Abstract:Patients who has been implanted with hip implant usually undergo revision surgery. The reason is that high stiff implants would cause non-physiological distribution loadings, which is also known as stress shielding, and finally lead to bone loss and aseptic loosening. Titanium implants are widely used in human bone tissues; however, the subsequent elastic modulus mismatch problem has become increasingly serious, and can lead to stress-shielding effects. This study aimed to develop a parametric design methodology of porous titanium alloy hip implant with gradient elastic modulus, and mitigate the stress-shielding effect. Four independent adjustable dimensions of the porous structure were parametrically designed, and the Kriging algorithm was used to establish the mapping relationship between the four adjustable dimensions and the porosity, surface-to-volume ratio, and elastic modulus. Moreover, the equivalent stress on the surface of the femur was optimized by response surface methodology, and the optimal gradient elastic modulus of the implant was obtained. Finally, through the Kriging approximation model and optimization results of the finite element method, the dimensions of each segment of the porous structure that could effectively mitigate the stress-shielding effect were determined. Experimental results demonstrated that the parameterized design method of the porous implant with gradient elastic modulus proposed in this study increased the strain value on the femoral surface by 17.1% on average. Consequently, the stress-shielding effect of the femoral tissue induced by the titanium alloy implant was effectively mitigated.

Abstract:In the process of artificial metal bone implantation, the mismatch of elastic modulus and bad permeability behavior lead to obvious stress shielding and poor bone tissue ingrowth. For these problems, this paper proposed a new design method of the variable porosity porous structure based on the stress line and Voronoi diagram. In the design process, the principal stress line was determined by mechanical finite element analysis, which controlled the seeds distribution of the 3D Voronoi diagram, making local porosity conforms to the stress distribution. In addition, the designed samples were fabricated through laser powder bed fusion (L-PBF) technology with TC4 powders. The mechanical characteristics were evaluated by static mechanical simulation with ABAQUS and compression tests, and the permeability characteristics were analyzed by fluid simulation with COMSOL and in vitro and vivo test. The research results showed that the designed structure obtained the relatively low elastic modulus and higher mechanical strength, the designed structure has closer permeability characteristics to the reconstructed natural bone, and was conducive to cell proliferation. So the porous structure based on the stress line and Voronoi diagram might have good application potential in the bionic design of bone implants.

Abstract:At present, selective laser melting (SLM) 3D printing technology can accurately control the internal pore structure and complex cell shape. Three types of reticulated meshes with cubic, G7 and composite structure cell shapes were fabricated by the SLM 3D printing technology using Ti–6Al–4V alloy powders. The bone stresses around the implant and the stresses in the implant were analyzed by ANSYS finite element software, which comprehensively evaluated the effect of porous dental implants with different spatial porosity characteristics on osseointegration. The results show that porous dental implants with composite structure of pore characteristics have improved mechanical and biological properties and can better promote the growth and integration of bone tissue.

Biomechanical Effects of 3D-Printed Bioceramic Scaffolds With Porous Gradient Structures on the Regeneration of Alveolar Bone Defect: A Comprehensive Study

Abstract:In the repair of alveolar bone defect, the microstructure of bone graft scaffolds is pivotal for their biological and biomechanical properties. However, it is currently controversial whether gradient structures perform better in biology and biomechanics than homogeneous structures when considering microstructural design. In this research, bioactive ceramic scaffolds with different porous gradient structures were designed and fabricated by 3D printing technology. Compression test, finite element analysis (FEA) revealed statistically significant differences in the biomechanical properties of three types of scaffolds. The mechanical properties of scaffolds approached the natural cancellous bone, and scaffolds with pore size decreased from the center to the perimeter (GII) had superior mechanical properties among the three groups. While in the simulation of Computational Fluid Dynamics (CFD), scaffolds with pore size increased from the center to the perimeter (GI) possessed the best permeability and largest flow velocity. Scaffolds were cultured in vitro with rBMSC or implanted in vivo for 4 or 8 weeks. Porous ceramics showed excellent biocompatibility. Results of in vivo were analysed by using micro-CT, concentric rings and VG staining. The GI was superior to the other groups with respect to osteogenicity. The Un (uniformed pore size) was slightly inferior to the GII. The concentric rings analysis demonstrated that the new bone in the GI was distributed in the periphery of defect area, whereas the GII was distributed in the center region. This study offers basic strategies and concepts for future design and development of scaffolds for the clinical restoration of alveolar bone defect.

Gyroid-based functionally graded porous titanium scaffolds for dental application: Design, simulation and characterizations

Abstract:To improve the mechanical compatibility and biocompatibility of porous scaffolds for dental application, functionally graded porous titanium scaffolds with average porosity of 20–80 % were designed and fabricated by selective laser melting. Finite element modeling in the oral environment showed that the maximum von-Mises strains of cortical and cancellous bone around graded scaffolds with 20–50 % porosities were in the range of 100–3000 με and 1000–3000 με, respectively, which is beneficial for bone regeneration. Additionally, the graded scaffolds exhibited higher compressive yield strength (206.1–477.5 MPa) yet suitable elastic modulus (8.5–20.7 GPa) compared with human cortical bone. Permeability of graded scaffolds increased with increasing porosity that all fall in the range of the permeability for human bone. Corrosion rate of graded scaffolds with 20–40 % porosity satisfied the corrosion resistance for metallic implants (2.5 × 10 −4 /mm/yr). Moreover, the graded scaffolds with 20–40 % porosity exhibited 0-I grade toxic effects on mouse embryonic osteoblast precursor cell proliferation in vitro. Taken together, the graded scaffold with 30 % porosity meets all requirements as a promising dental implant. In addition, this study provided a rational approach for screening the best geometrical design of the implants in dentistry.

Novel Design and Finite Element Analysis of Diamond-like Porous Implants with Low Stiffness

Abstract:The purpose of this study was to design porous implants with low stiffness and evaluate their biomechanical behavior. Thus, two types of porous implants were designed (Type I: a combined structure of diamond-like porous scaffold and traditional tapered thread. Type II: a cylindrical porous scaffold filled by arrayed basic diamond-like pore units). Three implant-supported prosthesis models were constructed from Type I, Type II and commercial implants (control group) and were evaluated by finite element analysis (FEA). The stress distribution pattern of the porous implants were assessed and compared with the control group. In addition, the stiffness of the cylindrical specimens simplified from three types of implants was calculated. The Type I implant exhibited better stress distribution than the Type II implant. The maximum stress between the cortical bone–Type I implant interface was 12.9 and 19.0% lower than the other two groups. The peak stress at the cancellous bone–Type I implant interface was also reduced by 16.8 and 38.7%. Compared with the solid cylinder, the stiffness of diamond-like pore cylinders simplified from the two porous implants geometry was reduced by 61.5 to 76.1%. This construction method of porous implant can effectively lower its stiffness and optimize the stress distribution at the implant–bone interface.

The internal density of the porous prosthesis can change with the structural stress and achieve the maximum porosity satisfying the strength and stiffness requirements. Compared with the regular porous structure, the optimum design can have a more uniform stress distribution, and the stiffness of the prosthesis is closer to the surrounding bone tissue.

Abstract:The porous scaffold – a substitute for artificial bone – is vital to bone defect repair and bone tissue regeneration in tissue engineering. In consideration of the limitations of few existing design methods, the significant difference between the internal pore structure of porous scaffolds and natural bone microstructure, and the low controllability of design methods. Based on Voronoi-Tessellation (VT) and random function theory, this paper proposed a design method with controllable pore density, adjustable pore shape and size, and heterogeneous pore distribution. The stress change law of porous scaffolds under compression was analyzed using finite element simulation experiments; the relationship between the porosity and the structural strength of porous scaffolds was obtained. Furthermore, the anisotropy of porous scaffolds was studied, and the change law of the macroscopic equivalent elastic modulus with the change in porosity of porous scaffolds was acquired. Under the same porosity, the mechanical properties of the porous scaffold with a uniform pore distribution and the porous scaffold designed in this paper were analyzed and compared; the damage extension laws of two different porous scaffolds were obtained. The results demonstrate that the design method proposed in this paper has high controllability, and the parametric modeling process is both random and controllable. The internal pore structures of porous scaffolds generated by the design method are closer to the microstructure of natural bone tissue – the structural strength satisfies the requirements of artificial implants. Abstract:The metal titanium is often used as a dental implant material, and the elastic modulus of solid titanium implants does not match the biological bone tissue, which can easily produce a stress shielding effect and cause implant failure. In this paper, a three-level gradient porous structure implant was designed, and its mechanical and biological adaptability were studied by finite element simulation analysis. Combined with the comprehensive evaluation of the mechanical and biological properties of implants of various structures, the analysis found that a porous implant with porosity of 59.86% of the gradient was the best structure. The maximum equivalent stress of this structure in the mandible that simulated the oral environment was 154.34 MPa, which was less than half of its theoretical compression yield strength. The strain of the surrounding bone tissue lies in the bone compared with other structures, the proportion of the active state of plastic construction is larger, at 10.51%, and the fretting value of this structure and the bone tissue interface is the smallest, at only 10 μm.
Biomechanical and Mechanostat analysis of a titanium layered porous implant for mandibular reconstruction: The effect of the topology optimization design

Abstract:A porous scaffold/implant is considered a potential method to repair bone defects, but its mechanical stability and biomechanics during the repair process are not yet clear. A mandibular titanium implant was proposed and designed with layered porous structures similar to that of the bone tissue, both in structure and mechanical properties. Topology was used to optimize the design of the porous implant and fixed structure. The finite element analysis was combined with bone "Mechanostat" theory to evaluate the stress and osteogenic property of the layered porous implant with 3 different fixation layouts (Model I with 4 screws, Model II with 5 screws and Model III with 6 screws) for mandibular reconstruction. The results showed that Model III could effectively reduce the stress shielding effect, stress within the optimized implant, defective mandible, and screws were respectively dropped 48.18%, 44.23%, and 57.27% compared to Model I, and the porous implant had a significant stress transmission effect and maintained the same stress distribution as the intact mandible after the mandibular defect was repaired. The porous implant also showed a significant mechanical stimulation effect on the growth and healing of the bone tissue according to the bone "Mechanostat" theory. The combination of porous structure with the topology technique is a promising option to improve the mechanical stability and osteogenesis of the implant, and could provide a new solution for mandibular reconstruction.

Abstract:To obtain high-performing biological fixation implants with excellent properties, it is necessary to comprehensively characterize the design method and molding process of new porous structures printed using 3D printing technology (selective laser melting). We adopted parametric modeling to design a bionic multi-level porous structure, evaluated the properties of the porous structure with finite element method, and optimized the design parameters. Finally, the properties of the 3D printed porous structure were characterized by a compression experiment and fractal theory. The results indicate that the porous structure designed by parametric modeling exhibits a good modeling effect. When a traditional porous structure is stressed, the stress mainly concentrates on the longitudinal bar, while the transverse bar is less stressed. In multi-level porous structures, the stress distributions of longitudinal and transverse bars are relatively uniform. No obvious molding defects were detected in the prepared porous structures. The interlaminar fracture mechanism of traditional porous structure is brittle fracture, and the fracture mechanism of multi-level porous structure is elastic fracture. The compressive strength and modulus increases with the increase in mean pore size and surface-to-volume ratio, and decreases with the increase in porosity. These results present a foundation for 3D printing of high-performing biological fixation implants.

Abstract:Human bone cells live in a complex environment, and the biomimetic design of porous structures attached to implants is in high demand. Porous structures based on Voronoi tessellation with biomimetic potential are gradually used in bone repair scaffolds. In this study, the mechanical properties and permeability of trabecular-like porous scaffolds with different porosity levels and average apertures were analyzed. The mechanical properties of bone-implant scaffolds were evaluated using finite element analysis and a mechanical compression experiment, and the permeability was studied by computational fluid dynamics. Finally, the attachment of cells was observed by confocal fluorescence microscope. The results show that the performance of porous structures can be controlled by the initial design of the microstructure and tissue morphology. A good structural design can accurately match the performance of the natural bone. The study of mechanical properties and permeability of the porous structure can help address several problems, including stress shielding and bone ingrowth in existing biomimetic bone structures, and will also promotes cell adhesion, migration, and eventual new bone attachment.

Abstract:Due to the material itself and the traditional geometric design, short implants have the problems of high stiffness, less osseointegrated area, and lower long-term reliability in clinical application. In this research, short implants with various porous structures were constructed and their biomechanical responses were evaluated, including biomechanical properties, compression capability, and stress distribution of surrounding bone tissue. Thus, three types of short implants were built: the diamond-like porous short implant (5.0 × 5.0 mm), the hexahedral porous short implant (5.0 × 5.0 mm), and the solid short implant (5.0 × 5.0 mm). Three implant-supported prosthetic models were built based on the above three implants, and their static mechanical properties were assessed. Besides, the average stiffness of regular cylindrical samples was calculated by compression test simulation. The result confirmed that hexahedral porous short implants showed minimum peak stress compared with the diamond-like porous and solid short implants. Similarly, the value of stress peaks in the bone-implant interface for the hexahedral porous short implant appeared lower than the other two short implants. Compression simulation indicated the average stiffness of cylinders after diamond-like and hexahedral porous design was 92.2% and 56.3% lower than the solid cylinder. Compared with the other two designs, the hexahedral porous short implant obtained better stress distribution, even avoiding bone overstress.

Abstract:Continuous gradient triply periodic minimal surface (TPMS) porous structure has been proven to be one of the most suitable structures for bone implants due to their excellent mechanical properties and high porosity. This study establishes a parametric modeling method for continuous gradient TPMS structures and optimizes the TPMS porous structure with a continuous gradient change in porosity. Ti-6Al-4V continuous gradient TPMS porous structures were prepared using powder bed fusion (PBF). The mechanical properties and permeability of the continuous gradient TPMS porous structure were studied. The results indicate that the porosity control parameter C for gradient continuous change follows a linear function, with the porosity increasing linearly within the specified range of values. The influence of the periodic parameter ω on the mechanical properties and permeability of different types of TPMS structures varies. The Gyroid continuous gradient structure aligns more closely with the mechanical properties and permeability of bone scaffolds. Furthermore, a TPMS continuous gradient porous structure that is more suitable for trabecular bone implants was obtained through topology optimization design. A bone implant model and object suitable for human trabecular bone were designed and printed, providing technical support for subsequent performance testing and application research of bone implants.

Abstract:In orthopedic implant development, incorporating a porous structure into implants can reduce the elastic modulus to prevent stress shielding but may compromise yield strength, risking prosthesis fracture. Bamboo's natural structure, with its exceptional strength-to-weight ratio, serves as inspiration. This study explores biomimicry using bamboo-inspired porous scaffolds (BISs) resembling cortical bone, assessing their mechanical properties and fluid characteristics. The BIS consists of two 2D units controlled by structural parameters α and β. The mechanical properties, failure mechanisms, energy absorption, and predictive performance are investigated. BIS exhibits mechanical properties equivalent to those of natural bone. Specifically, α at 4/3 and β at 2/3 yield superior mechanical properties, and the destruction mechanism occurs layer by layer. Besides, the Gibson-Ashby models with different parameters are established to predict mechanical properties. Fluid dynamics analysis reveals two high-flow channels in BISs, enhancing nutrient delivery through high-flow channels and promoting cell adhesion and proliferation in low-flow regions. For wall shear stress below 30 mPa (ideal for cell growth), α at 4/3 achieves the highest percentage (99.04%), and β at 2/3 achieves 98.46%. Permeability in all structural parameters surpasses that of human bone. Enhanced performance of orthopedic implants through a bionic approach that enables the creation of pore structures suitable for implants.

Additive manufacturing of Bio-inspired ceramic bone Scaffolds: Structural Design, mechanical properties and biocompatibility

Abstract:Ideal bone scaffolds require good biocompatibility and moderate mechanical properties, so as to promote the proliferation and differentiation of osteoblast cells, and achieve the good bone repair. Inspired by the porous structure of cancellous bone, 15 groups of bone scaffolds with variable irregularity (Ir1-5) and porosity (35.53-61.75%) were designed and fabricated by ceramic digital light processing (DLP) using 20 wt.% hydroxyapatite doped zirconia as the matrix material. The effects of structural parameters and material on mechanical properties and biocompatibility were studied. The shrinkage test results showed that the density of scaffolds was mainly affected by the porosity. The mechanical test results showed that Ir2 and Ir3 scaffolds had better compressive behaviors, and the compressive strength could be increased by 30% by regulating the irregularity. All scaffolds showed comparable mechanical properties to that of cancellous bone. Cell experiments showed that the effect of structure on cell proliferation, differentiation, and mineralization was most evident at the early stage of implantation. Meanwhile, the biocompatibility variation with the irregularity was consistent with mechanical properties. This study proved that a bio-inspired bone scaffold with excellent comprehensive properties could be obtained through reasonable design.

Abstract:Polyether-ether-ketone (PEEK) composites represent one of the most promising approaches to overcoming the weak osseointegration associated with the bioinertness of PEEK, making them highly suitable for clinical translation. Implants with porous structures fabricated by additive manufacturing offer the potential for long-term stability by promoting bone ingrowth. However, despite the importance of porous design, there is still no consensus on the optimal approach for PEEK-based composites. Given the significance of permeability and mechanical properties as functional indicators closely linked to osseointegration, the effects of material composition, structural design, and manufacturing processes on the permeability and mechanical properties of PEEK/hydroxyapatite (HA) scaffolds were systematically investigated in this study. In terms of permeability, the axial permeability of scaffolds with different pore sizes and representative volume elements varied within the range of 0.3-24.8 × 10 -9 m 2 . Among scaffolds with similar relative density, the Gyroid structure exhibited the lowest permeability, while the orthogonal structure demonstrated the highest. For cylindrical scaffolds, circumferential permeability decreased with increasing penetration depth, suggesting a potential reduction in bone ingrowth speed with depth. As for mechanical properties, the experimentally determined effective elastic modulus and effective yield strength of the scaffolds ranged from 675.1 MPa to 65.2 MPa and 43.5 MPa to 4.1 MPa, respectively. The permeability and mechanical properties of PEEK/HA scaffolds with relative density ranging from 35-50% aligned with the those of human cancellous bone. By applying heat treatment at 240°C for 120 minutes, the crystallinity of PEEK increased to 37.2%, resulting in a substantial improvement in both the strength and stiffness of the scaffolds. However, excessive crystallinity led to brittle fracture, which in turn reduced the strength of the scaffolds. This study employed a systematic research approach to investigate how material composition, structural design, and manufacturing processes influence the mechanical properties and permeability of PEEK composite bone scaffolds, which are crucial for bone ingrowth. The results offered insights that support the design, manufacturing, and performance evaluation of PEEK-based porous implants.

Abstract:Introduction: The bone ingrowth depth in the porous scaffolds is greatly affected by the structural design, notably the pore size, pore geometry, and the pore distribution. To enhance the bone regeneration capability of scaffolds, the bionic design can be regarded as a potential solution.Objectives: We proposed a Haversian system-like gradient structure based on the triply periodic minimal surface architectures with pore size varying from the edge to the center. And its effects in promoting bone regeneration were evaluated in the study.Methods: The gradient scaffold was designed using the triply periodic minimal surface architectures. The mechanical properties were analyzed by the finite element simulation and confirmed using the universal machine. The fluid characteristics were calculated by the computational fluid dynamics analysis. The bone regeneration process was simulated using a in silico computational model containing the main biological, physical, and chemical variation during the bone growth process. Finally, the in vitro and in vivo studies were carried out to verify the actual osteogenic effect.Results: Compared to the uniform scaffold, the biomimetic gradient scaffold demonstrated better performance in stress conduction and reduced stress shielding effects. The fluid features were appropriate for cell migration and flow diffusion, and the permeability was in the same order of magnitude with the natural bone. The bone ingrowth simulation exhibited improved angiogenesis and bone regeneration. Higher expression of the osteogenesis-related genes, higher alkaline phosphatase activity, and increased mineralization could be observed on the gradient scaffold in the in vitro study. The 12-week in vivo study proved that the gradient scaffold had deeper bone inserting depth and a more stable bone-scaffold interface.Conclusion: The Haversian system-like gradient structure can effectively promote the bone regeneration. Effect of lattice type on biomechanical and osseointegration properties of 3D-printed porous Ti6Al4V scaffolds

Abstract:Porous structure is an efficient tool for optimizing the elastic modulus and osseointegration properties of titanium alloy materials. However, the investigations on pore shape remain scarce. In this study, we created porous Ti6Al4V scaffolds with a pore size of 600 μm but different lattices (cubic pentagon, diamond, cuboctahedron). The mechanical and biological properties of the scaffolds were investigated in static simulation analysis, in vitro mechanical compression test, computational fluid dynamics, as well as cell and animal experiments. The results demonstrated that the calculated yield strength difference between the three Ti6Al4V porous scaffolds was negligible, at approximately 140 MPa, allowing them to match the strength requirements of human bones. The diamond scaffold has the lowest calculated elastic modulus (11.6 GPa), which is conducive for preventing stress shielding. The shear stress was largely concentrated in the diamond scaffold, and the stress range of 120–140 MPa accounted for the greatest share. The mouse MC3T3-E1 cells were found to attach to all three scaffolds, with the diamond scaffold displaying a higher degree of cell adherence. There was more proliferating cells on the diamond and cubic pentagon scaffolds than on the cuboctahedron scaffolds (P < 0.05). The diamond scaffold exhibited the highest alkaline phosphatase activity and calcium salt accumulation in cell differentiation tests. Besides, the expression of osteogenic genes on the diamond scaffold was higher than that on the cuboctahedron scaffold, the cubic pentagon scaffold displaying the lowest expression. The in vivo studies revealed that all three scaffolds fused well with the surrounding bone and that there was no loosening or movement of the prosthesis. Micro-computed tomography, corroborated by the staining results of hard tissues, revealed that the level of new bone formation was the highest in the diamond scaffold, followed by the cuboctahedron scaffold (P < 0.05). Taken together, the diamond scaffold is comparatively better at optimizing the elastic modulus and osseointegration properties of titanium alloy materials, and thus is a preferred choice for porous design.
