New insights in brain-to-eye transport: can excess cerebrospinal fluid in astronauts escape into the eye?

Abstract:Spaceflight-associated neuro-ocular syndrome (SANS) describes a constellation of neuro-ophthalmic findings that have been observed in astronauts during and after long-duration spaceflight [1, 2]. These neuro-ocular changes include optic disc edema (ODE), globe flattening, choroidal folds, and hyperopic refractive error shifts [1, 2]. An increased understanding of SANS emerges as a top priority for space agencies, particularly because this neuro-ocular syndrome could impact the success of future human, longer-duration missions, including a return to the Moon and manned missions to Mars [2]. Up to 70% of all astronauts who complete long-duration missions to the International Space Station develop early signs of ODE as quantified by optical coherence tomography [3]. The precise mechanisms underlying SANS-associated ODE remain incompletely understood and are currently an active focus of research. As discussed below, new findings regarding brain-to-eye transport could shed more light on the pathogenesis of ODE and other ocular changes in astronauts. In 2020, the Nedergaard laboratory discovered an "anterograde ocular glymphatic pathway" which removes fluid and metabolites from the posterior eye into the optic nerve in mice and rats [12]. After intravitreal injection of small tracer molecules, such as exogenous human β-amyloid (hAβ), tracers were transported over the glial lamina via an intra-axonal route in retinal ganglion cell axons and via the perivenous spaces of the retina and optic nerve head, and were further cleared via anterograde ocular glymphatic transport (Fig. 1) [7, 12]. The authors demonstrated that fluid flow from the retina to the optic nerve is driven by the pressure difference between IOP and ICP across the glial lamina and by light-induced pupil constriction [12]. Intriguingly, following tracer injection into the cisterna magna of mice, Cao et al. [18] now found that, under physiological conditions, small tracer molecules like exogenous hAβ were transported along two routes from the brain to the retina. Firstly, hAβ tracer was transported along the myelinated axon spaces within the optic nerve toward the eye. Secondly, hAβ tracer was transported along the perivascular spaces of the central retinal artery in the optic nerve and passed through the lamina cribrosa before being transported along the retinal vessels, ultimately spreading throughout the entire retina. Given that Cao et al. [18] used smaller molecular weight tracers than the previous studies reporting that CSF tracers do not pass through the lamina cribrosa, this suggests that access from the brain into the eye was restricted based upon molecular size. Cao et al. [18] also argued that brain-derived hAβ, following reaching the SAS of the optic nerve, could drain through optic nerve meningeal lymphatic vessels situated in the dura mater, and periorbital lymphatic vessels located in the connective tissue adjacent to the sclera. As noted above, these orbital lymphatic drainage systems may be affected by cephalad fluid shifts of prolonged microgravity, impeding CSF outflow and producing increased optic nerve sheath pressures [1].
