Abstract:Aim To explore the effect of adenylate cyclase(AC) antagonists SQ22536 and agonist forskolin on acute lung injury induced by lipopolysaccharide.Methods ICR mice were randomly divided into normal saline control group(N group), model group(group L), dexamethasone group(group D),AC antagonists s(group SQ) and AC agonist group(group F).The ALI mouse model was induced by instilling intratracheally with LPS(2 mg·kg-1), and 6 h later, the lung tissue and alveolar lavage fluid(BALF) were harvested, pathological changes in lung were observed, white blood cell and neutrophil, albumin content in BALF and myeloperoxidase(MPO) activity of lung tissue homogenate were determined, and tumor necrosis factor α(TNF-α), interleukin-1β(IL-1β), interleukin 6(IL-6) and cAMP content in lung homogenates were detected by ELISA.Results Compared with normal saline group, a large number of neutrophils infiltrated around the pulmonary vessel and airway 6 h after LPS intratracheal instillation in model group.White blood cells and neutrophils and protein content increased in BALF;MPO activity and cAMP levels increased in lung tissues.In the lung tissue TNF-α and IL-6, IL-1β content increased, compared with model group.Forskolin could improve the pathological changes of lung tissue, reduce the total number of leukocytes, number of neutrophils and protein content in BALF, and reduce MPO activity and TNF-α content in lung tissue, at the same time it increased the cAMP content;SQ22536 had no significant effect when compared with model group.Conclusion AC agonists have protective effects on LPS-induced acute lung injury in mice, and the mechanism may be related to elevating cAMP levels, inhibiting neutrophil adhesion and chemotaxis and reducing inflammatory factor levels.
Pathophysiological changes and injury markers for acute lung injury from blunt impact in infant rabbits

Abstract:Background: Traffic accidents, particularly blunt impacts, cause serious injuries in children. We aimed to assess inflammatory and injury responses in infant rabbits subjected to acute lung injury resulting from blunt impact, with the goal of identifying potential circulatory injury markers. Methods: Forty 4-week-old infant rabbits were subjected to a right chest impact using a Hopkinson bar with 2,600 g. Computed tomography was employed to assess injury severity. Pathological changes were observed using hematoxylin and eosin staining in the control, 0, 24, and 72 h groups, post-injury. Immunohistochemistry was used to examine surfactant protein A (SP-A) changes in right lung tissues and upper main bronchi. Serum levels of interleukin-6 (IL-6), IL-8, and SP-A were measured using ELISA within 24 h post-injury in the control, 0 h, and 24 h groups. Results: Following blunt injury, significant increases were observed in blood white blood cell count ( F = 101.556, P < 0.01) and neutrophil percentage ( F = 104.228, P < 0.01), which gradually decreased after 24 and 72 h. The lung wet/dry weight ratio indicated significant edema ( F = 79.677, P < 0.01), corroborated by hematoxylin and eosin staining showing edema, exudation, and marked granulocyte infiltration in the control, 0 h, 24 h and 72 h groups. SP-A levels decreased rapidly at 0 h, and recovered between 24 and 72 h in the right lung tissues ( F = 6.7, P < 0.05), left lung ( F = 15.825, P < 0.05) and upper main bronchi ( F = 59.552, P < 0.01). The ELISA results showed increasing trends for the control and 0 h groups, while decreasing trends were observed in 24 h group for IL-6 ( F = 58.328, P < 0.01) and IL-8 ( F = 41.802, P < 0.01). Conversely, SP-A exhibited a decreasing trend in the control and 0 h groups but increased in the serum of 24 h group ( F = 52.629, P < 0.01). Discussion: In cases of direct chest trauma in infant rabbits, particularly mild injuries without rib fractures. SP-A levels correlated with pathological changes across all groups and may serve as biomarkers for pediatric blunt lung impact.

Abstract:To investigate the potential risk factors of affecting progression from acute lung injury (ALI) to acute respiratory distress syndrome in severe trauma population.Twenty potential risk factors of affecting progression of acute lung injury were examined by univariate and multivariate logistic analyses among the severe trauma patients in a retrospective study.All of 375 specially severe trauma patients with ALI were included for analysis. The six risk factors that affected the progression from acute lung injury to acute respiratory distress syndrome were sepsis, duration of trauma, APACHE II score, DIC, aspiration of gastric contents, and advanced age. Specific risk factors also affected different patient subpopulations at different degrees.Impact of sepsis, DIC and duration of trauma that predict progression of ALI exists throughout the entire treatment period while aspiration of gastric contents and APACHE II score might affect aggravation of ALI only during the early period; due to deterioration of pulmonary function and severely traumatic injury, advanced age is still an independent risk factor; patients with these risk factors need aggressive supportive cares as early as possible in order to prevent further aggravations.
Evaluation of the Inflammatory Response in a Two-Hit Acute Lung Injury Model Using [18F]FDG Micropet

Abstract:The aim of this study was to investigate whether a two-hit acute lung injury (ALI) model is better than a one-hit model in simulating ALI, and to evaluate the inflammatory response in the lungs in these two models using micro-positron emission tomography (microPET) with [18F]fluorodeoxyglucose (FDG). Sprague Dawley rats were divided into four groups; rats in the lipopolysaccharide (LPS; n=10) and LPS-HCl (n=10) groups were challenged by the intraperitoneal administration of 5 mg/kg LPS, while rats in the normal saline (NS; n=3) and HCl (n=10) groups received the same volume of normal saline solution. Sixteen hours following the administration, the rats in the HCl and LPS-HCl groups received an acid instillation (IT) of 0.5 ml/kg HCl (pH=1.2), while the rats in the remaining two groups received the same volume of normal saline solution. The mean arterial blood pressure (MAP) and blood gas concentrations were measured in all four groups. MicroPET was performed 4 h following HCl IT and the lungs were excised for histopathological examination. The rats in the LPS-HCl group exhibited a higher arterial PaO2 and a lower arterial PaCO2 compared with the rats in the remaining groups. The MAP decreased markedly in the LPS-HCl group, but remained stable in the LPS, HCl and NS groups. MicroPET results identified that the region of interest ratio in the LPS-HCl group (9.00±1.41) was significantly higher compared with those in the LPS (4.01±0.60) and HCl (3.33±0.55) groups (P<0.01). In addition, histological examination showed that the mean lung injury score in the LPS-HCl group (12.70±0.95) was significantly higher compared with those in the HCl (8.40±1.26) and LPS (7.00±0.82) groups (P<0.01). The present study demonstrates that LPS pretreatment significantly magnifies and prolongs the inflammatory response to subsequent acid IT in the lungs. Moreover, it is simpler to induce ALI using the two-hit model than with the one-hit model, and [18F]FDG microPET is a useful tool for evaluating the inflammatory reaction during ALI.
Analysis of Clinical Risk Factors Associated with Mortality of Severely Injured Multiple Trauma Patients with Acute Lung Injury

Abstract:Background It is important to study the factors affecting the clinical mortality of the severe multiple trauma population. The present study was aimed to identify the potential risk factors that could affect mortality rate of acute lung injury (ALI) in severely injured multiple trauma population and to investigate the effects of certain risk factors on the prognosis of different patient subpopulations.Methods This is a follow-up study treating trauma as a single cause for emergency department (ED) and emergency intensive care unit (EICU) admissions. Patients identified with severe multiple trauma with early onset of ALI were enrolled from five trauma centers. Nineteen potential risk factors affecting the prognosis of ALI were examined by univariate and multivariate Logistic regression analyses to identify the ones that affected the mortality of these severe multiple trauma patients.Results There were 687 multiple trauma patients with post-traumatic ALI admitted to ED and EICU during the study period. The six risk factors that affected the mortality with unadjusted odd ratios (ORs) and 95% confidence intervals (Cls) were Acute Physiology Score and Chronic Health Evaluation Score (APACHE) II score, Injury Severity Score (ISS), duration of trauma, age, aspiration of gastric contents, and disseminated intravascular coagulation (DIC). Specific risk factors also affected different patient subpopulations at different degrees (surviving beyond 24 hours, 72 hours, 28 days and with multiple blood transfusions and higher injury scores).Conclusions Factors of APACHE II score, ISS and aspiration of gastric contents that could predict the mortality of ALI may exist in the early stage of trauma. Duration of trauma and DIC that greatly affected and predicted the short- and long-term development and mortality of ALI deserve special attention. Elderly patients (aged beyond 65 years) were the independent risk factor for the secondary sepsis and deterioration of pulmonary function. Patients with these risk factors need aggressive supportive care as early as possible in order to prevent further aggravation.
Changes in Pulmonary Surfactant after Acute Lung Injury Induced by Oleic Acid

Abstract:Objective:To investigate the changes in pulmonary surfactant after acute lung injury (ALI) induced by oleic acid.Methods:Thirtysix adult rabbits were anaesthetized,received tracheostomy tube insertion and then mechanically ventilated.The animals were randomly divided into two groups which were acute lung injury induced by oleic acid inhalation (OA group,n=18) and normal control treated by saline inhalation (NS group,n=18).Total static compliance of respiratory system (TRC),functional residual capacity (FRC),PaO 2 and the serum surfactant protein A level were monitored and assayed at 1,2 and 4 hours after operation.The level of SPA and SPA mRNA expression were determined using Western blot analysis and Northern blot method.After the animals were sacrificed,the tissue content of total phospholipid (TPL),desaturated phosphatidylcholine (DSPC),total proteins (TP) in lungs were analyzed.Results:DSPC/TPL, DSPC/TP and SPA in the BALF were significantly and progressively decreased (P0 05 or P0 01) from 1 hour after oleic acid inhalation whereas TP,STmin and serum SPA concentration were progressively increased (P0 05 or P0 01).SPA mRNA expression showed no change at 1 hour and significant decrease at 2 and 4 hours as compared with control (P0 05).The values of TRC,FRC and PaO 2 were negatively correlated to STmin (P0 05 or P0 001) in OA group.STmin was,in turn,negatively correlated to DSPC/TPL( P0 05 ) and SPA(P0 001) and positively correlated to TP in BALF(P0 005).Conclusions:The increase of STmin of bronchoalveolar lavage fluid(BALF) was closely associated with the decrease of DSPC/TPL and SPA and the increase of TP in lungs.This indicates the etiological role of PS inactivation in the impairment of respiratory function in acute lung injury induced by oleic acid inhalation.

Abstract:OBJECTIVES Biased thinking (to some extent overlapping with the concepts of cognitive distortions and cognitive errors) is a key concept in cognitive therapy of borderline personality disorder (BPD). Specific contents and cognitive processes related to BPD functioning are known. However, most studies are based on self-report measures which present a number of important limitations, in particular the difficulty in assessing non-conscious processes infused by affect. So far, no studies were conducted using valid observer-rated methodology addressing the question of biased thinking in BPD as it unfolds spontaneously in session. DESIGN This is a controlled interview study comparing two matched groups, BPD patients and healthy controls. METHODS A total of N= 25 clinical dynamic interviews with patients presenting with BPD were transcribed and rated using the Cognitive Errors Rating Scale (Drapeau, Perry, & Dunkley, 2008); their cognitive profiles were compared to those of N= 25 healthy controls who underwent the same procedure. RESULTS Overall, results indicated that no between-group difference in the frequency of specific biases was found. However, heightened levels of negative cognitive biases, in particular over-generalizing and fortune-telling, were associated with BPD. Furthermore, negative over-generalizing was associated with the number of BPD symptoms. CONCLUSIONS These results have high levels of ecological validity and are promising for the refinement of cognitive theory of BPD. Clinical implications for assessment and intervention are discussed.
The impact of clinical risk factors in the conversion from acute lung injury to acute respiratory distress syndrome in severe multiple trauma patients

Abstract:Acute lung injury (ALI) and acute respiratory distress syndrome (ARDS) are different stages of the same disease, the aggravated stage of ALI leading to ARDS. Patients with ARDS have higher hospital mortality rates and reduced long-term pulmonary function and quality of life. It is, therefore, important to prevent ALI converting to ARDS. This study evaluated 17 risk factors potentially associated with the conversion from ALI to ARDS in severe multiple trauma. The results indicate that the impact of pulmonary contusion, APACHE II score, gastrointestinal haemorrhage and disseminated intravascular coagulation may help to predict conversion from ALI to ARDS in the early phase after multiple-trauma injury. Trauma duration, in particular, strongly impacted the short- and long-term development of ALI. Being elderly (aged : 65 years) and undergoing multiple blood transfusions in the early phase were independent risk factors correlated with secondary sepsis, deterioration of pulmonary function and transfusion-related acute lung injury due to early multiple fluid resuscitation.
The Changes of Four Cell-Resource Biomarkers in Rats with Acute Respiratory Distress Syndrome

Abstract:Objective:To provide experimental evidence for panel determination in clinical practice, we characterized the early changes of surfactant proteins B (SP-B), plasminogen activator inhibitor-1 (PAI-1), high-mobility group box B1 (HMGB1) and soluble receptor for advanced glycation end products (sRAGE) in acute respiratory distress syndrome (ARDS) induced by lipopolysaccharide (LPS).Methods:In the Institute of Cardiopulmonary Cerebral Resuscitation, Sun Yat Sen University, 36 SD rats were randomly (random number) divided into the LPS group and control group. ARDS rat model in the LPS group was established by intrapulmonary injection of LPS (10 μg/mL) synchronously intrapulmonary injection of saline in the control group. Serum, bronchoalveolar lavage fluid (BALF) and lung tissues were collected at 6, 12 and 24 h after ( n=6 at each time point). After confirmed the success of ARDS model by arterial blood gas (PaO 2) and pathology, we determinated the expressions of SP-B, PAI-1, HMGB1 and sRAGE by enzyme linked immunoassay, immunohistochemistry and Western blot. Paired t test and repeated measurement ANOVA were used for comparison of these biomarkers between the two groups. Results:Progressively decreases of PaO 2 (80.38±1.74 vs 84.17±1.96; 72.20±1.70 vs 81.26±1.57; 68.52±2.45 vs 81.45±1.51) and synchronously worse deterioration at HE pathology were found in the LPS group than those in the control group ( P<0.05). Compared to the control group, a progressively significant increases in HMGB1 (ng/mL) (0.34±0.08, 0.43±0.12, 0.50±0.03), PAI-1 (ng/mL) (40.38±3.06, 50.02±4.82, 57.34±4.67) and SP-B (ng/mL) (2.21±0.11, 2.74±0.15, 3.07±0.28) but significant decreases at sRAGE (ng/mL) (0.17±0.04, 0.14±0.04, 0.11±0.02) were revealed in the serum of LPS group ( P<0.05). Another similarly significant increases in HMGB1 (ng/mL) (0.08±0.02, 0.13±0.02, 0.21±0.03), PAI-1 (ng/mL) (48.88±4.17, 56.66±4.01, 65.83±4.96) and SP-B (ng/mL) (0.68±0.13, 0.80±0.17, 1.12±0.14), but significant decreases at sRAGE (ng/mL) (0.14±0.05, 0.12±0.02, 0.11±0.02) in the BALF of the LPS group compared with those in control group ( P<0.05). Immunohistochemistry showed a consistent up-regulating started at 6 h appeared at PAI-1 (from lung interstitium to lung parenchyma), HMGB1 and SP-B (from lung parenchyma to lung interstitium) while a down-regulating at sRAGE in the LPS group. Western blot showed the elevated expression of SP-B, PAI-1 and HMGB1 but the descent expression of sRAGE following the treating time in the LPS group. Conclusions:In the early stage of ARDS, SP-B, PAI-1 and HMGB1 synchronously changes in serum, and BLAF and lung tissue had the similar tendency, of which can help clinician to early diagnose and identify ARDS by choosing one or combining the biomarkers.

Abstract:Trauma is a major factor that contributes to the risk for acute respiratory distress syndrome (ARDS). Biomarkers that predict the risk, diagnosis, treatment response and prognosis of ARDS after trauma have been widely investigated. In addition to their applications in clinical diagnosis and treatment, these biomarkers provide important insights into our understanding of the pathogenesis of ARDS. This review begins with a brief introduction regarding the incidence and pathogenesis of trauma-associated ARDS. Then, we focus on reviewing the clinical trials that have been designed to investigate the value of biomarkers in ARDS after trauma. Biomarkers with a confirmed value in ARDS have been organized on the basis of key pathogenic processes that are central to ARDS and are described in detail. Among these, angiopoietin 2 (Ang-2), L-selectin, Clara cell protein 16 (CC16), soluable receptor for advanced glycation end products (sRAGE), Surfactant protein D (SP-D), histones, mtDNAs and some biomarker panels had a certain association with the diagnosis and prognosis of trauma-related ARDS. Further investigations are needed regarding the design of trials, the best sampling approaches and the optimal combinations of the biomarker panels.
Determination of Serum Pulmonary Surfactant Protein A and Its Significance in Patients with Acute Lung Injury

Abstract:Objective:To dynamically assess the surfactant protein A(SPA) concentration in serum of patients with acute lung injury(ALI),and discuss its clinical significance.Methods:71 serum samples were collected from 20 ALI patients under mechanical ventilation (ALI group) and 22 from healthy volunteers (control group). In ALI patients,serum samples were obtained both at the time of their entry into the study (T E) and at the time of termination of ventilation (T L,i.e.,when the patients were weaned from ventilator or immediately prior to their death).For most patients,serum samples were also collected when their conditions altered obviously.For each blood sample collected in ALI group,arterial blood gas values and ventilator parameters were recorded to calculate PaO 2/FiO 2 ratio and static total respiratory compliance (TRC).Serum samples were stored for determination of SPA using Western dot blot method.Results:At T E,the serum SPA concentrations in ALI group were significantly higher than those of control group( P 0 001).In ALI group,the serum SPA concentrations in 8 patients who died from acute respiratory distress syndrome (ARDS) and multiple organ failure (MOF) were significantly higher than those of 10 patients who survived( P 0 01).At T L,the serum SPA concentrations in 8 patients who died from ARDS and MOF were significantly higher than those of 10 patients who survived and also their own values at T E( P 0 005 and P 0 05),whereas the serum SPA levels in 10 survivors were lower as compared with their own values at T E ( P 0 001).During the whole observation period,serum SPA concentrations in ALI patients were inversely related to their PaO 2/FiO 2 ratios and TRC values (all P 0 001).Conclusions:Serum SPA concentration in ALI patients is significantly elevated.Serum SPA is a lungspecific serum marker for evaluation of the severity and prognosis of ALI.
Study of Acute Lung Injury after Intestinal Ischemia-Reperfusion in Rats

Abstract:Objective To determine the pulmonary surfactant changes in acute lung injury of adult rats with ischemia reperfusion of intestine. Methods Healthy S D rats (bw 200～250g) were anesthetized and divided into 3 groups: A group as control; B group with SMA occlusion for 60min and C group with intestinal ischemia for 60min and reperfusion for 120min. The left lung was used for measurements of dry/wet weight ratio (D/W) and histopathology, whileas the right lung was lavaged and determined the total proteins (TP), disaturated phosphatidylcholine (DSPC), and total phospholipids (TPL) in the lavage fluid. Results 1. The values and activities of PS: TP:The TP were higher after intestinal ischemia ( P 0.01) and were obviously higher after intestinal ischemia reperfusion. ( P CvsA 0.001; P CvsB 0.05). TPL: The values of TPL and DSPC after ischemia reperfusion were lower than those in the control ( P 0.01). 2. There was pulmonary edema after intestinal ischemia and being more severe after reperfusion. (D/ W P BVsA 0.05; P CVsA 0.01) 3. Pathological examination: There was engorgement in pulmonary vessels, interstitial edema and inflammatory cellular infiltration in group B and group C. But in group C, hemorrhage and atelectasis were also present.Conclusion I/ R resulted in severe lung injuries, as seen by decrease of DSPC and increase of protein in alveolar space which may lead to a secondary surfactant deficiency and pulmonary dysfunction.
Early Identification of Acute Lung Injury in a Porcine Model of Hemorrhagic Shock

Abstract:Background: Acute lung injury (ALI) is a frequent complication after severe trauma. Lung-protective ventilation strategies and damage control resuscitation have been proposed for the prevention of ALI; however, there are no clinical or laboratory parameters to predict who is at risk of developing ALI after trauma. In the present study, we explored pulmonary inflammatory markers as a potential predictor of ALI using a porcine model of hemorrhagic shock. Materials and methods: Female swine were randomized to mechanical ventilation with low tidal volume (VT) (6 mL/kg) or high VT (12 mL/kg). After equilibration, animals underwent pressure-controlled hemorrhage (mean arterial pressure [MAP] 35 ± 5 mmHg) for 1 h, followed by resuscitation with fresh whole blood or Hextend. They were maintained at MAP of 50 ± 5 mmHg for 3 h in the postresuscitation phase. Bronchoalveolar lavage fluids were collected hourly and analyzed for inflammatory markers. Lung samples were taken, and porcine neutrophil antibody staining was used to evaluate the presence of neutrophils. ELISA evaluated serum porcine surfactant protein D levels. Sham animals were used as negative controls. Results: Pigs that underwent hemorrhagic shock had higher heart rates, lower cardiac output, lower MAPs, and worse acidosis compared with sham at the early time points (P < 0.05 each). There were no significant differences in central venous pressure or pulmonary capillary wedge pressure between groups. Pulmonary neutrophil infiltration, as defined by neutrophil antibody staining on lung samples, was greater in the shock groups regardless of resuscitation fluid (P < 0.05 each). Bronchoalveolar lavage fluid neutrophil levels were not different between groups. There were no differences in levels of porcine surfactant protein D between groups at any time points, and the levels did not change over time in each respective group. Conclusions: Our study demonstrates the reproducibility of a porcine model of hemorrhagic shock that is consistent with physiologic changes in humans in hemorrhagic shock. Pulmonary neutrophil infiltration may serve as an early marker for ALI; however, the practicality of this finding has yet to be determined.

Abstract:Objective: To evaluate the efficacy of surfactant and inhaled nitric oxide (iNO) in endotoxin-induced acute lung injury (ALI).Design: Prospective, randomised, controlled experimental study.Setting: A medical university hospital research laboratory.Intervention: Twenty-nine adult rabbits (2.4–3.4 kg) were given two doses of intravenous endotoxin (Escherichia coli) (0.01 mg/kg and, 12 h later, 0.1 mg/kg), and then subjected to mechanical ventilation. After 8 h these animals were allocated to four treatment groups: (1) control, (2) iNO at 20 ppm (NO), (3) surfactant at 100 mg/kg (Surf) and (4) both surfactant and iNO as in groups 2 and 3 (SNO), and ventilated for a further 6 h followed by broncho-alveolar lavage (BAL), analysis of surfactant contents in BAL fluid and histological examination of the lungs.Measurements and results: All the animals had developed ALI with respiratory failure 8 h after the second dose of endotoxin as evidenced by a decrease of PaO2/FIO2 from 520 ± 30 to 395 ± 19 mmHg and dynamic compliance (Cdyn) from 1.20 ± 0.11 to 0.73 ± 0.05 ml/cmH2O × kg, and an increase of intrapulmonary shunting (Qs/Qt) from 7.5 ± 0.8 % to 12.9 ± 1.0 % (all measurements p < 0.01 versus baseline). In the SNO group, values for PaO2/FIO2, Cdyn and Qs/Qt after 6 h were 301 ± 15 mmHg, 0.67 ± 0.05 ml/cmH2O × kg and 16.5 ± 0.8 %, compared to 224 ± 26 mmHg, 0.53 ± 0.04 ml/cmH2O × kg and 24.1 ± 2.0 %, respectively, in the control group (all measurements p < 0.01). Both Surf and NO groups showed intermediate levels of these parameters. In both Surf and SNO groups, the minimum surface tension of BAL fluid was lower, and the content of disaturated phosphatidylcholine/total protein higher, than in the control and NO groups (p < 0.01). Histological features of lung injury were less prominent and wet/dry lung weight ratio lower in the NO, Surf and SNO groups. Decreased surfactant protein A (SP-A) and its mRNA expression were found in all endotoxin-exposed groups, but the SP-A content of the SNO group was moderately improved in comparison to the control group. Surfactant aggregate size was not affected.Conclusion: Early application of surfactant and iNO moderately mitigated ALI as reflected by improvement of lung mechanics, pulmonary perfusion and morphology.

Abstract:Acute respiratory distress syndrome (ARDS) and acute lung injury have a diverse spectrum of causative factors including sepsis, aspiration of gastric contents, and near drowning. Clinical management of severe lung injury typically includes mechanical ventilation to maintain gas exchange which can lead to ventilator-induced lung injury (VILI). The cause of respiratory failure is acknowledged to affect the degree of lung inflammation, changes in lung structure, and the mechanical function of the injured lung. However, these differential effects of injury and the role of etiology in the structure-function relationship are not fully understood. To address this knowledge gap we caused lung injury with intratracheal hydrochloric acid (HCL) or endotoxin (LPS) 2 days prior to ventilation or with an injurious lavage (LAV) immediately prior to ventilation. These injury groups were then ventilated with high inspiratory pressures and positive end expiratory pressure (PEEP) = 0 cmH 2 O to cause VILI and model the clinical course of ARDS followed by supportive ventilation. The effects of injury were quantified using invasive lung function measurements recorded during PEEP ladders where the end-expiratory pressure was increased from 0 to 15 cm H 2 O and decreased back to 0 cmH 2 O in steps of 3 cmH 2 O. Design-based stereology was used to quantify the parenchymal structure of lungs air-inflated to 2, 5, and 10 cmH 2 O. Pro-inflammatory gene expression was measured with real-time quantitative polymerase chain reaction and alveolocapillary leak was estimated by measuring bronchoalveolar lavage protein content. The LAV group had small, stiff lungs that were recruitable at higher pressures, but did not demonstrate substantial inflammation. The LPS group showed septal swelling and high pro-inflammatory gene expression that was exacerbated by VILI. Despite widespread alveolar collapse, elastance in LPS was only modestly elevated above healthy mice (CTL) and there was no evidence of recruitability. The HCL group showed increased elastance and some recruitability, although to a lesser degree than LAV. Pro-inflammatory gene expression was elevated, but less than LPS, and the airspace dimensions were reduced. Taken together, those data highlight how different modes of injury, in combination with a 2 nd hit of VILI, yield markedly different effects.

Abstract:The purpose of this study was to establish a rat model of a brain injury with tracheotomy and compared the wetting effects of different airway humidification liquids, afterward, the best airway humidification liquid was selected for the clinical trial, thus providing a theoretical basis for selecting a proper airway humidification liquid in a clinical setting. Rats were divided into a sham group, group A (0.9% NaCl), group B (0.45% NaCl), group C (0.9% NaCl+ambroxol) and group D (0.9% NaCl+Pulmicort). An established rat model of traumatic brain injury with tracheotomy was used. Brain tissue samples were taken to determine water content, while lung tissue samples were taken to determine wet/dry weight ratio (W/D), histological changes and expression levels of SP-A mRNA and SP-A protein. 30 patients with brain injury and tracheotomy were selected and divided into two groups based on the airway humidification liquid instilled in the trachea tube, 0.45% NaCl and 0.9% NaCl+ambroxol. Blood was then extracted from the patients to measure the levels of SP-A, interleukin-6 (IL-6), interleukin-8 (IL-8) and tumour necrosis factor-α (TNF-α). The difference between group C and other groups in lung W/D and expression levels of SP-A mRNA and SP-A protein was significant (P<0.05). In comparison, the histological changes showed that the lung tissue damage was smallest in group C compared to the three other groups. Aspect of patients, 0.45% NaCl group and 0.9% NaCl+ambroxol group were significantly different in the levels of SP-A, IL-6, IL-8 and TNF-α (P<0.01). In the present study, 0.9% NaCl+ambroxol promote the synthesis and secretion of pulmonary surfactant, and has anti-inflammatory and antioxidant effects, which inhibit the release of inflammatory factors and cytokines, making it an ideal airway humidification liquid.
The Changes of Coagulation and Fibrinolysis System in Acute Lung Injury/acute Respiratory Distress Syndrome

Abstract:Objective To explore the influence of acute lung injury/acute respiratory distress syndrome(ALI/ARDS) on coagulation and fibronolysis system by examining the plasma levels of protein C(PC),thrombomodulin(TM) and plasminogen activator inhibitor-1(PAI-1). Methods The study was divided into two groups: ALI/ARDS patients(n=24) and normal volunteers(n=12).The plasma levels of PC antigen,TM antigen and PAI-1 antigen were determined by enzyme-linked immunosorbent assay(ELISA).The plasma level of active PC(APC) was determined by chromogenic substrate method.Furthermore,ALI/ARDS patients were estimated with APACHE Ⅱ score and multiple organ dysfunction syndrome(MODS) score.ALI/ARDS patients were studied by dividing patients into subsets based on the numbers of organ dysfunction and prognosis. Results The plasma levels of APC and PC antigen were significantly lower in patients with ALI/ARDS than those of the normal controls(P0.05);the levels of TM antigen and PAI-1 antigen were significantly higher in ALI/ARDS patients than those in the normal controls(P0.01).Furthermore,there was a significant negative correlation between the plasma level of APC in patients and APACHE Ⅱ score and MODS score(P0.01);a positive correlation was found between the level of PAI-1 antigen and APACHE Ⅱ score(P0.01) or MODS score(P0.05).However,there was no correlation between the levels of PC antigen,TM antigen and APACHE Ⅱ score or MODS score.The plasma level of APC in patients with single lung dysfunction was significantly higher than that in patients with MODS(P0.01);however,there was no difference in the levels of PC antigen,TM antigen and PAI-1 antigen between patients with single lung dysfunction and patients with MODS.The plasma levels of APC,PC antigen,TM antigen and PAI-1 antigen in patients who survived did not differ from those in patients who died. Conclusions The results suggest that there is an increased procoagulant activity and reduced fibrinolytic activity in patients with ALI/ARDS.Furthermore,there is a correlation between the extent of change in the coagulation and fibronolysis system and the seriousness of ALI/ARDS.

Abstract:Objectives: To characterize the endothelial progenitor cell mobilization in the models of moderate and severe lung injury, we hypothesized that there were differences in endothelial progenitor cell levels and mobilizing cytokines between moderate and severe lung injury. Design: Prospective, randomized, and controlled experimental study. Setting: University research laboratory center. Subjects: Fifteen healthy piglets. Interventions: Piglets were randomly allocated to control, moderate lung injury (acute lung injury), and severe lung injury (acute respiratory distress syndrome) groups. Lung injury was established by intravenous infusion of oleic acid. Animals were mechanically ventilated for 24-48 hours, and then animals were weaned from ventilation and cared for until day 7. Measurements and Main Results: Endothelial progenitor cells were quantified by flow cytometry. After 24 hours, the number of endothelial progenitor cells in peripheral blood increased in the acute lung injury group but was not altered in the acute respiratory distress syndrome group compared to the control group. The number of CD34(+)KDR(+), KDR(+)CD133(+), and CD34(+)KDR(+)CD133(+) cells was higher in the acute lung injury group than in the acute respiratory distress syndrome group. In bone marrow, the number of CD34(+)KDR(+) and KDR(+)CD133(+) cells was greater in acute respiratory distress syndrome animals but not altered in acute lung injury animals at 24 hours. Furthermore, plasma stromal cell-derived factor-1 and vascular endothelial growth factor concentrations were higher in acute lung injury than in acute respiratory distress syndrome at 24 hours. Matrix metalloproteinase-9 and soluble kit ligand levels in bone marrow were reduced in acute respiratory distress syndrome compared with acute lung injury. Lung CD34, KDR, and lung stromal cell-derived factor-1 messenger RNA expression were higher in the acute lung injury group than in the acute respiratory distress syndrome group. Furthermore, the expression of CD34, KDR, and CD133 messenger RNA in lung tissue was correlated with stromal cell-derived factor-1 in the lung. Conclusions: There was a rapid release of endothelial progenitor cells from bone marrow into circulation in moderate acute lung injury, and endothelial progenitor cell mobilization was impaired in acute respiratory distress syndrome.

Abstract:Improvement of pulmonary perfusion and blood oxygenation and prevention of acute lung injury (ALI) may rely on ventilation strategy. We hypothesized that application of a combined surfactant, inhaled nitric oxide (iNO), and pressure support ventilation (PSV) should more effectively protect the lungs from injury. Anesthetized and intubated adult rabbits weighing 2.8 +/- 0.3 kg were allowed to breathe room air while receiving oleic acid intravenously (60 microl/kg). Within 90 min this caused a reduction of Pa(O(2)) from 94 +/- 7 to 48 +/- 3 mm Hg and dynamic lung compliance (Cdyn) from 1.59 +/- 0.22 to 0.85 +/- 0.10 ml/cm H(2)O/kg (both p < 0.01), and increase of intrapulmonary shunting (Q S/Q T) from 9.4 +/- 1.2 to 27 +/- 5% (p < 0.05). PSV was subsequently applied with 3 cm H(2)O of continuous positive airway pressure and FI(O(2)) of 0.3, and the animals were randomly allocated to four groups, receiving: (1) PSV only (Control, n = 10); (2) iNO at 20 ppm (NO, n = 9); (3) surfactant phospholipids at 100 mg/kg (Surf, n = 8); and (4) surfactant at 100 mg/kg and iNO at 20 ppm (SNO, n = 8). PSV level was varied to maintain a tidal volume of 8 to 10 ml/kg for another 12 h or until early animal death. Five animals in the SNO, three each in the NO and Surf group, and one in the Control group survived 12 h (SNO versus Control, p < 0.05). The NO, Surf, and SNO groups had significantly improved mean Pa(O(2)) (> 70 mm Hg, p < 0.05), and reduced Q S/Q T (15, 19, and 17%, respectively, p < 0.05) at 6 and 12 h, but not in the Control group. The SNO group had the highest values of Cdyn at 12 h, alveolar aeration and disaturated phosphatidylcholine-to-total protein ratio in bronchoalveolar lavage fluid, and the lowest wet-to-dry lung weight ratio and lung injury score (p < 0.05). The results indicate that early alleviation of ALI by surfactant, iNO, and PSV is due to synergistic effects, and only PSV in this model had limited effects.
Impact of time intervals on drug efficacy and phenotypic outcomes in acute respiratory distress syndrome in mice

Abstract:Acute respiratory distress syndrome is a severe lung condition resulting from various causes, with life-threatening consequences that necessitate intensive care. The phenomenon can be modeled in preclinical models, notably through the use of lipopolysaccharide (LPS) instillation in mice. The phenotype induced closely recapitulates the human syndrome, including pulmonary edema, leukocyte infiltration, acute inflammation, impaired pulmonary function, and histological damage. However, the experimental designs using LPS instillations are extremely diverse in the literature. This highly complicates the interpretation of the induced phenotype chronology for future study design and hinders the proper identification of the optimal time frame to assess different readouts. Therefore, the definition of the treatment window in relation to the beginning of the disease onset also presents a significant challenge to address questions or test compound efficacy. In this context, the temporality of the different readouts usually measured in the model was evaluated in both normal and neutrophil-depleted male C57bl/6 mice using LPS-induction to assess the best window for proper readout evaluation with an optimal dynamic response range. Ventilation parameters were evaluated by whole-body plethysmography and neutrophil recruitment were evaluated in bronchoalveolar lavage fluids and in lung tissues directly. Imaging evaluation of myeloperoxidase along with activity in lung lysates and fluids were compared, along with inflammatory cytokines and lung extravasation by enzyme-linked immunoassays. Moreover, dexamethasone, the gold standard positive control in this model, was also administered at different times before and after phenotype induction to assess how kinetics affected each parameter. Overall, our data demonstrate that each readout evaluated in this study has a singular kinetic and highlights the key importance of the timing between ARDS phenotype and treatment administration and/or analysis. These findings also strongly suggest that analyzes, both in-life and post-mortem should be conducted at multiple time points to properly capture the dynamic phenotype of the LPS-ARDS model and response to treatment.
