Abstract:Introduction Hyper Immunoglobulin E Syndromes (HIES) are primary immunodeficiency disorders characterized by recurrent skin and lung infections, atopic dermatitis, and elevated IgE levels. Mutations in the STAT3 gene are responsible for the autosomal dominant hyper-IgE syndrome or STAT3-HIES. Achieving adequate management of atopic dermatitis in patients with HIES can be difficult. Dupilumab, an IL-4 and IL-13 inhibitor, is used in treatment of severe atopic dermatitis. There are few reports of its use in HIES in adults and limited reporting of its use in pediatric patients. Case Description A teenage boy with a history of STAT3-HIES presented for management of symptoms including eczema flares with superimposed infections. He had recurrent flares of eczema causing significant distress and poor quality of life. The painful and excoriated eczema brought him discomfort and often caused him to miss school. Despite high potency topical steroids and emollients along with oral antihistamines, his skin conditions were not very well controlled until Dupilumab was trialed. Discussion After initiation of Dupilumab, the patient's skin was almost completely clear within three doses. IgE levels decreased from 47293 IU/mL to 7528 IU/mL with 16 months of treatment and continued to decrease with continued therapy. The patient's skin has also continued to remain clear and manageable. This case demonstrates effective use of Dupilumab in a pediatric patient with STAT3-HIES to control severe atopic dermatitis symptoms. Through its mitigation of TH2 response, Dupilumab leads to decreased serum IgE levels and improvement of eczema/atopic dermatitis related to STAT3-HIES.

Abstract:Hyperimmunoglobulin E syndrome (HIES) is a group of rare genetic disorders characterized by severe atopic dermatitis and recurrent skin and pulmonary infections. The efficacy of dupilumab in pediatric patients with HIES‐associated severe atopic dermatitis is relatively understudied. Here, we present a series of three children with HIES, two with AD‐HIES caused by STAT3 mutations, and one with AR‐HIES caused by biallelic mutations in ZNF341. In all cases, dupilumab treatment led to sustained clearance of severe atopic dermatitis over multiple years, as well as improvements in systemic symptoms of HIES.

Abstract:Hypereosinophilic dermatitis (HED) is a subtype of hypereosinophilic syndrome (HES). Glucocorticoids are preferred for treatment but carry substantial side effect profiles. Symptoms of HED may recur after systemic glucocorticoid tapering. As an interleukin-4 receptor (IL-4Rα) monoclonal antibody targeting interleukin-4 (IL-4) and interleukin-13 (IL-13), dupilumab might be an efficacious adjuvant therapy for HED. We report a young male diagnosed with HED who suffered from erythematous papules with pruritus for over five years. Once reducing the dosage of glucocorticoid was, his skin lesions relapsed. After using dupilumab, the patient’s condition significantly improved with the glucocorticoid dosing decreased successfully. In conclusion, we report a new application of dupilumab in HED patients, especially with difficulties in reducing the glucocorticoid dose.
Hypereosinophilic Dermatitis: Successful Treatment with Dupilumab.

Abstract:Hypereosinophilic dermatitis (HED) is a subtype of hypereosinophilic syndrome. HED is characterized by eosinophilic granulocytes increased in peripheral blood and bone marrow and infiltrated in skin. The clinical manifestations of HED are diffussed by erythema, papule and maculopapule with severe itching. The etiology of HED is unknown. At present, in addition to HED with FIP1L1-PDGFRA fusion gene positive, whose treatment is tyrosine kinase inhibitor, other types of HED first-line treatment are oral glucocorticoids, supplemented by antihistamines and immunosuppressants. Dupilumab is a human monoclonal antibody, which inhibits the IL-4 and IL-13 signaling by binding to the IL-4R-α and IL-13R-α-1 subunits of the receptor. We report a 76-year-old male patient with HED whose peripheral blood eosinophils decreased from 20.7% to 4.1% after 8 weeks of dupilumab, and his pruritus was completely relieved. Dupilumab was discontinued after 6 months of treatment. It is exciting that the patient has not experienced relapse for 17 months after the discontinuation. No adverse event was reported.
Quick Treatment of Very Severe Refractory Hand and Foot Eczema with Dupilumab: A Case Report and Literature Review

Abstract:Previously case reports showed dupilumab may benefit for hand eczema treatment, but relatively comprehensive assessments are lacking. A 45-year-old male with multiple severe vesicles, bullae and pustule on the palmar aspects of both hand and foot diagnosed dyshidrotic eczema by pathology was treated with dupilumab at an initial dose of 600 mg subcutaneously, followed by 300 mg every 2 weeks. The physician's assessment of the patient revealed an excellent response to the treatment with dupilumab; the lesions and symptoms achieved dramatic improvement on the third day, and at 6 weeks, the hands and feet became completely normal without relapse in the past 1.5 years of discontinuation. Systematic literature searches were performed, and 6 case reports, 5 case series, 2 prospective observational studies and 1 retrospective review with a total of 150 patients were identified to describing the evaluation of efficacy and safety of dupilumab treatment for hand and foot eczema. Dupilumab appears to be safe and well tolerated with clinical benefit in recalcitrant hand and foot eczema. Larger randomized controlled trials using validated outcome measures and detailed hand eczema type and population classification are needed before dupilumab can be applied in clinical settings.

Abstract:Hypereosinophilic syndrome (HES) is characterized by blood and tissue hypereosinophilia leading to organ damage. Gastrointestinal involvement is the third most common manifestation. We present a patient with idiopathic HES with secondary eosinophilic esophagitis (EoE), gastritis, and enteritis, corticosteroids-dependent, azathioprine- and mepolizumab-refractory. The patient achieved clinical and histopathologic remission following dupilumab treatment. A 28 year-old female presented with chronic episodic nausea and emesis since childhood and initial diagnosis of primary eosinophilic gastrointestinal disease (EGID), improved with corticosteroids, refractory to azathioprine. She was found to have peripheral eosinophilia and multifactorial anemia, with iron, B12, and folate deficiencies. Esophageal, gastric, duodenal, and terminal ileum biopsies showed significant eosinophilic infiltrate. Bone marrow biopsy at age 31 confirmed HES diagnosis. By age 32, she became total parental nutrition (TPN)-dependent. She failed trials of benralizumab and mepolizumab [anti-interleukin (IL)-5 inhibitors], and cromolyn (mast-cell stabilizer). After developing new esophageal stricture, we initiated dupilumab (IL-4/13 inhibitor), recently FDA-approved for EoE. After 9 weeks, esophageal stricture, gut tissue eosinophilia, and prior intestinal ulcerations resolved. She ceased TPN and is tolerating a non-restricted diet, with complete symptom resolution. Our patient's complete remission with dupilumab shows promise for broadening its use in treating GI involvement in HES, along with primary EGIDs.

Abstract:Background: Hypereosinophilic syndrome (HES) is a heterogeneous group of disorders characterized by persistent eosinophilia and associated organ damage. Comorbid asthma in HES patients poses a therapeutic challenge, especially in cases resistant to standard eosinophil-lowering therapies. This case report describes the effective management of a 31-year-old patient with HES and asthma using dupilumab after unsuccessful treatments with mepolizumab and benralizumab. Case Presentation: A 31-year-old individual with a lifelong history of severe asthma and HES, presented with uncontrolled symptoms and 4 yearly exacerbations despite previous treatment with high-dose inhaled corticosteroids and two different biologics over the last 5 years: initially with mepolizumab (300-700mg q4w), which was withdrawn after 3 years due to diminishing efficacy and benralizumab (30mg q4w), which was ceased after 6 months due to lack of response. The patient exhibited poor asthma control and recurrent exacerbations, despite low blood eosinophil levels. Following a comprehensive evaluation, treatment with dupilumab was initiated. Results: Remarkable improvement was observed within first 2 weeks of starting dupilumab. At a 6-month checkpoint we observed a significant improvement in both ACQ (from 3.5 to 0.9) and AQLQ (2.3 to 5.9) scores. The patient did not experience any exacerbations and managed to reduce daily dose of oral corticosteroids from 25mg to 10mg of prednisone. No significant adverse effects or increase in eosinophil counts were noted during the treatment course.

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Background</h3><p>Atopic dermatitis (AD) is an allergic disease that affects individuals of various ages. Recently, the IL‐4/13 inhibitor dupilumab has gained regulatory approval for clinical use in AD patients. Dupilumab has been reported to reduce several markers of AD, including the serum levels of thymus and activation‐regulated chemokine (TARC/CCL17), blood lactate dehydrogenase (LDH), and serum total immunoglobulin E (IgE).</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Methods</h3><p>We retrospectively reviewed data from 40 AD patients who were treated with dupilumab. Eczema Area and Severity Index (EASI), Investigator's Global Assessment (IGA), body surface area (BSA) scores, TARC, LDH, total IgE, and eosinophil count in peripheral blood were assessed for a total of 32 weeks.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>The EASI, IGA, and BSA scores improved significantly with treatment, indicating a reduction in AD severity. Serum TARC and LDH levels also significantly decreased with treatment. Serum IgE levels were unchanged at 2 weeks of treatment but decreased significantly between 4 and 32 weeks. The number of eosinophils in the peripheral blood decreased at 4, 16, and 32 weeks after treatment initiation.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Conclusions</h3><p>Several studies have reported that serum TARC, LDH, and total IgE levels are reduced by dupilumab treatment. Our real‐world data are the first to demonstrate a reduction in blood eosinophilia in patients who receive clinical treatment with dupilumab.</p></section>

Abstract:Background Hyper-immunoglobulin E (IgE) syndromes (HIES) are a group of primary immune deficiencies disorders (PID) characterized by elevated serum IgE, eczema, recurrent skin, or respiratory system infections and may also be accompanied by some connective tissues and skeletal abnormalities. Currently, there is no complete cure or targeted treatment for HIES. Omalizumab is a humanized recombinant monoclonal antibody against IgE, reducing the level of free IgE, inhibiting the binding of IgE to receptors on the surface of effector cells, and reducing the activation of inflammatory cells and the release of multiple inflammatory mediators. However, the effect of omalizumab in treating HIES remains unknown. Herein, we described a case of an AD-HIES patient with chronic airway disease who benefited from omalizumab treatment. Case Presentation A 28-year-old Chinese woman was admitted for recurrent cough for 7 years, markedly elevated serum IgE level, and recurrent pneumonia caused by multiple pathogens, such as Pneumocystis jirovecii, Cytomegalovirus, Staphylococcus aureus, Aspergillus , and Mycobacterium tuberculosis . She had eczema-dermatitis, skin abscess, slightly traumatic fracture since childhood, and developed asthma and allergic bronchopulmonary aspergillosis (ABPA) lately. Using whole-exome sequencing, the STAT3 (c.1294G>T, p.Val432Leu) missense mutation for the autosomal dominant hyper-IgE syndrome was identified, and omalizumab was prescribed at 300 mg every 2 weeks. The patient responded well with the improvement of respiratory symptoms and lung function tests. The level of serum IgE remained stable on follow-up. Conclusion Omalizumab treatment proved beneficial in the case of HIES, especially with chronic airway disease, for which therapeutic options are limited. However, larger-scale prospective studies and long-term follow-up are required to establish the efficacy and safety of this therapeutic intervention.

Abstract:Background Atopic dermatitis (AD) is a chronic recurrent inflammatory disease, and dupilumab, a human monoclonal antibody, is the firstly approved biological drug for AD. Psoriasiform erythema (PE) during dupilumab treatment in adults has been reported. This study describes the risk of PE in children after initiation of dupilumab treatment. Objectives To evaluate the de novo cytokines gene expression in the transition of atopic dermatitis symptoms to psoriasiform erythema during dupilumab treatment in children. Methods Two 17-year-old teenage twin patients with AD were included in this study who developed psoriasiform erythema after initiation of dupilumab. The lesional skin biopsy specimens were obtained for the histopathological investigation and RNA Fluorescence In Situ Hybridization (RNA-FISH). Dermoscopy, cytometry (cytokine detection in the blood), and blood investigations were completed for the pedigree and the lesioned descriptions. Results Two twin patients with AD presented with erythematic scaly plaques on the back, scalp, abdomen, and extensor extremities after 20 weeks of dupilumab treatment. The transitional change of AD to psoriasiform erythema treated with dupilumab was observed. Our subjects' dermoscopy showed pinpoint bleeding and white scales on pink background. Histopathology features showed psoriasiform hyperplasia, epidermal hyperplasia (acanthosis), ectatic capillaries, perivascular lymphocytes infiltration, and parakeratosis, with the absence of the granular cell layer. mRNA (RNA-FISH) cytokines gene expression showed a significantly high concentration of IL-17A. Blood investigation results showed a high concentration of (Immunoglobulin E) IgE and Eosinophils, and cytokines detection in blood showed IL-5,6 and IL-17 in one patient; however, only IL-5 in another patient. The dupilumab was discontinued and initiated with Baricitinib. Baricitinib showed a significant reduction in skin lesions. Conclusion Psoriasiform erythema can appear during dupilumab treatment in atopic dermatitis children. Potently, by suppressing skewed Th2 activation in patients with AD, the balance might shift toward Th1/Th17 predominance, and psoriasis develops. Baricitinib is a potential drug for psoriasiform erythema with significant therapeutic effects.

The effect of chronic renal insufficiency on orthodontic tooth movement in rats.

Abstract:The purpose of this study was to determine the effects of impaired renal function on the amount of orthodontic tooth movement (OTM) in a rat model of renal insufficiency (RI). The animals were divided into three groups: a control unoperated group in which only OTM was done, a sham-operated group, and an RI group. OTM was done two weeks after performance of the sham operation or the surgery to induce RI. Renal failure was induced by electrocoagulation of one kidney cortex followed by contralateral nephrectomy. A fixed orthodontic appliance consisting of a 5 mm length of nickel-titanium closed-coil spring was ligated between the maxillary right incisor and first molar to deliver an initial mesial force of 60 g. Four weeks after RI induction, the amount of tooth movement was estimated by measuring the space created between the first and second molars on a stone cast. Bone density was measured by optical densitometry on radiographs and serum parathyroid hormone (PTH) was measured with a radioimmunoassay method. The results showed that induction of RI significantly increased both OTM (P < .05) and PTH levels (P < .01), although there was no significant difference in bone density by optical densitometry between the unoperated, sham-operated, and RI groups four weeks after RI induction. The results suggest that an increase of bone-remodeling activities, as evident from a significantly higher level of PTH, may be responsible for increase in tooth movement.
Clinical Manifestations and Genetic Analysis of 17 Patients with Autosomal Dominant Hyper-IgE Syndrome in Mainland China: New Reports and a Literature Review

Abstract:Purpose Autosomal dominant hyper-IgE syndrome (AD-HIES) is a rare complicated primary immunodeficiency disease (PID). Signal transducer and activator of transcription 3 (STAT3) gene mutation is found to cause AD-HIES. The distribution of AD-HIES patients with STAT3 deficiency in the Chinese population is not clear. Herein, we retrospectively report 17 AD-HIES patients with STAT3 deficiency and demonstrate their clinical, immunological, and genetic features. Methods Patients’ clinical data were collected from their medical records. Routine laboratory testing results included lymphocyte subset analysis and immunoglobulin quantification. STAT3 mutations were investigated by sequencing of genomic DNA. Results Among 575 patients with PID, 28 (4.87%) were clinically diagnosed as HIES. Among them, 17 (2.96%) were confirmed as STAT3 mutant AD-HIES. The ratio of male to female patients was 8:9. All of the 17 patients had NIH scores over 40 points. The mean ages at onset and diagnosis were 1.05 and 10.35 years, respectively. Three patients (17.65%, 3/17) died with a mean age of 13.33 years. Eczema, recurrent skin infection, and respiratory tract infection were the most common clinical symptoms and are present in all of the 17 patients in this study. Six patients (37.5%, 6/16) suffered complication from BCG vaccination. Noninfection symptoms are characteristic facial features in 17 patients (100%, 17/17), retention of primary teeth in 10 patients (90.91%, 10/11), and abnormal bone fractures in 7 patients (41.18%, 7/17). Eleven types of STAT3 mutations were identified in 17 patients, including 1 novel mutation. Conclusions We here retrospectively report the largest Chinese cohort of AD-HIES patients with STAT3 mutation. Unique features, when compared to existing literature reports, include (1) later age of diagnosis, (2) significantly higher rate of BCG complications, and (3) lower rate of candidiasis and chronic otitis media.

Abstract:Autosomal dominant hyper-IgE syndrome (AD-HIES) is a rare inherited primary immunodeficient disease (PIDs), which is caused by STAT3 gene mutations. Previous studies indicated a defective Toll-like receptor (TLR) 9-induced B cell response in AD-HIES patients, including proliferation, and IgG production. However, the other TLRs-mediated B cell responses in AD-HIES patients were not fully elucidated. In this study, we systematically studied the B cell response to TLRs signaling pathways in AD-HIES patients, including proliferation, activation, apoptosis, cytokine, and immunoglobulin production. Our results showed that the TLRs-induced B cell proliferation and activation was significantly impaired in AD-HIES patients. Besides, AD-HIES patients had defects in TLRs-induced B cell class switch, as well as IgG/IgM secretion and IL-10 production in B cells. Taken together, we first systematically reported the deficiency of TLRs driven B cell response in AD-HIES patients, which help to have a better understanding of the pathology of AD-HIES.

566 - Efficacy of dupilumab treatment in atopic hand and foot dermatitis across morphological subtypes: results from a phase 3, randomized, double-blind, placebo-controlled trial

Abstract:Abstract Introduction/Background Dupilumab has previously shown to significantly improve signs, symptoms, and quality of life in patients with moderate-to-severe atopic hand and foot dermatitis. Objective To investigate the efficacy of dupilumab across morphological subtypes in patients with moderate-to-severe atopic hand and foot dermatitis. Methods The phase 3, randomized, double-blind LIBERTY-AD-HAFT (NCT04417894) trial enrolled patients aged ≥12 years with moderate-to-severe (Investigator’s Global Assessment [IGA] score of 3/4) atopic hand and foot dermatitis. Patients were randomized to dupilumab monotherapy 300 mg every 2 weeks (q2w) in adults; 200/300 mg q2w in adolescents, or placebo for 16 weeks. This analysis reports the percent change from baseline in modified total lesion sign score (mTLSS) by hand and foot morphological subtypes: chronic dry fissured, hyperkeratotic (palmar/plantar), and other. Results 133 patients enrolled into this study and were randomized to dupilumab (n = 67) or placebo (n = 66). Atopic hand and foot morphologies were reported in 3 categories: chronic dry fissured (n = 63), hyperkeratotic (palmar/plantar; n = 37), and other (n = 33) In the “other” category, dyshidrotic was the most frequent morphology (n = 13). mTLSS values were similar at baseline for all morphological subtypes. At Week 16, greater improvements were seen in the percent change from baseline (SE) in mTLSS for patients receiving dupilumab vs placebo in all categories: chronic dry fissured (−65.6% [5.1] vs −31.5% [6.2]), hyperkeratotic (palmar/plantar; −58.2% [7.6] vs −11.8% [7.7]), and other (−64.5% [11.6] vs −29.9% [9.0]). Safety was consistent with the known dupilumab safety profile in patients with atopic dermatitis. Conclusions The efficacy of dupilumab remains consistent across different hand and foot dermatitis morphologies and shows higher treatment benefit across subtypes compared with placebo.

Defective Toll-Like Receptors Driven B Cell Response in Hyper IgE Syndrome Patients With <i>STAT3</i> Mutations

Abstract:Autosomal dominant hyper-IgE syndrome (AD-HIES) is a rare inherited primary immunodeficient disease (PIDs), which is caused by STAT3 gene mutations. Previous studies indicated a defective Toll-like receptor (TLR) 9-induced B cell response in AD-HIES patients, including proliferation, and IgG production. However, the other TLRs-mediated B cell responses in AD-HIES patients were not fully elucidated. In this study, we systematically studied the B cell response to TLRs signaling pathways in AD-HIES patients, including proliferation, activation, apoptosis, cytokine, and immunoglobulin production. Our results showed that the TLRs-induced B cell proliferation and activation was significantly impaired in AD-HIES patients. Besides, AD-HIES patients had defects in TLRs-induced B cell class switch, as well as IgG/IgM secretion and IL-10 production in B cells. Taken together, we first systematically reported the deficiency of TLRs driven B cell response in AD-HIES patients, which help to have a better understanding of the pathology of AD-HIES.
Laboratory Safety of Dupilumab, and Its Effect on Inflammatory Biomarkers, in Chinese Adults with Moderate-to-Severe Atopic Dermatitis: an Analysis of a Randomized, Double-Blind Phase III Study

Abstract:Objective: Limited information is available on the use of dupilumab for the treatment of atopic dermatitis (AD) in the Chinese population. Methods: We analyzed laboratory data from a previously published randomized, double-blind phase III trial (NCT03912259) to provide further insight into the safety of dupilumab in Chinese adults with moderate to severe AD. The trial participants received either 300 mg of dupilumab or placebo every 2 weeks for 16 weeks. Hematology, blood chemistry, serum thymus and activation-regulated chemokine (TARC), and total immunoglobulin E (IgE) were evaluated. Results: In total, 82 participants received dupilumab and 83 received placebo. With the exception of eosinophil counts of >0.8 × 109/L, which were found less frequently with dupilumab (9.8%) than with placebo (18.7%), the hematology and blood chemistry values were generally stable in both treatment groups. There were no clinically significant differences between the dupilumab and placebo groups, and no participants developed treatment-emergent abnormalities of potential clinical significance. However, compared with placebo, greater decreases in serum lactate dehydrogenase (mean change, −97.4 vs. −33.5 IU/L), TARC (median percent change, −78.6% vs. −30.8%), and total IgE (median percent change, −53.4% vs. −0.2%) were observed with dupilumab than placebo at week 16. Conclusion: Dupilumab demonstrated a favorable laboratory safety profile in Chinese adults with moderate to severe AD.

Abstract:Abstract Introduction/Background Dupilumab has previously shown overall efficacy in treating atopic hand and foot dermatitis. Objective To report the effect of dupilumab treatment on individual signs of atopic hand and foot dermatitis. Methods The phase 3, randomized, double-blind LIBERTY-AD-HAFT (NCT04417894) trial enrolled patients aged ≥12 years with moderate-to-severe (Investigator’s Global Assessment [IGA] score of 3/4) atopic hand and foot dermatitis. Patients were randomized to dupilumab monotherapy 300 mg every 2 weeks (q2w) in adults; 200/300 mg q2w in adolescents, or placebo for 16 weeks. This analysis presents the proportion of patients reporting absent, mild, moderate, or severe erythema, scaling/flaking, lichenification, vesiculation/erosion, edema, and fissures, assessed by the modified total lesion sign score (mTLSS) in hands and feet. Results At baseline, most patients had scores of moderate or severe signs on their hands. Of the 133 patients enrolled, over 65% of patients treated with dupilumab (n = 67) achieved an absent or mild score by Week 16 in each of the signs/symptoms assessed. Proportion of patients with absent or mild hand scores increased from baseline to Week 16 in erythema (9% vs 71.6%), scaling/flaking (16.4% vs 74.7%), lichenification (4.5% vs 65.6%), vesiculation/erosion (43.3% vs 89.6%), edema (44.7% vs 86.6%), and fissures (23.9% vs 83.5%). Proportion of patients with absent or mild foot scores increased from baseline to Week 16 in erythema (56.7% vs 80.6%), scaling/flaking (56.7% vs 82.1%), lichenification (53.8% vs 82.1%), vesiculation/erosion (76.1% vs 86.6%), edema (76.1% vs 88.1%), and fissures (77.6% vs 86.6%). Safety was consistent with the known dupilumab safety profile in patients with atopic dermatitis. Conclusions Dupilumab treatment in patients improves signs of hand and foot dermatitis, including erythema, scaling/flaking, lichenification, vesiculation/erosion, edema, and fissures.

Abstract:Abstract Individuals with severe atopic dermatitis (AD) may present with very high serum IgE levels (> 10 000 IU mL−1). Signal transducer and activator of transcription 3 hyper-IgE syndrome (STAT3-HIES), a rare immunosuppressive disorder, shares pathogenic similarities with AD, involving dysregulation in the STAT3 signalling pathway and clinical, genetic and serological features similar to AD. Despite these parallels, patients with AD with very high IgE levels are not routinely tested for genetic overlap with STAT3-HIES. This might be relevant in therapeutic intervention. We reviewed the literature to identify individuals with possible overlap of both conditions to help identify a novel mixed phenotype (AD/STAT3-HIES). Intervention with Janus kinase (JAK) inhibitors in this subgroup might be beneficial given their shared dysregulation in STAT3 signalling pathways. This review investigates the identification of the AD/STAT3-HIES phenotype and its response to Janus kinase (JAK) inhibitors. A literature review was undertaken on PubMed, Scopus, Ovid and Google Scholar. Search terms included ‘atopic dermatitis’, ‘hyper-IgE syndrome’, ‘HIES’, ‘Job syndrome’, ‘STAT3’, ‘JAK/STAT pathway’ and ‘Janus kinase inhibitors’. Inclusion criteria were studies published in English between January 2005 and December 2023 relevant to the clinical features, treatment, pathogenesis and genetics of AD or STAT3-HIES. Out of 152 studies identified during the search, 29 met the inclusion criteria. The Table outlines the features of AD, STAT3-HIES and areas of overlap (AD/STAT3-HIES). Clinically, AD and STAT3-HIES share characteristics including eczematous lesions and staphylococcal skin infections. STAT3-HIES is distinguished from AD by the presence of immunoparesis and extracutaneous manifestations. Both diseases commonly exhibit elevated IgE levels. In severe AD, studies reported IgE levels ranging from 356 to 2379 IU mL−1, while patients with STAT3-HIES demonstrated levels between 1000 and 46 850 IU mL−1. Genetically, both diseases are associated with the STAT3 and IL6 genes. JAK inhibitors proved effective in treating AD in multiple studies. To date, only one ex vivo study suggested a potential treatment utility of JAK inhibitors in dominant negative STAT3 mutation. We postulate that patients with AD with elevated IgE levels beyond the typical range may benefit from further genotyping (STAT3 and IL6 gene mutations) to identify the AD/STAT3-HIES phenotype.
