Abstract:We examined the hypothesis that pain increases negative person perception of irrelevant others in both medical and laboratory settings in three studies. Patients perceived a nurse as more negative if the injection they received from the nurse produced more pain (Pilot Study). Patients rated neutral faces as more negative after receiving an injection than before it (Study 1). Participants who performed a painful cold pressor task rated neutral faces as more negative than a control group, but this effect only appeared for those with low perceived social support (Study 2). These findings suggest that one's experience of physical pain may be metaphorically transferred to perceptions of completely irrelevant others, inducing negative interpersonal perception. And perceived social support can ameliorate the negative effect.

Abstract:People experience pain when they spend money. Because previous studies have shown that perceived social support reduces physical pain, this research examined whether perceived social support reduces spending pain. Our studies showed that both real and recalled social support reduced spending pain (Studies 1-3) and that perceived social support reduced the perceived importance of money as a protection mechanism, which in turn reduced spending pain (Studies 1 and 3). Moreover, the pain-buffering effect of social support was stronger for hedonic purchases than for utilitarian purchases (Study 2). This research broadens our understanding of the factors that enhance consumer experiences and the relationships among love, security, and pain. (C) 2014 Society for Consumer Psychology. High Levels of Vicarious Exposure Bias Pain Judgments

Abstract:The present study evaluated the effects of exposure to facial expression of pain, on observers' perceptions of pain expression. Participants were undergraduates shown brief video clips of the facial expressions of shoulder-pain patients displaying no pain or moderate pain. Participants were randomly allocated to either a high preexposure condition in which each clip was preceded by 10 other clips showing strong pain or a no-exposure control. On each test trial, participants indicated whether they thought the person they saw was in pain or not. Data were analyzed using signal detection theory methods. High prior exposure to pain was unrelated to sensitivity to pain expression, but did significantly diminish the likelihood of judging the other to be in pain. Results are discussed in terms of their implications for pain judgments of health-care professionals, adaptation-level theory, and the psychophysical method of selective adaptation.PERSPECTIVE: This paper provides an experimental demonstration that, when people have large amounts of exposure to others' expressions of pain, their estimation of others' pain is reduced. The findings offer 1 explanation for the widely observed underestimation bias in pain judgments and may suggest ways of changing it.
Social hierarchy modulates neural responses of empathy for pain

Abstract:Recent evidence indicates that empathic responses to others' pain are modulated by various situational and individual factors. However, few studies have examined how empathy and underlying brain functions are modulated by social hierarchies, which permeate human society with an enormous impact on social behavior and cognition. In this study, social hierarchies were established based on incidental skill in a perceptual task in which all participants were mediumly ranked. Afterwards, participants were scanned with functional magnetic resonance imaging while watching inferior-status or superior-status targets receiving painful or non-painful stimulation. The results revealed that painful stimulation applied to inferior-status targets induced higher activations in the anterior insula (AI) and anterior medial cingulate cortex (aMCC), whereas these empathic brain activations were significantly attenuated in response to superior-status targets' pain. Further, this neural empathic bias to inferior-status targets was accompanied by stronger functional couplings of AI with brain regions important in emotional processing (i.e. thalamus) and cognitive control (i.e. middle frontal gyrus). Our findings indicate that emotional sharing with others' pain is shaped by relative positions in a social hierarchy such that underlying empathic neural responses are biased toward inferior-status compared with superior-status individuals.
Does a diagnosis of depression influence observer ratings of pain severity? The mediating role of causal attributions of pain and pain genuineness

Abstract:British Journal of Pain, Ahead of Print. Researchers have been increasingly investigating observer and patient characteristics that may influence the assessment of pain in others. While rates of psychiatric conditions are high in chronic pain populations, surprisingly little attention has been given to if (and why) a comorbid psychiatric diagnosis may influence the estimation of pain in others. Using an experimental vignette paradigm, the current study examined whether a diagnostic label of major depressive disorder (MDD) would impact observer pain estimates of a woman with chronic pain, and whether causal attributions of pain and pain genuineness might help explain these effects. Participants (n = 188) were given a vignette describing a female patient with chronic pain (who either had MDD or no mental health concerns), viewed a brief video clip of the patient, and then were asked to provide a variety of ratings about the woman's pain. Results of a serial multiple mediation analysis revealed that participants in the MDD condition made greater psychological attributions for the woman's pain, which was associated with lower perceptions of pain genuineness, which was then associated with lower estimates of pain intensity. These findings suggest that a diagnosis of depression may indirectly influence observer estimates of another person's pain by heightening psychological attributions of pain, and making their pain seem less genuine. Further research is needed to elucidate the complex processes underlying pain estimation, including patient and observer characteristics, biases, and heuristics, in order to improve quality of care for those living with persistent pain.'
[Non-Hodgkin's lymphomas associated with the acquired immunodeficiency syndrome. A multicenter clinical study of 77 cases].

Abstract:BACKGROUND The aim of the present was to study the prevalence of non Hodgkin's lymphoma (NHL) in AIDS patients as well as the clinicopathologic characteristics, response to treatment and survival. METHODS From January 1984 to January 1991, 77 patients with NHL associated with AIDS diagnosed in 9 hospitals in Madrid were retrospectively studied. RESULTS Ninety-two per cent of the patients were men (mean age 30 years: range: 9-66 years), 62% were intravenous drug abusers and 20 (26%) homosexuals. Pathologic study determined that 62 (80%) patients had high grade NHL (44% small noncleaved), 17% immunoblastic and 20% unclassifiable, and 15 (20%) had intermediate grade (16% diffuse large cell) being all the cases of the B immunophenotype. Sixty-five per cent were in advanced stages and 69% had B symptoms. Extranodal localizations were present in 88%, bone marrow in 29% and CNS in 29%. Six cases had primary CNS lymphomas. 50% of the patients had less than 200 x 10(6)/l CD4 lymphocytes. Forty-seven patients were evaluable for response to chemotherapy: 12 (26%) showed a complete response 27 (57%) a partial response and 8 (17%) did not respond. Opportunistic infections developed in 18%. The estimated survival at 3 years was 14% (median 6 months). On univariate analysis the parameters related to the worst survival were: primary CNS lymphoma, liver involvement, lack of treatment response, LDH > or = 300 UI/l, alkaline phosphatase > or = 500 UI/l and ESR > or = 70 mm. CONCLUSIONS Non Hodgkin's lymphomas associated with AIDS usually behave in an "aggressive" way with a high frequency of advanced stages, B symptoms, high grade histologic subtypes and extranodal involvement. Response to treatment is poor, bone marrow toxicity frequent and survival short.
Pain modulates early sensory brain responses to task-irrelevant emotional faces

Abstract:Background: Pain can have a significant impact on an individual's life, as it has both cognitive and affective consequences. However, our understanding of how pain affects social cognition is limited. Previous studies have shown that pain, as an alarm stimulus, can disrupt cognitive processing when focal attention is required, but whether pain also affects task-irrelevant perceptual processing remains unclear. Methods: We examined the effect of laboratory-induced pain on event-related potentials (ERPs) to neutral, sad and happy faces before, during and after a cold pressor pain. ERPs reflecting different stages of visual processing (P1, N170 and P2) were analysed. Results: Pain decreased the P1 amplitude for happy faces and increased the N170 amplitude for happy and sad faces compared to the pre-pain phase. The effect of pain on N170 was also observed in the post-pain phase. The P2 component was not affected by pain. Conclusions: Our results suggest that pain alters both featural (P1) and structural face-sensitive (N170) visual encoding of emotional faces, even when the faces are irrelevant to the task. While the effect of pain on initial feature encoding seemed to be disruptive and specific to happy faces, later processing stages showed long-lasting and increased activity for both sad and happy emotional faces. Significance: The observed alterations in face perception due to pain may have consequences for real-life interactions, as fast and automatic encoding of facial emotions is important for social interactions.
Impact of attention cues on empathy for pain in college students with implicit weight stigma

Abstract:While studies about attention cues influencing empathy for pain have been reported, it is still unclear what pain reactivity in individuals with implicit weight stigma, whether and how the attention cues have an influence on empathy for pain in individuals with implicit weight stigma as well as its potential contributions to understanding empathy processes and developing interventions for weight stigma. Here, using the Single Category Implicit Association Test (SC-IAT), twenty-five participants were randomly selected to provide different stimulus conditions (e.g. the painful pictures). The event-related potential (ERP) and behavioral data were recorded when they were required to judge whether stimuli would be painful/ attractive or not to explore the impact of attention cues on empathy for pain in individuals with implicit weight stigma. Behavioral results showed participants responded slower to the thin body picture than the fat body picture. Moreover, top–down attention influences the late cognitive processing stage component P3 and LPC. Specifically, relative to pain judgment task, the fat body picture elicited larger P3 amplitudes than the thin body picture. In addition, relative to pain judgment task, the fat body picture elicited larger LPC amplitudes than the thin body picture; relative to the attractiveness judgment task, the thin body picture elicited larger LPC amplitudes than the fat body picture. These findings revealed that empathy for pain among college students with weight implicit stigma was influenced by top-down attention to attention cues, and presented evidence for the empathy deficits of individuals with implicit weight stigma who might avoid negative emotion when viewing other people's bodies.

Abstract:AbstractOur experience with the world is shaped not only directly through personal exposure but also indirectly through observing others and learning from their experiences. Using a conditioning paradigm, we investigated how directly and observationally learned information can affect pain perception, both consciously and non-consciously. Differences between direct and observed cues were manifest in higher pain ratings and larger skin conductance responses to directly experienced cues. However, the pain modulation effects produced by conditioning were of comparable magnitude for direct and observational learning. These results suggest that social observation can induce positive and negative pain modulation. Importantly, the fact that cues learned by observation and activated non-consciously still produced a robust conditioning effect that withstood extinction highlights the role of indirect exposure in placebo and nocebo effects.

Abstract:Objective: In this study, we were interested in determining whether we could alter a pain response in a chronic pain patient population by exposing participants to different videos prior to inducing acute pain.Design: This observational case series study required participants to report their pain level during the cold pressor task after viewing an instruction video.Setting: Recruitment and testing took place in a tertiary care multidisciplinary pain center.Subjects: Forty adults with chronic pain participated in the study and completed the cold pressor test.Methods: Prior to testing, questionnaires measuring pain, empathy, and catastrophic thinking were completed and participants were randomized to view an instructional video where an actress either demonstrated pain behavior or a stoic response during the cold pressor test.Results: Participants with higher levels of catastrophizing reported higher pain levels during the cold pressor test. Personal Distress Empathy measures of participants who viewed the pain catastrophizing video were significantly correlated with their final pain reports. Following the cold pressor task, participants' pain reports for their primary chronic pain sites were significantly reduced.Conclusions: These results support previous findings that people with chronic pain show the tendency toward increased acute pain experience if levels of catastrophizing and Personal Distress Empathy measures are higher. Participants reported attenuated chronic pain following induced pain, also in line with previous research suggesting a central endogenous inhibitory effect. Our findings shed light on the role of emotional and social components affecting the experience of pain in individuals with chronic pain.

Abstract:Social exclusion has a significant impact on cognition, emotion, and behavior. Some behavioral studies investigated how social exclusion affects pain empathy. Conclusions were inconsistent, and there is a lack of clarity in identifying which component of pain empathy is more likely to be affected. To investigate these issues, we used a Cyberball task to manipulate feelings of social exclusion. Two groups (social exclusion and social inclusion) participated in the same pain empathy task while we recorded event-related potentials (ERP) when participants viewed static images of body parts in painful and neutral situations. The results showed early N2 differentiation between painful and neutral pictures in the central regions in both groups. The pattern at the late controlled processing stage was different. Parietal P3 amplitudes for painful pictures were significantly smaller than those for neutral pictures in the social exclusion group; they did not differ in the social inclusion group. We observed a parietal late positive potential (LPP) differentiation between painful and neutral pictures in both groups. LPP amplitudes were significantly smaller in the social exclusion group than those in the social inclusion group for painful stimuli. Our results indicate that social exclusion does not affect empathic responses during the early emotional sharing stage. However, it down-regulates empathic responses at the late cognitive controlled stage, and this modulation is attenuated gradually. The current study provides neuroscientific evidence of how social exclusion dynamically influences pain empathy.

Abstract:In simple mechanical events, we can directly perceive causal interactions of the physical objects. Physical cues (especially spatiotemporal features of the display) are found to associate with causal perception. Here, we demonstrate that cues of a completely different domain-social cues-also impact the causal perception of physical events: The causally ambiguous events are more likely to be perceived as causal if the faces superimposed on the objects change from neutral to fearful. This effect has the following major properties: (a) The effect is caused by social information because it disappears when the faces are inverted or when the expression changes are unreasonable; (b) the social cues are integrated in a temporal window different from physical cues; and (c) the social cues impact the perception process rather than the decision process as the impact also appears in the causality-induced illusion. These findings suggest that the visual system relies on social information to infer the causal structure of the physical world.
The Effect of the Model’s Social Status on Placebo Analgesia Induced by Social Observational Learning

Abstract:Abstract Background Placebo analgesia can be induced by social observational learning. The aim of this study was to determine whether this effect can be influenced by the social status of a model. Methods Healthy volunteers were randomly assigned to three groups: a group that observed a video featuring a high-status model (introduced as a professor), a group that observed a video featuring a low-status model (introduced as a janitor), and a control group. Participants observed videos showing a model (of high or low status) undergoing the experimental procedure, during which he received pain stimuli. In each group, half of participants watched a video in which the model rated blue stimuli as more painful (6–8 on the numeric rating scale) and orange stimuli as less painful (1–3 on the numeric rating scale), whereas the other half of participants watched a video in which the model rated orange stimuli as more painful and blue stimuli as less painful. Participants in the control group did not watch any video. Then, all participants received 16 electrocutaneous pain stimuli of the same intensity, preceded by either blue or orange colors. The perceived social status of the model and the trait empathy of participants were measured. Results Placebo analgesia was induced in both experimental groups, yet no difference in the magnitude of the effect was found. However, we found that the participants’ individual ratings of the model’s social status predicted the magnitude of placebo analgesia. Conclusion This is the first study to show that the perception of a model’s social status is related to the magnitude of placebo analgesia induced by observational learning.

Abstract:Affective problems such as social anxiety and depression theoretically involve negative cognitive biases that trigger and maintain symptoms during everyday experiences. This study employed a social judgment paradigm to investigate possible biases in expectation of social acceptance, and subjective feelings of pain and reward. Healthy adult participants ( N = 120) were told their image had been judged by others. In 120 trials, they were shown photos of the judges and asked to anticipate whether they were liked by them or not, before being shown the judgment. Participants rated their level of pain and reward in each trial. Results indicated that social acceptance was expected less often by participants with higher levels of social anxiety. Self-reported pain was greatest after unexpected rejection. A greater likelihood of the presence of pain and higher self-reported pain were associated with higher levels of social anxiety and depression respectively. Self-reported reward was greatest after expected acceptance, and was not associated with social anxiety or depression. This study provides subjective experience information that has been missing from existing social judgment research. Moreover, these findings suggest that in social situations, those with social anxiety and depression more often expect rejection and experience rejection as more painful, respectively. These biases are potential maintaining factors and may be targets for further research and future intervention development.

Abstract:Witnessing another's pain can heighten pain in the observer. However, research has focused on the observer's intrapersonal experience. Here, a social transmission-chain explored the spread of socially-acquired nocebo hyperalgesia. Dyads of genuine participants were randomised to 'Generations' (G1-G3). G1-Demonstrators, observed by G2-Observers, experienced high/low thermal pain contingent on supposed activity/inactivity of a sham-treatment. G2 became Demonstrators, witnessed by G3-Observers. They experienced fixed low-temperature stimuli irrespective of sham-treatment 'activity'. G3 then Demonstrated for G4-Observers (a confederate), also experiencing low-temperature stimuli only. Pain ratings, electrodermal activity, and facial action units were measured. G1's treatment-related pain propagated throughout the chain. G2 and G3 participants showed heightened subjective and physiological response to sham-treatment, despite equivalent stimulus temperatures, and G3 never witnessing the initial pain-event. Dyadic interpersonal physiological synchrony (electrodermal activity) and psychological synchrony (Observer's ability to predict the Demonstrator's pain), predicted subsequent socially-acquired pain. Implications relate to the interpersonal spread of maladaptive pain experiences.
Vicarious facilitation of facial responses to pain: Does the others' expression need to be painful?

Abstract:Introduction Prior exposure to others' facial expressions of pain can lead to a facilitation of pain responses, including its corresponding response channel, namely facial responses to pain. It has been questioned, however, whether this vicarious pain facilitation occurs only when observing others' pain or whether the observation of other negative expressions can trigger similar facilitation of facial responses to pain. The study aimed to test this, by comparing the impact of viewing others' facial expressions of pain versus another negative expression (sadness) and two control expressions (neutral, happiness) on facial responses to pain. Method Participants (N = 56; 31 females), watched short video clips of computer‐generated facial expressions (pain, sadness, neutral & happiness) before they received painful and non‐painful heat stimuli. Facial responses were analysed using the Facial Action Coding System. In addition, subjective and autonomic responses were assessed. Results The prior exposure to others' expressions of pain and sadness versus neutral did not lead to significantly increased facial responses to pain. Likewise, subjective and autonomic pain responses were not facilitated. However, viewing others' expressions of happiness, consistently reduced facial as well as subjective and autonomic responses to pain compared to others' negative or neutral expressions. This dampening effect was not observed for non‐painful heat. Discussion Facial and other pain responses were most strongly affected by prior exposure to others' facial expressions of happiness, which led to a pain‐dampening effect. In contrast, the evidence for vicarious facilitation of pain was rather weak in the present study, with no evidence of pain‐specificity. Significance Statement Facial responses to pain – along with subjective and autonomic responses – are reduced when observing others' expressions of happiness, demonstrating pain modulation by positive affective social signals, which may also transfer to clinical contexts.

Abstract:Pain may initially contribute to the evolution of moral decision-making as it elicits avoidance behavior. The current study aims to support this perspective by conducting a behavioral study to investigate whether pain leads to a self-oriented tendency, and an exploratory electroencephalogram (EEG) study to examine how pain affects moral decision-making. In Experiment 1, 34 participants were recruited and treated with both capsaicin (pain condition) and hand cream (control condition) in separate days. After treatment, they were asked to complete a third-party punishment task. Results showed that pain increased punishment and decreased compensation towards unfair allocations in the task. In Experiment 2, 68 participants with either pain or control treatment participated in an EEG experiment. It revealed that pain enlarged the disparity of late positive potential (LPP) between fair and unfair situations, suggesting that individuals in pain may exert more cognitive effort when facing unfair allocations. Meanwhile pain did not affect the early components P2 and the medial frontal negativity, indicating unaffected attentional or anticipatory responses towards unfairness. It demonstrates that pain can effectively modulate responses to unfairness, manifesting as a self-oriented approach with negative consequences for others. It suggests a potential evolutionary impact of pain on moral decisions.

The interactions between them are thought to lead to increased pain load. However, the effect of social pain on physical pain perception during interactions remains unclear. Four experiments were conducted merging physical and social pains to examine the behavioral pattern and neural mechanism of the effect of social pain on physical pain perception. Seemingly paradoxical effects of social pain were observed, which both facilitated and inhibited physical pain perception under different attention orientations. Brain imaging revealed that the posterior insula encoded the facilitatory effect, whereas the frontal pole engaged in the inhibitory effect. At a higher level, the thalamus further modulated both processes, playing a switch-like role under different concern statuses of social pain. These results provide direct evidence for the dual-pathway mechanism of the effect of social pain on physical pain.
