Abstract:Background After tooth loss, alveolar bone atrophy and pneumatization of the maxillary sinus occur. It is difficult for implant placement to posterior maxilla because of the reduced residual bone height (RBH) available. Osteotome sinus floor elevation (OSFE) is a reliable technique and produced satisfied results. Compared with oral panoramic radiograph (OPG), the application of cone beam computed tomography (CBCT) that provides clinically valuable information is considered to be more accurate assessment. Aim/Hypothesis The purpose of this study was to compare radiographically the variances of measurements on OPG and CBCT, and to evaluate the effects of endo-sinus new bone formation after OSFE without grafting in a short time. Simultaneously, to analysis the potential related factors of endo-sinus bone gain (ESBG). Material and Methods A prospective study design was adopted. 87 implant placements were inserted in 80 patients. OPG and CBCT were used at preoperative, immediately in postoperative period and before stage II surgery (mean 5-month postoperative). The RBH was measured at preoperative and before stage II surgery, the implant protrusion length (IPL) was assessed at immediately postoperative and before stage II surgery, ESBG was reviewed before stage II surgery. Compare the RBH, IPL, ESBG between two groups from OPG and CBCT. Correlation analysis was conducted to explore the relationship between ESBG with IPL, RBH, age, gender and bone block lifted intraoperatively by osteotomes. Significance was set to 0.05. Results The mean RBH was 8.47 ± 1.78 mm on OPG and 8.08 ± 1.47 mm on CBCT preoperatively. After mean 5 months, it was found the averaged RBH was 9.58 ± 1.67 mm on OPG and 10.59 ± 1.51 mm on CBCT. The mean IPL was 2.44 ± 1.16 mm on OPG and 1.98 ± 1.09 mm on CBCT at immediately postoperative. The averaged IPL was 1.27 ± 1.30 mm on OPG and 0.05 ± 1.33 mm on CBCT before the stage II surgery, respectively. ESBG was 1.10 ± 0.76 mm on OPG and 1.90 ± 1.38 mm on CBCT. And there were all significant differences between two groups (P < 0.05). As shown by the numerical value, ESBG with bone block lifted were more than with no bone block lifted, but P value > 0.05 that indicating the difference was again not statistically significant difference between two groups. The relevant factors analysis detected that the final ESBG was positively correlated to IPL at immediately postoperative. Meanwhile, no other variables were detected showed to be associated with the final ESBG, including RBH, age, gender and bone block lifted. Conclusion and Clinical Implications In conclusion, compared with the OPG of 2D image, the errors of measurement could be reduced and the results could be assessed more accurately on CBCT. Approximate 2 mm endo-sinus bone augment could be acquired, and the final ESBG was explored to be positively correlated to IPL measured at immediately postoperative. Bone block lifted intraoperatively by osteotomes does not increase the amount of endo-sinus bone formation.
Changes in Bone Graft Height and Influencing Factors after Sinus Floor Augmentation by Using the Lateral Window Approach: A Clinical Retrospective Study of 1 to 2 Years.

Abstract:STATEMENT OF PROBLEM:Recent systematic reviews have reported resorption of bone grafts after augmentation, but the influencing factors are numerous and uncertain. Different brands of bone graft and other factors may affect the bone formation effect after sinus floor augmentation.PURPOSE:The purpose of this retrospective clinical study was to evaluate the graft material height changes after sinus floor augmentation with cone beam computed tomography (CBCT) and to investigate the potential influencing factors related to graft resorption.MATERIAL AND METHODS:Four midsagittal cut CBCT images of 157 posterior maxillary implants after maxillary sinus floor elevation by using the lateral window approach with bone grafts from 116 patients were obtained. Four CBCT scans had been performed immediately (T0), 6 months (T1) and 12 months after bone grafting (T2), and 1.5 to 2 years after treatment (T3), and the distance between the implant platform and the grafted mucosa of the maxillary sinus floor was measured at 3 locations. Correlation coefficients of these parameters were calculated. A linear mixed model was used to investigate potential factors influencing graft height reduction, including the patient's sex, age, smoking status, periodontal status, graft brand, implant site, implant level, placement time, and residual bone height.RESULTS:Mean ±standard deviation graft height at the mesial side of the implant (MeGH) was significantly decreased by 0.32 ±0.88 mm in the first 6 months, was gradually reduced after 6 months (0.26 ±0.43 mm), and then 1 to 2 years later increased to 0.39 ±0.97 mm. The changes in graft height at the center of the implant (CeGH) and graft height at the distal side (DiGH) groups were similar to those in the MeGH group. A significantly positive correlation was found among the height alterations in 3 locations (rs=0.954, P<.001). The linear mixed model showed that smoking was more likely to cause graft height reduction in MeGH (P=.034).CONCLUSIONS:Graft height significantly decreased after maxillary sinus augmentation over 1 to 2 years. Smoking had a negative effect on graft height changes, while the 2 brands of graft and other factors had no significant effect.

Abstract:AimRecent studies claim that haemostatic agents can be used as bone graft substitutes. The aim of this study was to compare the efficacy of alloplastic bone graft with absorbable gelatin sponge in prevention of periodontal defects distal to mandibular second molar after the surgical removal of impacted mandibular third molars.Materials and methodsA prospective, randomized, single-blind split-mouth study was designed. The study consisted of 25 patients requiring surgical removal of bilateral impacted mandibular 3rd molars. The surgical sites were randomly divided into 2 groups: group I: G-graft (hydroxyapatite + collagen, study group) and group II: Abgel (absorbable gelatin sponge, control group). Patients were recalled on lst and 7th postoperative days and 3rd and 6th postoperative months. Probing depth, alveolar bone levels and soft tissue wound healing were evaluated. Paired t test was used to compare pre and post-operative alveolar bone levels and probing depth (PD). Wilcoxon signed ranks test was used to compare the wound healing.ResultsThe soft tissue wound healing, PD and the distance between the cemento–enamel junction on the distal aspect of mandibular second molar (point A) and the alveolar crest on the distal aspect of the same tooth (point B) were significantly higher in group I as compared to group II.ConclusionThis study reveals an increase in the alveolar bone level, improvement of PD and better wound healing in group I. Group II subjects required longer healing time than the normal. The authors disagree the claim that the haemostatic agents can be used as bone graft substitutes. However, long-term, multicenter, randomized controlled clinical trials are required.
The effect of Bioactive Hydrogel versus Advanced-PRF on bone regeneration following impacted mandibular third molar surgery. Clinical and CBCT Analysis

Abstract:Objectives: The current study aim was to evaluate the effect of Bioactive Hydrogel versus Advanced-PRF on bone regeneration following impacted mandibular third molar surgery. Material & Methods: sixty patients who had been scheduled for surgical removal of mesio-angular or horizontal impacted mandibular 3rd molar teeth were randomly divided into three groups. Group A involved 20 patients received A-PRF-Xenograft at the surgical site. Group B involves 20 patients received Xenograft-Hydrogel at the surgical site. While group C involved 20 patients left to heal without graft as control. Clinical and radiographic evaluation were performed immediate and at 6 months postoperatively. Data was collected and analysed statistically. Results: At 6-month postoperative evaluation, the current study found that A-PRF-Xenograft group showed significant shorter mean pain duration (5.1±1.4 days) compared to Hydrogel-Xenograft group (5.3±1.5 days) and control group (5.8±1.4 days). A-PRF-Xenograft group showed the lowest mean values of PPD (3.7±0.6) followed by Hydrogel-Xenograft group (5±0.8) and followed by control group (6.6±0.6). A-PRF-Xenograft group showed the highest mean values of bone density (605.3±85.5) followed by Hydrogel-Xenograft group (522.4±83.5) followed by control group (286.4±44.7) Conclusion: According to the results of the present study, there was significant improvement regarding pain duration, mouth opening limitation, periodontal pocket depth and bone density at the surgical site where A-PRF-Xenograft was applied. Hydrogel-Xenograft Group was more superior to the control group regarding pain duration, PPD, and bone density.
Comparative Clinical, Radiographical and Biochemical Evaluation of Autogenous Bone Graft in Periodontal Infrabony Defects: A Randomized Clinical Trial

Periodontitis leads to loss of the toothsupporting tissues, eventually resulting in loss of teeth. Ideal treatment should involve not only the prevention of further attachment loss, but the regeneration of lost supporting tissues, comprising new alveolar bone and a new periodontal ligament [1]. Various attempts to stimulate periodontal regeneration in the past have focused on filling the defect with some type of material, providing a space or adding growth factors to induce the host cells to form bone [2]. Over the last decades different modalities of regenerative treatment have been developed and applied clinically [3]. Among all the graft materials only autografts and Demineralized Freeze-Dried Bone Allografts (DFDBA) have demonstrated a potential to achieve periodontal regeneration [3]. Historically, autografts were the first bone replacement grafts to be reported for periodontal applications. Autogenous grafts, which are harvested from the patients’ own body, are considered the gold standard among graft materials because they are superior at retaining cell viability. These grafts contain live osteoblasts and osteoprogenitor stem cells and heal by osteogenesis. It avoids any risk of rejection or disease transmission. Autogenous bone has osteoconductive, osteoinductive and osteogenic properties [4]. The choice of an autogenous donor site is often based on the amount of bone needed. Extra-oral bone grafts are usually taken from iliac crest, the tibia, or the skull and are used when large amounts of bone are needed. These procedures result in significant morbidity and discomfort to the patient. Conversely, when smaller amount of bone is required, intraoral sites for procuring the graft has several advantages, including reduced operating time & hospitalization as well as lower morbidity and avoidance of cutaneous scarring. But, a second surgical site is still needed. However, the need for second surgical site can be avoided with the use of bone scraper, back action chisel, rongeurs or a bone mill to obtain bone during the surgical procedure [2]. Among all these, bone scraper combines the benefits of both viability and easy procurement over trephines, bone mills or roungers. This hand held instrument (bone scraper) cuts and collects thin shavings of bone from cortical surfaces. The scraperderived matrix of ribbon like bone shavings and blood has a mortal like consistency and can be easily adapted to the alveolar deficiency to reshape the alveolar crest profile [5]. This controlled clinical study, has focused on the efficacy of autogenous bone graft collected with the help of bone scraper over open flap debridement in treatment of human periodontal infrabony defects. When placed into healthy bleeding bone, the negativecharged surface of autogenous bone graft accelerated the bone-growth cascade. The separation in charge between the solid phase of bone and extracellular matrix creates a potential difference between the solid and the liquid called the zeta potential, which influences the type and nature of proteins and cells harnessed by the surface [6].

Guided bone regeneration-associated tissue swelling: A digital three-dimensional assessment

Abstract:Statement of problem: Postoperative swelling following guided bone regeneration (GBR) may affect the dimensions of interim restorations and/or delivery timing. However, quantitative assessment of post-GBR swelling or its evaluation for possible impact on regenerative outcomes is lacking. Purpose: The purpose of this prospective clinical study was to quantify post-GBR swelling and correlate it with clinical parameters and outcomes. Material and methods: Participants (n=25) undergoing standardized extraction and GBR protocol were recruited. Site-specific swelling was measured as ridge width, height, and volume changes based on intraoral scans recorded preoperatively, immediately postoperatively (IP), and at 2 days, 7 days, 14 days, and 4 months. The parameters and outcomes assessed were gingival and mucosal thickness, flap advancement, surgery duration, wound opening, and bone gain. The Friedman 2-way analysis of variance by ranks was performed, and the Spearman correlation coefficients (ρ) were computed (α=.05). Results: Ridge width and height peaked at 2 days (2.1 mm for both from IP; P<.001 for both) and reached IP levels by 7 days and 14 days (P>.999 and P=.888, respectively). At 4 months, both decreased significantly compared with IP (-4.2 mm and -1.9 mm respectively, P<.001). Volume increases peaked at 2 days (19%, P<.001), and peak swelling (2 days) preceded maximum wound opening (7 days). Bone width at 4 months was correlated with gingival thickness (ρ=0.45, P=.043), mucosal thickness (ρ=0.51, P=.021), and flap advancement (ρ=0.58, P=.008). Conclusions: Following GBR, site-specific swelling peaked on postoperative day 2 and subsided by day 7 (width) or 14 (height). Soft tissue thickness and flap advancement affected post-GBR bone width.
Comparative evaluation of demineralized freeze-dried bone allograft with and without concentrated growth factor membrane in the treatment of periodontal intrabony defects: a randomized controlled clinical trial

Abstract:Background: The current study's aim was to evaluate and compare demineralized freeze-dried bone allograft (DFDBA) with and without concentrated growth factor (CGF) membrane in the treatment of periodontal intrabony defects on both a clinical and radiographic level. Methods: 30 stage III grade B periodontitis patients with bilateral intrabony defects were involved in the split mouth randomized controlled trial, and they received either DFDBA alone (control group) or DFDBA plus CGF membrane treatment (test group). At baseline and 6 and 12 months, the gingival index (GI), plaque index (PI), probing pocket depth (PPD), clinical attachment level (CAL), and gingival recession (GR) were assessed, whereas cone beam computed tomography was used to assess the bone defect height (BDH), vertical bone loss (VBL), bone defect depth (BDD), mesio-distal bone defect width (MDBDW), bucco-lingual bone defect width (BLBDW), and bone defect volume (CBCT) at baseline and 12 months. Results: The radiographic measures BDH, BDD, MDBDW, BLBDW, and BDV in the test group likewise showed a higher reduction in PPD and gain in CAL than the control group. The results of the intergroup comparison showed that the test group had statistically significant differences in BDH, BDD, and MDBDW. Conclusion: The data show that the test group achieved better results, with greater reductions in PPD, gains in CAL, and decreases in BDH, BDD, MDBDW, BLBDW, and BDV. Clinical relevance: The use of concentrated growth factor in conjunction with demineralized freeze-dried bone allograft may be suggested for the treatment of periodontal intrabony defects.

Abstract:Background/Objectives: Various bone substitutes have been recommended to augment the horizontal gap following immediate implantation. The purpose of this study was to compare the effectiveness of an autogenous mineralized dentin graft or a deproteinized bovine bone in horizontal gap augmentation following immediate implant placement in the maxillary anterior region. Methods: A total of 110 patients underwent tooth extraction followed by immediate implant placement. The patients were divided into two groups. The first group received an autogenous mineralized dentin graft (the test group) while the second group received a deproteinized bovine bone (the control group) to augment the horizontal gap. Preoperative (T0), immediate postoperative (T1), and 1-year postoperative (T2) cone beam computed tomography scans were taken from all the patients. Linear measurements were recorded 1 mm (R1) and 5 mm (R2) points apical to the implant platform at both T1 and T2 time intervals. Pink Esthetic Scores and prosthetic complications were evaluated as well. Results: There were 57 patients with a mean age of 45.42 ± 9.86 (range 24–63 years) selected as the test group and 53 patients with a mean age of 40.28 ± 11.69 (range 20–63 years) as the control group. The mean reduction in the buccal bone plate at R1 was 6.39 ± 3.78% in the test group and 6.99 ± 5.01% in the control group (p > 0.05). The mean reduction in the buccal bone plate at R2 was 5.46 ± 4.98% in the test group and 6.77 ± 7.60% in the control group (p 0.05). Conclusions: The efficiency of using an autogenous mineralized dentin graft for horizontal gap augmentation showed similar results in comparison to using a deproteinized bovine bone in relation to buccolingual socket reduction following immediate implantation.

Abstract:Background: Bone volume affects the long term success of an implant. Augmentation of the existing edentulous alveolar bone is often necessary to obtain excellent functional and esthetic restorations of the implants. The reconstruction of alveolar ridges for implant placement is still a challenging procedure, especially in the case of extensive vertical and horizontal atrophy. Here, we aimed to evaluate the effect of EDTA bone demineralization on bone graft consolidation to the native bone in comparison to alveolar bone decortication. Methods: A total of 14 subjects were divided into two groups (n = 14) . In the test group I (n = 7), alveolar cortical bone in the area of regeneration was demineralized by 24% EDTA. While, decortication was performed in group II (n = 7). Subsequently, defects in both groups were augmented by guided bone regeneration using resorbable membrane and bovine bone. After a healing period of 6 months, trephine cores were harvested for histological and histomorphometric analysis of the grafted areas and the buccolingual width dimension was evaluated radiographically Results: Histomorphometrical analysis demonstrated that the amount of newly formed bone in the test group (3.63±1.35 %) was greater than that in group II (2.52±0.78 %), and the difference was statistically significant (P = 0.029) Conclusions: Bone demineralization results in more width gain than mechanically decorticating the alveolar bone.

Abstract:Background The cortical shell technique is frequently associated with technical drawbacks, such as the lack of anatomical guidance during shell harvest and graft. This study aims to assess the horizontal bone gain and accuracy of a digitalized protocol that incorporates two interlocking patient‐specific stackable guides (PSSGs) to control the shell harvest, positioning, and fixation. Patients and Methods Twenty patients with deficient anterior mandibles were randomly allocated; 10 patients received freehand symphyseal shell harvest and fixation (the control group), whereas the other 10 received fully guided harvest and graft (study group) using (PSSGs), the first aided an accurate shell harvest, whereas the second conveyed shell fixation. The interposition gap among both groups was loaded with an equal mix of xenogeneic and autogenous particulates. The mean radiographic bone gain among both groups was calibrated 6 months postoperatively, and the accuracy of the digital plan was assessed by superimposing and comparing the virtually planned horizontal bone dimensions with the immediate postoperative actual bone dimensions. Results The mean 6‐month postoperative horizontal bone gain value of the study group was recorded as (4.97 ± 0.73) mm versus (4.45 ± 0.61) mm for the control group, with a statistically insignificant mean gain difference of (0.52) mm, (p = 0.101). The mean virtual preplanned horizontal bone gain was recorded (5.4 ± 0.6) versus (5.4 ± 0.6) for the immediate postoperative actual bone gain, which was also statistically insignificant (p = 0.9). Conclusion The (PSSGs) provided a precise method for graft harvest, position, and fixation, resulting in satisfactory alveolar ridge dimensions with intimate accuracy.

The Influence of Residual Alveolar Bone Height on Graft Composition after Maxillary Sinus Augmentation Using Two Different Xenografts: A Histomorphometric Comparative Study

Abstract:Aim: To evaluate the hypothesis of a correlation between the preoperative residual alveolar bone height (RBH) and graft maturation after maxillary sinus floor augmentation procedures using two different bone substitutes. Methods: A total of 20 patients who underwent unilateral maxillary sinus floor augmentation with either mineralized deproteinized bovine bone (DBBM) or a xenograft enriched with polymer and gelatin (NBS) were included in this prospective study. Six months after sinus surgery, bone biopsies were harvested with a 3.2 mm diameter trephine bur, prior to dental implant placement. Histomorphometric analysis was performed, and the results were correlated with the individual RBH. Implants were loaded after 5 months of insertion, and 1-year implant success and marginal bone level change were assessed. Results: RBH was 2.17 ± 1.11 mm (range 0.5–3.5 mm) and 2.14 ± 0.72 mm (range 0.5–3.0 mm) in the NBS and DBBM group, respectively. The biopsy analyses for the DBBM group showed woven bone increases by 5.08% per 1-mm increment of RBH; medullary spaces decreased by 9.02%, osteoid decreased by 4.4%, residual biomaterial decreased by 0.34%, and lamellar bone increased by 5.68% per 1-mm increase of RBH. In the NBS group, samples showed woven bone increases by 8.08% per 1-mm increase of RBH; medullary spaces decreased by 0.38%; osteoid increased by 1.34%, residual biomaterial decreased by 0.58%, and lamellar bone decreased by 5.50% per 1-mm increase of RBH. There was no statistically significant difference in the correlation between RBH and lamellar bone, woven bone, and osteoid, independently of the material used. Implant success was 100% in both groups, and marginal bone loss was 1.02 ± 0.42 mm in DBBM and 0.95 ± 0.31 mm in the NBS group after the 1-year follow-up. Conclusion: In spite of the absence of significance, the observed trend for woven bone to increase and medullary spaces to decrease when RBH increases deserves attention. Residual bone dimension might be a determinant in the bone graft maturation after maxillary sinus augmentation.

Abstract:Introduction: Periodontitis is a chronic inflammatory disease, in which destruction of the periodontium occurs. Not well-treated periodontitis causes teeth loss and problems regarding speech and appearance. Objectives: To evaluate the effectiveness of tooth particles as a graft material in the regeneration of periodontal vertical alveolar defects clinically and regeneration of bone defect experimentally. Material and methods: A total of 21 patients with stage III severe periodontitis, aged 25 to 45 years, having periodontal vertical defects were selected for the study. They were randomly divided into 3 groups, in which split-mouth study design was used. In group 1, right side was treated by open flap debridement (OFD) alone, and left side with autogenous tooth graft particles. In group 2, control side treated by OFD, and left side using beta-tricalcium phosphate (β-TCP) graft material. In group 3, left side was treated using autogenous tooth graft, and right side us-ing β-TCP graft. For the experimental part, a total of 21 rats had an intended surgical bony defect at diastema area, and were randomly divided into 3 groups. In group 1, defects were created without addition of any graft material. In group 2, defects were filled with β-TCP graft material. In group 3, autogenous tooth particles were used. Clinical data, including plaque index (PI), gingival index (GI), periodontal probing depth (PPD), clinical attachment level (CAL), and radiographic parameters were recorded at baseline and 3 and 6 months post-surgically. For the experimental part, animals were sacrificed one month later. Tissue specimens were prepared for histological analysis. Results: After 3 and 6 months post-treatment, there was an improvement in PI and GI in all the groups. Also PPD, CAL gain, and radiographic bone formation were significant for autogenous tooth-grafted sites and β-TCP-grafted sites, with a significant improvement in PPD and CAL for tooth-grafted sites compared with β-TCP-grafted sites. Histologically, the amount of new bone formation was higher in group 3 compared with group 1 and 2. Conclusions: It can be concluded that autogenous tooth particles can be considered as an effective and suitable bone graft material used in the treatment of intra-bony defects in patients with chronic severe periodontitis.

Abstract:Background The vertical increase of the alveolar ridge dimension using allograft or xenograft mixed with autogenous bone graft and covered by a nonabsorbable high‐density polytetrafluoroethylene (d‐PTFE) membrane is well documented in the literature. Purpose The aim of this study was to assess vital mineralized tissue formation in vertical ridge augmentation (VRA) procedures using autogenous bone chips mixed either with an allograft or a xenograft. Methods This prospective clinical trial recruited 16 partially edentulous patients to undergo vertical ridge augmentation in one or more sites, making up a total of 24 samples for histological evaluation. Patients were sequentially stratified into Group A (treated with a freeze‐dried bone allograft [FDBA] mixed with autogenous bone) or to Group B (treated with a bovine xenograft mixed with autogenous bone). Histological samples were analyzed according to the biomaterial used for VRA. Histological samples were obtained on the same day of membrane removal and implant placement. Results Thirty‐three implants were placed in 16 sites of regenerated bone via VRA, 13 patients with ridge augmentation in the posterior mandible, and 3 patients with VRA in the anterior maxilla. Group A (FDBA + autogenous) and Group B (xenograft + autogenous) showed a percent vital mineralized tissue (VMT) area of 67.64 ± 16.84 and 60.93 ± 18.25, respectively. A significant difference between the two biomaterials was not observed. Conclusion When mixed with autogenous bone, either allografts or xenografts may provide a successful augmentation. Either mixture could serve as reliable alternative in VRA for obtaining a high percentage of VMT.

Abstract:When treating intrabony defects to regenerate periodontal structure, platelet-rich plasma (PRP) alone or in conjunction with various grafting materials, enamel matrix proteins, and barrier membranes have shown promising results. This evidence-based review aimed to systematically appraise the efficacy of xenograft in combination with PRP in the surgical treatment of periodontal intrabony defects in comparison to xenograft alone in terms of clinical and radiographic outcomes in adults with periodontitis. Electronic and manual data were searched exhaustively by 2 reviewers, including PubMed-Medline, Cochrane Central Register of Controlled Trials, Embase and Scopus from the earliest available date till January 31, 2023 without restriction on language. Randomized controlled clinical trials of either parallel or a split-mouth design and studies where xenograft and PRP were in the intervention group or as a comparator was included. After screening the articles, 3 studies were included. The current systematic review reveals that PRP used as an adjunct with a xenograft resulted in a significant reduction in pocket depth and greater clinical attachment level gain in comparison to xenograft alone. The analysis of the present review concludes that PRP in conjunction with xenograft for the treatment of intrabony defects shows promising outcomes. However, owing to the short follow-up time and heterogeneity of included studies, caution should be followed while using this material clinically.

Abstract:Background: The present, randomized, controlled clinical and radiographic study was undertaken to compare the effectiveness of guided tissue regeneration (GTR) by using a collagen membrane barrier with or without decalcified freeze-dried bone allograft (DFDBA) in the treatment of periodontal infrabony defects characterized by unfavorable architecture. Materials and methods: Sixteen systemically healthy patients with 20 periodontal infrabony defects were selected for the study. Each patient had at least ≥ 5 mm clinical probing pocket depth (PPD) at the selected site and depth of intrabony component ≥ 3 mm as assessed by clinical and radiographic measurements. Baseline measurements included plaque index, papillary bleeding index, PPD, gingival recession, clinical attachment level and radiographic defect depth (DD). At the time of surgery, the defects were randomly assigned to either the test group (collagen membrane plus DFDBA) or the control group (collagen membrane only). Results: At the 6-month examination, PPPD reduction was significantly greater in the GTR + DFDBA group (4.06 ± 0.38 mm) compared with the GTR group (3.2 ± 0.74 mm). The mean gains of clinical attachment were 3.54 ± 0.36 mm in the test group and 2.50 ± 0.74 mm in the control group. Radiographic DD reduction was similarly greater in the GTR + DFDBA group (2.40 ± 0.51 mm) compared with the GTR group (1.60 ± 0.51 mm). Conclusions: The results of the present study indicate that the use of a GTR membrane with bone graft has significantly improved all clinical parameters tested as compared with the use of bioresorbable membrane alone in the treatment of infrabony defects characterized by unfavorable architecture.

Abstract:Background and objectives: All surgical procedures produce some secondary effects such as pain, swelling, and some functional impairments like difficulty in chewing mouth opening, speaking, sleeping, etc. Controlling postoperative discomfort may improve the patient's quality of life and acceptance of the treatment. The goal of this randomized single-blind trial was to see if using platelet-rich fibrin during endodontic surgery had a positive impact on pain and other aspects of the patient's quality of life in the first week after surgery. Methods: Fifteen patients with periapical lesions were treated with endodontic surgical procedures (control group). In another 15 patients, platelet-rich fibrin was placed into the bone defect in the form of a clot, and a suture was applied in addition to the surgical procedure (test group). During the first week after surgery, all patients completed a questionnaire to assess their major symptoms and daily activities. The results of the two groups' questionnaires were statistically compared. Results: The test group reported less pain, symptoms (swelling and bad taste), and functional activities compared with the control group but the difference was not statistically significant. Conclusion: The adjunct of platelet-rich fibrin to the endodontic surgical procedure produced some beneficial effects on patients' quality of life during the early postoperative stage. There were insufficient data in the literature and more clinical trials with larger samples have to be carried out in the future to find out the outcome
Systematic Analysis of Lower Second Molar Periodontal Health after Autogenous Dentine Graft from Wisdom Tooth Extraction

Abstract:Introduction: Postoperative complications stemming from lower wisdom tooth extractions are often overshadowed by the immediate need to address pain and existing disease. Nevertheless, an emerging body of evidence suggests the importance of preventing potential periodontal damage to the adjacent second molar resulting from the extraction surgery. To avert periodontal complications involves promoting bone formation within the extracted socket defect. Allogenic and artificial bone, as well as the patient’s bone, have conventionally filled the osseous void post-surgery. These approaches are costly and may extend the patient’s recovery. An Autogenous Dentine Graft (ADG), derived from the recently extracted wisdom tooth, presents, as a particulate with osteoconductive and osteoinductive properties, is ideal for preserving bone integrity. This review assesses the available evidence supporting the use of ADG in safeguarding against periodontal morbidity resulting from the surgical extraction of a lower third molar. Materials and methods: A systematic review of the literature was conducted, focusing on the periodontal health of the second molar and the bone height of the osseous defect postwisdom tooth extraction. Out of 76 identified articles, four studies were selected for the review. Results: Three months post-surgery, the mean periodontal probing depth distal to the second molar was 4.7mm in control sites and 2.83mm in sites treated with ADG. Over 12 months, the mean bone height exhibited a negative loss of 1.25mm in control sites and a positive gain of 0.9mm in sites treated with ADG. Conclusion: Presently, there is insufficient evidence to unequivocally support the use of an autogenous dentine graft following the surgical extraction of a wisdom tooth. However, early results are promising, and given the biocompatibility of a material derived from the patient, ADG is considered a safe option for patients at high risk of an osseous defect. Further research is needed to substantiate these findings and guide clinical decision-making.
Comparative Assessment of Autogenous Cancellous Bone Graft and Bovine-Derived Demineralized Bone Matrix for Secondary Alveolar Bone Grafting in Patients With Unilateral Cleft Lip and Palate

Abstract:Objective: Comparison between bovine-derived demineralized bone matrix (DMBM) and iliac crest graft over long term for secondary alveolar bone grafting (SABG) in patients with unilateral cleft lip and palate (UCLP) in terms of radiological and clinical outcomes. Design: Prospective, randomized, parallel groups, double-blind, controlled trial. Setting: Unit of Oral and Maxillofacial Surgery, Oral Health Science Centre, Postgraduate Institute of Medical Education & Research, Chandigarh. Participants: Twenty patients with UCLP. Interventions: Patients were allocated into group I (Iliac crest bone graft) and group II (DMBM) for SABG. Outcomes were assessed at 2 weeks, 6 months, and then after mean follow-up period of 63 months. Outcomes Measures: Volumetric analysis of the grafted bone in the alveolar cleft site was done through cone beam computed tomography using Cavalieri principle and modified assessment tool. Clinical assessment was performed in terms of pain, swelling, duration of hospital stay, cost of surgery, alar base symmetry, and donor site morbidity associated with iliac crest harvesting. Results: Volumetric analysis through Cavalieri principle revealed comparable bone uptake at follow-up of 6 months between group I (70%) and group II (69%). Modified assessment tool showed no significant difference between horizontal and vertical bone scores over short- and long-term follow-up. In group II, there was higher cost of surgery, but no donor site morbidity unlike group I. Conclusions: Demineralized bone matrix proved analogous to iliac crest bone graft as per volumetric analysis over shorter period. However, although statistically insignificant, net bone volume achieved was lower than the iliac crest graft at longer follow-up.

Abstract:Background The removal of impacted lower third molars (ILTMs) is associated with bone defects in the distal area of second molars. Different methods have been described to minimize these defects. Purpose The primary objective was to assess changes in probing depth (PD) over time (up to 36 months) between test (grafted) and control (ungrafted) groups; the graft was obtained from the extracted ILTM. Study Design, Setting, Sample This split-mouth randomized clinical trial was conducted at the Postgraduate Course in Oral Surgery of the Faculty of Dentistry of the Complutense University of Madrid. Adult patients requiring bilateral ILTM extraction with adjacent second molars were recruited, excluding pregnant/lactating women, patients in treatment with nonsteroidal anti-inflammatory drugs and patients with periodontal diseases. Predictor/Exposure/Independent Variable The predictor variable was the graft technique. The bone defect after ILTM removal was treated with autogenous tooth graft (ATG) in the test group, leaving the control group ungrafted. Main Outcome Variable PD on the distobuccal, distomedial, and distolingual surfaces was recorded in both groups and averaged at baseline (T0), 3 (T1), 6 (T2), and 36 months (T3) postoperatively. Covariates Sex, age, surgical time, ILTM situation and position between groups were assessed. Analyses ANOVA repeated measures for comparisons between groups and the Friedman test for comparisons within the groups over time were applied. Statistical significance was established with a confidence interval of 95% ( P < .05). Results The sample comprised 22 patients (6 males, 16 females) with a mean age of 21.68 ± 2.19 years; 44 ILTM extractions were performed. Statistically significant differences in PD average were found between groups ( P < .001, 95% confidence interval) at 3 (1.63 ± 0.29), 6 (1.76 ± 0.3), and 36 months (1.74 ± 0.36). Reductions from T0 to T3 of 2.74 ± 0.28 ( P < .001) and 0.54 ± 0.3 ( P = .43) were observed in test and control groups, respectively. Conclusion and Relevance ATG placed on the distal surface of lower second molars and almost completely filling the extraction socket improved PD 3, 6 and 36 months after ILTM. Furthermore, no significant changes in PD were observed over time; no major complications occurred. ATG appears to be a viable alternative graft material for this procedure.

Abstract:Background: Absorbable synthetic biopolymers have been used as bone filler in Periodontology, proving effective stimulants to bone regeneration. Aim: Copolymerized polylactic and polyglycolic acid is used as a bone filler and polyglactin 910 as a guided tissue regeneration (GTR) membrane to achieve regeneration in periodontal infrabony defects. Materials and methods: Forty patients with two- or three-walled infrabony defects were selected and randomly divided into two groups. Group A included patients treated with polylactic-polyglycolic acids 50:50 (Fisiograft ,Ghimsa SPA,Via Fucini, Italy) alone and Group B included patients treated with polylactic-polyglycolic acids (PLA-PGA)50:50 in conjunction with polyglactin acid 910 (Vicryl Mesh Johnson&Johnson , U.S.A ). Evaluation of clinical parameters probing depth and attachment level and radiographs was done preoperatively and 12 and 24 weeks postoperatively. Results: Both the groups showed statistically significant mean reduction in probing depth and gain in clinical attachment level and linear bone fill. Conclusions: Within the limit of this study, both the treatment modalities are beneficial for the treatment of infrabony defects.
