Abstract:Background: Urinary tract infections (UTIs) are the second most frequently encountered nosocomial infectious diseases, and they greatly increase the cost ofmedical care and prolong the duration of hospital stays.Aim: We aimed to evaluate the performance of the Sysmex UF-1000i analyser for the rapid prediction of UTIs in hospitalized patients with or without indwelling catheters at a comprehensive teaching hospital that encounters complex disease types.Methods: Urine specimens (n = 1016) were cultured and examined for WBC, RBC, bacteria (BACT) and yeast-like cell (YLC) count using the Sysmex UF-1000i. The results were compared with the urine culture results. Receiver operating characteristic curve analysis was applied to determine BACT and YLC cutoff values for bacterial and fungal UTIs independently. The diagnostic ability of the UF-1000i was also compared for patients with and without indwelling catheters.Findings: A cutoff value of 38.7/mu L was acceptable for ruling out bacterial UTIs. In this setting, we achieved a sensitivity of 90%, a negative predictive value of 94.5%, a false negative rate of 2.85% (29 cases), and avoided culturing in 52% of the samples. The BACT count presented a larger area under the curve for patients with indwelling catheters than for those without (0.939 vs. 0.861); however, no significant difference in the diagnostic ability of the two curves was found.Conclusion: The Sysmex UF-1000i analyser could be a reliable method for excluding bacterial UTIs in hospitalized patients with or without urinary catheters and could help clinicians determine whether antibiotic therapy is necessary. Abstract:Urinary tract infections (UTIs) caused by bacterial invasion can lead to life-threatening complications, posing a significant health threat to more than 150 million people worldwide. As a result, there is need for accurate and rapid diagnosis of UTIs to enable more effective treatment. Described here is an intelligent diagnostic system constructed for bacterial detection using an immunobiosensor, signal-amplification biochip, and image processing algorithm based on machine vision. This prototype can quickly detect bacteria by collection of enhanced luminescence enabled by the photonic crystals integrated into the biochip. By use of a machine vision algorithm, the very small luminescence signals are analyzed to provide a low detection limit and wide dynamic range. Abstract:The resistance of urinary tract pathogenic bacteria to various antibiotics is increasing, which requires the rapid detection of infectious pathogens for accurate and timely antibiotic treatment. Here, we propose a rapid diagnosis strategy for the antibiotic resistance of bacteria in urinary tract infections (UTIs) based on surface-enhanced Raman scattering (SERS) using a positively charged gold nanoparticle planar solid SERS substrate. Then, an intelligent identification model for SERS spectra based on the deep learning technique is constructed to realize the rapid, ultrasensitive, and non-labeled detection of pathogenic bacteria. A total of 54,000 SERS spectra were collected from 18 isolates belonging to 6 species of common UTI bacteria in this work to realize identification of bacterial species, antibiotic sensitivity, and multidrug resistance (MDR) via convolutional neural networks (CNN). This method significantly simplify the Raman data processing processes without background removing and smoothing, however, achieving 96% above classification accuracy, which was significantly greater than the 85% accuracy of the traditional multivariate statistical analysis algorithm principal component analysis combined with the K-nearest neighbor (PCA-KNN). This work clearly elucidated the potential of combining SERS and deep learning technique to realize culture-free identification of pathogenic bacteria and their associated antibiotic sensitivity.

Abstract:Bacterial antimicrobial resistance (AMR) driven by the abuse of antibiotics is a global highlight challenge, calling for a rapid, economical and generalizable bacterial detection technology. Here, in case of urinary tract infections (UTIs), a naked-eye, antibody-free and multi-functional bacterial assessment platform was designed, which consisted of concanavalin A modified gold nanoparticles (ConA-AuNPs), vancomycin modified gold nanoparticles (Van-AuNPs), and polymyxin B modified Prussian blue nanoparticles (PMB-PBNPs). Based on the fast agglutination of bacterial cells induced by concanavalin A, ConA-AuNPs could aggregate on bacterial cells of Escherichia coli and Staphylococcus aureus , resulting in a visible color change due to alteration of surface plasmon resonance properties within 30 min. Besides, due to the different affinity of vancomycin and polymyxin B to bacteria, Van-AuNPs preferred to bind to Gram-positive bacteria, generating colorimetric response within 2–3 h; while PMB-PBNPs could be reduced colourless Prussian white (PW) by the prior Gram-negative bacterial metabolization in contrast to Gram-positive bacterial metabolization within 4–6 h. Combining our platform with antibiotics, the minimum inhibitory concentration of bacteria could be determined within 4–8 h, which was proved by incubating Escherichia coli and Staphylococcus aureus with various antibiotics. The feasibility was verified by clinical samples, which was consistent with the classical clinical test within only 1/48 of the process timing. Therefore, this colorimetric nanoplatform orderly realized the rapid detection, species identification (Gram-positive and Gram-negative), and susceptibility evaluation of bacteria, satisfying multiple needs from timely clinical diagnosis to accurate medication guidance.

Abstract:Rapid point-of-care (POC) diagnosis of bacterial infection diseases provides clinical benefits of prompt initiation of antimicrobial therapy and reduction of the overuse/misuse of unnecessary antibiotics for nonbacterial infections. We present here a POC compatible method for rapid bacterial infection detection in 10 min. We use a large-volume solution scattering imaging (LVSi) system with low magnifications (1-2×) to visualize bacteria in clinical samples, thus eliminating the need for culture-based isolation and enrichment. We tracked multiple intrinsic phenotypic features of individual cells in a short video. By clustering these features with a simple machine learning algorithm, we can differentiate Escherichia coli from similar-sized polystyrene beads, distinguish bacteria with different shapes, and distinguish E. coli from urine particles. We applied the method to detect urinary tract infections in 104 patient urine samples with a 30 s LVSi video, and the results showed 92.3% accuracy compared with the clinical culture results. This technology provides opportunities for rapid bacterial infection diagnosis at POC settings.

Abstract:Various commercial diagnostic kits have been used to detect uropathogens. However, non-intensive, sensitive, and instantaneous detection of bacteria in urine is still at its infancy. Herein, a facile attempt was carried out to detect the most common uropathogens using anthocyanin-loaded silver nanoparticles as the indicator. The significance of the current research was to apply the crucial sensing ability of eggplant peel anthocyanin to detect each pathogen by exhibiting discrete colors, based on pH of the urine sample. The novelty of this study was the ability of a single indicator compound to distinctively differentiate multiple uropathogens, as monocultures or consortia, with accurate chromacity, simultaneously and instantaneously. Anthocyanin (AC) from eggplant ( Solanum melongena ) peels acted as a pH responsive sensor in the prepared indicator solution. The solution was used to sense three each of Gram-positive and Gram-negative uropathogens, as both monoculture and consortium. AC was loaded onto silver nanoparticles (AgNP) to stabilize the pigment. An evident color change due to the function of AC was considered as the end-point of detection. The optimized conditions for the study were obtained as: 50 μL indicator solution, 10 5 CFU.mL −1 inoculum, and instantaneous sensing time. Deprotonation and protonation of AC due to ammonia and acids, evolved from the microbes, subsequently varied the pH level in the medium. These resulted in the generation of colors as hues of yellow, orange, green and blue and semi-quantitatively represented using average RGB values and delta E analysis obtained after the indicator solution was added to 24 h bacterial growth. In conclusion, the proposed solution can be a potential alternative to the conventional diagnostic tools as a specific, sensitive, rapid, precise and affordable indicator for point-of-care diagnosis of urinary tract infection.

Abstract:Fast and accurate detection of microbial cells in clinical samples is highly valuable but remains a challenge. Here, a simple, culture-free diagnostic system is developed for direct detection of pathogenic bacteria in water, urine, and serum samples using an optical colorimetric biosensor. It consists of printed nanoarrays chemically conjugated with specific antibodies that exhibits distinct color changes after capturing target pathogens. By utilizing the internal capillarity inside an evaporating droplet, target preconcentration is achieved within a few minutes to enable rapid identification and more efficient detection of bacterial pathogens. More importantly, the scattering signals of bacteria are significantly amplified by the nanoarrays due to strong near-field localization, which supports a visualizable analysis of the growth, reproduction, and cell activity of bacteria at the single-cell level. Finally, in addition to high selectivity, this nanoarray-based biosensor is also capable of accurate quantification and continuous monitoring of bacterial load on food over a broad linear range, with a detection limit of 10 CFU mL-1 . This work provides an accessible and user-friendly tool for point-of-care testing of pathogens in many clinical and environmental applications, and possibly enables a breakthrough in early prevention and treatment.
Machine learning-assisted image label-free smartphone platform for rapid segmentation and robust multi-urinalysis

Abstract:This study presents a groundbreaking approach for the early detection of chronic kidney disease (CKD) and other urological disorders through an image-label-free, multi-dipstick identification method, eliminating the need for complex machinery, label libraries, or preset coordinates. Our research successfully identified reaction pads on 187 multi-dipsticks, each with 11 pads, leveraging machine learning algorithms trained on human urine data. This technique aims to surpass traditional colourimetric methods and concentration-colour curve fitting, offering more robust and precise community screening and home monitoring capabilities. The developed algorithms enhance the generalizability of machine learning models by extracting primary colours and correcting urine colours on each reaction pad. This method's cost-effectiveness and portability are significant, as it requires no additional equipment beyond a standard smartphone. The system's performance rivals professional medical equipment without auxiliary lighting or flash under regular indoor light conditions, effectively managing false positives and negatives across various categories with remarkable accuracy. In a controlled experimental setting, we found that random forest algorithms, based on a Bagging strategy and applied in the HSV colour space, showed optimal results in smartphone-assisted urinalysis. This study also introduces a novel urine colour correction method, significantly improving machine learning model performance. Additionally, ISO parameters were identified as crucial factors influencing the accuracy of smartphone-based urinalysis in the absence of additional lighting or optical configurations, highlighting the potential of this technology in low-resource settings.

Abstract:Rapid and accurate detection of bacteria in urine is the key to improving the diagnostic efficiency of urinary tract infections. In this work, a microfluidic device was developed for visually quantifying pathogenic bacteria in urine without any external equipment. This pioneering strategy has high sensitivity, selectivity and accuracy for rapid detection of Escherichia coll. and Staphylococcus aureus simultaneously. It not only shortens the entire test time to 1.5 h, but also successfully reduces the detection limit to 1 CFU/mL, which significantly improves the early diagnosis rate of bacterial infections. Due to the strong affinity of aptamers to target bacteria and the amplification of signals by platinum nanoparticle probes (Pt NP probes) with high catalytic activity, the sensitivity of the device has reached 1 CFU/mL. Moreover, the detection results of UTIs in clinical specimens were consistent with the gold standard, which not only verifies the good accuracy and reliability of the method in such a complex liquid environment, but also implies that the application of the device is paving a new path for biomarker detection and disease diagnosis.

Abstract:Successful discrimination of complex biological samples by sensor array remains a challenging problem. Herein, a three-unit fluorescence sensor array was fabricated using fluorescent molecules, which interact with proteins and generate rich response signals, to realize the discrimination of urinary proteins. As a proof of concept, six urinary proteins were selected as analytes. Results showed 100% accuracy in differentiation and quantitative discrimination of every single protein and their complex mixtures. Moreover, protein profiles of urine samples from healthy people and patients with different urinary diseases, including tubular injury (TI), diabetic nephropathy (DN), and nephropathy (Nep), were well distinguished.

Abstract:Rapid identification and antimicrobial susceptibility testing (AST) of bacteria are key interventions to curb the spread and emergence of antimicrobial resistance. The current gold standard identification and AST methods provide comprehensive diagnostic information but often take 3 to 5days. Here, a compound Raman microscopy (CRM), which integrates Raman spectroscopy and stimulated Raman scattering microscopy in one system, is presented and demonstrated for rapid identification and AST of pathogens in urine. We generated an extensive bacterial Raman spectral dataset and applied deep learning to identify common clinical bacterial pathogens. In addition, we employed stimulated Raman scattering microscopy to quantify bacterial metabolic activity to determine their antimicrobial susceptibility. For proof-of-concept, we demonstrated an integrated assay to diagnose urinary tract infection pathogens, S. aureus and E. coli. Notably, the CRM system has the unique ability to provide Gram-staining classification and AST results within similar to 3h directly from urine samples and shows great potential for clinical applications.

Abstract:Urinalysis is attractive in non‐invasive early diagnosis of bladder cancer compared with clinical gold standard cystoscopy. However, the trace bladder tumor biomarkers in urine and the particularly complex urine environment pose significant challenges for urinalysis. Here, we report a clinically adoptable urinalysis device that integrates molecular‐specificity indium gallium zinc oxide field‐effect transistor (IGZO FET) biosensor arrays, a device control panel and an Internet terminal for directly analyzing 5 bladder tumor‐associated proteins in clinical urine samples for bladder cancer diagnosis and classification. The IGZO FET biosensors with engineered sensing interfaces provide high sensitivity and selectivity for trace proteins identification in complex urine environment. Integrating with a machine‐learning algorithm, our device could identify bladder cancer with an accuracy of 95.0% in a cohort of 197 patients and 75 non‐BC individuals, distinguishing cancer stages with an overall accuracy of 90.0% and assessing bladder cancer recurrence after surgical treatment. A Dual Fluorescence Turn‐On Sensor Array Formed by Poly(para‐aryleneethynylene) (PPE) and Aggregation‐Induced Emission (AIE) Boosts Sensitivity in Multiplex Bacterial Recognition

Abstract:Bacterial infections have emerged as the leading causes of mortality and morbidity worldwide. Array-based sensing methods provide significant advantages for the simultaneous detection of multiple bacteria, thereby offering immense potential for the prompt diagnosis of infectious diseases. However, the detection of multiple bacteria at low concentrations to meet clinical testing requirements remains exceptionally challenging. Herein, we developed a dual-channel fluorescence "turn-on" sensor array, comprising six electrostatic complexes formed from one negatively charged poly(para-aryleneethynylene) (PPE) and six positively charged aggregation-induced emission (AIE) fluorophores. The 6-element array enabled the simultaneous identification of 20 bacteria (OD600 = 0.005) within 30s (99.0% accuracy), demonstrating significant advantages over the array constituted by the 7 separate elements that constitute the complexes. Meanwhile, the array realized different mixing ratios and quantitative detection of prevalent bacteria associated with urinary tract infection (UTI). It also excelled in distinguishing six simulated bacteria samples in artificial urine. Remarkably, the limit of detection for E. coli and E. faecalis was notably low, at 0.000295 and 0.000329 (OD600), respectively. Finally, optimized by diverse machine learning algorithms, the designed array achieved 96.7% accuracy in differentiating UTI clinical samples from healthy individuals using a random forest model, demonstrating the great potential for medical diagnostic applications.

Abstract:Urinary tract infections (UTIs) are a severe public health problem caused by mono- or poly-bacteria. Culture-based methods are routinely used for the diagnosis of UTIs in clinical practice, but those are time consuming. Rapid and unambiguous identification of each pathogen in UTIs can have a significant impact on timely diagnoses and precise treatment. Matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-TOF MS) is an alternative method for the identification of pathogens in clinical laboratories. However, a certain number of pure bacteria are required for MALDI-TOF MS analysis. Here, we explored a strategy combining magnetic enrichment and MALDI-TOF MS for the rapid identification of pathogenic bacterial mixtures in urine. Fragment crystallizable mannose-binding lectin-modified Fe3O4 (Fc-MBL@Fe3O4) was used for rapid enrichment and the individual-peak-based similarity model as the analytical tool. Within 30 min, a mixture of the four most prevalent UTI-causing bacteria, Escherichia coli, Klebsiella pneumoniae, Staphylococcus aureus, and Pseudomonas aeruginosa, was successfully identified using this method. This rapid MALDI-TOF MS-based strategy has potential applications in the clinical identification of UTI pathogens.
A clinical microscopy dataset to develop a deep learning diagnostic test for urinary tract infection

Abstract:Urinary tract infection (UTI) is a common disorder. Its diagnosis can be made by microscopic examination of voided urine for markers of infection. This manual technique is technically difficult, time-consuming and prone to inter-observer errors. The application of computer vision to this domain has been slow due to the lack of a clinical image dataset from UTI patients. We present an open dataset containing 300 images and 3,562 manually annotated urinary cells labelled into seven classes of clinically significant cell types. It is an enriched dataset acquired from the unstained and untreated urine of patients with symptomatic UTI using a simple imaging system. We demonstrate that this dataset can be used to train a Patch U-Net, a novel deep learning architecture with a random patch generator to recognise urinary cells. Our hope is, with this dataset, UTI diagnosis will be made possible in nearly all clinical settings by using a simple imaging system which leverages advanced machine learning techniques.

Abstract:Colorimetry often suffers from deficiency in quantitative determination, susceptibility to ambient illuminance, and low sensitivity and visual resolution to tiny color changes. To offset these deficiencies, we incorporate deep machine learning into colorimetry by introducing a convolutional neural network (CNN) with powerful parallel processing, self-organization, and self-learning capabilities. As a proof of concept, a plasmonic nanosensor is proposed for the colorimetric detection of glucose by coupling Benedict's reagent with gold nanoparticles (AuNPs), which relies on the assemble of AuNPs into dendritic nanochains by Cu 2 O. The distinct difference of refractive index between Cu 2 O and Au and the localized surface plasmon resonance coupling effect among AuNPs leads to a broad spectral shift as well as abundant color changes, thereby providing sufficient data for self-learning enabled by machine learning. The CNN is then used to fully diversify the learning and training of the images from standard samples under different ambient conditions and to obtain a classifier that can not only recognize tiny color changes that are imperceptible to human eyes, but also exhibit high accuracy and excellent anti-environmental interference capability. This classifier is then compiled as an application (APP) and implanted into a smartphone with Android environment. 306 clinical urine samples were detected using the proposed method and the results showed a satisfactory correlation (87.6%) with that of a standard blood glucose test method. More importantly, this method can be generalized to other applications in colorimetry, and more broadly, in other scientific domains that involve image analysis and quantification.

Abstract:Bacterial urinary tract infections cause huge challenges to public health and require rapid diagnostic methods, while lateral flow immunoassay (LFIA) belongs to the most known point-of-care detection technology. Capture-antibody (CA) is used for labeling targets in traditional LFIA, yet the production of antibodies is time-consuming and expensive. In this study, we reported a multimodal CA-independent LFIA (MCI-LFIA) method for rapid diagnosis of bacterial urinary tract infections with high flexibility and accuracy. The assay employs p-mercaptophenylboronic acid-modified Au nanoflower (namely AuNF-PMBA nanomaterials) as multifunctional labels, which possess colorimetric-Raman-photothermal properties and outstanding bacteria capture ability. Take Escherichia coli (E. coli) as model bacteria, the limit of detection (LOD) of this MCI-LFIA for E. coli was 103 cfu/mL by naked-eye, which is three orders of magnitude lower than conventional CA-dependent sandwich method. For quantitative detection, LOD was 103 cfu/mL in colorimetric mode, 102 cfu/mL in Raman mode and 102 cfu/mL in photothermal mode. Significantly, the developed AuNF − PMBA-based MCI-LFIA successfully detected target pathogens in clinical E. coli-positive human urine samples within 45 min with high accuracy. This diagnostic platform may be easily adapted for detection of other target pathogens for bacterial diseases in point-of-care settings with good reproducibility and excellent specificity.

Abstract:Urine examination is a basic program in routine check-ups that can reveal multiple parameters about our health and thus has significant meaning. However, instruments for urine analysis in hospitals are usually bulky and expensive. In this study, we have constructed a pocket-sized and CMOS imaging-based analytical device for urine diagnosis at home or on the spot. The shape of traditional dipsticks was transformed into matrix arrays and pasted on a reaction unit for colorimetric arrays. Chromaticity values were used to establish the functional relationship between the red-green-blue intensities of images and the concentrations of different indices. The size of the entire system was reduced to 30 x 30 x 45 mm, and once charged this device can continuously work for >4 h accommodating more than 1000 tests. To validate the performance of our device, more than 200 real human urine samples were tested and the results generated by our portable device and a commercial instrument show a great consistency ( the consistency for most results is >90%) between the two systems.
