Abstract:Excessive forces may cause root resorption and insufficient forces would introduce no effect in orthodontics. The objective of this study was to investigate the optimal orthodontic forces on a maxillary canine, using hydrostatic stress and logarithmic strain of the periodontal ligament (PDL) as indicators. Finite element models of a maxillary canine and surrounding tissues were developed. Distal translation/tipping forces, labial translation/tipping forces, and extrusion forces ranging from 0 to 300 g (100 g=0.98 N) were applied to the canine, as well as the force moment around the canine long axis ranging from 0 to 300 g·mm. The stress/strain of the PDL was quantified by nonlinear finite element analysis, and an absolute stress range between 0.47 kPa (capillary pressure) and 12.8 kPa (80% of human systolic blood pressure) was considered to be optimal, whereas an absolute strain exceeding 0.24% (80% of peak strain during canine maximal moving velocity) was considered optimal strain. The stress/strain distributions within the PDL were acquired for various canine movements, and the optimal orthodontic forces were calculated. As a result the optimal tipping forces (40–44 g for distal-direction and 28–32 g for labial-direction) were smaller than the translation forces (130–137 g for distal-direction and 110–124 g for labial-direction). In addition, the optimal forces for labialdirection motion (110–124 g for translation and 28–32 g for tipping) were smaller than those for distal-direction motion (130–137 g for translation and 40–44 g for tipping). Compared with previous results, the force interval was smaller than before and was therefore more conducive to the guidance of clinical treatment. The finite element analysis results provide new insights into orthodontic biomechanics and could help to optimize orthodontic treatment plans.
The impact of ethnicity and self-reported health on psychological well-being: A comparative study of Kurdish-born and Swedish-born people

Abstract:Although immigrants generally have poorer health and higher psychological distress than the native population, information on Kurdish immigrants′ psychological well-being is limited. The aims of the study were to examine the association between ethnicity and poor psychological well-being, and to assess the relationships between socio-demographic characteristics, self-reported health, somatic pain, gastrointestinal complaints and poor psychological well-being. Methods: Immigrants with self-reported Kurdish ethnicity (men, n=111; women, n=86) in Sweden from the national sample of immigrants aged 27–60 and 1407 Swedes (1996) were studied. Unconditional logistic regression was performed using the Stata Software program. In the logistic model adjusted for age, sex, employment and self-reported health, the odds ratio for Kurdish-born subjects for having poor psychological well-being is twice as high as for Swedish-born subjects. Subjects with poor self-reported health had more than a threefold higher odds ratio for having poor psychological well-being compared with those with good self-reported health. Furthermore, being female, having somatic pain and recurrent gastrointestinal complaints regardless of ethnicity increased the odds for having poor psychological well-being. Being Kurdish-born and/or reporting poor health in addition to age, female gender, somatic pain and recurrent gastrointestinal complaints is significantly and independently related to poor psychological well-being even when adjusted for all confounders.
Effect of varying auxiliaries on maxillary incisor torque control with clear aligners: A finite element analysis

Abstract:Introduction: This study aimed to evaluate the effects of varying auxiliaries on tooth movement and stress distribution when maxillary central incisors were torqued 1° with a clear aligner through finite element analysis. Methods: Three-dimensional finite element models, including maxillary alveolar bone, periodontal ligament, dentition, and clear aligner, were constructed. According to the auxiliaries designed on the maxillary central incisor, 5 models were created: (1) without auxiliaries (control model), (2) with the power ridge, (3) with the semi-ellipsoid attachment, (4) with the horizontal rectangular attachment, and (5) with the horizontal cylinder attachment. The tooth movement and periodontal ligament stress distribution after a palatal root torque of 1° were analyzed for each of the 5 models. Results: With 1° torque predicted, the maxillary central incisor without auxiliaries showed a tendency of labial tipping, mesial tipping, and intrusion. The rotation center moved occlusally in the power ridge model. The labiolingual inclination variation increased in the semi-ellipsoid attachment model but decreased in the power ridge model. The maxillary central incisor is twisted in the distal direction in the power ridge model. The maxillary central incisor of the horizontal rectangular attachment and the horizontal cylinder attachment model behaved similarly to the control model. Periodontal stresses were concentrated in the cervical and apical areas. The maximum von Mises stresses were 11.6, 12.4, 3.81, 1.14, and 11.0 kPa in the 5 models. The semi-ellipsoid attachment model exhibited a more uniform stress distribution than the other models. Conclusions: Semi-ellipsoid attachment performed better efficacy on labiolingual inclination, and power ridge performed better efficacy on root control. However, a distal twist of maxillary incisors could be generated by the power ridge.

Abstract:Introduction: Orthodontics a unique branch in dentistry is inter-related with biology, mathematics and engineering sciences. Biomechanics is fundamental to the practice of orthodontics. Stresses generated at any point during orthodontic treatment are critical in remodeling of bone and type of tooth movement. Aims and Objectives: The aim of the following study is to determine the initial stresses produced in the tooth, periodontal ligament (PDL) and bone when force is applied on the tooth for intrusion and tipping on labial and lingual sides in the presence of varying alveolar bone heights. Materials and Methods: A geometric model generated using AutoCAD (Autodesk, Inc.). Six three-dimensional finite element models of a maxillary central incisor were designed with PDL and varying alveolar bone heights after applying the boundary conditions of the model, force was applied on the tooth.Results: Results were obtained with color coded three-dimensional fringe patterns. Conclusion: Alveolar bone loss causes an increase in the maximum initial stresses relative to the bone heights.
Effect of different post materials and designs on upper central tooth torque and intrusion load: A finite element analysis study

Abstract:Purposes: The aim of this study was to examine the effects of post material type and the presence of ferrules on the torque and intrusion load of the upper central tooth using finite element analysis. Methods: The upper central tooth and surrounding tissues (cortical bone, cancellous bone, and periodontal ligament) were modelled in three dimensions using the Spaceclaim software. Five simulated models (SM) different modifications were made to this main model: metal cast post (SM1) and glass fiber post-core with zirconium crown and without a ferrule (SM2), metal cast (SM3) post and glass fiber post with a ferrule and zirconium crown (SM4) and only zirconium crown (SM5). In all five simulations, in order to simulate lingual root torque movement, a total load of 40 grams was applied to the bracket slot as 20 grams of force couples and in order to simulate intrusion movement, a load of 40 grams was applied to the superior wall of the bracket slot. The stress caused by the applied loads on the root surfaces was determined using finite element analysis. Maximum principal stress (MPS) value was used in the comparison. Results: The highest root surface MPS values for both intrusion and torque loads belonged to SM2 (3.864 and 0.379MPa, respectively). The presence of ferrules in both intrusion and torque loads reduced the stress by approximately half (from 3.864 to 2.004MPa). In all five models, the radicular area with higher stress was located in the cervical third on the lingual surface when both torque and intrusion loads were applied. Conclusion: The amount and localization of stress was affected by the type of post material. The variation in stress values between the materials remains within a safe range (0.099 and 3.87MPa), making both materials suitable for use under orthodontic forces.

Abstract:Introduction Space closure by molar protraction has always been a challenge in orthodontic treatment due to larger root surface area which requires additional anchorage. Ideally, there should be little or no tipping. However, the protraction forces, being occlusal and buccal to the centre of resistance (CR) of the tooth, cause tipping and rotations. Aim The aim of the study was to assess the effect of bracket slot and archwire dimensions on posterior tooth movement during space closure in sliding mechanics and evaluate the length of power arm to bring about translatory movement of teeth using three-dimensional finite element analysis. Materials and methods A model of the maxillary teeth was created and converted to a finite element format through a meshing software, Hypermesh. Two three-dimensional models, each with a combination of 0.017"× 0.022" archwire in 0.018" slot (model 1) and 0.019"×0.025" archwire in 0.022" slot (model 2), were generated. Power arms of different lengths were attached to the first molar. Miniscrew was inserted between the canine and first premolar. Results In model one, the power arm of 10-mm height provided controlled tooth movement than the one with 6 mm height, and in model two, power arms of both 6-mm and 10-mm height produced controlled tooth movement. Conclusions As the force was raised apically from the slot, more translation was observed. Power arm of 6-mm height can be used due to anatomic limitation of the vestibule.
Efficacy of in-house clear aligner therapy mechanics on root torque: an in-vitro study

Abstract:Abstract Background This study aimed to evaluate the torque effectiveness of different attachment types and power ridges used in clear aligner therapy (CAT) on the upper lateral incisors. Methods A typodont model simulating oral conditions was developed. Two hundred lateral incisor samples were divided into five groups based on attachment types and clear aligner modifications: no attachments (NA), horizontal ellipsoid attachment (HEA), vertical ellipsoid attachment (VEA), buccal power ridge (BPR), and buccal and palatal power ridge (BPPR). Subsequently, a 5° palatal torque moment was applied to the lateral incisors. In-house clear aligners were inserted, and 3D scanning was performed after each aligner step. Post-treatment 3D models were compared with pre-treatment models. The lateral incisors were assessed for torque, tipping, and rotational movements. Results The HEA group (41.6%) and the BPPR group (40.8%) showed significantly higher torque effectiveness than the NA group (20%) when 5° of torque was delivered. The planned movement was approximated when 1° and 2° of torque were applied in the NA group (114% and 98%, respectively). The NA and BPR groups showed the highest loss of anchorage. Conclusions When more than 3° of torque was applied, HEA and BPPR were more effective than NA in achieving the CAT’s planned torque of the upper lateral incisors, although with an efficacy rate of approximately 40%. Highlights It is challenging to achieve torque movement using CAT. When less than 3° of torque was required, additional mechanics were not necessary. BPPR and HEA showed higher effectiveness when 5° of torque was applied. The efficacy of BPR was similar to that of NA.

Comparative evaluation of the effects of different torque settings on dentinal crack formation using single- and multi-file system: An in vitro study

Abstract:Context: To improve efficiency, biomechanical preparation in root canal treatment is shifting from manual SS to nickel-titanium (NiTi) rotary devices. While multi-file NiTi systems entail crack and fracture issues, modern single-file systems address these concerns. Aims: The aim of this study was to evaluate and compare the effects of different torque settings on dentinal crack formation using single-file systems (SFS) (One Curve [OC]) and multi-file systems (ProTaper Next [PTN]) at different levels of the tooth. Subjects and methods: The study was conducted on 45 freshly extracted human mandibular premolars divided into groups: OC at minimal and maximal torque, PTN at minimal and maximal torque, and a control group. After canal preparation, teeth were horizontally sectioned at 3, 6, and 9 mm from the apex, and then examined for cracks using a stereomicroscope. Statistical analysis used: This was analyzed using Chi-square test. Results: PTN group: Highest crack rates at the middle (55.6%) and apical (77.8%) thirds with maximum torque; OC group: Highest rates at the middle (22.2%) with minimal torque and apical (11.1%) with maximum torque. Conclusions: Maximal torque settings had more incidence of cracks compared to minimal torque settings. It can be stated that SFS (OC) produced less cracks compared to multi-file system (PTN) at both minimal and maximal torque settings.

Abstract:The analyses substantially delineating "integrative studies of large urban areas as bio- physical-social complexes" and the suggestions by Pickett et al. in "Beyond Urban Legends" (Bioscience 2008 58 139-150) provide an initial framework for a theory of urban ecology. This article intends to contribute to the project by: 1) improving the philosophical rigor of critical concepts and epistemologies; 2) making explicit the complementary theoretical and empirical work in urban ecology already being done that can be better integrated, for example, studies from outside the U.S. and uses of actor network theory; 3) bringing forward more disciplines and theories which successfully deploy modes of thinking, research procedures, and practices more adequate to the phenomena at all scales and levels of particularity, i.e., micro, phenomenal, macro, to fill in some of the empirical gaps in the middle, specifically those having to do with human values and the richness of the everyday lifeworld. In addition to what is available within complexity theory itself, chief among the approaches to be utilized are phenomenology, ethnographic thick description, and actor network theory.
Improved survival after lymphadenectomy for nodal metastasis from an unknown primary melanoma.

Abstract:PURPOSE No primary lesion is identified in 10% to 20% of patients presenting with palpable evidence of regional metastatic melanoma. Because the prognostic significance of unknown primary melanoma (MUP) is unclear, we compared clinical outcomes of patients with MUP and known primary melanoma (MKP) with regional nodal metastases. PATIENTS AND METHODS We reviewed our 13,000-patient prospective melanoma database (1971 through 2005) to identify patients managed with regional lymphadenectomy for palpable nodal metastases from MUP or MKP. Multivariate analysis identified prognostic factors significant for survival. MUP and MKP were then matched by significant covariates. Overall survival (OS) was estimated by Kaplan-Meier method and compared by log-rank analysis. RESULTS Multivariate analysis of data from 1,571 study patients identified four significant covariates associated with worse prognosis: age >or= 60 years (hazard ratio [HR] = 1.294; P = .0017), male sex (HR = 1.335; P = .0004), nodal tumor burden >or= one (HR = 1.256; P < .0001), and known primary (HR = 1.507; 95% CI, 1.220 to 1.862; P = .0001). Five-year OS was significantly higher for 262 patients with MUP than for 1,309 patients with MKP (55% +/- 6% v 44% +/- 3%; P = .0021). Computerized matching of MUP and MKP by four significant covariates (age, sex, nodal tumor burden, and decade of diagnosis) yielded 221 matched pairs. Median and 5-year OS rates were 165 months and 58% +/- 7%, respectively, for MUP as compared with 34 months and 40% +/- 7%, respectively, for MKP (P = .0006). CONCLUSION Lymphadenectomy is effective for nodal metastasis from MUP. The significantly better postoperative survival for MUP versus MKP suggests a strong endogenous immune response against the primary melanoma. Immunologic studies to identify cell-mediated and antibody components of this response may lead to new approaches for determining melanoma prognosis and treatment.
Evaluation of stress distribution values at the temporomandibular joint and distalization of teeth during en-masse retraction of the mandibular dentition using buccal shelf bone screws: A FEM study

Abstract:Introduction: Present Finite-element-method (FEM) study evaluates the stress distribution values at the Temporomandibular-Joint (TMJ) during en-masse retraction of the mandibular dentition using buccal shelf bone screws with varying magnitudes of forces. Material and methods: Nine replicas of a pre-existing three-dimensional finite element model of the craniofacial skeleton and articular disc from Cone-Beam-Computed-Tomography (CBCT) data and Magnetic-Resonance-Imaging (MRI) data of a patient were used. Buccal shelf (BS) bone screws were inserted buccal to the mandibular second molar region. Forces of magnitudes 250 gm, 350 gm and 450 gm were applied through NiTi coil springs along with stainless-steel archwires of sizes 0.016 × 0.022-inch, 0.017 × 0.025-inch and 0.019 × 0.025-inch. Results: On the articular disc, the maximum stress was observed at the inferior region and the inferior part of the anterior and posterior zone at all force levels. The stress on the articular disc and displacement of teeth increased with increase in force levels in all the 3 archwires. The maximum stress at the articular disc and displacement of teeth was observed for 450 gm force and the least for 250 gm force. No significant difference in the amount of displacement of teeth or the stresses produced at the articular disc was seen with increasing the size of the archwire. Conclusion: With the present FEM study, we can deduce that it is better to use forces of lower levels on patients with temporomandibular-disorders (TMD) so as to reduce the stresses produced at the TMJ to avoid worsening of the TMD condition.

Abstract:Objective: The study was designed to evaluate and compare stress distribution intrans cortical section of bone with titanium implant and zirconia implant model under vertical and oblique forces in anterior maxillary region. Materials and Methods: A three‐dimensional finite element model was designed using ANSYS 13.0 software. Around the prepared implant, bone was constructed with definitive differentiation of outer cortical and inner cancellous. Two straight abutment was constructed, crowns of 9mm mesiodistal width and 11mm cervicoincisal length were created and they were cemented with 50 micron meter cemental layer. The bone-implant interface for both the models was bonded, simulating complete osseointegration and the dental implant, abutment and crown were assumed to be connected as a single unit. Force application was performed in both oblique and vertical conditions using 100 N as a representative masticatory force. For oblique loading, a force of 100 N was applied at 45° from the vertical axis. Von Mises stress analysis was evaluated. Results: The results of the study showed cortical stress in the Titanium and Zirconia model under oblique forces were 81.317 MPa and 78.405 MPa, respectively. Cortical stress in the titanium and zirconia implant model under vertical forces was 46.161 MPa and 46.097 MPa, respectively. Conclusion: Results from this study showed the zirconia implant model led to relative decrease in von Mises stress in trans cortical section of bone compared to titanium under vertical and oblique forces in anterior maxillary region. Solution properties of tadpole-type cationic amphiphilic dendrimers consisting of an alkyl chain, a quaternary ammonium, and a poly(amidoamine) dendron.

Abstract:Tadpole-type amphiphilic dendrimers consisting of an alkyl chain, a quaternary ammonium bromide (qb) and a poly(amidoamine) dendron (den) (CnqbdenGm, where n represents an alkyl chain with a length of 10,14, or 18 carbons; m is the generation number G of dendron taking the value -0.5, 0.5, or 1.5) were synthesized using N, N-dimethylethylenediamine as a central scaffold. Electrical conductivity, surface tension, pyrene fluorescence, and dynamic light scattering measurements were used to characterize the properties of the dendrimers. In addition, the effect of the alkyl chain length and the generation number of a dendron on these properties was evaluated through a comparison with those of the corresponding previously reported amphiphilic dendrimers with lactobionamide sugar terminal groups (CnqbdenGmLac) and conventional cationic monomeric surfactants (CnTAB). Both critical micelle concentration (cmc) and surface tension (except for C18 series) were lower than those of CnqbdenGmLac and CnTAB with the same alkyl chain length, indicating that the synthesized amphiphilic dendrimers have an excellent micelle-forming ability in solution and high adsorption ability at the air/water interface, in spite of the large bulky dendron structure. When the alkyl chain length and the generation number of the amphiphilic dendrimers were increased, the surface tension became high because of the curved long alkyl chain and the bulky structure of dendron. Further, CnqbdenGm formed micelles with a small size in solution, and the micelles of CnqbdenG(-0.5) had almost a constant size despite the changes in the concentration, while those of CnqbdenG0.5 and CnqbdenG1.5 became smaller with increasing concentration. The difference in the behavior results from the difference in the number of amide groups in the low- and high-generation dendrons.

Abstract:Introduction The aim of this study is to evaluate stress and displacement effects of maxillary posterior intrusion mechanics with mini-implant anchorage by using finite element method. Materials and Methods A computer stimulation of three-dimensional model maxilla with all teeth, PDL, bone, mini-implants, brackets, arch wire, force element, and transpalatal arch was constructed on the basis of average anatomic morphology. Finite element analysis was done to evaluate the amount of stress and its distribution during orthodontic intrusive force. Results Increased Von Mises stress values were observed in mesio-cervical region of first molar. The middle third of second premolar and second molar and regions adjacent to force application sites also showed relatively high stress values. Minimum stress values were observed in apical region of first premolar as it is away from force application. Conclusion Using three mini-implant and transpalatal arches, this study demonstrates that significant amount of true intrusion of maxillary molars could be obtained with lesser concentration of stresses in the apical area recorded.
Evaluation of stress and displacement of maxillary canine during the single canine retraction in the maxillary first premolar extraction cases- A finite element study

Abstract:Objectives: This finite element study aimed to simulate maxillary canine movement during anterior teeth retraction. Materials and methods: Three methods of maxillary canine movement including miniscrew sliding with high hooks (MSH), miniscrew sliding with low hooks (MSL), and the traditional sliding method (TS) without using miniscrews were simulated using three-dimensional finite element analysis. The initial displacement of the maxillary canine, the maximum principal stress of the periodontal ligament and the Von Mises stress were calculated. Results: The distolingual tipping movements of the canine were shown in three movement modes. MSH showed a small tendency to lingual tipping movement and a extrusion movement while MSL had the largest lingual inclination. TS demonstrated a tendency toward distolingual torsion displacement. Compressive stress values were mainly concentrated in the range - 0.003 to -0.006 MPa. For tensile stress, the distribution of MSH and MSL was concentrated in the range 0.005 to 0.009 MPa, TS was mainly distributed about 0.003 MPa. Von Mises equivalent stress distribution showed no significant difference. Conclusions: The loss of tooth torque was inevitable, irrespective of which method was used to close the extraction space. However, miniscrew application and higher hooks reduced the loss of torque and avoided lingual rotation. Clinical relevance: This study shows that miniscrew implants with different hooks can better control the movement of the maxillary canines. The non-invasive nature of the finite element analysis and its good simulation of dental stress and instantaneous motion trend have a clinical advantage in the analysis of tooth movement.
Torque Control of the Maxillary Incisors in Lingual and Labial Orthodontics: A 3-Dimensional Finite Element Analysis

Abstract:Introduction: Lingual orthodontics has developed rapidly in recent years; however, research on torque control variance of the maxillary incisors in both lingual and labial orthodontics is still limited, especially studies with 3-dimensional finite element methods. Thorough understanding of the biomechanical differences of incisor torque control during lingual and labial orthodontic treatment is critical for the best results. Methods: A 3-dimensional finite element model of the maxilla and the maxillary incisors was made with 98,106 nodes, 71,944 10-node solid elements, and 5236 triangle shell units. Horizontal retraction force, vertical intrusive force, and lingual root torque were applied to simulate labial and lingual orthodontic treatment. Then the distribution of the stress-strain (maximum and minimum principal stresses; maximum and minimum principal strains) in the periodontal ligament, the total displacement, and the vector graph of displacement of the nodes of the maxillary central incisor were analyzed and compared between labial and lingual orthodontics. Results: Loads of the same magnitude produced translation of the maxillary incisor in labial orthodontics but lingual crown tipping of the same tooth in lingual orthodontics. This suggests that loss of torque control of the maxillary incisors during retraction in extraction patients is more likely in lingual orthodontic treatment. Conclusions: Lingual orthodontics should not simply follow the clinical experience of the labial techniques but should increase lingual root torque, increase vertical intrusive force, and decrease horizontal retraction force properly to achieve the best orthodontic results. Effect of Implant-Abutment Connection and Abutment Angulations on Peri-implant Stress Levels: A Finite Element Analysis Study

Abstract:Introduction: Placement of an implant in the maxillary anterior region is challenging due to the angulation of bone in this area. Angled abutments may be used to achieve proper restorative contours. The present study was undertaken to examine and compare the stress levels of implants in the maxillary anterior region having different types of internal connections and different abutment angulations. Material and methods: Implants with three types of abutment connections, internal conical, Morse taper, and internal hex, were modeled using SolidWorks software. Three abutment angulations of 0, 15, and 30 degrees were used for each type of implant. A 100 N axial load was applied to the implants, and the stresses on the implant, abutment, and bone were analyzed by finite element analysis. Results: Among the straight abutments, the most stress was in model 3A (62.60 MPa). The stress value among angled abutments was highest with 30-degree angled abutments. The value was highest in model 3C (94.83 MPa). Internal hex connection showed the highest stress levels in all degrees of angulation of the abutment, and Morse taper connection showed the least amount of stress in all three abutment angles. The most stress concentration was seen in the cortical bone on the buccal surface in the implant-abutment junction. Conclusion: The Morse taper design of implant exhibited the least-highest stress levels on the alveolar bone. The stress levels increased with the increasing angulation of the implant or implant-abutment connection.

Abstract:Objectives To use the finite element method (FEM) to investigate the effect of various attachment configurations and trimming line designs of orthodontic aligners on their biomechanical performance. Method A 3D upper jaw model was imported into 3D design software. The upper right central incisor tooth (Tooth 11) was made mobile, and its periodontal ligament (PDL) and bone structures were designed. Aligners were modelled with three distinct attachment configurations: No attachment, rectangular horizontal, rectangular vertical, and two trimming line designs; scalloped and straight extended, with a homogeneous thickness of 0.6 mm. These models were then imported into an FE software. Simulations were conducted for three different movements, including facial translation, distalization, and extrusion. Results Forces were recorded at 1.3–2.6 N during facial translation, 1.4–5.9 N in distalization, and 0.0–2.0 N in extrusion. The straight extended trimming line consistently generated higher forces than the scalloped design. Attachments had no significant impact on force components during facial translation but were more effective in distalization and extrusion. The combination of a straight extended trimming line with horizontal attachments exhibited the least stresses at the apical third during distalization, and the highest stresses during extrusion, suggesting superior retention. Conclusions Rectangular attachments offer limited benefits in facial translation, but horizontal rectangular attachments can intensify load in distalization and are crucial for force generation in extrusion. Horizontal attachments are preferred over vertical options. Additionally, the straight extended trim line enhances control of tooth movement and can replace attachments in certain cases. Clinical Relevance These findings provide biomechanical evidence and an optimal protocol to guide clinical practice in planning diverse teeth movements. The emphasis is on the influence of attachment utilization and the specific design of aligner trimming lines to enhance control over tooth movement.

In vitro biomechanical analysis of torque capabilities of various 0.018″ lingual bracket-wire systems: total torque play and slot size

Abstract:Objectives: To determine the total torque play of various rectangular titanium molybdenum alloy (TMA)/stainless steel (SS) wires in various 0.018″ upper incisor lingual brackets and slot size measurements. Methods: TMA (0.0175″ × 0.0175″, 0.0170″ × 0.025″, 0.0182″ × 0.0182″, 0.0182″ × 0.025″) and SS wires (0.016″ × 0.022″, 0.016″ × 0.024″, 0.018″ × 0.025″) were twisted in standard (Hiro, Incognito™, Joy®, Kurz 7th generation, STb™: fixation with elastic modules) and self-ligating brackets (Evolution SLT®, In-Ovation® L MTM: closed ligation mechanism) from -20 degrees to +20 degrees with a custom-made machine. The total torque play was calculated by extrapolating the linear portion of the twist/moment curves to the x-axis and adding the absolute negative and positive angle values at the intercepts. The bracket slot height was measured before and after the experiments with a series of pin gauges with round profile. Results: Brackets in ascending order for total torque play with the most slot-filling wire TMA 0.0182″ × 0.025″: Evolution SLT® (0 degree ± 0 degree), Incognito™ (2.2 degrees ±1.1 degrees), Hiro (5.1 degrees ±3.0 degrees), In-Ovation® L MTM (6.3 degrees ±2.2 degrees), STb™ (6.6 degrees ±1.8 degrees), Kurz 7th generation (7.1 degrees ±0.8 degrees), and Joy® (12.0 degrees ±0.8 degrees). Wires in ascending order for total torque play with the most precise slot Incognito™: TMA 0.0182″ × 0.025″ (2.2 degrees ±1.1 degrees), TMA 0.0182″ × 0.0182″ (2.4 degrees ±0.9 degrees), SS 0.018″ × 0.025″ (5.5 degrees ±1.0 degrees), TMA 0.0170″ × 0.025″ (9.4 degrees ±1.8 degrees), TMA 0.0175″ × 0.0175″ (13.0 degrees ±1.5 degrees), SS 0.016″ × 0.024″ (16.1 degrees ±1.4 degrees), SS 0.016″ × 0.022″ (17.8 degrees ±1.0 degrees); differences between some of the experimental groups were not statistically significant. Bracket slot dimensions in ascending order: Evolution SLT® (less than 0.452mm), Incognito™ (0.460mm ±0.002mm), In-Ovation® L MTM (0.469mm ±0.001mm), Hiro (0.469mm ±0.010mm), STb™ (0.471mm ±0.002mm), Kurz 7th generation (0.473mm ±0.002mm), and Joy® (greater than 0.498mm). Limitations: The applied method must be questioned when used with brackets with incomplete slot walls (Evolution SLT®). Slot measurement with pin gauges may not register bracket wing deformation. Conclusions: All brackets showed a differing slot size from the nominal 0.018″ (0.457mm). Incognito™ presented the most precise and Joy® the widest slot. The main wires for the retraction phase SS 0.016″ × 0.022″/SS 0.016″ × 0.024″ showed poor torque control. Among the finishing TMA wires, TMA 0.0175″ × 0.0175″ exhibited the highest and TMA 0.0182″ × 0.0182″/TMA 0.0182″ × 0.025″ the smallest torque play. Significance: The manufacturers could profit from this investigation towards optimization of the dimensional precision of their products. The orthodontist must be aware of the torque play of the wire-bracket combinations to be able to plan and individualize the appliance by third order customization.
Effects of Mandibular Canine Intrusion Obtained Using Cantilever Versus Bone Anchorage: A Comparative Finite Element Study

Abstract:Background This study was conducted to assess and compare the effects of mandibular canine intrusion obtained using a cantilever having different toe-in bends and with mini-implants using the three-dimensional (3D) finite element method (FEM). Methodology 3D models of the mandibular right quadrant were created using FEM. Brackets and molar tubes were also modeled. In the first model, the mandibular canine intrusion was produced using a cantilever loop with compensatory toe-in bends (0°, 4°, 6°, and 8°). In another model, the intrusion was done using two mini-implants. Force was applied using an elastic chain. The amount of intrusion and the associated labial tipping of canine that occurred in both the models was assessed and compared using FEM analysis. Results The pure intrusion of the canine was produced by the 6° toe-in bend of the cantilever. The labial tipping of the canine was also reduced. The highest amount of periodontal ligament stress was observed around the canine root with a 0° toe-in bend. In the posterior segment, the molar displayed a slight tendency for extrusion and distal crown tipping. Conclusions The intrusion mechanics using a cantilever simulated in this study may achieve pure mandibular canine intrusion with minimal labial tipping when a compensatory toe-in of 6° is incorporated into the cantilever.
