Abstract:This study experiments with TikTok videos to promote clinical trial participation. More specifically, it examines how short-form video sources (doctors vs. prior volunteers for clinical trials) influence perceived source credibility, self-efficacy, and behavioral intention to participate in clinical trials. Findings from this online experiment ( N = 396) showed that doctor sources led to greater behavioral intention through enhancing source credibility compared to prior volunteer sources. Alternatively, prior volunteer sources increased behavioral intention via enhanced self-efficacy for participants with low trust in doctors. These findings contribute to the understanding of how short-form video sources act as heuristic cues, leading to persuasion outcomes. Overall, we recommend featuring doctors when using video-based messages to promote clinical trial participation. Also, this study emphasizes the need for health communication practitioners to consider prior volunteers as spokespersons when targeting populations with low baseline trust in doctors.

"Influencing the influencers:" a field experimental approach to promoting effective mental health communication on TikTok

Abstract:A substantial body of social scientific research considers the negative mental health consequences of social media use on TikTok. Fewer, however, consider the potentially positive impact that mental health content creators ("influencers") on TikTok can have to improve health outcomes; including the degree to which the platform exposes users to evidence-based mental health communication. Our novel, influencer-led approach remedies this shortcoming by attempting to change TikTok creator content-producing behavior via a large, within-subject field experiment (N = 105 creators with a reach of over 16.9 million viewers; N = 3465 unique videos). Our randomly-assigned field intervention exposed influencers on the platform to either (a) asynchronous digital (.pdf) toolkits, or (b) both toolkits and synchronous virtual training sessions that aimed to promote effective evidence-based mental health communication (relative to a control condition, exposed to neither intervention). We find that creators treated with our asynchronous toolkits—and, in some cases, those also attending synchronous training sessions—were significantly more likely to (i) feature evidence-based mental health content in their videos and (ii) generate video content related to mental health issues. Moderation analyses further reveal that these effects are not limited to only those creators with followings under 2 million users. Importantly, we also document large system-level effects of exposure to our interventions; such that TikTok videos featuring evidence-based content received over half a million additional views in the post-intervention period in the study's treatment groups, while treatment group mental health content (in general) received over three million additional views. We conclude by discussing how simple, cost-effective, and influencer-led interventions like ours can be deployed at scale to influence mental health content on TikTok.

A Peer-Led Social Media Intervention to Improve Interest in Research Careers Among Urban Youth: Mixed Methods Study

Abstract:Background: Novel methods to boost interest in scientific research careers among minority youth are largely unexplored. Social media offers a unique avenue toward influencing teen behavior and attitudes, and can therefore be utilized to stimulate interest in clinical research. Objective: The aim of this study was to engage high-achieving minority youth enrolled in a science pipeline program to develop a targeted social media marketing campaign for boosting interest in clinical research careers among their peers. Methods: Students enrolled in the Training Early Achievers for Careers in Health program conducted focus groups in their communities to inform themes that best promote clinical research. They then scripted, storyboarded, and filmed a short video to share on social media with a campaign hashtag. Additionally, each student enrolled peers from their social circle to be subjects of the study. Subjects were sent a Career Orientation Survey at baseline to assess preliminary interest in clinical research careers and again after the campaign to assess how they saw the video, their perceptions of the video, and interest in clinical research careers after watching the video. Subjects who did not see the video through the online campaign were invited to watch the video via a link on the postsurvey. Interest change scores were calculated using differences in Likert-scale responses to the question "how interested are you in a career in clinical research?" An ordinal logistic regression model was used to test the association between watching a peer-shared video, perception of entertainment, and interest change score controlling for underrepresented minorities in medicine status (Black, American Indian/Alaska Native, Native Hawaiian, or Pacific Islander), gender, and baseline interest in medical or clinical research careers. Results: From 2014 to 2017, 325 subjects were enrolled as part of 4 distinct campaigns: #WhereScienceMeetsReality, #RedefiningResearch, #DoYourResearch, and #LifeWithoutResearch. Over half (n=180) of the subjects watched the video via the campaign, 227/295 (76.9%) found the video entertaining, and 92/325 (28.3%) demonstrated baseline interest in clinical research. The ordinal logistic regression model showed that subjects who viewed the video from a peer (odds ratio [OR] 1.56, 95% CI 1.00-2.44, P=.05) or found the video entertaining (OR 1.36, 95% CI 1.01-1.82, P=.04) had greater odds of increasing interest in a clinical research career. Subjects with a higher baseline interest in medicine (OR 1.55, 95% CI 1.28-1.87, P<.001) also had greater odds of increasing their interest in clinical research. Conclusions: The spread of authentic and relevant peer-created messages via social media can increase interest in clinical research careers among diverse teens. Peer-driven social media campaigns should be explored as a way to effectively recruit minority youth into scientific research careers.
Effects of folic acid on homocysteine in persons classified by methylenetetrahydrofolate reductase genotype.

Abstract:Woodside et al (1) recently published an interesting study of the effects of folic acid and vitamins B-6 and B-12 on total plasma homocysteine (tHcy) concentrations in 132 men stratified for the common heat-labile polymorphism [a C-to-T substitution at nucleotide 677 (C677T)] in the gene for methylenetetrahydrofolate reductase (MTHFR). On the basis of results from a subgroup of 50 subjects in this study, the authors concluded that persons who are homozygous (T/T) for the polymorphism are less responsive to the effects of folic acid and B-vitamin supplementation than persons homozygous for the nonthermolabile allele (C/C). They also hypothesized that doses of folic acid > 1 mg/d may be required to reduce plasma tHcy concentrations in the T/T subgroup. These results are in contrast with those from our previous studies of the effects of supplementation with 1 or 2 mg folic acid/d in 242 men and women classified by coronary artery disease status, multivitamin use, and the C677T MTHFR genotype (2). The results of our study, and those of studies by Jacques et al (3) and Deloughery et al (4), suggest that T/T homozygotes are more sensitive to the adverse tHcy-raising effects of reduced plasma folate concentrations. Moreover, our T/T subjects experienced much greater decreases in plasma tHcy concentrations after receiving 1 or 2 mg folic acid/d for 3 wk than did C/C subjects (2). Among subjects who were not previously taking multivitamins, the mean reductions in plasma tHcy concentrations were 220.9%, 213.1%, and 27.1% in persons with the T/T, C/T, and C/C genotypes, respectively (P = 0.019 for T/T compared with C/C; Figure 1). The decreases in plasma tHcy concentrations were not significantly different for subjects receiving 1 compared with 2 mg folic acid/d. Subjects who were previously taking multivitamins experienced smaller decreases in plasma tHcy concentrations after folic acid supplementation, but the T/T homozygotes still showed a trend for greater responsiveness than the C/C or C/T subjects (changes in plasma tHcy concentrations: 210.2%, 23.3%, and 23.2% for T/T, C/T, and C/C genotypes, respectively; NS) (2). Geometric mean plasma tHcy concentrations among T/T, C/T, and C/C subjects were not significantly different after folic acid supplementation. Thus, although individuals with the T/T genotype had higher plasma tHcy concentrations at baseline, our results suggest that these individuals were more sensitive to the tHcy-lowering effects of folic acid supplementation. The reasons for the disparities among the results of these studies are unknown, but additional investigations, including further analyses of the data from Woodside et al, may help to elucidate possible contributory factors.
The Role of Social Media in Enhancing Clinical Trial Recruitment: Scoping Review

Abstract:Background Recruiting participants into clinical trials continues to be a challenge, which can result in study delay or termination. Recent studies have used social media to enhance recruitment outcomes. An assessment of the literature on the use of social media for this purpose is required. Objective This study aims to answer the following questions: (1) How is the use of social media, in combination with traditional approaches to enhance clinical trial recruitment and enrollment, represented in the literature? and (2) Do the data on recruitment and enrollment outcomes presented in the literature allow for comparison across studies? Methods We conducted a comprehensive literature search across 7 platforms to identify clinical trials that combined social media and traditional methods to recruit patients. Study and participant characteristics, recruitment methods, and recruitment outcomes were evaluated and compared. Results We identified 2371 titles and abstracts through our systematic search. Of these, we assessed 95 full papers and determined that 33 studies met the inclusion criteria. A total of 17 studies reported enrollment outcomes, of which 9 achieved or exceeded their enrollment target. The proportion of participants enrolled from social media in these studies ranged from 0% to 49%. Across all 33 studies, the proportion of participants recruited and enrolled from social media varied greatly. A total of 9 studies reported higher enrollment rates from social media than any other methods, and 4 studies reported the lowest cost per enrolled participant from social media. Conclusions While the assessment of the use of social media to improve clinical trial participation is hindered by reporting inconsistencies, preliminary data suggest that social media can increase participation and reduce per-participant cost. The adoption of consistent standards for reporting recruitment and enrollment outcomes is required to advance our understanding and use of social media to support clinical trial success.

Abstract:PurposeAlthough leveraging social media to access healthcare information is nothing new, a boom in short video apps offers new potential for disseminating health-related information. However, it is still unclear how short video apps might facilitate and benefit users’ consumption of health information. Furthermore, the technology features of short video apps complicate attempts to conduct research about them; as a consequence, they have been understudied. For addressing these concerns, this study adopts an affordance perspective to investigate the relationship between affordances and user experience and to examine factors that contribute to users’ intention to continue using short video apps to obtain health information.Design/methodology/approachDrawing upon affordance theory, we constructed a research model that integrates four types of affordances (livestreaming, searching, meta-voicing and recommending), three types of user experience (immersion, social presence and credibility perception), and user’s intention to continue use. We employed an online survey and obtained a sample of 372 valid responses from TikTok (DouYin) users in China. The partial least squares (PLS) method was used to analyze the data.FindingsThe study found that the user experience, in terms of social presence, immersion and credibility perception, can significantly predict users’ intention to continue using short video apps to obtain health information. Furthermore, the user experience was positively associated with the different affordances provided by the short video apps.Originality/valueThe findings of this study have several implications. First, the study contributes to the health information behavior literature by incorporating the aspect of user experience. Moreover, the study extends the application of affordance theory to users’ health information acquisition, and it carries some practical implications on how to leverage the great potential of short video apps to serve public health communication better.

Abstract:This study explores how people engage with educational and motivational content on TikTok. We conducted a mixed-methods content analysis of N = 400 health videos from the prosocial EduTok campaign. Two theories guided our analysis of content: the motivational theory of role modeling and the health belief model. Our results revealed that audiences most frequently engaged with educational videos related to diet, exercise, and sexual health. Role model appeals were prominently featured and highly engaged with. However, these videos often framed health promotion through an ideal lens, without including information needed for behavior change attainability. The prevalence of constructs from the health belief model in videos varied. Videos emphasizing prevention, cues to action, and behavior antecedents including perceived benefits and severity attracted more views and engagement than videos that did not include these concepts. We further observed a trend in which content creators used severity in a sensational manner to elicit shock and outrage, which in turn boosted content virality. When included, videos featuring efficacy appeals garnered higher engagement. However, these appeals were less common and had limited reach. Overall, our findings provide implications for using role modeling and theory-driven appeals in social media eHealth communication.

Abstract:Introduction: TikTok is a widely used social media platform that allows users to post original short-form videos, typically under three minutes long. Of the 121 million TikTok users in the United States, the majority fall in the 18-34 age range. While most videos are classified as "entertainment" users can explore an array of hashtags, including health-related messages. Given its potential to disseminate information on cancer prevention, our project explored the content of #CancerPrevention videos on TikTok. Methods: Using a naïve TikTok account via a web browser, the team extracted the URLs of the top 157 TikTok videos tagged with "CancerPrevention". We simultaneously collected analytics of the number of likes the videos received, comments, bookmarks, shares, length of video, and upload date. We developed a content codebook that was refined over several sessions to enhance agreement. The team double-coded the top 60 videos that were available and in English. The remaining videos were not double-coded before the URL linking to the video expired, thus limiting our sample to 60. Average coder agreement was 89%. New data: The 60 videos included in the analysis totaled 679,346 likes, 12,567 comments, 180,151 bookmarks, and 69,300 shares. The median length of a video was 53 seconds (range 5 seconds to 7 minutes 24 seconds). The oldest video was posted in March 2021 and the newest in October 2023. Nearly half of the videos appeared to be from health professionals (43.5%), while a quarter were from lay individuals (22.6%). However, most videos were not affiliated with medical organizations, non-profits, or other reputable organizations (93.5%). As the hashtag suggested, most videos targeted a general audience (86.4%) and focused on cancer prevention (68.3%). Half of the videos discussed diet and nutrition as cancer prevention strategies (51.7%), while very few emphasized exercise (13.3%), tobacco use (6.7%), or alcohol use (8.3%). Most videos addressed general cancer prevention (69.5%), with colon cancer being the most frequently specified type (13.6%), followed by breast cancer (5.1%). Very few discussed pharmacology or industry-related topics (6.7%). More than half (56.7%) promoted unconventional cancer prevention strategies, such as avoiding sugar consumption, which is claimed in some videos "to feed cancer cells" and eating specific foods to prevent cancer. Conclusions: Despite uncertainties about its future, TikTok remains a highly popular social media platform among youth and young adults in the U.S., with the ability to rapidly disseminate messages to a wide audience. However, our project revealed that TikTok content often lacks evidence-based information, which can potentially lead to the spread of misinformation. Public health practitioners can use this opportunity to evaluate which messages garner the most engagement and develop research-based content that is relevant and accurate, countering the spread of inaccurate or harmful information. Abstract:With video-based social media platforms such as TikTok and Douyin gaining increasing popularity among young people, it is imperative for advertisers and social media practitioners to understand the role of these platforms before placing an advertisement. This experimental study examined four social media platforms (TikTok and Facebook for American users, and Douyin and WeChat for Chinese users) to analyze how young social media users perceive advertisements on each platform. The findings showed that social media users in the US are more critical of video advertisements on TikTok than on Facebook. In addition, the study found that American users are more generous than their Chinese counterparts while rating the personalities featured in advertised brands.

Abstract:The proliferation of short-form video platforms such as Tiktok and Twitter has prompted a major shift in advertising stratagem. Marketers are growing increasingly in favor of short, vertically filmed advertisements in response to a dramatic increase in short-form video media consumption over the last three years. This study explores the comparative effectiveness of short-form advertisements compared to traditional, long-form advertisements. We find that short form advertisements have an advantage over long form advertisements in reach and viewer engagement, proving more effective in our sample of participants than the long form advertisement despite a disadvantage in perceived trust and product relevance. We are able to attribute much of this effectiveness to the relatability, humor, personal touch, and audience-friendly content of short form advertisements. These results underscore a significant transition in the marketing industry, indicating an industrial preference for home-made narrative and individuality over traditional metrics of cinematic polish and trustworthiness.
Digital technologies used in clinical trial recruitment and enrollment including application to trial diversity and inclusion: A systematic review

Abstract:Background Many clinical trials fail because of poor recruitment and enrollment which can directly impact the success of biomedical and clinical research outcomes. Options to leverage digital technology for improving clinical trial management are expansive, with potential benefits for improving access to clinical trials, encouraging trial diversity and inclusion, and potential cost-savings through enhanced efficiency. Objectives This systematic review has two key aims: (1) identify and describe the digital technologies applied in clinical trial recruitment and enrollment and (2) evaluate evidence of these technologies addressing the recruitment and enrollment of racial and ethnic minority groups. Methods We conducted a cross-disciplinary review of articles from PubMed, IEEE Xplore, and ACM Digital Library, published in English between January 2012 and July 2022, using MeSH terms and keywords for digital health, clinical trials, and recruitment and enrollment. Articles unrelated to technology in the recruitment/enrollment process or those discussing recruitment/enrollment without technology aspects were excluded. Results The review returned 614 results, with 21 articles (four reviews and 17 original research articles) deemed suitable for inclusion after screening and full-text review. To address the first objective, various digital technologies were identified and characterized, which included articles with more than one technology subcategory including (a) multimedia presentations (19%, n = 4); (b) mobile applications (14%, n = 3); (c) social media platforms (29%, n = 6); (d) machine learning and computer algorithms (19%, n = 4); (e) e-consenting (24%, n = 5); (f) blockchain (5%, n = 1); (g) web-based programs (24%, n = 5); and (h) virtual messaging (24%, n = 5). Additionally, subthemes, including specific diseases or conditions addressed, privacy and regulatory concerns, cost/benefit analyses, and ethnic and minority recruitment considerations, were identified and discussed. Limited research was found to support a particular technology's effectiveness in racial and ethnic minority recruitment and enrollment. Conclusion Results from this review illustrate that several types of technology are currently being explored and utilized in clinical trial recruitment and enrollment stages. However, evidence supporting the use of digital technologies is varied and requires further research and evaluation to identify the most valuable opportunities for encouraging diversity in clinical trial recruitment and enrollment practices.

Abstract:Importance Marginalized populations have lower levels of clinical trial representation than other populations. Tailoring recruitment materials and providing incentives may improve representation. Objective To determine whether culturally tailored video improves parents' decision to enroll (DTE) Black children in a research registry. Design, Setting, and Participants A randomized clinical trial, using mixed methods and consecutive sampling, was conducted in an urban primary care pediatric clinic. A post hoc study assessed the association of a monetary incentive with DTE. Ninety-three parents were recruited before their child's scheduled clinic visit. Participants completed a survey during which they were randomly assigned to review a paragraph (control group), generic video, or tailored video and received a gift card. Video watchers completed questions about its effectiveness. All participants were invited to join the registry. In the post hoc study, consenting participants followed the control group protocol and the coordinator promised them an ink pen for participating. The study was conducted between November 15, 2022, and August 29, 2023. Intervention A survey regarding the importance of being included in research studies was administered after participants watched a culturally tailored video. Main Outcome and Measures The primary outcome was DTE. Covariates were message effectiveness, clinical trial knowledge, and trust. Intention-to-treat analysis was performed. Results A total of 125 individuals were included. All groups had similar mean (SD) parent age (overall, 32.6 [7.35] years), gender (110 [88.0%] women), and race (116 [92.8%] Black or African American). There was no significant difference in the odds of DTE between the appeal video and control (odds ratio [OR], 1.19; 95% CI, 0.38-3.72) or between the generic video and control (OR, 1.91; 95% CI, 0.54-6.71). However, DTE was higher for all 3 gift card groups vs the ink pen incentive group (eg, control vs ink pen incentive: OR, 5.01; 95% CI, 1.68-14.95). After adjusting for trust, monetary incentives were associated with higher odds of DTE (adjusted OR, 5.92; 95% CI, 2.44-14.39). Conclusions and Relevance In this randomized clinical trial, targeted messaging was not associated with a higher decision to enroll in a pediatric clinical trial registry. There may be a role for combining holistic messaging with monetary incentives to improve enrollment. Trial Registration ClinicalTrials.gov Identifier:NCT06138145

Abstract:YouTube and TikTok have gained increasing recognition as social network sites to support online knowledge acquisition, sharing, and application via social media platforms in the medical field. This study examines which aspect of TikTok and YouTube stimulates doctors, nurses, and any other YouTube and TikTok in the medical setting, to rely on them as sources of knowledge acquisition and sharing to keep their medical repertoire updated. A hybrid model is designed to investigate users’ acceptance of YouTube and TikTok as social media platforms. The model focuses on four main external factors: content richness, innovativeness, satisfaction, and enjoyment. These factors are connected with two TAM constructs which are perceived ease of use and perceived usefulness. The results have shown that both YouTube and TikTok are affected by PEOU, PU, personal innovativeness, flow theory, and content richness. Both social media networks provide up-to-date sources described as useful, enjoyable, and relevant. Nevertheless, the comparative results have shown that YouTube has deeply influenced users’ medical perception and knowledge compared to TikTok. It is created for the very mere purpose of socialization and self-expression. In contrast, YouTube is used for educational and non-educational purposes due to the type of uploaded content and time management. Therefore, TikTok developers and influencers should initiate highly specialized videos and create content that raises awareness of medical field issues.
Using TikTok for public and youth mental health – A systematic review and content analysis

Abstract:Clinical Child Psychology and Psychiatry, Ahead of Print. Globally, TikTok is now the fastest growing social media platform among children and young people; but it remains surprisingly under-researched in psychology and psychiatry. This is despite the fact that social media platforms have been subject to intense academic and societal scrutiny regarding their potentially adverse effects on youth mental health and wellbeing, notwithstanding the inconsistent findings across the literature. In this two part study, we conducted a systematic review concerning studies that have examined TikTok for any public health or mental health purpose; and a follow-up content analysis of TikTok within an Irish context. For study 1, a predetermined search strategy covering representative public and mental health terminology was applied to six databases – PSYCINFO, Google Scholar, PUBMED, Wiley, Journal of Medical Internet Research, ACM – within the period 2016 to 2021. Included studies were limited to English-speaking publications of any design where TikTok was the primary focus of the study. The quality appraisal tool by Dunne et al., (2018) was applied to all included studies. For study 2, we replicated our search strategy from study 1, and converted this terminology to TikTok hashtags to search within TikTok in combination with Irish-specific hashtags. As quantified by the app, the top two "most liked" videos were selected for inclusion across the following three targeted groups: official public health accounts; registered Irish charities; and personal TikTok creators. A full descriptive analysis was applied in both studies. Study 1 found 24 studies that covered a range of public and mental health issues: COVID-19 (n = 10), dermatology (n = 7), eating disorders (n = 1), cancer (n = 1), tics (n = 1), radiology (n = 1), sexual health (n = 1), DNA (n = 1), and public health promotion (n = 1). Studies were predominately from the USA, applied a content analysis design, and were of acceptable quality overall. In study 2, 29 Irish TikTok accounts were analysed, including the accounts of public health authorities (n = 2), charity or non-profit (n = 5), and personal TikTok creators (n = 22). The overall engagement data from these accounts represented a significant outreach to younger populations: total likes n = 2,588,181; total comments n = 13,775; and total shares n = 21,254. TikTok has been utilised for a range of public health purposes, but remains poorly engaged by institutional accounts. The various mechanisms for connecting with younger audiences presents a unique opportunity for youth mental health practitioners to consider, yet there were distinct differences in how TikTok accounts used platform features to interact. Overall, there is an absence of high quality mixed methodological evaluations of TikTok content for public and mental health, despite it being the most used platform for children and young people.

Abstract:The availability of effective antimicrobial agents has greatly decreased the need for surgical intervention in patients who have a pyogenic lung abscess. We describe 3 patients with lung abscesses caused by gram-negative bacteria who failed to respond to medical treatment and who were believed to be unable to withstand lobectomy. Percutaneous insertion of a drainage tube directly into the abscess brought about a dramatic clinical response, with prompt closure of the cavity. This procedure provides an alternative to thoracotomy and lobectomy in treating lung abscesses that fail to respond to medical therapy.

Abstract:Background: In response to the unprecedented challenges posed by the COVID-19 pandemic, conventional recruitment approaches were halted, causing the suspension of numerous clinical trials. Previously, Facebook (Meta Platforms, Inc) has emerged as a promising tool for augmenting participant recruitment. While previous research has explored the use of Facebook for surveys and qualitative studies, its potential for recruiting participants into randomized controlled trials (RCTs) remains underexplored. Objective: This study aimed to comprehensively examine the effectiveness of using Facebook groups and pages to facilitate participant recruitment during the COVID-19 pandemic for an RCT on the effectiveness of a remote parenting program, 1-2-3 Magic, in families who have children with attention-deficit/hyperactivity disorder (ADHD) in the United Kingdom. Methods: We disseminated 5 Facebook posts with an attached digital flyer across 4 prominent ADHD UK support groups and pages run by the National Attention Deficit Disorder Information and Support Services, reaching an audience of around 16,000 individuals over 2 months (January 7 to March 4, 2022). Eligibility criteria mandated participants to be parents or caregivers of a child with diagnosed ADHD aged 12 years or younger, be residing in the United Kingdom, have access to stable internet, and have a device with the Zoom (Zoom Video Communications) app. Participants were required to have never attended 1-2-3 Magic training previously. Prospective participants expressed their interest through Microsoft Forms (Microsoft Corporation). The trial aimed to recruit 84 parents. It is important to note that the term "parent" or "caregiver" in the RCT and in this study within a trial refers to anybody who has legal responsibility for the child. Results: Overall, 478 individuals registered their interest through Microsoft Forms within the stipulated 2-month window. After the eligibility check, 135 participants were contacted for a baseline meeting through Zoom. The first 84 participants who attended a baseline meeting and returned a completed consent form were enrolled. Subsequently, another 16 participants were added, resulting in a final sample of 100 participants. This recruitment strategy incurred negligible expenses and demanded minimal human resources. The approach yielded favorable outcomes by efficiently attracting eligible participants in a condensed time frame, transcending geographical barriers throughout the United Kingdom, which would have been tedious to achieve through traditional recruitment methods. Conclusions: Our experience demonstrated that digital flyers posted in the targeted Facebook groups were a cost-effective and quick method for recruiting for an RCT, which opened during the COVID-19 pandemic when lockdown restrictions were in place in the United Kingdom. Trialists should consider this low-cost recruitment intervention for trials going forward, and in the case of a global pandemic, this novel recruitment method enabled the trial to continue where many have failed. Abstract:Purpose Short-form video advertisements have recently gained popularity and are widely used. However, creating attractive short video advertisements remains a challenge for sellers. Based on the visual-audio perspective and signaling theory, this study investigated the impacts of three visual features (number of shots, pixel-level image complexity and vertical versus horizontal formats) and two audio features (speech rate and average spectral centroid) on user engagement behavior. Design/methodology/approach We conducted a field study on TikTok. To test our various hypotheses, we used regression analysis on 2,511 videos containing product promotion information posted by 60 sellers between January 1, 2020 and November 20, 2021. Findings For visual variables, the number of shots and pixel-level image complexity were found to have nonlinear (inverted U-shaped) relationships with user engagement behavior. The vertical video form was found to have a positive effect on comments and shares. In the case of audio variables, speech rate was found to have a significant positive effect on shares but not on likes and comments. The average spectral centroid was found to have significant negative influences on likes and comments. Practical implications This study provides specific suggestions for sellers who create short-form videos to improve user engagement behavior. Originality/value This study contributes to the literature on short-form video advertising by extending the potential drivers of user engagement behavior. Additionally, from a methodological perspective, it contributes to the literature by using computer vision and speech-processing techniques to analyze user behavior in a video-related context, effectively overcoming the limitations of the widely adopted survey method.
