Abstract:Virtual surgical planning (VSP) is commonly used in orthognathic surgery. A precise soft-tissue predictability would be a helpful tool, for determining the correct displacement distances of the maxilla and mandible. This study aims to evaluate the soft-tissue predictability of the VSP software IPS CaseDesigner® (KLS Martin Group, Tuttlingen, Germany). Twenty patients were treated with bimaxillary surgery and were included in the study. The soft-tissue simulation, done by the VSP was exported as STL files in the engineering software Geomagic Control XTM (3D systems, RockHill, SC, USA). Four months after surgery, a 3D face scan of every patient was performed and compared to the preoperative simulation. The quality of the soft-tissue simulation was validated with the help of a distance map. This distance map was calculated using the inter-surface distance algorithm between the preoperative simulation of the soft-tissue and the actual scan of the postoperative soft-tissue surface. The prediction of the cranial parts of the face (upper cheek, nose, upper lip) was more precise than the prediction of the lower areas (lower cheek, lower lip, chin). The percentage of correctly predicted soft-tissue for the face in total reached values from 69.4% to 96.0%. The VSP system IPS CaseDesigner® (KLS Martin Group; Tuttlingen, Germany) predicts the patient’s post-surgical soft-tissue accurately. Still, this simulation has to be seen as an approximation, especially for the lower part of the face, and continuous improvement of the underlying algorithm is needed for further development.

Development of an artificial intelligence model for predicting implant size in total knee arthroplasty using simple X-ray images

Abstract:Background: Accurate estimation of implant size before surgery is crucial in preparing for total knee arthroplasty. However, this task is time-consuming and labor-intensive. To alleviate this burden on surgeons, we developed a reliable artificial intelligence (AI) model to predict implant size. Methods: We enrolled 714 patients with knee osteoarthritis who underwent total knee arthroplasty from March 2010 to February 2014. All surgeries were performed by the same surgeon using implants from the same manufacturer. We collected 1412 knee anteroposterior (AP) and lateral view x-ray images and retrospectively investigated the implant size. We trained the AI model using both AP and lateral images without any clinical or demographic information and performed data augmentation to resolve issues of uneven distribution and insufficient data. Using data augmentation techniques, we generated 500 images for each size of the femur and tibia, which were then used to train the model. We used ResNet-101 and optimized the model with the aim of minimizing the cross-entropy loss function using both the Stochastic Gradient Descent (SGD) and Adam optimizer. Results: The SGD optimizer achieved the best performance in internal validation. The model showed micro F1-score 0.91 for femur and 0.87 for tibia. For predicting within ± one size, micro F1-score was 0.99 for femur and 0.98 for tibia. Conclusion: We developed a deep learning model with high predictive power for implant size using only simple x-ray images. This could help surgeons reduce the time and labor required for preoperative preparation in total knee arthroplasty. While similar studies have been conducted, our work is unique in its use of simple x-ray images without any other data, like demographic features, to achieve a model with strong predictive power.

Abstract:C-arm cone-beam CT (CBCT) can provide intraoperative 3D imaging capability for surgical guidance, but workflow and radiation dose are the significant barriers to broad utilization. One main reason is that each 3D image acquisition requires a complete scan with a full radiation dose to present a completely new 3D image every time. In this paper, we propose to utilize patient-specific CT or CBCT as prior knowledge to accurately reconstruct the aspects of the region that have changed by the surgical procedure from only a sparse set of x-rays. The proposed methods consist of a 3D-2D registration between the prior volume and a sparse set of intraoperative x-rays, creating digitally reconstructed radiographs (DRRs) from the registered prior volume, computing difference images by subtracting DRRs from the intraoperative x-rays, a penalized likelihood reconstruction of the volume of change (VOC) from the difference images, and finally a fusion of VOC reconstruction with the prior volume to visualize the entire surgical field. When the surgical changes are local and relatively small, the VOC reconstruction involves only a small volume size and a small number of projections, allowing less computation and lower radiation dose than is needed to reconstruct the entire surgical field. We applied this approach to sacroplasty phantom data obtained from a CBCT test bench and vertebroplasty data with a fresh cadaver acquired from a C-arm CBCT system with a flat-panel detector. The VOCs were reconstructed from a varying number of images (10-66 images) and compared to the CBCT ground truth using four different metrics (mean squared error, correlation coefficient, structural similarity index and perceptual difference model). The results show promising reconstruction quality with structural similarity to the ground truth close to 1 even when only 15-20 images were used, allowing dose reduction by the factor of 10-20.
Real-time Prediction of Soft Tissue Deformations Using Data-driven Nonlinear Presurgical Simulations

Abstract:Imaging modalities provide clinicians with real-time visualization of anatomical regions of interest (ROI) for the purpose of minimally invasive surgery. During the procedure, low-resolution image data are acquired and registered with high-resolution preoperative 3D reconstruction to guide the execution of surgical preplan. Unfortunately, due to the potential large strain and nonlinearities in the deformation of soft biological tissues, significant mismatch may be observed between ROI shapes during pre- and intra-operative imaging stages, making the surgical preplan prone to failure. In an effort to bridge the gap between the two imaging stages, this paper presents a data-driven approach based on artificial neural network for predicting the ROI deformation in real-time with sparsely registered fiducial markers. For a head-and-neck tumor model with an average maximum displacement of 30 mm, the maximum surface offsets between benchmarks and predictions using the proposed approach for 98% of the test cases are under 1.0 mm, which is the typical resolution of high-quality interventional ultrasound. Each of the prediction processes takes less than 0.5 s. With the resulting prediction accuracy and computational efficiency, the proposed approach demonstrates its potential to be clinically relevant.

Abstract:Abstract BackgroundThe high anatomical complexity of maxillofacial defects caused by tumor can pose a formidable challenge for clinicians when designing an appropriate plan for surgical reconstruction. The intention of this work was to restore the complex anatomy with maximum possible facial functionality and aesthetics of the patient. Based on the medical images generated by computed tomography (CT) scan an optimal therapeutic planning for complex maxillofacial reconstruction was designed. MethodFirstly, the volumetric data sets were carefully evaluated and deeply inspected for accurate diagnosis. Regarding 3D visualization of the CT scan images 3D virtual models for regions of interest were created using a special software of 3D Slicer. Using the resulting 3D virtual models a well-defined virtual surgical planning was generated for multiple surgical procedures, including the osteotomies for bone defects, harvesting autogenous bone graft and creating a customized implant. ResultsThe relevant patient-specific anatomical models for real surgery were translated into the 3D printed physical models, with which the surgeons can rehearse the surgery before coming into the operating room. Precisely defined multiple surgeries for complex maxillofacial reconstruction were proposed in this research that could be transferred to the real-time surgery. ConclusionsThe proposed surgical approach will be beneficial both for the surgeons and patient, including improvement in surgical precision and outcomes, reduction in operating time, as well as understanding surgical procedures and decision making etc.

Abstract:Background: Virtual surgical planning (VSP) is state of the art in routine clinical work. Visualization of soft tissue changes adds important information for surgical planning. The aim of this study was to evaluate accuracy of soft tissue prediction of 2 VSP systems in patients undergoing an intraoral quadrangular Le Fort II osteotomy.Methods: VSP was performed with the software application IPS Case Designer (IPS) and Dolphin Imaging 11.95 (DOL) in bone and soft tissue structure. Distances were measured at the 3 levels of the face at the infraorbital rim, the sinus floor, and the lateral incisor level with 2 VSP systems (DOL and IPS).Results: A convenience sample of 19 patients was included in the study with a mean age of 21.9 years. From cranial to caudal, mean differences between simulation and postintervention data were as follows: infraorbital rim level: DOL and ST0: mean difference: 2.90 mm; IPS and ST0: 1.70 mm; sinus floor level: DOL and ST0: mean difference: 3.57 mm; IPS and ST0: 1.34 mm; and lateral incisor level: DOL and ST0: mean difference: 2.48 mm; IPS and ST0: 2.25 mm.Conclusions: Generally, both VSP systems are suitable for planning an intraoral quadrangular Le Fort II osteotomy. Especially in the infraorbital region, improvement of the algorithm is required for trustworthy prediction of soft tissue changes.
Three-dimensional virtual planning in mandibular advancement surgery: Soft tissue prediction based on deep learning

Abstract:The study aimed at developing a deep-learning (DL)-based algorithm to predict the virtual soft tissue profile after mandibular advancement surgery, and to compare its accuracy with the mass tensor model (MTM). Subjects who underwent mandibular advancement surgery were enrolled and divided into a training group and a test group. The DL model was trained using 3D photographs and CBCT data based on surgically achieved mandibular displacements (training group). Soft tissue simulations generated by DL and MTM based on the actual surgical jaw movements (test group) were compared with soft-tissue profiles on postoperative 3D photographs using distance mapping in terms of mean absolute error in the lower face, lower lip, and chin regions. 133 subjects were included - 119 in the training group and 14 in the test group. The mean absolute error for DL-based simulations of the lower face region was 1.0 ± 0.6 mm and was significantly lower (p = 0.02) compared with MTM-based simulations (1.5 ± 0.5 mm). CONCLUSION: The DL-based algorithm can predict 3D soft tissue profiles following mandibular advancement surgery. With a clinically acceptable mean absolute error. Therefore, it seems to be a relevant option for soft tissue prediction in orthognathic surgery. Therefore, it seems to be a relevant options.

Abstract:Preoperative planning of complex osteotomies in craniomaxillofacial surgery, in conjunction with a surgeon's expertise, is essential for achieving an optimal result. However, the soft tissue changes that accompany facial bone movements cannot yet be accurately predicted. Bony tissue, because of its greater density, can be better predicted, but it alone does not account for the final aesthetic result. A new approach using not only three-dimensional (3-D) surface models of the patient's anatomy, but also a corresponding volumetric model, is discussed. This 3-D planning software was used in the treatment of 15 patients and was found to provide a good correlation between simulation and postoperative outcome.

Abstract:Abstract The VECTRA® XT 3D simulation program is a valuable tool for implant selection during the decision-making process of breast augmentations. The software allows for the assignment of automatically or manually positioned anatomical landmarks and provides accompanying measurements. This study aimed to present a strategy for standardizing landmark placement on Vectra® images during primary breast augmentations, optimize implant selection, and insure consistency in breast width data collection. We performed a retrospective analysis of patient data wherein breast width measurements were obtained through Clinical Assessment (CA) and the Vectra® program using both automatically (VA) and manually (VM) placed landmarks. Consideration was also given to the thickness of medial and lateral breast tissues. The suggested implant base width from all groups was then compared to the actual implant (AI) used during surgical procedures. Comparing the change rates in relation to the actual implant width revealed that rates in CA measurements were significantly lower than those in VA (p < 0.05), and similarly lower than those in VM (p < 0.05). Furthermore, change rates in VM were significantly lower than in VA (p < 0.05). Each group showed a significant positive correlation with the actual implant width. The Vectra® XT 3D simulation program is a valuable adjunct in breast augmentation. However, relying on it solely may lead to inaccuracies in the assessment of breast dimensions due to automatic landmarking. To achieve standardized measurements, landmarks can be marked on the patient before creating the Vectra® image. Our results underline the superiority of clinical judgment over simulation programs in establishing a reliable surgical plan.

Abstract:Background In China, traditional preoperative planning of unilateral breast reconstruction mainly depends on anthropometric measurement and visual assessment. Thus, the lack of objective assessment of breast volume and shape would likely result in suboptimal reconstruction outcomes. Three-dimensional surface imaging (3D-SI), which could provide objective measurement data of the breast, may be a promising solution to this problem. Methods A retrospective review of patients undergoing tissue expander (TE)/implant breast reconstruction without any mammoplasty surgery on the contralateral sides in our hospital from August 2013 to May 2018 was performed. All the patients underwent unilateral mastectomy with immediate or delayed insertion of TE, followed by an exchange of a silicone gel implant without contralateral procedures. 3D images were obtained at the time of preoperation, the routine expansion visit, and post-exchange of implant. The breast volume measured by 3D-SI served as a guide to conduct the surgery management, such as in deciding the total volume of expansion and guiding the final implant size selection. 3D-SI also provided objective data to evaluate the final outcomes of the reconstruction. Results Fifty-one patients were included in this study, in which eighteen patients underwent immediate TE insertion and thirty-three patients underwent delayed TE insertion. The ptosis degree of contralateral breasts was assessed as follows: forty-four were normal, and seven showed mild ptosis. The average expansion degree was controlled at 161.6% ± 14.1% compared to the contralateral breasts. The volume of implants exchanged had a strong linear correlation with the 3D volume of the contralateral breasts at the end of expansion ( P < 0.01). The mean time of follow-up was 9.1 ± 6.6 months. There was only one patient who experienced TE leakage with secondary infection and received TE exchange. For the immediate reconstruction group, the overall breast symmetry improved at the completion of implant exchange ( P < 0.01), with an average asymmetry of 5.3% ± 4.0% compared with 10.6% ± 6.1% initially. For the delayed reconstruction group, the reconstructed side achieved good volume symmetry to the contralateral side ( P > 0.05). There was no significant difference in breast basal width between bilateral breasts post-reconstruction ( P > 0.05). Conclusion 3D-SI serves as a valuable adjunct by providing accurate 3D volume of breasts within TE/implant breast reconstruction in Chinese patients without obvious breast ptosis, which could facilitate surgeons to achieve good reconstructive outcomes. Automatic Segmentation of MRI in Prospective Breast Volume Evaluation: Comparison of Different Assessments for Immediate Breast Reconstruction

Abstract:Summary Background Assessment of breast volume is essential in preoperative planning of immediate breast reconstruction (IBR) surgery in order to achieve a satisfactory cosmetic outcome. This study introduced a breast volume measurement tool that can perform segmentation of magnetic resonance images (MRI) and calculate breast volume automatically. We compared the accuracy and reliability of this measurement method with four other conventional modalities. Methods Patients scheduled to undergo mastectomy with IBR between 2016 and 2021 were enrolled in the study. Five different breast volume assessments, including automatic segmentation of MRI, manual segmentation of MRI, 3D surface imaging, mammography, and the BREAST-V formula, were used to evaluate different breast volumes. The results were validated using water displacement volumes of the mastectomy specimens. Results In this pilot study, a total of 50 female patients met the inclusion criteria and contributed 54 breast specimens to volumetric analysis. There was a strong linear association between the MRI methods and the water displacement methods (automatic segmentation: r=0.911, p<0.001; manual segmentation: r=0.924, p<0.001), followed by 3D surfacing imaging (r=0.858, p<0.001), mammography (r=0.841, p<0.001), and the Breast-V formula method (r=0.838, p<0.001). Breast volumes measured by automatic and manual segmentation of MRI had lower mean relative errors (30.3%+/-22.0 and 28.9%+/-19.8 respectively) than 3D surface imaging (38.9%+/-31.2), Breast-V formula (44.8%+/-25.8) and mammography (60.3%+/-37.6). Conclusion Breast volume assessment using the MRI methods had better accuracy and reliability than the other methods in our study. Breast volume measurement using automatic segmentation of MRI could be more efficient.

Abstract:In recent years, the demand for dental implants has surged, driven by their high success rates and esthetic advantages. However, accurate prediction of missing teeth for precise digital implant planning remains a challenge due to the intricate nature of dental structures and the variability in tooth loss patterns. This study presents a novel framework for accurate prediction of missing teeth in different patterns, facilitating digital implant planning. The proposed framework begins by estimating point-to-point correspondence among a dataset of dental mesh models reconstructed from CBCT images of healthy subjects. Subsequently, tooth dictionaries are constructed for each tooth type, encoding their position and shape information based on the established point-to-point correspondence. To predict missing teeth in a given dental mesh model, sparse coefficients are learned by sparsely representing adjacent teeth of the missing teeth using the corresponding tooth dictionaries. These coefficients are then applied to the dictionaries of the missing teeth to generate accurate predictions of their positions and shapes. The evaluation results on real subjects shows that our proposed framework achieves an average prediction error of 1.04mm for predictions of single missing tooth and an average prediction error of 1.33mm for the prediction of 14 missing teeth, which demonstrates its capability of accurately predicting missing teeth in various patterns. By accurately predicting missing teeth, dental professionals can improve the planning and placement of dental implants, leading to better esthetic and functional outcomes for patients undergoing dental implant procedures.
Evaluation of the 3D Augmented Reality-Guided Intraoperative Positioning of Dental Implants in Edentulous Mandibular Models

Abstract:PURPOSE:This research aimed to propose a three-dimensional (3D) augmented reality navigation method with point cloud-based image-patient registration that could merge virtual images in the real environment for dental implants using a 3D image overlay and to evaluate its feasibility.MATERIALS AND METHODS:A total of 12 rapid prototyping mandibular models were fabricated using a 3D printing method and were divided into two groups: 3D augmented reality-guided group and traditional two-dimensional (2D) image-guided group. A point cloud-based preoperative image-to-patient registration method was introduced to replace the traditional point-to-point registration. After the registration, dental implant surgery was performed in the two model groups using an augmented reality-guided navigation method and a traditional two-dimensional image-guided navigation method. The planned and actual postoperative implant positions were compared for measuring positional implantation errors. The surgery time was also recorded and compared between the two groups.RESULTS:In the model experiment, the root-mean-square deviation of registration was 0.54 mm, and the implant surgery results showed < 1.5-mm mean linear deviation and < 5.5-degree angular deviation. The augmented reality-guided implantation showed smaller horizontal, vertical, and angular errors in the apical areas of the central incisor and the canine region. The surgery time using the augmented reality-guided navigation method was significantly shorter than that using the two-dimensional (2D) image-guided navigation method (P < .05). Moreover, the volunteer experiment demonstrated that the preoperative 3D models in situ accurately overlaid onto the surgical site.CONCLUSION:The proposed point cloud-based registration method can achieve excellent registration accuracy. Dental implant placement guided by the proposed 3D augmented reality navigation method showed better accuracy and applicability, as well as higher efficiency, than the traditional 2D image navigation method.

Abstract:The use of quantitative medical imaging data to initialize and constrain mechanistic mathematical models of tumor growth has demonstrated a compelling strategy for predicting therapeutic response. More specifically, we have demonstrated a data-driven framework for prediction of residual tumor burden following neoadjuvant therapy in breast cancer that uses a biophysical mathematical model combining reaction-diffusion growth/therapy dynamics and biomechanical effects driven by early time point imaging data. Whereas early work had been based on a limited dimensionality reduction (two-dimensional planar modeling analysis) to simplify the numerical implementation, in this work, we extend our framework to a fully volumetric, three-dimensional biophysical mathematical modeling approach in which parameter estimates are generated by an inverse problem based on the adjoint state method for numerical efficiency. In an <i>in silico</i> performance study, we show accurate parameter estimation with error less than 3% as compared to ground truth. We apply the approach to patient data from a patient with pathological complete response and a patient with residual tumor burden and demonstrate technical feasibility and predictive potential with direct comparisons between imaging data observation and model predictions of tumor cellularity and volume. Comparisons to our previous two-dimensional modeling framework reflect enhanced model prediction of residual tumor burden through the inclusion of additional imaging slices of patient-specific data.

Digital three-dimensional visualization of intrabony periodontal defects for regenerative surgical treatment planning

Abstract:Background: In the regenerative treatment of intrabony periodontal defects, surgical strategies are primarily determined by defect morphologies. In certain cases, however, direct clinical measurements and intraoral radiographs do not provide sufficient information on defect morphologies. Therefore, the application of cone-beam computed tomography (CBCT) has been proposed in specific cases. 3D virtual models reconstructed with automatic thresholding algorithms have already been used for diagnostic purposes. The aim of this study was to utilize 3D virtual models, generated with a semi-automatic segmentation method, for the treatment planning of minimally invasive periodontal surgeries and to evaluate the accuracy of the virtual models, by comparing digital measurements to direct intrasurgical measurements. Methods: Four patients with a total of six intrabony periodontal defects were enrolled in the present study. Two months following initial periodontal treatment, a CBCT scan was taken. The novel semi-automatic segmentation method was performed in an open-source medical image processing software (3D Slicer) to acquire virtual 3D models of alveolar and dental structures. Intrasurgical and digital measurements were taken, and results were compared to validate the accuracy of the digital models. Defect characteristics were determined prior to surgery with conventional diagnostic methods and 3D virtual models. Diagnostic assessments were compared to the actual defect morphology during surgery. Results: Differences between intrasurgical and digital measurements in depth and width of intrabony components of periodontal defects averaged 0.31 ± 0.21 mm and 0.41 ± 0.44 mm, respectively. In five out of six cases, defect characteristics could not be assessed precisely with direct clinical measurements and intraoral radiographs. 3D models generated with the presented semi-automatic segmentation method depicted the defect characteristics correctly in all six cases. Conclusion: It can be concluded that 3D virtual models acquired with the described semi-automatic segmentation method provide accurate information on intrabony periodontal defect morphologies, thus influencing the treatment strategy. Within the limitations of this study, models were found to be accurate; however, further investigation with a standardized validation process on a large number of participants has to be conducted.
Evaluation of Incision Margin and Radiotherapy Adjustment in Breast Cancer-Conserving Surgery Based on VR Pathology 3D Reconstruction

Abstract:At present, the incidence of breast cancer is high worldwide and the most effective way to treat breast cancer is surgery. This study mainly explores the application of pathological three-dimensional reconstruction based on VR technology in the evaluation of the margins of breast-conserving surgery and the adjustment of radiotherapy for breast cancer. In the process of making pathological large slices, the slice markers are counted and the upper and outer cutting edges are located and placed in a large embedding box to prevent tissue deformation. ACDSEE6.0 is used to convert the aligned image BMP format into JPG format, the JPG images of each breast are cropped in batches under the same range and size, and then 3DDOCTOR1.0 is imported to edit the images. The boundary edit tool is activated to segment the target area layer by layer to create a boundary data set of the target structure. The stored file is opened and 3D rendering simple surface and volume rendering on the computer is run to display the surface data 3D display and the volume data 3D display. During VR image processing, the data are imported into the VR image workstation and the Radio Dexter software system is used for image adjustment, trimming, and sequence extraction. Then, the preliminary processed CT and MRI data are displayed in pairs. Through multiplane comparison, manual or automatic fusion tools are used to reduce the gap between image sequences. The surgical process is simulated under VR: by changing the window size and viewing angle; using virtual electric drills, cutting tools, and other virtual instruments; ad selecting the operating microscope perspective; and simulating steps such as breast opening, visual field exposure, and tumor resection. The breast tomographic image was reconstructed in multiple planes, and two-dimensional images of any plane could be observed. 10 cases showed high-density shadows and 5 cases showed isodensity shadows. This research contributes to the precise treatment of breast-conserving surgery.

Postoperative facial prediction for mandibular defect based on surface mesh deformation

Abstract:Objectives: This study aims to introduce a novel predictive model for the post-operative facial contours of patients with mandibular defect, addressing limitations in current methodologies that fail to preserve geometric features and lack interpretability. Methods: Utilizing surface mesh theory and deep learning, our model diverges from traditional point cloud approaches by employing surface triangular mesh grids. We extract latent variables using a Mesh Convolutional Restricted Boltzmann Machines (MCRBM) model to generate a three-dimensional deformation field, aiming to enhance geometric information preservation and interpretability. Results: Experimental evaluations of our model demonstrate a prediction accuracy of 91.2 %, which represents a significant improvement over traditional machine learning-based methods. Conclusions: The proposed model offers a promising new tool for pre-operative planning in oral and maxillofacial surgery. It significantly enhances the accuracy of post-operative facial contour predictions for mandibular defect reconstructions, providing substantial advancements over previous approaches.

Abstract:Background and Aim:The goal of this study was to analyze the validity and prediction accuracy of a newly-developed procedure for three-dimensional soft tissue prediction based on Finite Element Method, and to compare the results with prediction produced using an existing two-dimensional prediction program (Dentofacial Planner Plus™).Patients and Methods:In twelve patients who underwent combined surgical-orthodontic treatment, profile prediction was generated using both procedures preoperatively and then compared at predefined measurement points with the patient’s actual postoperative soft tissue status.Results:The deviations observed depended on the facial region, whereby the prediction errors for both procedures were much greater in the lower facial third than in the midfacial third. Calculating in all the measurement points, the mean horizontal prediction error was 0.32 mm for the Finite Element Method and 0.75 mm for the Dentofacial Planner Plus™. Overall, we were able to demonstrate the new procedure’s superior validity and quality of visualization. In addition to profile prediction, the procedure allows a differentiated three-dimensional assessment of esthetically important regions such as the cheeks, nasolabial folds and the nasal wings. Additional X-radiation is not necessary in this risk-free and stress-free procedure.Conclusion:Three-dimensional soft tissue prediction employing finite element modeling is a useful aid for implementing esthetically-optimized treatment planning.
