Abstract:Objective To observe the changes of tip perfusion index(TPI),systolic blood pressure(SBP),diastolic blood pressure(DBP),heart rate(HR),bispectral index(BIS),and blood noradrenaline(NE)and norepinephrine(E)under different stimulations,so as to evaluate the clinical value of TPI in monitoring the stress response during laparoscopic cholecystectomy.Methods Thirty patients of American Society of Anesthesiologists(ASA)grade Ⅰ-Ⅱ undergoing laparoscopic cholecystectomy under general anesthesia were enrolled in this study.The SBP,DBP,HR,TPI,BIS,the skin temperature of finger,NE and E levels were observed after the patients entering the operation room.Anesthesia was induced with midazolan 0.05 mg/kg,fentanyl 5 μg/kg and propofol effect compartment concentration 4.5 μg/mL by target-controlled infusion.Tracheal intubation was facilitated with vecuronium 0.12 mg/kg.Anesthesia was maintained with TCI of propofol(target effect-site concentration was set at 3.0 μg/mL)and remifentanil(0.25 μg·kg-1·min-1)was used for anesthesia maintenance.The patients were mechanically ventilated.The TPI,the skin temperature of finger,BIS,SBP,DBP,HR,NE and E were observed at the following time points:before induction of anesthesia(T0),after induction of anesthesia and before intubation(T1),immediately intubation(T2),5 min after intubation(T3),immediately skin incision(T4),1 min after pneumoperitoneum(T5)and 5 min after pneumoperitoneum(T6),immediately straw cholecy(T7),1 min after stopping pneumoperitoneum(T8),immediately extubation(T9),and 5 min after extubation(T10).Results Compared with the baseline level at T0,the SBP,DBP and HR values at T2,T5-T7 were significantly higher(P<0.05);and TPI values at T2,T4-T7 were significantly lower than that at T0(P<0.05).The skin temperatures were not significantly different at different time points(P>0.05).The BIS values at T1-T8 were significantly lower than that at T0(P<0.05),and it recovered to the level of T0 at T9 and T10.The plasma NE level at T1 was significantly lower than that at T0(P<0.05),and those at T2-T7 was significantly higher than that at T0(P<0.05).Plasma level of E at T5 was significantly higher than that at T1(P<0.05).We also found that TPI was negatively correlated with SBP,DBP,HR,plasma NE and E(r=-0.963,-0.908,-0.957,-0.845 and-0.324,respectively,P<0.05),and there were no relation of TPI with BIS and the skin temperature(r=-0.188,0.067,respectively,P>0.05).Conclusion TPI can be used to monitor the perioperative stress-induced changes of sympathetic tone in a noninvasive,real time,and continuous manner,and can it may serve as a convenient,effective indicator for clinical anesthesia management.
C-reactive Protein/Albumin Ratio As a Prognostic Indicator in Posttraumatic Shock and Outcome of Multiple Trauma Patients

Abstract:OBJECTIVE:C-reactive protein (CRP)/albumin ratio (CAR) is a new inflammation-based index for predicting the prognosis of various diseases. The CAR determined on admission may help to predict the prognostic value of multiple trauma patients.METHODS:A total of 264 adult patients with severe multiple trauma were included for the present retrospective study, together with the collection of relevant clinical and laboratory data. CAR, CRP, albumin, shock index and ISS were incorporated into the prognostic model, and the receiver operating characteristic (ROC) curve was drawn. Then, the shock index for patients with different levels of CAR was analyzed. Finally, univariate and multivariate logistic regression analyses were performed to identify the independent risk factors for the 28-day mortality of multiple trauma patients.RESULTS:A total of 36 patients had poor survival outcomes, and the mortality rate reached 13.6%. Furthermore, after analyzing the shock index for patients with different levels of CAR, it was revealed that the shock index was significantly higher when CAR was ≥4, when compared to CAR <2 and 2≤ CAR <4, in multiple trauma patients. The multivariate logistic analysis helped to identify the independent association between the variables CAR (P=0.029) and shock index (P=0.019), and the 28-day mortality of multiple trauma patients.CONCLUSION:CAR is higher in patients with severe multiple trauma. Furthermore, CAR serves as a risk factor for independently predicting the 28-day mortality of multiple trauma patients. The shock index was significantly higher when CAR was ≥4 in multiple trauma patients.
Stress-induced Hyperglycemia Ratio As an Independent Risk Factor of In-hospital Mortality in Nonresuscitation Intensive Care Units: A Retrospective Study.

Abstract:Purpose: To determine whether the stress-induced hyperglycemia ratio (SHR) is independently associated with in-hospital mortality in critically ill patients in nonresuscitation ICUs.Methods: In this retrospective cohort study, clinical - and laboratory-related data from patients first ad- mitted to nonresuscitation ICUs were extracted from an open-access database of > 50,000 ICU admissions. Patients were assigned to one of two groups according to an SHR threshold of 1.1. The primary end point of this study was the in-hospital mortality rate. The associations between SHR and length of stay in the ICU and hospital, duration of mechanical ventilation use, and vasopressor use were secondary end points. Logistic regression models were established in the analysis of in-hospital mortality risk, and areas under the receiver operating characteristic curve (AUC) were analyzed to investigate the association between the primary end point and SHR used alone or together with the Simplified Acute Physiology Scale (SAPS) II score. The Youden index, specificity, and sensitivity of SHR and SAPS-II were also assessed. Findings: In this study, 1859 patients were included, 187 of whom (10.06%) died during hospitalization. The group with an SHR of >= 1.1 had a greater in - hospital mortality rate (13.7% vs 7.4%; P < 0.001), longer length of stay both in the ICU and in the hospi- tal, a longer duration of mechanical ventilation use, and a greater rate of vasopressor use. On adjustment for multivariate risk, a 0.1-point increment in SHR was sig- nificantly associated with in-hospital mortality (OR=1.08; 95% CI, 1.00-1.16; P=0.036). The AUC of the association between risk and the SAPS-II score was significantly greater than that with SHR (0.797 [95% CI, 0.576-0.664] vs 0.620 [95% CI, 0.764- 0.830]; P < 0.001). The AUC with SAPS-II + SHR was significantly greater than that with SAPS-II used alone (0.802 [95% CI, 0.770-0.835] vs 0.797 [95% CI, 0.764-0.830]; P=0.023). The Youden index, specificity, and sensitivity of SAPS-II + SHR were 0.473, 0.703, and 0.770, respectively.Implications: Stress-induced hyperglycemia, as eval- uated using the SHR, was associated with increased in - hospital mortality and worse clinical outcomes in these critically ill patients in nonresuscitation ICUs. SHR was an independent risk factor for in-hospital mortality, and when used together with the SAPS-II, added to the capacity to predict mortality in these patients in nonresuscitation ICUs. Abstract:Objective To explore the clinical significance of pancreatic Islets function and Hs-CRP in crically ill patients with stress hyperglycemia. Methods The levels of fasting blood glucose (FBG), fasting insulin (FINS), fasting C-peptide (FCP), high sensitivity C reactive protein (Hs-CRP) were measured after admission and 7 days in 36 crically ill patientswith stress hyperglycemia. and the HOMA-IR、ISI and HOMA-B were calculated. Results The levels of FBG, FINS, FCP, Hs-CRP, HOMA-IR were markedly higher after admission than 7 days (p <0.001), The ISI were markedly lower after admission than 7 days (p <0.001), the HOMA-B were no difference (p >0.05). The level of Hs-CRP was positie correlated with FBG, FINS, FCP, HOMA-IR and negtive correlated with the ISI.Conclusion The increased insulin resistence and the decreased insulin action may play important role in glucose metabolism disorder, inflammatory reaction correlated with the insulin resistence.
Objective Response of Saliva Biomarkers After High-Stress and Mass Casualty Scenarios: A Pilot Study

Abstract:Introduction: Surgeons and military personnel are subjected to high-stress scenarios, which leads to greater rates of burnout. There is room to optimize performance and longevity in these careers by better understanding the body's stress response and applying it to stress management training. This study aims to understand the physiological response in those engaged in trauma scenarios by examining 6 hormones and 42 cytokines during the Intensive Surgical and Trauma Skills Course held at Strategic Operations Inc in San Diego, CA. Methods: Thirty-seven military medical students participated in full immersion, hyper-realistic, and experiential mass casualty high-stress scenarios. Participants were exposed to both operating and emergency room scenarios. Saliva samples were taken after stress inoculation (postinoculation) and again 1 hr after each scenario (recovery). Saliva samples were analyzed using plex assays from Eve Technologies. Data were grouped and analyzed by location and timing using mixed effect nonlinear models. Distributions were compared by location and were analyzed with respect to biomarker levels over the 4-day training period. Results: For emergency room scenarios, there was a decrease in the following cytokines from postinoculation to recovery: epidermal growth factor (EGF), granulocyte colony stimulating factor (G-CSF), epidermal growth factor, granulocyte colony stimulating factor, interleukin 1alpha (IL-1α), interleukin 1beta (IL-1 β), IL-1RA, IL-4, IL-8, IL-10, IL-18, monocyte chemotactic protein (MCP) 1, macrophage colony stimulating factor (M-CSF), macrophage derived chemokine (MDC), CXC motif ligand 9 (MIG/CXCL9), regulated upon activation, normal T cell expressed and secreted (RANTES), and vascular endothelial growth factor (P < 0.05, t > 3.0). For operating room scenarios, there was a decrease in the following cytokines: EGF, G-CSF, IL-1α, IL-1 β, IL-1RA, IL-6, IL-8, IL-10, IL-18, M-CSF, MDC, MIG/CXCL9, RANTES, and tumor necrosis factor alpha. In operating room scenarios, an increase in the hormone levels of progesterone and triodothyronine were observed. The cytokines observed in both groups included EGF, IL-1α, RANTES, MDC, EGF, G-CSF, IL-1RA, IL-18, MIG/CXCL9, IL-8, IL-1β, M-CSF, and IL-10. These significant biomarkers were also graphed and visualized as variable throughout the week. Conclusions: These preliminary data narrow a wide array of stress biomarkers to a smaller, significant group. Surgeons and military personnel are subjected to extraordinary levels of stress with narrow margins for error. To optimize performance and outcomes, it is pertinent to understand the physiological stress response. Future investigation and pairing of cytokine measurements with neuropsychological and performance-based testing will target opportunities to direct training and identify profiles of individuals well suited for stressful environments.
Role of the Stress Index in Predicting Mortality among Patients with Traumatic Femoral Fractures

Abstract:Background: Traumatic femoral fractures, often resulting from high-energy impacts such as traffic accidents, necessitate immediate management to avoid severe complications. The Stress Index (SI), defined as the glucose-to-potassium ratio, serves as a predictor of mortality and adverse outcomes in various trauma contexts. This study aims to evaluate the prognostic value of the SI in patients with traumatic femoral fractures. Methods: This retrospective cohort study included adult trauma patients aged 20 or above with traumatic femoral fractures from the Trauma Registry System at a level 1 trauma center in southern Taiwan between 1 January 2009 and 31 December 2022. At the emergency room, serum electrolyte levels were assessed using baseline laboratory testing. By dividing blood glucose (mg/dL) by potassium (mEq/L), the SI was calculated. The best cut-off value of the SI for predicting mortality was determined using the Area Under the Curve (AUC) of Receiver Operating Characteristic (ROC). Results: A total of 3717 patients made up the final group, of which 3653 survived and 64 died. In comparison to survivors, deceased patients had substantially higher blood glucose levels (199.3 vs. 159.0 mg/dL, p < 0.001) and SIs (53.1 vs. 41.6, p < 0.001). The optimal SI cut-off value for predicting mortality was 49.7, with a sensitivity of 53.1% and a specificity of 78.7% (AUC = 0.609). High SI was associated with increased mortality (4.2% vs. 1.0%, p < 0.001) and longer hospital stays (12.8 vs. 9.5 days, p < 0.001). The adjusted odds ratios of mortality, controlled by comorbidities, the Glasgow Coma Scale, and the Injury Severity Score, were significantly higher in patients with a higher SI (AOR 2.05, p = 0.016) than those with a lower SI. Conclusions: Elevated SI upon admission correlates with higher mortality and extended hospital stay in patients with traumatic femoral fractures. Although the SI has a moderate predictive value, it remains a useful early risk assessment tool, necessitating further prospective, multi-center studies for validation and standardization.

[Predictive Value of Stress-Induced Hyperglycemia on 28 D Risk of All-Cause Death in Intensive Care Patients].

Abstract:OBJECTIVE:To investigate the relationship between stress glucose elevation and the risk of 28 d all-cause mortality in intensive care unit (ICU) patients, and to compare the predictive efficacy of different stress glucose elevation indicators. METHODS:ICU patients who met the inclusion and exclusion criteria in the Medical Information Mart for Intensive Care Ⅳ (MIMIC-Ⅳ) database were used as the study subjects, and the stress glucose elevation indicators were divided into Q1 (0-25%), Q2 (>25%- 75%), and Q3 (>75%-100%) groups, with whether death occurred in the ICU and the duration of treatment in the ICU as outcome variables, and demographic characteristics, laboratory indicators, and comorbidities as covariates, Cox regression and restricted cubic splines were used to explore the association between stress glucose elevation and the risk of 28 d all-cause death in ICU patients; and subject work characteristics [receiver operating characteristic (ROC) and the area under curve (AUC)] were used to evaluate the predictive efficacy of different stress glucose elevation indicators, The stress hyperglycemia indexes included: stress hyperglycemia ratio (SHR1, SHR2), glucose gap (GG); and the stress hyperglycemia index was further incorporated into the Oxford acute severity of illness score (OASIS) to investigate the predictive efficacy of the improved scores: the AUC was used to assess the score discrimination, and the larger the AUC indicated, the better score discrimination. The Brier score was used to evaluate the calibration of the score, and a smaller Brier score indicated a better calibration of the score. RESULTS:A total of 5 249 ICU patients were included, of whom 7.56% occurred in ICU death. Cox regression analysis after adjusting for confounders showed that the HR (95%CI) for 28 d all-cause mortality in the ICU patients was 1.545 (1.077-2.217), 1.602 (1.142-2.249) and 1.442 (1.001-2.061) for the highest group Q3 compared with the lowest group Q1 for SHR1, SHR2 and GG, respectively, and The risk of death in the ICU patients increased progressively with increasing indicators of stressful blood glucose elevation (Ptrend < 0.05). Restricted cubic spline analysis showed a linear relationship between SHR and the 28 d all-cause mortality risk (P>0.05). the AUC of SHR2 and GG was significantly higher than that of SHR1: AUCSHR2=0.691 (95%CI: 0.661-0.720), AUCGG=0.685 (95%CI: 0.655-0.714), and AUCSHR1=0.680 (95%CI: 0.650-0.709), P < 0.05. The inclusion of SHR2 in the OASIS scores significantly improved the discrimination and calibration of the scores: AUCOASIS=0.820 (95%CI: 0.791-0.848), AUCOASIS+SHR2=0.832 (95%CI: 0.804-0.859), P < 0.05; Brier scoreOASIS=0.071, Brier scoreOASIS+SHR2=0.069. CONCLUSION:Stressful glucose elevation is strongly associated with 28 d all-cause mortality risk in ICU patients and may inform clinical management and decision making in intensive care patients.
Evaluating the Impact of Nutritional Risk on Stress-Induced Hyperglycemia and Trauma Patient Outcomes

Abstract:Introduction: Stress-induced hyperglycemia (SIH) and malnutrition are common in trauma patients and are linked to worse outcomes. This study examined the influence of nutritional status, determined by the Geriatric Nutritional Risk Index (GNRI), on the incidence of SIH in trauma patients. Methods: A retrospective analysis was conducted on adult trauma patients admitted to a Level I trauma center from 1 January 2009 to December 31, 2021. Patients were categorized into four groups: SIH, diabetic hyperglycemia (DH), diabetic normoglycemia (DN), and non-diabetic normoglycemia (NDN). Nutritional status was assessed using GNRI: high risk (GNRI < 82), moderate risk (82 ≤ GNRI 98). Incidence of SIH and outcomes were analyzed across GNRI groups. Results: SIH was associated with higher mortality across all GNRI groups compared to NDN, with the highest rate (45.7%) in the high-risk group. Mortality decreased as GNRI increased in all glucose groups. NDN patients had the lowest mortality rates across GNRI groups. There was no correlation between GNRI and SIH incidence (p = 0.259). Conclusion: SIH significantly influenced mortality across all nutritional status groups, with the highest impact in malnourished patients. Although malnutrition did not affect SIH incidence, both SIH and poor nutritional status independently contributed to worse trauma outcomes. Targeted management of hyperglycemia and nutritional deficiencies is crucial for improving survival.

Effect of Tight Glucose Control on Systemic Inflammatory Response and Immune Function during Critical Illness

Abstract:Objective To investigate the correlation of stress hyperglycemia with serum levels of C reactive protein(CRP), interleukin-6(IL-6) and macrophage migration inhibiting factor(MIF) that reflect the inflammatory status and with the expression of human leucocyte antigen DR(HLA-DR) on peripheral monocytes in critically illed patients with trauma or infection; and to investigate the effect of intensive insulin therapy and glycemic control on inflammatory response and immune function in critical illness. Methods Thirty-three critically illed patients were randomly divided into the control group(maintenance blood glucose at a level between 9.99 and 11.10 mmol/L), glycemia controlling group 1(maintenance blood glucose at a level between 4.44 and 6.11 mmol/L), and glycemia controlling group 2(maintenance blood glucose at a level between 6.66 and 8.33 mmol/L). Critically illed patients with APACHEⅡscore≥10 after trauma or infection were enrolled. Patients with insulin-dependent diabetes mellitus, immune disease or history of long-term immunoregulating medicine taking were excluded; patients less than 18 years old were also excluded. Target glycemia was obtained by insulin infusion. The glycemia level was determined every one to two hours. The interval of glycemic measurement might prolong when target glycemia was achieved. The expression of human leucocyte antigen DR on peripheral monocytes was measured in all 33 patients with flow cytometry on 1, 4 and 7 of intensive care in parallel with C reactive protein. In the meantime, Enzyme-linked immunoadsorbent assay(ELISA) was used to determine the interleukin-6 and macrophage migration inhibiting factor. The same nutritional support was applied to all patients. Data were expressed as Mean±SD and analyzed by using Analysis of Variance and correlation and linear regression. A p value <0.05 was regarded as statistically significant in all the tests conducted in this study. Results 1) The incidence of stress hyperglycemia was very high, being 97% in this study. There was no hypoglycemia happened in these patients. 2) Compared with the control group and glycemia controlling group 2, glycemia controlling group 1 needed more insulin to achieve target glycemia. 3) There were much increase in serum levels of C reactive protein, interleukin-6 and macrophage migration inhibiting factor and decrease in the expression of human leucocyte antigen DR on peripheral monocytes in all 33 critically illed patients with trauma or infection. C reactive protein was positively correlated with the level of blood glucose, human leucocyte antigen DR was negatively correlated with the level of blood glucose. 4) The levels of C reactive protein, interleukin-6 and macrophage migration inhibiting factor decreased while the expression of human leucocyte antigen DR on peripheral monocytes gradually recovered with the patients improving. 5) Compared with the control group and glycemia controlling group 2, the human leucocyte antigen DR expression of peripheral monocytes in glycemia controlling group 1 increased. There were significant difference on 4 and 7 after admission in Surgical Intensive Care Unit(P<0.05). C reactive protein and interleukin-6 were much decreased on 7 d after intensive insulin therapy(P<0.05); macrophage migration inhibiting factor was also decreased, but the deference was not significant statistically(P>0.05). Conclusion 1) Serum inflammatory factors are much increased in critically ill patients with trauma or infection. Their immune functions are affected. Stress hyperglycemia happens at the same time. Some inflammatory factors are positively correlated with the level of blood glucose. Immune functions are negatively correlatied with the level of blood glucose. Inflammatory factors gradually decrease and immune functions recover with weakening of stress. 2) Tight glucose control with intensive insulin therapy can increase the human leucocyte antigen DR expression of peripheral monocytes. It also can attenuate the systemic inflammatory response in critically illed patients. 3) Control of glycemia to the normal range may have anti-inflammatory and immunoregulatory effect.

Abstract:Aims: Stress hyperglycemia ratio (SHR), an emerging indicator of critical illness, exhibits a significant association with adverse cardiovascular outcomes. The primary aim of this research endeavor is to evaluate the association between fasting SHR and contrast-induced acute kidney injury (CI-AKI). Methods: This cross-sectional study comprised 3,137 patients who underwent coronary angiography (CAG) or percutaneous coronary intervention (PCI). The calculation of fasting SHR involved dividing the admission fasting blood glucose by the estimated mean glucose obtained from glycosylated hemoglobin. CI-AKI was assessed based on elevated serum creatinine (Scr) levels. To investigate the relationship between fasting SHR and the proportion of SCr elevation, piecewise linear regression analysis was conducted. Modified Poisson's regression analysis was implemented to evaluate the correlation between fasting SHR and CI-AKI. Subgroup analysis and sensitivity analysis were conducted to explore result stability. Results: Among the total population, 482 (15.4%) patients experienced CI-AKI. Piecewise linear regression analysis revealed significant associations between the proportion of SCr elevation and fasting SHR on both sides (≤ 0.8 and > 0.8) [β = -12.651, 95% CI (−23.281 to −2.022), P = 0.020; β = 8.274, 95% CI (4.176 to 12.372), P < 0.001]. The Modified Poisson's regression analysis demonstrated a statistically significant correlation between both the lowest and highest levels of fasting SHR and an increased incidence of CI-AKI [(SHR < 0.7 vs. 0.7 ≤ SHR < 0.9) β = 1.828, 95% CI (1.345 to 2.486), P < 0.001; (SHR ≥ 1.3 vs. 0.7 ≤ SHR < 0.9) β = 2.896, 95% CI (2.087 to 4.019), P < 0.001], which was further validated through subgroup and sensitivity analyses. Conclusion: In populations undergoing CAG or PCI, both lowest and highest levels of fasting SHR were significantly associated with an increased occurrence of CI-AKI.

The Clinical Significance of Acute Insulin Resistance During Surgical Sepsis

Abstract:Objective To investigate the clinical significance of acute insulin resistance during surgical sepsis. Methods Forty surgical patients with sepsis in experimental group and twenty patients with similar APACHEⅡ but without sepsis in experimental control group were enrolled in our study. In the experimental group, the patients were categorized to sepsis group and septic shock and MODS (SM) group according to clinical manifestations, and were also grouped according to different causes of sepsis, i.e. severe acute pancreatitis (SAP) group, intestinal fistula group and other diseases group. Determinations of fasting blood glucose, fasting plasma insulin, clinical nutritional parameters, plasma TNF-α, APACHEⅡ, sepsis-related organ failure assessment (SOFA) and severe scores were simultaneously performed at designated time points. Insulin resistance index was calculated using homeostasis model assessment (HOMA) to assess insulin sensitivity of surgical patients with sepsis. Results (1)A significant elevation of lgHOMA-IR levels was found in 84% of tests in sepsis. Peak lgHOMA-IR values increased in all patients. (2)LgHOMA-IR was significantly higher in patients with sepsis than those without sepsis from the first 24 hours (1.22±0.23 vs 0.73±0.21, P＜0.01) to the 7th day (0.46±0.32 vs 0.30±0.13, P＜0.01). (3)There was no significant difference in lgHOMA-IR among SAP, intestinal fistula and other groups at different times (P＞0.05). (4) There was significant difference in lgHOMA-IR between sepsis group and SM group during the whole septic episode (P＜0.01). During the septic episode, there were significant correlation between lgHOMA-IR and APACHEⅡ(r=0.591, P＜0.01), lgHOMA-IR and SOFA (r=0.496, P＜0.01), lgHOMA-IR and SS(r=0.553, P＜0.01), and lgHOMA-IR and TNF-α(r=0.658, P＜0.01). (5)lgHOMA-IR was independently directly correlated with LDL, prealbumin, cholesterol and triglyceride. R^2 of the equation was 0.188. Conclusion There usually is insulin resistance in surgical sepsis, regardless the primary diseases. Insulin resistance may simply be a marker reflecting an underlying physiological derangement, implying indicates higher mortality. Among several clinical nutrition parameters, LDL, prealbumin, cholesterol, and triglyceride showed closer relationship with insulin resistance in patients with sepsis.

Abstract:A clinicopathological entity associated with an elevated serum IgG4 level and abundant infiltration of IgG4-positive plasma cells is widely known as IgG4-related disease (IgG4 RD). IgG4RD has been identified with Mikulicz’s disease in relation to the aspect of autoimmune systemic disease (1), although such characteristics were first recognized in autoimmune pancreatitis (AIP) (2). Thereafter, many cases have been reported and it has been revealed that IgG4RD shows various changes in multiple organs. Recently, comprehensive diagnostic criteria have been published for the establishment of IgG4RD by the collaboration of two IgG4RD study groups organized by the Ministry of Health, Labor and Welfare of Japan (3). In particular, renal lesions are frequently detected and associated with the involvement of other organs. The main change in the kidney is tubulointerstitial nephritis, but other characteristics including glomerulopathy are also reported (4). From this point of view, diagnostic criteria for IgG4-related kidney disease (IgG4RKD) have also been published by the working group of the Japanese Society of Nephrology (5). In the case reported by Oshima et al. (6), the diagnosis of definite IgG4RKD was established by both criteria (3, 5). This case has two interesting features, which are valuable for investigating the pathogenesis of IgG4RD. One is that the onset follows malignant lymphoma and lymphadenopathy, and the other is that Henoch-Schönlein purpura (HSP) is a complication. As for the former feature, IgG4RD categorized as plasmacytosis should be differentiated from lymphoma or other malignancies. However, it is possible that lymphoma may be a precursor of IgG4RD. Yamamoto et al. (7) showed that the standardized incidence ratio for malignancies including lymphoma in IgG4RD is substantially higher than that in general population. Accordingly, we should consider the relationship between IgG4RD and malignancies. Recently, it has been clarified that helper T2 and regulatory T cells (Treg) play a pivotal role in the pathogenesis of IgG4RD (8-10). On the other hand, it is known that Treg infiltrates are observed in a variety of cancers and are related to the prognosis of neoplastic diseases including lymphoma (11). Therefore it will be interesting to study the linkage of IgG4RD and neoplastic diseases through Treg mechanism. As for the latter feature, the complications of IgG4RKD and HSP have been reported in a few cases (12, 13). Clinical features, i.e., eosinophilia and a high level of serum IgE, show that IgG4RD is induced not only by autoimmunity but also by allergy. The preceding occurrence of HSP also suggests that allergy could be involved in the etiology of IgG4RD. IgG4RD is a still mysterious disease entity, and it has various characteristics of autoimmune disease. The current case by Oshima et al. (6) presents intriguing problems of IgG4RD to researchers who confront this disease.

Abstract:Objective To investigate the correlation of stress hyperglycemia with serum levels of C reactive protein(CRP),interleukin-6(IL-6)and Tumor Necrosis Factor-α(TNF-α)in critical obstetrics patients and to explore the effect of IIT on these immune factors.Methods 42 critical obstetrics patients,who were admitted in the ICU of the first affiliated hospital of the chongqing medical university between January 2005 and December 2007,were randomly divided into the IIT group(maintenance blood glucose at a level between 4.4 and 6.1mmol/L)and routine group(maintenance blood glucose at a level between 10.0 and 11.1mmol/L).Levels of CRP,IL-6 and TNF-α were measured before the treatment and then measured again at 1,3 and 5d after treatment.Results In the routine group,the serum levels of CRP,IL-6 and TNF-α in all cases were significantly increased and were positively correlated with the level of blood glucose.The CRP level of IIT group at 5 d after treatment was(40.92±14.46)mg/L which was significantly decreased compared with the level of routine group which was(87.50±30.20)mg/L(P0.05).The IL-6 level of IIT group at 5 d after treatment was(28.76±6.62)ng/L which was significantly decreased compared with the level of routine group which was(47.45±17.24)ng/L(P0.05).The TNF-α level of IIT group began to decrease to(254.37±39.50)ng/L at 1 d after treatment compared with the level of the routine group which was(325.17±29.47)ng/L(P0.05).Then the TNF-α level of IIT group was significantly decreased to(99.71±13.43)ng/L at 5 d after treatment compared with the level of routine group which was(254.73±19.43)ng/L(P0.05).Conclusion Intensive insulin therapy could obviously regulate the inflammatory reaction of post-injury by controlling glycemia within the normal level to inhibit excessive inflammatory reaction and have a good prognosis.

Abstract:Background: Mental stress and imbalance of its two neural stress systems, the autonomic nervous system (ANS) and the hypothalamic–pituitary–adrenal (HPA) axis, are associated with chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS) and erectile dysfunction (ED). However, the comprehensive analyses of psychological stress and stress systems are under-investigated, particularly in CP/CPPS patients complicated by lower urinary tract symptoms (LUTS) and ED. Materials and methods: Participants were 95 patients in CP/CPPS+ED group, 290 patients in CP/CPPS group, 124 patients in ED group and 52 healthy men in control group. The National Institutes of Health Chronic Prostatitis Symptom Index (NIH-CPSI), the International Index of Erectile Function-5 (IIEF-5) and the International Prostate Symptom Score (IPSS) were used for assessing the disease severity of CP/CPPS, LUTS and ED. Psychometric self-report questionnaires including the Beck Anxiety Inventory (BAI), Perceived Stress Scale (PSS), Type A Personality Test (TAPT) and Symptom Checklist 90 (SCL-90) were completed for distress from physical symptoms. Twenty-five subjects per group were randomly selected for further investigating the changes of the HPA axis and ANS. Saliva samples were taken on 3 consecutive days at 8 specific times with strict reference to time of morning awakening for evaluation of free cortisol. Heart rate variability (HRV) as marker of the ANS was measured using 24 h electrocardiography, and time-and frequency-domain variables were analyzed. Results: The BAI and SCL-90 scores were significantly higher in the CP/CPPS+ED, CP/CPPS and ED groups compared with the control group ( p < 0.01). The PSS scores of both groups with ED were significantly higher than the control group ( p < 0.01). Compared with the CP/CPPS group, the differences of PSS, SCL-90 and TAPT scores were statistically significant in CP/CPPS+ED patients ( p < 0.01). The IPSS scores were shown to have significantly positive correlations with BAI ( r = 0.32, p < 0.0001), PSS ( r = 0.18, p < 0.01) and SCL-90 ( r = 0.19, p < 0.01) in the CP/CPPS patients. However, in all subjects, the IIEF-5 scores were shown to have significantly negative correlations with BAI ( r = −0.17, p < 0.001), PSS ( r = −0.25, p < 0.0001), SCL-90 ( r = −0.20, p < 0.001) and quality of life score in NIH-CPSI ( r = −0.14, p = 0.0075). Cortisol awakening response (CAR) parameters and diurnal cortisol levels did not significantly vary between the four groups. Time-dependent parameters of HRV also did not differ significantly across groups. In the frequency domain analysis, low frequency (LF) was significantly lower in ED patients when compared with CP/CPPS+ED patients ( p = 0.044) and healthy controls ( p = 0.005), high frequency (HF) power was significantly higher in healthy controls compared to patients with ED ( p < 0.001), CP/CPPS ( p < 0.001) and CP/CPPS+ED ( p < 0.001), and the CP/CPPS+ED group had significantly higher LF/HF ratio than the control group ( p = 0.001). Conclusion: CP/CPPS and ED patients score exceedingly high on most psychosocial variables. The symptom scores of LUTS and ED positively correlate with the severity of psychological stress. Our findings also suggest that the ANS sympathovagal imbalance is associated with ED and LUTS in CP/CPPS, whereas HPA axis activity is not.

Abstract:Background: Stress hyperglycemia ratio (SHR) has shown a predominant correlation with transient adverse events in critically ill patients. However, there remains a gap in comprehensive research regarding the association between SHR and mortality among patients experiencing cardiac arrest and admitted to the intensive care unit (ICU). Methods: A total of 535 patients with their initial ICU admission suffered cardiac arrest, according to the American Medical Information Mart for Intensive Care (MIMIC)-IV database. Patients were stratified into four categories based on quantiles of SHR. Multivariable Cox regression models were used to evaluate the association SHR and mortality. The association between SHR and mortality was assessed using multivariable Cox regression models. Subgroup analyses were conducted to determine whether SHR influenced ICU, 1-year, and long-term all-cause mortality in subgroups stratified according to diabetes status. Results: Patients with higher SHR, when compared to the reference quartile 1 group, exhibited a greater risk of ICU mortality (adjusted hazard ratio [aHR] = 3.029; 95% CI: 1.802-5.090), 1-year mortality (aHR = 3.057; 95% CI: 1.885-4.958), and long-term mortality (aHR = 3.183; 95% CI: 2.020-5.015). This association was particularly noteworthy among patients without diabetes, as indicated by subgroup analysis. Conclusion: Elevated SHR was notably associated with heightened risks of ICU, 1-year, and long-term all-cause mortality among cardiac arrest patients. These findings underscore the importance of considering SHR as a potential prognostic factor in the critical care management of cardiac arrest patients, warranting further investigation and clinical attention.

Abstract:Objective: To assess the relationship in healthy adults and critically ill patients between: patient position, body mass index (BMI), patient body temperature, interface pressure (IP) and tissue reperfusion (TR). Also to determine the relationship in critically ill patients between: Sequential Organ Failure Assessment (SOFA) score, Braden Scale score for predicting pressure injury risk, Acute Physiology and Chronic Health Evaluation II (APACHE II) severity of disease classification score, IP and TR. Methods: This study took place in a 27-bed intensive care unit (ICU) of an Australian tertiary hospital. IP and TR outcomes were measured at the sacrum and greater trochanter. Repeated measures analyses of variance (ANOVAs) and doubly multivariate repeated measures ANOVAs were conducted using peak pressure index (PPI), peak time (PT), settled time constant (STC) and normalised hyperaemic area (NHA) measures of TR as outcomes. Participant type, body mass index (BMI), Braden and APACHE II scores and patient body temperature were considered as between-groups factors and covariates. Results: We recruited 23 low- and high-acuity ICU patients and nine healthy adult volunteers. Not all IP readings could be obtained from ICU patients. TR readings were collected from all recruited patients, but not all TR measurements were mutually uncorrelated. Controlling for age, PPI readings differed between participant types (p=0.093), with the highest values associated with high-acuity patients and the lowest with healthy adults; the association was not substantive when controlling for age and BMI. Age was a significant variable (p=0.008), with older participants having higher scores than younger ones. No statistically significant associations between any measured parameter and TR variables were observed. However, temperature was revealed to be related to TR (p=0.091). Conclusions: Although not powered to detect significant effects, this pilot analysis has determined several associations of importance, with differences in outcomes observed between low- and high-acuity ICU patients; and between ICU patients and healthy volunteers.

Abstract:Purpose: This study aims to investigate the relationship between peripheral blood oxytocin (OT), vasopressin (AVP), and subjectively perceived stress and cortisol in male opioid addicts based on the hypothalamic-pituitary-adrenal (HPA) axis. We also investigate the impact of exercise on reducing subjectively perceived stress, craving level, negative reinforcement, anxiety, sleep quality, plasma OT, AVP, and cortisol levels. Methods: Participants were divided into 28 subjects in the low-stress control group (LSC group), 29 subjects in the medium-high stress control group (MTHSC group), and 28 subjects in the moderate-high-stress exercise group (MTHSE group), based on their subjectively perceived stress levels. Subjects in the MTHSE group performed 12 weeks of combined aerobic resistance training (60 min per day, 5 days per week). Plasma OT, AVP, and cortisol concentrations were analyzed via Elisa. PSQI was used to assess the subjective perceived stress, craving, negative reinforcement, anxiety, and sleep quality level, respectively. Mixed-effects ANOVA and Pearson correlation analysis were employed to explore the impact and correlation between different parameters. Result: Plasma OT levels significantly increased (95% CI: –7.48, –2.26), while plasma AVP (95% CI: 2.90, 4.10), and cortisol (95% CI: 19.76, 28.17) levels significantly decreased in the MTHSE group after exercise. The PSS (95% CI: 1.756, 4.815), "Desire and Intention" (95% CI: 1.60, 2.71), and "Negative reinforcement" (95% CI: 0.85, 1.90) (DDQ), SAS (95% CI: 17.51, 26.06), and PSQI (95% CI: 1.18, 3.25) scores of the MTHSE group were significantly decreased after exercise. Plasma OT, plasma cortisol, craving, negative reinforcement and anxiety were negatively correlated. Plasma AVP was positively correlated with craving. Conclusion: As an auxiliary treatment, exercise improves the plasma OT, AVP, and cortisol levels of opioid addicts, and reduces their subjective perceived stress level, desire, negative reinforcement level, anxiety level, and sleep quality. In addition, peripheral plasma OT, AVP, and cortisol may play a role as potential peripheral biomarkers to predict stress in male opioid addicts.

Abstract:Background: To determine the impact of glucose levels at admission and during first week (early phase) on clinical outcomes in patients with acute pancreatitis (AP) and to investigate the relationship between stress hyperglycaemia (SHG) and hypertriglyceridaemia (HTG). Methods: Two independent and prospective databases were retrospectively analysed (n = 1792). Patients admitted with pain of less than 48 hours and confirmed AP were included. SHG was defined as admission blood glucose ≥ 10.00 mmol/L (non-diabetic) or ≥ 16.67 mmol/L (diabetic). Blood glucose records for the first week were inspected to determine whether SHG lasted ≥ 48 hours (persistent) or < 48 hours (transient). Clinical outcomes were compared between designated patient groups using multivariate and trend analyses. The correlation between SHG and HTG (serum triglyceride ≥ 5.65 mmol/L) was also analysed. Results: On admission, SHG was present in 27.8% (499/1792) patients; during the first 48 hours of admission, transient and persistent SHG was found in 31% (556/1792) and 8.0% (144/1792) patients, respectively. Admission SHG was associated with higher incidence of persistent organ failure, acute necrotic collection, major infection, and mortality as well as prolonged length of hospital stay (all P < 0.05). Duration of SHG was also associated with worsened clinical outcomes (all P < 0.05). In HTG-AP patients, more severe clinical outcomes were observed in those who concomitantly had SHG ( P < 0.05). Conclusions: Admission and persistent SHG during the first week of admission worsens clinical outcomes of AP patients. These effects are more pronounced when admission HTG co-existed.
