Abstract:Real-time prediction of clinical interventions remains a challenge within intensive care units (ICUs). This task is complicated by data sources that are noisy, sparse, heterogeneous and outcomes that are imbalanced. In this paper, we integrate data from all available ICU sources (vitals, labs, notes, demographics) and focus on learning rich representations of this data to predict onset and weaning of multiple invasive interventions. In particular, we compare both long short-term memory networks (LSTM) and convolutional neural networks (CNN) for prediction of five intervention tasks: invasive ventilation, non-invasive ventilation, vasopressors, colloid boluses, and crystalloid boluses. Our predictions are done in a forward-facing manner to enable "real-time" performance, and predictions are made with a six hour gap time to support clinically actionable planning. We achieve state-of-the-art results on our predictive tasks using deep architectures. We explore the use of feature occlusion to interpret LSTM models, and compare this to the interpretability gained from examining inputs that maximally activate CNN outputs. We show that our models are able to significantly outperform baselines in intervention prediction, and provide insight into model learning, which is crucial for the adoption of such models in practice.

Predicting Intubation for Intensive Care Units Patients: A Deep Learning Approach to Improve Patient Management

Abstract:Objective For patients in the Intensive Care Unit (ICU), the timing of intubation has a significant association with patients' outcomes. However, accurate prediction of the timing of intubation remains an unsolved challenge due to the noisy, sparse, heterogeneous, and unbalanced nature of ICU data. In this study, our objective is to develop a workflow for pre-processing ICU data and to develop a customized deep learning model to predict the need for intubation. Methods To improve the prediction accuracy, we transform the intubation prediction task into a time series classification task. We carefully design a sequence of data pre-processing steps to handle the multimodal noisy data. Firstly, we discretize the sequential data and address missing data using interpolation. Next, we employ a sampling strategy to address data imbalance and standardize the data to facilitate faster model convergence. Furthermore, we employ the feature selection technique and propose an ensemble model to combine features learned by different deep learning models. Results The performance is evaluated on Medical Information Mart for Intensive Care (MIMIC)-III, an ICU dataset. Our proposed Deep Feature Fusion method achieves an area under the curve (AUC) of the receiver operating curve (ROC) of 0.8953, surpassing the performance of other deep learning and traditional machine learning models. Conclusion Our proposed Deep Feature Fusion method proves to be a viable approach for predicting intubation and outperforms other deep learning and classical machine learning models. The study confirms that high-frequency time-varying indicators, particularly Mean Blood Pressure (MeanBP) and peripheral oxygen saturation (SpO2), are significant risk factors for predicting intubation.

Abstract:The COVID-19 pandemic continues to have a devastating global impact, and has placed a tremendous burden on struggling healthcare systems around the world. Given the limited resources, accurate patient triaging and care planning is critical in the fight against COVID-19, and one crucial task within care planning is determining if a patient should be admitted to a hospital's intensive care unit (ICU). Motivated by the need for transparent and trustworthy ICU admission clinical decision support, we introduce COVID-Net Clinical ICU, a neural network for ICU admission prediction based on patient clinical data. Driven by a transparent, trust-centric methodology, the proposed COVID-Net Clinical ICU was built using a clinical dataset from Hospital Sirio-Libanes comprising of 1,925 COVID-19 patient records, and is able to predict when a COVID-19 positive patient would require ICU admission with an accuracy of 96.9% to facilitate better care planning for hospitals amidst the on-going pandemic. We conducted system-level insight discovery using a quantitative explainability strategy to study the decision-making impact of different clinical features and gain actionable insights for enhancing predictive performance. We further leveraged a suite of trust quantification metrics to gain deeper insights into the trustworthiness of COVID-Net Clinical ICU. By digging deeper into when and why clinical predictive models makes certain decisions, we can uncover key factors in decision making for critical clinical decision support tasks such as ICU admission prediction and identify the situations under which clinical predictive models can be trusted for greater accountability.

Abstract:Background : Acute heart failure (AHF) in the intensive care unit (ICU) is characterized by its criticality, rapid progression, complex and changeable condition, and its pathophysiological process involves the interaction of multiple organs and systems. This makes it difficult to predict in-hospital mortality events comprehensively and accurately. Traditional analysis methods based on statistics and machine learning suffer from insufficient model performance, poor accuracy caused by prior dependence, and difficulty in adequately considering the complex relationships between multiple risk factors. Therefore, the application of deep neural network (DNN) techniques to the specific scenario, predicting mortality events of patients with AHF under intensive care, has become a research frontier. Methods : This research utilized the MIMIC-IV critical care database as the primary data source and employed the synthetic minority over-sampling technique (SMOTE) to balance the dataset. Deep neural network models—backpropagation neural network (BPNN) and recurrent neural network (RNN), which are based on electronic medical record data mining, were employed to investigate the in-hospital death event judgment task of patients with AHF under intensive care. Additionally, multiple single machine learning models and ensemble learning models were constructed for comparative experiments. Moreover, we achieved various optimal performance combinations by modifying the classification threshold of deep neural network models to address the diverse real-world requirements in the ICU. Finally, we conducted an interpretable deep model using SHapley Additive exPlanations (SHAP) to uncover the most influential medical record features for each patient from the aspects of global and local interpretation. Results : In terms of model performance in this scenario, deep neural network models outperform both single machine learning models and ensemble learning models, achieving the highest Accuracy, Precision, Recall, F1 value, and Area under the ROC curve, which can reach 0.949, 0.925, 0.983, 0.953, and 0.987 respectively. SHAP value analysis revealed that the ICU scores (APSIII, OASIS, SOFA) are significantly correlated with the occurrence of in-hospital fatal events. Conclusions : Our study underscores that DNN-based mortality event classifier offers a novel intelligent approach for forecasting and assessing the prognosis of AHF patients in the ICU. Additionally, the ICU scores stand out as the most predictive features, which implies that in the decision-making process of the models, ICU scores can provide the most crucial information, making the greatest positive or negative contribution to influence the incidence of in-hospital mortality among patients with acute heart failure.

Abstract:Background In-hospital mortality, prolonged length of stay (LOS), and 30-day readmission are common outcomes in the intensive care unit (ICU). Traditional scoring systems and machine learning models for predicting these outcomes usually ignore the characteristics of ICU data, which are time-series forms. We aimed to use time-series deep learning models with the selective combination of three widely used scoring systems to predict these outcomes. Materials and methods A retrospective cohort study was conducted on 40,083 patients in ICU from the Medical Information Mart for Intensive Care-IV (MIMIC-IV) database. Three deep learning models, namely, recurrent neural network (RNN), gated recurrent unit (GRU), and long short-term memory (LSTM) with attention mechanisms, were trained for the prediction of in-hospital mortality, prolonged LOS, and 30-day readmission with variables collected during the initial 24 h after ICU admission or the last 24 h before discharge. The inclusion of variables was based on three widely used scoring systems, namely, APACHE II, SOFA, and SAPS II, and the predictors consisted of time-series vital signs, laboratory tests, medication, and procedures. The patients were randomly divided into a training set (80%) and a test set (20%), which were used for model development and model evaluation, respectively. The area under the receiver operating characteristic curve (AUC), sensitivity, specificity, and Brier scores were used to evaluate model performance. Variable significance was identified through attention mechanisms. Results A total of 33 variables for 40,083 patients were enrolled for mortality and prolonged LOS prediction and 36,180 for readmission prediction. The rates of occurrence of the three outcomes were 9.74%, 27.54%, and 11.79%, respectively. In each of the three outcomes, the performance of RNN, GRU, and LSTM did not differ greatly. Mortality prediction models, prolonged LOS prediction models, and readmission prediction models achieved AUCs of 0.870 ± 0.001, 0.765 ± 0.003, and 0.635 ± 0.018, respectively. The top significant variables co-selected by the three deep learning models were Glasgow Coma Scale (GCS), age, blood urea nitrogen, and norepinephrine for mortality; GCS, invasive ventilation, and blood urea nitrogen for prolonged LOS; and blood urea nitrogen, GCS, and ethnicity for readmission. Conclusion The prognostic prediction models established in our study achieved good performance in predicting common outcomes of patients in ICU, especially in mortality prediction. In addition, GCS and blood urea nitrogen were identified as the most important factors strongly associated with adverse ICU events.

Abstract:Background: Cardiac arrest (CA) is one of the leading causes of death among patients in the intensive care unit (ICU). Although many CA prediction models with high sensitivity have been developed to anticipate CA, their practical application has been challenging due to a lack of generalization and validation. Additionally, the heterogeneity among patients in different ICU subtypes has not been adequately addressed. Objective: This study aims to propose a clinically interpretable ensemble approach for the timely and accurate prediction of CA within 24 hours, regardless of patient heterogeneity, including variations across different populations and ICU subtypes. Additionally, we conducted patient-independent evaluations to emphasize the model's generalization performance and analyzed interpretable results that can be readily adopted by clinicians in real-time. Methods: Patients were retrospectively analyzed using data from the Medical Information Mart for Intensive Care-IV (MIMIC-IV) and the eICU-Collaborative Research Database (eICU-CRD). To address the problem of underperformance, we constructed our framework using feature sets based on vital signs, multiresolution statistical analysis, and the Gini index, with a 12-hour window to capture the unique characteristics of CA. We extracted 3 types of features from each database to compare the performance of CA prediction between high-risk patient groups from MIMIC-IV and patients without CA from eICU-CRD. After feature extraction, we developed a tabular network (TabNet) model using feature screening with cost-sensitive learning. To assess real-time CA prediction performance, we used 10-fold leave-one-patient-out cross-validation and a cross–data set method. We evaluated MIMIC-IV and eICU-CRD across different cohort populations and subtypes of ICU within each database. Finally, external validation using the eICU-CRD and MIMIC-IV databases was conducted to assess the model's generalization ability. The decision mask of the proposed method was used to capture the interpretability of the model. Results: The proposed method outperformed conventional approaches across different cohort populations in both MIMIC-IV and eICU-CRD. Additionally, it achieved higher accuracy than baseline models for various ICU subtypes within both databases. The interpretable prediction results can enhance clinicians' understanding of CA prediction by serving as a statistical comparison between non-CA and CA groups. Next, we tested the eICU-CRD and MIMIC-IV data sets using models trained on MIMIC-IV and eICU-CRD, respectively, to evaluate generalization ability. The results demonstrated superior performance compared with baseline models. Conclusions: Our novel framework for learning unique features provides stable predictive power across different ICU environments. Most of the interpretable global information reveals statistical differences between CA and non-CA groups, demonstrating its utility as an indicator for clinical decisions. Consequently, the proposed CA prediction system is a clinically validated algorithm that enables clinicians to intervene early based on CA prediction information and can be applied to clinical trials in digital health.

Abstract:Given the limited capacity to accurately determine the necessity for intubation in intensive care unit settings, this study aimed to develop and externally validate an interpretable machine learning model capable of predicting the need for intubation among ICU patients. Seven widely used machine learning (ML) algorithms were employed to construct the prediction models. Adult patients from the Medical Information Mart for Intensive Care IV database who stayed in the ICU for longer than 24 h were included in the development and internal validation. The model was subsequently externally validated using the eICU-CRD database. In addition, the SHapley Additive exPlanations method was employed to interpret the influence of individual parameters on the predictions made by the model. A total of 11,988 patients were included in the final cohort for this study. The CatBoost model demonstrated the best performance (AUC: 0.881). In the external validation set, the efficacy of our model was also confirmed (AUC: 0.750), which suggests robust generalization capabilities. The Glasgow Coma Scale (GCS), body mass index (BMI), arterial partial pressure of oxygen (PaO 2 ), respiratory rate (RR) and length of stay (LOS) before ICU were the top 5 features of the CatBoost model with the greatest impact. We developed an externally validated CatBoost model that accurately predicts the need for intubation in ICU patients within 24 to 96 h of admission, facilitating clinical decision-making and has the potential to improve patient outcomes. The prediction model utilizes readily obtainable monitoring parameters and integrates the SHAP method to enhance interpretability, providing clinicians with clear insights into the factors influencing predictions.

Predicting Extubation Failure in Intensive Care: The Development of a Novel, End-to-End Actionable and Interpretable Prediction System

Abstract:Predicting extubation failure in intensive care is challenging due to complex data and the severe consequences of inaccurate predictions. Machine learning shows promise in improving clinical decision-making but often fails to account for temporal patient trajectories and model interpretability, highlighting the need for innovative solutions. This study aimed to develop an actionable, interpretable prediction system for extubation failure using temporal modelling approaches such as Long Short-Term Memory (LSTM) and Temporal Convolutional Networks (TCN). A retrospective cohort study of 4,701 mechanically ventilated patients from the MIMIC-IV database was conducted. Data from the 6 hours before extubation, including static and dynamic features, were processed through novel techniques addressing data inconsistency and synthetic data challenges. Feature selection was guided by clinical relevance and literature benchmarks. Iterative experimentation involved training LSTM, TCN, and LightGBM models. Initial results showed a strong bias toward predicting extubation success, despite advanced hyperparameter tuning and static data inclusion. Data was stratified by sampling frequency to reduce synthetic data impacts, leading to a fused decision system with improved performance. However, all architectures yielded modest predictive power (AUC-ROC ~0.6; F1 <0.5) with no clear advantage in incorporating static data or additional features. Ablation analysis indicated minimal impact of individual features on model performance. This thesis highlights the challenges of synthetic data in extubation failure prediction and introduces strategies to mitigate bias, including clinician-informed preprocessing and novel feature subsetting. While performance was limited, the study provides a foundation for future work, emphasising the need for reliable, interpretable models to optimise ICU outcomes.

APRICOT-Mamba: Acuity Prediction in Intensive Care Unit (ICU): Development and Validation of a Stability, Transitions, and Life-Sustaining Therapies Prediction Model

Abstract:The acuity state of patients in the intensive care unit (ICU) can quickly change from stable to unstable. Early detection of deteriorating conditions can result in providing timely interventions and improved survival rates. In this study, we propose APRICOT-M (Acuity Prediction in Intensive Care Unit-Mamba), a 150k-parameter state space-based neural network to predict acuity state, transitions, and the need for life-sustaining therapies in real-time in ICU patients. The model uses data obtained in the prior four hours in the ICU and patient information obtained at admission to predict the acuity outcomes in the next four hours. We validated APRICOT-M externally on data from hospitals not used in development (75,668 patients from 147 hospitals), temporally on data from a period not used in development (12,927 patients from one hospital from 2018-2019), and prospectively on data collected in real-time (215 patients from one hospital from 2021-2023) using three large datasets: the University of Florida Health (UFH) dataset, the electronic ICU Collaborative Research Database (eICU), and the Medical Information Mart for Intensive Care (MIMIC)-IV. The area under the receiver operating characteristic curve (AUROC) of APRICOT-M for mortality (external 0.94-0.95, temporal 0.97-0.98, prospective 0.96-1.00) and acuity (external 0.95-0.95, temporal 0.97-0.97, prospective 0.96-0.96) shows comparable results to state-of-the-art models. Furthermore, APRICOT-M can predict transitions to instability (external 0.81-0.82, temporal 0.77-0.78, prospective 0.68-0.75) and need for life-sustaining therapies, including mechanical ventilation (external 0.82-0.83, temporal 0.87-0.88, prospective 0.67-0.76), and vasopressors (external 0.81-0.82, temporal 0.73-0.75, prospective 0.66-0.74). This tool allows for real-time acuity monitoring in critically ill patients and can help clinicians make timely interventions.

A Hybrid Decision Tree and Deep Learning Approach Combining Medical Imaging and Electronic Medical Records to Predict Intubation Among Hospitalized Patients With COVID-19: Algorithm Development and Validation

Abstract:Background Early prediction of the need for invasive mechanical ventilation (IMV) in patients hospitalized with COVID-19 symptoms can help in the allocation of resources appropriately and improve patient outcomes by appropriately monitoring and treating patients at the greatest risk of respiratory failure. To help with the complexity of deciding whether a patient needs IMV, machine learning algorithms may help bring more prognostic value in a timely and systematic manner. Chest radiographs (CXRs) and electronic medical records (EMRs), typically obtained early in patients admitted with COVID-19, are the keys to deciding whether they need IMV. Objective We aimed to evaluate the use of a machine learning model to predict the need for intubation within 24 hours by using a combination of CXR and EMR data in an end-to-end automated pipeline. We included historical data from 2481 hospitalizations at The Mount Sinai Hospital in New York City. Methods CXRs were first resized, rescaled, and normalized. Then lungs were segmented from the CXRs by using a U-Net algorithm. After splitting them into a training and a test set, the training set images were augmented. The augmented images were used to train an image classifier to predict the probability of intubation with a prediction window of 24 hours by retraining a pretrained DenseNet model by using transfer learning, 10-fold cross-validation, and grid search. Then, in the final fusion model, we trained a random forest algorithm via 10-fold cross-validation by combining the probability score from the image classifier with 41 longitudinal variables in the EMR. Variables in the EMR included clinical and laboratory data routinely collected in the inpatient setting. The final fusion model gave a prediction likelihood for the need of intubation within 24 hours as well. Results At a prediction probability threshold of 0.5, the fusion model provided 78.9% (95% CI 59%-96%) sensitivity, 83% (95% CI 76%-89%) specificity, 0.509 (95% CI 0.34-0.67) F1-score, 0.874 (95% CI 0.80-0.94) area under the receiver operating characteristic curve (AUROC), and 0.497 (95% CI 0.32-0.65) area under the precision recall curve (AUPRC) on the holdout set. Compared to the image classifier alone, which had an AUROC of 0.577 (95% CI 0.44-0.73) and an AUPRC of 0.206 (95% CI 0.08-0.38), the fusion model showed significant improvement (P<.001). The most important predictor variables were respiratory rate, C-reactive protein, oxygen saturation, and lactate dehydrogenase. The imaging probability score ranked 15th in overall feature importance. Conclusions We show that, when linked with EMR data, an automated deep learning image classifier improved performance in identifying hospitalized patients with severe COVID-19 at risk for intubation. With additional prospective and external validation, such a model may assist risk assessment and optimize clinical decision-making in choosing the best care plan during the critical stages of COVID-19.

Abstract:Background For the intensivists, accurate assessment of the ideal timing for successful weaning from the mechanical ventilation (MV) in the intensive care unit (ICU) is very challenging. Purpose Using artificial intelligence (AI) approach to build two-stage predictive models, namely, the try-weaning stage and weaning MV stage to determine the optimal timing of weaning from MV for ICU intubated patients, and implement into practice for assisting clinical decision making. Methods AI and machine learning (ML) technologies were used to establish the predictive models in the stages. Each stage comprised 11 prediction time points with 11 prediction models. Twenty-five features were used for the first-stage models while 20 features were used for the second-stage models. The optimal models for each time point were selected for further practical implementation in a digital dashboard style. Seven machine learning algorithms including Logistic Regression (LR), Random Forest (RF), Support Vector Machines (SVM), K Nearest Neighbor (KNN), lightGBM, XGBoost, and Multilayer Perception (MLP) were used. The electronic medical records of the intubated ICU patients of Chi Mei Medical Center (CMMC) from 2016 to 2019 were included for modeling. Models with the highest area under the receiver operating characteristic curve (AUC) were regarded as optimal models and used to develop the prediction system accordingly. Results A total of 5,873 cases were included in machine learning modeling for Stage 1 with the AUCs of optimal models ranging from 0.843 to 0.953. Further, 4,172 cases were included for Stage 2 with the AUCs of optimal models ranging from 0.889 to 0.944. A prediction system (dashboard) with the optimal models of the two stages was developed and deployed in the ICU setting. Respiratory care members expressed high recognition of the AI dashboard assisting ventilator weaning decisions. Also, the impact analysis of with- and without-AI assistance revealed that our AI models could shorten the patients’ intubation time by 21 hours, besides gaining the benefit of substantial consistency between these two decision-making strategies. Conclusion We noticed that the two-stage AI prediction models could effectively and precisely predict the optimal timing to wean intubated patients in the ICU from ventilator use. This could reduce patient discomfort, improve medical quality, and lower medical costs. This AI-assisted prediction system is beneficial for clinicians to cope with a high demand for ventilators during the COVID-19 pandemic.

Abstract:The use of invasive mechanical ventilation (IMV) is crucial in rescuing patients with respiratory dysfunction. Accurately predicting the demand for IMV is vital for clinical decision-making. However, current techniques are invasive and challenging to implement in pre-hospital and emergency rescue settings. To address this issue, a real-time prediction method utilizing only non-invasive parameters was developed to forecast IMV demand in this study. The model introduced the concept of real-time warning and leveraged the advantages of machine learning and integrated methods, achieving an AUC value of 0.935 (95% CI 0.933–0.937). The AUC value for the multi-center validation using the AmsterdamUMCdb database was 0.727, surpassing the performance of traditional risk adjustment algorithms (OSI(oxygenation saturation index): 0.608, P/F(oxygenation index): 0.558). Feature weight analysis demonstrated that BMI, Gcsverbal, and age significantly contributed to the model's decision-making. These findings highlight the substantial potential of a machine learning real-time dynamic warning model that solely relies on non-invasive parameters to predict IMV demand. Such a model can provide technical support for predicting the need for IMV in pre-hospital and disaster scenarios.

Abstract:Accurate mortality prediction allows Intensive Care Units (ICUs) to adequately benchmark clinical practice and identify patients with unexpected outcomes. Traditionally, simple statistical models have been used to assess patient death risk, many times with sub-optimal performance. On the other hand deep learning holds promise to positively impact clinical practice by leveraging medical data to assist diagnosis and prediction, including mortality prediction. However, as the question of whether powerful Deep Learning models attend correlations backed by sound medical knowledge when generating predictions remains open, additional interpretability tools are needed to foster trust and encourage the use of AI by clinicians. In this work we show a Deep Learning model trained on MIMIC-III to predict mortality using raw nursing notes, together with visual explanations for word importance. Our model reaches a ROC of 0.8629 (+/-0.0058), outperforming the traditional SAPS-II score and providing enhanced interpretability when compared with similar Deep Learning approaches.

Abstract:Predicting clinical patients' vital signs is a leading critical issue in intensive care units (ICUs) related studies. Early prediction of the mortality of ICU patients can reduce the overall mortality and cost of complication treatment. Some studies have predicted mortality based on electronic health record (EHR) data by using machine learning models. However, the semi-structured data (i.e., patients' diagnosis data and inspection reports) is rarely used in these models. This study utilized data from the Medical Information Mart for Intensive Care III. We used a Latent Dirichlet Allocation (LDA) model to classify text in the semi-structured data of some particular topics and established and compared the classification and regression trees (CART), logistic regression (LR), multivariate adaptive regression splines (MARS), random forest (RF), and gradient boosting (GB). A total of 46,520 ICU Patients were included, with 11.5% mortality in the Medical Information Mart for Intensive Care III group. Our results revealed that the semi-structured data (diagnosis data and inspection reports) of ICU patients contain useful information that can assist clinical doctors in making critical clinical decisions. In addition, in our comparison of five machine learning models (CART, LR, MARS, RF, and GB), the GB model showed the best performance with the highest area under the receiver operating characteristic curve (AUROC) (0.9280), specificity (93.16%), and sensitivity (83.25%). The RF, LR, and MARS models showed better performance (AUROC are 0.9096, 0.8987, and 0.8935, respectively) than the CART (0.8511). The GB model showed better performance than other machine learning models (CART, LR, MARS, and RF) in predicting the mortality of patients in the intensive care unit. The analysis results could be used to develop a clinically useful decision support system.

Abstract:Intubation for mechanical ventilation (MV) is one of the most common high-risk procedures performed in Intensive Care Units (ICUs). Early prediction of intubation may have a positive impact by providing timely alerts to clinicians and consequently avoiding high-risk late intubations. In this work, we propose a new machine learning method to predict the time to intubation during the first five days of ICU admission, based on the concept of cure survival models. Our approach combines classification and survival analysis, to effectively accommodate the fraction of patients not at risk of intubation, and provide a better estimate of time to intubation, for patients at risk. We tested our approach and compared it to other predictive models on a dataset collected from a secondary care hospital (AZ Groeninge, Kortrijk, Belgium) from 2015 to 2021, consisting of 3425 ICU stays. Furthermore, we utilized SHAP for feature importance analysis, extracting key insights into the relative significance of variables such as vital signs, blood gases, and patient characteristics in predicting intubation in ICU settings. The results corroborate that our approach improves the prediction of time to intubation in critically ill patients, by using routinely collected data within the first hours of admission in the ICU. Early warning of the need for intubation may be used to help clinicians predict the risk of intubation and rank patients according to their expected time to intubation.

Abstract:Background: Timely and accurate outcome prediction plays a critical role in guiding clinical decisions for hypertensive ischemic or hemorrhagic stroke patients admitted to the ICU. However, interpreting and translating the predictive models into clinical applications are as important as the prediction itself. This study aimed to develop an interpretable machine learning (IML) model that accurately predicts 28-day all-cause mortality in hypertensive ischemic or hemorrhagic stroke patients. Methods: A total of 4,274 hypertensive ischemic or hemorrhagic stroke patients admitted to the ICU in the USA from multicenter cohorts were included in this study to develop and validate the IML model. Five machine learning (ML) models were developed, including artificial neural network (ANN), gradient boosting machine (GBM), eXtreme Gradient Boosting (XGBoost), logistic regression (LR), and support vector machine (SVM), to predict mortality using the MIMIC-IV and eICU-CRD database in the USA. Feature selection was performed using the Least Absolute Shrinkage and Selection Operator (LASSO) algorithm. Model performance was evaluated based on the area under the curve (AUC), accuracy, positive predictive value (PPV), and negative predictive value (NPV). The ML model with the best predictive performance was selected for interpretability analysis. Finally, the SHapley Additive exPlanations (SHAP) method was employed to evaluate the risk of all-cause in-hospital mortality among hypertensive ischemic or hemorrhagic stroke patients admitted to the ICU. Results: The XGBoost model demonstrated the best predictive performance, with the AUC values of 0.822, 0.739, and 0.700 in the training, test, and external cohorts, respectively. The analysis of feature importance revealed that age, ethnicity, white blood cell (WBC), hyperlipidemia, mean corpuscular volume (MCV), glucose, pulse oximeter oxygen saturation (SpO 2 ), serum calcium, red blood cell distribution width (RDW), blood urea nitrogen (BUN), and bicarbonate were the 11 most important features. The SHAP plots were employed to interpret the XGBoost model. Conclusions: The XGBoost model accurately predicted 28-day all-cause in-hospital mortality among hypertensive ischemic or hemorrhagic stroke patients admitted to the ICU. The SHAP method can provide explicit explanations of personalized risk prediction, which can aid physicians in understanding the model.

Abstract:Interpretability is fundamental in healthcare problems and the lack of it in deep learning models is currently the major barrier in the usage of such powerful algorithms in the field. The study describes the implementation of an attention layer for Long Short-Term Memory (LSTM) neural network that provides a useful picture on the influence of the several input variables included in the model.A cohort of 10,616 patients with cardiovascular diseases is selected from the MIMIC III dataset, an openly available database of electronic health records (EHRs) including all patients admitted to an ICU at Boston's Medical Centre. For each patient, we consider a 10-length sequence of 1-hour windows in which 48 clinical parameters are extracted to predict the occurrence of death in the next 7 days. Inspired from the recent developments in the field of attention mechanisms for sequential data, we implement a recurrent neural network with LSTM cells incorporating an attention mechanism to identify features driving model's decisions over time.The performance of the LSTM model, measured in terms of AUC, is 0.790 (SD=0.015). Regard our primary objective, i.e. model interpretability, we investigate the role of attention weights. We find good correspondence with driving predictors of a transparent model (r=0.611, 95% CI [0.395, 0.763]). Moreover, most influential features identified at the cohort-level emerge as known risk factors in the clinical context.Despite the limitations of study dataset, this work brings further evidence of the potential of attention mechanisms in making deep learning model more interpretable and suggests the application of this strategy for the sequential analysis of EHRs.
