Abstract:Introduction Obesity and heart failure (HF) are two interrelated epidemics. The treatment of morbidly obese patients with advanced HF and cardiogenic shock is challenging with limited options for definitive therapy. Guidelines recommend against transplantation for patients with BMI (body mass index) >35 kg/m2. The threshold for ventricular assist device (VAD) in a patient with morbid obesity is usually very high, BMI cutoff varying between centers. We present a morbidly obese patient with non-ischemic cardiomyopathy (NICM) admitted with cardiogenic shock, on dual inotropes and impella 5.5, who undergoes robotic sleeve gastrectomy with significant weight reduction, followed by VAD HM3 implantation with intent of bridge to transplant. Case Report A 30-year-old man with NICM, type 2 DM, hypertension, obstructive sleep apnea, morbid obesity (BMI 52) presented with signs and symptoms of acute decompensated HF and cardiogenic shock SCAI C. His RHC demonstrated RA 22, RV 52/11, 22, PA 56/27 (40), PCWP 33, CO 3.2, CI 1.13, SVR 1270, mixed venous 36%. Milrinone was started along with aggressive diuresis, and due to worsening dyspnea, he required VA ECMO cannulation with Impella CP as LV venting strategy. Over next few days, ECMO was decannulated, and impella CP was upgraded to Impella 5.5 via axillary approach. Two weeks later, he underwent robotic sleeve gastrectomy while supported by Impella 5.5 and inotropic therapy. His hemodynamics prior to the procedure were RA 14, PA 56/11 (31), CO 5.2, CI 2, SVR 1192 on milrinone 0.5, dobutamine 5 and impella 5.5 at p8 support. Three weeks post-op, he underwent successful LVAD HM 3 implantation, and was discharged home on post-op day 19 (BMI 39). Weight trends displayed in Figure 1. He continues to lose weight, BMI 34 at 3 month follow up, and he will be listed for transplantation 6 months after LVAD implantation. Conclusion There have been some case reports of bariatric surgery in patients supported already with durable LVADs however this is the first report of using percutaneous temporary mechanical support. Such approach helps HF patients with morbid obesity become more suitable candidates for advanced therapies such as durable LVAD and heart transplantation.

Bridge to Weight Loss: A Case Series

Abstract:Durable left ventricular assist devices (LVADs) are a well-established therapeutic option for patients with advanced heart failure. These devices are often used to “bridge” patients to an orthotopic heart transplantation (HT). Unfortunately, many patients on LVAD support with a body mass index (BMI) above a certain value are not eligible for HT due a lack of suitable donors and the association between obesity and poor outcomes after HT. This case series describes three individuals on LVAD support who were able to successfully lose enough weight to qualify to be listed for an HT. We highlight a systematic, multidisciplinary approach to implementing guideline-driven weight loss strategies, including some aggressive methods ( ie , meal replacements, weight loss medications, and bariatric surgery). In addition to describing the weight loss outcomes, we also discuss barriers and medical challenges during weight loss that are unique to this population.

Abstract:Background Acute decompensated heart failure (HF) refractory to medical management deteriorates quickly to cardiogenic shock, ultimately requiring mechanical circulatory support (MCS). These devices aim to provide circulatory support as a bridge to advanced therapies. We report patient outcomes with the latest minimally invasive temporary MCS device: Impella 5.5 with SmartAssist. Methods All patients who underwent Impella 5.5 implantation between January 2020 and December 2023 were analyzed. Duration of Impella support and peri-implant parameters were collected from electronic medical records. ANOVA was performed and utilized a p-value of <0.05 as significant for statistical analysis. Results During our review period, a total of 99 patients were supported with Impella 5.5 in the setting of cardiogenic shock. 92 were bridged to heart transplant (BTT), of which 17 also received combined kidney transplantation. Two underwent durable VAD implantation (BTVAD), and 3 had native heart recovery (BTR) and explant. Two patients died, yielding a survival rate of 98%. For BTT group, the median age was 60 years (52-65) at implant and a median BMI of 27 kg/m2 (24-31) with a mean support duration of 29 days (SD ±24). Baseline GFR was 49 mL/min/1.73m2 (35-65). In the BTVAD group, the median BMI was 43 kg/m2 (39-46) with an average duration of support of 22 days (SD ±8) with a GFR of 55 mL/min/1.73m2 (38-73). The BTR group had a median BMI of 29 kg/m2 (27-30) with an average duration of support of 15 days (SD ±7) and a lower baseline GFR of 36 mL/min/1.73m2 (26 – 37) compared to BTT or BTVAD. The two patients that died were supported for an average of 42 days (SD ±9.2) and expired due to non-candidacy in the setting of newly detected prostate cancer in one patient and multi-organ failure in another. All BTVAD and BTT patients did not have a stroke or major vascular complications before terminal therapy. Conclusion Our center highlights the role of early Impella 5.5 placement in the stabilization of patients in cardiogenic shock, thereby providing both patients and providers time for decision-making while optimizing end-organ perfusion beyond the accepted duration of 14 days.Impella Data - Entire CohortImpella Data - Stratified by Outcome

Abstract:The use of temporary mechanical circulatory support in cardiogenic shock (CS) is increasing. The Impella (Abiomed) is a percutaneous, microaxial ventricular assist device approved for short-term use in CS that can be implanted peripherally. Direct aortic placement is an alternative commonly performed when the sternum is open, for example, in post-cardiotomy shock or when the peripheral vasculature is of insufficient size or quality for implantation. Herein, we describe direct aortic implantation of the Impella 5.5 via right mini-thoracotomy for a patient in CS secondary to decompensated heart failure as a bridge to candidacy and, ultimately, transplantation.

Abstract:A 61-year-old male presented with high-risk ACS. His LVEF was 25%, with hypokinesis of anterior and inferior walls and coronary angiography showed significant and diffuse three vessel disease, which was not amenable to percutaneous or surgical revascularization. While hospitalized, he developed cardiogenic shock and required hemodynamic support with inotropes and IABP. As a result, he underwent complex PCI to his RCA to improve his cardiac output. The PCI was successful, but his hemodynamic parameters did not change and he remained reliant on inotropic and IABP support. Whenever the IABP would be removed, he would develop flash pulmonary edema and acute renal insufficiency. His work-up for durable LVAD or heart transplantation led to several relative contraindications (severe protein-calorie malnutrition, poor insight, poor social support). so the advanced heart failure team discussion recommended ‘Bridge to Decision’ MCS therapies while those relative contraindications resolved. After two episodes of recurrent cardiogenic shock after removal of IABP, the decision was made to proceed with left axillary Impella® 5.5 insertion for semi-durable hemodynamic support that will allow for physical rehabilitation. Impella® 5.5 was inserted with gradual improvement in hemodynamic status, but he remained Inotropic and vasopressor dependent. His course was further complicated by AKI, for which he was started on continuous veno-venous hemodialysis that was gradually transitioned to intermittent hemodialysis and his renal function gradually returned to normal. At that point, weaning off of Impella® 5.5 was attempted, but he developed flash pulmonary edema and cardiogenic shock. On day 24, the Impella® 5.5 system malfunctioned, with resultant hemodynamics. Another Impella® 5.5 system was inserted via left axillary access and hemodynamic parameters improved (pulmonary edema resolved). He subsequently started ambulating and his nutritional status improved significantly. Throughout this period, the patient's mental status remained intact and he never required intubation, without any infections/sepsis, hemolysis or vascular complications. On day 83 from the original implant, and day 59 from the second implant, he underwent removal of the Impella 5.5 system with subsequent durable LVAD placement with a Heartmate III device on day 90.Our case describes the safe and effective utilization of the Impella® 5.5 temporary MCS system for a prolonged period of time (83 days), as a Bridge-to-Decision strategy.

Abstract:For adults with advanced heart failure, class II/III obesity (body mass index ≥35 kg/m 2 ) represents major challenges, and it is even considered a contraindication for heart transplantation (HT) at many centers. This has led to growing interest in preventing and treating obesity to help patients with advanced heart failure become HT candidates. Among all weight-loss strategies, bariatric surgery (BSx) has the greatest weight loss efficacy and has shown value in enabling select patients with left ventricular assist devices (LVADs) and obesity to lose sufficient weight to access HT. Nevertheless, both BSx and antiobesity medications warrant caution in the LVAD population. In this review, the authors describe and interpret the available published reports on the impact of obesity and weight-loss strategies for patients with LVADs from general and HT candidacy standpoints. The authors also provide an overview of the journey of LVAD recipients who undergo BSx and review major aspects of perioperative protocols.

Ablation of the thyroid remnant and 131I dose in differentiated thyroid cancer.

Abstract:AIMS To compare the efficacy of remnant ablation following a single low dose (specific activity of 131I administered, 1074-1110 MBq) vs. a single high dose (mostly 2775-3700 MBq) of 131I in patients with differentiated thyroid cancer and to determine whether or not the extent of surgery influences outcome. METHODS Nineteen studies have reported the results of low dose 131I ablation. Of these, 11 met our criteria for a comparative analysis. Two additional cohorts of ours were added and these were analysed in two groups based on the extent of surgery (near-total [NT; Woodhouse1] vs. sub-total [ST; Woodhouse2]). There were 518 low dose and 449 high dose patients in all. RESULTS The average failure of a single low dose was 46 +/- 28% (SD). Meta-analysis revealed a statistically significant advantage for a single high over a single low dose and a pooled reduction in relative risk of failure of the high dose of about 27% (P < 0.01). From this we estimate that for every seven patients treated one more would be ablated given a high rather than a low dose (assuming a low dose failure risk of 50%). Also, a significantly greater proportion of patients are ablated after a single high or low dose, if they underwent near-total as opposed to sub-total thyroidectomy (summary relative risk (RR) 1.4; P < 0.05). CONCLUSION High dose 131I is more efficient than low dose for remnant ablation particularly after less than total thyroidectomy. Results suggest that patients with differentiated thyroid cancer should routinely have a total thyroidectomy followed by high dose 131I (2775-3700MBq) for ablation of the remnant.

Abstract:Abstract Increasing numbers of female candidates are running for Congress in American national elections. Despite the rise in female candidates running for office, women are not significantly increasing their presence in the House and Senate. A much hypothesized influence over the electoral fates of female candidates is the role of gender stereotypes. However, political science scholars have struggled to pinpoint the effect of stereotypes on vote choice, if there is any effect. This essay compares the way social psychology and political science scholars theoretically, conceptually and empirically test for gender stereotype influence over evaluations of female candidates and politicians. Differences emerge in the theoretical assumptions made in the two disciplines, the types of measures used in research, and the empirical tests conducted to demonstrate the presence or absence of stereotypes in evaluations of women. The discussion explores how scholars studying female candidates and politicians can integrate insights from social psychology to clarify the role of stereotypes in candidate evaluation and choice.

Abstract:A 66-year-old man with post-myocardial infarction ventricular septal rupture, apical aneurysm, and pseudoaneurysm presenting in cardiogenic shock received a surgically placed temporary microaxial transvalvular left ventricular assist device. This stabilized hemodynamics and end-organ function, and he subsequently underwent successful heart transplantation. A temporary microaxial transvalvular left ventricular assist device can effectively bridge patients with select mechanical complications of myocardial infarction beyond ventricular septal rupture.
Impella 5.5 Bridge to Heart Transplant: An Institutional Series and a Closer Look at Device Removal Technique

Abstract:Limited donor organ availability often necessitates mechanical circulatory support, and recently the Impella 5.5, as a bridge to heart transplant. Of 175 Impella 5.5-supported patients at our institution, 45 underwent transplantation in the largest series to date, for whom we analyzed outcomes. Two methods of complete device explant were evaluated: central Impella transection and removal via axillary graft. Median Impella days were 25 (16–41); median waitlist days were 21 (9–37). Eighty-nine percent (40/45) of patients had device placement via right axillary artery. Seventy-six percent (34/45) underwent central transection for device removal. Four patients (8.9%) required short-term venoarterial extracorporeal membranous oxygenation (VA ECMO) postoperatively for primary graft dysfunction (PGD). Two patients (4.4%) suffered postoperative stroke. Five patients (11.1%) required new RRT postoperatively. One patient (2.2%) returned to the operating room (OR) for axillary graft bleeding. A higher chance of procedural complications was found with the axillary removal technique ( p = 0.014). Median intensive care unit (ICU) days, length of stay (LOS), and postoperative days to discharge were 46 (35–63), 59 (49–80), and 18 (15–24), respectively. Ninety-eight percent (44/45) survived to discharge. Thirty-day survival was 95.6% (43/45), with 1 year survival at 90.3% (28/31). Eighty-eight percent (37/42) remain without rejection. In our institutional experience, Impella 5.5 is a safe and reliable bridge to transplant.

Abstract:T1 deoxyribonucleic acid (DNA) infection of spheroplasts was characterized by the following. A small number of the DNA molecules initiated infectious centers, and a small number of the spheroplasts were infected by T1 DNA. Once a favorable encounter of T1 DNA with spheroplast occurred, a minimum of 20 to 30 min was required for T1 DNA to enter the spheroplast. The mature T1 particles produced in the infection of spheroplasts by T1 DNA were released in a burst, but the average burst size was quite small compared with a normal burst of the phage-infected bacteria. T1 DNA preparations, capable of causing viral growth in spheroplasts, did not require detectable amounts of protein for infectivity, were homogeneous in band and boundary sedimentation, and had a guanine plus cytosine content of 48% and a minimal molecular weight of 35 × 106. Denatured T1 DNA, like denatured λDNA, did not infect spheroplasts. Renatured T1 DNA was not infectious; this was in marked contrast to renatured λDNA.
Structured Weaning From the Impella Left Ventricular Micro-Axial Pump in Acute Myocardial Infarction With Cardiogenic Shock and Protected Percutaneous Coronary Intervention: Experience From a Non-Cardiac Surgical Centre

Abstract:BACKGROUND: The Impella (Abiomed, Danvers, MA, USA) temporary percutaneous left ventricular assist device is increasingly used as mechanical circulatory support in patients with acute myocardial infarction-cardiogenic shock (AMICS) or those undergoing high-risk protected percutaneous coronary intervention (PCI). The optimal weaning regimen remains to be defined.METHOD: We implemented a structured weaning protocol in a series of 10 consecutive patients receiving Impella support for protected PCI or AMICS treated with PCI in a high volume non-cardiac surgery centre. Weaning after revascularisation was titrated to native heart recovery using both haemodynamic and echocardiographic parameters.RESULTS: Ten patients (eight male, two female; aged 43-70 years) received Impella support for AMICS (80%) or protected PCI (20%). Cardiogenic shock was of Society for Cardiac Angiography & Interventions grade C-E of severity in 80%, and median left ventricular end-diastolic pressure was 31 mmHg. Protocol implementation allowed successful weaning in eight of 10 patients with a median support time of 29 hours (range, 4-48 hours). Explantation was associated with an increase in heart rate (81 vs 88 bpm; p=0.005), but no significant change in Cardiac Index (2.9 vs 2.9 L/min/m2), mean arterial pressure (79 vs 82 mmHg), vasopressor requirement (10% vs 10%), or serum lactate (1.0 vs 1.0). Median durations of intensive care and hospital stay were 3 and 6 days, respectively. At 30 days, the mortality rate was 20%, with median left ventricular ejection fraction of 40%.CONCLUSIONS: A structured and dynamic weaning protocol for patients with AMICS and protected PCI supported by the Impella device is feasible in a non-cardiac surgery centre. Larger studies are needed to assess generalisability of such a weaning protocol.

Abstract:Mechanical circulatory support (MCS) as a bridge to lung transplant is an infrequent but accepted pathway in patients who have refractory end-stage pulmonary failure. The American Association of Thoracic Surgeons Expert Consensus Guidelines, published in 2023, recommends venovenous (VV) extracorporeal membrane oxygenation (ECMO) as the initial configuration for those patients who have failed conventional medical therapy, including mechanical ventilation, while waiting for lung transplantation and needing MCS. Alternatively, venoarterial (VA) ECMO can be used in patients with acute right ventricular failure, hemodynamic instability, or refractory respiratory failure. With the advancement in percutaneous venopulmonary (VP) ECMO cannulation techniques, this option is becoming an attractive configuration as bridge to lung transplantation. This configuration enhances stability of the right ventricle, prevents recirculation with direct introduction of pulmonary artery oxygenation, and promotes hemodynamic stability during mobility, rehabilitation, and sedation-weaning trials before lung transplantation. Here, we present a case series of eight percutaneous VP ECMO as bridge to lung transplant with all patients mobilized, awake, and successfully transplanted with survival to hospital discharge.

Temporary Mechanical Circulatory Support after Cardiac Surgery

Abstract:Postcardiotomy shock in the cardiac surgical patient is a highly morbid condition characterized by profound myocardial impairment and decreased systemic perfusion inadequate to meet end-organ metabolic demand. Postcardiotomy shock is associated with significant morbidity and mortality. Poor outcomes motivate the increased use of mechanical circulatory support (MCS) to restore perfusion in an effort to prevent multiorgan injury and improve patient survival. Despite growing acceptance and adoption of MCS for postcardiotomy shock, criteria for initiation, clinical management, and future areas of clinical investigation remain a topic of ongoing debate. This article seeks to (1) define critical cardiac dysfunction in the patient after cardiotomy, (2) provide an overview of commonly used MCS devices, and (3) summarize the relevant clinical experience for various MCS devices available in the literature, with additional recognition for the role of MCS as a part of a modified approach to the cardiac arrest algorithm in the cardiac surgical patient.

Abstract:Perfusion, Ahead of Print. IntroductionPump thrombosis remains a feared complication following placement of durable left ventricular assist devices (LVAD) and can be particularly detrimental to individuals being bridged to heart transplantation. Complications associated with this malfunction not only increase morbidity related to right heart failure, hemolysis, and other organ failure, but may ultimately jeopardize a patient's heart transplant candidacy. Additionally, reoperation for durable ventricular device replacement not only poses additional surgical risks to patients but can potentially complicate or even prohibit transplantation in the future.Case reportThis case report describes a novel configuration of temporary, groin-free, percutaneously-deployed biventricular mechanical circulatory devices to support a patient with biventricular failure due to partial LVAD thrombosis.Discussion/conclusionThe use of less invasive mechanical support measures, such as the approach described here, may help patients achieve adequate hemodynamic support while allowing them to remain ambulatory and facilitate successful bridging to heart transplantation.

Cardiological Challenges Related to Long-Term Mechanical Circulatory Support for Advanced Heart Failure in Patients with Chronic Non-Ischemic Cardiomyopathy

Abstract:Long-term mechanical circulatory support by a left ventricular assist device (LVAD), with or without an additional temporary or long-term right ventricular (RV) support, is a life-saving therapy for advanced heart failure (HF) refractory to pharmacological treatment, as well as for both device and surgical optimization therapies. In patients with chronic non-ischemic cardiomyopathy (NICM), timely prediction of HF's transition into its end stage, necessitating life-saving heart transplantation or long-term VAD support (as a bridge-to-transplantation or destination therapy), remains particularly challenging, given the wide range of possible etiologies, pathophysiological features, and clinical presentations of NICM. Decision-making between the necessity of an LVAD or a biventricular assist device (BVAD) is crucial because both unnecessary use of a BVAD and irreversible right ventricular (RV) failure after LVAD implantation can seriously impair patient outcomes. The pre-operative or, at the latest, intraoperative prediction of RV function after LVAD implantation is reliably possible, but necessitates integrative evaluations of many different echocardiographic, hemodynamic, clinical, and laboratory parameters. VADs create favorable conditions for the reversal of structural and functional cardiac alterations not only in acute forms of HF, but also in chronic HF. Although full cardiac recovery is rather unusual in VAD recipients with pre-implant chronic HF, the search for myocardial reverse remodelling and functional improvement is worthwhile because, for sufficiently recovered patients, weaning from VADs has proved to be feasible and capable of providing survival benefits and better quality of life even if recovery remains incomplete. This review article aimed to provide an updated theoretical and practical background for those engaged in this highly demanding and still current topic due to the continuous technical progress in the optimization of long-term VADs, as well as due to the new challenges which have emerged in conjunction with the proof of a possible myocardial recovery during long-term ventricular support up to levels which allow successful device explantation.
Comparison of Intraoperative Blood Product Use During Heart Transplantation in Patients Bridged with Impella 5.5 versus Durable Left Ventricular Assist Devices

Abstract:Objective To determine if the intraoperative transfusion requirements differ based on the mechanical circulatory device used as a bridge to heart transplantation. Design A single-center retrospective analysis of intraoperative transfusion requirements in all patients undergoing heart or heart/kidney transplantation between November 2018 and July 2021 who were bridged with a temporary (Impella 5.5) or durable left ventricular assist device (LVAD). Setting A tertiary care hospital. Participants 43 adult patients bridged to heart or heart/kidney transplantation with a temporary or durable LVAD between 2018 and 2021 Interventions Recording of baseline characteristics and intraoperative transfusion requirements, including packed red blood cells (pRBCs), fresh frozen plasma (FFP), cryoprecipitate, autologous blood salvage, and platelets. The difference in cardiopulmonary bypass (CPB) times, intensive care unit length of stay (ICU) , and the vasoactive inotrope score following transplantation were also recorded. Measurements and Main Results The primary outcome was the volume of blood products transfused intraoperatively. Patients who underwent bridge to transplantation using the Impella 5.5 had a statistically significant lower median transfusions of cryoprecipitate (155mL versus 200mL p=0.015), autologous blood salvage (675mL versus 1125mL p=<0.01), and platelets (412mL versus 675mL =<0.01). Additionally, there was a trend towards lower transfusion of intra-operative pRBC (4.5 units versus 6.5 units p=0.29) and FFP (675mL versus 800mL p=0.11) , but these were not statistically significant. Conclusions The results suggest a reduction in certain intraoperative transfusion requirements in patients undergoing heart transplantation bridged with the Impella 5.5 versus durable left ventricular assist device.

Abstract:BACKGROUND: Obesity is an independent risk factor for heart failure (HF). Substantial weight loss has been shown to reverse obesity-related cardiomyopathy. This study aimed to report our institution's experience with laparoscopic sleeve gastrectomy (LSG) in patients with morbid obesity and end-stage HF.METHODS: Between 2018 and 2022, 26 patients with end-stage HF were referred for LSG. Of 26 patients, 16 underwent an operation, and 10 did not. After institutional review board approval, a retrospective electronic medical record review was performed to evaluate (i) age, (ii) preoperative weight, (iii) decrease in body mass index (BMI) score, (iv) whether the patient underwent heart transplantation, and (v) mortality. Data analysis was performed using Stata/SE (version 17.0; StataCorp). The Wilcoxon rank-sum test was used to compare continuous variables between the cohorts, and the Pearson chi-square test was used for binary variables with Bonferroni correction applied.RESULTS: The LSG and non-LSG cohorts had comparable ages (P = .088) and starting BMI score (P = .918), and a proportion of patients had a ventricular assist device (P = .191). Patients who underwent LSG lost significantly more weight than the patients who did not, with an average decrease in BMI score of 8.9 kg/m2 (SD, ±6.13) and 1.1 kg/m2 (SD, ±4.10), respectively (P = .040). Of note, 6 patients (37.5%) who underwent LSG eventually underwent transplantation, compared with 2 patients (20.0%) from the matched cohort (P = .884). Of the 26 patients, there were 6 deaths: 2 (12.5%) in the LSG cohort and 4 (40.0%) in the non-LSG cohort (P = .525).CONCLUSION: LSG may be safe and effective for weight loss in patients with HF. This operation may provide patients affected by obesity with end-stage HF the lifesaving opportunity to achieve transplant candidacy.
