Abstract:Background: Upper limb balance is one of the important physical fitness parameters for all populations, especially overhead athletes like swimmers. Upper extremity star excursion balance test (UESEBT) is a comprehensive dynamic balance assessment, this study aims to explore the reliability and validity of UESEBT among adolescent swimmers.Methods: This cross-sectional study recruited 70 adolescent swimmers. All participants were required to complete UESEBT, upper quarter Y-balance test (UQYBT), maximal isometric strength (MIS) tests in upper limb, closed kinetic chain upper extremity stability test (CKCUEST), trunk flexor endurance test (TFET) and lateral trunk endurance test (LTET). The intra- and inter-operator reliability and the correlation of UESEBT with other physical performances were conducted.Results: For reliability, the intra- and inter-operator reliability of all directions and composite score were high-to-excellent (ICC = 0.706-1.000) among all participants. For validity, the UESEBT has a moderate-to-strong correlation with UQYBT (r = 0.42-0.72, p < 0.001), and a weak-to moderate one with CKCUEST (r = 0.25-0.42, p < 0.05). Furthermore, the UESEBT performance showed weak-to-moderate correlations with MIS (r = 0.24-0.44, p < 0.05). UESEBT was correlated to LTET (r = 0.24-0.33, p < 0.05) whereas no relationship was found with TFET.Conclusions: UESEBT was a reliable and valid tool to screen upper extremity dynamic balance among adolescent swimmers. UESEBT provides more detailed information in eight directions to assess the upper limb sport performance. Further study should explore the prediction ability of UESEBT for injury.

Abstract:The purpose of this study was to consider the validity of measuring the Star Excursion Balance Test (SEBT), which has been used as a measure of balance ability in athletes, for community-dwelling elderly people. To achieve this purpose, we examined the relationship between the existing balance ability measures, leg strength, and the SEBT. The subjects were 36 elderly people (mean age: 71.1 years). We used the SEBT, functional reach test (FRT), timed up-and-go test (TUG), and Mini-BESTest to evaluate balance ability and the 30-second chair-stand test (CS-30) to assess leg strength; we also assessed the 10-m walk time. The correlation coefficient with the SEBT was calculated. The significant level was 5%. A significant positive correlation was observed between the total score of Mini-BESTest and the SEBT reach distance in the anterior direction (r = 0.364, p < 0.05) and the posteromedial direction (r = 0.407, p < 0.05). A significant positive correlation was observed between anticipatory postural control and the SEBT reach distance in the anterior directions (r = 0.403, p < 0.05), and postural responses and SEBT reach distance in the posterolateral direction (r = 0.360, p < 0.05), which were subsystem category of Mini-BESTest. No significant correlation was found in the other items. The correlation with Mini-BESTest indicates that SEBT might be a valid tool to evaluate the dynamic balance ability of community-dwelling elderly people. However, their correlation coefficient is not high, suggesting that SEBT can evaluate elements different from Mini-BESTest.

Abstract:Background/Objectives: The Star Excursion Balance Test (SEBT) is a simple and feasible tool for assessing dynamic balance in individuals with knee osteoarthritis (KOA). It has an advantage as it replicates dynamic balance better than other static balance tools. This study aims to determine how reliable SEBT is among people with end-stage KOA, as well as how responsive it is and how well it correlates with performance-based outcome measures after TKA. Methods: Patients on the waiting list for TKA performed SEBT in the anterior, posteromedial and posteriorlateral directions twice within 7 days. The measurements were repeated 6 and 12 months after TKA. The participants completed performance-based outcome measurements (PBOMs) and the Oxford Knee Score (OKS) before and after TKA to estimate correlation. Results: In all directions, the intraclass correlation coefficient range (ICC) was 0.998–0.993, and there were no significant differences between the test and re-test mean SEBT scores. The standard error of measurement (SEM) ranged from 0.37% to 0.68%, and the minimum detectable change (MDC) ranged from 1.02% to 1.89%. The post TKA SEBT results show significant improvement, with a large effect size. There were large-to-medium correlations between SEBT and PBOMs before and after TKA, while OKS correlated only before surgery. The magnitude of change in SEBT, PBOMs and OKS did not correlate. Conclusions: SEBT is an extremely reliable tool for assessing dynamic balance in all three directions of severe KOA patients. It is sensitive enough to detect balance changes at 6 and 12 months post TKA. SEBT cannot be used to reflect the change in functional outcome improvement after TKA.

Abstract:Background: The Star Excursion Balance Test (SEBT) has been used as a rehabilitation exercise. To improve its efficacy, efficiency, and method variations, the Y-Balance Test (YBT) with anterior (A), posterolateral (PL), and posteromedial (PM) directions of the SEBT has been recommended. Electromyographic activity has been reported to change when the same task is performed on various surfaces.Hypothesis/Purpose: To compare the EMG activity of trunk and LE muscles during the performance of the YBT on stable and unstable surfaces.Study Design: Cross-Sectional study.Methods: Healthy adults with no history of chronic ankle instability were recruited for the study. Surface electromyography was collected for bilateral (ipsilateral [i] and contralateral [c]) rectus abdominis (RA), external oblique (EOB), erector spinae (ES). While, gluteus maximus (GMAX), gluteus medius (GMED), medial hamstrings (MH), biceps femoris (BF), vastus medialis (VM), rectus femoris (RF), vastus lateralis (VL), anterior tibialis (AT), and medial gastrocnemius (MG) on the stance leg (ipsilateral side), during the performance of the YBT. The unstable surface was introduced using a Thera-Band stability trainer. Differences in electromyography were examined for each reach direction and muscle between the stable and unstable surfaces (p≤ 0.05).Results: Twenty (10 male, 10 female) subjects participated (age: 27.5 ± 4.0 years, height:167 ± 1.0 cm, weight: 66.5 ± 13.0 kg, body fat: 14.1 ± 6.2%). Significantly higher muscle activity for the unstable surface (p<0.05) with moderate to large effect sizes were observed for the following muscles in the A direction: GMED, GMAX, VM, RF, and MG; PL direction: iEOB, iES, cES, GMED, BF, VM, RF, and MG; and PM direction iEOB, iES, GMED, BF, VM, and RF. Significantly higher muscle activity for the stable surface (p = 0.007) was observed in MH muscle in the A direction. No significant differences (p>0.05) between the stable and unstable surfaces were observed in iRA, cRA, cEOB, VL, and AT for any of the directions of the YBT.Conclusion: An increase in muscle activity was observed during YBT on unstable versus stable surfaces for some muscles.Level of Evidence: 2B.
The Development and Reliability of an Updated Dance-Specific Star Excursion Balance Test Protocol

Abstract:Background: While the previous research has made crucial developments in a dance-specific version of the SEBT, current modifications to the SEBT have not conclusively produced a valid dance-specific dynamic balance test. Purpose: The aim of this paper was to utilize the most practical dance-specific variations from previous research and incorporate them into a reliable test to be considered for future screenings for dancers. Methods: Twenty-one female dancers voluntarily took part in the research (age: 20.86 ± 3.68 years). This protocol consists of 3 stages, each increasing in difficulty; stage one: dsSEBT Average Tempo, stage two: dsSEBT Block, and stage three: a combination of stage 1 and stage 2, the dsSEBT Average Tempo on Block. Reach distance (% of limb length), error scores, and average time to complete each stage were recorded. A stage completion criteria was developed to move from one stage to the next wherein certain reach distance and error score standards needed to be met. Results: Between previous research using the same participants and the current study, each reach direction exhibits a statistically significant correlation (P < .05) with good to excellent ICC values ranging from .750 to .918, suggesting that test-retest reliability is high. Overall, 90.48% of participants succeeded in passing stage one, 19.05% of total participants passed stage two, and only 4.76% of the 21 participants passed all 3 stages with statistically significant differences detected for reach distance and incomplete trials between stages (P < .05). Conclusion: This data suggests that the test is challenging enough to show dancers weaknesses and push the limits of their balance capabilities. With a clear increase in difficulty from stage to stage, the test adds layers of demanding tasks designed to test the dancer proprioceptively.

Abstract:Balance ability assessment is crucial for screening mobility decline group and introducing early interventions. However, it is a big challenge to identify the population with moderate balance problems. Force platform test provides a tool for acquiring and analyzing the Center-of-Pressure (COP) signal of the human body. In this paper, the envelope curve area (ECA), and trajectory mean velocity (TMV) of COP signal were extracted as the magnitude and responsiveness parameters of COP. The tests on 136 subjects indicate that ECA and TMV offer comprehensive and efficient indices for assessing balance ability and suggest that the most reliable results are achieved under eyes opened condition.

Abstract:Background Lower limb stabilizing muscle strength imbalance and changes in functional performance and dynamic balance have been reported as predictors of lower extremity injuries. Lower limb functional tests are commonly applied in clinical practice to assess functional performance as well as used as a measure of progression during rehabilitation. Among them, the Star Excursion Balance Test (SEBT) and the Single Leg Hop Test (SLHT) stand out as easy-to-apply and low-cost tools. Objectives To evaluate the relationship between lower limb stabilizing muscle strength and performance in functional tests in individuals without history of injury. Methods This is a quantitative cross-sectional study. As eligibility criteria, male individuals, aged 18 to 30 years, with no history of previous injury to the lower limbs were included. The data collection was divided into two days. On the first day, anamnesis was performed, anthropometric data were collected, familiarization with the muscle strength test was carried out and the functional tests SLHT and SEBT were applied to the Dominant Limb (DL) and Non-Dominant Limb (NDL). On the second day, the strength of the stabilizing muscles of the hip (lateral rotators and abductors), knee (quadriceps and hamstrings) and ankle (inverters and evertors) were assessed using a portable Lafayette® dynamometer stabilized by an inelastic band. Three maximal voluntary isometric contractions were performed, lasting five seconds, with a thirty-second interval between each contraction, bilaterally. The peak strength of each movement was recorded and later normalized by body mass. Statistical analysis was performed using the SPSS 18.0® software, applying the Shapiro-Wilk normality test and the Pearson correlation test. A significance level of α<0.05 was adopted. Results 20 male individuals were collected. A moderate positive correlation was observed between peak strength of the NDL lateral rotators and hip abductors with performance in the DL SLHT and posteromedial SEBT. In addition, the strength of the knee extensors of both limbs was positively correlated with performance in the SLHT of the NDL. Furthermore, a correlation was observed between peak strength of NDL lateral rotators, DL ankle inverters and hip abductors with NDL posteromedial SEBT. Conclusion The maximum isometric strength of the lower limbs stabilizing muscles is related to the performance in functional tests. Implications The weakness of the stabilizing muscles of the hip and knee is directly related to a lower performance in functional tests, which may cause instability during movements, resulting in biomechanical changes that increase the risk of injury to the lower limbs. Still, the findings of this study elucidate that the performance analysis should take into account the bilateral force for the parameters of rehabilitation and injury prevention.

Reliability of the Instrumented Modified Clinical Test of Sensory Interaction on Balance Using a Virtual Balance Device

Abstract:The purpose of this study was to evaluate the test-retest reliability of the instrumented version of the modified Clinical Test of Sensory Interaction on Balance (i-mCTSIB) using the VirtuSense VirtuBalance System™ (VSTBalance), a virtual balance device, in healthy young adults. Fifty-four subjects aged 20-27 years (Mean age 23.07, SD ± 1.6), participated in the study. A one-group design was utilized. Three trials of the i-mCTSIB were performed on two separate days to measure the mean sway velocity of the trunk under four conditions. Within-day reliability of trials 1-3 was estimated with intraclass correlation coefficients (ICC3,1) and between-day reliability was estimated using the averages of trials 1-3 on day 1 and day 2 (ICC3,3). Within-day reliability was moderate on day 1 (ICC = 0.511-0.672) and day 2 (ICC = 0.539 -0.677). Between-day reliability was moderate to good (ICC = 0.705-0.810). The lower bounds of the confidence intervals of within-day reliability estimates were 0.341-0.548, while the lower bounds of the confidence interval for the between-day reliability estimates were 0.390-0.671. Reliability of the VSTBalance for balance assessment using the i-mCTSIB test is moderate to good. Between-day reliability was higher than within-day reliability. When using the VSTBalance to assess balance with the i-mCTSIB, clinicians should provide practice of each condition contained in the test to improve reliability. The higher between-day reliability as compared to within-day reliability suggests that clinicians should use the averages of three trials when assessing balance performance change across time for each condition of the i-mCTSIB test when assessing balance using the VSTBalance.
Test-retest, intra- and inter-rater reliability of the reactive balance test in patients with chronic ankle instability

Abstract:Introduction: The Reactive Balance Test (RBT) could be a valuable addition to research on chronic ankle instability (CAI) and clinical practice, but before it can be used in clinical practice it needs to be reliable. It has already been proven reliable in healthy recreational athletes, but not yet in patients with CAI who have shown persistent deficits in dynamic balance. The study aimed to determine the test-retest, intra-, and inter-rater reliability of the RBT in patients with CAI, and the test-retest and inter-rater reliability of the newly developed RBT score sheet. Methods: We used a repeated-measures, single-group design to administer the RBT to CAI patients on three occasions, scored by multiple raters. We included 27 participants with CAI. The study used multiple reliability measures, including Pearson r, intra-class correlations (ICC), standard error of measurement (SEM), standard error of prediction (SEP), minimal detectable change (MDC), and Bland–Altman plots, to evaluate the reliability of the RBT's outcome measures (visuomotor response time and accuracy). It also assessed the test-retest and inter-rater reliability of the RBT score sheet using the same measures. Results: The ICC measures for test-retest reliability were similar for accuracy (0.609) and VMRT (0.594). Intra-rater reliability had high correlations and ICCs for accuracy ( r = 0.816, ICC = 0.815) and VMRT ( r = 0.802, ICC = 0.800). Inter-rater reliability had a higher ICC for VMRT (0.868) than for accuracy (0.690). Conclusion: Test-retest reliability was moderate, intra-rater reliability was good, and inter-rater reliability showed moderate reliability for accuracy and good reliability for VMRT. Additionally, the RBT shows robust SEM and mean difference measures. The score sheet method also demonstrated moderate test-retest reliability, while inter-rater reliability was good to excellent. This suggests that the RBT can be a valuable tool in assessing and monitoring balance in patients with CAI.

Abstract:Objective: To study the correlation between dynamic balance ability and lower limb muscle strength of university students. Methods: 50 students of Zunyi Medical University (23 boys and 27 girls) were selected as the subjects. The dynamic balance ability of university students was tested by the YBT test system. Isokinetic muscle strength tester tests knee flexion and extension muscle strength under different angular velocity conditions. Results: 1) At different angular velocities, there was a moderate positive correlation between knee flexion and extension muscle strength and YBT reaching all directions. Only when the angular velocity was set to 200° / s, there was a low positive correlation between knee flexion muscle strength and the front side of the YBT test. 2) There is a more significant correlation between the maximum distance of each direction of dynamic balance and the peak torque of knee extensor than that of flexor. Research conclusions: For university students, there is a significant indigenous correlation between the directions of YBT arrival, and the correlation between the posterolateral and the posterolateral is stronger. The stronger the lower limb muscle strength is, the better the dynamic balance ability is, and the correlation between dynamic ability and knee extensor muscle strength is higher, which may be related to the fact that knee flexion control balance can more effectively activate knee extensor muscle group. University students generally have bilateral asymmetry of the lower limbs.
Biomechanical assessment of dynamic balance: Specificity of different balance tests

Abstract:Dynamic balance is vitally important for most sports and activities of daily living, so the assessment of dynamic stability has become an important issue. In consequence, a large number of balance tests have been developed. However, it is not yet known whether these tests (i) measure the same construct and (ii) can differentiate between athletes with different balance expertise. We therefore studied three common dynamic balance tests: one-leg jump landings, Posturomed perturbations and simulated forward falls. Participants were 24 healthy young females in regular training in either gymnastics (n = 12) or swimming (n = 12). In each of the tests, the participants were instructed to recover balance as quickly as possible. Dynamic stability was computed by time to stabilization and margin of stability, deduced from force plates and motion capture respectively. Pearson's correlations between the dynamic balance tests found no significant associations between the respective dynamic stability measures. Furthermore, independent t-tests indicated that only jump landings could properly distinguish between both groups of athletes. In essence, the different dynamic balance tests applied did not measure the same construct but rather task-specific skills, each of which depends on multifactorial internal and external constraints. Our study therefore contradicts the traditional view of considering balance as a general ability, and reinforces that dynamic balance measures are not interchangeable. This highlights the importance of selecting appropriate balance tests.
Structural validity of the Mini-Balance Evaluation Systems Test in individuals with spinocerebellar ataxia: A Rasch analysis study

Abstract:Objective To examine the structural validity of the Mini-Balance Evaluation Systems Test (Mini-BESTest) in individuals with spinocerebellar ataxia (SCA). Design : Methodological research on data gathered in a cross-sectional study. A Rasch analysis was conducted (partial credit model). Setting : In-patients in a hospital rehabilitation setting. Participants : A pooled sample of patients with SCA (n=65 [total 110 data]; 23 women, 42 men; mean ± SD age 63.1 ± 9.9 yrs). Intervention : Not applicable. Main Outcome Measures : We evaluated the Mini-BESTest's category structure, unidimensionality, and measurement accuracy (0: unable to perform or requiring help, to 2: normal performance). Results : The Mini-BESTest rating scale fulfilled the category functioning criteria. The analysis of the standardized Rasch residuals showed the scale's unidimensionality, but there were seven-item pairs indicating local dependence. All of the items fit the underlying scale construct (dynamic balance) with the exception of item #1, "Sit to stand," which was an underfit. The Mini-BESTest demonstrated adequate reliability (person separation reliability = 0.87) and separated the patients into five strata. The item-difficulty measures ranged from −4.49 to 2.02 logits, and the person ability-item difficulty matching was very good (the mean of person ability = −0.07 logits and the mean of item difficulty = 0.00). No floor or ceiling effects were detected. The keyform identified items with small (#11, "Walk with head turns, horizontal") and large (#3, "Stand on one leg") item thresholds. Conclusions : The Mini-BESTest has a unidimensional balance assessment scale with good category structure and reliability even for individuals with SCA. However, it also has some inherent shortcomings such as fit statistics, local item dependencies, and item thresholds. The results obtained when the Mini-BESTest is administered to patients with cerebellar ataxia should thus be interpreted cautiously.

Comparison of Functional Movement Screen, Star Excursion Balance Test, and Physical Fitness in Junior Athletes with Different Sports Injury Risk

Abstract:Objectives . The aim of this study was to assess the relationships between functional movement screen (FMS), star excursion balance test (SEBT), agility T test, and vertical jump test scores and sports injury risk in junior athletes. We compared these assessments and the differences between groups with high and low risks of sports injury. Subjects and Methods . Eleven volleyball, 12 basketball, and 9 handball athletes were recruited. All participants followed the routine training in school sports teams. Weekly training schedules followed a similar pattern. The 32 junior athletes ( age = 16.06 ± 0.21 years ; height = 167.28 ± 6.32 cm ; and body mass = 68.45 ± 9.67 kg ) were assessed using the FMS, SEBT, agility T test, and vertical jump test in random order. The correlations of composite and individual item scores of these assessments were analyzed, and the differences between groups with high and low risks of sports injury were compared. Results . All participants completed the study protocol. No significant differences were observed between FMS, SEBT, agility T test, and vertical jump test scores in groups with high and low risks of sports injuries. Fair and moderate-to-good correlations were observed for anterior reach maximum of SEBT and deep squat ( r = 0.47 , P = 0.02 ) as well as inline lunge ( r = 0.53 , P = 0.01 ) of FMS. The hurdle step of FMS also exhibited moderate-to-good ( r = 0.52 , P = 0.01 ) and fair ( r = 0.42 , P = 0.04 ) correlations with posterior medial and posterior lateral reach maximum of SEBT, respectively. A fair correlation was observed between posterior lateral reach maximum of SEBT and rotary stability of FMS ( r = − 0.23 , P = 0.03 ). Fair and moderate-to-good correlations were identified for agility T test and maximum anterior reach in the SEBT ( r = − 0.42 , P = 0.04 ) and trunk stability push-up in the FMS, respectively ( r = − 0.57 and P = 0.006 ). Conclusions . Junior athletes with a high risk of sports injury did not exhibit differences in terms of FMS, SEBT, and physical fitness test scores. Deep squat, hurdle step, inline lunge, and rotary stability scores in the FMS were correlated with the item scores in the SEBT, which may be due to the use of similar movement patterns. Scores for anterior reach maximum in the SEBT and trunk stability push-up in the FMS were correlated with agility T test scores, suggesting a similar task requirement of trunk stability and dynamic weight shifting ability.

Abstract:BackgroundPrevious research on balance mostly focused on the assessment, training, and improvements of balance through interventions. We investigated tools commonly used to study static balance. Differences in postural stability were analyzed using multiscale entropy (MSE) and feature analysis.MethodsA force plate and inertial sensor were used to collect acceleration and center-of-pressure (COP) nonlinear signals. MSE was also used to detect fractal correlations and assess the complexity of univariate data complexity. Fifteen healthy subjects participated in the experiments. Each stood on a force plate and wore a sensor while attempting to maintain postural stability for 30 s in four randomized experiments to evaluate their static balance via a copositive experiment with eyes open/closed and with standing on one foot or both feet. A Wilcoxon-signed rank test was used to confirm that the conditions were significant. Considering the effect of the assessment tools, the influence of the visual and lower limb systems on postural stability was assessed and the results from the inertial sensor and force plate experiments were compared.ResultsForce plate usage provided more accurate readings when completing static balance tasks based on the visual system, whereas an inertial sensor was preferred for lower-limb tasks. Further, the eyes-open-standing-on-one-foot case involved the highest complexity at the X, Y, and Z axes for acceleration and at the ML axis for COP compared with other conditions, from which the axial directions can be identified.ConclusionsThe findings suggested investigation of different evaluation tool choices that can be easily adapted to suit different needs. The results for the complexity index and traditional balance indicators were comparable in their implications on different conditions. We used MSE to determine the equipment that measures the postural stability performance. We attempted to generalize the applications of complexity index to tasks and training characteristics and explore different tools to obtain different results.Trial registrationThis study was approved by the Research Ethics Committee of National Taiwan University and classified as expedited on August 24, 2017. Abstract:Predictions based on theories of verbal learning proposed by Spence and Eysenck were compared by presenting either a non-competitive or competitive list of paired-associates to groups of sixth form grammar school boys containing 42 and 59 Ss respectively. The only relation found between drive level as measured by either the MAS or A-state scale was that medium-scoring MAS Ss performed significantly worse than high- or low-scoring Ss on the competitive list. Support was obtained for the hypotheses, firstly, that performance would be related to Eysenck's hypothetical dimension of arousal ranging from stable extraversion to neurotic introversion in the manner described by the inverse-U relation, and, secondly, that good performance on the competitive list would be related to extraversion.

Abstract:Background There is a high incidence of falls in older adults with knee osteoarthritis (OA). Adequate dynamic balance and mobility reduce the risk of falls; however, there are currently no validated, advanced tests of dynamic balance and mobility for individuals with knee OA. Objective The purpose of this study was to determine the convergent validity, known-groups validity, and test-retest reliability of a dynamic test of balance and mobility, the Community Balance and Mobility Scale (CB&M), in a knee OA population. Design A cross-sectional design was used. Methods Twenty-five individuals aged 50 years and older with medial knee OA and an equal number of healthy controls completed the CB&M and other tests of balance and mobility, including the Berg Balance Scale, the Timed “Up & Go” Test, a test of maximal single-leg stance time, and the 10-Meter Walk Test (self-selected and fast walking speed). Convergent validity of balance tests with the CB&M was assessed using Pearson product moment correlation coefficients, and known-groups validity was assessed using independent t tests. Test-retest reliability of the CB&M was assessed using intraclass correlation coefficients (ICCs) and standard error of measurement (SEM). Results Scores on the CB&M were significantly correlated with all measures of balance and mobility for those with knee OA. There were significant differences in CB&M scores between groups. Scores on the CB&M were highly reliable in people with knee OA (ICC=.95, 95% confidence interval [95% CI]=0.70 to 0.99; SEM=3, 95% CI=2.68 to 4.67). Limitations Few participants had severe knee OA. Conclusions The CB&M displayed moderate convergent validity, excellent known-groups validity, and high test-retest reliability. The CB&M can be used as a valid and reliable tool to assess dynamic balance and mobility deficits in people with knee OA.
