Abstract:Chemical eye injury (CEI) is an acute emergency which can threaten sight and life. These commonly occur at home or the workplace with the former being generally mild and the latter more severe and bilateral. Major workplace accidents involve other parts of the body and can be associated with inhalation or ingestion of the chemical. Alkali injuries cause damage by saponification of tissue and deeper penetration as a consequence. Acid injuries cause rapid coagulation of tissue, which impedes penetration and limits damage. Irritants such as alcohols, cause superficial epithelial denudation. Severe chemical insult can affect all anterior segment structures causing iris, pupil and lens abnormalities. Eye pressure is variably affected and can be low or high or start as one and rapidly change to the other. Chorioretinal changes in the form of vasculopathy are seen and ascribed to be secondary to anterior segment inflammation rather than due to the direct effect of CEI. Final outcome related to structure and function is determined by the injurious agent, duration of exposure, nature of treatment and the rapidity with which it is instituted. Prevention of further damage by profuse and prolonged eye wash, after ascertaining pH of both eyes, together with exploration and removal of all particulate matter, is the key. Other management principles include a complete and thorough assessment, control of inflammation, facilitation of healing and prevention and management of sequelae and complications. Intraocular pressure is often forgotten and must be assessed and managed. Management often requires a multidisciplinary approach.

Abstract:Ocular chemical injuries vary in severity, with the more severe end of the spectrum having profound visual consequences and medicolegal implications. Grading of ocular injuries is critical for determining acute treatment and visual prognosis. Poor immediate management results in more challenging treatment of acute disease. Similarly, poorly controlled acute disease results in more treatment-resistant chronic ocular disease. Despite several decades of research and public health initiatives, simple and effective interventions such as wearing protective eyewear and immediate irrigation of eyes remain as key challenges. Education and prevention are therefore important public health messages. Hurdles in the acute management of disease include poor evidence-base for commonly used treatments (e.g. based on experimental animal studies), reduced treatment adherence rates and high clinic non-attendance rates. The evolution of treatment strategies, particularly limbal stem cell transplantation, has revolutionised the visual and cosmetic outcomes in chronic phases of disease. It is therefore increasingly important to consider tertiary referral for patients with limbal stem cell failure or vision-limiting corneal scarring.

Abstract:Objective: To study the clinical characteristics of patients with severe ocular chemical injury in Shanghai and to determine their relationship with visual outcome.Design: Retrospective case series.Participants: A total of 79 525 in-patients at Shanghai Eye, Ear, Nose, and Throat Hospital between 1998 and 2008.Methods: Medical records of severe ocular chemical injuries were reviewed retrospectively. Patients' age, gender, occupation, location where injury occurred, the nature of chemical agent, complication, and visual acuity at final follow-up were described. The relationship between visual impairment and clinical characteristics was analyzed.Main Outcome Measures: Estimated prevalence rate, demographics of patients, nature of injury, prognosis, and factors associated with visual impairment.Results: The estimated prevalence rate in Shanghai of severe ocular chemical injury was 1.58 per 100 000. One hundred ninety cases with 239 eyes were identified for analyzing the clinical characteristics. The average age of all subjects was 35.3 +/- 14.8 years with a male: female ratio of 6:1. Factory and construction worker (n = 141; 74.2% of total cases) were the most common occupations. Most of the accidents occurred at workplace (n = 144; 74.8%); the majority of injuries were caused by alkali agents (n = 129; 67.9%). Complications were found in 222 eyes (92.9%). A visual acuity of >= 6/60 was achieved only in 5 eyes (2.1%). In multivariate logistic regression analysis, being visually impaired was significantly associated with the patients' complication (P = 0.021).Conclusions: The prevalence of severe ocular chemical injury in Shanghai is low. Most subjects needed surgery to restore visual function. Successful management provides a good prognosis for most patients.

Sympathetic Ophthalmia Caused by a Severe Ocular Chemical Burn: a Case Report and Literature Review.

Abstract:We herein presented a case of sympathetic ophthalmia induced by chemical burns. A 39-year-old male was referred to our retinal clinic complaining of sharply decreased vision in his right eye, with photophobia and headache. He had suffered severe chemical burns five months ago, and his left eye underwent amniotic membrane transplantation combined with tarsorrhaphy for persistent corneal ulceration. A comprehensive examination was performed. After excluding other infectious ocular diseases and systemic inflammatory disease, Vogt-Koyanagi-Harada was considered. A regimen of 1.5 mg/kg prednisone per day was initiated, and tapered to a weekly dose. The inflammation in the patient's right eye was improved, and the best-corrected visual acuity recovered to 20/50. He continued to be seen by his corneal specialist for follow-up on the left eye. A regular B-scan was performed, and atrophy of the left eyeball was detected 2 months later. Upon removal of the left eyelid sutures, corneal perforation with a prolapsed iris was found. Afterwards, the inflammation of the right eye reoccurred, and the diagnosis was revised to sympathetic ophthalmia. The patient underwent an enucleation procedure on the left eye, and a massive prednisolone regimen was initiated, and then tapered slowly over one year. Pathological results showed diffuse epithelioid cells, polymorphonuclear giant cells, and lymphocyte infiltration in the uveal tissue. Sympathetic ophthalmia was confirmed. The right eye remained unremarkable at 41 months of follow-up. An uncommon case is presented of sympathetic ophthalmia following severe chemical burns. It is notable that sympathetic ophthalmia developed after chemical burns to the ocular surface and corneal perforation, despite reconstructive treatment combined with tarsorrhaphy.
Demographic profile and clinical characteristics of patients presenting with acute ocular burns

Abstract:Purpose: To study and compare the demographic and clinical profile of acute ocular burns (AOB) in children and adults. Methods: This retrospective case series included 271 children (338 eyes) and 1300 adults (1809 eyes) who presented to two tertiary eye care centers within one month of sustaining AOB. Data regarding demographics, causative agents, severity of injury, visual acuity (VA), and treatment were collected and analyzed. Results: Males were more commonly affected particularly among adults (81% versus 64%, P < 0.00001). Among children, 79% sustained domestic injuries, whereas 59% of adults had work-place injuries (P < 0.0001). Most cases were due to alkali (38%) and acids (22%). Edible lime (chuna, 32%), superglue (14%), and firecrackers (12%) in children, and chuna (7%), insecticides, lye, superglue (6% each), toilet cleaner (4%) and battery acid (3%) in adults, were the main causative agents. The percentage of cases with Dua grade IV-VI was greater in children (16% versus 9%; P = 0.0001). Amniotic membrane grafting and/or tarsorrhaphy were needed in 36% and 14% of affected eyes in children and adults, respectively (P < 0.00001). The median presenting VA was logMAR 0.5 in children and logMAR 0.3 in adults (P = 0.0001), which improved significantly with treatment in both groups (P < 0.0001), but the final VA in eyes with Dua grade IV-VI burns was poorer in children (logMAR 1.3 versus logMAR 0.8, P = 0.04). Conclusion: The findings clearly delineate the at-risk groups, causative agents, clinical severity, and treatment outcomes of AOB. Increased awareness and data-driven targeted preventive strategies are needed to reduce the avoidable ocular morbidity in AOB.

Abstract:Abstract Ocular chemical burns are a significant cause of visual impairment. This study aims to characterize ocular chemical burns in a southwestern Colombia referral center and identify associated factors with low vision. A cross-sectional study was carried out based on the review of medical records of patients diagnosed with chemical eye burns who consulted the emergency ophthalmology service between January 2016 and December 2019. Descriptive statistics were performed. Associations with low vision (Best Corrected Visual Acuity, BCVA, ≥20/70 in Snellen chart) in the last follow-up appointment were identified using a multivariate logistic regression with odds ratios (OR) and 95% confidence intervals (95% CI). 219 eyes affected by chemical burns were identified from 174 patients with a mean age of 39 years (SD±20), out of which 57.5% (n=100) were men. We ran a multivariate model adjusted by sex, eye wash, type of chemical, and Dua's classification. We found that the odds of low vision for patients without eyewash before the consult were three times the odds of those who had it (Adjusted OR [aOR]=3.5, 95%CI=1.3-9.4) and almost five times for those with Dua's classification greater than 1 (aOR=4.7, 95%CI=1.7-12.9). The ocular chemical burns reported in this study occurred more in young people of productive age. Acids were the principal causal agent. The association between lack of early management and the severity of the chemical burn with low vision has been highlighted. Ocular burns remain a relevant cause of consultation on the ophthalmology service in Cali, and prevention strategies are required.

Abstract:Abstract Introduction & Objectives : Chemical ocular injury is an emergency case and commonly occured among children. Alkaline chemical ocular injury is more destructive especially due to saponification. One of the rare chemical ocular injury is alkaline trauma by Potassium Permanganate (KMNO4) Methods : A case of a three-year-old boy patient brought by his mother in emergency department at Cicendo Eye Hospital with severe pain, redness, and swelling of the left eye. Ten hours earlier, the patient was playing and accidentally exposed to KMNO4. Visual acuity of the right eye was 6/6 and the left eye was difficult to examine. Ophthalmological examination revealed superior and inferior palpebra are swelling, KMNO4 adhered tightly on conjunctival tarsal superior, inferior, and conjunctival bulbi inferior until fornix and temporal area. Cornea was hazed and iris shadow test positive so he was diagnosed with Chemical Trauma Ropper Hall grade II OS. Treatment begins with irrigation 2 litres of normal saline until pH 7 along with debridement of KMNO4 from conjunctival tarsal superior inferior and bulbi. The patient underwent installation of transplanted amniotic membrane and debridement under anasthesia Results : Management in the initial phase is irrigation. The definitive therapy requires surgery to speed up the healing process. Complications that often occur are corneal limbal cell deficiency, dry eye, symblepharon formation, secondary glaucoma, and complicated cataracts Conclusion : Alkali chemical injury is hazardous and can lead to severe vision threatening complications because of saponification. Prompt and aggressive management is required in such cases to give them the best chance of a more favourable outcome

Abstract:PURPOSE: There have been several reports in the literature demonstrating the adverse effects of multiple ocular cosmetic procedures, such as eyelash extensions, eyeliner tattoo, and eyelash dyeing. To our knowledge, there is limited literature on the adverse effects specifically attributed to the chemicals and process of eyelash extension removal. Our purpose is to demonstrate the possible ocular injuries from misapplication of eyelash extension removal solvent.OBSERVATIONS: We present a unique case of a 46-year-old female with a prior history of laser assisted in-situ keratomileusis (LASIK) who presented with bilateral chemical conjunctivitis and diffuse lamellar keratitis (DLK) secondary to epithelial defects following the misapplication of eyelash extension removal gel.CONCLUSIONAND IMPORTANCE: Given that our patient suffered significant dry eyes, corneal haze, and visual fluctuation, we believe this case underscores the importance of continuing closer and careful evaluation into the chemicals present in these cosmetics to improve the safety of our patients and to limit such incidents from occurring hereafter.

Abstract:AIM:To investigate the risk factors for eye removal following ocular trauma at a major ophthalmology department in China.METHODS:A retrospective study of patients who underwent eye removal surgery following ocular trauma was completed. Clinical outcomes were consulted in detail through the hospital's computed medical data system. Patients' information including age, gender, cause of ocular trauma, affected eye, and education level was collected and recorded in a standardized database. Chi-squared test, Student's t-test, Fisher's exact test, and bivariate correlation analysis were used for statistical comparisons.RESULTS:The present study included 1675 removal eyes from 1674 patients over the 20-year period. Patients included 80.5% males and 19.5% females, with mean age of 38y. The majority of the patients (70.7%) were blue-collar workers (physical laborers), and 1098 patients (65.6%) did not receive high school education. Work-related injuries were the most common reason for eye removal (n=739, 44.1%), of which 441 cases (59.7%) were related to metal/nail wounds. The most frequent injury type in males was work-related injuries (49.7%), whereas the most frequent injury type in females was home-related injuries (25.8%).CONCLUSION:Work-related injuries are a leading cause of severe ocular injury resulting in eye removal. In addition, men and undereducated patients are more likely to undergo eye removal surgery following ocular trauma. This study identified multiple high-risk factors leading to eye removal following ocular trauma, which is of great importance for preventing severe eye injuries.

Abstract:The 192-beam National Ignition Facility (NIF), a football-stadium sized laser at the Lawrence Livermore National Laboratory (LLNL) in Livermore, CA, is now fully operational. 1.1MJ of ultraviolet laser energy has been demonstrated using 192 beams. A single NIF bundle (8 beams) has generated 78 kJ in the ultraviolet; this corresponds to the planned 192-beam peak energy of 1.8 MJ. The facility is being systematically ramped up in preparation for the recommencement of experiments at NIF in spring 2009.

Abstract:Objective. We aim to evaluate the clinical effect of combined topical 20% autologous serum eye drops (ASEs) along with silicone–hydrogel soft contact lenses (SCLs) in the treatment of chemical burn-induced bilateral corneal persistent epithelial defects (PEDs) and to review the literature of related studies. Methods. From January 1, 2017, to December 31, 2019, we conducted a retrospective chart review of 8 patients with chemical burn-induced bilateral corneal PEDs who were unsuccessfully treated with conventional medical therapy and were then treated with combined topical 20% (v/v) ASEs and silicone–hydrogel CLs. The clinical effects and effectiveness of the combined treatment were evaluated. Results. The bilateral corneal PEDs healed in all sixteen eyes of the eight patients within 2 weeks. The patients did not report any discomfort associated with the combined treatment. Improved ocular comfort/visual acuity and decreased conjunctival injection correlated with healing. No recurrent corneal epithelial breakdown was noted during the 3-month posttreatment follow-up. Conclusions. The combined treatment of silicone–hydrogel CLs and ASEs can help to stabilize the ocular surface and successfully treat chemical burn-induced bilateral corneal PEDs. It may be considered as an alternative treatment method for patients with bilateral chemical burn-induced corneal PEDs with potential corneal melting.

Analysis of medical expenditure and socio-economic status in patients with ocular chemical burns in East China: a retrospective study

Abstract:Background Little has been known regarding the relationship between ocular chemical injury and victims’ medical expenditure, income loss and socio-economic status changes. So we conduct this retrospective cross-sectional study in patients with ocular chemical burns in East China. Methods Fifty-six patients were enrolled and required to complete a self-report questionnaire consisting of the following contents: entire expenditure on medical treatment; the victims’ personal and household per capita income, and income loss caused by the injury; and the changes of socioeconomic status as well. Results The median expense of medical treatment was CNY 40,000 (approximately US$5,900). The medical expenditure rose significantly with increased injury severity, prolonged hospital stay, and increased frequency of surgery. More than half victims (51.8 %, 29/56) paid all or the majority of medical expense by themselves. The expense of only 5 victims was mainly paid by medical insurance, accounting for less than ten percent (8.9 %, 5/56). The victims’ personal and household per capita income both decreased significantly after the injury, with the median reduction being CNY 24,000 and CNY 7,800 (approximately US$3600 and US$1200) per year respectively. The reduction amplitude of personal and household per capita income rose with increased injury severity and prolonged time of care required. The injury caused emotional depression or anxiety in 76.8 % (43/56) victims, and the relationship with their relatives got worse in 51.9 % (29/56) patients. Moreover, only 21.4 % (12/56) patients felt that the whole society gave them care and concern after the injury, whereas 46.4 % (26/56) and 28.6 % (16/56) felt indifference or discrimination from society as a whole ( X 2 = 16.916, P = 0.028). Conclusions The medical expense was a huge economic burden to most victims of ocular chemical burns, and personal and household per capita income of the victims decreased significantly after injury, both of which had a close relationship with the injury severity. Formal legislation was urgently needed to compel the employer to purchase injury or medical insurance and provide more compulsory protection to the population working in high risk occupations. In addition, psychological counseling and instruction shouldn’t be neglected in the aid and treatment of victims.
[Preliminary Clinical Observations of Retina Injury in Alkaline Ocular Trauma Patients].

Abstract:Objective: To investigate the features of the full-field electroretinogram (FERG) in patients with ocular surface alkaline injury, and to clarify the clinical significance of FERG examination in alkaline ocular trauma. Methods: Retrospective series of case study. A total of 22 patients with chemical ocular surface injury who were treated in the Department of Ophthalmology of Peking University Third Hospital from May 2018 to January 2020 were selected, including 20 males (25 eyes) and 2 females (2 eyes). The age ranged from 19 to 60 years old (average, 41). There were 17 eyes with alkaline ocular trauma, 7 eyes with thermal burn, and 3 eyes with acid ocular trauma. Seventeen contralateral healthy eyes in patients with unilateral mechanical trauma were used as controls. Fourteen alkaline trauma eyes with visual acuity records were further observed according to the degree of visual impairment. All patients were subjected to different flash intensity stimuli in accordance with a standard of the International Society for Clinical Electrophysiology of Vision for dark-adapted and light-adapted full-field FERG. One-way ANOVA and SNK-q was used for the comparison between groups. The unpaired t test was used for the comparison of patients with different vision. Results: Compared with healthy eyes the injured eyes' amplitude of dark adaption FERG b-wave under the dark stimulation of 0.01 cd·s·m-2 in alkaline ocular trauma, thermal burn, acid ocular trauma was (135±85), (169±55), and (112±43) versus (341±53) μV, respectively; compared with healthy eyes, the differences were statistically significant (F=31.38; q=8.94, 5.70， 5.45；all P<0.01). Compared with healthy eyes the injured eyes' amplitude of dark adaption FERG a-wave under the dark stimulation of 3.00 cd·s·m-2 in alkaline ocular trauma, thermal burn, acid ocular trauma was (178±78), (172±35), and (99±53) versus (334±60) μV, respectively; compared with healthy eyes, the differences were statistically significant (F=24.33; q=7.04， 5.60， 5.80；all P<0.01). Compared with healthy eyes the injured eyes' amplitude of dark adaption FERG b-wave under the dark stimulation of 3.00 cd·s·m-2 in alkaline ocular trauma, thermal burn, acid ocular trauma was (354±79), (342±77), and (352±201) versus (600±78) μV, respectively; compared with healthy eyes, the differences were statistically significant (F=27.68; q=8.11， 6.51， 4.48； all P<0.01). Compared with healthy eyes the injured eyes' amplitude of dark adaption FERG OPs under the dark stimulation of 3.00 cd·s·m-2 in alkaline ocular trauma, thermal burn, acid ocular trauma was (97±54), (107±41), and (45±22) versus (206±32) μV, respectively; compared with healthy eyes, the differences were statistically significant (F=25.03; q=7.36， 5.13， 5.96； all P<0.01).There was no significant difference in FERG between patients with visual acuity≥0.2 and those with visual acuity<0.2 (P>0.05) after alkaline ocular trauma. Conclusions: By detecting the amplitude change of FERG wave form of chemical ocular trauma, in this study, the amplitude of FERG wave form in alkaline ocular trauma was generally decreased. It's suggested that alkaline ocular trauma can cause damage to the rod and cone systems of retinal. (Chin J Ophthalmol, 2020, 56: 514-518).

Resurgence of Acanthamoeba keratitis in Auckland, New Zealand: a 7-year review of presentation and outcomes

Abstract:Purpose: To investigate the presentation, clinical characteristics and outcomes of Acanthamoeba keratitis (AK) in Auckland, New Zealand over a 7-year period. Methods: Retrospective analysis of all cases of AK treated by the tertiary corneal service at Auckland City Hospital/ University of Auckland Department of Ophthalmology (August 2001 to May 2008). Data were collected regarding age, gender, contact lens history, presenting signs and symptoms, diagnosis at first presentation, time to final diagnosis, identifiable risk factors, presenting and final visual acuity, results of microbiological testing, medical treatment, surgical interventions, recurrence of disease and length of follow up. All photographs and in vivo confocal microscopy images were reviewed. Results: Twenty-five eyes of 25 patients were identified with a diagnosis of AK (mean age 40 +/- 13 years). Ninety-six per cent were contact lens wearers. Mean time to diagnosis was 41 +/- 49 days (range 0-181 days, median 21 days). Fourteen patients (56%) had been treated with topical corticosteroids prior to the diagnosis. Early diagnosis of AK (<21 days) was associated with significantly better final visual acuity and did not require any surgical intervention compared with those diagnosed at a later stage. Six patients, all in the late diagnosis group, required surgical intervention. Conclusions: AK has become significantly more common in New Zealand in the current decade. This study highlights the fundamental importance of early diagnosis and appropriate management in ensuring favourable outcomes. Practitioners should maintain a clinical suspicion of AK, especially as 96% of the subjects in this study were contact lens wearers.
Clinical profile and management of ocular superglue injuries: Case series and review of literature

Abstract:Purpose: To describe the clinical profile and management of patients with ocular superglue injury (OSI). Methods: This retrospective study included all patients with OSI who presented at a tertiary eye care institute between 2016 and 2020. Data regarding demographics, clinical profile, and management were collected. Results: A total of 66 eyes of 58 patients (24 children, 34 adults) with a median age of 22.5 years [interquartile range (IQR): 11.3–31] were included. All cases sustained accidental injuries, with domestic injury at home being the most common location of injury among children (79%) and adults (53%) ( P = 0.39). The median visual acuity at presentation was worse in children [0.3 logMAR (IQR: 0.2–0.4)] as compared to adults [0.1 logMAR (IQR: 0.1–0.3)] ( P = 0.03)]. The most common clinical sign at presentation was conjunctival congestion in 77% of eyes (51/66) followed by polymerized glue stuck to the eyelashes and eyelids in 52% of eyes (34/66). The median duration from the time of injury to presentation was 2 hours in both groups. All eyes resolved with medical management. Examination under anesthesia was required in three children (13%) to evaluate the extent of OSI. None of the patients had long-term ocular complications. Conclusion: Improper and careless handling of superglue in the domestic setting may cause accidental ocular injuries that require immediate medical attention. OSI represents less severe ocular injuries that respond to medical therapy alone and is not associated with long-term visual morbidity. Modifications in the packaging of superglue containers and awareness about their deleterious effects could prevent these injuries.

Abstract:Verätzungen des Auges können aufgrund einer Dysregulation inflammatorischer Reaktionen zu schweren Gewebeschäden des Augapfels mit kornealer Fibrogenese und Erblindung sowie zur Vernarbung der Lider und der ableitenden Tränenwege führen. Augenverätzungen gelten deshalb als Notfälle höchster Dringlichkeit, wobei die frühzeitige intensive Spülbehandlung die wichtigste Sofortmaßnahme bildet. Chemical burns of the ocular surface (CBOS) are emergencies of highest urgency. Therefore, an adequate emergency care is mandatory. Following a precise analysis of the initial damage, a staged therapeutic approach is used to prevent persistent impairment of the ocular surface. In the acute stage, the prevention of complications is targeted (symblepharon, conjunctival scarring, lacrimal stenosis, corneal ulceration, intraocular inflammation, elevated intraocular pressure, etc.). In later stages, if complications have developed, a secondary restoration of the ocular surface is focussed. Sometimes this requires several surgical interventions. Based on a review of international literature, this review highlights the pathophysiology according to different chemical agents, CBOS stages as well as main therapy strategies in early and advanced stages of CBOS. Acute treatment aims to lower inflammation, oxidative stress and tries to promote reepithelialisation. Besides conjunctival scarring, loss of goblet cells and corneal opacification a limbal stem cell insufficiency is the most harming complication. Several new techniques have been developed to recover the ocular surface with a sufficient and clear epithelial layer in order to avoid neovascularization of the cornea. The knowledge concerning the high risk potential for persistent visual impairment in CBOS patients and the ability for appropriate emergency care should be kept in every physician's mind dealing with CBOS. Augenverätzungen gelten als Notfälle höchster Dringlichkeit. Seit 2021 liegt von DOG (Deutsche Ophthalmologische Gesellschaft) und BVA (Berufsverband der Augenärzte Deutschlands) die S1-Leitlinie 045-018 "Akute Verätzung am Auge" vor. Entsprechend ihrer chemischen Beschaffenheit werden die ätzenden Substanzen in 3 Gruppen – Laugen (alkalische Agenzien), Säuren und Mischagenzien – unterteilt. Säuren führen zu einer Koagulationsnekrose, Basen verursachen eine Kolliquationsnekrose. Zu den wesentlichen Prognosefaktoren bei Verätzungen der Augen gehören Einwirkzeit, pH-Wert, Dissoziationskonstante (pK-Wert), Konzentration, Osmolarität, mechanische Energie, Temperatur, Flächenausmaß. Die frühzeitige intensive Spülbehandlung ist die wichtigste Sofortmaßnahme. Amphotere Spüllösungen können sowohl Säuren als auch Basen gegenüber neutralisierend wirken. Hyperosmolare Lösungen verringern den Wassergehalt im traumatisiertem Gewebe und reduzieren den ödembedingten Begleitschaden. Gebrannter (ungelöschter) Kalk erfordert die mechanische Entfernung der Partikel. Ein alleiniges Spülen würde die Freisetzung von Hydroxidionen fördern (pH-Wert-Anstieg) und die Ätzwirkung verstärken. In der Akutphase sollen Komplikationen vermieden werden. Neben der Förderung der Re-Epithelialisierung ist die Reduktion proteolytischer Enzyme anzustreben. In der Spätphase müssen Komplikationen behandelt werden. Letztere umfassen vor allem Narbenbildung und Limbusstammzellinsuffizienz. Hier haben sich verschiedene Modifikationen von Stammzelltransplantationstechniken etabliert. Vision-Related Quality of Life in Patients with Ocular Chemical Burns

Abstract:PURPOSE:To assess vision-related quality of life in patients with ocular chemical burns by the application of the 25-item National Eye Institute Visual Function Questionnaire (NEI VFQ-25).METHODS:Eighty-seven patients with ocular chemical burns were enrolled in the study from January 1 through May 31, 2010. Apart from the collection of sociodemographic and clinical data, NEI VFQ-25 with an additional appendix question, being translated to Chinese, was administered to all subjects. Main outcome measures were comparison of the NEI VFQ-25 subscale item scores among subgroups and multivariate analysis of the NEI VFQ-25 subscale scores.RESULTS:Fifty-five subjects were bilaterally burned and the rest were unilaterally injured. The mean age of enrolled subjects was 39.4 ± 11.6 years, with the majority being male (98.9%) and worker (77.0%); the mean composite score of all subjects was 40.4 ± 23.8. The composite score and majority subscale scores of binocularly injured patients were significantly lower than those of monocularly injured patients. Further comparisons among groups divided by either clinical severity classification or best-corrected visual acuity (BCVA) produced similar results. The BCVA of both the better-seeing eye and the worse-seeing eye strongly correlated with the NEI VFQ-25 composite score (ρ = 0.664 and 0.498, both P = 0.000). Multivariate regression analysis revealed that the VFQ-25 composite score correlated significantly with the following independent variables: BCVA of the better-seeing eye and the worse-seeing eye, the injury classification of the less severely injured eye, and correct and immediate irrigation after injury as well.CONCLUSIONS:Ocular chemical burns have a significant and extensive impact on patients' visual function outcomes and vision-related quality of life.

Abstract:Context: Severe pediatric ocular injury remains a frequent and difficult problem. Vitrectomy is a major technique that has been used to manage severely damaged eyes. However, limited follow-up studies exist currently. Aims: To evaluate the clinical features and predictive factors of visual and anatomic outcomes in Eye Injury Vitrectomy Study (EIVS). Settings and Design: Retrospective, consecutive, interventional case series. Materials and Methods: Reviewing and analyzing records of children, aged 15 years or younger, who had undergone vitrectomy for eye injuries in EIVS database between January 1997 and December 2009. Statistical analysis used: Descriptive analyses and multiple Logistic regressions were employed for all variables using SPSS software (version 17.0, SPSS Inc.). Results: Eighty-seven children (89 eyes) with more than 6 months follow-up and complete records identified in EIVS were included in this study. Average follow-up was 12.7 months. The mean age was 9.4 +/- 3.8 years. Seventy-seven eyes (86.5%) had open globe injuries, and 12 (13.5%) had closed globe injuries. Seventeen eyes (19.3%) presented with endophthalmitis. Seventy-five eyes (88.2%) presented with visual acuity of 4/200 or worse; however, 42 eyes (47.7%) achieved 4/200 or better vision with anatomical restoration after vitrectomy. Multiple analysis showed that choroidal damage, large scleral wound, and endophthalmitis were significantly associated with unfavorable outcome, the OR values were 16.7 (95% CI: 2.7-102.4, P = 0.002), 10.9 (95% CI: 1.7-71.6, P = 0.013), and 6.6 (95% CI: 1.0-42.4, P = 0.048), respectively. Conclusions: Vitrectomy intervention resulted in favorable visual and anatomic outcomes in almost half of the injured eyes. Choroidal damage, large scleral wound, and endophthalmitis were the prognostic indicators of unfavorable outcome.
