Higher IR and LBR and lower MPR may be achieved by selection of optimal embryo types for patients of different ages. Two high-quality embryos need to be transferred in women older than 38 years. For women aged 36 to 37 years, 1 high-quality embryo or 1 high-quality plus 1 fair-quality embryo should be singled out for transfer. For women younger than 35 years, a single high-quality embryo should be selected for transfer.

[Frozen-Thawed Embryo Transfer (FET) Versus Fresh Embryo Transfer in Clinical Pregnancy Rate During in Vitro Fertilization-Embryo Transfer].

Abstract:OBJECTIVE:To investigate whether frozen-thawed embryo transfer (FET) instead of fresh embryo transfer for the first embryo transfer (ET) in a controlled ovarian hyperstimulation (COH) cycle improves the in vitro fertilization-ET clinical pregnancy rate. METHODS:The investigators retrospectively analyzed the clinical data of 1341 IVF-ET cycles under COH, including 1169 fresh ET cycles (Group T1) and 172 FET cycles as the first embryo transfer in COH cycle (Group C1). Logistic regression was used to model the probability of clinical pregnancy rate based on age, ET method and other cycle-specific factors. RESULTS:The clinical pregnancy rate was significantly higher in Group C1 than in Group T1 (63.4% vs 49.4%, P < 0.01). Multivariate logistic regression analysis revealed significant associations between the number of high-quality embryos transferred, patient age and ET method in clinical pregnancy rate. After adjusting for the number of high-quality embryos transferred and age, the clinical pregnancy rate remained significantly higher in Group C1 than in Group T1 (odds ratio: 1.75, 95% confidence interval: 1.25 - 2.46; P < 0.01). Significant differences were found in age (< 35 years old) and the number of retrieved oocytes (> or = 15). CONCLUSION:Using FET instead of fresh embryo transfer can improve the clinical pregnancy rate and the efficiency of IVF-ET.
Predictive Value of the Number of Frozen Blastocysts in Live Birth Rates of the Transferred Fresh Embryos

Abstract:Abstract Background Blastocyst development by extended culture in vitro allows the embryos to ‘select’ themselves, thus successful growth to the blastocyst stage is a reflection of the developmental competence of cleavage stage embryos in a cohort. The study aims to determine whether the number of frozen blastocysts is associated with live birth rates of the transferred fresh embryos. Method The retrospective study included 8676 cycles of first fresh embryo transfer from January 2016 to June 2019 at a fertility center of a university hospital. The patients with ≥ 10 oocytes retrieved were divided into three groups according to the number of frozen blastocysts: 0 (group 1), 1–2 (group 2), and ≥ 3 (group 3). The primary outcome measure was the live birth. The secondary outcome measures included clinical pregnancy rates and implantation rates. Logistic regression analysis was also performed. Results Live birth rates in patients with ≥ 3 and 1–2 frozen blastocysts were 47.6% and 46.1%, respectively, which were significantly higher than that in patients without blastocyst (36.0%). The clinical pregnancy rate in group 3 was 65.1%, which was also significantly higher than the other two groups (47.0% and 59.2%). The implantation rates were 35.5%, 47.6%, and 56.0% in the three groups, respectively (P < 0.001). Compared with groups of frozen blastocysts, 0 frozen blastocyst yielded a lower rate of live birth (the adjusted odds ratio: 0.526, 95% CI: 0.469, 0.612). Conclusion In patients with optimal ovarian response that retrieved ≥ 10 oocytes, fresh embryos transfer followed by having blastocysts frozen is a strong indicator of pregnancy achievement, but the number of frozen blastocysts (if not = 0) has limited value in predicting live birth rates.

Abstract:BACKGROUND:Elective single embryo transfer (eSET) has been increasingly advocated, but concerns about the lower pregnancy rate after reducing the number of embryos transferred have encouraged transfer of multiple embryos. Extended embryo culture combined with electively freezing all embryos and undertaking a deferred frozen embryo transfer might increase pregnancy rate after eSET. We aimed to establish whether elective frozen single blastocyst transfer improved singleton livebirth rate compared with fresh single blastocyst transfer.METHODS:This multicentre, non-blinded, randomised controlled trial was undertaken in 21 academic fertility centres in China. 1650 women with regular menstrual cycles undergoing their first cycle of in-vitro fertilisation were enrolled from Aug 1, 2016, to June 3, 2017. Eligible women were randomly assigned to either fresh or frozen single blastocyst transfer. The randomisation sequence was computer generated, with block sizes of two, four, or six, stratified by study site. For those assigned to frozen blastocyst transfer, all blastocysts were cryopreserved and a delayed frozen-thawed single blastocyst transfer was done. The primary outcome was singleton livebirth rate. Analysis was by intention to treat. This trial is registered at the Chinese Clinical Trial Registry, number ChiCTR-IOR-14005405.FINDINGS:825 women were assigned to each group and included in analyses. Frozen single blastocyst transfer resulted in higher rates of singleton livebirth than did fresh single blastocyst transfer (416 [50%] vs 329 [40%]; relative risk [RR] 1·26, 95% CI 1·14-1·41, p<0·0001). The risks of moderate or severe ovarian hyperstimulation syndrome (four of 825 [0·5%] in frozen single blastocyst transfer vs nine of 825 [1·1%] in fresh single blastocyst transfer; p=0·16), pregnancy loss (134 of 583 [23·0%] vs 124 of 481 [25·8%]; p=0·29), other obstetric complications, and neonatal morbidity were similar between the two groups. Frozen single blastocyst transfer was associated with a higher risk of pre-eclampsia (16 of 512 [3·1%] vs four of 401 [1·0%]; RR 3·13, 95% CI 1·06-9·30, p=0·029).INTERPRETATION:Frozen single blastocyst transfer resulted in a higher singleton livebirth rate than did fresh single blastocyst transfer in ovulatory women with good prognosis. The increased risk of pre-eclampsia after frozen blastocyst transfer warrants further studies.FUNDING:The National Key Research and Development Program of China.

Abstract:Are there differences in pregnancy outcomes when comparing frozen embryo transfer versus fresh embryo transfer among aged women? Frozen embryo transfer yields lower live birth rate for women aged 35-39, but higher rate for women ≥40 compared to fresh embryo transfer. Advanced maternal age was defined as over 35 years old, while recent literature lifted the threshold to 40. An increasing number of women are postponing childbirth, but reproductive issues become more common after the age of 35, with a significant decline in fertility, particularly after 40, even with IVF. Advanced age affects pregnancy rates, prompting some clinicians to prefer fresh embryo transfer to achieve pregnancy quickly. However, it may be less effective with potential negative effects of ovarian stimulation on endometrial receptivity. Frozen embryo transfer is seen as a better strategy, but data specific to advanced maternal age is lacking. This cohort study analyzed the Human Fertilization and Embryology Authority anonymized data from 2011 to 2018 in the United Kingdom. This comprised of a total of 232,942 cycles with advanced maternal age, in which 65,066 patients undergoing frozen embryo transfer cycles, and 167,876 patients undergoing fresh embryo transfer cycles. Data on women with advanced maternal age undergoing IVF/ICSI cycles were analyzed to compare live birth rate and miscarriage rate. The cycles with patients’ age <35, donor oocyte / embryo, PGT, or without embryo to transfer were excluded. Log-binominal regression analysis was performed adjusting for patient age, cause of infertility, number of embryos transferred, year of treatment, and IVF or ICSI cycle. We analyzed data from 232,942 cycles with patients age ≥35, and found that FET cycles exhibited a higher live birth rate than that of fresh embryo transfer cycles (25.5% vs. 24.9%, RR, 1.03; 95% CI, 1.01–1.123). Miscarriage rate (4.6% vs. 3.3%, RR, 1.41; 95% CI, 1.35–1.48) was also different. However, the adjusted RR (aRR) of live birth rate was 0.91; 95% CI, 0.88–0.93, after adjusting for potential confounders. For those patients aged 35-39, live birth rate (28.4% vs. 29.7%, aRR, 0.86; 95% CI, 0.83–0.88) following FET cycles was lower than that of fresh embryo transfer cycles. While FET cycles showed a higher rate of miscarriage (4.6% vs. 3.2%, aRR, 1.83; 95% CI, 1.70–1.98). When patients age≥40, FET cycles showed a higher rate of live birth (18.2% vs. 14.0%, aRR, 1.24; 95% CI, 1.16–1.32) and miscarriage rate (4.7% vs. 3.4%, aRR, 1.72; 95% CI, 1.52–1.94). Besides the live birth rate was sharply decreased than patients aged 35-39 both in FET and fresh embryo transfer cycles. The conclusion of the present study might be limited by the anonymous nature of the dataset. Further randomized trials may be required to provide direct evidence for the best embryo transfer strategy for the advanced age women. The study shows that FET exhibited a higher live birth rate in women aged ≥ 40, but lower for those aged 35-39 than fresh embryo transfer. Additionally, FET demonstrated a higher miscarriage rate. Therefore, it is necessary to tailor the embryo transfer strategy for advanced maternal age women. not applicable
Clinical outcomes after fresh versus frozen embryo transfer in women with advanced reproductive age undergoing in vitro fertilization: a propensity score-matched cohort study

Abstract:This retrospective cohort study aimed to compare clinical outcomes following fresh or frozen embryo transfer (FET) in women with advanced reproductive age (ARA). Women aged 35-45 years who underwent their first autologous fresh or frozen cleavage stage embryo transfer cycle in the Centre for Assisted Reproduction of Shanghai First Maternity and Infant Hospital between January 2016 and December 2020 were included. The primary outcome was live birth after the first embryo transfer of the in vitro fertilization (IVF) cycle. Multiple covariates were used for propensity score matching (PSM) and generalized estimating equations were performed to examine the independent association between FET and live birth. Of the total 1453 patients, 327 patients had FET and 1126 patients had fresh ET. After the PSM procedure, 274 patients were included in each group. The live birth rate was 24.8% in the FET group and 25.2% in the fresh ET group (OR 0.98, 95% CI: 0.67-1.44, P = 0.92). Other pregnancy, perinatal and neonatal outcomes were all comparable between the two groups. This study showed that FET did not improve live birth and other clinical outcomes as compared with fresh embryo transfer in women with ARA who underwent their first IVF cycle.
Fresh vs. frozen embryo transfer: new approach to minimize the limitations of using national surveillance data for clinical research

Abstract:Objective To assess the benefit of frozen vs. fresh elective single embryo transfer using traditional and novel methods of controlling for confounding. Design Retrospective cohort study using data from the National Assisted Reproductive Technology Surveillance System. Setting Not applicable. Patient(s) A total of 44,750 women aged 20–35 years undergoing their first lifetime oocyte retrieval and embryo transfer in 2016–2017, who had ≥4 embryos cryopreserved. Intervention(s) Fresh elective single embryo transfer and frozen elective single embryo transfer. Main Outcome Measure(s) The primary outcome was a singleton live birth. Secondary outcomes included rates of total live birth (singleton plus multiple gestations), twin live birth, clinical intrauterine gestation, total pregnancy loss, biochemical pregnancy, and ectopic pregnancy. Outcomes for infants included gestational age at delivery, birth weight, and being small for gestational age. Result(s) The eligibility criteria were met by 6,324 fresh and 2,318 frozen cycles. Patients undergoing fresh and frozen transfer had comparable mean age (30.69 [standard deviation {SD} 0.08] years vs. 31.06 [SD 0.08] years) and body mass index (24.76 [SD 0.20] vs. 25.65 [SD 0.15]); however, women in the frozen cohort created more embryos (8.1 [SD 0.12] vs. 6.8 [SD 0.08]). Singleton live birth rates in the fresh vs. frozen groups were 51.4% vs. 48.8% (risk ratio 1.05; 95% confidence interval [CI], 1.00–1.10). After adjustment with a log-linear regression model and propensity score analysis, the difference in singleton live birth rates remained nonsignificant (adjusted risk ratio, 1.05; 95% CI, 0.97–1.14 and 1.02; 95% CI, 0.96–1.08, respectively). A novel dynamical model confirmed inherent fertility (probability of ever achieving a pregnancy) was balanced between groups (odds ratio, 1.23; 95% CI 0.78–1.95]). The per-cycle probability of singleton live birth was not different between groups (odds ratio 1.11 [95% CI 0.94–1.3]). Conclusion(s) In this retrospective cohort study of fresh vs. frozen elective single embryo transfer, there was no statistically significant difference in singleton live birth rate after adjustment using log-linear models and propensity score analysis. The successful application of a novel dynamical model, which incorporates multiple assisted reproductive technology cycles from the same woman as a surrogate for inherent fertility, offers a novel and complementary perspective for assessing interventions using national surveillance data.

Inductive perturbation tolerance improvement of hybrid networks using an integrated expert system

Abstract:The design and integration of an expert system is presented to solve the instability and unreliability problems caused by exogenous perturbations in hybrid networks. The proposed expert system performs a directional induction technique on the state vectors of the storage units and linearizes the non-linear dynamics on input and link voltages and currents based on the frequency of dispatch, state of charge, depth of discharge, and utilization factors of storage units. The expert system then identifies optimal, real-time control action on the power variables of the storage units that satisfies various performance and reliability targets. Results show that the expert system based inductive perturbation tolerance approach could provide improvements leading to an optimal performance improvement and enables the hybrid network to satisfy system reliability criteria by compensating for dynamic and transient disturbances as compared to other static and dynamic approaches such as heuristic-based, fitness-based or prediction-based techniques.
Single versus double blastocyst transfer in first and second frozen-thawed embryo transfer cycle in advance-aged women: a two-center retrospective cohort study

Abstract:Abstract Background The present evidence is deficient for the trade-offs between the pros and cons of single blastocyst transfer (SBT) versus double blastocyst transfer (DBT) in frozen-thawed embryo transfer cycles for women in advanced reproductive age, especially in the second cycle. The current study aimed to investigate the impact of transferred blastocyst numbers on pregnancy outcomes in the first and second embryo transfer for women ≥ 35 years. Methods This was a retrospective cohort study including 1284 frozen-thawed blastocyst transfer (FBT) cycles from two reproductive centers. We analyzed the pregnancy outcomes after SBT and DBT in the first and second FBT cycles. Moreover, stratified analysis was conducted by maternal age. Results In the first FBT cycle, the LBR was higher in the DBT group than that in the SBT group [52.3% vs. 33.9%; adjusted odds ratio (aOR), 1.65; 95% confidence interval (CI), 1.26–2.15, P < 0.001]. However, the LBR of the DBT group showed no remarkable difference compared with that of the SBT group in the second cycle of FBT (44.3% vs. 33.3%; aOR, 1.30; 95% CI, 0.81–2.08; P = 0.271). Furthermore, stratified analysis by age showed a higher LBR for the DBT group than the SBT group in patients aged 38–42 years (43.1% vs. 33.9%; aOR, 2.27; 95% CI, 1.05–4.90; P = 0.036). Conclusions The present study demonstrated that the SBT regimen is a better choice for both, the first and second frozen-thawed embryo transfer cycles, for women aged 35–37 years. Additionally, the DBT regimen is still recommended to achieve a high LBR in women aged 38–42 years in the second FBT cycle. These findings may be beneficial for deciding the embryo transfer regimens in women of advanced reproductive age.

Predicting the probability of a live birth after a freeze-all based in vitro fertilization-embryo transfer (IVF-ET) treatment strategy

Abstract:Background: The predictors for live birth rate (LBR) following one episode of in vitro fertilization (IVF) cycle for patients using a "freeze-all" strategy are not entirely clear. Methods: A retrospective cohort study utilizing a prediction model was developed to assess the relationship to the LBR. Women undergoing IVF with a freeze-all strategy were screened. Univariate models were first fitted for female age at oocytes retrieval/frozen-thawed embryo transfer (FET), body mass index (BMI), duration and etiology of infertility, previous IVF failures, total dose and duration of gonadotrophin, ovarian sensitivity index (OSI), number of oocytes collected, method of fertilization, number of embryos created, number and stage of embryos frozen, type and number of FET cycles, endometrial thickness (EMT)/pattern, hormone level on transplantation day, storage duration, number of embryos thawed and damaged thawed embryos, number and stage of embryos transferred and number of different quality embryos transferred. Variables with P<0.05 in the univariate model were selected for further analysis of the final multivariate discrete-time logistic regression model. Results: A total of 7,602 women undergoing one ovarian stimulation resulted in 9,964 FETs, of whom 3,066 (40.33%) had a live-birth after their first FET and 3,929 (51.68%) after total FETs. The EMT and woman's age at oocyte retrieval were the most important predictors. In the first FET, the LBR of women with an EMT ≤8 mm [27.40%; 95% confidence interval (CI): (21.60-33.81%)] was significantly lower than that of women with EMT between 9 and 11 mm [36.51%; 95% CI: (34.25-38.81%)] and thicker than 12 mm [44.23%; 95% CI: (42.22-46.25%)] (P<0.05). The optimistic and conservative cumulative LBRs of women younger than 31 years [87.5%; 95% CI: (86.32-88.61%) and 63.04%; 95% CI: (61.36-64.69%)] were significantly decreased in women aged 31-35, 36-40 and >40 (P<0.001). Conclusions: Our study provides an effective prediction model for a woman's chance of having a baby after a "freeze-all" policy. The use of EMT and female age as tools to identify LBR are shown to be justified, and repeated FETs cannot reverse the age-dependent decline in fertility.

Abstract:Introduction: The utilization of frozen embryo transfer not only enhances reproductive outcomes by elevating the likelihood of live birth and clinical pregnancy but also improves safety by mitigating the risks associated with ovarian hyperstimulation syndrome (OHSS) and multiple pregnancies. There has been an increasing debate in recent years regarding the advisability of making elective frozen embryo transfer the standard practice. Our study aims to determine the optimal choice between fresh and frozen embryo transfer, as well as whether the transfer should occur at the cleavage or blastocyst stage. Method: In this retrospective cohort study conducted in Taiwan, data from the national assisted reproductive technology (ART) database spanning from January 1st, 2013, to December 31st, 2017, were analyzed. The study included 51,762 eligible female participants who underwent ART and embryo transfer. Pregnancy outcomes, maternal complications, and singleton neonatal outcomes were evaluated using the National Health Insurance Database from January 1st, 2013, to December 31st, 2018. Cases were categorized into groups based on whether they underwent fresh or frozen embryo transfers, with further subdivision into cleavage stage and blastocyst stage transfers. Exposure variables encompassed clinical pregnancy rate, live birth rate, OHSS, pregnancy-induced hypertension, gestational diabetes mellitus (DM), placenta previa, placental abruption, preterm premature rupture of membranes (PPROM), gestational age, newborn body weight, and route of delivery. Results: Frozen blastocyst transfers showed higher rates of clinical pregnancy (CPR) and live births (LBR) compared to fresh blastocyst transfers. Conversely, frozen cleavage stage transfers demonstrated lower rates of clinical pregnancy and live birth compared to fresh cleavage stage transfers. Frozen embryo transfers were associated with reduced risks of OHSS but were linked to a higher risk of pregnancy-induced hypertension compared to fresh embryo transfers. Additionally, frozen embryo transfers were associated with a higher incidence of large for gestational age infants and a lower incidence of small for gestational age infants. Conclusion: The freeze-all strategy may not be suitable for universal application. When embryos can develop to the blastocyst stage, FET is a favorable choice, but embryos can only develop to the cleavage stage, fresh embryo transfer becomes a more reasonable option.

Abstract:Abstract Objective To investigate whether the morphologic parameters of euploid blastocyst influence the live birth rate (LBR) following single frozen-thawed embryo transfer (FET) cycles? Methods A retrospective cohort analysis involving autologous single FET cycles after preimplantation genetic testing for aneuploidy (PGT-A) through next generation sequencing (NGS) by a university-based reproductive medical center that was performed from June 2017 to September 2019.Women were divided into three age groups (< 30, 30–34 and ≥ 35 years). The primary outcome measure was LBR. Outcomes were compared to determine the association between different blastocyst quality (Good, Average and Poor), inner cell mass (ICM) grade (A and B), and trophectoderm (TE) grade (A, B and C) and LBR. Results We included 232 single FET cycles for analysis, the total LBR was 48.48%. In the youngest group (< 30 years, n = 86), LBR were compared between cycles with various blastocyst quality (72.22% for good quality, 54.55% for average quality and 34.78% for poor quality; P = 0.019), ICM grade (70.59% for grade A and 42.03% for grade B; P = 0.035) and TE grade (85.71% for grade A,57.58% for grade B and 34.78 for grade C; P = 0.015). Similarly, in the 30–34 years group, LBR ranged from 50.00% for good quality to 45.45% for poor quality ( P = 0.870), from 35.29% for ICM grade A to 51.22% for ICM grade B ( P = 0.291), from 60.00% for TE grade A to 45.45% for TE grade C ( P = 0.634). Likewise, in the oldest group (≥ 35years, n = 47), LBR were also comparable between these subgroups, but no significant differences were seen in blastocyst morphologic parameters and LBR ( P > 0.05). Conclusion After single FET cycles, the LBR was associated with morphologic parameters of euploid blastocysts, especially in women < 30 years old. However, these differences were not found in women older than 30 years. We suggested that for older women whose embryos undergoing PGT-A with NGS to be euploid have the same development potential regardless of their blastocyst morphology.

Abstract:(Abstracted from N Engl J Med 2018;378:126–136) A previous randomized controlled trial involving women with polycystic ovary syndrome (PCOS) compared fresh-embryo transfer with elective freezing of all embryos followed by frozen-embryo transfer. The data showed that frozen-embryo transfer had a higher live-birth rate in these women compared with fresh-embryo transfer. Whether frozen-embryo transfer also increases live-birth rates among ovulatory women with infertility without PCOS is uncertain.

Abstract:INTRODUCTION: Different transfer techniques have been developed to optimize the embryo transfer process and maximize the probability of pregnancy after transfer. The purpose of this study is to compare the afterload (AL) embryo transfer technique and the post-mock (PM) embryo transfer technique. METHODS: We designed a retrospective cohort study which included women who underwent an autologous, fresh or cryopreserved embryo transfer (ET) between 01 July 2013 and 01 September 2016 at our academic center. Embryo transfer was performed with either the afterload technique (AL) or the immediate post-mock (PM) transfer technique. The primary outcome of interest was ongoing pregnancy rate (OPR) defined as fetal cardiac activity on first trimester ultrasound. This study was approved by the Tripler Army Medical Center Department of Clinical Investigation Scientific Review Committee and Institutional Review Board. RESULTS: There was no significant difference in OPR between patients that underwent AL transfers and PM transfers (AL 51%, PM 57%; P=.378). Ongoing pregnancy rate for frozen embryo transfers was superior for AL transfers compared to PM transfers (AL 68%, PM 31%; P=.005). Ongoing pregnancy rate was superior with the PM technique for fresh embryo transfers (AL 43%, PM 63%; P=.002). Ongoing pregnancy rate decreased with age in the AL cohort, but not in the PM cohort (P=.004). CONCLUSION: There appears to be no overall difference in OPR between AL and PM transfer technique. However, with vitrified embryos, the AL technique demonstrated a higher OPR, and with fresh embryos, the PM technique resulted in a superior OPR.

Abstract:RESEARCH QUESTION Are the cumulative live birth rates following a freeze-all strategy compared with a fresh transfer strategy including all subsequent frozen-thaw cycles from the same oocyte retrieval similar? DESIGN Secondary outcome in a multicentre randomised controlled trial including 460 women allocated in a 1:1 ratio to (1)Freeze-all strategy including GnRH agonist trigger and single vitrified-warmed blastocyst transfer in a subsequent modified-natural FET cycle or (2)Fresh transfer strategy with hCG trigger and single blastocyst in the fresh cycle. Women were recruited over a 2-year period from May 2016 to September 2018. The minimum follow-up time from the start of ovarian stimulation was 2 years. RESULTS Baseline and treatment-related characteristics were similar between the two groups and equal number of patients had additional frozen-thawed embryo transfers following the initial fresh or frozen transfer in the two groups. Combining all fresh and frozen transfers from the included oocyte retrieval, the cumulative live birth rate was 42.6% (95/223) in the Freeze-all group versus 41.7% (96/230) in the Fresh strategy group (risk ratio (RR): 1.0;95% CI: 0.87-1.19, P=0.93). The mean time to the first pregnancy was 125.8±60.8 versus 82.74 ± 85.9 days in the Freeze all and Fresh transfer group, respectively. The total number of deliveries from all subsequent FET cycles was similar between the freeze all strategy and fresh transfer strategy group, respectively: 15.2%(34/223) versus 12.6%(29/239), p=0.5. CONCLUSIONS When comparing a freeze-all strategy with a fresh transfer strategy we find no significant difference in cumulative live birth rates between the two strategies in a randomised controlled trial setting.

Abstract:Background In frozen–thawed embryo transfer (FET) cycles, endometrial thickness (EMT) has been used routinely as the main clinical monitoring index. However, the current findings are conflicting. Method This was a single-center retrospective study of 2,054 couples (2,562 cycles) who underwent FET (including cleavage stage embryos and blastocysts) between January 2017 and August 2020 in the reproductive centers of First Affiliated Hospital of Xinjiang Medical University. The primary outcome measure was the ongoing pregnancy rate (OPR); the secondary outcome was the clinical pregnancy rate. Results After stratified analysis and adjusting for confounders such as maternal age, duration of infertility, number of high-quality embryos transferred, endometrial preparation protocol, number of transfer cycles, and stages of embryo transferred, we found a curvilinear relationship between EMT and the OPR in women < 35 years of age. For women with EMT ≤ 8 mm, the OPR increased by 150% for cleavage stage embryo transfer for every 1 mm increase in the EMT; similarly, it increased by 97% for blastocyst stage FET. However, there was a linear relationship between EMT and OPR in women aged ≥ 35 years. When blastocysts were transferred, for every 1 mm increase in the EMT the OPR increased significantly by 12%. But OPR after frozen-thawed cleavage stage embryos transfer did not increase significantly with increased EMT Conclusions Our study showed that the OPR increased significantly with increased EMT between young women aged < 35 years with EMT ≤ 8 mm and older women who underwent transfer of blastocysts.

Abstract:Background and objectives: In contemporary society, socially active women are increasingly planning their fertility for later in life. The fertility outcomes for advanced-age patients, even with egg donation, are often suboptimal due to endometrial aging. Recurrent implantation failure (RIF) is one of the core problems for assisted reproductive technology (ART), especially for advanced-age patients. High-quality, euploid embryos and synchronization between the embryonic stage and the uterine endometrial lining are crucial for positive outcomes. The study aims to improve ART outcomes with personalized embryo transfer (pET) according to endometrial receptivity analysis (ERA) in advanced-age patients with challenging reproductive histories, and RIF by utilizing, donor oocytes and preimplantation genetic testing for aneuploidy (PGT-A) for embryo testing. Methods: A randomized, controlled observational follow-up study was conducted from 2020 to 2023. After obtaining informed consent, 320 patients with RIF were selected. Patients were allocated into the study group and control group 1 based on consistent application of randomization principles, while control group 2 was selected separately. The study group included patients undergoing PGT-A and ERA, aged 35-45 years, with a mean age of 40.5±3.7 years. Control group 1 comprised patients undergoing PGT-A, aged 35-45 years, with a mean age of 40±4.2 years. Control group 2 consisted of patients undergoing PGT-A and ERA, aged less than 35 years, with a mean age of 31.6±2.2 years. Results: Results suggest that ERA may improve implantation and pregnancy outcomes in advanced-age patients, particularly those with RIFs. The pregnancy rate was significantly higher in the study group (77.9%), compared to control group 1 (57.6%) (p=0.0007), and no significant difference compared to control group 2 (77.3%) (p=0.94). The implantation rate was higher in the study group (54.1%) than in control group 1 (39.4%) (p=0.0009), and there was no significant difference between the study group and control group 2 (50%, p=0.87). The live birth rate was also higher in the study group (71.3%), compared to control group 1 (39.4%) (p<0.0001). There were no significant differences between the study group and control group 2 (65.9%, p=0.50). Conclusion: pET guided by ERA significantly improves pregnancy, implantation, and live birth rates in advanced-age patients with challenging reproductive histories. pET provides ART outcomes with no significant difference between advanced-age patients and younger patients with pET guided by ERA.

Abstract:BACKGROUND: Elective freezing of all good quality embryos and transfer in subsequent cycles, i.e. elective frozen embryo transfer (eFET), has recently increased significantly with the introduction of the GnRH agonist trigger protocol and improvements in cryo-techniques. The ongoing discussion focuses on whether eFET should be offered to the overall IVF population or only to specific subsets of patients. Until recently, the clinical usage of eFET was supported by only a few randomized controlled trials (RCT) and meta-analyses, suggesting that the eFET not only reduced ovarian hyperstimulation syndrome (OHSS), but also improved reproductive outcomes. However, the evidence is not unequivocal, and recent RCTs challenge the use of eFET for the general IVF population.OBJECTIVE AND RATIONALE: This systematic review and meta-analysis aimed at evaluating whether eFET is advantageous for reproductive, obstetric and perinatal outcomes compared with fresh embryo transfer in IVF/ICSI cycles. Additionally, we evaluated the effectiveness of eFET in comparison to fresh embryo transfer in different subgroups of patients undergoing IVF/ICSI cycles.SEARCH METHODS: We conducted a systematic review, using PubMed/Medline and EMBASE to identify all relevant RCTs published until March 2018. The participants included infertile couples undergoing IVF/ICSI with or without preimplantation genetic testing for aneuploidy (PGT-A). The primary outcome was the live birth rate (LBR), whereas secondary outcomes were cumulative LBR, implantation rate, miscarriage, OHSS, ectopic pregnancy, preterm birth, pregnancy-induced hypertension, pre-eclampsia, mean birthweight and congenital anomalies. Subgroup analyses included normal and hyper-responder patients, embryo developmental stage on the day of embryo transfer, freezing method and the route of progesterone administration for luteal phase support in eFET cycles.OUTCOMES: Eleven studies, including 5379 patients, fulfilling the inclusion criteria were subjected to qualitative and quantitative analysis. A significant increase in LBR was noted with eFET compared with fresh embryo transfer in the overall IVF/ICSI population [risk ratio (RR) = 1.12; 95% CI: 1.01-1.24]. Subgroup analyses indicated higher LBRs by eFET than by fresh embryo transfer in hyper-responders (RR = 1.16; 95% CI: 1.05-1.28) and in PGT-A cycles (RR = 1.55; 95% CI: 1.14-2.10). However, no differences were observed for LBR in normo-responders (RR = 1.03; 95% CI: 0.91-1.17); moreover, the cumulative LBR was not significantly different in the overall population (RR = 1.04; 95% CI: 0.97-1.11). Regarding safety, the risk of moderate/severe OHSS was significantly lower with eFET than with fresh embryo transfer (RR = 0.42; 95% CI: 0.19-0.96). In contrast, the risk of pre-eclampsia increased with eFET (RR = 1.79; 95% CI: 1.03-3.09). No statistical differences were noted in the remaining secondary outcomes.WIDER IMPLICATIONS: Although the use of eFET has steadily increased in recent years, a significant increase in LBR with eFET was solely noted in hyper-responders and in patients undergoing PGT-A. Concerning safety, eFET significantly decreases the risk of moderate and severe OHSS, albeit at the expense of an increased risk of pre-eclampsia.
