Abstract:AIMS To compare the efficacy of remnant ablation following a single low dose (specific activity of 131I administered, 1074-1110 MBq) vs. a single high dose (mostly 2775-3700 MBq) of 131I in patients with differentiated thyroid cancer and to determine whether or not the extent of surgery influences outcome. METHODS Nineteen studies have reported the results of low dose 131I ablation. Of these, 11 met our criteria for a comparative analysis. Two additional cohorts of ours were added and these were analysed in two groups based on the extent of surgery (near-total [NT; Woodhouse1] vs. sub-total [ST; Woodhouse2]). There were 518 low dose and 449 high dose patients in all. RESULTS The average failure of a single low dose was 46 +/- 28% (SD). Meta-analysis revealed a statistically significant advantage for a single high over a single low dose and a pooled reduction in relative risk of failure of the high dose of about 27% (P < 0.01). From this we estimate that for every seven patients treated one more would be ablated given a high rather than a low dose (assuming a low dose failure risk of 50%). Also, a significantly greater proportion of patients are ablated after a single high or low dose, if they underwent near-total as opposed to sub-total thyroidectomy (summary relative risk (RR) 1.4; P < 0.05). CONCLUSION High dose 131I is more efficient than low dose for remnant ablation particularly after less than total thyroidectomy. Results suggest that patients with differentiated thyroid cancer should routinely have a total thyroidectomy followed by high dose 131I (2775-3700MBq) for ablation of the remnant.

Abstract:Percutaneous mechanical circulatory support (PMCS) devices are effective in the treatment of patients with cardiogenic shock and acute decompensated heart failure, as a bridge to the recovery of heart function or further treatment. The intra-aortic balloon pump (IABP) is now the most widely used PMCS. New PMCS devices including Transseptal device of Tandem-Heart, transaortic valve device of Impella and extracorporeal membrane oxygenation (ECMO) might provide more effective hemodynamic supports. Doctors should choose appropriate PMCS devices and their working modes, according to patient's clinical conditions, based on the working principles and hemodynamic effects of the devices, in order to achieve the best effects and help patients live through the crisis of diseases.

Abstract:Purpose of review: There has been a significant expansion of the use of mechanical circulatory support (MCS) devices for patient with acute coronary syndromes (ACS) with cardiogenic shock (CS) and in patients undergoing high-risk percutaneous interventions (PCI). The purpose of this review is to provide an overview of the indications and outcomes of these devices in high-risk cardiac patients. Recent findings: Early revascularization of the culprit-lesion is the immediate goal in ACS patients with CS and the use of pulmonary artery catheters has been associated with improved outcomes in patients with cardiogenic shock. The MCS devices that are used for myocardial support include the intra-aortic balloon pump (IABP), the left ventricle (LV) to aorta pumps, left atrium (LA) to arterial pumps, and right atrial (RA) to arterial pumps. This review provides an overview on the use of these devices in patients with ACS and CS and those undergoing high-risk PCI. Attention is focused on the IABP, the Impella (LV-aorta pump), the TandemHeart (LA-arterial pump), and veno-arterial extracorporeal membrane oxygenation (RA-arterial pump). The indications, evidence, and complications of each device are reviewed. Each device varies in its physiological effect on native heart function, complexity in insertion, and complications. The use of MCS devices for high-risk PCI and CS has increased in recent years and have demonstrated efficacy in supporting a vulnerable myocardium. Although recommendations can be made for use of each device in certain clinical scenarios, further evidence through registries and clinical trials is necessary to guide appropriate device utilization.

Abstract:The field of durable mechanical circulatory support (MCS) has undergone an incredible evolution over the past few decades, resulting in significant improvements in longevity and quality of life for advanced heart failure patients. Despite these successes, substantial opportunities for further improvements remain, including in pump design and ancillary technology, peri- and post-operative management, and the overall patient experience. Ideally, durable MCS devices would be fully implantable, automatically controlled, and minimize need for anticoagulation. Reliable and long term total artificial hearts for bi-ventricular support would be available; and surgical, peri- and post-operative management would be informed by the individual patient phenotype along with computational simulations. In this review, we summarize emerging technological innovations in these areas, focusing primarily on innovations in late preclinical or early clinical phases of study. We highlight important considerations that the MCS community of clinicians, engineers, industry partners and venture capital investors should consider to sustain evolution of the field.

Abstract:Despite advancements in algorithms concerning the management of cardiogenic shock, current guidelines still lack the adequate integration of mechanical circulatory support devices. In recent years, more and more devices have been developed to provide circulatory with or without respiratory support, when conservative treatment with inotropic agents and vasopressors has failed. Mechanical circulatory support can be contemplated for patients with severe, refractory, or acute-coronary-syndrome-related cardiogenic shock. Through this narrative review, we delve into the differences among the types of currently used devices by presenting their notable advantages and inconveniences. We address the technical issues emerging while choosing the best possible device, temporarily as a bridge to another treatment plan or as a destination therapy, in the optimal timing for each type of patient. We also highlight the diverse implantation and removal techniques to avoid major complications such as bleeding and limb ischemia. Ultimately, we hope to shed some light in the gaps of evidence and the importance of conducting further organized studies around the topic of mechanical circulatory support when dealing with such a high mortality rate.

Abstract:Introduction: Cardiogenic shock (CS) is a complex life-threatening condition that results from primary cardiac dysfunction, leading to persistent hypotension and systemic hypoperfusion. Among the therapeutic options for CS are various percutaneous mechanical circulatory support (MCS) devices that have emerged as an increasingly effective hemodynamic support option. Percutaneous therapies can act as short-term mechanical circulatory assistance and can be split into intra-aortic balloon pump (IABP) and non-IABP percutaneous mechanical devices. Areas covered: This review will evaluate the MCS value while considering the mortality rate improvements. We also aim to outline the function of pharmacotherapies and percutaneous hemodynamic MCS devices in managing CS patients to avoid the onset of end-organ dysfunction and improve both early and late outcomes. Expert opinion: Given the complexity, acuity and high mortality associated with CS, and despite the availability and efficacy of pharmacological management, MCS is required to achieve hemodynamic stability and improve survival. Various percutaneous MCS devices are available with varying indications and clinical outcomes. The rates of early mortality and complications were found to be comparable between the four devices, yet, IABP seemed to show the most optimal clinical profile whilst ECMO demonstrated its more long-term efficacy.

Abstract:Six new 1,4-dihydropyridine derivatives were evaluated in vitro for antimicrobial and cytotoxic effects and in vivo for antineoplastic activity. These compounds inhibited the growth of most of Gram-positive and Gram-negative bacteria at concentrations of 50 and 100 micrograms/ml. Concentrations effective against fungi were somewhat lower (25-50 micrograms/ml). The growth of mycobacteria was inhibited at concentrations of 3.1-25 micrograms/ml. Compound IV inhibited the growth of pathogenic mycobacteria including M. tuberculosis resistant to SM and INH at 3.1 or 6.2 micrograms/ml. In cytotoxicity assays, compound II, IV and V appeared the most active. However, none of the 1,4-dihydropyridine derivatives affected the survival time of mice with P388 and L1210 leukemias or melanoma B16. The growth of subcutaneous tumors of sarcoma 180 was inhibited by compounds I, III, IV and V. The effect was dose related.

Abstract:Improved expertise and technological advancements have enabled the safe and effective performance of complex and high-risk-indicated percutaneous coronary intervention (CHIP) in patients previously considered inoperable or high-risk. Mechanical circulatory support (MCS) devices play a crucial role in stabilizing hemodynamics during percutaneous coronary intervention (PCI) -related ischemia, thereby reducing the risk of major adverse events and achieving a more complete revascularization. However, the use of MCS devices in protected PCI is not without risks, including peri-procedural myocardial infarction (MI), bleeding, and access-related complications. Despite numerous observational studies, there is a significant lack of randomized clinical trials comparing different MCS devices in various CHIP scenarios and evaluating their long-term safety and efficacy profiles. This review aims to summarize the current evidence regarding the benefits of MCS devices during CHIPs, offer a practical guide for selecting appropriate devices based on clinical scenarios, and highlight the unanswered questions that future trials need to address.

Abstract:Objective To explore the clinical application of centrifugal ventricular assist device (VAD) and extracorporeal membrane oxygenation (ECMO) at Shanghai Children's Medical Center and summarize the experiences of selecting an ideal method of mechanical circulatory support (MCS) in pediatric cases. Methods From January 2005 to December 2015, 60 cases were support by MCS devices. There were congenital heart defects (n=56) and acute myocarditis (n=4). The mechanical circulatory support devices were used for supporting circulatory functions because of heart failure or arrhythmia. Thirty-two cases were supported with centrifugal VAD (VAD group) and 26 with ECMO (ECMO group) and other 2 cases were started by VAD then transferred to ECMO (VAD-ECMO group). Results The duration for VAD was from 3 to 458 hours. Twenty-two cases were weaned and 20(62.50%) were successfully discharged. The duration of ECMO was from 26 to 498 hours. Sixteen cases were weaned and 14 (53.85%) successfully discharged. None survived in VAD-ECMO group. Bleeding and neurological complications were two major mortal causes. ALCAPA (anomalous origin of coronary artery from pulmonary artery) cases had the biggest ratio in VAD group and the survival rate was 81.82%. Conclusions Pediatric patients with cardiac failure are suitable for VAD and ECMO supports. Cardiac function, respiratory function, age, complications and costs should be considered for selecting from these two methods. Abstract:Recurrent traumatic damage by mechanical action is a major cause of catheter exit site problems in CAPD patients. The purpose of this study was to determine the efficacy of a new external immobilization device (Immobilizer) on wound healing characteristics at the catheter exit site. Sixteen Tenckhoff catheters were surgically implanted in four Yucatan micropigs (two catheters per flank). Immobilization devices were placed on one catheter on each side of each animal; the other catheter was allowed free movement. The devices were changed daily, and exit sites were examined for wound healing progress and infection. Fourteen days following implantation the animals were sacrificed, and the catheter exit sites explanted for histologic examination. The results of this study suggest that immobilization of catheters with this device improves wound healing at the exit site, as evidenced by decreased (p less than 0.05) marsupialization/epidermal down-growth (nonimmobilized 2.9 mm vs. immobilized 1.6 mm), and an increased (p less than 0.05) macrophage/epithelioid cell/giant cell response within the exit tract or in the Dacron cuff (granulomatous response histology score of 2.3 for immobilized vs. a histology score of 1.6 for nonimmobilized). Thus, use of this device may help reduce catheter exit site problems in CAPD patients.

Abstract:Mechanical circulatory support (MCS) devices, including veno-arterial extracorporeal membrane oxygenation (VA-ECMO) and Impella, have been widely used for patients with cardiogenic shock (CS). However, hemodynamics with each device and combination therapy is not thoroughly understood. We aimed to elucidate the hemodynamics with MCS using a pulsatile flow model. Hemodynamics with Impella CP, VA-ECMO, and a combination of Impella CP and VA-ECMO were assessed based on the pressure and flow under support with each device and the pressure–volume loop of the ventricle model. The Impella CP device with CS status resulted in an increase in aortic pressure and a decrease in end-diastolic volume and end-diastolic pressure (EDP). VA-ECMO support resulted in increased afterload, leading to a significant increase in aortic pressure with an increase in end-systolic volume and EDP and decreasing venous reservoir pressure. The combination of Impella CP and VA-ECMO led to left ventricular unloading, regardless of increase in afterload. Hemodynamic support with Impella and VA-ECMO should be a promising combination for patients with severe CS.

Abstract:Background Acute decompensated heart failure (HF) refractory to medical management deteriorates quickly to cardiogenic shock, ultimately requiring mechanical circulatory support (MCS). These devices aim to provide circulatory support as a bridge to advanced therapies. We report patient outcomes with the latest minimally invasive temporary MCS device: Impella 5.5 with SmartAssist. Methods All patients who underwent Impella 5.5 implantation between January 2020 and December 2023 were analyzed. Duration of Impella support and peri-implant parameters were collected from electronic medical records. ANOVA was performed and utilized a p-value of <0.05 as significant for statistical analysis. Results During our review period, a total of 99 patients were supported with Impella 5.5 in the setting of cardiogenic shock. 92 were bridged to heart transplant (BTT), of which 17 also received combined kidney transplantation. Two underwent durable VAD implantation (BTVAD), and 3 had native heart recovery (BTR) and explant. Two patients died, yielding a survival rate of 98%. For BTT group, the median age was 60 years (52-65) at implant and a median BMI of 27 kg/m2 (24-31) with a mean support duration of 29 days (SD ±24). Baseline GFR was 49 mL/min/1.73m2 (35-65). In the BTVAD group, the median BMI was 43 kg/m2 (39-46) with an average duration of support of 22 days (SD ±8) with a GFR of 55 mL/min/1.73m2 (38-73). The BTR group had a median BMI of 29 kg/m2 (27-30) with an average duration of support of 15 days (SD ±7) and a lower baseline GFR of 36 mL/min/1.73m2 (26 – 37) compared to BTT or BTVAD. The two patients that died were supported for an average of 42 days (SD ±9.2) and expired due to non-candidacy in the setting of newly detected prostate cancer in one patient and multi-organ failure in another. All BTVAD and BTT patients did not have a stroke or major vascular complications before terminal therapy. Conclusion Our center highlights the role of early Impella 5.5 placement in the stabilization of patients in cardiogenic shock, thereby providing both patients and providers time for decision-making while optimizing end-organ perfusion beyond the accepted duration of 14 days.Impella Data - Entire CohortImpella Data - Stratified by Outcome

Abstract:Introduction: Mechanical circulatory support (MCS) can help to maintain hemodynamic stability, improve cardiac function, reduce cardiac load, and is an important method for the treatment of advanced heart failure. However, traditional MCS systems [IABP, Impella, TandemHerat, veno-arterial extracorporeal membrane oxygenation (VA-ECMO)] are associated with limitations including trauma, a high rate of complications (hemolysis, bleeding) and require complex care from nurses. Case summary: We report a case of left heart failure resulting from dilated cardiomyopathy in a 24 years-old man. A catheter was placed through the right jugular vein and a drainage tube was positioned under ultrasound guidance through the superior vena cava, right atrium, atrial septum, to the left atrium, and returned to the axillary artery using an extracorporeal magnetic levitation ventricular assist device (VAD). The patient was successfully supported for 10 days and bridged to heart transplant. Discussion: To the best of our knowledge, this is the first report of the use of an extracorporeal magnetic levitation VAD for MCS via a percutaneous approach. Our findings support the wider use of this strategy for patients awaiting myocardial recovery or who require heart bridging or transplantation.

Abstract:T1 deoxyribonucleic acid (DNA) infection of spheroplasts was characterized by the following. A small number of the DNA molecules initiated infectious centers, and a small number of the spheroplasts were infected by T1 DNA. Once a favorable encounter of T1 DNA with spheroplast occurred, a minimum of 20 to 30 min was required for T1 DNA to enter the spheroplast. The mature T1 particles produced in the infection of spheroplasts by T1 DNA were released in a burst, but the average burst size was quite small compared with a normal burst of the phage-infected bacteria. T1 DNA preparations, capable of causing viral growth in spheroplasts, did not require detectable amounts of protein for infectivity, were homogeneous in band and boundary sedimentation, and had a guanine plus cytosine content of 48% and a minimal molecular weight of 35 × 106. Denatured T1 DNA, like denatured λDNA, did not infect spheroplasts. Renatured T1 DNA was not infectious; this was in marked contrast to renatured λDNA.

Abstract:Mechanical circulatory support is, nowadays, a well established treatment modality for end stage heart failure. Patients candidate for cardiac transplantation who decompensate while awaiting graft can be supported by long-term left ventricular assist devices (long-term LVAD). These devices are intracorporeal pumps, expensive and complicated to install, but offer the advantages of a high cardiac output and a good patient mobility. Echocardiography is usually applied to any kind of LVAD to accomplish: preoperative evaluation to exclude contraindications, intraoperative when circulatory support starts to assess left ventricular (LV) filling and unloading, postoperative evaluation during intensive care unit course to prevent hypovolemia, cardiac tamponade and right ventricular failure, controls during assistance for suspected device malfunction. Short-term devices are utilized as acute support after the initial resuscitation of the patient. These devices are bedside extracorporeal pumps, less expensive and less complicated to install. They provide a brief but sufficient time to wait patient recovery (''bridge to recovery'') or to evaluate further therapies (''bridge to long-term device'' or ''bridge to transplantation''). Echocardiography has an important role during implantation to guide cannulae positioning, to prevent insufficient LV unloading, to detect echo contrast enhancement with blood stagnation and intracardiac clot formation, to titrate pharmacologic support, and to assess myocardial recovery.
