Abstract:Abstract Obliteration of persistently discharging open mastoid cavities is one surgical option to achieve a dry ear. All the currently described techniques involve the use of random pattern local tissue flaps or free grafts. Ten patients have undergone obliteration procedures using the vascularized temporoparietal fascia flap. This resulted in rapid epithelialization by six weeks in seven out of 10 cases. The remaining three patients have persistent non healed areas over the medial attic wall, but are not troubled by otorrhoea. The indications, technique and complications of mastoid obliteration by this axial pattern flap are described.
Predicting Neonatal Outcome in Isolated Congenital Diaphragmatic Hernia Using Ultrasonographic Pulmonary Measurements

Abstract:272 Abstract Objective: To study the potential of different methods of evaluating pulmonary measurements to predict neonatal outcome in cases with isolated congenital diaphragmatic hernia (CDH). Materials and Methods: Between January 2002 and December 2004, thirty-one fetuses with isolated CDH were prospectively evaluated. Fetal lung volumes were estimated by the rotational technique on three-dimensional ultrasonography (3D-US) and fetal weight by the Hadlock equation on two-dimensional ultrasound examinations, which allowed calculating the ultrasonographic fetal lung/body weight ratio (US-FLW), the observed/expected fetal lung volumes (o/e-FLV) and the lung- over-head ratio (LHR). These measurements were compared to each other and to neonatal outcome. Results: Good correlations were observed between the US-FLW ratio and the o/e-FLV (r=0.90, p<0.001) and the LHR (r=0.64, p<0.001). The US-FLW ratio was significantly lower in neonatal death cases (median: 0.009, range: 0.004-0.021) than in survivals (median: 0.011, range: 0.008-0.020, p=0.018), as well as the o/e-FLV (p=0.03) and the LHR (p=0.03). Accuracies of the US-FLW, the o/e-FLV and the LHR in predicting neonatal outcome were 64.52% (21/31), 80.65% (25/31) and 77.42% (24/31), respectively. Discussion: In isolated CDH, fetal size is directly related to neonatal outcome, which can be evaluated by different methods. However, the o/e-FLV seems to be the best parameter for this purpose.

Abstract:Purpose This study aimed to assess the short‐term clinical efficacy of combining posterior meniscal root repair with meniscal centralization technology in the treatment of medial meniscus posterior root tears (MMPRTs) and notable meniscus extrusion. Methods In this retrospective analysis, patients aged 43–76 years with only chronic posterior root tears of the medial meniscus and notable extrusion were followed up for 12–14 months. Inclusion criteria: (1) persistent medial knee joint pain affecting daily life, presenting with joint space tenderness; (2) showing the ineffectiveness of conservative treatment for a minimum of 3 months; conservative treatment includes non‐invasive methods such as nonsteroidal anti‐inflammatory drugs, physical therapy and massage, but does not include intra‐articular injections. (3) Kellgren–Lawrence Grades 0–II osteoarthritis (OA) of the knee joint; and (4) diagnosis of a posterior root tear of the medial meniscus with notable meniscus extrusion confirmed through knee‐joint plain magnetic resonance imaging (MRI) scans, where coronal image revealed a meniscus body extrusion of ≥3 mm. Exclusion criteria: (1) Kellgren–Lawrence Grades III–IV OA in the knee joint; (2) presence of knee joint infection or other structural damage to the knee joint; (3) history of previous knee joint surgery; (4) demonstrating joint instability or abnormal lower‐limb alignment (varus deformity >5°); and (5) presence of severe mental illness, coagulation disorders, or other serious medical conditions. The Lysholms score, the Hospital for Special Surgery (HSS) knee score, the visual analogue scale (VAS) score and meniscal extrusion values were evaluated before and after surgery in 23 patients through a comparative analysis. Results The results of the follow‐up conducted 12–14 months post‐operatively indicated a significant improvement in the patients' conditions. In particular, the Lysholms scores (preoperative: 50.9 ± 10.2; 1‐year post‐surgery: 72.0 ± 8.4), HSS knee scores (preoperative: 45.9 ± 10.6; 1‐year post‐surgery: 79.1 ± 11.1) and VAS scores (preoperative: 4.0 ± 1.1; 1‐year post‐surgery: 0.9 ± 0.7) exhibited notable enhancements. Furthermore, compared to the preoperative values, the degree of meniscus extrusion measured by coronal MRI of the knee joint significantly improved post‐operatively (preoperative: 3.7 ± 0.8 mm; 1‐year post‐surgery: 2.2 ± 0.6 mm). Abstract:Objective: Refixation of a posterior root lesion of the medial meniscus via a tibial drill tunnel and prevention of extrusion using a meniscotibial suture (centralization). Indications: Posterior root lesion of the medial meniscus. Contraindications: Grade 4 cartilage damage in the corresponding compartment, uncorrected varus or valgus deformities, symptomatic instabilities, extensive degenerative tears apart from the root region. Surgical technique: Knee arthroscopy via the high anterolateral standard portal. Diagnostic arthroscopy to check indication. Locate the insertion zone on the tibial plateau and local debridement until the bone of the tibial plateau is visible. Insertion of a targeting device and drilling of a targeting wire into the center of the insertion zone in the area of the intercondylar eminence. Overdrill the target wire with a 4.5 mm drill. Reinforcement of the medial meniscus posterior horn with braided suture material. The reinforcing thread is inserted into the bone tunnel via an eyelet wire with a thread loop. Optional additional centralization with incision in the middle part of the meniscus. Reinforcement of the meniscus base with braided suture material using the "outside in" technique and fixation of the inner meniscus base at the edge of the tibial plateau using a transosseous extraction suture or a suture anchor. Postoperative management: Six weeks nonweight-bearing (0 kg), then gradually increased load. Range of motion: 4 weeks E/F 0-0-60°, 2 weeks 0-0-90°, optionally use of a valgus brace (varus of < 5°). Results: In root lesions of the medial meniscus, transosseous refixation significantly improves knee function (Lysholm, Hospital for Special Surgery, International Knee Documentation Committee, visual analog scale for pain, Tegner, and Knee Injury and Osteoarthritis Outcome scores) and reduces osteoarthritis progression. However, a transosseous suture alone could not significantly reduce postoperative extrusion. However, previous studies have shown that additional centralization can significantly reduce extrusion.

Abstract:Treatment of meniscus injuries has evolved from open to arthroscopic, from total to partial meniscectomy, and ultimately towards meniscus preservation. In theory, almost all tear types can be repaired, including root tears, (oblique) radial tears, horizontal cleavage tears, vertical tears, and even complex tears, as a result of improved surgical techniques and tools. Meniscus repair outcomes literature may be confounded by the lack of inclusion of control groups, as well as concomitant anterior cruciate ligament injury and reconstruction, augmentation with fibrin clot or platelet-rich plasma or other biologics, suture configuration, and timing of repair. Radial tears represent a most difficult pattern due to limited fixation strength, suturing circumferential fibers and technical challenge. However, successful meniscus repair outcomes can be obtained in this difficult tear pattern. The key to success for any radial repair, regardless of technique, is to create a tensionless repair, and one of the key components is anatomic reduction of the meniscus. Using all-inside suture devices to place a traction stitch can assist in reducing the meniscus. Then, all-inside or inside-out techniques may be used to repair the meniscus to the capsule in an anatomic position (vertical mattress), with rip-stop sutures to create horizontal or oblique sutures across the tear. With the meniscus reduced anatomically, mismatch from intra- to extracapsular distance is prevented.

Abstract:Bucket-handle meniscus tears alter knee biomechanics and significantly elevate the risk of osteoarthritis when the tissue is not preserved. Consequentially, meniscus repair is preferred whenever possible to optimize load distribution and knee stability, with the overall goal of joint preservation. Bucket-handle meniscus tears have a higher rate of repair failure compared with many other tear patterns, and both the inside-out (IO) and all-inside (AI) techniques are acceptable options, each with distinct advantages and disadvantages. Both have demonstrated improved clinical outcomes with similar failure rates. IO is cost-effective and allows less meniscal trauma due to small-diameter needles, whereas disadvantages include increased surgical time, increased risk of infection, and the need for 1 or more skilled assistants. AI avoids extra incisions and extra assistants and shows decreased surgical time, whereas complications can include hardware irritation, chondral and neurovascular injury (particularly for posterolateral repairs), and motion loss. Hybrid repair using AI for posterior tears and IO for middle-third and outside-in for the anterior meniscus is cost-effective and efficacious. Biologics to promote healing through mechanical stimulation of the tear surfaces, notch marrow venting, trephination, and/or platelet-rich plasma can optimize the repair.
Association Between Transtibial Meniscus Root Repair and Rate of Meniscal Healing and Extrusion on Postoperative Magnetic Resonance Imaging: A Prospective Multicenter Study

Abstract:Background: Prospective studies evaluating second-look imaging of meniscus root repair using a transtibial pull-out technique are limited; therefore, optimal surgical indications and the technique for meniscus root repair remain uncertain. Hypothesis: It was hypothesized that there would be a high rate of healing, improvement in meniscal extrusion, and prevention of articular cartilage degeneration and subchondral bone abnormalities after meniscus root repair. Study Design: Case series; Level of evidence, 4. Methods: Consecutive patients undergoing transtibial root repair were prospectively enrolled at 2 orthopaedic centers between March 2017 and January 2019. Pre- and postoperative magnetic resonance imaging (MRI) scans were reviewed by a musculoskeletal radiologist in a blinded fashion for meniscal healing, quantification of extrusion, articular cartilage grade, subchondral bone changes, and coronary/meniscotibial ligament abnormalities. Given persistent extrusion observed on postoperative MRI scans, an additional 10 patients gave consent and were enrolled for immediate (before weightbearing) postoperative MRI scans. Results: A total of 45 patients (16 male, 29 female; mean ± standard deviation age, 42.3 ± 12.9 years; body mass index, 31.6) were prospectively enrolled in the study; there were 47 meniscus root repairs: 29 medial and 18 lateral (2 with both). Postoperative MRI was obtained at an average of 6.3 months (range, 5.1-8 months); 98% of meniscal repairs had evidence of healing. Mean extrusion increased significantly, from 1.9 ± 1.5 mm preoperatively to 2.6 ± 1.4 mm postoperatively ( P = .03). There was no significant progression of chondromalacia grade, subchondral edema, insufficiency fracture, subchondral cysts, or subchondral collapse. In the additional 10-patient cohort, the mean preoperative extrusion (1.6 ± 1.2 mm) was not significantly different from that immediately postoperatively (2.0 ± 1.0 mm; P = .23). Conclusion: Prospective MRI analysis of transtibial meniscus root repair confirmed a high rate of meniscal healing and no observable progression of cartilage degeneration or subchondral bone abnormalities at the short-term follow-up. Abstract:Posterior medial meniscus root tears (PMMRTs) make up a relatively notable proportion of all meniscus pathology and have been definitively linked to the progression of osteoarthritis (OA). While known risk factors for development of OA in the knee include abnormal tibial coronal alignment, obesity and female gender, PMMRTs have emerged in recent years as another significant driver of degenerative disease. These injuries lead to an increase in average contact pressure in the medial compartment, along with increases in peak contact pressure and a decrease in contact area relative to the intact state. Loss of the root attachment impairs the function of the entire meniscus and leads to meniscal extrusion, thus impairing the force-dissipating role of the meniscus. Anatomic meniscus root repairs with a transtibial pullout technique have been shown biomechanically to restore mean and peak contact pressures in the medial compartment. However, nonanatomic root repairs have been reported to be ineffective at restoring joint pressures back to normal. Meniscal extrusion is often a consequence of nonanatomic repair and is correlated with progression of OA. In this study, the authors will describe the biomechanical basis of the natural history of medial meniscal root tears and will support the biomechanical studies with a case series including patients that either underwent non-operative treatment (5 patients) or non-anatomic repair of their medial meniscal root tears (6 patients). Using measurements derived from axial MRI, the authors will detail the distance from native root attachment center of the non-anatomic tunnels and discuss the ongoing symptoms of those patients. Imaging and OA progression among patients who were treated nonoperatively before presentation to the authors will be discussed as well. The case series thus presented will illustrate the natural history of meniscal root tears, the consequences of non-anatomic repair, and the findings of symptomatic meniscal extrusion associated with a non-anatomic repair position of the meniscus.

Abstract:The case fatality was the lowest (3.8%) among 1512 cases with severe acute respiratory syndrome (SARS) in Guangdong Province, China. Rational use of corticosteroid, non-invasive ventilation and the integration of traditional Chinese medicine and modern medicine may partly have contributed to the lowest fatality figure. There was a close linkage between civet cats and humans in terms of transmission of SARS. Strict control of the wild-animal market may be significant in preventing a new outbreak of SARS this year.

Abstract:Objectives: Meniscal root disruption results in a loss of hoop strain resistance, increased articular cartilage contact pressure, and acceleration of degenerative changes. There is limited data comparing the outcomes of similarly matched patients with a medial meniscus posterior root tear treated with non-operative management, partial meniscectomy, or repair. The purpose of this study is to compare treatment failure, clinical outcome scores, and radiographic findings for a matched cohort of patients who underwent either non-operative management, partial meniscectomy, or transtibial pull-through repair for a medial meniscus posterior root tear (MMPRT). Our hypothesis is that patients who underwent meniscus root repair will have lower rates of progression to arthroplasty than patients who were treated with non-operative management or partial meniscectomy. Methods: Patients who underwent transtibial medial meniscus posterior horn root repair were matched by meniscus laterality, age, sex and K-L grades to patients treated non-operatively or with a partial meniscectomy. Progression to arthroplasty rates, IKDC and Tegner scores, and radiographic outcomes were analyzed between groups. Results: Forty-five patients were included in this study (15 non-operative, 15 partial meniscectomy, 15 root repair). Progression to arthroplasty demonstrated significant differences among treatment groups at a mean 74 months (non-operative 4/15, partial meniscectomy 9/15, meniscus repair 0/15, p=.0003). The meniscus root repair group had significantly less arthritic progression, as measured by change in K-L grade from pre-op to post-op (non-operative 1.0, partial mensicectomy 1.1, and meniscus repair 0.1, p=.001). Conclusion: Meniscus root repair leads to significantly less arthritis progression and subsequent knee arthroplasty compared to non-operative management and partial meniscectomy in a demographically matched cohort.

Abstract:The meniscus is important for load distribution, shock absorption and stability of the knee joint. Meniscus injury or meniscectomy results in decreased function of the meniscus and increased risk of knee osteoarthritis. To preserve the meniscal functions, meniscal repair should be considered as the first option for meniscus injury. Although reoperation rates are higher after meniscal repair compared with arthroscopic partial meniscectomy, long-term follow-up of meniscal repair demonstrated better clinical outcomes and less severe degenerative changes of osteoarthritis compared with partial meniscectomy. In the past, the indication of a meniscal repair was limited both because of technical reasons and due to the localised vascularity of the meniscus. Meanwhile, it spreads today as the development of the concept to preserve the meniscus and the improvement of meniscal repair techniques. Longitudinal vertical tears in the peripheral third are considered the 'gold standard' indication in terms of meniscus healing. Techniques for meniscal repair include 'inside-out', 'outside-in' and 'all-inside' strategies. Surgical decision-making depends on the type, size and location of the meniscus injury. Meniscal root tears substantially affect meniscal hoop function and accelerate cartilage degeneration; therefore, meniscus root repair is necessary to prevent the progression of osteoarthritis change. For symptomatic meniscus defects after meniscectomy, transplantation of allograft or collagen meniscus implant may be indicated, and acceptable clinical results have been obtained. Recently, meniscus extrusion has attracted attention due to increased interest in early osteoarthritis. The centralisation techniques have been proposed to reduce the meniscus extrusion by suturing the meniscus-capsule complex to the edge of the tibial plateau. Long-term clinical outcomes of this procedure may change the strategy of treating meniscus extrusion. When malalignment of the lower leg is accompanied with meniscus pathologies, knee osteotomies are a reasonable option to protect the repaired meniscus by unloading the pathological compartment. Advancements in biological augmentation such as bone marrow stimulation, fibrin clot, platelet-rich plasma, stem cell therapy and scaffolds have also expanded the indications for meniscus surgery. In summary, improved repair techniques and biological augmentation have made meniscus repair more appealing to treat that had previously been considered irreparable. However, further research would be necessary to validate the efficacy of these specialised technique.
Medial Meniscus Root Reconstruction using Gracillis Tendon Autograft in Managing Medial Meniscus Posterior Root Tear: A Preliminary Case Series

Introduction:Medial Meniscus Posterior root tears (MMPRT) are defined as complete radial tear within 10 mm from the meniscus attachment or bony root avulsion. Currently, repair of meniscal root injuries is the treatment of choice with the aim of restoring joint kinematics, contact pressures, and delaying the development of OA. Although encouraging results have been reported, the repair techniques do not provide any biological substance to stimulate biological healing, remembering that the attachment of medial meniscus root to the tibial plateu contain a ligament-like structures, the usage of graft may recreate that ligamentous structure. Therefore the purpose of this case series is to describe the clinical outcome of medial meniscus root reconstruction in 3 cases of degenerative MMPRT.Method:Three patients was diagnosed with MMPRT by a history taking, physical examination and the use of non-contrast knee MRI. All of the patients underwent arthroscopic surgical reconstruction with 2 tunnel transtibial pullout repair (TPR) technique and the use of gracillis tendon autograft (3 mm in diameter, 6 cm in length) that is passed through the meniscus. The surgery was performed by one orthopaedic surgeon. Follow up was done at 3 month after the surgery. KOOS and VAS are used to assess both patient subjective outcome and pain score.Result and Discussion:Patients underwent reconstruction shows an improvement in KOOS and VAS score. The average improvement in total KOOS is 43% and VAS score is 6,3 points. From the study by Wendel et al. meniscus root reconstruction can provide reattachment to the anatomic footprint and cartilaginous graft to facilitate healing. Also in agreement with the study by Li et al arthroscopically assisted meniscus root reconstruction with gracillis autograft showed superior clinical outcome and meniscus root healing rate.Conclusion:Meniscus root reconstruction showed a promising result in managing patients with MMPRT. No serious complications are reported.

Medial meniscus posterior root tear (MMPRT) refers to the avulsion or complete radial tear occurring within a distance of 1 cm from the posterior tibial attachment of the medial meniscus. Root tears result in the extrusion of the meniscus, which subsequently leads to elevated joint contact pressures. These condition increased pressures contribute to the rapid progression of joint space narrowing and the occurrence of subchondral bone edema that were indistinguishable from total meniscectomy.Studies have reported that MMPRT represents A substantial percentage of meniscus tears, accounting for approximately 10 to 20% of all meniscus tears diagnosed. MMPRT frequency was reportedly 21.5% in all arthroscopic surgeries for meniscus medial tear. Nevertheless, despite the significant role that these injuries play in the development of degenerative joint disease, they frequently remain undetected during the initial evaluation. MMPRT are relatively common among middle-aged adults, female sex, and higher BMI with clear onset of significant injury. This might be attributed to aging associated degenerative alteration that makes the meniscus more susceptible to root detachment.The primary objective of managing MMPRT is to mitigate the progression of osteoarthritis by reinstating optimal joint contact pressures and kinematics. Given the unfavourable results observed with conservative treatment, surgical repair is currently regarded as the preferred choice. This approach has demonstrated encouraging clinical outcomes specifically in patients who meet the appropriate indications. Patients undergoing medial meniscus root repair have better functional scores, less osteoarthritis progression, and less conversion to total knee replacement compared to meniscectomy. The most common repair method for MMPRT is using transtibial pull-out repair at anatomic footprint of the meniscus root. Nevertheless, the pull-out suture method exhibits a vulnerability at the location of meniscus suturing, where cuts frequently occur during cyclical loading. Additionally, this technique cannot restore the meniscus extrusion. Furthermore, directly repair of the meniscus to bone may lead to a biologically weak structure at the footprint of the meniscus root. Nonanatomical repair itself shows less tibiofemoral contact area and increased mean contact pressure compared to anatomic repair.Several additional procedures have been proposed to maximize the result of meniscus root repair, including peripheral meniscocapsular release, centralization suture, and root reconstruction to minimize meniscus extrusion. Abstract:Background: Root tears are functionally compromising injuries that require an anatomic repair for functional restoration of native joint mechanics. Posterior root tears of the lateral meniscus are commonly associated with anterior cruciate ligament (ACL) ruptures as lateral root tears contribute to anterolateral rotational instability. Two-tunnel transtibial root repair allows for root repair with superior pullout strength and can be combined with ACL reconstruction to treat this injury pattern. Indications: Two-tunnel transtibial root repair is indicated in active patients with meniscal root tears that have low-grade chondromalacia (grade II or less). It is contraindicated in patients with advanced degenerative changes (grade III-IV), severe mal-alignment, and extrusion related to advanced degenerative changes; those who cannot complete postoperative rehabilitation; and patients with significant medical comorbidities. Technique Description: After repairing a vertical horn tear of the posterior medial meniscus, a vertical tear of the posterior root of the lateral meniscus was observed. Following preparation of the anatomic footprint, 2 transtibial tunnels were drilled and sutures were passed and secured using a surgical button over the anterior tibial cortex. Horizontal repair of the tear was then performed using 2 all-inside devices. Subsequently, an ACL reconstruction was performed. Results: At 11 months postoperative, the patient has continued to progress as expected. She was able to ambulate without pain, had a symmetric and balanced gait, and was able to return to running. Discussion/Conclusion: Lateral meniscal root repairs using 2 transtibial tunnels and concomitant ACL reconstruction have yielded good and reliable clinical outcomes with low failure rates and low rates of revision surgery.

Abstract:Over the last several years, there has been a shift from arthroscopic partial meniscectomy to meniscal repair, especially in the younger patient. In case of a necessary partial meniscectomy, some patients have unremittent symptoms of pain and effusion corresponding to the postmeniscectomy syndrome. In these patients without large coronal malalignment, meniscal allograft transplantation is a valuable option to restore contact pressures, promote a chondroprotective microenvironment, and potentially delay secondary surgical interventions symptoms. In the adolescent population, meniscal allograft transplantation has been shown to effectively improve patient-reported outcomes with a low conversion to arthroplasty. However, these treatments are far from ideal, and prevention is certainly better than the cure: timely diagnosis of meniscus injuries, appropriate treatment with meniscus repair rather than partial meniscectomy, even in the complex tear patterns, and consideration of corrective osteotomy for milder cases of malalignment.

Abstract:Australia is now one of the safest countries in the world in which to be born. This is largely a result of the many advances in obstetric and neonatal medicine of the past 50 years. However, the “medicalisation” of birth has tended to diminish women's satisfaction with their experience of childbirth. It has been shown that women are most satisfied by care from a single practitioner, and when they themselves have input into decision‐making. Although maternal satisfaction is important, it should not be promoted at the expense of the health of mothers and babies. More realistic antenatal education and preparation should be available for all pregnant women so that both maternal satisfaction and good perinatal outcomes can be achieved.
