to study these three methods' relationship when measuring myopia with astigmatism. In order to make the analysis more clear, we divided the 117 normal eyes into different groups according to their subjective manual refraction and redid the statistical analysis. Correlations among three methods show significant in sphere, cylinder and axis in all groups, with sphere's correlation coefficients largest(R>0.98, P<0.01) and cylinder's smallest (0.90<R<0.91,P<0.01). The Paired t-test before grouping showed the difference of three optometry methods in measuring sphere was significant (P<0.01) while axis's difference was not significant (P>0.01). Auto-refractor had significant change from the other two methods when measuring cylinder (P<0.01). The results after grouping differed a little from the analysis among total people. Although three methods showed significant change from each other in certain parameters, the amplitude of these differences were not large, which indicated that the coherence of auto-refractor, wavefront aberrometer and subjective refraction is good. However, we suggested that wavefront aberration measurement could be a good starting point of optometry, subjective refraction is still necessary for refinement.

Abstract:Purpose: To evaluate a system based on a Hartmann-Shack wavefront sensor attached to a phoropter that allows the user to obtain real-time information about the refractive state of the eye and the accommodation response (AR).Methods: The objective refractions (ME) and ARs of 73 subjects (50 women, 23 men; ages, 19-69 years) were assessed with the system developed while placing in the phoropter the subjective refraction (MS) plus a set of trial lenses with differences in spherical equivalent power (ΔM) between ±2 diopters (D).Results: The objective estimations (ME) showed a good correlation with the subjective values (MS) (r = 0.989; P < 0.001). The means of the ARs presented a region where the accommodation remained stable (ΔM from +2 D to about 0 D), followed by another in which the response increased progressively (ΔM from about 0 to -2 D) with the magnitude of the accommodation stimulus. The analysis of variance within subjects applied to ARs introducing age and MS as covariates showed an increasing effect size of age from medium to large between ΔM of -0.5 and -2 D. In contrast, MS had a medium effect size (between ΔM of +2 and 0 D).Conclusions: The implemented system permitted an objective estimation of the refraction of the eye and its AR. Because it is coupled to a phoropter, the system can be used to retrieve the AR during subjective refraction procedures.Translational Relevance: The developed system can be used as a supporting tool during subjective refraction to provide certainty about the true state of accommodation.

Abstract:PURPOSE: To assess agreement between a new all-in-one non-contact optical biometer based on optical low coherence reflectometry (SW-9000 mu m Plus; Suoer) and a swept-source optical coherence tomography biometer (OA-2000; Tomey).METHODS: Each eye was scanned three times in a row by each device at random. The measured ocular parameters included central corneal thickness (CCT), anterior chamber depth (ACD), lens thickness (LT), axial length (AL), flat kera-tometry (Kf), steep keratometry (Ks), mean keratometry (Km), astigmatism, corneal diameter (CD), and pupil diameter (PD). The paired t test was used to show the differences between the SW-9000 and OA-2000. Bland-Altman plots and the 95% limits of agreement (LoA) were applied to assess the consis-tency of the measurements.RESULTS: Sixty eyes from 60 healthy participants were ex-amined, with a mean spherical equivalent refraction of-5.58 +/- 2.31 diopters and a mean age of 30.40 +/- 6.07 years. The Bland-Altman plots showed high agreement for AL, ACD, LT, Kf, Ks, Km, astigmatism, and CD measurements (95% LoA: -0.06 to 0.04 mm,-0.10 to 0.06 mm,-0.12 to 0.11 mm,-0.30 to 0.29 D,-0.35 to 0.38 D,-0.29 to 0.30 D,-0.30 to 0.34 D, and -0.50 to 0.06 mm, respectively), whereas the agreement for CCT and PD were moderate (95% LoA: 7.12 to 20.43 mu m,-0.75 to 1.19 mm, respectively).CONCLUSIONS: The new all-in-one non-contact biometer had high agreement with the OA-2000 biometer on the AL, ACD, LT, Kf, Ks, Km, astigmatism, and CD measurements. For most of the ocular parameters assessed, they were clinically interchangeable.

Abstract:BACKGROUND A cluster of biopsy-confirmed interstitial lung disease among workers at a nylon flock plant led to a request for National Institute for Occupational Safety and Health investigators to conduct a health hazard evaluation. METHODS Part of the overall evaluation, reported here, involved a cross-sectional medical survey of current employees. The survey consisted of a questionnaire, spirometry and diffusing capacity testing, and chest radiograph. RESULTS Workers assigned to production and maintenance jobs reported frequent eye and throat irritation, respiratory symptoms, and systemic symptoms (i.e., generalized aches and fevers). Most reported improvement when away from work. Frequent respiratory/systemic symptom prevalence was significantly associated with departmental category, with days and hours worked per week, and with working on a flocking range. Compared to asymptomatic workers, symptomatic workers had similar mean ratios of forced expiratory volume in one second to forced vital capacity, but lower mean percent of predicted values for both forced vital capacity and diffusing capacity. All acceptable chest radiographs were classified as category 0 for small opacities. CONCLUSIONS Findings of this study, along with those from studies reported elsewhere, implicate occupational exposure to flock-associated dust as a significant respiratory health hazard at this plant.

Abstract:Purpose: This study aimed to evaluate differences in objective and subjective accommodation dynamically and simultaneously. Methods: Thirty-four pre-presbyopic healthy volunteers (mean age ± SD, 41.0 ± 3.2 years) participated in this study. Initially, the reaction time for detecting a change in the target was measured at near. Dynamic accommodation was then monocularly recorded using an open-view Shack-Hartmann aberrometer and compared with the amplitude and velocity of subjective accommodation. Results: The objective amplitude of accommodation (0.97 ± 0.32 diopter [D]) was significantly greater than the subjective amplitude of accommodation (0.62 ± 0.43 D; P < 0.001). The accommodative velocity was significantly faster for the "before the accommodation" response time (0.47 ± 0.38 D/s) than the "after the accommodation" response time (0.21 ± 0.22 D/s; P = 0.007). Conclusions: The human eye under the monocular condition quickly adjusts to the focal plane to clearly archive the nearby object, and the focal plane thereafter is slowly and accurately adjusted to the visual target after visual recognition.

Abstract:Aim: To study the magnitude of objective accommodative response (OAR) with account to the method of image defocusing. The latter can be realized by bringing the object nearer to the eye or by adding a minus lens in front of the viewer. Material and methods: We examined 63 patients (126 eyes) with different refraction aged from 7 to 31 years. All patients underwent a series of OAR measurements on WR-5100K Grand Seiko Binocular Open-Field Autorefkeratometer under different conditions, including negative spherical lenses of -3.0 D and -5.0 D, test optotypes corresponding to visual acuity of 0.2 and 0.7 and presented at a distance of 5 m, and a set of variably sized optotypes presented at a distance of 33 cm and 20 cm. Results: A disparity was found between the magnitude of accommodative responses evoked through different means: placing a negative lens in front of the viewer while he/she looks into the distance or bringing the object closer to the eye. In almost all cases lens-induced responses were less pronounced than distance-induced. In all myopic children, accommodative lag was longer at near than at distance. Generally, in all groups, OAR to distant objects as well as responses to a 3.0 D accommodative task at near did not depend on the size of the object. There was an insignificant and statistically unreliable difference for objects placed 20 cm away (5.0 D accommodative task): OAR to a smaller font was stronger than that to a larger one. Conclusion: In children and adults with emmetropia, hyperopic children, and myopic adults, longer accommodative lags were characteristic of lens-induced responses. In children with myopia, longer lags were observed at near. Moreover, at near, OAR to a smaller font was stronger than that to a larger one.

Abstract:Purpose: To assess agreement between a new aberrometer (Osiris-T; CSO) employing pyramid wavefront sensor technique and Scheiner-Smirnov aberrometer (OPD-Scan III; Nidek) on measuring ocular, corneal, and internal aberrations in healthy participants. Methods: The measurements were conducted three times consecutively by an experienced examiner. The total root mean square (RMS) aberrations, higher order aberration RMS, coma Z 3 ±1 , trefoil Z 3 ±3 , spherical aberration Z 4 0 , and astigmatism II Z 4 ±2 up to 7th order were exported in both 4-and 6-mm pupil zones. The parameters between the two devices were statistically compared using the paired t -test, and the differences assessed with Bland-Altman plots and 95% limits of agreement. Results: This prospective study included 70 right eyes of 70 healthy participants with an average age of 25.94 ± 6.59 years (range: 18 to 47 years). The mean difference in the two devices ranged from 0.01 μm for astigmatism II Z 4 ±2 to 0.63 μm for total RMS in 4 mm and from 0.01 to 1.41 μm in 6-mm pupil size. The Bland-Altman analysis of ocular, corneal, and internal aberrations indicated high agreement between the two devices and the maximum absolute values for 95% limits of agreement ranged from 0.03 to 1.06 μm for 4-mm pupil diameters and 0.12 to 1.13 μm for 6-mm pupil diameters. Conclusions: The newly developed pyramid wavefront sensor technique aberrometer demonstrated a high agreement with a Scheiner-Smirnov aberrometer when measuring ocular, corneal, and internal aberrations in healthy participants. Comparison of Self Refraction accuracy with Cycloplegic Subjective Refraction in Young Adults

Abstract:Background: Uncorrected refractive error is one of the major causes of blindness. Self-refraction methods are currently practiced to correct refractive error. Self-adjustable eyeglasses are available easily in the present online buying era. Hence this study aimed to compare the visual and refractive outcomes between Self-refraction (SR) eyeglasses and Cycloplegic subjective Refraction (CSR). Methodology: This observational cross-sectional study included 59 participants (21 males and 38 females) within the age of 18 to 30yearsand refractive error between +3.00D to -6.00D took part in this study. Subjects with a previous history of non-strabismic binocular vision anomaly, astigmatic error, pathological eye problems, ocular surgery or trauma were excluded from the study. The subjects were allowed to estimate their refractive error with DialVision eyeglasses followed by CSR, which was performed by a single examiner. The visual and refractive outcomes of SR and CSR were collected and analyzed. Result: The study participants mean age was 22(2) years.The comparison of visual acuity and duochrome between SR and CSR using Paired T test showed a statistically significant difference (P<0.01). Visual acuity attained from SR resulted in a mean acuity of 0.14(0.05) logMAR compared to CSR 0.002(0.007) logMAR. Duochrome test identified 92% of participants being under corrected with SR. No statistically significant difference was observed in contrast sensitivity and cover test (P>0.01). Conclusion: Visual acuity with SR was acceptable but wasn't better than CSR. Appreciable under correction of refractive error with SR raises concern. Encouraging a patient to correct their own refractive error without a comprehensive eye examination can lead to various adverse effects. Even though SR might bring a brief relief towards the burden of uncorrected refractive error, dispensing spectacle based on standard refraction procedure is imperative for good visual performance.
Comparison of trial lens and computer-aided fitting in orthokeratology: A multi-center, randomized, examiner-masked, controlled study

Abstract:Purpose: To compare the efficacy and safety between traditional lens fitting and computer-aided fitting methods for orthokeratology (OrthoK) in the Chinese population. Methods: A multi-center, examiner-masked, randomized controlled study was conducted with a one-year follow-up period, enrolling 280 participants with spherical equivalent (SE) ranging from -0.5D to -4.0D. Participants were assigned to either the computer-aided orthokeratology fitting group (trial group) or the traditional lens fitting group (control group) using stratified randomization based on age (8 to 13 years, 13 to 18 years, and ≥ 18 years) to ensure a minimum of 30 cases in each sub-age group. Ocular examinations included visual acuity, objective and subjective refraction, corneal endothelial cell density, corneal topography, intraocular pressure, axial length, and ocular health assessment. Successful lens-correction was defined as the residual refraction with the OK lens, which should not exceed ± 0.5D, and/or an uncorrected visual acuity of no worse than 0.1 logMAR. Statistical analysis involves t-tests, analysis of variance, and Chi-squared tests. Results: 215 subjects were included in the statistical analysis (109 in the trial group and 106 in the control group). In both groups, compared to baseline data, the uncorrected visual acuity (UCVA) improved significantly, with SE reduced and central corneal curvature flattened greatly after wearing OrthoK lens (P < 0.05 for all). Compared to the control group, the trial group exhibited a higher successful rate in correcting UCVA (93.6 % vs. 84.0 %, P = 0.03) and slightly better correction in refraction (77.1 % vs. 66.0 %, P = 0.07) at 1-month follow-up. However, no significant differences were observed in the axial length elongation, corneal changes, or the incidence of adverse events between the two groups. Conclusion: These findings indicate the higher efficiency and slightly better performance in correcting myopia and improving UCVA of computer-aided lens fitting approach compared to the traditional one, but similar outcomes in controlling axial elongation.
