Abstract:Objective: The aim of the present study was to evaluate the effectiveness and safety of nicorandil in improving the area of myocardial infarction in patients with acute myocardial infarction (AMI). Methods: One hundred and twenty patients with acute ST-segment elevation myocardial infarction (STEMI) admitted to our hospital between December 1, 2018 and December 31, 2019 were selected and randomly allocated to the experimental group (group A, n = 60) and the control group (group B, n = 60). In the experimental group, an infusion of nicorandil was given intravenously before the first balloon dilation or 1 minute before the stent placement, and with the completion of the infusion, nicorandil maintenance infusion was given. In the control group, only balloon dilation and stent placement were undertaken. Results: The postoperative peak levels of myoglobin, creatine kinase isoform and hypersensitive troponin T were significantly lower in group A than in group B (p < 0.05). Moreover, the left ventricular ejection fraction (LVEF) on the 180th day post operation was substantially greater in group A than in group B (p < 0.01), and the area of myocardial infarction was significantly smaller in patients in group A than those in group B on the 180th day post operation (p < 0.01). In terms of the safety, there were no statistically significant differences in the incidence of slow flow/no reflow, malignant arrhythmias, and hypotension within 24 hours post operation between the two groups (p > 0.05), and no major adverse cardiovascular event (MACE) occurred in either group during the postoperative follow-up period of 180 days (p > 0.05). Conclusion: Intravenous administration of nicorandil in patients with STEMI during the perioperative percutaneous coronary intervention (PCI) period was effective in reducing the area of myocardial infarction and myocardial injury without increasing the incidence of malignant arrhythmias, hypotension, or composite cardiovascular events during the drug administration period.
Mild volume acute normovolemic hemodilution is associated with lower intraoperative transfusion and postoperative pulmonary infection in patients undergoing cardiac surgery -- a retrospective, propensity matching study

Abstract:Background Perioperative allogenic transfusion is required in almost 50% of patients undergoing cardiac surgery and is associated with higher risk of mortality and morbidity (Xue et al., Lancet 387:1905, 2016; Ferraris et al., Ann Thorac Surg 91:944–82, 2011). Acute normovolemic hemodilution (ANH) is recommended as a potential strategy during cardiac surgery, but the blood conservation effect and the degree of ANH was still controversial. There is also an increasing concern about the improved outcomes associated with ANH. Therefore, a better understanding of the effect of mild volume ANH during cardiac surgery is urgently needed. Methods This retrospective study included 2058 patients who underwent cardiac surgery between 2010 and 2015. The study population was split into two groups (with and without mild volume ANH). Propensity score adjustment analysis was applied. We reported the association between the use of mild volume ANH and perioperative outcomes. Results A total of 1289 patients were identified. ANH was performed in 358 patients, and the remaining 931 patients did not receive any ANH. Five hundred of the total patients (38.8%) received perioperative RBC transfusions, 10% (129/1289) of patients received platelet, and 56.4% (727/1289) of patients received fresh frozen plasma transfusions. Mild volume ANH administration was significantly associated with decreased intraoperative RBC transfuse rate (8.5% vs. 14.4%; p = 0.013), number of RBC units ( p = 0.019), and decreased postoperative pulmonary infection (6.8 vs. 11.3%; p = 0.036) during cardiac surgery. However, there was no significant difference regarding intraoperative fresh frozen plasma (FFP) and platelet concentrate transfusions, as well as postoperative and total perioperative allogeneic transfusions. Furthermore, there was no significant difference regarding postoperative outcomes including mortality, prolonged wound healing, stroke, atrial fibrillation, reoperation for postoperative bleeding and acute kidney injury. There was also no difference in postoperative ventilation time, length of ICU and hospital stay. Conclusion Based on the 5-year experience of mild volume ANH in cardiac surgeries with CPB in our large retrospective cohort, mild volume ANH was associated with decreased intraoperative RBC transfusion and postoperative pulmonary infection in Chinese patients undergoing cardiac surgery. However, there was no significant difference regarding postoperative and total perioperative allogeneic transfusions.

Abstract:BACKGROUND:The efficacy and safety of nicorandil were evaluated in Chinese patients with stable angina pectoris (AP) in a double-blind, multicenter, active-controlled, randomized clinical trial.METHODS AND RESULTS:After a 2-week washout period, 232 patients with stable AP were randomized to receive either nicorandil (5 mg tid; 115 patients) or isosorbide mononitrate (ISMN: 20 mg bid; 117 patients) for 2 weeks. Exercise capacity, number of weekly anginal attacks, nitroglycerin (NTG) consumption, and safety were evaluated. Nicorandil and ISMN significantly prolonged the time to 1 mm ST-segment depression in an exercise tolerance test. Both drugs improved the total exercise time and the time to onset of chest pain. There was no significant difference between the 2 groups. Nicorandil significantly decreased the number of anginal attacks and NTG consumption. ISMN decreased the number of anginal attacks significantly; however, there was no significance in NTG consumption, and the ratio of anginal attack reduction was at least 50% was significantly higher with nicorandil. Nicorandil was well tolerated and there was no safety profile difference compared with ISMN. Thus, nicorandil may have equivalent or better antianginal effect than ISMN.CONCLUSIONS:Nicorandil is beneficial as treatment for AP.

Abstract:Introduction: Postoperative pulmonary complications (PPCs) are common and associated with increased morbidity, mortality, and medical cost. They are gaining increasing concerns among patients receiving neurological surgery. Chronic obstructive pulmonary disease (COPD) affect a large section of whole population and is also one of the risk factors of PPCs in the perioperative setting. Ipratropium bromide is the inhalation solution for the treatment of COPD. Studies showed the perioperative nebulization of ipratropium bromide could increase the lung function and decrease the incidence of postoperative pneumonia in COPD patients underwent thoracic surgery. The purpose of this study is to investigate the effect of perioperative nebulization of ipratropium bromide on PPCs in COPD patients underwent neurosurgical surgery. Methods and analysis: This study is a multicenter retrospective study in China. Patients who meet the inclusion/exclusion criteria are selected from 7 neurosurgical centers in China. According to whether ipratropium bromide is used in perioperative period, the patients are divided into exposure group and control group. The primary outcome is the incidence of postoperative pneumonia. Secondary outcomes are unplanned intubation, postoperative mechanical ventilation >= 48 hours, respiratory failure, atelectasis, death, and length of stay. Ethics and dissemination: This study was approved by the ethics committee (EC) of the School of Public Health, Fudan University, Shanghai, China. Waived by the ethics committee, no written consent form was obtained since we used the registry data. The study results will be communicated via publication.
Perioperative myocardial injury in patients with coronary artery disease during elective lower limb surgery

Abstract:Aim. To assess the prevalence of ischemic myocardial injury and the cardioprotective effect of nicorandil by assessing high-sensitivity cardiac troponin (hs-cTn) in patients with stable coronary artery disease (CAD) during elective lower limb surgery, as well as to identify predictors of adverse cardiac events. Material and methods. The study included 70 patients with stable coronary artery disease hospitalized for elective autogenous femoropopliteal bypass (FPB) surgery. After randomization, all patients were divided into two following groups: control group — 35 patients; main group — 35 patients, who, in addition to standard therapy, were prescribed nicorandil (Cordinic, PIQ-PHARMA) in a single dose of 20 mg 2 hours before surgery. In the postoperative period, the incidence of myocardial injury was assessed by hs-cTn increase. The obtained primary data were subjected to mathematical processing using the R-Studio software package (R language). Results. At baseline, patients in both groups were comparable in clinical characteristics, therapy, and duration of vascular surgery. In the main group of patients receiving nicorandil, a significant decrease in the incidence of perioperative myocardial injury was noted. In 5 patients of the control group, hs-cTn level 24 hours after surgery exceeded the threshold value, which indicated myocardial injury in the early postoperative period. In the nicorandil group, there was no hs-cTn increase (14% vs 0%, p=0,027). Regression analysis identified a predictor of perioperative myocardial injury — left ventricular ejection fraction (LVEF) <50%. LVEF <50% increases the risk of myocardial injury in the early postoperative period by 7,36 times (p=0,04) and 9,15 times (p=0,048) according to univariate and multivariate regression analysis, respectively. Conclusion. Perioperative myocardial injury is a common complication in patients with CAD undergoing lower extremity revascularization. The use of nicorandil (Cordinic, PIQ-PHARMA) before elective revascularization surgery helps reduce the incidence of ischemic myocardial injury in the early postoperative period and is not accompanied by adverse reactions, which rationales this approach to improve the clinical outcomes of lower extremity revascularization in patients with stable CAD. A predictor of myocardial injury, determined by hs-cTn, is a LVEF <50%.

Abstract:Monomers, dimers, trimers, dendrimers and oligomers of metalloporphyrins form supramolecular complexes with fullerene derivatives via electrostatic interactions, π-π interactions and coordination bonds. Photoexcitation of the supramolecular complexes resulted in photoinduced electron transfer from the porphyrin moiety to the fullerene moiety to produce the charge-separated states as revealed by laser flash photolysis measurements. The rate constants of photoinduced charge separation and charge recombination in supramolecular complexes of multi-metalloporphyrins and fullerenes were also determined by laser flash photolysis measurements and the results depending on the number of porphyrins in the supramolecular complexes are discussed in terms of efficiency of photoinduced energy transfer and charge separation as well as the lifetimes of charge-separated states. The photoelectrochemical performances of solar cells composed of supramolecular complexes of monomers, dimers, dendrimers and oligomers of metalloporphyrins with fullerenes are compared in relation to the rate constants of photoinduced charge separation and charge recombination.
Incidence and Severity of Acute Myocardial Injury after Thoracic Surgery: Effects of Nicorandil

Abstract:Aim. To study the perioperative dynamics of myocardial injury biomarkers high-sensitivity cardiac troponin I (hs-cTnI), ischemia-modified albumin (IMA) and soluble ST2 (sST2) when taking nicorandil in lung cancer patients with concomitant coronary heart disease (CHD) undergoing surgical lung resection. Material and methods. The study included 54 patients (11 women and 43 men) with non-small cell lung cancer and concomitant stable CHD who underwent lung resection in the volume of lobectomy or pneumonectomy. Patients were randomly assigned to the nicorandil group (oral administration 10 mg BID for 7 days before and 3 days after surgery; n=27) and the control group (n=27). In the study groups, the perioperative dynamics of hscTnI, IMA and sST2, determined in the blood before and 24 and 48h after surgery, were compared. We calculated the incidence of acute myocardial injury in the groups, which was diagnosed in cases of postoperative hs-cTnI increase of more than one 99 th percentile of the upper reference limit. The associations of nicorandil intake and acute myocardial injury were evaluated. Results. The groups were comparable in gender, age, basic clinical characteristics, as well as baseline levels of myocardial injury biomarkers. After the intervention, both samples showed an increase in the hs-cTnI and sST2 levels and a decrease in IMA concentration (all p<0.02 for related group differences). In the nicorandil group, in comparison with the control one, 48h after surgery, we found lower mean levels of hs-cTnI [16.7 (11.9;39.7) vs 44.3 (15.0;130.7) ng/l; p<0.05) and sST2 [62.8 (43.6;70.1) vs 76.5 (50.2;87.1) ng/ml; p<0.05), concentration increase rates of hs-cTnI [14.8 (0.7;42.2) vs 32.5 (14.0;125.0) ng/l; p<0.01) and sST2 [24.4 (10.3;42.4) vs 47.4 (17.5;65.3) ng/ml; p<0.05), as well as highest concentrations for the entire postoperative period of hs-cTnI [30.7 (12.0;53.7) vs 79.0 (20.3;203.3) ng/L, p<0.01] and sST2 [99.8 (73.6;162.5) vs 147.8 (87.8;207.7) ng/mL; p<0.05]. The serum IMA decreased when taking nicorandil to a greater extent [-8.0 (-12.6; -2.0) vs -2.7 (-6.0; +5.5) ng/ ml; p<0.01] 24h after surgery. Acute myocardial injury was diagnosed in 7 people in the nicorandil group (25.9%) and in 15 in the control one (55.6%; pχ 2 =0.027). The adjusted odds ratio of acute myocardial injury when taking nicorandil was 0.35 (95% confidence interval 0.15-0.83, p=0.017). Conclusion. Taking nicorandil in patients with lung cancer and concomitant CHD who underwent surgical lung resection is associated with a lower postoperative increase in hs-cTnI and sST2 and a reduced risk of acute myocardial injury, which may indicate the cardioprotective effect of nicorandil under acute surgical stress conditions.
Effect of Remote Ischemic Preconditioning on Outcomes in Adult Cardiac Surgery: A Systematic Review and Meta-analysis of Randomized Controlled Studies

Abstract:BACKGROUND: Remote ischemic preconditioning (RIPC) has been demonstrated to prevent organ dysfunction in cardiac surgery patients. However, recent large, prospective, multicenter, randomized controlled trials (RCTs) had controversial results. Thus, a meta-analysis of RCTs was performed to investigate whether RIPC can reduce the incidence of acute myocardial infarction (AMI), acute kidney injury (AKI), and mortality in adult cardiac surgery patients. METHODS: Study data were collected from Medline, Elsevier, Cochrane Central Register of Controlled Trials and Web of Science databases. RCTs involving the effect of RIPC on organ protection in cardiac surgery patients, which reported the concentration or total release of creatine kinase-myocardial band, troponin I/troponin T (TNI/TNT) after operation, or the incidence of AMI, AKI, or mortality, were selected. Two reviewers independently extracted data using a standardized data extraction protocol where TNI or TNT concentrations; total TNI released after cardiac surgery; and the incidence of AKI, AMI, and mortality were recorded. Review Manager 5.3 software was used to analyze the data. RESULTS: Thirty trials, including 7036 patients were included in the analyses. RIPC significantly decreased the concentration of TNI/TNT (standard mean difference [SMD], −0.25 ng/mL; 95% confidence interval [CI], −0.41 to −0.048 ng/mL; P = .004), creatine kinase-myocardial band (SMD, −0.22; 95% CI, −0.07–0.35 ng/mL; P = .46), and the total TNI/TNT release (SMD, −0.49 ng/mL; 95% CI, −0.93 to −0.55 ng/mL; P = .03) in cardiac surgery patients after a procedure. However, RIPC could not reduce the incidence of AMI (relative risk, 0.89; 95% CI, 0.70–1.13; P = .34) and AKI (relative risk, 0.88; 95% CI, 0.72–1.06; P = .18), and there was also no effect of RIPC on mortality in adult cardiac surgery patients. Interestingly, subgroup analysis showed that RIPC reduced incidence of AKI and mortality of cardiac surgery patients who received volatile agent anesthesia. CONCLUSIONS: Our meta-analysis demonstrated that RIPC reduced TNI/TNT release after cardiac surgery. RIPC did not significantly reduce the incidence of AKI, AMI, and mortality. However, RIPC could reduce mortality in patients receiving volatile inhalational agent anesthesia.
Protective Effect of Nicorandil on Contrast-Induced Acute Kidney Injury After Emergency Percutaneous Coronary Intervention

Abstract:Objective: To investigate the protective effect of nicorandil on contrast-induced acute kidney injury (CIAKI) in patients with acute ST-segment elevation myocardial infarction (STEMI) after emergency percutaneous coronary intervention (PCI). Methods: This is a single-center, retrospective control study. A total of 156 patients with STEMI were divided into the nicorandil group (n = 55) and the control group (n = 101). The incidence of CIAKI, defined as an increase of >25% or absolute values > 44.2 μmol/L in serum creatinine (Scr) from baseline within 72 h of exposure to a contrast agent after exclusion of other causes, was the primary endpoint. The secondary endpoints were: (1) changes of Scr, estimated glomerular filtration rate (eGFR), uric acid, and β2-microglobulin at 24/48/72 h and 5 to 7 days after PCI; (2) the peak value difference of creatine kinase isoenzymes (CK-MB) after PCI; (3) adverse events within 6 months after PCI. Results: The overall incidence of CIAKI was 21.8%; the incidence of CIAKI in the nicorandil group was significantly lower (12.7% [7/55]) than in the control group (26.7% [27/101]) (P = .043). Compared with the control group, Scr, uric acid, and β2-microglobulin levels were lower, and the level of eGFR was higher in nicorandil group (P all < .05). The peak value of CK-MB in the nicorandil group was lower than that in the control group (105.30 [56.61, 232.04] vs 178.00 [77.08, 271.91]U/L, P = .042). There was no significant difference in adverse events between the 2 groups within 6 months after PCI. Moreover, multivariate logistic regression analysis showed that hypertension and diabetes were independent risk factors for CIAKI, while nicorandil treatment was a protective factor. Conclusion: Our data suggest that intravenous nicorandil after emergency PCI has a protective effect on the occurrence of CIAKI in STEMI patients.

Abstract:Objectives: To assess cardiac troponin I and creatine kinase-myocardial band levels, electrocardiogram changes and major adverse cardiac events after treatment with nicorandil before primary percutaneous coronary intervention. Methods: The comparative, analytical study was conducted from October to November 2022 at the Pharmacology Department of Army Medical College, National University of Medical Sciences, Rawalpindi, Pakistan, in collaboration with the Rawalpindi Institute of Cardiology, Rawalpindi. The sample comprised ST-elevated myocardial infarction patients of either gender aged at least 30 years with an ejection fraction of at least 35% undergoing primary percutaneous coronary intervention. Participants were selected based on the above-mentioned inclusion and informed consent was taken before their enrolment in this research study. The sample was randomised into control group A receiving conventional acute coronary syndrome treatment, and intervention group B receiving nicorandil in addition to the conventional treatment. Cardiac troponin I and creatine kinase-myocardial band levels, electrocardiogram changes, and major adverse cardiac events noted and compared. Data was analysed using SPSS 26. Results: Of the 140 patients, 70(50%) were in each of the 2 groups. In group B, 60(85.7%) patients achieved a completely settled ST segment on electrocardiogram compared to 25(35.7%) in group A (p=0.001). There was a significant inter-group differences with respect to cardiac troponin I value 6 hours after percutaneous coronary intervention and major adverse cardiac events (p<0.05), but creatine kinase-myocardial band level was no significantly different between the groups (p=0.761). Conclusion: Prophylactic use of nicorandil in ST-elevated myocardial infarction patients decreased the incidence of reperfusion injury. Efficacy of Oral Nicorandil to Prevent Contrast-Induced Nephropathy in Patients with Chronic Renal Dysfunction Undergoing an Elective Coronary Procedure

Abstract:Objectives: This prospective, randomized study was to investigate the role of nicorandil in the prevention of contrast-induced nephropathy (CIN) in patients with chronic renal dysfunction undergoing an elective coronary procedure. Methods: A total of 252 eligible patients were enrolled in this study and allocated into the control group (n = 125) or nicorandil group (n = 127). Both groups received the standard hydration treatment, and patients in the nicorandil group were orally administrated 10 mg of nicorandil (t.i.d.) beginning 2 days before and continuing for 2 days after an elective coronary procedure. Serum creatinine (SCr) and cystatin C (CysC) were measured at 24 h before and 24, 48, and 72 h after the procedure. The occurrences of CIN and adverse events within 1 year were recorded. Results: The nicorandil group had relatively lower SCr and CysC levels and a higher eGFR at 24 and 48 h after the procedure than the control group (p < 0.05). The incidence of CIN was significantly decreased in the nicorandil group compared to the control group. The multivariate logistic regression model revealed that nicorandil treatment was an independent protective factor for CIN (OR 0.669, 95% CI 0.522–0.857, p = 0.001). The multivariate COX proportional hazard model showed that nicorandil treatment was an independent protective predictor for adverse events (HR 0.881, 95% CI 0.781–0.993, p = 0.037). Conclusions: Nicorandil could exhibit a protective effect against CIN in patients with chronic renal dysfunction undergoing an elective coronary procedure and reduce the adverse events within 1 year after the procedure, which is superior to hydration treatment only.

Abstract:We investigated the preventive effect of nicorandil on contrast-induced nephropathy (CIN) in patients with moderate renal insufficiency undergoing percutaneous coronary intervention (PCI). A total of 250 patients with a creatinine clearance (crCl) ≤60 mL/min undergoing PCI were randomly assigned to either a nicorandil group (nicorandil 10 mg 3 times/d and hydration; n = 125) or a control group (hydration only; n = 125). The first end point was the incidence of CIN defined as an increase in serum creatinine (Scr) levels by ≥0.5 mg/dL or ≥25% within 72 hours after exposure to the contrast medium. The secondary end points were (1) changes in Scr, blood urea nitrogen, and crCl and (2) the incidence of major adverse events during hospitalization. The incidence of CIN was 1.6% (2/125) in the nicorandil group and 9.6% (12/125) in the control group ( P = .011). There was no obvious difference in the incidence of major adverse events during hospitalization between the nicorandil and the control group (4.0% vs 4.8%, P = 1.000). Multivariate logistic regression analysis showed that nicorandil was a protective factor for CIN (odds ratios = 0.126, 95% confidence interval: −19.996 to −0.932, P = .012). Prophylactic administration of nicorandil may prevent against CIN in patients with moderate renal insufficiency undergoing PCI.

Abstract:Abstract Background: Contrast-induced nephropathy (CIN) is a potentially serious complication of intravenous or intra-arterial contrast administration during angiographic procedures that results in renal dysfunction. This meta-analysis assesses the efficacy and safety of nicorandil for the prevention of CIN in patients undergoing percutaneous coronary intervention (PCI) or coronary angiography (CAG). Methods: Cochrane Central Register of Controlled Trials, MEDLINE, Embase, and ClinicalTrials.gov were used to perform a thorough literature search from their inception to July 2024. A random-effects meta-analysis was performed on RevMan and pooled estimates were presented as forest plots. The Mantel-Haenszel method was used for dichotomous outcomes and risk ratios (RRs) were calculated along with 95% confidence intervals (95% CI). Results: This meta-analysis included 12 RCTs consisting of 2787 participants (nicorandil: 1418, control: 1394). The use of nicorandil was protective against CIN (RR 0.38, 95% CI 0.29-0.50). The incidence of major adverse events was comparable in both groups (RR 0.77, 95% CI 0.52-1.13, p=0.18). Similarly, the use of nicorandil did not affect the risk of developing stroke (RR 1.05), myocardial infarction (RR 0.90), heart failure (RR 0.81), cardiac death (RR 0.90), and dialysis (RR 0.70). Conclusion: This study revealed that nicorandil effectively reduced the risk of developing CIN in patients undergoing angiographic procedures like PCI or coronary angiography. However, more RCTs should be conducted for a more definitive conclusion.
The effects of nicorandil on microvascular function in patients with ST segment elevation myocardial infarction undergoing primary PCI

Abstract:Background: Nicorandil, as a selective potassium channel opener, has dual action including coronary and peripheral vasodilatation and cardioprotective effect through ischemic preconditioning. Considering those characteristics, nicorandil was suggested to reduce the degree of microvascular dysfunction. Methods: Thirty-two patients with ST-elevation myocardial infarction undergoing primary percutaneous coronary intervention (pPCI) were included in the study. Index of microvascular resistance (IMR) was measured in all patients immediatelly after pPCI before the after administration of Nicorandil. ST segment resolution was monitored before intervention and 60 min after terminating the procedure. Echocardiographic evaluation of myocardial function and transthoracic Doppler derived Coronary flow reserve (CFR) of infarct related artery (IRA) was performed during hospitalization and 3 months later. Results: IMR was significantly lower after administration of Nicorandil (9.9 ± 3.7 vs. 14.1 ± 5.1, p < 0.001). There was significant difference in ST segment elevation before and after primary PCI with administration of Nicorandil (6.9 ± 3.7 mm vs. 1.6 ± 1.6 mm, p < 0.001). Transthoracic Doppler CFR measurement improved after 3 months (2.69 ± 0.38 vs. 2.92 ± 0.54, p = 0.021), as well as WMSI (1.14 ± 0.17 vs. 1.07 ± 0.09, p = 0.004). Conclusion: Intracoronary Nicorandil administration after primary PCI significantly decreases IMR, resulting in improved CFR and ventricular function in patients with STEMI undergoing primary PCI.

Abstract:In recent years, the development of thrombolysis and percutaneous coronary intervention (PCI) have positive impacts on outcome of the patients with acute myocardial infarction (AMI), but the reperfusion injury followed by these treatments per se would lower the benefit. After carrying on a large amount of research, people have found that ischaemic preconditioning can decrease the reperfusion injury and reduce the infarct size, which is a strong endogenous myocardial protective form of reducing reperfusion injury. But it must be operated in a transient time before myocardial ischaemia, which is the bottleneck of its wide clinical application. But recent research has found that ischaemic postconditioning can exert cardioprotective effect as well when operated after a short period of myocardial ischaemia, which has become a hotspot of coronary heart disease therapy researches. Since the mitochondria potassium ATP (mito-KATP) channel is at the downstream of signal pathway of preconditioning and postconditioning, the activation of mito-KATP channel can directly induce preconditioning and postconditioning. Nicorandil is a medicine which can act as agonist of mito-KATP channel and has nitrates effect. The pharmacologic preconditioning caused by Nicorandil has been confirmed in animal experiments and clinical tests. By activating mito-KATP channel, Nicorandil can cause pharmacologic postconditioning, theoretically. However, at present there are only a few animal experiments validating the postconditioning property of nicorandil. No further clinical experiments develops this topic. To test whether nicorandil can induce pharmacologic postconditioning or not, and to test whether it can reduce the infarct size of the acute anterior myocardial infarction, which achieve the result of limiting the injury of reperfusion. According to the criteria of inclusion and exclusion, the final experimental subjects are 43 patients with acute anterior myocardial infarction. All of them were treated in the emergency department of Zhongshan People's Hospital during the period of March 2011 to January 2012. They were randomly divided into two groups. There were 21 patients in group NIC and 22 in group ISDN. Patients in NIC group were taken 10 mg Nicorandil (NIC) orally before PCI procedure and treated with 5 mg Nicorandil three times a day for 7 days. Patients in ISDN group were given Isosorbide Dinitrate (ISDN) in the same method. By observing the changes of ECG ST-segment before operation and 1 h after operation, the ST-segment resolution rate was gained; and the blood samples were taken from all patients before operation and 6 h, 12 h, 18 h, 24 h, 48 h, 72 h after operation, which are used to observe the dynamic changes of CKMB. At last, the ECT myocardial perfusion examinations were performed in the patients on the 7th day after operation, which can reflect the infarct size of the acute anterior myocardial infarction. Compared with the ISDN, nicorandil can considerably reduce the peak value of creatine kinase myocardial band. NIC group (171.6±105.7 U/l) vs ISDN group (246.4±108.4 U/l), p<0.05. They can also reduce the area under the curve. NIC group (4309±2575) vs ISDN group (5854±2352), p<0.05. In addition, nicorandil can reduce the proportion of ECT myocardial perfusion defect extent in left ventricle: NIC Group (30.38±11.83%) vs ISDN group (38.64±14.59%), p<0.05. And ECT perfusion defect score is: NIC Group (25.19±11.96) vs ISDN group (33.27±14.03), p<0.05. Moreover, NIC group had a better ECG ST-segment resolution. There are 18 patients in NIC group and 13 patients in ISDN group having experienced ST-segment completely resolution. The ST-segment resolution rate of the two groups are 85.71% and 59.09%, p<0.05, respectively. (1) Before giving reperfusion in the patients with acute anterior myocardial infarction, oral nicorandil treatment within 1 h can improve ECG ST-segment resolution, reduce the peak value of CKMB and the area under the curve of CKMB, and decrease the proportion of ECT myocardial perfusion defect extent in left ventricle and ECT perfusion defect score. (2) By activating the mito-KATP channel, nicorandil can induce pharmacologic postconditioning effect, limit the infarct size of the acute anterior myocardial infarction and decrease the reperfusion injury, which eventually form cardioprotective effects. Besides, this effect is no related to nitrate effect.

Protective Effect of Nicardipine Against Pulmonary Ischemia-Reperfusion Injury in Patients Undergoing Cardiac Valve Replacement

Abstract:Objective To study the influence of pulmonary ischemia-reperfusion (I/R) induced by cardio-pulmonary bypass (CPB) on pulmonary function and the preventive effect of nicardipine.Methods Sixteen patients scheduled for cardiac valve replacement were randomly divided into two groups : control group ( n = 8) and nicardipine group ( n = 8) . In nicardipine group nicardipine 0.02 mg kg-1 was given at the beginning of CPB; while in control group normal saline was given instead of nicardipine. All patients were operated upon under TIVA with large doses of fentanyl. Swan-Ganz catheter was placed via internal jugular vein after induction of anesthesia. Mean pulmonary arterial pressure (MPAP), pulmonary vascular resistance index (PVRI) and lung compliance were measured and calculated before CPB (T0 ), 5min after declamping of vena cava (T1 ), at tennination of CPB (T2) and at the end of operation (T3 ). At the same time points arterial and mixed venous blood samples were taken for determination of TNF-a, SOD and LPO concentrations and polymorphonuclear leukocyte (PMN) count, intrapulmonary PMN trapping (PMNa-PMNv) and blood gases and calculation of PaO2/FiO2 , P(A-2,)O2 difference and Qs/Qt. The vena cava cross-clamping time was defined as pulmonary ischemia time. Results (1) In control group MPAP, PVRI, PaO2/FiO2 and Qs/Qt were significantly deteriorating after vena cava declamping (T1-T3) as compared with the baseline valves (T0) (P 0.05 or 0.01) , while in nicardipine group no significant changes were seen and the difference between the two groups was significant ( P 0.05) . (2) In both groups TNF-a concentration and PMN count in arterial and mixed venous blood were significantly increased after vena cava declamping (T1-T3). (3) The intrapulmonary PMN trapping was increased at T, (5min after vena cava declamping) as compared with the baseline (TD) only in control group. (4) The LPO concentration in arterial and mixed venous blood was significantly increased after vena cava declamping in both groups, but the increase was smaller in nicardipine group. (5) The SOD activity in arterial and mixed venous blood was significantly decreased at T1-3 only in control group, while in nicardipine group no significant changes in SOD activity were seen after declamping and the difference between the two groups was significant at T2 and T3 . Conclusion Pulmonary I/R injury leads to pulmonary dysfunction during CPB and PMN activated by inflammation plays an important role. Nicardipine 0.02 mg kg-1 can protect the lung from damage caused by I/R injury.
Efficacy of oral nicorandil in patients with end-stage renal disease: a retrospective chart review after coronary angioplasty in Japanese patients receiving hemodialysis

Abstract:Background: Patients receiving hemodialysis for end-stage renal disease (ESRD) are at high risk for death from ischemic heart disease (IHD). Nicorandil, a hybrid compound of adenosine triphosphate-sensitive potassium channel opener and nitric oxide donor, has been reported to improve the clinical prognosis of patients with IHD. Objective: This study sought to investigate the efficacy of oral nicorandil in reducing the risks for cardiovascular events (CVEs) and CVE-related death in patients receiving hemodialysis for ESRD after undergoing percutaneous coronary intervention (PCI) for angina pectoris. Methods: For this retrospective chart review, we used data from telephone interviews and medical charts from 3 hospitals in Japan. Data from patients aged <80 years who were receiving hemodialysis for ESRD and who had undergone successful PCI for angina between January 1999 and December 2004 were included in the analysis. Patients were stratified based on status of nicorandil treatment before PCI, as follows: patients receiving nicorandil 5 mg PO TID (the recommended dosage in Japan) for >1 month before PCI (nicorandil group) or those who did not receive nicorandil (control group). We investigated 6-year follow-up data on the primary end point, defined as CVEs (ie, unplanned hospital admission for worsening anginal status, or CVE-related death). The secondary end point was CVE-related death. After the data were initially analyzed, we performed a propensity-matched analysis to minimize selection bias. Results: Data from 356 patients were included in the study (235 men, 121 women; mean [SD] age, 69 [9] years; mean [SD] weight, 52.3 [9.1] kg; nicorandil group, 198 patients; control group, 158 patients). According to the estimated propensity scores, 107 patients from each group were matched. There were no differences between the 2 groups in the baseline characteristics. On propensity-matched patient analysis, the estimated rates of patients who were CVE-free at 6 years were 33.5% in the nicorandil group and 21.8% in the control group on Kaplan-Meier analysis (hazard ratio [HR] = 0.53; 95% CI, 0.36-0.78; P < 0.002), and the rates of 6-year survival (ie, patients who did not experience CVE-related death) were 92.7% in the nicorandil group and 85.8% in the control group (HR = 0.27; 95% CI, 0.07-0.89; P = 0.047). Cox multivariate analysis found that nico-randil treatment status was an independent predictor of CVEs (HR = 0.40; 95% CI, 0.18-0.91; P = 0.028) and CVE-related death (HR = 0.38; 95% CI, 0.14-0.78; P = 0.030). Conclusion: Results obtained in this retrospective study suggest the potential efficacy of nicorandil treatment in improving clinical outcomes in patients with IHD receiving hemodialysis following PCI.
Effects of oral nicorandil on myocardial damage, left ventricular cardiac function, and adverse reactions in patients with slow/no-reflow phenomenon during PCI for acute ST-elevation myocardial infarction

Abstract:Objective To evaluate the effects of oral nicorandil after percutaneous coronary interventions (PCI) in reducing myocardial damage and improving left ventricular cardiac function as well as reducing adverse events in patients with slow/no-reflow phenomenon during PCI for acute ST-segment elevation myocardial infarction (STEMI).Methods Totally 110 patients with the slow/no-reflow phenomenon during PCI for STEMI were randomly classified into the observation group (n=55) and control group (n=55).Based on their conditions, patients in the two groups were given secondary coronary prevention drugs including aspirin, clopidogrel, angiotensin converting enzyme inhibitors, angiotensin receptor antagonists, β blockers, statins, ect.Moreover, patients in the observation group were administered oral nicorandil (5 mg, tid) for 30 days.The serum creatine kinase (CK) and creatine kinase isoenzyme (CK-MB) were detected by immunofluorescence at the admission of hospital, and 6, 14, 18, 24, and 48 h after PCI.The levels of plasma brain natriuretic peptide (BNP) were detected by immunoradiometric assay at the admission of hospital and 24 h, 72 h, 7 d and 30 d after PCI.Left ventricular end-diastolic diameter (LVEDD), left ventricular end-diastolic diameter (LVESD), and left ventricular ejection fraction (LVEF) were measured at the admission of hospital, and 7 d, 30 d after PCI.The incidence of malignant arrhythmia, severe heart failure, complications, death of acute myocardial infarction, new myocardial infarction, and target vessel revascularization (TVR) were recorded within 30 days after PCI.Results In the observation group, there were 28 patients (50.91%), 38 patients (69.09%), 54 patients (98.18%) with ECG-related lead elevation ST segment drop> 50% peak at immediate moment, 72 h, and 30 d after PCI, respectively, while in the control group, there were 24 patients (43.64%), 34 patients (61.82%), 49 cases (89.09%) with ECG-related lead elevation ST segment drop> 50% peak, respectively.The incidence of decrease in ECG related lead elevation ST segment drop> 50% peak at immediate moment, 72 h, 30 d after PCI was significantly higher in the observation group as compared with that of the control group (all P<0.05).The peak levels of CK and CK-MB after PCI within 24 h in the observation group and control group were statistically different between the two groups (1205 IU/L vs.1520 IU/L, 192 ug/L vs.238 ug/L, both P<0.05).BNP, LVEDD, and LVESD within 30 days after PCI were significantly lower in the observation group (P<0.05), whereas LVEF levels were higher than those of the control group (P<0.05).The above indicators within 7 days were not statistically different between the two groups (all P>0.05).Within 30 days after PCI, there were 12 (21.82%) of malignant arrhythmia, 8 (14.55%) of severe heart failure, 0 of complication for acute coronary syndrome, 0 of death, 5(9.09%) of new myocardial infarction, 0 of TVR in the control group, while there were 4 (7.27%) of malignant arrhythmia, 2 (3.64%) of severe heart failure, 0 of complication for acute coronary syndrome, 0 of death, 4(7.27%) of new myocardial infarction, 0 of TVR in the observation group.The incidence of malignant arrhythmia and serious heart failure within 30 days after PCI was significantly lower in the observation group than in the control group (both P<0.05).Conclusion For patients with slow/no-reflow phenomenon during PCI for STEMI, oral nicorandil may reduce myocardial damage, improve left ventricular cardiac function, and decrease the incidence of malignant arrhythmia and serious heart failure within 30 days after PCI.
Effects of nicorandil on PI3K/Akt signaling pathway and its anti-apoptotic mechanisms in coronary microembolization in rats

Abstract:Coronary microembolization (CME) is a common complication of percutaneous coronary intervention (PCI) for acute coronary syndrome. It leads to myocardial apoptosis and cardiac dysfunction. Nicorandil pretreatment can prevent PCI-related myocardial injury and reduce the incidence of no- or slow-reflow phenomena. This cardioprotective effect is probably attributable to the suppression of CME-induced cardiomyocyte apoptosis, but the specific mechanisms have not been clarified. We aimed to investigate the protective effects of nicorandil pretreatment on CME-induced myocardial injury and clarify the underlying mechanisms. <i>In vivo</i> studies, we used echocardiography, cardiac-enzymes measurement, hematoxylin-basic fuchsin-picric acid staining, TUNEL assay, and western blot, and found that CME significantly increased apoptotic cardiomyocytes in the infarct and peri-infarct areas in rats. The PI3K/Akt signaling pathway was involved in cardiomyocyte apoptosis. Nicorandil pretreatment given 7 days before CME effectively reduced cardiomyocyte apoptosis and myocardial injuries in rats, mainly through the activation of PI3K/Akt signaling. <i>In vitro</i> studies further showed that nicorandil reduced hypoxia-induced cardiomyocyte apoptosis and improved cardiomyocyte-survival rate. The PI3K-specific inhibitor LY294002 reduced these cardioprotective effects, indicating that they were attributable to the activation of the PI3K/Akt signaling pathway. In conclusion, nicorandil has significant cardioprotective effects in CME mainly through the activation of the PI3K/Akt signaling pathway and reduction of CME-induced cardiomyocyte apoptosis. Our findings may provide important support for the pre-PCI use of nicorandil to reduce post-PCI myocardial injuries.
